Abstract The resources and platforms available on the internet for collecting and sharing information and performing genomic sequence analysis have made it possible to follow closely the evolution the evolution of SARS-CoV-2. However, the current monkeypox outbreak in the world brings us back to the need to use these resources to appraise the extent of this outbreak. The objective of this work was an analysis of the information presented so far in the genomic database GISAID EpiPox™, using various tools available on the web. The results indicate that the monkeypox outbreak is referred as MPXV clade II B.1 lineage and sub-lineages, isolated from male patients mainly from the European and American continents. In the current scenario, the access to genomic sequences, epidemiological information, and tools available to the scientific community is of great importance for global public health in order to follow the evolution of pathogens.


Resumen Los recursos y plataformas disponibles en Internet para recopilar, compartir información y realizar análisis de secuencias genómicas han permitido seguir de cerca la evolución del SARS-CoV-2. El actual brote global de viruela del mono en el mundo, requiere de nuevo utilizar estos recursos para conocer el alcance de este brote. El objetivo de este trabajo fue un análisis de la información presentada hasta el momento en la base de datos genómica EpiPox™ de GISAID, utilizando diversas herramientas disponibles en la web. Los resultados indican que el brote de la viruela del mono o símica está referido al linaje y sub-linajes B.1 del clado II de MPXV, aislado principalmente de pacientes hombres de Europa y América. En el escenario actual, el acceso a las secuencias genómicas, la información epidemiológica, y las herramientas disponibles para la comunidad científica son de gran importancia para la salud pública mundial con el fin de seguir la evolución de los patógenos.

Abstract By the end of 2021, the Omicron variant of SARS-CoV-2, the coronavirus responsible for COVID-19, emerges, causing immediate concern, due to the explosive increase in cases in South Africa and a large number of mutations. This study describes the characteristic mutations of the Omicron variant in the Spike protein, and the behavior of the successive epidemic waves associated to the sub-lineages throughout the world. The mutations in the Spike protein described are related to the virus ability to evade the protection elicited by current vaccines, as well as with possible reduced susceptibility to host proteases for priming of the fusion process, and how this might be related to changes in tropism, a replication enhanced in nasal epithelial cells, and reduced in pulmonary tissue; traits probably associated with the apparent reduced severity of Omicron compared to other variants.


Resumen A finales de 2021 surge la variante Omicron del SARS-CoV-2, el coronavirus responsable de la COVID-19, causando preocupación inmediata, debido al aumento explosivo de casos en Suráfrica, y a su gran cantidad de mutaciones. Este estudio describe las mutaciones características de la variante Ómicron en la proteína de la Espiga (S) y el comportamiento de las sucesivas olas epidémicas asociadas a la circulación de sus sub-linajes en todo el mundo. Las mutaciones en la proteína S descritas están relacionadas con su capacidad para evadir la protección provocada por las vacunas actuales, así como su posible susceptibilidad reducida a las proteasas del hospedero para la preparación del proceso de fusión. Se infiere cómo esto podría estar relacionado con su cambio en el tropismo, con una replicación mayor en las células epiteliales nasales y menor en el tejido pulmonar, rasgos probablemente asociados a su aparente menor gravedad en comparación con otras variantes.

Abstract By the end of 2021, the Omicron variant of concern (VOC) emerges in South Africa. This variant caused immediate concern, due to the explosive increase in cases associated with it and the large number of mutations it exhibits. In this study, the restriction sites that allow detecting the mutations K417N and N440K in the Spike gene are described. This analysis allows us to propose a rapid method for the identification of cases infected with the Omicron variant. We show that the proposed methodology can contribute to provide more information on the prevalence and rapid detection of cases of this new VOC.


Resumen Para finales de 2021 surge la variante de preocupación (VOC por sus siglas en inglés) Ómicron en Sudáfrica. Esta variante causó de forma inmediata preocupación, debido al aumento explosivo de casos asociados a ella y al gran número de mutaciones que exhibe. En este estudio, se describen los sitios de restricción que permiten detectar dos de estas mutaciones en el gen de la espiga, las mutaciones K417N y N440K. Este análisis permite proponer un método rápido para la identificación de casos infectados con la variante Ómicron. Mostramos que la metodología propuesta puede contribuir a proporcionar más información sobre la prevalencia y a detectar rápidamente los casos de esta nueva VOC.

