Introducción: Aun son escasos los estudios que recomiendan los pacientes con fibrilación auricular en los programas de rehabilitación cardiaca. Objetivo: Evaluar los efectos del entrenamiento físico en pacientes con fibrilación auricular en régimen de rehabilitación cardiovascular. Metodología: Se realizó un estudio pre-experimental, prospectivo, tipo pretest- postest con 18 pacientes con fibrilación auricular que asistieron durante 12 semanas a las sesiones de rehabilitación cardiaca. Se evaluó el paciente al inicio y finaldel programa con ergometría con gases espirados, análisis de sangre y ecocardiograma transtorácico. Resultados: Predominaron el sexo masculino (72.2 %), el diagnóstico de miocardiopatías (33.3 %), la hipertensión arterial (100 %) y la fibrilación auricular persistente de larga duración (55.6 %). Se encontró una diferencia estadísticamente significativa para cada variable morfofuncional: índice de masa corporal (28.0±5.0 vs. 26.9±4.4, p< 0.0001), consumo de oxígeno pico (13.6±3.0 vs. 15.3±2.9, p<0.0001), consumo de oxígeno en el umbral anaerobio (9.9±2.2 vs. 10.7±2.0; p=0.007), equivalente ventilatorio para dióxido de carbono (27.7±4.7 vs. 26.6±4.7; p<0.0001), equivalente ventilatorio para el oxígeno (24.2±5.0 vs. 23.8±4.9 p=0.001), pulso de oxígeno (10.2±2.5 vs. 12.1±2.1; p< 0,0001), unidades metabólicas (3.8±0.9 vs. 4.3±0.9; p<0.0001) y fracción de eyección ventricular izquierda (55,7±8,7 vs. 58,2±7,5, p=0,003). Las variables metabólicas disminuyeron significativamente (p<0.0001). La clase funcional de Weber mejoró en 5 pacientes (27.8 %). Conclusiones: El entrenamiento físico de pacientes con fibrilación auricular durante 12 semanas de rehabilitación cardiovascular mejoró la capacidad funcional, con incrementos evolutivos de la fracción de eyección ventricular izquierda y disminución de las variables metabólicas, sin generar riesgos ni complicaciones.


Effects of physical training of patients with atrial fibrillation in cardiovascular rehabilitation regimen Introduction: There are still few studies that recommend patients with atrial fibrillation in cardiovascular rehabilitation programs. Objective: To evaluate the effects of physical training in patients with atrial fibrillation undergoing cardiovascular rehabilitation. Method: A pre-experimental, prospective, pretest-posttest study was carried out with 18 patients with atrial fibrillation who attended cardiac rehabilitation sessions for 12 weeks. The patient was evaluated at the beginning and end of the program with stress test with expired gases, blood test and transthoracic echocardiogram. Results: The male sex (72.2%), the diagnosis of cardiomyopathies (33.3%), arterial hypertension (100%) and longterm persistent atrial fibrillation (55.6%) predominated. A statistically significant difference was found for each morphofunctional variable: body mass index (28.0±5.0 vs. 26.9±4.4, p<0.0001), peak oxygen consumption (13.6±3.0 vs. 15.3±2.9, p<0.0001), oxygen at the anaerobic threshold (9.9±2.2 vs. 10.7±2.0; p=0.007), ventilatory equivalent for carbon dioxide (27.7±4.7 vs. 26.6±4.7; p<0.0001), ventilatory equivalent for oxygen (24.2± 5.0 vs. 23.8±4.9 p=0.001), oxygen pulse (10.2±2.5 vs. 12.1±2.1; p<0.0001), metabolic units (3.8±0.9 vs. 4.3±0.9; p<0.0001) and fraction of left ventricular ejection (55.7±8.7 vs. 58.2±7.5, p=0.003). The metabolic variables decreased significantly (p<0.0001). Weber's functional class improved in 5 patients (27.8%). Conclusions: Physical training of patients with atrial fibrillation during 12 weeks of cardiovascular rehabilitation improved functional capacity, with progressive increases in left ventricular ejection fraction and decrease in metabolic variables, without generating risks or complications.

