BACKGROUND: Several safety measures like movement restrictions, closure of educational institutions, and social distancing measures continue over the world including Bangladesh during the COVID-19 pandemic. This study aimed to examine the patterns of eating behaviors, physical activity, and lifestyle modifications among adolescents during the COVID-19 pandemic residing in Bangladesh. METHODS: A cross-sectional study was performed among 490 adolescents in Bangladesh from December 2020 to May 2021. The survey was carried out through a semi-structured web-based questionnaire that asked questions about socio-demographics (i.e., age, sex, marital status, education, residence), perceived health status and quality of life, anthropometrics (i.e., height, weight), dietary habits (i.e., frequency of eating, daily intake of certain foods, number of meals eaten daily), and physical activity (i.e., modified version of the International Physical Activity Questionnaire Short Form [IPAQ-SF]), as well as, pre- and during COVID-19 information on stress, and sleep. RESULTS: During the pandemic, 43.7% participants reported weight gain; and 23.5% reported an increased number of meals per day during COVID-19. Additionally, the participants' eating habits diverged from the local balanced diet principles and were more akin to 'unhealthy' eating patterns. Though, during the COVID-19 pandemic, physical exercise slightly increased (>3 times/ week: 8.2% vs. 13.5%; p<0.001) compared to pre-COVID-19 period, the screen time for entertainment increased drastically (>5 hours/ week: 12.2% vs. 27.3%; p<0.001). Compared to the pre-pandemic, a sizeable proportion of individuals experienced more physical tiredness, emotional exhaustion, irritation, and stress (p<0.001) during the pandemic. During the pandemic, 47.5% of participants experienced different sleep difficulties. CONCLUSIONS: Although lockdowns and social distancing are important safety measures to protect people from COVID-19, findings reveal that they might cause a variety of lifestyle changes, physical inactivity, and psychological issues in Bangladeshi adolescents.

Despite sporadic and irregular studies on heavy metal(loid)s health risks in water, fish, and soil in the coastal areas of the Bay of Bengal, no chemometric approaches have been applied to assess the human health risks comprehensively. This review aims to employ chemometric analysis to evaluate the long-term spatiotemporal health risks of metal(loid)s e.g., Fe, Mn, Zn, Cd, As, Cr, Pb, Cu, and Ni in coastal water, fish, and soils from 2003 to 2023. Across coastal parts, studies on metal(loid)s were distributed with 40% in the southeast, 28% in the south-central, and 32% in the southwest regions. The southeastern area exhibited the highest contamination levels, primarily due to elevated Zn content (156.8 to 147.2 mg/L for Mn in water, 15.3 to 13.2 mg/kg for Cu in fish, and 50.6 to 46.4 mg/kg for Ni in soil), except for a few sites in the south-central region. Health risks associated with the ingestion of Fe, As, and Cd (water), Ni, Cr, and Pb (fish), and Cd, Cr, and Pb (soil) were identified, with non-carcinogenic risks existing exclusively through this route. Moreover, As, Cr, and Ni pose cancer risks for adults and children via ingestion in the southeastern region. Overall non-carcinogenic risks emphasized a significantly higher risk for children compared to adults, with six, two-, and six-times higher health risks through ingestion of water, fish, and soils along the southeastern coast. The study offers innovative sustainable management strategies and remediation policies aimed at reducing metal(loid)s contamination in various environmental media along coastal Bangladesh.

