RESUMO

Vortex matter in layered high-[Formula: see text] superconductors, including iron-pnictides, undergo several thermodynamic phase transitions due to the complex interplay of pinning energy, thermal energy and elastic energy. Moreover, the presence of anisotropy makes their vortex physics even more intriguing. Here, we report a detailed vortex dynamics study, using dc magnetization measurements, in a triclinic iron-pnictide superconductor (Ca[Formula: see text]La[Formula: see text])[Formula: see text](Pt[Formula: see text]As[Formula: see text])(Fe[Formula: see text]As[Formula: see text])[Formula: see text], with a superconducting transition temperature, T[Formula: see text] [Formula: see text] 31 K. A second magnetization peak (SMP) feature is observed for magnetic field perpendicular (H[Formula: see text]c) and parallel (H[Formula: see text]ab) to the crystal plane. However, its fundamental origin is quite different in both directions. For H[Formula: see text]c, the SMP can be well explained using an elastic-to-plastic vortex creep crossover, using collective creep theory. In addition, a possible rhombic-to-square vortex lattice phase transition is also observed for fields in between the onset-field and peak-field related to the SMP. On the other hand, for H[Formula: see text]ab, a clear signature of an order-disorder vortex phase transition is observed in the isothermal M(H) measurements at T [Formula: see text] 6 K. The disordered phase exhibits the characteristics of entangled pinned vortex-liquid. We construct a comprehensive vortex phase diagram by displaying characteristic temperatures and magnetic fields for both crystal geometries in this unique superconducting compound. Our study sheds light on the intricate vortex dynamics and pinning in an iron-pnictide superconductor with triclinic symmetry.

Objective: The present study aims to evaluate the viability of adult human neural cells in culture obtained from traumatized brain tissues collected in emergency surgery procedures. Methods: Exploratory, descriptive, quantitative and cross-sectional study evaluating samples obtained from patients who underwent traumatic brain injury with extrusion of brain tissue submitted to cell culture in a standardized medium, being preserved during 168h. After observation under phase contrast microscopy and immunohistochemical processing for neuronal (MAP-2) and glial (GFAP) markers, morphometric parameters of neural cells (cell body area, dendritic field length and fractal dimension) were evaluated using ImageJ software, with data obtained after 24, 72 and 168h being compared using non-parametric Kruskal Wallis test, followed by Dunn's post hoc test. Results: The explant of the nervous tissue revealed a consolidated pattern of cell migration into the culture medium. Cell proliferation, upon reaching confluence, presented an aspect of cellular distribution juxtaposed along the culture medium at all time points analyzed. Both neurons and glial cells remained viable after 168h in culture, with their morphologies not varying significantly throughout the time points evaluated. Immunohistochemistry for MAP-2 showed a relatively well-preserved cytoskeletal organization. GFAP immunoreactivity revealed activated astrocytes especially at the later time point. Conclusions: Our results point out the viability of cell culture from traumatized human nervous tissue, opening up perspectives for the use of substances of natural origin that may contribute neuroprotectively to neuronal maintenance in culture, allowing future translational approach.