OBJECTIVES: To non-invasively assess tissue lesion secondary to ischaemia applied during knee replacement surgery. Secondary objectives: to assess whether this lesion correlates with the duration of ischaemia and whether instrumental and gender variables influence it. MATERIAL AND METHODS: Prospective cohort study. Pre and postoperative serum lactate levels have been determined as an indicator of glycolytic activity secondary to ischaemia in 88 patients. Serum lactate determination was performed by reactive strips of enzymatic-amperometric detection on capillary blood. RESULTS: Preoperative serum lactate levels (mean and SD): 2.467±1.036 mmol/L. Postoperative serum lactate levels: 3.938±2.018 mmol/L. Ischaemia time 102.98±18.25minutes. Postoperative serum lactate levels were significantly higher than preoperative lactate levels. There are no statistical differences according to the time that the ischaemia was prolonged, gender or type of instrumentation used. CONCLUSIONS: In our study, postoperative serum lactate values were significantly higher than preoperative lactate values, with no correlation to the duration of ischaemia during knee replacement surgery.

BACKGROUND: Brain injury is an important, potentially devastating, complication in cardiac surgery. A significant number of patients suffer perioperative complications involving the central nervous system. Clinical manifestations of brain injury are associated with significantly increased mortality, morbidity and health resource utilization. Serum biomarkers have been studied in cardiac surgery to measure the degree and incidence of brain injury and to improve patient management. METHODS: All relevant studies were identified by computerized searches of PubMed using the following Medical Subject Headings and keywords term: biomarker, cardiac surgery, brain injury and neurological complication. RESULTS: The biomarkers that appear to be better indicators of brain injury severity in cardiac surgery and its consequences are S100B protein, neuron-specific enolase, glial fibrillary acidic protein, tau protein, matrix metalloproteinase-9, ubiquitin C terminal hydroxylase-L1 and neurofilaments. CONCLUSIONS: These biomarkers, independently of clinical and radiological findings, show global cerebral situation at the cellular level and the degree of brain dysfunction. However, up to date, there is no biomarker entirely suitable for the detection of brain injury after cardiac surgery.

OBJECTIVE: To describe, in a 7-year follow-up study, the use of infliximab in patients with refractory posterior uveitis and scleritis. METHODS: A 7-year follow-up case series study was performed. Patients with posterior uveitis and scleritis refractory to conventional therapies (steroids and at least one immunosuppressive agent) were included. Three infliximab intravenous doses of 5 mg/kg were administered at weeks 0, 2, and 6. Further infusions were allowed in patients undergoing a relapse of the uveitis after initial remission. All patients were followed up for at least 8 months. We defined uveitis improvement as an increase in the best-corrected visual acuity or an objective and significant improvement in retinal exudates and/or haemorrhages, cystoid macular oedema (CME), and vitreous haze. Infliximab-related adverse events, final prednisone doses, and the number of immunosuppressive agents used were recorded. A descriptive analysis was performed. RESULTS: A total of 11 patients (17 eyes were affected at baseline) were included, 63% were women, the mean age was 43+/-14 years, and the median follow-up was 80 months (p25-p75: 50-80). After infliximab treatment, six eyes maintained their basal visual acuity, nine eyes showed improvement, and two worsened (in the two patients diagnosed with choroiditis). Vitreous haze, active retinal vasculitis, and CME, but not chorioretinal lesions, improved in all patients. All patients tapered their daily steroid dose and the number of immunosuppressive agents. No infliximab-related adverse events were reported. CONCLUSIONS: Infliximab could be an effective and safe treatment in patients with posterior uveitis and scleritis refractory to conventional therapy.

OBJECTIVE: To investigate whether there is a possible viral transmission using mesenchymal stem cells (MSCs) in autologous or allogeneic transplantation in the context of osteoarthritis (OA) patients. The presence of parvovirus B19 (B19), varicella zoster virus (VZV), and human herpesvirus-6 (HHV-6) was studied in MSCs from bone marrow of patients with OA and healthy controls. METHODS: MSCs were prepared from bone marrow aspirates obtained from 18 patients undergoing joint replacement as a result of OA and from 10 healthy controls. DNA was extracted from primary MSCs' culture established from these cells and quantitative real-time polymerase chain reaction was performed to analyse the prevalence and viral load of B19, VZV and HHV-6. RESULTS: The prevalence of total viral DNA among patients with OA was 16.7% (3/18), with a mean viral load of 29.7 copies/microg of DNA. One out of 18 was positive for B19 (viral load, 61.2 copies/microg of DNA), two for VZV (mean viral load, 14.4 copies/microg of DNA), and none for HHV-6. The prevalence of total viral DNA in the control group was 20% (2/10), with a mean viral load of 13.4 copies/microg of DNA. Both positive results were of B19 parvoviruses. There were no statistically significant differences among patients and controls. CONCLUSIONS: This first approach to the viral prevalence in MSCs of bone marrow in OA patients and healthy controls seems to show a very low risk of viral transmission or reactivation in a possible MSCs' transplantation.

