RESUMO

OBJECTIVE: This cross-sectional study aimed to evaluate the presence of bone changes on magnetic resonance imaging (MRI) scans of patients with temporomandibular joint (TMJ) anterior disc displacement (ADD) with reduction (ADDWR) and without reduction (ADDWoR). METHODS: TMJ-MRI scans were screened for the presence of ADD. 285 scans presented ADD, being further divided into ADDWR (n = 188) and ADDWoR (n = 97). Bone changes on the mandibular condyle and articular eminence were also assessed and computed. The chi-square test compared the association of these bone changes with the presence of ADDWR and ADDWoR, with a significance level of 5 %. Also, the prevalence ratio (PR) was calculated. RESULTS: In the mandibular condyle, subchondral cyst (p = 0.035, PR = 1.08) and bone edema (p = 0.044, PR = 2.40), more prevalent on ADDWR, and generalized sclerosis (p = 0.015, PR = 1.04), more prevalent on ADDWoR, presented significant association with ADD. On the articular eminence, generalized sclerosis (p = 0.015, PR = 1.04) and articular surface flattening (p = 0.003, PR = 1.19) presented significant association with ADD, both more prevalent on ADDWoR. CONCLUSION: Bone changes are usual findings in TMJ with ADD diagnosis. The real influence of ADD is not fully clear, although clinicians should be aware of patients with this condition, to provide an early diagnosis and improve patient´s prognosis.

RESUMO

OBJECTIVE: The aim of this study was to evaluate the influence of binarization methods (BnMs) on the fractal dimension (FD) of the alveolar bone in digital radiographic images. STUDY DESIGN: Five identical periapical radiographs were obtained from each of 20 anatomic regions. Using ImageJ/Fiji software, a standardized region of interest was positioned in the trabecular bone of all radiographs, 14 BnMs were independently applied, and FD was calculated. The Friedman test and Dunn post hoc tests were used to evaluate the influence of BnMs on FD (α = 0.05). The intraclass correlation coefficient (ICC) was used to assess correlation among BnMs. RESULTS: No significant differences were observed in the threshold value between the BnMs. No significant differences in FD were observed among 9 BnMs (Default, Huang, Iso Data, Li, Mean, Min Error[I], Moments, Otsu, and Shanbhag). These methods presented significantly higher FDs than those from 3 other BnMs (Max Entropy, Renyi Entropy, and Yen), which did not significantly differ from each other. The Triangle method significantly differed from all BnMs, and the Percentile method produced significantly higher FDs than Huang, IsoData, Li, Moments, Otsu, and Shanbhag. Most BnMs presented good or excellent correlation (ICC ≥ 0.76). CONCLUSIONS: Binarization methods influence FD of the alveolar bone from digital periapical radiographs; thus, this technical aspect is relevant for comparison purposes and should be standardized. For comparison of published FD research, the Default method is recommended.

RESUMO

Abstract The study aimed to assess the influence of excessive use of a photostimulable phosphor plate (PSP) on the density, image noise, and contrast of radiographs. For that, radiographs of an acrylic block were acquired with a PSP of the Express intraoral system to assess the density and image noise. Initially, five images were obtained and exported (first group). After 400 exposures to X-rays and scannings of PSP, other five images were obtained and exported (second group). The same procedure was done after 800 (third group), 1200 (fourth group), 1600 (fifth group), and 2000 acquisitions (sixth group), resulting in 30 images to be evaluated. The mean and standard deviation of the gray values were calculated for the images using the ImageJ software. For contrast analysis, radiographs of an aluminum step-wedge were acquired with a new PSP following the same acquisition intervals. The percentage of contrast variation was calculated. Another two unused PSP receptors were employed to evaluate the method's reproducibility. The comparison of the results among the acquisition groups was performed with one-way Analysis of Variance (α=0.05). Intraclass Correlation Coefficient (ICC) assessed the reproducibility of the receptors. Image noise did not differ among the groups (p>0.05). There was a slight increase in density after 400 acquisitions and a slight difference in contrast for all acquisition groups without a pattern of increase or decrease (p<0.05). ICC showed excellent reliability for the methods. Therefore, excessive PSP use slightly affected the radiograph's density and contrast.

