RESUMO

Excess weight and obesity have high prevalence rates globally and are associated with significant morbidity and mortality rates. In the wake of the COVID-19 pandemic, telehealth emerged as an effective tool for promoting healthy behaviours in the management of chronic conditions. This study employed a three-month quasi-experimental design with pre- and post-intervention measurements, assessing the progress of 82 adults assigned either to an intervention group (IG) or a control group (CG). A total of 66 participants completed the study, with 33 in each group. The IG had access to a telehealth-based platform providing educational content on healthy lifestyle habits and were followed up both in-person and remotely. The CG were monitored as usual, i.e., at their primary care nurses' discretion. The IG exhibited a statistically significant reduction in weight, BMI, and abdominal circumference, along with improved adherence to a heart-healthy diet, specifically the Mediterranean diet, and higher levels of physical activity. The nurse-led intervention (Care4US+), utilising telehealth from primary care, has proven effective in enhancing healthy behaviours and reducing cardiovascular risk factors.

OBJECTIVES: To describe the prenatal neuroimaging spectrum of rhombencephalosynapsis (RES) and criteria for its classification according to the severity of vermian anomaly. METHODS: In this multicenter retrospective study of fetuses with RES between 2002 and 2020, the medical records and brain ultrasound and magnetic resonance images were evaluated comprehensively to determine the severity of the vermian anomaly and the presence of associated brain findings. RES was classified, according to the pattern of vermian agenesis and the extent of the fusion of the hemispheres, as complete RES (complete absence of the vermis) or partial RES (further classified according to the part of the vermis that was missing and, consequently, the region of hemispheric fusion, as anterior, posterior, severe or mixed RES). Findings were compared between cases with complete and those with partial RES. RESULTS: Included in the study were 62 fetuses with a gestational age ranging between 12 and 37 weeks. Most had complete absence of the vermis (complete RES, 77.4% of cases), a 'round-shaped' cerebellum on axial views (72.6%) and a transverse cerebellar diameter (TCD) < 3rd centile (87.1%). Among the 22.6% of cases with partial RES, 6.5% were classified as severe partial, 6.5% as partial anterior, 8.1% as partial mixed and 1.6% as partial posterior. Half of these cases presented with normal or nearly normal cerebellar morphology and 28.5% had a TCD within the normal limits. Infratentorially, the fourth ventricle was abnormal in 88.7% of cases overall, and anomalies of the midbrain and pons were frequent (93.5% and 77.4%, respectively). Ventriculomegaly was observed in 80.6% of all cases, being more severe in cases with complete RES than in those with partial RES, with high rates of parenchymal and septal disruption. CONCLUSIONS: This study provides prenatal neuroimaging criteria for the diagnosis and classification of RES, and identification of related features, using ultrasound and magnetic resonance imaging. According to our findings, a diagnosis of RES should be considered in fetuses with a small TCD (severe cerebellar hypoplasia) and/or a round-shaped cerebellum on axial views, during the second or third trimester, especially when associated with ventriculomegaly. Partial RES is more common than previously thought, but presents an extreme diagnostic challenge, especially in cases with normal or nearly-normal cerebellar morphobiometric features. © 2021 International Society of Ultrasound in Obstetrics and Gynecology.

BACKGROUND AND OBJECTIVE: Eosinophilic esophagitis (EoE) is an increasingly prevalent chronic inflammatory disease of the esophagus with an immunoallergic etiology. Few studies have been published on EoE in children and adolescents. The objective of this study was to analyze the demographic, clinical, serologic, endoscopic-histologic, and atopic characteristics of pediatric patients with EoE and to identify atopic and digestive comorbidities. METHODS: We conducted a prospective observational study in children and adolescents (<16 years) assessed in a specialized multidisciplinary EoE unit in a tertiary referral hospital in a central region of Spain between 2011 and 2015. RESULTS: Thirty-five patients were included in the study. Twenty-eight (80%) were male. The mean age was 9.6 years, 83% were atopic, and 28% reported a family history of atopy. The most common symptom was dysphagia (51%). Eosinophilia was detected in the blood of 60% of patients. Eosinophil cationic protein and total IgE were elevated in 88% and 77% of patients, respectively. The most frequent endoscopic finding was linear grooves (57%). Skin tests with aeroallergens were positive in 82% of patients (pollen 62% and food 60%). The main atopic comorbidities were asthma (48%) and rhinoconjunctivitis (37%). Digestive diseases were more often associated with gastritis and Helicobacter pylori infection (17%). CONCLUSIONS: Our results are similar to those previously reported. EoE is more common in boys and in individuals with a history of atopy and sensitization to airborne allergens and food. These results support the consideration of EoE as an atopic disease and underline the important role of allergists in early diagnosis and treatment.