Cyclophosphamide (CY) is an alkylating agent used to treat a variety of autoimmune disorders. Water intoxication is a well-known complication of high-dose intravenous (i.v.) CY, but is rare in patients treated with low dose i.v. CY. We describe two patients with lupus nephritis and water intoxication following low dose i.v. CY. The first patient was treated with oral prednisolone and azathioprine for eight weeks with inadequate response and persistent renal inflammatory activity. Eight hours after the first i.v. CY pulse she had a grand mal seizure. The second patient had WHO class III lupus nephritis, and after a single i.v. CY pulse developed vomiting, diarrhoea and grand mal seizures. They were both fluid-restricted and their serum sodium levels returned to normal. In conclusion, even at low doses i.v. CY may induce hyponatremia related to inappropriate antidiuretic hormone secretion. This potentially life-threatening complication of i.v. CY could be minimized by avoidance of overhydration following pulse i.v. CY.

UNLABELLED: Intravenous antibiotic therapy (IVAT) is usually prescribed for patients hospitalized with community-acquired pneumonia (CAP). Studies have associated prolonged IVAT with longer hospital stays and higher costs. The aim of this study was to determine the factors that influence the expense generated by and mean stay of patients hospitalized for pneumonia, with special attention to the influence of IVAT duration. MATERIAL AND METHODS: One hundred twenty-five CAP patients admitted to the respiratory medicine wards of our hospital were randomly assigned to five different staff physicians. IVAT was prescribed following the norms of the Spanish Society of Respiratory Medicine and Chest Surgery (SEPAR). IVAT was withdrawn when the attending physician considered it appropriate. We collected epidemiological, comorbidity, clinical and analytical data. Complications were recorded and severity of CAP was classified using the model proposed by Fine. Follow-up care was given at an outpatient clinic until symptoms disappeared and chest films resolved. Multivariate analysis determined the factors predicting mean hospital stay and high cost. Costs were calculated based on data issued by the billing department. RESULTS: The mean cost of care was 307,274 pesetas, mean duration of IVAT was 5.8 days and mean hospital stay was 9.4 days. Multivariate analysis showed that cost was related to mean hospital stay and IVAT. Mean hospital stay was associated with IVAT, the presence of respiratory insufficiency and the day of the week when admission took place (with weekend admission leading to longer stays). CONCLUSIONS: The duration of IVAT in CAP influences mean hospital stay and cost, without adding any evident therapeutic benefit (in the group of patients selected). Recommendations for diagnosing and treating CAP may be advisable.

OBJECTIVE: To analyze the mechanisms involved in the characteristic hyperexpression of CD23 on peripheral blood B cells from patients with rheumatoid arthritis (RA). METHODS: Peripheral blood mononuclear cells (PBMC) were obtained from patients with active disease and activated during 18 h with an anti-CD3 monoclonal antibody in the presence or absence of blocking antibodies to CD154 or CD40. PBMC were further purified by rosetting and CD23 expression was assessed on B cells by flow cytometry after double staining (CD19/CD23). Lymphocytes were also isolated from synovial fluid (SF). CD154 expression was analyzed on PB or SF CD4+ T cells after double staining (CD4/CD154) by flow cytometry at basal conditions and after different stimuli [anti-CD3 or phorbol myristic acetate (PMA) plus ionomycin]. Co-culture experiments between SF and PB cells were performed to analyze the involvement of the CD40-CD154 interaction on CD23 expression. CD154 and CD23 expression was also analyzed on synovial membrane by immunohistochemical techniques. RESULTS: A high proportion of activated CD23 B cells was detected in patients with RA. Blocking experiments with both anti-CD40 and anti-CD154 Mab showed a significant reduction in the proportion of PB B cells expressing CD23. Following activation with anti-CD3 Mab or PMA plus ionomycin, CD154 expression was mainly induced on PB CD4+ T cells. In co-culture experiments, SF T cells were more efficient than PB T cells in inducing CD40 dependent CD23 expression on PB B cells. In addition, CD4+ T cells from synovial membrane clearly expressed CD154. CONCLUSION: Our results establish a link between CD154-CD40 pathway and CD23 expression on PB B cells from patients with RA. T cells from the synovial microenvironment were active participants in this CD23 expression, presumably in the context of cell recirculation.