OBJECTIVE: The aim of this study was to evaluate the influence of the extensive use of a photostimulable phosphor (PSP) receptor on the diagnosis of proximal carious lesions. STUDY DESIGN: Forty posterior teeth were paired and mounted in 20 phantoms. Using a PSP receptor of the Express intraoral system, 1 radiograph of each phantom was acquired, totaling 20 radiographs (group 1). After 400 acquisitions, another radiograph of each phantom was obtained, totaling 20 new radiographs (group 2). This procedure was also done after 800 (group 3), 1,200 (group 4), and 1,600 (group 5) acquisitions, resulting in 100 radiographs. Another 40 radiographs (20 phantoms × 2 groups) were obtained with a second PSP sensor to evaluate the diagnostic replicability of the receptors. All 140 radiographs were assessed independently by 5 examiners for the presence of proximal caries. Area under the receiver operating characteristic curve, sensitivity, and specificity were calculated and compared among the groups by analysis of variance. The Student t test evaluated the diagnostic replicability of the receptors. The significance level was set at 5% (P < .05). RESULTS: The diagnostic outcomes for proximal caries did not differ among the groups (P ≥ .37). The diagnostic values did not differ among the radiographs acquired with the 2 PSP receptors (P ≥ .75). CONCLUSIONS: The diagnosis of proximal carious lesions was not affected by extensive use of the PSP receptor.

RESUMO

Abstract This crossover study aimed to compare the anesthetic effects of buffered 2% articaine with 1:200,000 epinephrine with that of non-buffered 4% articaine with 1:200,000 epinephrine. Forty-seven volunteers were administered two doses of anesthesia in the buccal region of the second mandibular molars in two sessions using 1.8 mL of different local anesthetic solutions. The onset time and duration of pulp anesthesia, soft tissue pressure pain threshold, and the score of pain on puncture and burning during injection were evaluated. The operator, volunteers, and statistician were blinded. There were no significant differences in the parameters: onset of soft tissue anesthesia (p = 0.80), duration of soft tissue anesthesia (p = 0.10), onset of pulpal anesthesia in the second (p = 0.28) and first molars (p = 0.45), duration of pulp anesthesia of the second (p = 0.60) and first molars (p = 0.30), pain during puncture (p = 0.82) and injection (p = 0.80). No significant adverse events were observed. Buffered 2% articaine with 1:200,000 epinephrine did not differ from non-buffered 4% articaine with 1:200,000 epinephrine considering anesthetic success, safety, onset, duration of anesthesia, and pain on injection.

RESUMO

Introduction: The aim of this study was to assess the agreement between oral and maxillofacial radiologists (OMFR) and oral and maxillofacial surgeons (OMFS) for the detection of bifid mandibular canal (BMC) and accessory mental foramen (AMF) using cone-beam computed tomography (CBCT). Material and Methods: This retrospective study involved 22 examiners (11 OMFR and 11 OMFS) who independently assessed 30 CBCT volumes from patients (n = 60 hemi-mandibles) under preoperative radiographic evaluation for implant placement. The examiners scored the presence of BMC and AMF in each hemimandible. The interexaminer agreements were assessed using Fleiss' kappa statistics. Results: For intra-examiner agreement, 40% of the sample was reevaluated. The interexaminer agreement between OMFR and OMFS was slight (0.12) for the detection of BMC and fair (0.24) for AMF. The agreement among OMFR for detection of BMC was fair (0.22), and it was slight among OMFS (0.15). The agreement among OMFR for detection of AMF was substantial (0.61), and among OMFS it was fair (0.22). Agreements between OMFR and OMFS were slight for BMC and fair for AMF, independently of the years of experience. Intraexaminer agreement ranged from 60% to 90% among OMFR and from 55% to 90% among OMFS. Conclusion: A slight and a fair agreement between OMFR and OMFS was found for the detection of BMC and AMF, respectively. In general, OMFR obtained higher agreement among themselves, mainly for detection of AMF.

