RESUMO

Primary follicular lymphoma of the duodenum (FLD) is a rare variant of follicular lymphoma (FL), which represents only 1-4% of gastrointestinal non-hodgkin lymphomas (NHL). It usually appears in the second portion of the duodenum as micronodular lesions and the diagnosis is often incidental. Unlike other NHLs, the prognosis is excellent and the treatment ranges from "watch and wait" to rituximab-based immunochemotherapy regimens, depending on the symptoms and the presence of systemic involvement.

BACKGROUND: There is a paucity of literature examining the differences between patient-reported outcome measures after planned and unplanned cesarean delivery using a validated quality of recovery tool. The Obstetric Quality of Recovery-10 (ObsQoR-10) scoring tool has been validated to quantify functional recovery after cesarean delivery. We aimed to use the ObsQoR-10 to compare the postoperative recovery characteristics of patients undergoing planned and unplanned cesarean deliveries. METHODS: We conducted a prospective single-center observational study. Patients undergoing planned and unplanned cesarean deliveries under neuraxial anesthesia were asked to complete the ObsQoR-10 questionnaire 24 hours, 48 hours, and 1 week postpartum. We collected information on total in-hospital postoperative opioid consumption and patients´ perception of readiness for discharge at 24 and 48 hours postpartum. Additionally, patient characteristics were collected to assess their correlation with our findings. RESULTS: We included 112 patients (56 in each group). No statistical differences in ObsQoR-10 scores at 24 hours, 48 hours, and 1 week postpartum were observed between the planned and unplanned cesarean deliveries. Additionally, there was no difference between the groups in patients' perception of readiness for hospital discharge at 24 and 48 hours and opioid consumption in the first 2 days after surgery. Most patients in both groups did not think they would be ready for discharge at 24 hours postpartum. Analysis of the individual components of ObsQoR-10 at 24 hours showed a difference in the responses assessing the severity of shivering (higher in unplanned cesarean deliveries) and the ability to look after personal hygiene (lower in unplanned cesarean deliveries). CONCLUSIONS: As assessed by the ObsQoR-10, no significant difference in the quality of recovery was observed between patients undergoing planned and unplanned cesarean delivery.

Introduction. There is growing consensus globally that the consumption of ultra- processed food (UPF) can negatively affect the nutritional status of children. Objective. The present study aims to evaluate associations between the consumption of UPF and the nutritional status in a sample of Uruguayan and Brazilian preschoolers belonging to two studies: the ENDIS Study and the Pelotas 2015 Birth Cohort. Materials and methods. We conducted a cross-sectional analysis. The main outcome measure was obesity defined as BMI for age and sex ≥ +3 z-scores. The score of UPF consumption was the main exposure measured. Each positive answer of habitual intake was added up to create a UPF score ranging from zero to six or more UPF. Crude and adjusted logistic regressions were performed for the associations between UPF consumption and nutritional status in preschoolers. Results. The final sample consisted of 8,687 preschool children, 50.8% belonging to the Uruguayan study, while the remaining 49.2% belonged to the Brazilian study. Nearly 5% of the sample of young children were obese. We didn't observe a relationship between the score of UPF consumption and obesity, the odds ratio (OR) was 1.04 (95% CI, 1.00­1.09). Adjustments resulted in modest attenuation of the relationship and a lack of statistical significance. However, in children under 48 months, the score of UPF consumption was directly associated with childhood obesity. Conclusions. Results suggest that higher consumption of UPF is associated with obesity in Uruguayan and Brazilian preschool children under 4 years of age. These findings suggest that actions to reduce ultra- processed food consumption could lead to diminish obesity patterns and bring important public health benefits(AU)