BACKGROUND: The role of rotavirus as main etiologic agent of diarrhea has been well documented worldwide, including in Venezuela. However, information about the prevalence of gastrointestinal viruses such as calicivirus, adenovirus and astrovirus is limited and the contribution of other agents as Aichi virus and klassevirus is largely unknown. To explore the etiological spectrum of diarrhea associated with agents other than rotaviruses, 227 stool samples from children under 5 years old with acute gastroenteritis, collected in Valencia (Venezuela) from 2001 to 2005, and previously tested as rotavirus-negative, were analyzed for caliciviruses, adenoviruses, astroviruses, Aichi viruses, klasseviruses, picobirnaviruses and enteroviruses by specific RT-PCRs. RESULTS: At least one viral agent was detected in 134 (59%) of the samples analyzed, mainly from children under 24 months of age and most of them belonging to the lowest socioeconomic status. Overall, enterovirus was identified as the most common viral agent (37.9%), followed by calicivirus (23.3%), adenovirus (11.5%), astrovirus (3.5%), klassevirus (1.3%) and Aichi virus (0.4%), while no picobirnavirus was detected. Klasseviruses were found during 2004 and 2005 and Aichi viruses only in 2005, indicating their circulation in Venezuela; meanwhile, the rest of the viruses were detected during the whole study period. Coinfections with two or more viruses were found in 39 (29.1%) of the infected children, most under 24 months of age. Adenovirus was involved as the coinfecting agent in at least 46.9% of the cases, but no differences concerning socio-demographic variables were observed between the coinfected and the single infected children. CONCLUSIONS: The results show that various enteric viruses, including enteroviruses, caliciviruses and adenoviruses, accounted for a significant proportion of infantile diarrhea cases in Venezuela before rotavirus vaccine implementation. In addition, emerging viruses as Aichi virus and klassevirus were found, indicating the need to continue monitoring their spreading into the communities. Efforts are needed to develop more accurate methods to identify the major causes of diarrhea and to provide tools for more effective preventive measures.

El virus chikungunya (CHIKV) es un Alfavirus causante de la fiebre chikungunya (CHIKF). En Venezuela, una región desprovista de inmunidad contra CHIKV y con presencia de Aedes aegypti y Aedes albopictus, el primer caso importado fue reportado por las autoridades sanitarias en junio de 2014. Por la relevancia del hecho, se analizaron 94 muestras de pacientes febriles que acudieron a los centros de salud públicos y privados del estado Aragua entre enero y diciembre de 2014, mediante la detección de los fragmentos de los genes nsP4 (Alfavirus) y E1 (CHIKV) utilizando técnicas moleculares, como Transcripción Reversa acoplada a Reacción en Cadena de la Polimerasa (RT-PCR) y/o secuenciación nucleotídica. Los resultados indicaron positividad en 19,2 % de las muestras analizadas. Se vieron afectados pacientes con edades entre 6 y 66 años, con predominio del sexo femenino (12/18). Clínicamente, todos los pacientes positivos a CHIKV manifestaron signos y síntomas asociados a CHIKF, tales como fiebre (18/18), artralgia (18/18) y erupción (16/18), entre otros. A pesar de que la positividad puede considerarse baja con relación a lo reportado en otras comunidades, este estudio representa el primer reporte local de detección molecular de CHIKV en Venezuela (estado Aragua) durante el año 2014.


Chikungunya virus is an Alphavirus that causes chikungunya Fever (CHIKF). In Venezuela, a region devoid of immunity against CHIKV and presence of Aedes aegypti and Aedes albopictus. The first imported case was reported by health authorities in June 2014. The relevance of the fact, 94 samples of febrile patients who came to the centers of public and private health Aragua state between january and december for detection of the nsP4 (Alphavirus) and E1 (CHIKV) fragments were analyzed by molecular techniques (Reverse Transcriptase Polymerase Chain Reaction and/or nucleotide sequencing). The results showed 19.2 % of positivity by CHIKV. Clinically all CHIKV positive patients showed signs and symptoms related with CHIKF, such as fever (18/18), arthralgia (18/18) and rash (16/18), among others. Were affected patients between the ages of 6 and 66 years with a predominance of the female sex (12/18). Although the positivity may be considered low compared to those reported in other communities, this represents the first local report of molecular detection of CHIKV in Venezuela (Aragua state) during 2014.