INTRODUCTION AND IMPORTANCE: Gastrointestinal stromal tumors (GISTs) are rare mesenchymal tumors with varied clinical presentations. This case report highlights the significance of recognizing atypical GIST presentations, such as hematemesis, especially in resource-limited settings. CASE PRESENTATION: A 52-year-old male from an economically disadvantaged background presented with hematemesis and severe anemia. Initial investigations suggested iron deficiency anemia, but further evaluation revealed a gastric mass, raising suspicion of GIST. Despite limited resources, a distal radical gastrectomy confirmed the GIST diagnosis, and the patient underwent surgical resection followed by imatinib therapy. CLINICAL DISCUSSION: This case underscores the diagnostic challenges posed by GISTs and the importance of imaging studies, given their often nonspecific symptoms. Limited resources and economic constraints in low-income settings can hinder comprehensive diagnosis and treatment. Access to specialized oncological services is crucial for accurate management. CONCLUSION: Early recognition and management of GIST, even in atypical presentations like hematemesis, can significantly impact patient outcomes. This case report highlights the need for improved healthcare infrastructure in low-resource settings and calls for initiatives to ensure equitable access to diagnostic tests and appropriate treatment for rare diseases like GIST.

Crinum asiaticum L. (Amaryllidaceae) is a perennial bulbous herb, locally utilized for possessing multifaceted pharmacological properties including anticancer, immune-stimulating, analgesic, antiviral, antimalarial, antibacterial and antifungal, in addition to its popularity as an aesthetic plant. Separation of MeOH extract of C. asiaticum leaves yielded three known compounds as cycloneolitsol (1), hippeastrine (2) and ß-sitosterol (3). Among these, compounds 1 and 2 were subjected to the cytotoxic assay and found that they induced mild effect against HCT116, Huh7 and DU145 cell lines with the IC50 values from 73.76 to 132.53 µM. When tested for TRAIL-resistance abrogating activity, 1 (100 µM) along with TRAIL (100 ng/mL) showed moderate activity in AGS cells producing 25 % more inhibition than the agent alone. Whereas 2 (20 and 30 µM) in combination with TRAIL (100 ng/mL) exhibited strong activity in abrogating TRAIL-resistance and caused 34 % and 36 % more inhibition in AGS cells, respectively. The in-silico studies of compound 2 revealed high docking hits with the TRAIL-associated anti-apoptotic proteins which give a justification for the regulatory interactions to induce such abrogating activity. It is still recommended to conduct further investigations to understand their exact molecular mechanism.

Groundwater is an essential resource in the Sundarban regions of India and Bangladesh, but its quality is deteriorating due to anthropogenic impacts. However, the integrated factors affecting groundwater chemistry, source distribution, and health risk are poorly understood along the Indo-Bangla coastal border. The goal of this study is to assess groundwater chemistry, associated driving factors, source contributions, and potential non-carcinogenic health risks (PN-CHR) using unsupervised machine learning models such as a self-organizing map (SOM), positive matrix factorization (PMF), ion ratios, and Monte Carlo simulation. For the Sundarban part of Bangladesh, the SOM clustering approach yielded six clusters, while it yielded five for the Indian Sundarbans. The SOM results showed high correlations among Ca2+, Mg2+, and K+, indicating a common origin. In the Bangladesh Sundarbans, mixed water predominated in all clusters except for cluster 3, whereas in the Indian Sundarbans, Cl--Na+ and mixed water dominated in clusters 1 and 2, and both water types dominated the remaining clusters. Coupling of SOM, PMF, and ionic ratios identified rock weathering as a driving factor for groundwater chemistry. Clusters 1 and 3 were found to be influenced by mineral dissolution and geogenic inputs (overall contribution of 47.7%), while agricultural and industrial effluents dominated clusters 4 and 5 (contribution of 52.7%) in the Bangladesh Sundarbans. Industrial effluents and agricultural activities were associated with clusters 3, 4, and 5 (contributions of 29.5% and 25.4%, respectively) and geogenic sources (contributions of 23 and 22.1% in clusters 1 and 2) in Indian Sundarbans. The probabilistic health risk assessment showed that NO3- poses a higher PN-CHR risk to human health than F- and As, and that potential risk to children is more evident in the Bangladesh Sundarban area than in the Indian Sundarbans. Local authorities must take urgent action to control NO3- emissions in the Indo-Bangla Sundarbans region.

