RESUMO

PURPOSE: To evaluate the effect of automated tube voltage selection (ATVS) on radiation dose at computed tomography (CT) worldwide encompassing all body regions and types of CT examinations. MATERIALS AND METHODS: No patient information was accessed; therefore, institutional review board approval was not sought. Data from 86 centers across the world were analyzed. All CT interactions were automatically collected and transmitted to the CT vendor during two 6-week periods immediately before and 2 weeks after implementation of ATVS. A total of 164 323 unique CT studies were analyzed. Studies were categorized by body region and type of examination. Tube voltage and volume CT dose index (CTDIvol) were compared between examinations performed with ATVS and those performed before ATVS implementation. Descriptive statistical methods and multilevel linear regression models were used for analysis. RESULTS: Across all types of CT examinations and body regions, CTDIvol was 14.7% lower in examinations performed with ATVS (n = 30 313) than in those performed before ATVS implementation (n = 79 275). Relative reductions in mean CTDIvol were most notable for temporal bone CT (-56.1%), peripheral runoff CT angiography (-48.6%), CT of the paranasal sinus (-39.6%), cerebral or carotid CT angiography (-36.4%), coronary CT angiography (-25.1%), and head CT (-23.9%). An increase in mean CTDIvol was observed for renal stone protocols (26.2%) and thoracic or lumbar spine examinations (6.6%). In the multilevel model with fixed effects ATVS and examination type, and the interaction of these variables and the random effect country, a significant influence on CTDIvol for all fixed efects was revealed (ATVS, P = .0031; examination type, P < .0001; interaction term, P < .0001). CONCLUSION: ATVS significantly reduces radiation dose across most, but not all, body regions and types of CT examinations.

RATIONALE AND OBJECTIVES: To investigate the diagnostic performance of a comprehensive computed tomography (CT) protocol for both cardiac and aortoiliac evaluation of patients considered for transcatheter aortic valve replacement (TAVR) using a single, low-volume contrast medium (CM) injection. MATERIALS AND METHODS: Forty-four TAVR candidates were retrospectively analyzed. All underwent retrospectively electrocardiogram-gated cardiac CT followed by high-pitch CT angiography of the aortoiliac vasculature using one of two single injection protocols of 320 mgI/mL iodine CM: group A (n = 22), iodine delivery rate-based (1.28 gI/s), 60-mL CM volume, 4.0 mL/s flow rate; group B (n = 22), clinical routine protocol, 100-mL CM volume, 4.0 mL/s flow rate. Mean arterial attenuation, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) were calculated. Subjective image quality was assessed. RESULTS: Aortic root and iliofemoral dimensions could be analyzed in all cases. Patient characteristics showed no significant differences. Mean attenuation at the levels of the aortic root (285.8 ± 83.0 HU vs 327.5 ± 70.8 HU, P = .080) and the iliofemoral access route (256.8 ± 88.5 HU vs 307.5 ± 93.2 HU, P = .071), as well as SNR and CNR were nonsignificantly lower in group A compared to group B. Subjective image quality was equivalent. CONCLUSIONS: In multimorbid TAVR patients, the performance of a combined CT protocol using a single low-volume CM bolus is feasible with maintained image quality compared to a standard protocol.