Introducción. Existe creciente consenso a nivel mundial de que el consumo de alimentos ultraprocesados puede afectar negativamente el estado nutricional de los niños. Objetivo. El presente estudio tiene como objetivo evaluar asociaciones entre el consumo de ultraprocesados y el estado nutricional en una muestra de preescolares uruguayos y brasileños pertenecientes a dos estudios: el Estudio ENDIS y la Cohorte de Nacimiento de Pelotas 2015. Materiales y métodos. Se realizó un análisis transversal. La principal medida de resultado fue la obesidad definida como el IMC/edad y sexo ≥ +3 puntuaciones z. La puntuación del consumo de ultraprocesados fue la principal exposición. Cada respuesta positiva de la ingesta habitual se sumó para crear una puntuación de ultraprocesados que oscilaba entre cero y seis o más. Se realizaron regresiones logísticas crudas y ajustadas. Resultados. La muestra final estuvo constituida por 8.687 niños, de los cuales el 50,8% pertenecía al estudio uruguayo, mientras que el 49,2% restante pertenecía al estudio brasileño. Casi el 5% de la muestra de niños pequeños eran obesos. No observamos relación entre la puntuación del consumo de ultraprocesados y la obesidad, el odds ratio (OR) fue de 1,04 (IC 95%, 1,00-1,09). Los ajustes dieron lugar a modesta atenuación de la relación y falta de significación estadística. Sin embargo, en menores de 48 meses la puntuación de consumo de ultraprocesados se asoció directamente con la obesidad infantil. Conclusiones. Los resultados sugieren que un mayor consumo de ultraprocesados se asocia con obesidad en prescolares uruguayos y brasileños menores de 4 años. Estos hallazgos sugieren que las acciones para reducir el consumo de alimentos ultraprocesados podrían conducir a una disminución de los patrones de obesidad y traer importantes beneficios para la salud pública(AU)

Open-search methods allow unbiased, high-throughput identification of post-translational modifications in proteins at an unprecedented scale. The performance of current open-search algorithms is diminished by experimental errors in the determination of the precursor peptide mass. In this work we propose a semi-supervised open search approach, called ReCom, that minimizes this effect by taking advantage of a priori known information from a reference database, such as Unimod or a database provided by the user. We present a proof-of-concept study using Comet-ReCom, an improved version of Comet-PTM. Comet-ReCom increased identification performance of Comet-PTM by 68%. This increased performance of Comet-ReCom to score the MS/MS spectrum comes in parallel with a significantly better assignation of the monoisotopic peak of the precursor peptide in the MS spectrum, even in cases of peptide coelution. Our data demonstrate that open searches using ultra-tolerant mass windows can benefit from using a semi-supervised approach that takes advantage from previous knowledge on the nature of protein modifications. SIGNIFICANCE: The present study introduces a novel approach to ultra-tolerant database search, which employs prior knowledge of post-translational modifications (PTMs) to improve identification of modified peptides. This method addresses the limitations related to experimental errors and precursor mass assignation of previous open-search methods. Thus, it enables the study of the biological significance of a wider variety of PTMs, including unknown or unexpected modifications that may have gone unnoticed using non-supervised search methods.

Los desafíos en salud mental en Chile son múltiples y complejos. Por ello, la valorización de los recursos existentes permite establecer una línea base para mejorar el acceso, la cobertura y la efectividad de las medidas. El presente artículo tiene por objetivo estimar y analizar el gasto en salud mental realizado en Chile entre 2014 y 2021, abordando tanto la atención no ambulatoria como la ambulatoria. Así, el cálculo de los gastos en atención no ambulatoria para salud mental se obtuvo de la metodología de gasto por enfermedad (11,7% del total). Por otro lado, la estimación en atención ambulatoria para salud mental consideró múltiples fuentes de información por niveles de atención público y privado (56,9%). Adicionalmente, se estimaron los gastos en medicamentos realizados por las personas en el retail (31,4%). El gasto estimado en salud mental público alcanzó los $208 miles de millones, lo que representa un 1,7% del gasto público en salud de Chile el 2021. Asimismo, a nivel privado el gasto en salud mental fue de $332 miles de millones, esto es un 3,4% del gasto privado en salud. Además, se resalta la relevancia de considerar los costos asociados al reposo médico por salud mental que son 2 veces más que el gasto estimado. Se concluye comparando las estimaciones con otros países, mencionando limitaciones por falta de acceso a la información y algunos desafíos. Además, se destaca el problema de acceso a las prestaciones debido al alto porcentaje de copago (66%), y la necesidad de aumentar el presupuesto público destinado a la salud mental.