Toxic metals and freshwater fish's metalloid contamination are significant environmental concerns for overall public health. However, the bioaccumulation and sources of metal(loids) in freshwater fishes from Bangladesh still remain unknown. Thus, the As, Pb, Cd, and Cr concentrations in various freshwater fish species from the Rupsha River basin were measured, including Tenualosa ilisha, Gudusia chapra, Otolithoides pama, Setipinna phasa, Mystus vittatus, Glossogobius giuris, and Pseudeutropius atherinoides. An atomic absorption spectrophotometer was used to determine metal concentrations. The mean concentrations of metal(loids) in the fish muscle (mg/kg) were found to be As (1.53) > Pb (1.25) > Cr (0.51) > Cd (0.39) in summer and As (1.72) > Pb (1.51) > Cr (0.65) > Cd (0.49) in winter. The analyzed fish species had considerably different metal(loid) concentrations with seasonal variation, and the distribution of the metals (loids) was consistent with the normal distribution. The demersal species, M. vittatus, displayed the highest bio-accumulative value over the summer. However, in both seasons, none of the species were bio-accumulative. According to multivariate statistical findings, the research area's potential sources of metal(loid) were anthropogenic activities linked to geogenic processes. Estimated daily intake, target hazard quotient (THQ), and carcinogenic risk (CR) were used to assess the influence of the risk on human health. The consumers' THQs values were < 1, indicating that there were no non-carcinogenic concerns for local consumers. Both categories of customers had CRs that fell below the permissible range of 1E - 6 to 1E - 4, meaning they were not at any increased risk of developing cancer. The children's group was more vulnerable to both carcinogenic and non-carcinogenic hazards. Therefore, the entry of metal(loids) must be regulated, and appropriate laws must be used by policymakers.

Due to its harmful effects on ecosystems and human health, microplastic (MP) pollution has become a significant environmental problem on a global scale. Although MPs' pollution path and toxic effects on marine habitats have been examined worldwide, the studies are limited to the rare biodiversity estuary region of Hatiya Island from the northern Bay of Bengal. This study aimed to investigate the MP pollution path and its influencing factors in estuarine sediments and water in rare biodiversity Hatiya Island in the northern Bay of Bengal. Sixty water and sediment samples were collected from 10 sampling sites on the Island and analyzed for MPs. The abundance of MPs in sediment ranged from 67 to 143 pieces/kg, while the abundance in water ranged from 24.34 to 59 pieces/m3. The average concentrations of MPs in sediment and water were 110.90 ± 20.62 pieces/kg and 38.77 ± 10.09 pieces/m3, respectively. Most identified MPs from sediment samples were transparent (51%), while about 54.1% of the identified MPs from water samples were colored. The fragment was the most common form of MP in both compartments, with a value of 64.6% in sediment samples and 60.6% in water samples. In sediment and water samples, almost 74% and 80% of MP were <0.5 mm, respectively. Polypropylene (PP) was the most abundant polymer type, accounting for 51% of all identified polymers. The contamination factor, pollution load index, polymer risk score, and pollution risk score values indicated that the study area was moderately polluted with MPs. The spatial distribution patterns and hotspots of MPs echoed profound human pathways. Based on the results, sustainable management strategies and intervention measures were proposed to reduce the pollution level in the ecologically diverse area. This study provides important insights into evaluating estuary ecosystem susceptibility and mitigation policies against persistent MP issues.