Several studies have shown that adaptation of various viruses to grow in certain cell lines of vertebrates, leads to the selection of virus variants that bind heparan sulfate (HS) with high affinity. In this study we investigated the susceptibility of strains of dengue virus (DENV) to oversulfated heparin an analogue of HS after passages in BHK-21 cells. Field isolates of the four serotypes of DENV with a limited number of passes in mosquito cells C6/36HT were serially passaged eight times in BHK-21 cells. The adaptation of the DENV to the cell culture selected viral variants with an increased replicative capacity in BHK-21 cells and an increased susceptibility to heparin compared with the original not adapted strains, with a more significant inhibition of the infectivity in DENV-2 and DENV-4.The E protein of the adapted strains showed changes in the amino acid sequence, particularly at the position K204R to DENV-1, N67K to DENV-2, K308R and V452A for DENV-3 and E327G to DENV-4. These substitutions implicated a gain of basic residues that increased the net positive charge of the protein. These results suggest that adaptation of DENV strains to BHK-21 cells implies changes in the envelope protein, changes associated to the protein reactivity with heparin, the inhibitory effectiveness of this compound varying depending on the viral strain. In addition, these results suggest that the HS can play an important role in the infectivity of the DENV strains adapted to vertebrate cell culture, but not in the infectivity of non-adapted DENV isolates.

Estudios previos han demostrado que la adaptación de diversos virus a crecer en líneas celulares de vertebrados, conduce a la selección de variantes virales que unen al heparán sulfato (HS) con alta afinidad. En el presente trabajo se determinó la susceptibilidad de cepas del virus dengue (DENV) a la heparina hipersulfatada un análogo al HS, después de pases seriados en células BHK-21. A aislados de campo de los cuatro serotipos de DENV, se les realizaron ocho pases seriados en células BHK-21. La adaptación de los DENV al cultivo celular seleccionó variantes virales con una aumentada capacidad replicativa en células BHK-21 y una incrementada susceptibilidad a la heparina, en relación a las respectivas cepas no adaptadas, obteniéndose una inhibición de la infectividad más significativa en DENV-2 y DENV-4. Las cepas de DENV adaptadas presentaron cambios en la secuencia de aminoácidos de la proteína de envoltura (E), en particular una substitución K204R para DENV-1, N67K para DENV-2, K308R y V452A para DENV-3 y E327G para DENV-4. Estas sustituciones implicaron ganancia de residuos básicos que incrementaron la carga neta positiva de la proteína. Los resultados sugieren, que la adaptación de cepas de DENV a células BHK-21 selecciona variantes virales sensibles a la heparina y que la efectividad de este compuesto varía dependiendo de la cepa viral. Además sugieren que el HS puede jugar un papel importante en la infectividad de las cepas de DENV adaptadas al cultivo celular, a diferencia de los aislados de DENV no adaptados.

The epidemiology and clinical symptoms in infants and young children with acute sporadic viral gastroenteritis due to viral etiologies other than rotaviruses have not been studied thoroughly in developing countries. Fecal specimens from 480 children <5 years of age who were admitted to a large children's hospital in the city of Valencia, Venezuela, with acute diarrhea during January to December 2003 were collected and screened by ELISA and RT-PCR for rotavirus, adenovirus, norovirus, sapovirus, and astrovirus. Viral isolates were partially characterized by phylogenetic analysis. Norovirus viral load was determined by qRT-PCR. Viruses were identified in 205 (43%) of the 480 stool samples collected. Rotavirus was the virus detected most frequently (21%), followed by norovirus (13%), adenovirus (5%), sapovirus (3%), and astrovirus (2%). Viral infection rates were highest in the 6- to 11-month-old group (49%) and lowest in children >24 months old. Norovirus GII was more prevalent (90%) than GI (10%). Enteric adenovirus (serotypes 40/41) was present in 43% of the adenovirus-positive samples. Rotavirus infection caused more severe clinical symptoms than the other viruses detected, with more vomiting (84%) and dehydration (11%) that led to hospital admission of 20% of the children with acute gastroenteritis. Rotavirus and norovirus showed marked and opposite seasonal patterns. No association was observed between disease severity and viral load in children infected with norovirus. These results not only confirm the impact of rotavirus infection in Venezuela but also indicate that other enteric viruses, especially noroviruses, contribute significantly to sporadic acute gastroenteritis and to the burden of disease.