BACKGROUND: Corticosteroids remain the cornerstone of therapy for giant-cell arteritis, but relapse during dose tapering and corticosteroid-related adverse events often complicate management of this condition. Although several approaches, including combined therapy with cytotoxic agents, have been suggested to overcome these problems, no study has clearly shown benefits of alternate treatments. OBJECTIVE: To analyze the safety and efficacy of combined therapy with corticosteroids and methotrexate in giant-cell arteritis. DESIGN: Randomized, double-blind, placebo-controlled trial. SETTING: University-based clinic. PATIENTS: 42 patients with new-onset giant-cell arteritis according to biopsy. INTERVENTION: High initial doses of corticosteroid were given; the dose was then tapered quickly until therapy was completely withdrawn. Methotrexate or placebo was given weekly from the start of corticosteroid therapy for 24 months. MEASUREMENTS: Number of relapses, cumulative dose of corticosteroid, and number of adverse events were assessed on completion of follow-up. RESULTS: Compared with combined prednisone and placebo therapy, treatment with prednisone and methotrexate reduced the proportion of patients who experienced at least one relapse (45% vs. 84.2%; P = 0.02) and the proportion of patients who experienced multiple relapses (P = 0.004). The mean cumulative dose of prednisone was 4187 +/- 1529 mg in the methotrexate group and 5489.5 +/- 1396 mg in the placebo group (mean difference, 1302 mg [95% CI, 350 to 2253 mg]; P = 0.009). Overall, the rate and severity of adverse events were similar between groups. Treatment was discontinued in 3 patients in the methotrexate group who experienced definite drug-related adverse events. In sensitivity analysis that included patients lost to follow-up, differences between groups in number of relapses and cumulative dose of prednisone were significant. CONCLUSIONS: Treatment with methotrexate plus corticosteroid is a safe alternative to corticosteroid therapy alone in patients with giant-cell arteritis and is more effective in controlling disease.

Immunosenescence is characterized by an increase in autoantibody production. Because both T and B cell stimulation are key events for producing antibodies, we investigated early T and B cell activation by means of CD23 and CD40L (two very early activation antigens). PBMC from elderly humans (EH) were studied following culture with either medium, anti-CD3mAb, rIL-4, or PMA + ionomycin. CD23 expression on elderly B cells after anti-CD3 challenge of PBMC, a reflect of T-dependent B cell activation, was clearly defective. Conversely, CD23 expression on EH B cells following activation with soluble factors as rIL-4 was preserved. CD40L expression was also impaired in EH T cells following anti-CD3 challenge. However, activation by means of PMA and/or ionomycin was preserved both in T cells (CD40L expression) and in B cells (CD23 expression). These results indicate that a defective T-dependent B cell activation related to defective T cell activation located between surface membrane and PKC/ionomycin function is an intrinsic characteristic of immunosenescence. We have not found intrinsic B-cell defects, and we conclude that the characteristically impaired early B cell activation in EH is mostly due to T cell defects.

This is a retrospective study of 19 cases of subtalar dislocation treated in our Department during the last 15 years. The average age of the patients was 31, and the majority were male (63%). Medial dislocations predominated (84%) and 37% were open. Associated osteocartilaginous lesions were very frequent and affected 13 patients (68%). Surgery was required for all the open injuries and for 2 of the closed dislocations. Closed reduction was impossible in one, and another required surgery to treat an associated vascular injury. Seventeen of the 19 patients were reviewed after an average of 7.9 years. The results were good in 6, fair in another 6, and poor in the remaining 5. The results of all the open lesions was only fair or poor. Four patients needed a triple arthrodesis. Factors resulting in a poor result were open lesions, associated bone lesions, and prolonged immobilization.

OBJECTIVE: To assess the possible involvement of the bowel in the spectrum of anterior uveitis (AU). METHODS: Ileocolonoscopy and histology studies were performed on 27 patients with AU. RESULTS: Patients with AU had a higher incidence of chronic intestinal inflammation (CII) than controls. CII was present in AU regardless of HLA-B27 status, sacroiliitis or NSAID intake, and was more prevalent in uveitis with high recurrence. CONCLUSION: Bowl inflammation ia a component of the spectrum of AU. This finding supports a HIL-B27 independent common pathogenic link between AU and spondyloarthropathy.