Background and Aims: To reduce death rates for critical patients hospitalized in intensive care units (ICUs), coronavirus (COVID-19) lacks proven and efficient treatment methods. This cross-sectional study aims to evaluate how physicians treat severe and suspected COVID-19 patients in the ICU department in the absence of an established approach, as well as assess the rational use of the medication in the ICU department. Methods: Between June 16, 2021, and December 10, 2022, a total of 428 prescriptions were randomly gathered, including both suspected (yellow zone) and confirmed (red zone) COVID-19 patients. For data management, Microsoft Excel 2021 was utilized, while STATA 17 provided statistical analysis. To find associations between patients' admission status and demographic details, exploratory and bivariate analyses were conducted. Results: Of the 428 patients admitted to the ICU, 228 (53.27%) were in the yellow zone and 200 (46.73%) were in the verified COVID-19 red zone. The majority of patients were male (54.44%), and the age range from 41 to 60 was the most common (41.82%). No significant deviation was detected to the yellow and red groups' prescription patterns. A total of 4001 medicines (mean 9.35/patient) were prescribed. Antiulcerants, antibiotics, respiratory, analgesics, anticoagulants, vitamins and minerals, steroids, cardiovascular, antidiabetic drugs, antivirals, antihistamines, muscle relaxants, and antifungal treatments were widely prescribed drugs. Enoxaparin (67.06%) appeared as the most prescribed medicine, followed by montelukast (60.51%), paracetamol (58.41%), and dexamethasone (51.64%). Conclusion: The prescription patterns for the yellow and red groups were comparable and mostly included symptomatic treatment. Respiratory drugs constituted the most frequent therapeutic class. Polypharmacy should be taken under considerations. In ICU settings, the outcomes emphasize the need of correct diagnosis, cautious antibiotic usage, suitable therapy, and attentive monitoring.

BACKGROUND: Mastalgia, or breast pain, is a prevalent and distressing condition experienced by women, impacting their daily lives and causing complications. It is common among women of reproductive age, with associations found with premenstrual syndrome, fibrocystic breast disease, psychological distress, and, in rare cases, breast cancer. While Western societies have high frequencies of Mastalgia, it is less diagnosed in Asian cultures including Bangladesh. This study aimed to investigate the prevalence and factors associated with Mastalgia among early reproductive-aged women in Bangladesh. METHODS: A cross-sectional survey was conducted, and data were collected from different regions of the country. A convenience sampling method was used to take 1,214 participants for the study. A pre-tested semi-structured questionnaire was used to collect data. Bivariate and multivariate analyses were conducted to ascertain factors that were significantly associated with Mastalgia. The data were analyzed using the SPSS software 26 version. RESULTS: All the participants were female (mean age: 22.87 ± 2.64 years; age ranges: 18-35 years). The prevalence of Mastalgia was found to be 35.5%. The study was conducted using a self-reported semi-structured questionnaire. Participants with a higher education level and higher income were more likely to experience Mastalgia. A family history of breast cancer and severe abdominal pain during menstruation were also identified as associated factors for Mastalgia (p < 0.05). CONCLUSIONS: This study enhances our understanding of Mastalgia in Bangladesh, offering insights for healthcare and policy. Further research is required to uncover its root causes and develop effective interventions. The study highlights the prevalence of mastalgia and its related factors, emphasizing the necessity for increased awareness and support for affected women.

Water contamination undermines human survival and economic growth. Water resource protection and management require knowledge of water hydrochemistry and drinking water quality characteristics, mechanisms, and factors. Self-organizing maps (SOM) have been developed using quantization and topographic error approaches to cluster hydrochemistry datasets. The Piper diagram, saturation index (SI), and cation exchange method were used to determine the driving mechanism of hydrochemistry in both surface and groundwater, while the Gibbs diagram was used for surface water. In addition, redundancy analysis (RDA) and a generalized linear model (GLM) were used to determine the key drinking water quality parameters in the study area. Additionally, the study aimed to utilize Explainable Artificial Intelligence (XAI) techniques to gain insights into the relative importance and impact of different parameters on the entropy water quality index (EWQI). The SOM results showed that thirty neurons generated the hydrochemical properties of water and were organized into four clusters. The Piper diagram showed that the primary hydrochemical facies were HCO3--Ca2+ (cluster 4), Cl---Na+ (all clusters), and mixed (clusters 1 and 4). Results from SI and cation exchange show that demineralization and ion exchange are the driving mechanisms of water hydrochemistry. About 45 % of the studied samples are classified as "medium quality"," that could be suitable as drinking water with further refinement. Cl- may pose increased non-carcinogenic risk to adults, with children at double risk. Cluster 4 water is low-risk, supporting EWQI findings. The RDA and GLM observations agree in that Ca2+, Mg2+, Na+, Cl- and HCO3- all have a positive and significant effect on EWQI, with the exception of K+. TDS, EC, Na+, and Ca2+ have been identified as influencing factors based on bagging-based XAI analysis at global and local levels. The analysis also addressed the importance of SO4, HCO3, Cl, Mg2+, K+, and pH at specific locations.