BACKGROUND: Dengue virus (DENV) is a member of the genus Flavivirus of the family Flaviviridae. DENV are comprised of four distinct serotypes (DENV-1 through DENV-4) and each serotype can be divided in different genotypes. Currently, there is a dramatic emergence of DENV-3 genotype III in Latin America. Nevertheless, we still have an incomplete understanding of the evolutionary forces underlying the evolution of this genotype in this region of the world. In order to gain insight into the degree of genetic variability, rates and patterns of evolution of this genotype in Venezuela and the South American region, phylogenetic analysis, based on a large number (n = 119) of envelope gene sequences from DENV-3 genotype III strains isolated in Venezuela from 2001 to 2008, were performed. RESULTS: Phylogenetic analysis revealed an in situ evolution of DENV-3 genotype III following its introduction in the Latin American region, where three different genetic clusters (A to C) can be observed among the DENV-3 genotype III strains circulating in this region. Bayesian coalescent inference analyses revealed an evolutionary rate of 8.48 x 10⁻4 substitutions/site/year (s/s/y) for strains of cluster A, composed entirely of strains isolated in Venezuela. Amino acid substitution at position 329 of domain III of the E protein (A→V) was found in almost all E proteins from Cluster A strains. CONCLUSIONS: A significant evolutionary change between DENV-3 genotype III strains that circulated in the initial years of the introduction in the continent and strains isolated in the Latin American region in recent years was observed. The presence of DENV-3 genotype III strains belonging to different clusters was observed in Venezuela, revealing several introduction events into this country. The evolutionary rate found for Cluster A strains circulating in Venezuela is similar to the others previously established for this genotype in other regions of the world. This suggests a lack of correlation among DENV genotype III substitution rate and ecological pattern of virus spread.

En un estudio realizado en 1984 en la Maternidad "Concepción Palacios", Pérez-Schael y col. detectaron excreción asintomática de rotavirus en el 56% de los recién nacidos evaluados. El objetivo de este trabajo fue estudiar la infección por rotavirus en neonatos de la Maternidad y comparar datos con aquellos obtenidos anteriormente. Además, se investigó la presencia de calicivirus en dicha población. Entre agosto y diciembre de 2004, se recolectaron 307 muestras de heces provenientes de 215 neonatos sanos. Para la detección de los agentes virales se utilizaron ensayos tipo ELISA comerciales y no comerciales. Estos ensayos identificaron como positivas a rotavirus y calicivirus a un total de 14 y 58 muestras, respectivamente. Sin embargo, al realizar pruebas para corroborar la presencia de rotavirus por microscopia electrónica, EGPA y RT-PCR y para calicivirus por RT-PCR, ninguna de las muestras señaladas como positivas pudo ser confirmada. Estos resultados sugieren la no-circulación tanto de rotavirus como de calicivirus dentro de la Maternidad. Es posible que la interrupción de la transmisión de rotavirus dentro de la Institución pueda deberse a cambios en el manejo del par madre-neonato introducidos en la Maternidad desde 1995, promovidos por la OMS y UNICEF.


In a study carried out in 1984 at the "Concepción Palacios" Maternity Hospital, Perez-Schael et al (J Med Virol 1984, 14:127) detected asymptomatic excretion of rotavirus in 56% of the neonates evaluated. The purpose of this work was to study rotavirus infection in neonates at the Maternity Hospital and compare the new data with those previously obtained. We also studied calicivirus presence in said population. Between August and December 2004, 307 feces samples were collected from 215 healthy neonates. Commercial and non commercial ELISA type assays were used for detection of viral agents. These assays identified a total of 14 and 58 samples as rotavirus and calicivirus positive respectively. Nevertheless, when carrying out tests to corroborate rotavirus presence with electron microscopy, EGPA and RT-PCR, and for calicivirus with RT-PCR, none of the samples previously shown as positive could be confirmed. These results suggest the non-circulation of both rotavirus and calicivirus within the Maternity Hospital. It is possible that the interruption of rotavirus transmission within this Institution could be due to changes in the management of the mother-child unit promoted by WHO and UNICEF, and introduced at the Maternity Hospital since 1995.