Stenotrophomonas maltophilia is an opportunistic pathogen, often associated with nosocomial infections. Ten S. maltophilia were isolated from clinical samples during the period January 2021 and June 2022. Eight (80%) patients had cancer as a background disease and 2 patients had coronavirus disease 2019. A fatal outcome was recorded in 4 cases (40% of patients). All the isolates were susceptible to minocycline and levofloxacin. Trimethoprim/sulfamethoxazole and ceftazidime resistance rates were 20% and 40% respectively. Eight different patterns were observed by Pulsed-Field Gel Electrophoresis, only two isolates being clonally identical. The isolation of S. maltophilia in clinical settings requires the implementation of infection prevention measures.

Introduction: A high prevalence of advanced breast cancer (BC) is a common scenario in Latin America. In Peru, the frequency of BC at Stages III/IV is ≈50% despite implementation of a programme for breast cancer screening (BCS) along the country. We carried out a study to assess the feasibility and develop an instrument to evaluate the knowledge, barriers and perception about BCS in a nationwide pilot study in Peru among candidates for BCS. Methods: We conducted a systematic review of 2,558 reports indexed in PubMed, Scopus, Web of Science, Medline-Ovid and EMBASE, regarding to our study theme. In total, 111 were selected and a 51-items survey was developed (eight items about sociodemographic characteristics). Patients were recruited in public hospitals or private clinics, in rural and urban areas of nine departments of Peru. Results: We surveyed 488 women from: Lima (150), Cajamarca (93), Ica (59), Arequipa (56), Loreto (48), Ancash (38), Junín (15), Puerto Maldonado (15) and Huancavelica (14); 27.9% of them were from rural areas. The mean of age was 53.3 years (standard deviation ± 9.1). Regarding education level, 29.8% had primary, 33.2% secondary and 37.0% higher education. In total, 28.7% of women did not know the term 'mammogram' and 47.1% reported never receiving a BCS (36.9% from urban and 73.5% from rural population). In women that underwent BCS, only 67% knew it is for healthy women. In total, 54.1% of patients had low levels of knowledge about risk factors for BC (i.e. 87.5% of women respond that injuries in the breast produce cancer). Cultural, economic and geographic barriers were significantly associated with having a mammogram where 56.9% of participants considered a cost ≤ 7 USD as appropriate. Mammogram was perceived as too painful for 54.9% of women. In addition, women with a self-perception of low-risk for BC and a fatalistic perception of cancer were less likely to have a BCS. Conclusion: We found that it is feasible to conduct a large-scale study in Peru. The results of this pilot study highlight an urgent need of extensive education and awareness about BCS in Peru.

Introducción: La educación médica ha propiciado estrategias de enseñanza-aprendizaje que integran nuevas actividades pedagógicas y evaluativas acordes con el avance en el conocimiento. Los juegos educacionales (JE) surgen como una opción en pro del mejoramiento en los objetivos y resultados educacionales, con una utilidad potencial dentro del proceso formativo de los cirujanos. Hay investigaciones crecientes sobre el tema. Objetivo: evaluar de forma cualitativa el estado de la evidencia disponible sobre la efectividad de los JE como estrategia pedagógica en estudiantes de postgrado de cirugía general. Método: revisión de alcance de la literatura, según la metodología del Joanna Briggs Institute. Se evaluó el impacto educacional de los JE según la clasificación de Kirkpatrick. Resultados: la evidencia acerca de la utilidad los JE en la educación médica en cirugía es limitada. Existe una heterogeneidad de metodologías, procesos y resultados educativos producto de implementar JE como instrumento de evaluación o de instrucción. No obstante, es posible un impacto positivo de acuerdo con las investigaciones. No hay una evaluación de los alcances y el rol de los JE en la formación de los cirujanos. Conclusión: los JE se constituyen en una estrategia novedosa a considerar en la educación en cirugía general. Su implementación se condiciona a la construcción de una evidencia sólida, reproducible y basada en objetivos claros de enseñanza-aprendizaje. Sin embargo, la investigación en el tema es incipiente.