Novel, non-thermal and economically benign pre-treatment process was developed for enhancing valorisation potential of brown sludge generated by dairy industry wastewater treatment plant (WWTP). Vortex-based hydrodynamic cavitation (HC) device was used to quantify influence of pretreatment by measuring biomethane potential (BMP) of untreated and treated brown sludge. Pre-treatment parameters, primarily, pressure drop and number of passes through the cavitation device were varied to quantify influence on BMP. BMP tests were performed at 39 °C containing 5% of total solids in each reactors using an automatic BMP measurement system containing 15 reactors with each volume of 500 mL fitted with overhead stirrer. HC treatment increased the soluble chemical oxygen demand (sCOD) by more than 25% which increased the BMP. HC treatment was able to push the BMP of treated sludge to more than 80% of the theoretical BMP. Volatile solids (VS) removal was more than 65%. Highest methane yield was 376 mL/g-VS of sludge. The methodology and results presented here show significant potential to valorise brown dairy sludge via vortex based hydrodynamic cavitation.

Toxic metal(loid)s (TMLs) in agricultural soils cause detrimental effects on ecosystem and human health. Therefore, source-specific health risk apportionment is very crucial for the prevention and control of TMLs in agricultural soils. In this study, 149 surface soil samples were taken from a coal mining region in northwest Bangladesh and analyzed for 12 TMLs (Pb, Cd, Ni, Cr, Mn, Fe, Co, Zn, Cu, As, Se, and Hg). Positive matrix factorization (PMF) and absolute principal component score-multiple linear regression (APCS-MLR) receptor models were employed to quantify the pollution sources of soil TMLs. Both models identified five possible sources of pollution: agrochemical practice, industrial emissions, coal-power-plant, geogenic source, and atmospheric deposition, while the contribution rates of each source were calculated as 28.2%, 17.2%, 19.3%, 19% and 16.3% in APCS-MLR, 22.2%, 13.4%, 24.3%, 15.1% and 25.1% in PMF, respectively. Agrochemical practice was the major source of non-carcinogenic risk (NCR) (adults: 32.37%, children: 31.54%), while atmospheric deposition was the highest source of carcinogenic risk (CR) (adults: 48.83%, children: 50.11%). NCR and CR values for adults were slightly higher than for children. However, the trends in NCR and CR between children and adults were similar. As a result, among the sources of pollution, agrochemical practices and atmospheric deposition have been identified as the primary sources of soil TMLs, so prevention and control strategies should be applied primarily for these pollution sources in order to protect human health.