RESUMO

BACKGROUND: Obesity and bariatric surgery may be related with mental and oral disorders. AIM: To evaluate the impact of bariatric surgery on anxiety, initial dental caries lesion and gingival bleeding in obese patients. METHODS: Eighty-nine patients were divided in two groups: Control Group (CG) - obese patients and Experimental Group (EG) - patients submitted to bariatric surgery. EG was analyzed before and 12 months after bariatric surgery; for the CG, was respected an interval of 12 months between the evaluations. International Caries Detection and Assessment System, Gingival Bleeding Index and Trace-State Anxiety Inventory were used. Medical profile, anthropometrics data, sociodemographic and behavioral variables were considered. RESULTS: There were no statistically significant differences between groups in evaluation times regarding to initial dental caries lesion and anxiety. However, the number of teeth with initial dental caries lesion (p=0.0033) and gingival bleeding (p<0.0001) increased significantly after bariatric surgery in EG. CONCLUSION: These results reinforce the need for multi-professional team follow-up, including dental care, for both obese and bariatric patients.

RESUMO

ABSTRACT The aim of this study was to emphasize the importance of monitoring vital signs, especially blood pressure, during dental care. Through a narrative review, the changes in blood pressure that may occur during outpatient procedures and the measures to be taken were discussed. The available literature was consulted in databases (PubMed, Scielo, Web of Science and Google Scholar), complemented by the analysis of the bibliographic references included and updated until April 2021. The keywords were used: "Anamnesis", "Arterial Pressure", "Arterial Hypertension", "Hypertensive Crisis", "Risk factors", "Stroke", "Subclavian Theft Syndrome" and "Atherosclerosis". It was concluded that care should be applied to all patients, but mainly to those over 50, hypertensive or not, with hypercholesterolemia, diabetes, anxious and fearful. All vital signs must be monitored, including the pain score. The situations of anxiety, pain and fear are triggers for changes in blood pressure even in normotensive patients as in controlled hypertensive patients. The blood pressure measurement, in the first consultation, must be performed in both arms and repeated annually or whenever there is a change in the patient's health status. In all other consultations, blood pressure control, as well as the assessment of all vital signs, must be performed before, during, after the procedure and before discharge. In the face of any major change, it is necessary to assess the risk/benefit ratio of the continuity of the procedure, so that risks to patients are avoided or minimized, as well as for the necessary referrals to be made.

OBJECTIVES: This study aimed to compare the effects of Passiflora incarnata, Erythrina mulungu, and midazolam in controlling anxiety in patients undergoing mandibular third molar extraction. METHODS: The volunteers underwent extraction of their third mandibular molars in a randomized, placebo-controlled, triple-blind, and parallel clinical trial. Passiflora incarnata (500 mg), Erythrina mulungu (500 mg), or midazolam (15 mg) was orally administered 60 min before the surgery. The anxiety level of participants was evaluated using questionnaires and measurements of physical parameters, including heart rate (HR), blood pressure (BP), and oxygen saturation (SpO2). RESULTS: A total of 200 volunteers were included in this clinical trial. Considering each procedure independently, no significant differences (p > 0.05) in BP, HR, and SpO2 were observed among the protocols. CONCLUSIONS: Passiflora incarnata showed a similar effect to midazolam but differed from placebo and mulungu, which were unable to control anxiety in this situation. Therefore, the results suggest that Passiflora configures an herbal medicine with an anxiolytic effect, adequate to use in third molar extractions. CLINICAL RELEVANCE: The use of Passiflora incarnata may be an alternative to benzodiazepines for controlling anxiety in patients scheduled for oral surgery under local anesthesia. TRIAL REGISTRATION: ClinicalTrials.gov : ANSI-388.427.