Summary Background: Medical education has fostered teaching-learning strategies that integrate new pedagogical and evaluative activities in accordance with the advancement in knowledge. Educational games (EG) emerge as an option for improving educational objectives and results, with potential utility within the training process of surgeons. There is growing research on the subject. Objective: To qualitatively evaluate the state of the available evidence on the efficacy of EG as a pedagogical strategy in general surgery postgraduate students. Method: Scoping review of the literature, according to the methodology of the Joanna Briggs Institute. The educational impact of the EG was evaluated according to the Kirkpatrick classification. Results: Evidence about the usefulness of EGs in medical education in surgery is limited. There is a heterogeneity of methodologies, processes and educational results resulting from implementing EG as an evaluation or instructional instrument. However, a positive impact is possible according to research. There is no evaluation of the scope and role of EBs in the training of surgeons. Conclusions: The EG constitute a novel strategy to consider in general surgery education. Its implementation is conditional on the construction of solid, reproducible evidence based on clear teaching-learning objectives. However, research on the subject is incipient.

Self-sampling methods for HPV testing have been demonstrated to be highly sensitive and specific. The implementation of these methods in settings with a lack of infrastructure or medical attention has been shown to increase the coverage of cervical cancer screening and detect cervical abnormalities in the early stages. The aim of this study is to compare the acceptability of urine and vaginal self-sampling methods versus clinician sampling among rural women. A total of 120 women participated. Each participant self-collected urine and vaginal samples and underwent clinician sampling for Pap smear and HPV testing. After the sample collection, a questionnaire to qualify the device, technique, and individual acceptability was applied, and the additional overall preference of three sample tests was evaluated. Results: The characteristics of the participants were as follows: median age of 35 years; 40.8% were married; 46.7% had a primary level of education; median age of sexual onset of 17.6 years. Compared with clinician sampling, both vaginal self-sampling, OR 20.12 (7.67-52.8), and urine sampling, OR 16.63 (6.79-40.72), were more comfortable; granted more privacy: vaginal self-sampling, OR 8.07 (3.44-18.93), and urine sampling, OR 19.5 (5.83-65.21); were less painful: vaginal self-sampling, OR 0.07 (0.03-0.16), and urine sampling, OR 0.01 (0-0.06); were less difficult to apply: vaginal self-sampling, OR 0.16 (0.07-0.34), and urine sampling, OR 0.05 (0.01-0.17). The overall preference has shown an advantage for vaginal self-sampling, OR 4.97 (2.71-9.12). No statistically significant preference was demonstrated with urine self-sampling versus clinician sampling. Conclusions: Self-sampling methods have a high acceptance in rural communities. Doubts on the reliability of self-sampling often appear to be a limitation on its acceptability. However, the training and education of the community could increase the uptake of these methods.

Resumen Introducción: el Síndrome de Cohen es una enfermedad genética monogénica autosómica recesiva, que se origina a partir de mutaciones en el gen VPS13B (COH1). Se caracteriza por obesidad, retraso psicomotor, microcefalia, hipotonía, miopía progresiva, distrofia retiniana, neutropenia intermitente y rasgos faciales particulares. Objetivo: presentar el segundo caso reportado en Colombia, que fue confirmado mediante estudio molecular. También se presenta una breve revisión de la literatura médica más reciente sobre esta patología. Caso clínico: adolescente de 14 años con microcefalia, trastorno cognitivo, malformaciones menores asociadas, neutropenia y obesidad, con mutación homocigota del gen VPS13B. Conclusión: a pesar de ser un síndrome poco común, con importante variabilidad fenotípica, debe sospecharse con base en los criterios clínicos y en las patologías asociadas.