The aim of this study was to determine the prevalence of several viral antibodies in non-human primates from two zoological gardens from Venezuela. Only two out of 66 sera were positive for antibodies to dengue virus by hemagglutination inhibition. Six out of 62 sera exhibited antibodies against Hepatitis B virus (HBV) core antigen. Viral DNA was detected by nested PCR in one positive serum and in three negative without serological evidence of HBV infection. Sequence analysis showed the circulation of HBV genotype F, predominant in Venezuela. Antibodies against rotavirus antigens were found in 87 percent (20/23) of Old World primates and in 50 percent (13/26) of New World primates. Both the prevalence of antibodies and the mean O.D. value by ELISA were significantly lower in New World primate sera. These results suggest that non-human primates do not seem to represent an important reservoir for dengue virus infection, highly endemic in Venezuela. Anthropozoonotic infection of HBV seems to occur among primates not only from the Old but also from the New World, reinforcing the importance of vaccination of species at risk. This study also suggests a lower frequency of infection by rotavirus of non-human primates from the New World, compared to primates from the Old World.


En este estudio se determinó la prevalencia de anticuerpos contra varios virus en primates no humanos de dos parques zoológicos de Venezuela. Sólo dos de 66 sueros fueron positivos, por inhibición de la hemaglutinación, para anticuerpos contra virus dengue. Seis de 62 sueros presentaron anticuerpos contra la cápside del virus de la hepatitis B virus (VHB). Se detectó el ADN viral, mediante PCR en dos rondas, en uno de éstos y en tres sueros sin evidencia serológica de infección por VHB. El análisis de la secuencia mostró la circulación del VHB genotipo F, predominante en Venezuela. Un 87 por ciento (20/23) de los sueros de primates del Viejo Mundo y un 50 (13/26) de los del Nuevo Mundo mostraron anticuerpos contra antígenos de rotavirus. Tanto la prevalencia de anticuerpos como los valores promedio de D.O. por ELISA fueron significativamente menores en sueros de primates del Nuevo Mundo. Los primates no humanos no parecen jugar un papel importante como reservorio de la infección por virus dengue, altamente prevalente en el país. La infección por cepas humanas del VHB en primates sugiere infección antroponótica y la importancia de vacunar las especies a riesgo. Los resultados sugieren igualmente una menor frecuencia de infección por rotavirus en primates del Nuevo Mundo.

There is evidence that severe dengue disease is associated with alterations of the microvascular endothelium. We examined the hypothesis that activation and damage of microvascular endothelial cells (EC) could be induced by inflammatory mediators present in dengue patient's sera. We cultured human microvascular EC (HMEC-1) in vitro with sera from patients with acute dengue infection. Sera from patients with acute dengue induced an increase in ICAM-1 expression on HMEC-1. This effect was greater with samples from the acute febrile phase than with samples from the convalescent phase of the disease. Acute dengue sera had elevated levels of TNF-alpha and the endothelial activating effect of acute dengue sera was inhibited up to 80% by pre-treatment with monoclonal antibodies against TNF-alpha. Furthermore, acute dengue sera induced apoptosis in HMEC-1. These findings support the pathophysiologic significance of microvascular EC and serum inflammatory mediators in dengue.

Surface antigen negative hepatitis B virus (HBV) infection was evaluated in Venezuela, by molecular characterization of blood samples positive for antibodies to core antigen (anti-HBc) and negative for surface antigen (HBsAg) in blood donors (residual infections). HBV DNA was found in 11/258 samples (4.3%), and was significantly associated with high levels of anti-HBc antibodies (>25 UI/ml, P < 0.05), while no correlation was found between the presence of HBV DNA and the levels of anti-HBs. Synonymous and non-synonymous mutations were found in the HBV surface region (but not vaccine escape mutants) and in the precore/core region (precore mutants in 2/7 samples and 33-45 bp deletions near the N-terminal core region in 4/19 samples). While HBV genotype F prevails among HBsAg positive samples from blood donors in Venezuela, residual infection isolates were mainly genotypes A and D. Phylogenetic analysis of viral surface and core region revealed discrepancies in genotype designation in 6/9 samples, suggesting the presence of mixed infection or recombination. In conclusion, HBV residual infection in Venezuela does not seem to be frequently observed in HBV genotype F. This type of infection is frequently associated with variants exhibiting mutations in the surface gene that might be affecting the correct recognition by commercial tests, with precore mutants and with core internal deletions. These variants do not seem to cause severe liver disease, and on the contrary, were found circulating at low viremia.