RESUMO

OBJECTIVE: The aim of this study was to compare the local anesthesia efficacy of ropivacaine 0.75% compared to lidocaine 2% with 1:100,000 epinephrine for postoperative analgesia following extraction of impacted mandibular third molars. STUDY DESIGN: In this randomized, double-blind crossover clinical trial, 30 participants underwent surgical removal of bilateral impacted mandibular third molars under local anesthesia using ropivacaine 0.75% or lidocaine 2% with 1:100,000 epinephrine. The pain was recorded on a visual analog scale at 4, 8, 12, 24, and 48 h postoperatively. The use of analgesics and the presence of adverse effects were recorded. RESULTS: The duration of soft tissue anesthesia in the ropivacaine group was significantly longer than that in the lidocaine group. The lidocaine group recorded significantly higher visual analog scale scores at all postoperative time intervals, except in the final 48-h period. Analgesic use was higher in the lidocaine group. Rescue medication was used by 2 patients in each group (6.7%). Significantly more postoperative bleeding was seen in the ropivacaine group. CONCLUSION: Ropivacaine 0.75% injection before the surgical procedure may be associated with preventive analgesia for extraction of impacted mandibular third molars.

BACKGROUND: The aim of this study was to compare the ability of liposomal and non-liposomal lidocaine and prilocaine in hydrogel formulations to promote topical anesthesia in palatal mucosa during upper molar extractions. METHODS: In this randomized, cross over, triple-blinded clinical trial, a liposomal and a non-liposomal formulation of the eutectic mixture of local anesthetics, 2.5% lidocaine and 2.5% prilocaine, were used to promote palatal anesthesia without the local anesthetic infiltration during bilateral upper molars extractions. RESULTS: From the total of 40 patients included in this study, the non-liposomal eutectic lidocaine-prilocaine formulation failed in 40% of cases, unlike the liposomal formulation, which was effective for all patients (Fisher's exact test, p < 0.0001). Furthermore, the liposomal formulation (26.75 ± 7,47 min) induced longer anesthesia duration (t-test, p < 0.0001) than the non-liposomal formulation (16.78 ± 4.75 min). No mucosal ulceration or discomfort was reported for both formulations. CONCLUSION: The liposomal formulation was able to induce adequate anesthesia in palatal mucosa during dental extraction, avoiding the local anesthetic infiltration. For the first time, a topical formulation allowed upper molars surgical removal without injection of any local anesthetic agent into palatal mucosa in adults.

RESUMO

BACKGROUND: Antiproliferative and cytotoxic effects of lidocaine have been reported in tumor cells. However, the use of these drugs is restricted due to their short action with rapid dispersion from the injected site. The complexation of local anesthetics in 2-hydroxypropyl-ß-cyclodextrin (HP-ß-CD) is able to improve pharmacological features. OBJECTIVE: This study evaluated the antitumor effects of lidocaine and the complex HP-ß-CD-lidocaine (HP-ß-CD-lido). METHODS: In vitro;, human adenocarcinoma (HeLa) and keratinocytes (HaCaT) were exposed to lidocaine formulations and cell viability, proliferation and apoptosis induction were measured. In vivo;, Walker 256 carcinoma cells were subcutaneously injected into the plantar region of the rat right hind paw. The animals were treated with a local application of 5% lidocaine or 5% HP-ß-CD-lido. Doxorubicin (3 mg/Kg/day, intraperitoneal) was used as a positive control. Edema sizes were measured daily and the release of cytokines (TNF-α, IL-1α and CXCL-1) and prostaglandin E2 was evaluated. Histological analysis was also performed. RESULTS: HaCaT IG50 values were 846 µM and 2253 µM for lido and HP-ß-CD-lido, respectively. In HeLa cells, the IG50 was 1765 µM for lido and 2044 µM for HP-ß-CD-lido. Lidocaine formulations significantly reduced the paw edema on day 6 after Walker 256 cells inoculation. However, there were no differences in the release of inflammatory mediators in comparison to the control group. CONCLUSION: Lidocaine formulations were able to reduce the edema in vivo;, without affecting the tumor- induced inflammatory response. The antiproliferative effects of lidocaine formulations may have contributed to tumor reduction.