Objetivo: Se realiza una revisión de los primeros casos intervenidos en nuestro centro de inestabilidad glenohumeral recidivante de hombro por vía artroscópica. El objetivo del trabajo es analizar la influencia de la curva de aprendizaje en los resultados obtenidos. Material y métodos: Estudiamos 137 pacientes intervenidos en el Hospital 12 de Octubre de Madrid, entre Febrero de 1999 y Marzo de 2010; 101 pacientes cumplían los criterios de inclusión, y dichos pacientes fueron divididos en dos grupos por orden cronológico: los 50 primeros y los 50 segundos. No existieron diferencias estadísticamente significativas en cuanto a sexo, edad y lateralidad entre ambos grupos (p = 0.51, p = 0.15 y p = 0.23, respectivamente), siendo por tanto los grupos comparables. Comparamos entre ambos grupos los resultados clínicos, evaluados por el número de luxaciones, reintervenciones y complicaciones: migración de los implantes, artrosis y neuropatía del nervio axilar. Comparamos también los resultados funcionales valorados por las escalas de Constant y Rowe. Resultados: Se produjeron cuatro episodios de reluxación en el grupo 1 y seis episodios en el grupo 2. Respecto al número de reintervenciones, se realizaron tres en el grupo 1 y seis en el grupo 2. No existen diferencias estadísticamente significativas en el número de episodios de reluxación y reintervención (p = 50 y p = 0.48, respectivamente). En nuestra serie la curva de aprendizaje no parece asociarse con mayor número de complicaciones ni índice de reluxaciones.

The objective of this paper is to analyze the influence of the learning curve on the results obtained. INTRODUCTION: Neuromyelitis optica, or Devic's disease, is an inflammatory, demyelinating disease of the central nervous system that selectively affects the optic nerves and the spinal cord, with a high rate of relapses. Anti-aquaporin-4 (AQP4) antibodies are a highly specific marker for this condition. CASE REPORT: A 66-year-old female with longitudinally extensive dorsal transverse myelitis with complete remission following steroidal treatment and later acute relapse, with palsy in one limb. The differential diagnoses considered included a spinal tumour and arteriovenous malformation of the spinal cord. Being positive for AQP4 was the decisive factor in the final diagnosis. CONCLUSIONS: Early detection of anti-AQP4 antibodies together with appropriate immunotherapy can be the key to a better prognosis. An early diagnosis is essential to be able to start treatment at an early stage and thus prevent relapses and severe sequelae.

OBJECTIVES: Arthroscopic repair of Bankart lesion has become the treatment of choice of anterior shoulder instability. Our objective is to analyze the medium-term results of arthroscopic Bankart repair. MATERIAL AND METHODS: Between January 1999 and November 2007, 86 shoulders of 85 patients with diagnosis of relapsing shoulder dislocation were treated arthroscopically. After a mean 62-month follow-up (minimum 24 months) the results obtained were retrospectively assessed according the Rowe and Constant functional scales. A statistical analysis was done of the relation between functional results and age, sex, the side operated, capsulorrhaphy, rehabilitation and postoperative immobilization in our series. Moreover, the validity of MRI for diagnosing Bankart lesion was assessed. RESULTS: Eighty-five percent of patients had good or excellent results according to the Rowe scale. The mean Constant scale score was 90.6. There-dislocation rate in our series was 9%. The statistical analysis showed that female sex and immobilization time were related with worse results according to the Constant scale. CONCLUSIONS: In our hands, arthroscopic Bankart repair provides results similar to those in other published series. MRI is a useful diagnostic test in our setting, albeit its important implications for the diagnosis of Bankart lesion. Female sex and prolonged immobilization were related with worse functional results.

Primary ovarian lymphoma is a rare entity. We submit a case of a 34-year-old black patient presenting with a bilateral adnexal tumor. She underwent hysterectomy with double salpingo-oophorectomy followed by polychemotherapy treatment. Histology confirmed Epstein-Barr virus-positive bilateral Burkitt's lymphoma. The patient died from septic shock after a month of treatment. Endemic Burkitt's lymphoma has a predilection for the female genital tract, manifesting itself clinically as a pelvic mass and less frequently as a menstrual disorder. It is a rare entity in our environment but should be kept in mind when treating patients of African origin.

A 34-year-old woman, after 2 weeks of treatment with phenytoin and amitriptyline, developed fever and cutaneous lesions consisting of a generalized maculopapular rash and eosinophilia. Her biochemical data showed abnormal liver functions with increased levels of SGOT, SGPT, LDH, gamma-glutamyl transpeptidase and alkaline phosphatase. The skin biopsy pattern was compatible with phenytoin drug eruption of the erythemamultiforme-like type (lymphocytic exocytosis, isolated dyskeratotic cells, vacuolation of basal cells and incontinence of pigment). The patch tests were positive with phenytoin (patch test biopsy showed a typical eczematous pattern). The patch test with amitriptyline was negative. An oral challenge with amitriptyline showed an erythematous maculopapular rash. The challenge with phenytoin was not carried out because the previously abnormal liver function tests contraindicated the challenge. Although there are a few cases reported, the patch tests could be useful for diagnosing phenytoin allergy. Cross-reactivity between phenytoin and amitriptyline is possible.

RESUMO

The light optic investigation had been performed in rat embryos at the age of 10-20 days to study a successive formation of the small intestine wall and correlations between changes occurring in the nuclear volume and mitotic activity in its epithelium and mesenchyme. According to the ectodermal and mesodermal structures and to the changes in the nuclear volume and to the number of mitoses, it is possible to distinguish three periods. Certain correlations have been revealed between the nuclear volume and mitotic activity, which are of various significance during prespecific and specific differentiation