Abstract Introduction: cohen's syndrome is an autosomal recessive monogenic genetic disease, which originates from mutations in the VPS13B (COH1) gene. It is characterized by obesity, psychomotor retardation, microcephaly, hypotonia, progressive myopia, retinal dystrophy, intermittent neutropenia, and classic facial features. Objective: to present the second case reported in Colombia, which was confirmed by molecular study. A brief review of the most recent medical literature on this pathology is also presented. Clinical case: a 14-year-old adolescent with microcephaly, cognitive disorder, minor associated malformations, neutropenia, and obesity, with a homozygous VPS13B gene mutation. Conclusion: despite being a rare syndrome, with significant phenotypic variability, it should be suspected based on clinical criteria and associated pathologies.

The stabilization of Pickering emulsions by nanoparticles has drawn great interest in the field of food science and technology. In this study, α-Lactalbumin nanoparticles prepared by the desolvation and cross-linking method from protein solutions with initial pH values of 9 and 11 were used to stabilize squalene-rich amaranth oil Pickering o/w emulsions. The effect of different concentrations of nanoparticles on the size, size distribution, ζ potential, and emulsion stability was evaluated using dynamic light scattering, electron microscopy, and light backscattering. Dependence of the emulsions' droplet size on the nanoparticle concentration was observed, and the critical coverage ratio was reached when 5-10% nanoparticles concentration was used. Our findings suggest that α-LA nanoparticles at a 10% concentration can be used as novel stabilizers for Pickering emulsions to provide protection for beneficial lipophilic bioactive compounds. This is the first time that native α-LA nanoparticles have been used as stabilizers of Pickering emulsions.

BACKGROUND: HPV primary screening has shown effectiveness for cancer prevention; however, gynaecological examination is considered uncomfortable. Self-sampling methods increase the acceptance of screening. The aim of this study is to compare the sensitivity and specificity of clinician sampling versus vaginal and urine self-sampling for HPV diagnosis. METHODS: A diagnostic test study was conducted in a rural parish of Cuenca, Ecuador. A total of 120 women participated. Each participant self-collected urine and vaginal samples and underwent clinician sampling for HPV testing. The latter was considered as the golden standard. All three samples were processed with the same amplification and hybridization protocol for HPV detection (Hybribio) following the manufacturer's instructions. RESULTS: Characteristics of the participants were: median age 35 years; 40.8% married; 46.7% had a primary level of education; and median age of sexual onset, 17.6 years. The prevalence of any type of HPV with clinician sampling was 15.0%, 17.5% with urine sampling and 18.3% with vaginal self-sampling. Self-sampling sensitivity reached 94.4% (IC 74.2-99.9), and specificity 92.1% (IC 85.2-95.9). Urine sampling had a sensitivity of 88.8% (IC 67.2, 96.9), and specificity 94.1% (IC 67.2-96.9). The negative predictive value was 98.9% (IC 94.2-99.8) for vaginal self-sampling and 97.6% (IC 92.6-99.4) for urine sampling. CONCLUSIONS: This study shows that vaginal and urine self-sampling methods have similar sensitivity and specificity compared with clinician sampling for the diagnosis of HPV. The correlation between HPV genotypes among the three tests is satisfactory.

OBJECTIVE: Assess the safety and efficacy of upcoming stem cell treatments and analyze their effects on the cognitive and behavioral impairments in patients diagnosed with autism. METHODS: We included controlled and noncontrolled, randomized and non-randomized trials evaluating stem cell therapy as a treatment in patients with autism spectrum disorder compared to placebo or without comparator. DATA SOURCES: Scopus, Web of Science, MEDLINE and EMBASE. Risk of bias was assessed using Cochrane's Risk of Bias tool and the NIH's Quality Assessment Tool for Studies With No Control Group. RESULTS: Eleven trials including 461 patients proved eligible. ABC scale meta-analysis showed a mean raw of -11.97 in the intervention groups (95 % CI -91.45 to 67.52, p < 0.01). CARS scale reported a mean raw of -9.08 (95 % CI -15.43 to -2.73, p < 0.01). VABS scale was reported by their domains: communication domain reported a mean raw of 2.69 (95 % CI 1.30 to 4.08, p = 0.92); daily living domain, 1.99 (95 % CI 0.83 to 3.15, p = 0.51); motor domain, 1.06 (95 % CI -0.37 to 2.48, p = 0.20); socialization domain, 3.09 (95 % CI 1.71 to 4.48, p = 0.61); adaptive behavior domain, 2.10 (95 % CI 1.04 to 3.16, p = 0.36). Furthermore, the most common side effects reported included fever, hyperactivity, vomit, headache, and aggressiveness; no serious adverse events were reported. CONCLUSIONS: The body of evidence suggests that stem cell therapy significantly improves scales in patients with autism spectrum disorder, hence, future studies should help us have more confidence in the results. We found no serious adverse events related to the stem cell therapy.