AIM: The aim of these recommendations is to set forth an individualized approach to the management of early postmenopausal women (i.e., within the first 10 years after natural menopause) covering all aspects of lifestyle and therapeutic management, with or without menopause hormone therapy (MHT). MATERIALS AND METHODS: Literature review and consensus of French expert opinion. Recommendations were graded according to the HAS methodology and levels of evidence derived from the international literature, except when there was no good-quality evidence. SUMMARY RECOMMENDATIONS: The beginning of menopause is an ideal time for each woman to evaluate her health status by assessing her bone, cardiovascular, and cancer-related risk factors that may be amplified by postmenopausal estrogen deficiency and by reviewing her lifestyle habits. Improving lifestyle, including nutrition and physical activity, and avoiding risk factors (notably smoking), should be recommended to all women. MHT remains the most effective treatment for vasomotor symptoms but it could be also recommended as first-line treatment for the prevention of osteoporosis in early postmenopausal women at low to moderate risk for fracture. The risks of MHT differ depending on its type, dose, duration of use, route of administration, timing of initiation, and whether a progestogen is used. There is reasonable evidence that using transdermal estradiol in association with micronized progesterone or dydrogesterone may limit both the venous thromboembolic risk associated with oral estrogens and the risk of breast cancer associated with synthetic progestins. Treatment should be individualized to each woman, by using the best available evidence to maximize benefits and minimize risks, with periodic reevaluation of its benefit-risk balance. For bothersome genitourinary syndrome of menopause (GSM) symptoms, vaginal treatment with lubricants and moisturizers is recommended as first-line treatment together with low-dose vaginal estrogen therapy, depending on the clinical course. No recommendation of an optimal duration of MHT can be made, but it must take into consideration the initial indication for MHT as well as each woman's benefit-risk balance. Management of gynecological side-effects of MHT is also examined. These recommendations are endorsed by the Groupe d'Etude sur la Ménopause et le Vieillissement hormonal (GEMVI) and the Collège National des Gynécologues-Obstétriciens Français (CNGOF).

Nowadays, the development of new technological solutions in the medical field, in particular biosensors, is a priority and a ground for great scientific and financial investment. From glucose sensors to highly sensible and more precise molecular tools, this biotechnological field has gone through an exponential growth, but still the applications are very limited to the future potential foreseen in the medical area. In the last decade, the advances in the genomic field have permitted the identification of specific biomarkers related to certain diseases, becoming one of the main approaches used in clinical diagnosis. Biomarkers have different clinical values, in the sense that they may provide preventive, predictive, prognostic and therapeutic response related information, not being exclusively used for diagnostic purposes, but also be applied in health management and disease treatment. Therefore, biomarkers allied with biosensors have the potential to revolutionize the way healthcare is managed. The vast choice of bioreceptors such as nucleic acids, antibodies, antigens, enzymes and even whole cells, consents the diagnosis of diseases ranging from viral and bacterial infections to cancer and metabolism disorders. The major appeal of these sensing platforms is that it provides a fast, cost-effective, reliable, highly sensitive and easy way to obtain an earlier clinical diagnosis, which can significantly affect the survival rate or patient's prognosis. This review will explore some of the most recent devices available and its clinical applications.

Amazonian livelihoods are largely dependent on rivers, with local protein consumption mainly relying on several species of fish. The UJER (Upper Juruá Extractive Reserve - Reserva Extrativista do Alto Juruá) is located in the state of Acre, bordering Peru and several indigenous areas. Here we summarize the data we collected in 1993/1994 on the population living along the banks of the Juruá, Tejo, Bagé, Igarapé São João and Breu rivers on crop cultivation, animal husbandry, and use of game and fish resources. We interviewed 133 individuals (94 on the Juruá and Tejo, 16 on Bagé, 16 on Igarapé São João and 7 on Breu rivers). Our results include a comprehensive description on local livelihoods, including the most important fish species for local subsistence considering gender and seasonality, the main husbandry and game species, and the items cultivated on the local agriculture. Whenever more recent information was available in the literature, we compared changes in livelihoods over time in the same region and also with the recent patterns observed in the Lower and in the Middle Juruá River. We hope to provide useful information to understand temporal changes in local livelihoods, which can help adapt and shape the ecological management in the region.

Foram entrevistados 133 indivíduos (94 nos rios Juruá e Tejo, 16 em Bagé, 16 em Igarapé São João e 7 no rio Breu). Nossos resultados incluem uma descrição abrangente dos meios de subsistência locais, incluindo as espécies de peixes mais importantes para a subsistência local, considerando gênero e sazonalidade, bem como as principais espécies de criação, de caça e os itens cultivados na agricultura local., Quando foi possível obter informações da literatura, comparamos as mudanças nos meios de subsistência ao longo do tempo na mesma região e também com os padrões recentes observados no Baixo e no Médio Rio Juruá. Esperamos fornecer informações úteis para o entendimento das mudanças temporais nos meios de subsistência locais, o que pode ser útil ao manejo ecológico da região.