El virus dengue (VD) es responsable de un espectro de enfermedades que van desde una enfermedad febril autolimitante (FD, fiebre por dengue) hasta las formas severas, fiebre hemorrágica/síndrome de shock por dengue (FHD/SSD). El propósito del presente estudio fue la confirmación serológica y molecular de un brote de dengue en el Estado Falcón, Venezuela con la finalidad de confirmar la etiolgía de la enfermedad, determinar los serotipos infectantes y la relación del diagnóstico clínico con las respuestas inmunitarias en los pacientes con infección activa. Se analizaron 54 sueros de pacientes con diagnóstico clínico de infección por VD, mediante inmunoensayos enzimaticos desarrollados en Venezuela (ELISA, IgM e IgG) y por PCR. El 78 por ciento de los pacientes mostró evidencia de infección por VD (PCR+ y/o IgM), 48 por ciento presentaron viremia demostrada por PCR+ y 57 resultaron positivos a IgM, lo que sugiere que un alto número de los casos reportados como dengue en el país se deben efectivamente a la infección por VD. Un hecho resaltante fue el alto porcentaje (76 por ciento) de pacietnes con infección pasada o secundaria al VD (IgG positivos), dentro de los cuales se encontraban la totalidad de los pacientes diagnósticados clínicamente con FHD (n=8). De los pacientes PCR positivos, el 27 por ciento correspondió al serotipo 1,54 por ciento al serotipo 2 y 19 al serotipo 4. Para el momento de esta evaluación no se determinó la presencia del serotipo 3, aunque se conocía de su presencia en la cercana Isla de Aruba, la combinación de métodos serológicos y moleculares nos permitió obtener una información bastante precisa de este brote.

Caliciviruses are a well-established cause of respiratory, vesicular and hemorrhagic diseases in animals. In addition, these viruses are an important cause of enteric diseases in humans. Recently, molecular analysis of several bovine enteric calicivirus isolates indicated that they are genetically close to human enteric calicivirus. To investigate if bovine enteric caliciviruses circulate in Venezuela, 129 stool samples collected between 1994 and 2000 were assayed by reverse transcription-polymerase chain reaction amplification. The presence of calicivirus was confirmed in one of the samples analyzed, collected in the Lara State from a healthy calf, 2 months old. Phylogenetic studies based on partial RNA polymerase sequences indicated that the Venezuelan isolate (Bo/NV/Lara/2000/VE) is most closely related to the genogroup III, genus Noroviruses.

The genetic diversity and phylogenetic relationships of a collection of strains of dengue virus type 1 (DV-1), isolated from different parts of the world, were investigated. Phylogenetic trees derived from the complete sequence of the E gene of 44 strains suggested the existence of five genetic types defined by a maximum nucleotide divergence within each group of 6%. The 22 strains from America were classified into a single genetic type that included strains associated either with classical dengue or hemorrhagic dengue episodes. Using a maximum likelihood procedure based on a single rate with dated tips model and substitution rates calculated at the third codon position, evolution of the five DV-1 genotypes was shown to conform to a molecular clock. The average rate of evolution was estimated to be approximately 16.2 x 10(-4) substitutions/third codon position site/year. Using this estimate, divergence among the DV-1 genotypes was calculated to have occurred approximately 100 years ago. Very low average value of the ratio of nonsynonymous-to-synonymous nucleotide substitutions, relative to the respective sites (0.046), indicated that the evolution of the E gene of the DV-1 is subject mostly to purifying selection.