Objetivo Realizar un análisis bibliométrico sobre el liderazgo femenino en medicina para conocer el panorama actual y dirigir futuras investigaciones. Métodos Se realizó un análisis bibliométrico descriptivo retrospectivo de la literatura disponible en MEDLINE en relación con el liderazgo de mujeres médicas de 1973 al 2019. Se utilizó Pubmed y FABUMED, y la siguiente estrategia: ("Leadership"[Mesh]) AND "Physicians, Women"[Mesh] (1973:2019[dp]). Se incluyeron artículos originales, de revista, ensayos, informes científicos y reseñas, para analizar toda la literatura indexada relacionada. Para la obtención del factor de impacto (FI), se utilizó el Journal Citation Reports (7)2017/2018 de las revistas encontradas para estimar la calidad de cada una de ellas. Resultados Un total de 310 referencias fueron encontradas en 139 revistas publicadas, el 71,2% fueron artículos originales, el resto revisiones. Hubo un incremento en las publicaciones de 1996 al 2019 en un 11,9%. Las revistas con mayor porcentaje de publicaciones fueron: Academic Medicine y J Womens Health (Larchmt), con 7,7% y 7,1%. Los países con mayores publicaciones fueron Estados Unidos, con 66 (56%) y Reino Unido con 10 (8,6%). América latina tiene cuatro publicaciones y Colombia no cuenta con ninguna publicación indexada. Conclusión El liderazgo femenino en medicina y las publicaciones sobre el tema han aumentado en las últimas décadas en países industrializados. Por su parte, Latinoamérica debe aumentar sus esfuerzos en publicar en revistas con alto factor de impacto para trabajar por la equidad de género.


Objective Perform a biometrical analysis of female leadership in medicine, in order to know the actual perspective and guide future research. Materials and Methods: A retrospective descriptive bibliometric analysis of the literature available in MEDLINE was performed in relation to the leadership of female physicians from 1973 to 2019, through MEDLINE database, using GoPubmed and FABUMED. The following strategy was deployed: ("Leadership"[Mesh]) AND "Physicians, Women"[Mesh] (1973:2019[dp]). Original articles, journal articles, essays, scientific reports and reviews were included to analyze all related indexed literature. To obtain the impact factor (FI), the Journal Citation Reports (7) 2017/2018 of the journals found was used to estimate the quality of each one of them. Results A total of 310 references were found in 139 published journals, 71.2% were original articles; the remaining 22% were reviews. There was a 11.9% increase in publications from 1996 to 2019. The journals with the highest rate of publications were: Academic Medicine and J Womens Health (Larchmt) with 7.7% and 7.1%. Countries with the highest number of publications were United States with 66 (56%), and United Kingdom with 10 (8.6%). Latin America has four publications, while Colombia has none. Conclusion Female leadership in the medical field and the publications regarding this subject have increased in the last decades, mainly in developed countries. Latin America has to make significant efforts to produce more publications in journals with high impact factor, in order to close the gender gap in the medical field.