This study aimed to evaluate comparatively the effects of propofol or isoflurane on hemodynamic variables in piglets that received inspired oxygen fraction (FIO2) of 0.5 under spontaneous ventilation. Therefore, sixteen piglets weighing 16±1.1kg, were randomly divided into two groups: GI (Isoflurane and FIO2 of 0.5) and GP (Propofol and FIO2 of 0.5). Heart rate (HR), systolic, diastolic and mean arterial pressure (SAP, DAP and MAP), central venous pressure (CVP), cardiac output (CO), mean pulmonary arterial pressure (mPAP) and mean capillary pulmonary pressure (mCPP) were assessed 40 minutes after anesthetic induction (T0), followed by 15 minutes intervals (from T15 to T60). The variables cardiac index (CI), stroke volume (SV), stroke index (SI), total peripheral resistance (TPR), total peripheral resistance index (TPRI), pulmonary vascular resistance (PVR), and pulmonary vascular resistance index (PVRI) were calculated. SAP and TPRI were significantly different between groups at T30 and T60 (P< 0.05) with higher GP values being recorded. There were no differences in the other variables, however, GP presented mean closer to normality on most of the analyzed variables. Therefore, we conclude that total intravenous anesthesia with propofol presented greater stability of the hemodynamic variables evaluated.(AU)


O objetivo deste estudo foi avaliar comparativamente os efeitos do propofol ou do isoflurano sobre as variáveis hemodinâmicas em leitões que receberam fração inspirada de oxigênio (FIO2) de 0,5 sob ventilação espontânea. Dezesseis leitões, pesando 16±1,1kg, foram divididos aleatoriamente em dois grupos: GI (isoflurano e FIO2 de 0,5) e GP (propofol e FIO2 de 0,5). A frequência cardíaca (FC), a pressão arterial sistólica, a diastólica e a média (PAS, PAD e PAM), a pressão venosa central (PVC), o débito cardíaco (DC),a pressão média da artéria pulmonar (PAPm) e a pressão média capilar pulmonar (PCPm) foram avaliados 40 minutos após a indução anestésica (T0), seguida por intervalos de 15 minutos (de T15 a T60). As variáveis índice cardíaco (IC), volume sistólico (VS), índice sistólico (SI), resistência periférica total (RPT), índice de resistência periférica total (IRPT), resistência vascular pulmonar (RVP) e índice de resistência vascular pulmonar (IRVP) foram calculadas. PAS e IRPT foram significativamente diferentes entre os grupos em T30 e T60 (P<0,05) com maiores valores de GP sendo registrados. Não houve diferenças nas demais variáveis, entretanto o GP apresentou médias próximas da normalidade na maioria das variáveis analisadas. Portanto, concluiu-se que a anestesia intravenosa total com propofol apresentou maior estabilidade das variáveis hemodinâmicas avaliadas.(AU)

It is important to identify the best inspired fraction of oxygen in a variety of situations, including sevoflurane or isoflurane anesthesia, in spontaneously breathing rabbits. For this, 64 rabbits were assigned to eight groups: GI100 (FiO2= 1,0 + isoflurane), GS100 (FiO2= 1,0 + sevoflurane), GI80 (FiO2= 0,8 + isoflurane), GS80 (FiO2= 0,8 + sevoflurane), GI60 (FiO2= 0,6 + isoflurane), GS60 (FiO2= 0,6 + sevoflurane), GI21 (FiO2= 0,21 + isoflurane), GS21 (FiO2= 0,21 + sevoflurane). The induction was performed with (2.5MAC) of the anesthetic. The vaporizer was setted at 1.5 MAC and FiO2 as attributed for each group. After the induction, the concentration was changed to 1 MAC. Measurements of parameters were performed 30 minutes after induction (T0), and then at 15 minute intervals (from T15 to T60). The arterial partial pressures of oxygen (PaO2), alveolar oxygen partial pressure (PAO2) and alveolar-arterial oxygen gradient [P(A-a)O2] were higher with the use of high FiO2. The GI80 showed higher levels of PaO2 FiO2 ratio and respiratory index (RI). In conclusion, the FiO2 of 0.21 is not indicated, because it causes hypoxemia. The isoflurane determines better ventilation when compared to sevoflurane, but isoflurane associated with 80% of oxygen promotes intrapulmonary shunt increase.(AU)