The coastal areas of Bangladesh have poor accessibility to fresh drinking water and the groundwater is not suitable for drinking, cooking, and other domestic uses due to high levels of salinity and potentially toxic elements. The current study focuses on understanding of the distribution of some physicochemical parameters (temperature, pH, EC, TDS, and salinity) and chemical elements (Fe, Mn, Zn, Ca, Mg, Na, K, Cu, Co, Pb, As, Cr, Cd, and Ni) with health perspective in drinking water from the southwestern coastal area of Bangladesh. The physicochemical properties of the water samples were examined with a multiparameter meter, while the elemental concentrations were analyzed using atomic absorption spectrometer. Water quality index (WQI) and irrigation indices were utilized to determine the drinking water quality and irrigation feasibility, respectively, whereas hazard quotients (HQs) and hazard index (HI) were used to assess the probable pathways and the associated potential risks to human health. The concentrations of some toxic elements in measured samples were relatively higher compared to drinking water guidelines, indicating that ground and surface water are not apt for drinking and/or domestic uses. The multivariate statistical approaches linked the source of the pollutants in the studied water body mostly to the geogenic origin including saline water intrusion. WQI values ranged from 18 to 430, reflecting excellent to unsuitable categories of water quality. The assessment of human health risks due to exposure to contaminated water demonstrated both carcinogenic and non-carcinogenic health risks in the exposed residents of the study area. Therefore, appropriate long-term coastal area management strategies should be adopted in the study region for environmental sustainability. The findings of this research will be supportive in understanding the actual situation of fresh drinking water in the area for policymakers, planners, and environmentalists to take effective necessary measures to ensure safe drinking water in the study area.

The Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) is an instrument to screen substance-use-related health risks. However, little is known whether the ASSIST could be further shortened while remaining psychometrically sound across different countries, languages, gender identities, and sexual-orientation-based groups. The study aimed to validate a shortened 11-item ASSIST (ASSIST-11). Using the International Sex Survey data, 82,243 participants (M age = 32.39 years) across 42 countries and 26 languages completed questions from the ASSIST-11 regarding gender identity, sexual orientation, and other information. Confirmatory factor analysis (CFA) and multigroup CFA (MGCFA) evaluated the ASSIST-11's structure and tested measurement invariance across groups. Cronbach's α and McDonald's ω were used to examine the internal consistency. Cohen's d and independent t-tests were used to examine known-group validity. The ASSIST-11 was unidimensional across countries, languages, age groups, gender identities (i.e., men, women, and gender-diverse individuals), and sexual orientations (i.e., heterosexual and sexual minority individuals). Cronbach's α was 0.63 and McDonald's ω was 0.68 for the ASSIST-11. Known-group validity was supported by Cohen's d (range between 0.23 and 0.40) with significant differences (p-values<0.001). The ASSIST-11 is a modified instrument with a unidimensional factor structure across different languages, age groups, countries, gender identities, and sexual orientations. The low internal consistency of the ASSIST-11 might be acceptable as it assesses a broad concept (i.e., use of several different substances). Healthcare providers and researchers may use the ASSIST-11 to quickly assess substance-use information from general populations and evaluate the need to follow up with more detailed questions about substance use.

Total 288 Ross-308-day-old male broiler chicks were randomly distributed into six dietary treatment groups in a two-way ANOVA with 2 × 3 factorial arrangements (two factors, i.e., dietary protein and energy having two types of protein, e.g., plant, animal and three different sources of energy, e.g., soybean oil, rice bran oil and sunflower oil) to justify if animal protein-soybean oil based broiler diet optimizes net profit at the expense of desirable ω-6 fatty acids in the breast muscle of the broiler chicken. Average daily feed intake (ADFI), final live weight (FLW), average daily gain (ADG), feed efficiency (FE), carcass characteristics, cardio-pulmonary morphometry, fatty acid profile of the breast muscle and cost-benefit analysis were measured. Results indicated that animal protein significantly increased 4.27% FLW, 6.13% ADFI, 4.31% ADG and 2.93% wing weight. Accordingly, soybean oil increased 4.76% FLW, 3.80% ADG and 1.36% dressing percentage at the expense of 12.07% proventriculus weight compared with sunflower oil. The generalized linear model identified no interaction effects of the sources of protein and energy on overall performance of the birds. Replacement of vegetable protein by animal protein decreased 14.01% ∑ω-3, 12.16% ∑ω-6 and 12.21% sum of polyunsaturated fatty acids (∑PUFA) and concomitantly increased 10.82% sum of saturated fatty acids (∑SFAs) in the breast muscle (Pectoralis major). Accordingly, replacement of sunflower oil by soybean oil decreased 29.17% ∑ω-3, 6.71% ∑ω-6, 11.62% sum of monounsaturated fatty acids (∑MUFAs) and 7.33% ∑PUFAs and concurrently increased 18.36% ∑SFAs in the breast muscle of the broiler birds. It was concluded that animal protein-soybean oil-based broiler diet optimized net profit at the expense of desirable ω-3 and ω-6 fatty acids in the breast muscle of the broiler chicken.