The Instituto Venezolano de Investigaciones Cientificas (IVIC) is a government-funded multidisciplinary academic institution dedicated to research, development and technology in many areas of knowledge. Biomedical projects and publications comprise about 40 percent of the total at IVIC. In this article, we present an overview of some selected research and development projects conducted at IVIC which we believe contain new and important aspects related to malaria, ancylostomiasis, dengue fever, leishmaniasis and tuberculosis. Other projects considered of interest in the general area of tropical medicine are briefly described. This article was prepared as a small contribution to honor and commemorate the centenary of the Instituto Oswaldo Cruz

En cuatro ciudades de Venezuela se llevó a cabo un estudio para evaluar las características epidemiológicas, clínicas y etiológicas de la diarrea aguda en niños menores de 5 años. Entre junio de 1993 y mayo de 1995 se estudiaron 2.552 niños con diarrea y 793 controles que fueron atendidos en el hospital. Para el análisis estadístico de los resultados se empleó la prueba exacta de Fisher. Los rotavirus fueron los agentes más importantes, tanto por su frecuencia (30 por cien) como por su asociación con la deshidratación (58 por cien). Le siguieron en importancia Campylobacter spp. (13 por cien) y Escherichia coli serogrupos O clásicos (9 por cien), pero su asociación con la diarrea solo fue estadísticamente significativa en los niños menores de 3 meses, hecho de particular importancia desde el punto de vista del tratamiento. Se confirmó la importancia de la edad como factor determinante de la prevalencia y gravedad de la diarrea


In four cities of Venezuela a study was carried out to evaluate the epidemiological, clinical, and etiological characteristics of acute diarrhea in children under 5 years of age. The study was done between June 1993 and May 1995 and involved children who were seen in a hospital, 2 552 with diarrhea and 793 controls. The Fisher exact test was used for the statistical analysis of the results. Rotaviruses were the most important agents, both in terms of their frequency (30%) and their association with dehydration (58%). Following in importance were Campylobacter spp. (13%) and Escherichia coli classical O serogroups (9%), but their association with diarrhea was only statistically significant among children less than 3 months old, a fact that is particularly important from the standpoint of treatment. The importance of age was confirmed as a determining factor in the prevalence and severity of diarrhea.

La determinación de anticuerpos IgM anti-cápside (anti-AgcHB) del Virus de la Hepatitis B (VHB) es de gran utilidad para la identificación de infecciones agudas por este virus. El objeto de este estudio fue el desarrollar un sistema inmunodiagnóstico para la detección de anticuerpos IgM anti-AgcHB) Para la realización del ensayo fue necesaria la producción del AgcHB recombinante expresado en Escherichia coli. Una vez obtenido y purificado el antígeno se procedió a estandarizar y evaluar el ensayo. Un ELISA sandwich de captura resultó ser el último para la detección de anticuerpos IgM anti-AgcHB. Se realizó entonces la evaluación de 110 sueros humanos a través de dos ensayos inmunodiagnósticos, el inmunoensayo diagnóstico desarrollado y un estuche comercial. El inmunoensayo desarrollado arrojó una alta sensibilidad (99 por ciento) y especificidad (93 por ciento) al compararlo con el estuche comercial. Los resultados obtenidos permitieron evidenciar la utilidad del sistema de determinación de anticuerpos IgM anti-AgcHB como marcador serológico de una infección causada por el VHB, en particular en áreas endémicas de Sur América

Viral hepatitis serological markers were analized in two groups of pregnant women residing in Caracas from:1) a maternity unit at the moment of delivery (106 sera, low income population), and 2) a private clinic during the third trimester of pregnancy (105 sera, medium-high economic class population). A higher percent positivity was observed in the maternity unit compared to the private clinic for hepatitis A virus (HAV) as measured by anti-HAV activity (96 por ciento vs 48 per cent; p<0.01 per cent), for hepatitis B virus (HBV) as measured by anti-HBc activity (13 per cent vs 2 per cent; p<0.01 per cent), but not for HBV carriage, as measured by HBsAg (3.8 per cent vs 0 per cent; p=0.06 per cent). These differences appear to correlate with the socio-economic level. All the HBsAg positive sera were HBsAg negative and negative for the presence of DNA by PCR, confirming the low rate of perinatal transmission observed in Venezuela. Two out of 106 sera (1.9 per cent) were positive for HCV antibodies in the maternity unit and 0/105 in the private clinic, although these differences were non significant (NS). Two out of 106 sera (1.9 per cent) were positive for HEV antibodies in the maternity unit and 1/80 (1.3 per cent) in the private clinic (NS). The anti-HEV seropositivity probably reflects a past infection. The importance of testing these viral markers during pregnancy is discussed