Tornou-se importante identificar a melhor fração inspirada de oxigênio em variadas situações, incluindo anestesia pelo sevoflurano ou isoflurano, em coelhos respirando espontaneamente. Para isso, 64 coelhos foram distribuídos em oito grupos: GI100 (FiO 2 = 1,0 + isoflurano), GS100 (FiO 2 = 1,0 + sevoflurano), GI80 (FiO 2 = 0,8 + isoflurano), GS80 (FiO 2 = 0,8 + sevoflurano), GI60 (FiO 2 = 0,6 + isoflurano), GS60 (FiO 2 = 0,6 + sevoflurano), GI21 (FiO 2 = 0,21 + isoflurano) e GS21 (FiO 2 = 0,21 + sevoflurano). A indução foi com 2,5 CAM do anestésico. Ajustou-se o vaporizador para 1,5 CAM, e a FiO 2 foi atribuída a cada grupo. Em seguida, a CAM foi reajustada para 1,0. Iniciaram-se as mensurações 30 minutos após a indução (M0), seguidas em intervalos de 15 minutos (de M15 a M60). As pressões parciais de oxigênio (PaO 2 ), a pressão parcial alveolar de oxigênio (P A O 2 ) e a diferença alvéolo-arterial de oxigênio [P(A-a)O 2 ] foram maiores com o emprego de altas FiO 2 . O GI80 apresentou maiores valores na relação entre PaO 2 e FiO 2 e índice respiratório (IR). Conclui-se que a FiO 2 0,21 não é indicada, pois provoca hipoxemia. No entanto, utilizada com isoflurano, determina melhor ventilação quando comparado ao sevoflurano, porém seu uso, associado a 80% de oxigênio, promove maior formação de shunt intrapulmonar.(AU)

OBJECTIVE: To evaluate the impact of hysterectomy in case of genital prolapse on the anatomical and functional results, and on per and post operative complications compared with uterine preservation. MATERIAL AND METHODS: We conducted a review of the Pubmed, Medline, Embase and Cochrane literature using the following terms and MeSH (Medical Subject Headings of the National Library of Medicine): uterine prolapse; genital prolapse; prolapse surgery; vaginal prolapse surgery; abdominal prolapse surgery; hysterectomy; hysteropexy; sacrocolpopexy; surgical meshes; complications; sexuality; neoplasia; urinary; incontinence; cancer. RESULTS: Among the 168 abstracts studied, 63 publications were retained. Whatever performance of hysterectomy or not, anatomical and functional results were similar in abdominal surgery (sacrocolpopexy) (OR=2.21 [95% CI: 0.33-14.67]) or vaginal surgery (OR=1.07 [95% CI: 0.38-2.99]). There was no difference in terms of urinary symptoms or sexuality after surgery. Hysterectomy was associated to a higher morbidity (bleeding, prolonged operating time, longer hospital stay), to an increased risk of mesh exposure particularly in case of total hysterectomy (8.6%; 95% CI: 6.3-11). CONCLUSION: In the absence of evidence of superiority in terms of anatomical and functional outcomes, with an increased rate of complications, concomitant hysterectomy with prolapse surgery should probably not be performed routinely.

Abstract The lack of knowledge about the majority of fish species harvested in Amazonian small-scale fisheries, in association with impacts from hydroelectric power plants, may lead to biodiversity loss and a decrease in the protein food supply for riverine Amazonians. This study uses existing datasets on fisheries and riverine developmental projects to infer effects associated with fish losses where actual data and outcomes are not available. The targeted fish species' status may be regarded as either threatened or there being no knowledge of their conservation requirements, biology or ecology. Among the 90 Amazonian fish species that are the most important for the diet of the riverine fishers, 78% are not assessed or their biological information is unknown, according to the IUCN Red List. Consequently, the effects created by the thoroughly disregarded trade-off between energy generation and food security in the planning of Amazonian land use have been worsened by the lack of biological and ecological information on fish species.