OBJECTIVES: To perform a systematic review of the growing body of literature evaluating the prognostic value of epicardial fat volume (EFV) quantified by cross-sectional imaging. METHODS: Two independent reviewers performed systematic searches on both PubMed and Scopus using search terms developed with a medical librarian. Peer-reviewed articles were selected based on the inclusion of outcome data, utilization of epicardial fat volume and sufficient reporting for analysis. RESULTS: A total of 411 studies were evaluated with nine studies meeting the inclusion criteria. In all, the studies evaluated 10,252 patients. All nine studies were based on CT measurements. Seven studies evaluated the prognostic value of EFV unadjusted for calcium score, and six of these studies found a significant association between EFV and clinical outcomes. Seven studies evaluated the incremental value of EFV beyond calcium scoring, and six of these studies found a significant association. CONCLUSIONS: The majority of studies suggest that EFV quantification is significantly associated with clinical outcomes and provides incremental prognostic value over coronary artery calcium scoring. Future research should use a binary cutoff of 125 mL for evaluation of EFV to provide consistency with other research. OBJECTIVES: To evaluate the effect of automated tube voltage selection on radiation dose and image quality at cardiovascular CT angiography (CTA). METHODS: We retrospectively analysed paired studies in 72 patients (41 male, 60.5 ± 16.5 years), who had undergone CTA acquisitions of the heart or aorta both before and after the implementation of an automated x-ray tube voltage selection algorithm (ATVS). All other parameters were kept identical between the two acquisitions. Subjective image quality (IQ) was rated and objective IQ was measured by image noise, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and figure of merit (FOM). Image quality parameters and effective dose were compared between acquisitions. RESULTS: Overall subjective image quality improved with the percentage of cases scored as adequate or higher increasing from 79 % to 92 % after implementation of ATVS (P = 0.03). SNR (14.1 ± 5.9, 15.7 ± 6.1, P = 0.009), CNR (11.6 ± 5.3, 13.2 ± 5.6, P = 0.011), and FOM (19.9 ± 23.3, 43.8 ± 51.1, P < 0.001) were significantly higher after implementation of ATVS. Mean image noise (24.1 ± 8.4 HU, 22.7 ± 7.1 HU, P = 0.048) and mean effective dose (10.6 ± 5.9 mSv, 8.8 ± 5.0 mSv, P = 0.003) were significantly lower after implementation of ATVS. CONCLUSIONS: Automated tube voltage selection can operator-independently optimize cardiovascular CTA image acquisition parameters with improved image quality at reduced dose. KEY POINTS: • Automatic tube voltage selection optimizes tube voltage for each individual patient. • In this population, overall radiation dose decreased while image quality improved. • This tool may become valuable for improving dose/quality ratio.

OBJECTIVE: The purpose of this article is to prospectively determine the value of stress dual-energy CT (DECT) myocardial perfusion imaging to coronary CT angiography (CTA) for the assessment of coronary artery disease (CAD) in a high-risk population. SUBJECTS AND METHODS: We prospectively enrolled 29 consecutive patients who were referred for cardiac SPECT examinations for known or suspected CAD to also undergo pharmacologic stress cardiac DECT. In 25 patients, cardiac catheterization was available as the reference standard for morphologically significant stenosis. The performance of coronary CTA alone, DECT myocardial perfusion alone, and the combination of both was assessed by calculating sensitivity, specificity, and AUC values. RESULTS: For morphologically significant stenosis, coronary CTA alone and myocardial DECT assessment alone had 95% sensitivity and 50% specificity. The combined approach yielded 100% sensitivity and 33% specificity if either was positive and 90% sensitivity and 67% specificity if both were positive. The AUC value was highest (0.78) if both were positive. For hemodynamically significant lesions, coronary CTA alone had 91% sensitivity and 38% specificity, and DECT alone had 95% sensitivity and 75% specificity. The combined approach yielded 100% sensitivity and 38% specificity if either was positive and 86% sensitivity and 75% specificity if both were positive. AUC values were highest for DECT alone (0.85) and the "both positive" evaluation (0.80). CONCLUSION: The combined analysis of coronary CTA and DECT myocardial perfusion reduces the number of false-positives in a high-risk population for CAD and outperforms the purely anatomic test of coronary CTA alone for the detection of morphologically and hemodynamically significant CAD.

The exact pathophysiology of contrast-induced nephropathy (CIN) is not fully clarified, yet the osmotic characteristics of contrast media (CM) have been a significant focus in many investigations of CIN. Osmotic effects of CM specific to the kidney include transient decreases in blood flow, filtration fraction, and glomerular filtration rate. Potentially significant secondary effects include an osmotically induced diuresis with a concomitant dehydrating effect. Clinical experiences that have compared the occurrence of CIN between the various classes of CM based on osmolality have suggested a much less than anticipated advantage, if any, with a lower osmolality. Recent animal experiments actually suggest that induction of a mild osmotic diuresis in association with iso-osmolar agents tends to offset potentially deleterious renal effects of high viscosity-mediated intratubular CM stagnation.