RESUMO

OBJECTIVES: The aim of this study was to evaluate glycemic levels in diabetic patients before, during, and after extractions using 2% lidocaine with 1:100,000 epinephrine (Lido/Epi) and 3% prilocaine with 0.03 IU/mL felypressin (Prilo/Fely). MATERIALS AND METHODS: A double-blind, randomized clinical trial was conducted to evaluate changes in body parameters and glycemic levels in diabetic patients undergoing two anesthetic protocols during dental extractions. During surgery, we evaluated blood pressure (BP), heart rate (HR), saturation (SpO2), and capillary glycemic levels (Gly). These parameters were measured at the following surgical moments: basal, 30 min after medication, incision, tooth removal, suture, and 30 and 60 min after anesthesia. RESULTS: Data analysis showed no differences between the groups considering age, weight, and time spent in surgery. Increased systolic BP and decreased diastolic BP were observed in the lido/epi group. No difference was observed in the prilo/fely group among the surgical moments or between the groups regarding BP. No difference was observed in HR and SpO2 between the groups at any surgical moment. However, differences were found when compared the differences in glycemic and basal levels in both groups with greater decreases in blood glucose values for the lido/epi group. In anxiety level evaluation, there was no difference between the different surgical moments. CONCLUSION: Thus, both lido/epi and prilo/fely (maximum 3.6 mL) can be safely used in controlled diabetic patients CLINICAL RELEVANCE: The use of lidocaine associated with epinephrine did not increase glycemic levels but leads to decrease over time when associated with an anxiety reduction protocol, offering some advantage over prilocaine plus felypressin for diabetic patients.

RESUMO

Objective: To evaluate the radioprotective effect of a homeopathic solution in salivary function and parotid glands morphology of irradiated rats. Materials and Methods: The sample consisted of 150 rats randomly divided into 6 groups. The groups were named based on the substance administered: Control- saline solution; Irradiated Control- saline solution and 15 Gy of X radiation; Alcohol-hydroalcoholic solution dynamized at 15 CH; Irradiated Alcohol- hydroalcoholic solution dynamized at 15 CH and 15 Gy of X radiation; Homeopathy- 0.25 ml (1mL/kg) of the irradiated hydroalcoholic solution and dynamized at 15 CH; Irradiated homeopathy- homeopathic solution and 15 Gy of X radiation. Each group was subdivided into 5 different subgroups, based on the time point of euthanasia: 12 hours, 3, 10, 17, and 24 days. The medication was administered for 7 days before and 7 days after the radiation treatment. On the day of euthanasia, salivation was induced with pilocarpine and collected. The animals were then sacrificed and the parotid glands were removed. Results: Salivary function analysis showed that only group irradiated homeopathy euthanized on day 17 had a statistically significant difference when compared to other irradiated groups, presenting a higher salivation flow rate. The only group that showed a statistically significant difference in the number of acini over time was the irradiated alcohol group, which presented a tendency of reduction. Conclusion: The homeopathic solution presented a late radioprotective effect based on salivary function and morphological analysis of the parotid gland.

Abstract Excessive weight is associated with periodontitis because of inflammatory mediators secreted by the adipose tissue. Periodontal impairments can occur during pregnancy due to association between high hormonal levels and inadequate oral hygiene. Moreover, periodontitis and excessive weight during pregnancy can negatively affect an infant's weight at birth. Objective This observational, cross-sectional study aimed to evaluate the association between pre-pregnancy overweight/obesity, periodontitis during the third trimester of pregnancy, and the infants' birth weight. Methodology The sample set was divided into 2 groups according to the preconception body mass index: obesity/overweight (G1=50) and normal weight (G2=50). Educational level, monthly household income, and systemic impairments during pregnancy were assessed. Pocket probing depth (PPD) and clinical attachment level (CAL) were obtained to analyze periodontitis. The children's birth weight was classified as low (<2.5 kg), insufficient (2.5-2.999 kg), normal (3-3.999 kg), or excessive (≥4 kg). Bivariate analysis (Mann-Whitney U test, t-test, chi-squared test) and logistic regression (stepwise backward likelihood ratio) were performed (p<0.05). Results G1 showed lower socioeconomic levels and higher prevalence of arterial hypertension and gestational diabetes mellitus during pregnancy than G2 (p=0.002). G1 showed higher means of PPD and CAL (p=0.041 and p=0.039, respectively) and therefore a higher prevalence of periodontitis than G2 (p=0.0003). G1 showed lower infants' birth weight than G2 (p=0.0004). Excessive maternal weight and educational levels were independent variables associated with periodontitis during the third trimester of pregnancy (X2[2]=23.21; p<0.0001). Maternal overweight/obesity was also associated with low/insufficient birth weight (X2[1]=7.01; p=0.008). Conclusion The present findings suggest an association between excessive pre-pregnancy weight, maternal periodontitis, and low/insufficient birth weight.