Resumo A falta de conhecimento sobre a maioria das espécies alvo de comunidades pesqueiras da Amazonia, associada ao impacto das hidrelétricas pode levar ao descréscimo da biodiversidade e na disponibilidade de proteína para os ribeirinhos da Amazônia. As espécies alvo são vulneráveis ou pouco conhecidas em sua biologia ou ecologia. Dentre 90 espécies de peixes importantes na dieta dos ribeirinhos, 78% não são estudadas ou sua biologia é desconhecida, de acordo com a lista da UICN. Dessa forma, os efeitos criados pela negligenciada relação de custo e benefício entre a produção de energia e a segurança alimentar no planejamento da Amazônia tem ainda piorado a situação de desconhecimento sobre as espécies de peixes.

The effects of different concentrations of oxygen and nitrous oxide on blood gas parameters in pigs maintained under spontaneous or pressure-controlled ventilation, with or without positive end-expiratory pressure (PEEP), were compared. Forty-eight pigs were randomly divided into six groups, submitted to different concentrations of compressed air or N2O, associated with different fractions of inspired oxygen (FiO2). The group subject to 30% of compressed air (GA30) showed the closest proximity to the physiological range of partial pressure (PaO2) expected for the species. For oxygen saturation (SaO2), the values obtained were below the lower physiological limit in the group administered 30% N2O (GN30). Use of PEEP positively interfered in PaCO2 independent of FiO2, however, its effectiveness can be compromised when complemented by N2O-based anesthesia. For SaO2, only GN30 showed values lower than adequate for maintaining tissue oxygenation. The pH, base deficit and bicarbonate in arterial blood were influenced by FiO2 and N2O. In conclusion, the use of compressed air maintains blood gas parameters at their most stable, especially GA30 and PEEP, which seemed to positively influence the experimental groups, with some interference from FiO2 and N2O.(AU)


Compararam-se os efeitos de diferentes concentrações do óxido nitroso ou oxigênio sobre variáveis hemogasométricas, em suínos mantidos em ventilação espontânea ou controlada à pressão, associada ou não à pressão expiratória final positiva (PEEP). Foram utilizados 48 porcos, distribuídos em seis grupos. Administraram-se diferentes concentrações de ar comprimido ou N2O, associadas a diversas frações de oxigênio inspirado (FiO2). O grupo sujeito a 30% de ar comprimido (GA30) mostrou maior proximidade do intervalo fisiológico da pressão parcial de oxigênio (PaO2). Para a saturação de oxigênio (SaO2), observaram-se valores aquém do limite inferior fisiológico no grupo administrado com 30% de N2O (GN30). A utilização da PEEP é capaz de interferir positivamente na PaCO2, independentemente da FiO2, porém tem a efetividade comprometida quando há complemento da anestesia com o N2O. Para a SaO2, apenas o GN30 esboçou valores inferiores aos adequados para manutenção da oxigenação tecidual. O pH, o déficit base e o bicarbonato no sangue arterial foram influenciados pela FiO2 e pelo N2O. Concluiu-se que o uso do ar comprimido mantém os parâmetros hemogasométricos mais estáveis, com destaque para o GA30 e a PEEP, o que parece influenciar positivamente os grupos experimentais, mas com interferência da FiO2 e do N2O.(AU)