OBJECTIVES: This study evaluated the performance of a novel automated software tool for epicardial fat volume (EFV) quantification compared to a standard manual technique at coronary CT angiography (cCTA). METHODS: cCTA data sets of 70 patients (58.6 ± 12.9 years, 33 men) were retrospectively analysed using two different post-processing software applications. Observer 1 performed a manual single-plane pericardial border definition and EFVM segmentation (manual approach). Two observers used a software program with fully automated 3D pericardial border definition and EFVA calculation (automated approach). EFV and time required for measuring EFV (including software processing time and manual optimization time) for each method were recorded. Intraobserver and interobserver reliability was assessed on the prototype software measurements. T test, Spearman's rho, and Bland-Altman plots were used for statistical analysis. RESULTS: The final EFVA (with manual border optimization) was strongly correlated with the manual axial segmentation measurement (60.9 ± 33.2 mL vs. 65.8 ± 37.0 mL, rho = 0.970, P < 0.001). A mean of 3.9 ± 1.9 manual border edits were performed to optimize the automated process. The software prototype required significantly less time to perform the measurements (135.6 ± 24.6 s vs. 314.3 ± 76.3 s, P < 0.001) and showed high reliability (ICC > 0.9). CONCLUSIONS: Automated EFVA quantification is an accurate and time-saving method for quantification of EFV compared to established manual axial segmentation methods. KEY POINTS: • Manual epicardial fat volume quantification correlates with risk factors but is time-consuming. • The novel software prototype automates measurement of epicardial fat volume with good accuracy. • This novel approach is less time-consuming and could be incorporated into clinical workflow.

RESUMO

Williams syndrome is a relatively common (1 in 10,000 live births) genetic disorder caused by a deletion involving chromosome 7 that results in a variety of clinically significant abnormalities, including developmental delay, behavioral changes, hypercalcemia, and a distinct "elfin" facial appearance. Congenital cardiovascular disease that presents in childhood is responsible for most of the morbidity and mortality associated with this disorder. The purpose of this pictorial essay is to review imaging findings of some of the more common cardiovascular manifestations of Williams syndrome and to highlight some of the unique anatomic variations that can be seen in these patients.

OBJECTIVES: This study sought to determine whether a 50%-reduced radiation dose protocol using iterative reconstruction (IR) preserves image quality and diagnostic accuracy at coronary computed tomography angiography (CTA) as compared with a routine dose protocol using traditional filtered back projection (FBP). BACKGROUND: IR techniques show promise to decrease radiation requirements at coronary CTA. No study has performed a direct head-to-head, intraindividual comparison of IR algorithms with FBP vis-à-vis diagnostic accuracy and radiation dose at coronary CTA. METHODS: Sixty consecutive subjects (45 men, 53.3 ± 9.4 years of age) prospectively underwent coronary catheter angiography (CCA) and 2 coronary CTA scans. One coronary CTA acquisition used routine radiation dose settings and was reconstructed with FBP. For another scan, the tube current-time product was reduced by 50%, and data were reconstructed with IR. Studies were blindly and randomly interpreted. Image quality, radiation dose, and diagnostic accuracy were compared using CCA as the reference standard. RESULTS: Sensitivity and specificity for diagnosing ≥50% coronary artery stenosis on a per-segment level were 88.5% and 92.1% with FBP and 84.2% and 93.4% with IR, respectively. On a per-patient level, sensitivity and specificity were 100% and 93.1% with FBP and 96.8% and 89.7% with IR, respectively (all p > 0.05). With FBP versus IR, the area under the receiver-operating characteristic curve was 0.903 (95% confidence interval [CI]: 0.881 to 0.922) and 0.888 (95% CI: 0.864 to 0.909) on a per-segment level, and 0.966 (95% CI: 0.883 to 0.996) and 0.932 (95% CI: 0.836 to 0.981) on a per-patient level, respectively (p = 0.290 and 0.330). Compared with FBP, the iterative series showed no significant (p > 0.05) differences in image quality analyses. Median dose-length product was 52% lower for the IR protocol compared with the FBP protocol (109.00 [interquartile range: 82.00 to 172.50] mGy·cm vs. 52.00 [interquartile range: 39.00 to 84.00] mGy·cm, p < 0.001). CONCLUSIONS: Compared with a routine radiation dose FBP protocol, 50% reduced dose acquisition using IR preserves image quality and diagnostic accuracy at coronary CTA.

