RESUMO

Recently, computed tomography with photon-counting detector (PCD-CT) has been developed to enable high-resolution imaging at a lower radiation dose. PCD-CT employs a photon-counting detector that can measure the number of incident X-ray photons and their energy. The newly released PCD-CT (NAEOTOM Alpha, Siemens Healthineers, Forchheim, Germany) has been in clinical use at our institution since December 2022. The PCD-CT offers several advantages over current state-of-the-art energy-integrating detector CT (EID-CT). The PCD-CT does not require septa to create a detector channel, while EID-CT does. Therefore, downsizing the anode to achieve higher resolution does not affect the dose efficiency of the PCD-CT. CT is an indispensable modality for evaluating ear ossicles. The ear ossicles and joints are clearly depicted by PCD-CT. In particular, the anterior and posterior legs of the stapes, which are sometimes unclear on conventional CT scans, can be clearly visualized. We present cases of congenital anomalies of the ossicular chain, ossicular chain dislocation, tympanosclerosis, and cholesteatoma in which PCD-CT was useful. This short article reports the usefulness of PCD-CT in the 3D visualization of the ear ossicles.

BACKGROUND: While many studies have evaluated the change in lung volume before and after lung resection and correlated this with pulmonary function test results, there is very little evidence on the changes in ventilation perfusion ratio (V/Q) before versus after lung resection. In the present pilot study, we evaluated if V/Q mapping can be constructed using dual energy CT images. METHODS: Thirty-one lung cancer patients planned for pulmonary resection were included in this study. To evaluate ventilation, Xenon-enhanced CT was performed. This was immediately followed by perfusion CT. The two images were registered manually as well as using dedicated softwares, and division between ventilation pixels and perfusion pixels were done to produce the V/Q map. Also, in order to characterize the distribution of the V/Q, the following numerical indices were calculated; mean, median, mode, standard deviation (SD), coefficient of variation (CV), skewness, kurtosis, and fractal dimension (FD). Pulmonary function tests and blood gas parameters were measured using standard institutional procedures. RESULTS: In the whole group, VC, %VC, and FEV1 decreased significantly after resection. FEV1.0% was increased significantly after resection. No significant changes were seen in PaO2, PaCO2, and DLCO/VA before and after resection. The mean, median, mode, SD, skewness, kurtosis and FD of the V/Q did not change significantly before and after resection. A marginal but significant decrease in CV was seen before versus after resection. CONCLUSIONS: Overall, it was considered that the V/Q maps could be adequately generated in this study. With further accumulation of data, V/Q map generated by dual energy CT may become one of the potentially useful tools for functional lung imaging. PURPOSE: This study aimed to evaluate the utility of virtual monoenergetic images for detecting washout of small (≤2 cm) hepatocellular carcinoma (HCC) in the equilibrium phase. METHODS: We performed 120-kVp-equivalent linear-blended (M120) and monoenergetic reconstructions from 40 to 90 keV by standard (40, 50, 60, 70, 80, 90) and novel noise-reduced (nMERA: 40+, 50+, 60+, 70+, 80+, 90+) monoenergetic reconstruction algorithms. Image quality and tumor visibility of delayed washout of HCCs in the equilibrium phase were compared between standard monoenergetic reconstruction algorithm and nMERA by objective and subjective analyses. RESULTS: Contrast-to-noise ratio of the tumor at 40+ was the highest, whereas the score of tumor visibility peaked at 50+. The score of overall image quality at 40+ was significantly lower than those on all other image series, and the image quality among other image series were not significantly different. CONCLUSIONS: Virtual monoenergetic image reconstructed with nMERA 50+ was most appropriate to detect washout of small HCCs.

PURPOSE: The purpose of this study was to investigate the utility incremental diagnostic value of combined assessment with coronary CT angiography (CCTA) and myocardial CT perfusion imaging (CTP) using dual-energy technology with an Adenosine Triphosphate (ATP) load technique. MATERIALS AND METHODS: Twenty-one patients underwent ATP-provocation dual-energy CT and CAG. We compared the diagnostic accuracy with CAG, for ischemic region due coronary stenosis by CCTA alone and CCTA combined with CTP (Combined CCTA/CTP). RESULTS: All of 21 patients CTP images could be evaluated, however 8 CCTA images could not be evaluated by calcification and motion artifact, so assessability was 61.9% (13/21) for CCTA alone, and 100% for Combined CCTA/CTP. With CAG results as a comparison, the sensitivity, specificity, positive predictive value, and negative predictive value were, respectively, 83.3% (20/24), 74.4% (29/39), 66.7% (20/30), and 87.8% (29/33) for CCTA alone, and 66.7% (16/24), 92.3% (36/39), 84.2% (16/19), and 81.8% (36/44) for combined CCTA/CTP. The diagnostic accuracy of the two methods were 77.8% (49/63) and 82.5% (52/63). CONCLUSION: Dual-energy CT may be a useful modality for perfusion assessment and correlated well with the severity of stenosis on CAG. This technique may even be of use in cases of severe calcification in the coronary artery wall.

OBJECTIVE: The purpose of this study was to compare estimates of calcified plaque volume in the carotid arteries based on contrast-enhanced dual-energy CT angiograms with volume estimates based on native CT scans. CONCLUSION: Detection of hard plaque in the carotid arteries with dual-energy CT angiography was successful in all patients. Estimates of calcified plaque volume based on dual-energy CT angiograms correlated well with those based on native single-energy CT scans but at a lower attenuation threshold (130 HU as opposed to 180 HU) were underestimates compared with those on the native CT scans.