The objective was to study the use of ultrasound as a complementary test in the breeding soundness evaluation in male pigs and study the pattern of echogenicity of the testicular parenchyma in boars of different racial groups. Twenty-six adult boars from four different racial groups were used, 10 from the Piau breed (group 1), four from the commercial and finishing group (group 2), six Pietrain breed (group 3) and six from the Duroc breed (group 4). All animals were evaluated for breeding soundness evaluation and the ultrasound examination of the testicles. The groups of animals that were evaluated showed no difference in the main semen parameters that were evaluated, except for the sperm volume, concentration of the ejaculated sperm and the supravital staining; the lowest figures were for the animals from the Piau breed (group 1). In relation to the testicular biometrics, Duroc animals (group 4) had a greater scrotal width compared to the other groups. But when we assessed the intensity of pixels of the testicles, there was a difference between groups. The groups 2 (finishing animals), 3 (Pietrain) and 4 had no difference between themselves. Group 3 had greater pixel intensity in relation to group 1. Of the 26 animals studied, five showed an abnormality during ultrasound evaluation, like hydrocele, hyperechoic mass in the testicular parenchyma, cyst in the head of the epididymis and the presence of fluid in the head and tail of the epididymis. The various animal groups studied did not differ in the principal reproductive parameters evaluated, showing that despite the great variability of reproductive traits between breeds and within the same breed, the breeding soundness evaluation, the more complete it is, is essential for the selection of breeders and the ultrasonography of the reproductive system becomes an important addition in this examination.

Polymyositis is an idiopathic inflammatory myopathy, characterized by proximal muscle weakness and sometimes extramuscular manifestations. We report the case of a 51-year-old male, with history of complete heart block, which required pacemaker implantation, and subsequently heart failure, presenting to the emergency department with worsening of dyspnea and peripheral edema. He was admitted to the Internal Medicine ward with acute heart failure and started on diuretic therapy. During hospitalization, he was discovered to have marked rhabdomyolysis. Examination revealed proximal symmetrical muscle weakness and arthralgia. The immunological study, electromyography and muscle biopsy confirmed polymyositis. The patient was started on prednisolone with clinical improvement and resolution of rhabdomyolysis. The presence of conduction defect, ventricular dysfunction, mitral valve regurgitation, segmental hypokinesia (myocardial scintigraphy without perfusion defects) and pulmonary hypertension, as well as elevated troponin with improvement after specific therapy, points to cardiac involvement. Polymyositis is a rare entity, with an insidious evolution and a myriad of extramuscular features that can mimic other conditions. In particular, cardiac involvement may be the first and only recognized manifestation. The key point for the diagnosis is to contemplate the possibility of polymyositis.

OBJECTIVES: To characterize and describe treatment with ulipristal acetate (UPA) in a preoperative setting and to evaluate the safety, effectiveness, and Health Related Quality of Life (HRQoL) outcomes in a French population treated according to standard clinical practice. MATERIALS AND METHODS: Multicentre, prospective, non-interventional study (PREMYA) of patients diagnosed with moderate to severe symptoms of uterine fibroids and undergoing a preoperative treatment with UPA 5mg (Esmya®). Patients were followed for a total of 15months (3months UPA treatment and 12months after). Data were collected approximatively every 3months according to centre usual visit schedule. RESULTS: A total of 206 women were enrolled in France, of whom 197 were found to be eligible for data analysis. Physicians' assessments of patients' overall symptomatic changes, as measured on the Clinical Global Impression-Improvement (CGI-I) scale, indicated that 83.7% of patients were improved at end of treatment (month 3). On the patients' treatment benefit scale (PTBS), 94.7% of patients reported an improvement. These 2 measurements, pain and quality of life, remained improved after treatment cessation and during the entire period of follow-up. Only 58.4% of patients underwent surgery within the timeframe of the study follow-up of which the majority were of a conservative/minimal invasive nature. CONCLUSION: Many patients did not undergo surgery during the planned 12months follow-up period after treatment whereas all patients had an indication of surgery. All measurements of treatment outcome were markedly improved by 3 months of UPA 5mg treatment. CLINICAL TRIAL NUMBER: NCT01635452.