Iterative techniques are a valuable computed tomography image reconstruction alternative to filtered back projection. In repetitive cycles, iterative algorithms reduce image noise virtually independently of spatial resolution. In light of substantially decreased image noise, tube voltage or current reductions are enabled, resulting in significant radiation dose savings while preserving image quality. Moreover, iterative reconstruction techniques have the advantage of minimizing calcium blooming and metal artifacts. Iterative reconstruction may therefore lead to more exact coronary artery evaluation at constant x-ray tube settings and appears beneficial in clinically challenging scenarios such as overly obese patients, calcified coronary arteries and presence of iatrogenic hardware. For cardiac computed tomography, iterative reconstruction represents a promising and readily available tool.

PURPOSE: To determine whether coronary artery anomalies can be detected on noncontrast computed tomography (CT) coronary artery calcium scoring (CCS) studies. MATERIALS AND METHODS: A total of 126 patients (mean age 62 years; 35 women) underwent noncontrast CCS and contrast enhanced coronary CT angiography (cCTA). Thirty-three patients were diagnosed with a coronary anomaly on cCTA, whereas coronary anomalies were excluded in 93. Two observers (reader 1 [R1] and reader 2 [R2]), blinded to patient information independently evaluated each CCS study for: 1) visibility of coronary artery origins, 2) detection of coronary anomalies, and 3) benign or malignant (ie, interarterial) course. Using cCTA as the reference standard, sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of CCS studies for detecting coronary anomalies were calculated. RESULTS: Of the 33 coronary anomalies, 16 were benign and 17 malignant. Based on noncontrast CCS studies, R1 and R2 correctly identified the left main origin in 123/126 (97.6%) and 121/126 (96%) patients; the left anterior descending origin in 125/126 (99.2%) and 122/126 (96.8%); the circumflex origin in 120/126 (95.2%) and 105/126 (83.3%); and the right coronary artery origin in 117/126 (92.9%) and 103/126 (81.7%), respectively. R1 and R2 identified 34 and 27 coronary anomalies and classified 19 and 15 as malignant, respectively. Interobserver reproducibility for detection of coronary anomalies was good (k = 0.76). Interobserver agreement for detection of malignant variants was even stronger (k = 0.80). On average, coronary artery anomalies were diagnosed with 85.2% sensitivity, 96.4% specificity, 90.5% PPV, and 94.1% NPV on noncontrast CCS studies. CONCLUSION: Benign and malignant coronary artery anomalies can be detected with relatively high accuracy on noncontrast-enhanced CCS studies. CCS studies should be reviewed for signs of coronary artery anomalies in order to identify malignant variants with possible impact on patient management.

OBJECTIVES: Performance evaluation of a fully automated system for calculating computed tomography (CT) coronary artery calcium scores from contrast medium-enhanced coronary CT angiography (cCTA) studies. METHODS: One hundred and twenty-seven patients (58 ± 11 years, 71 men) who had undergone cCTA as well as an unenhanced CT calcium scoring study where included. Calcium scores were computed from cCTA by an automated image processing algorithm and compared with calcium scores obtained by standard manual assessment of unenhanced CT calcium scoring studies. Results were compared vis-a-vis (1) absolute calcium score values, (2) age-, gender- and race-dependent percentiles, and (3) commonly used calcium score risk classification categories. RESULTS: One hundred and nineteen out of 127 (93.7%) studies were successfully processed. Mean Agatston calcium score values obtained by traditional non-contrast CT calcium scoring studies and derived from contrast medium-enhanced cCTA did not significantly differ (235.6 ± 430.5 vs 262.0 ± 499.5; P > 0.05). Calcium score risk categories and Multi-Ethnic Study of Atherosclerosis (MESA) percentiles showed very high correlation (Spearman rank correlation coefficient = 0.97, P < 0.0001/0.95, P < 0.0001) between the two approaches. CONCLUSIONS: Calcium score values automatically computed from cCTA are highly correlated with standard unenhanced CT calcium scoring studies. These results suggest a radiation dose- and time-saving potential when deriving calcium scores from cCTA studies without a preceding unenhanced CT calcium scoring study.