Microplastic (MP) pollution has become an escalating problem in Bangladesh due to its rapid urbanization, economic growth, and excessive use of plastics; however, data on MP pollution from fresh water resources in this country are limited. This study investigated microplastics pollution in riverbed sediments in the peripheral rivers of Dhaka, the capital of Bangladesh. Twenty-eight sediment samples were collected from the selected stations of the Buriganga, Turag, and Balu Rivers. Density separation and wet-peroxidation methods were employed to extract MP particles. Attenuated total reflectance-Fourier transform infrared spectroscopy was used to identify the polymers. The results indicated a medium-level abundance of MPs in riverbed sediment in comparison with the findings of other studies in freshwater sediments worldwide. Film shape, white and transparent color, and large-size (1-5 mm) MPs were dominant in the riverbed sediment. The most abundant polymers were polyethylene (PE), polypropylene (PP), and polyethylene terephthalate (PET). Pollution load index (PLI) values greater than 1 were observed, indicating that all sampling sites were polluted with MPs. An assessment of ecological risks, using the abundance, polymer types, and toxicity of MPs in the sediment samples, suggested a medium to very high ecological risk of MP pollution of the rivers. The increased abundance of MPs and the presence of highly hazardous polymers, namely; polyurethane, acrylonitrile butadiene styrene, polyvinyl chloride, epoxy resin, and polyphenylene sulfide, were associated with higher ecological risks. Scanning electron microscopy (SEM) analysis indicated that the MPs were subjected to weathering actions, reducing the size of MPs, which caused additional potential ecological hazards in these river ecosystems. This investigation provides baseline information on MP pollution in riverine freshwater ecosystems for further in-depth studies of risk assessment and developing strategies for controlling MP pollution in Bangladesh.

Monitoring of heavy metal concentrations in soil and their accumulation in vegetables grown in the newly shifted tannery area of Savar, Bangladesh, is crucial for human health. Heavy metals (i.e., Cr, Pb, Cu, Zn, Ni, and Cd) concentrations in soil and vegetable samples were determined by atomic absorption spectrophotometer (AAS). In soil, 3220 mg/kg Cr was observed, which was 32-fold greater than the WHO/FAO recommended limit. Ecological risk indices such as the contamination factor, enrichment factor, pollution load index, and geoaccumulation index showed metal levels as moderately to very highly contaminated. The non-carcinogenic risk (NCR) was found to be higher, and the carcinogenic risk (CR) exceeded the acceptable value 1 × 10-6 and posed greater risks to children than adults, especially for Cr in soil. The main exposure pathway for soil metals was 97.8-99.9% due to oral ingestion. The concentration of heavy metals especially Cr, Pb, Zn, and Cd, in vegetables was alarming as they crossed the safety limit. The calculated mean hazard index (8.71) for vegetable samples showed elevated levels of potential NCR, while CR for Cr and Cd, exceeded the acceptable limit of 1 × 10-6, indicating the probability of cancer risk to humans through the consumption of vegetables. This study revealed a to-and-fro analysis of the present scenario of the tannery area, giving importance to human health and the environment.