An increasing body of evidences has demonstrated the ability of the mature adipocyte to dedifferentiate into a population of proliferative-competent cells known as dedifferentiated fat (DFAT) cells. As early as the 1970s, in vitro studies showed that DFAT cells may be obtained by ceiling culture, which takes advantage of the buoyancy property of lipid-filled cells. It was documented that DFAT cells may acquire a phenotype similar to mesenchymal stem cells and yet may differentiate into multiple cell lineages, such as skeletal and smooth muscle cells, cardiomyocytes, osteoblasts, and adipocytes. Additionally, recent studies showed the ability of isolated mature adipocytes to dedifferentiate in vivo and the capacity of the progeny cells to redifferentiate into mature adipocytes, contributing to the increase of body fatness. These findings shed light on the potential for use of DFAT cells, not only for clinical purposes but also within the animal science field, because increasing intramuscular fat without excessive increase in other fat depots is a challenge in livestock production. Knowledge of the mechanisms underlying the dedifferentiation and redifferentiation of DFAT cells will allow the development of strategies for their use for clinical and animal science purposes. In this review, we highlight several aspects of DFAT cells, their potential for clinical purposes, and their contribution to adipose tissue mass in livestock.

Abstract The lack of knowledge about the majority of fish species harvested in Amazonian small-scale fisheries, in association with impacts from hydroelectric power plants, may lead to biodiversity loss and a decrease in the protein food supply for riverine Amazonians. This study uses existing datasets on fisheries and riverine developmental projects to infer effects associated with fish losses where actual data and outcomes are not available. The targeted fish species status may be regarded as either threatened or there being no knowledge of their conservation requirements, biology or ecology. Among the 90 Amazonian fish species that are the most important for the diet of the riverine fishers, 78% are not assessed or their biological information is unknown, according to the IUCN Red List. Consequently, the effects created by the thoroughly disregarded trade-off between energy generation and food security in the planning of Amazonian land use have been worsened by the lack of biological and ecological information on fish species.


Resumo A falta de conhecimento sobre a maioria das espécies alvo de comunidades pesqueiras da Amazonia, associada ao impacto das hidrelétricas pode levar ao descréscimo da biodiversidade e na disponibilidade de proteína para os ribeirinhos da Amazônia. As espécies alvo são vulneráveis ou pouco conhecidas em sua biologia ou ecologia. Dentre 90 espécies de peixes importantes na dieta dos ribeirinhos, 78% não são estudadas ou sua biologia é desconhecida, de acordo com a lista da UICN. Dessa forma, os efeitos criados pela negligenciada relação de custo e benefício entre a produção de energia e a segurança alimentar no planejamento da Amazônia tem ainda piorado a situação de desconhecimento sobre as espécies de peixes.

BACKGROUND: Urine monitoring programs represent an important strategy for early diagnosis of reactivation of BK polyomavirus (BKV) in kidney transplant recipients. This study analyzes a BKV urine screening model in kidney transplant patients. METHODS: Urinary screening for BKV reactivation was performed by urinary decoy cell and polymerase chain reaction (PCR) tests in samples from 32 consecutive kidney transplant patients, collected in a 6-month follow-up period. PCR in plasma samples and BKV immunohistochemical studies to assess BKV renal disease, if a kidney biopsy was indicated, were performed. RESULTS: The urinary screening for BKV among 32 renal receptors was positive in 18 patients (56%) by the concomitant use of the decoy cells and/or qualitative PCR at some time during the study period. Transfusion before transplantation was significantly associated with urinary decoy cell positive screening (odds ratio = 11; 95% confidence interval = 1.47 to 82.4; P < .05); and so was male sex (odds ratio = 2.02; 95% confidence interval = 1.07 to 3.83; P < .05). The clinical management of screening positive cases consisted of decreasing or changing the immunosuppression regimen. Sixteen renal biopsies were performed. Immunohistochemistry for SV40 T antigen was negative in all biopsies. After 1 year of follow-up, no patient developed BKV-associated nephropathy, and there was no difference in renal function between patients positive and negative for BKV urinary screening. CONCLUSIONS: Early urinary monitoring is effective in detection of BKV replication and represents a good strategy to minimize the deleterious effects caused by the presence of the virus on preservation of graft function.
