INTRODUCTION: Stenotrophomonas maltophilia is a Gram-negative, opportunistic pathogen associated with a high morbidity and mortality rate. We report our clinical experience in treating a patient with infected pancreatic necrosis caused by multidrug-resistant (MDR) S. maltophilia with a novel drug combination. CASE REPORT: A 65-year-old male with history of type II diabetes was admitted with acute pancreatitis, voluminous ascites, and signs of sepsis after undergoing an echo-endoscopy procedure with pancreas biopsy to investigate a Wirsung duct dilatation. Retroperitoneal fluid culture revealed S. maltophilia resistant to colistin and with intermediate susceptibility to trimethoprim-sulfamethoxazole and levofloxacin. The synergy between aztreonam (ATM) and ceftazidime/avibactam (CZA) was demonstrated using the combined disk pre-diffusion test. CONCLUSIONS: There are sparse data providing guidance on the optimal regimen against MDR S. maltophilia infections. Although in this case a surgical excision was essential, combination of ATM and CZA provided effective synergistic antimicrobial treatment with clinical cure of severe acute pancreatitis infected with S. maltophilia. The combined disk pre-diffusion test with ATM and CZA requires no special equipment and can be routinely performed in clinical microbiology labs. Combination of ATM with CZA should be considered for cases of MDR S. maltophilia infections with limited treatment options.

Denaturing high-performance liquid chromatography (dHPLC) was developed to screen DNA variations by separating heteroduplex and homoduplex DNA fragments by ion-pair reverse-phase liquid chromatography. In this study, we have evaluated the dHPLC screening method and direct sequencing for the detection of GATA1 mutations in peripheral blood and bone marrow aspirates samples from children with Down syndrome (DS). Cases were ascertained consecutively as part of an epidemiological study of DS and hematological disorders in Brazil. A total of 130 samples corresponding to 115 children with DS were analysed using dHPLC and direct sequencing methods to detect mutations in GATA1 exons 2, 3 and 4 gene sequences. The overall detection rate of sequencing and dHPLC screening methods was similar. Twenty mutations were detected in exon 2 and one mutation in exon 3 (c.231_232 dupGT) sequences of acute megakaryoblastic leukemia and transient leukemia samples. Four GATA1 mutations were newly described [c.155C > G; c.156_178 del23 bp; c.29_30 del GG; c.182C > A and c.151A > T,c.153_162 del 10 bp). Out of four, three had single nucleotide change. In conclusion, our results indicate that dHPLC is an efficient and valuable tool for GATA1 mutational analysis.

RESUMO

Através de um levantamento bibliográfico o autor discute as vantagens e desvantagens da utilização da Fixação Interna Rígida quando comparada a osteossíntese a fios de aço, comprovando a superioridade técnica, a menor incidência de infecções pós-operatórias e o menor custo de tratamento da Fixação Interna Rígida em relação a Ostessíntese a Fios de Aço.


The autor through a medical literature study dicusses and evaluates the advantages and disadvantages of the use of Rigid Internal Fixation when compared to Wire Osteosynthesis demonstrating that Rigid Internal Fixation is better taking into consideration the technical point of view than Wire Osteosynthesis on account of the lower incidence of infections and the cost benefit.

BACKGROUND: The usefulness of daunorubicin (DAUNO) and doxorubicin, members of the anthracycline class of anticancer drugs, is limited by their cardiotoxicity. The purpose of our echocardiographic study was to assess the left ventricular (LV) function in long-term pediatric cancer survivors who had received DAUNO and dox as part of their therapy. PATIENTS AND METHODS: Seventy patients and 70 age, sex, and body surface area matched healthy controls were evaluated. Among the patients, the mean cumulative anthracycline dose was 321.6 (range, 150 to 868 mg/m); the mean interval from cancer diagnosis to evaluation was 13.9 years (range, 7 to 30 y). RESULTS: Fifteen of the 70 (21.4%) patients had a reduced ejection fraction (EF). Compared with the healthy controls, the mean EF and E/A were decreased to low normal levels in the patients, whereas deceleration time and isovolumetric relaxation time were prolonged. The E/A and deceleration time were significantly different for the females but not the males. The Tissue Doppler Index was normal but the Myocardial Performance Index was prolonged and correlated with EF (r=-0.499, P<0.001). There was a correlation between EF with cumulative anthracycline dose (r=-0.306; P=0.010) and time off therapy (r=-0.281; P=0.019). Diastolic indices suggested a tendency toward abnormal LV relaxation. Myocardial Performance Index seems to be a good index for monitoring LV status, because it was prolonged as EF decreased. CONCLUSIONS: This study suggests that long-term survivors who received doxorubicin and DAUNO may be found to have subclinical features of myocardial dysfunction when evaluated years after the completion of therapy.

This study evaluates the diagnosis, therapy and survival of 14 patients with primary intracranial germ cell tumors during the period from 1991 to 2001. There were 11 males and 3 females. Mean age was 12.2 years old (20 days-18 years). On admission, the most common symptoms were headache (10/14), vomiting (6/14) and visual (6/14). The tumor was in pineal and hypothalamic region in 10 cases, suprasellar in 3 cases, and in the cerebral parenchyma in 1 case. Histologically there were 1 embryonal carcinoma, 5 germinomas, 2 mature teratomas, 1 immature teratoma and 5 mixed germ cell tumors. Treatment differed among the patients according to the type of tumor. Three patients died after tumor progression or relapse and one patient died from another condition. The remaining patients are alive and without disease.

RESUMO

We report a case of a 16-month-old girl with a primary endodermal sinus tumor (EST) of the parotid gland. The girl was admitted to the hospital with a left side cervical tumor with a quick growth (within 3 wk). The tumor was surgically resected. The gross examination showed a 7.0-cm hemorrhagic and fragmentized mass. Diagnosis of an EST was established on the hematoxylin and eosin-stained slides from the resected material. Elevated serum alpha-fetoprotein (AFP) levels were detected and follow-up examinations of the patient did not show ESTs in other locations or other manifestations due to the tumor. The patient received chemotherapy after the surgery and remained alive without evidence of disease at the time of this writing (2 years after the diagnosis). As far as we know, this is the second report in the literature of a parotid gland's EST.

Centric relation (CR) has been considered a maxillomandibular position of choice for some dental and prosthetic procedures. Although regarded as a fully reproducible relation, there is great controversy about its clinical use and recording technique, especially in patients with signs and symptoms of temporomandibular disorders (TMD). This study aimed at evaluating the effectiveness of a training program on intra- and interexaminer agreement when determining the clinical record of the CR position. Forty individuals constituted the sample, divided into symptomatic (TMD) and asymptomatic groups. Three previously calibrated examiners performed the initial assessment and the second evaluation after 30 days in a blind design, using DawsonÆs bilateral manipulation technique with and without an anterior stop. The amount of frontal and sagittal deviations in relation to intercuspal position, the presence of pain and discomfort during manipulation, and the first occlusal contact in CR were analyzed. Kendall and Kappa tests with a 5 percent level of significance were used to determine agreement. Values for both intra- and interexaminer agreement were deemed good. The best results were obtained for frontal deviations and assessment of pain (or absence of it) during manipulation. Sagittal deviations showed the lowest agreement in both examinations. The authors concluded that a calibration program could be effective for intra- and interexaminer agreement when recording centric relation. However, caution is recommended when analyzing some isolated items