Oxaliplatin is a chemotherapeutic agent with direct toxic action on deoxyribonucleic acid (DNA), which is known to cause an arrest in its synthesis and inducing cell death. It is a crucial medication for colorectal carcinoma, and in combination with other medications has demonstrated to exhibit synergism, managing to increase patients' survival, especially when compared to monotherapy with 5-fluoracil. Neurotoxicity is its most well-known adverse effect. However, other less frequent secondary effects have been described in case reports, among them liver injury, which is usually secondary to liver sinusoid injury. Despite the wide frequency of the use of this drug, the relationship of oxaliplatin with the development of portal non-cirrhotic hypertension is largely unknown, which translates into a sub-diagnosis, representing an additional risk to patients who develop this complication. We present the case of an adult patient, who during treatment with the FOLFOX scheme for colorectal carcinoma, presents signs suggestive of portal hypertension, without other risk factors besides the administration of oxaliplatin.

RESUMO

ROS proto-oncogene 1 (ROS1) encodes a type I integral membrane protein with tyrosine kinase activity and whose activating alterations are involved in the aggressiveness of several tumor types. Fusions involving ROS1 gene are present in 1-2% of lung adenocarcinomas and other solid tumors. Entrectinib, also known as RXDX-101, is a potent second-generation, multitarget oral inhibitor against NTRK1, NTRK2, NTRK3, ALK, and ROS1 with the ability to cross the blood-brain barrier. Results of Phase I and II trials have led the Food and Drug Administration to grant approval to entrectinib for the treatment of patients with metastatic, ROS1-positive non-small cell lung cancer (NSCLC). In this review, we will describe the biology of ROS1, as well as results of the efficacy and safety of different clinical trials evaluating entrectinib in ROS1-positive NSCLC.

BACKGROUND: The objective of the study was to review the outcome of patients with parotid cancer treated with postoperative radiotherapy at Complejo Hospitalario de Navarra in the last ten years. MATERIALS AND METHODS: We retrospectively reviewed patients treated with adjuvant radiotherapy between January 2008 and December 2018. We analyzed demographic data, histopathologic findings, local control (LC) and overall survival (OS). RESULTS: A total of 40 patients received postoperative radiotherapy during the period mentioned. There were 22 men (55%) and 18 women (45%). Median age was 58 years (19-90). By tumor histology, the most common was squamous cell carcinoma (22.5%) followed by ex-pleomorphic adenoma (15%) and adenoid cystic carcinoma (10%). According to Surgery, 19 patients (47.5%) underwent a total parotidectomy, 20 (50%) partial parotidectomy, and 1 (2.5%) a radical parotidectomy. Twenty-one patients (51.2%) underwent cervical dissection, most of them being supraomohyoid (31.7%). Reasons for adjuvant RT were: R1 resection (35% of the patients), high grade tumors (27.5%) and 17.5% because R1 surgery and R1. Radiation was administered using IMRT in most patients to a total dose of 60 Gy in 30 fractions. The 5-year overall survival (OS) (Kaplan-Meier) was 81% (95% CI: 68.5-96.2%), and 10-years - 64%. The 5-year local control (LC) (Kaplan-Meier) was 82.4% (95% CI: 91.46-73.33%) and the 10-year LC - 72.2% (95% CI: 54.9-96%). To date, only 4 patients (10%) have died due to their parotid tumor. CONCLUSION: The adjuvant radiotherapy added to surgery, significantly reduces the risk of recurrence in high-risk patients with a very acceptable survival rate.

