RESUMO

Reaction of mast cells of adult male Wistar rats (n=15) in the zone of polypropylene mesh fixation was studied by histochemical, immunohistochemical, and traditional morphological methods on days 1, 5, 10, and 30 after implantation. Immediately after the intervention, mast cells stimulated the processes aimed at wound healing. Secretion of mast cells was clearly regulatory. These cells migrated to the zone of injury for subsequent activation of their function. The number of cNOS+ mast cells near the polypropylene mesh was maximum on day 1 and the number of iNOS+ mast cells peaked on day 5 of the experiment, which probably represented a compensatory reaction. Presumably, stimulation of fibrillogenesis was largely due to the activatory effect of mast cells on the fibroblast function, but not to collagen production by these mast cells.

AIM: To evaluate the effect of various methods of thoracoplasty on local production of cytokines in patients with pulmonary tuberculosis. MATERIAL AND METHODS: The main group consisted of patients after traditional thoracoplasty. Thoracoplasty was compared with endoprosthesis of polypropylene mesh 'Surgipro - SPMM-149'. TNF-α and IL-10 cytokines in exudate from surgical area were measured by using of solid-phase enzyme immunoassay (OOO 'Vector-Best', Russia). RESULTS: It was revealed that local production of pro- and anti-inflammatory cytokines is more pronounced after thoracoplasty with polypropylene mesh 'Surgipro - SPMM-149'. Local cytokine balance changes towards inflammation were more pronounced after thoracoplasty with mesh implant and did not depend on the degree of drug sensitivity.

The effect of hyperlipidemia on the systemic production of cytokines was evaluated in the plasma of rats after anterior abdominal wall reconstruction with polypropylene (Esfil) and polytetrafluorethylene (Ecoflon) endoprostheses. The reference group was formed from animals without hyperlipidemia, in which anterior abdominal wall plasty was carried out with the use of the same endoprostheses. Intact rats without hyperlipidemia and intervention served as the control. Hypercytokinemia was detected on days 1-10 after reconstructive surgery on the anterior abdominal wall. Polytetrafluoroethylene alloplasty was associated with higher plasma production of pro- and anti-inflammatory cytokines, which characterized mainly the implant reactogenicity. Hyperlipidemia modified the reactivity of immunocompetent cells to the endoprosthesis. A progressive decrease in the content of proinflammatory cytokines (IL-1ß and TNF-α) was observed by day 10 after endoprosthesis implantation, this decrease being more manifest in response to polytetrafluoroethylene. High plasma levels of pro- and antiinflammatory cytokines (IL-1ß, INF-α, IFN-γ, and IL-10) were detected in hyperlipidemic rats on day 1 after polypropylene plasty; by day 10, the plasma level of IFN-γ increased, which reflected positive activation of Th1 lymphocytes.

RESUMO

It was analyzed the results of treatment of 23 patients with large paracolostomic hernias. Twenty patients underwent colostomy suturing and hernial ring Onlay-plasty with polypropylene mesh without tension. Onlay-plasty of hernial ring with own tissues and polypropylene mesh and colostomy reconstruction outside of implant were performed in 3 patients. Onlay-alloplasty with polypropylene mesh "PROLENE" is effective method of treatment of postoperative paracolostomic ventral hernias with colostomy closing as well as with its reconstruction outside of implant.

RESUMO

We studied the effects of Esphyl polypropylene mesh and Ecoflon polytetrafluoroethylene endoprostheses on local and systemic production of cytokines. Polytetrafluoroethylene is a more reactogenic material than polypropylene; it stimulates mainly the local production of pro-inflammatory cytokines. The local anti-inflammatory effect of polypropylene was less pronounced, but persisted for longer time.