The objective of this study was to verify the sexual dimorphism through linear measurements (length of the body and dentinoid process, length of the dentinoid process, length of the vertebral foramen, width of the vertebral foramen, distance of the transverse processes, distance from the left transverse process to the spinal process , Distance from the right transverse process to the spinous process) of the second cervical vertebra known as the Áxis. For this purpose, 181 Axioms were analyzed, of which 103 were male and 78 were female, in the age range of 22 to 85 years, belonging to the "Tomographic and Osteological Biobank Prof. Eduardo Daruge of FOP / UNICAMP ". These measurements were made by the stainless - hardned ® digital pachymetro 150 mm Mauá - São Paulo, Brazil, after the inter and intra - operator calibration performed through the intraclass correlation test, resulting in a value of 0.98 considered excellent. It was verified that all the measures studied are dimorphic and it was possible to establish a new model of logistic regression, based on the data obtained from the bones. It was concluded that the logistic regression model generated Germano Gender = [- 22.7 + (0.16 × Body length) + (0.31 × Vertebral foramen length) + (0.28 × Distance of transverse processes), with 72.4% of hit.

RESUMO

BACKGROUND: Systemic bone loss may lead to more severe periodontal destruction, decreasing local bone mineral density. AIM: A cross-sectional designed was performed to study associations among alveolar bone pattern, salivary leptin concentrations, and clinical periodontal status in premenopausal obese and eutrophic women. METHODS: Thirty morbid obese (G1) and 30 normal-weight (G2) women were included. Anthropometric and periodontal measurements (probing depth - PD, clinical attachment levels - CAL, presence of calculus, bleeding on probing -BOP, and plaque accumulation) were assessed. OHIP-14 was used for assessment of oral health impact on quality of life. Panoramic radiography was used to obtain the panoramic mandibular index (PMI), mandibular cortical index (MCI), and mental index (MI). Intraoral periapical (PA) radiography was taken to measure the total trabecular bone volume. Leptin was measured in saliva of fasted overnight women. RESULTS: Groups 1 and 2 differed in all anthropometric aspects, but height. Pocket depth, calculus, BOP, and plaque index were worse in G1. No differences between groups were found considering OHIP. Normal-weight subjects showed higher proportion of dense bone trabeculae than obese subjects for pre-molars, but not for molars. Mental and panoramic mandibular indexes did not differ and were in normal level. Leptin concentration was dependent only on BMI. CONCLUSION: Obesity affected the periodontal conditions, the alveolar bone pattern, and the salivary leptin concentration.

RESUMO

OBJECTIVE: This study aimed to evaluate different materials as soft tissue simulators and the influence of soft tissues in cone beam computed tomography. STUDY DESIGN: Images of 5 piglet heads were acquired with intact soft tissues, with the tissues stripped, and with the use of different soft tissue simulators, following the same acquisition protocol. Four different materials were tested, individually or in combination: acrylic, water, utility wax, and expanded polystyrene (EPS). Pixel intensity values of 8 quadrangular regions, that is, upper and lower teeth and alveolar bone, were obtained. The mean values were used for comparison by analysis of variance (ANOVA; α = 5%). RESULTS: No differences were observed for the "No Material," "EPS," "Acrylic," and "EPS and Wax" groups for the lower anterior and posterior teeth, the upper posterior tooth, and the anterior and posterior bone, and for the "No Material," "EPS," and "EPS and Wax," groups for the lower posterior bone. All groups showed statistical differences for the lower anterior bone and the upper anterior tooth. CONCLUSIONS: Expanded 2-cm thick polystyrene, with or without 1-cm thick utility wax, was effective for most regions, followed by acrylic 0.5 cm. Soft tissues were not of great influence in most regions. Water was not an effective material for any of the regions.