Introducción: los profesionales de enfermería son los cuidadores que prestan la mayor parte de la atención directa que reciben los pacientes. Comprender lo que los pacientes perciben como comportamientos de cuidado es esencial para adaptar las intervenciones de enfermería de tal manera que se llegue a satisfacer las necesidades del paciente. Objetivo: describir la percepción de los comportamientos de cuidado otorgados por enfermería que tienen los profesionales del área en una clínica de alta complejidad de la ciudad de Medellín (Colombia). Método: estudio descriptivo, transversal. Se incluyeron 81 profesionales de enfermería que laboraban en la institución para el 2018. Se utilizó un instrumento que mide la percepción del cuidado otorgado por el profesional, el cual se denomina: evaluación de los comportamientos de cuidado otorgado por enfermería (ECCOE). Resultados: de los 81 profesionales de enfermería que participaron en el estudio, la percepción de cuidado otorgado por enfermería tuvo una valoración global de 89,4%. En la relación de las puntuaciones promedio obtenidas en cada subescala, se observó mayores promedios para el soporte/protección/ambiente con 92,3%, asistencia en las necesidades humanas con 92,0%, humanismo/fe-esperanza/sensibilidad con 91,8%, y promedios menores en fuerzas existenciales/fenomenológica/espiritual con 88,6% ayuda/confianza con 86,5%, expresión de sentimientos positivos/negativos con 85,5% y enseñanza/aprendizaje con 84,7%. Conclusión:los profesionales perciben los comportamientos de cuidado de manera positiva. Fue posible establecer aspectos como ayudar al paciente a fijarse metas para su salud, planificar cómo lograrlas y no abandonarlo ni alejarse cuando esté triste. Los hallazgos permiten orientar los comportamientos de cuidado para adaptar las intervenciones de enfermería y satisfacer las necesidades del paciente.


Nurse practitioners are the caregivers who provide most of the direct care received by patients. Understanding what patients perceive as caring behaviors is essential to tailor nursing interventions and meet the patient's needs. Objective: To describe the perception of nursing care behaviors that professionals in the area have in a high complexity clinic in the city of Medellín (Colombia). Method: a descriptive, cross-sectional study. Eighty-one nursing professionals who worked in the institution for 2018 were included. An instrument that measures the perception of the care provided by the professional called Assessment of Nursing Care Behaviors (ECCOE by its Spanish acronym) was used. Results: of the 81 nurse practitioners who participated in the study, the perception of care provided by nursing had a global assessment of 89.4%. Regarding the average scores obtained in each subscale, higher averages were observed for support / protection / environment with 92.3%; assistance in human needs with 92.0%; humanism / faith-hope / sensitivity with 91.8%; and lower averages in existential / phenomenological / spiritual forces with 88.6%; help / trust with 86.5%, expression of positive / negative feelings with 85.5%; and teaching / learning with 84.7%. Conclusion: professionals perceive care behaviors positively. It was possible to establish aspects such as helping the patient set goals for his health, planning how to achieve them, and not abandoning him or walking away when he is sad. The findings allow guiding care behaviors to adapt nursing interventions and meet the needs of the patient.

Compreender o que os pacientes percebem como comportamentos de cuidado é essencial para adequar as intervenções de enfermagem de forma a atender às necessidades do paciente. Objetivo: descrever a percepção dos comportamentos assistenciais de enfermagem que os profissionais da área têm em uma clínica de alta complexidade na cidade de Medellín (Colômbia). Método: estudo descritivo, transversal. Foram incluídos 81 profissionais de enfermagem que atuavam na instituição em 2018. Foi utilizado um instrumento que mede a percepção do cuidado prestado pelo profissional, nomeado: avaliação dos comportamentos de cuidado de enfermagem (ACCOE). Resultados: dos 81 profissionais de enfermagem que participaram do estudo, a percepção do cuidado prestado pela enfermagem teve uma avaliação global de 89,4%. Na relação dos escores médios obtidos em cada subescala, foram observadas médias superiores para suporte / proteção / meio ambiente com 92,3%, atendimento às necessidades humanas com 92,0%, humanismo / fé-esperança / sensibilidade com 91,8% e médias inferiores nas forças existenciais / fenomenológicas /espirituais com 88,6% ajuda / confiança com 86,5%, expressão de sentimentos positivos / negativos com 85,5% e ensino /aprendizagem com 84,7%. Conclusão: os profissionais percebem os comportamentos de cuidado de forma positiva. Foi possível estabelecer aspectos como ajudar o paciente a traçar metas para sua saúde, planejar como alcançá-las e não abandoná-lo ou ir embora quando está triste. Os achados permitem nortear comportamentos de cuidado para adequar as intervenções de enfermagem e atender às necessidades do paciente