BACKGROUND: Food adulteration is an increasingly recognized global public health problem. In low- and middle-income countries like Bangladesh, adulteration is difficult to detect and respond to. We explored customers' perceptions on food adulteration, perception of risk and connections between information, participant characteristics and patterns of adulterated food concerns that impact risk perception in urban Bangladesh. METHODS: A formative study was conducted in Dhaka, between June and August 2015 at a supermarket and a wet market. We explored community awareness and response to chemical contaminants (adulterants) among participants from a range of socio-economic backgrounds. The team conducted 38 in-depth interviews with 12 customers and 4 staff from a supermarket, and 12 customers and 10 vendors from a wet market. Participants were selected purposively. Audio recorded data were coded based on thematic content and analyzed manually. RESULTS: We asked participants how common foods were likely adulterated, and most gave figures of 70% or more. They reported that foods were adulterated with chemicals or artificial colors, especially fish, milk, and vegetables. The supermarket more commonly sold packaged foods with nutritional and expiry information on the label; and offered convenience in terms of building size, layout, and cleanliness. All customers from the wet market thought that foods were cheaper and fresher than from supermarkets. Supermarket customers expressed greater concern about adulterated foods than wet market customers. Most participants from both markets reported that food adulteration is invisible, adulterated foods cannot be avoided, and have long-term negative health impacts including cancer, diabetes, paralysis, heart attack, and others. Nearly half of customers from both markets were concerned about the poor nutritional value of adulterated food. Participants from both settings expressed the need for access to credible information about adulteration to help choose safe foods. The majority expressed the need for government action against those who are responsible for adulteration. CONCLUSIONS: Food adulteration was considered a major health threat. The government could act on food adulteration prevention if provided credible population-based data on disease burden, a model food sampling and testing protocol, a model for inspections, organizational strengthening and training, example social and behavioral change communications with estimated costs.

BACKGROUND: Uniformity and compliance with clinical practice guidelines (CPGs) for use of palivizumab in preventing severe respiratory syncytial viral infection in Australian high-risk infants remain unclear. METHODS: An online survey was conducted across the Australian and New Zealand Neonatal Network (ANZNN) to determine clinical practices around palivizumab. A literature search was also performed to identify and compare national and international guidelines. RESULTS: A total of 65 of 422 ANZNN members completed the survey. Respondents included 61 senior medical staff of consultants/staff specialists (78%) and four nursing staff (6%). According to the survey, infants most likely to be recommended palivizumab included preterm infants born <29 weeks gestational age (GA) (30%), children with chronic lung diseases (CLDs) born <32 weeks GA (40%), and with hemodynamically significant heart disease (35%). Many of the respondents (53%) stated that CPGs for palivizumab were developed locally. Literature search identified 20 guidelines (10 international and 10 domestic); 16 (80%) recommended palivizumab use in preterm infants, 16 (80%) recommended use in infants with CLD, 17 (85%) in congenital heart disease and 6 (30%) in bronchopulmonary dysplasia (BPD). Eight (40%) guidelines provided specific recommendations for immunocompromised infants. Canada, Western Australia, and American Academy of Paediatrics provided recommendations for Indigenous children. Frequency and dosage of palivizumab was universal across all CPGs. None of the international guidelines obtained were from low- or middle-income countries. CONCLUSIONS: Standardization of CPGs may improve clinical decision making around use of palivizumab in high-risk infants.

Approximately 80% of patients with epilepsy reside in poor resource settings. Despite the continued advancements and development of new treatment approaches, epilepsy remains a major health problem in developing countries. Consistent findings of epidemiologic studies reflect that both prevalence and treatment gap are higher in the developing world. The objective of this short review was to evaluate current treatment options and low-intensity, pulsed-focused ultrasound (FUS) as a potential new treatment option for epilepsy. Although some of the patients could be candidates for surgery, many factors, including poor health-care infrastructure, socioeconomic status, risks and complications associated with the surgery, and patients' preferences and attitudes toward the surgical procedure, limit the adherence to get surgical therapies. Low-intensity FUS, a novel and noninvasive therapeutic approach, has the potential to be approved by regulatory bodies and added to the list of standard treatment options for epilepsy. Improved understanding of epilepsy's prevalence and incidence in developing worlds, identification of potential new therapeutic options, and their evaluation through continuous studies and clinical trials are needed to reduce the burden of epilepsy and the treatment gap.
