BACKGROUND: Indoleamine-2,3-dioxygenase (IDO) helps orchestrate immune suppression and checkpoint inhibitor resistance in hepatocellular carcinoma (HCC). BMS-986,205 is a novel oral drug that potently and selectively inhibits IDO. This Phase I/II study evaluated the safety and tolerability of BMS-986,205 in combination with nivolumab as first-line therapy in advanced HCC. METHODS: Adults with untreated, unresectable/metastatic HCC received BMS-986,205 at two dose levels (50-100 mg orally daily) in combination with fixed dose nivolumab (240mg/m2 IV on Day 1 of each 14-day cycle). The primary objective was to determine the safety and tolerability of this combination; secondary objectives were to obtain preliminary efficacy. RESULTS: Eight patients received a total of 91 treatment cycles in the dose escalation phase. All patients were Child Pugh A and 6 patients had underlying viral hepatitis. In the 6 evaluable patients, no dose-limiting toxicities (DLTs) were observed. The most common treatment-related adverse events (TRAEs) were aspartate transaminase (AST) and alanine transaminase (ALT) elevation (3 patients) and diarrhea, maculopapular rash and increased alkaline phosphatase (2 patients each). Grade 3 events were diarrhea and AST elevation (1 patient), and hyperglycemia and pancreatitis requiring treatment discontinuation (1 patient). No grade 4-5 events occurred. Partial response was observed in 1 patient (12.5%) and stable disease in 3 patients (37.5%), yielding a disease control rate of 50%. Median PFS was 8.5 weeks; median OS was not reached. CONCLUSION: Combination BMS-986,205 and nivolumab showed a manageable safety profile with durable benefit as first-line therapy in a meaningful subset of advanced HCC patients.

Introducción: El asma es una patología respiratoria caracterizada por inflamación cró-nica y reversible de las vías aéreas. Esta se asocia con factores de riesgo modificables y no modificables que influyen sobre su control y exacerbaciones. En países como Puer-to Rico y Cuba, la prevalencia del asma es significativamente mayor a la global (22,8%, 23% y 6,6%, respectivamente).


Introduction: Asthma is a respiratory pathology characterized by chronic and reversible airway inflammation. It is associated with modifiable and non-modifiable risk factors that influence its control and exacerbations. In countries such as Puerto Rico (22.8 %) and Cuba (23 %), the prevalence of asthma is significantly higher than the global prevalence (6.6 %).

Abstract Objective: The objective of the study was to establish the prognostic value of CSNRT regarding the necessity for pacemaker implantation in patients with atrial flutter (AFL) post-ablation. Methods: This prospective cohort study, conducted at the National Institute of Cardiology "Ignacio Chavez" in Mexico City, assessed patients who had undergone ablation procedures to correct AFL, posterior to which an autonomic blockade was performed, and CSNRT was measured. Results: The sample for this investigation was 40 patients. These were subdivided into two study groups depending on their requirement of pacemaker implant post-ablation (Pacemaker P, No Pacemaker NP). Sinus node (SN) dysfunction was diagnosed in 13 (32.5%) of the 40 participants, 10 (71.43%) of which required a pacemaker implant, while only 4 participants (28.57%) with normal SN function required pacemakers. Ten out of the 14 patients (71.43%) who required a pacemaker had an elevated CSNRT > 500 ms (p ≤ 0.01). Post-ablation CSNRT mean was 383.54 ms ± 67.96 ms in the NP group versus 1972.57 ms ± 3423.56 ms in the P group. Furthermore, SN pause in the P group had a mean of 1.86 s ± 0.96 s versus the NP group with 1.196 s ± 0.52 s. Conclusion: CSNRT has the potential to be a quantitative prognostic tool for the assessment of future pacemaker implants in patients with AFL post-ablation. This could aid in the timely diagnosis of sinus node dysfunction, which could, in the long run, result in the reduction of cardiac functional capacity loss due to cardiac remodeling.


Resumen Objetivo: Establecer el valor pronóstico del TRNSC basado en la necesidad de marcapasos en pacientes diagnosticados con aleteo atrial, pos-ablación. Métodos: Este cohorte prospectivo, realizado en el Instituto Nacional de Cardiología "Ignacio Chávez" en la Ciudad de México, evaluó pacientes sometidos a ablación para corregir el aleteo atrial; se midió el TRNSC post bloqueo autonómico. Resultados: La muestra de 40 pacientes se subdividió en 2 grupos según su requerimiento de marcapasos posterior a la ablación (P y NP). Se diagnosticó disfunción del nodo sinusal en 13 participantes (32.5%), de los cuales 10 (71.43%) requirieron marcapasos en comparación a 4 (28.57%) con función normal. En el grupo P la pausa del nodo sinusal post-ablación tuvo una media de 1.86 ± 0.96 s versus el grupo NP con 1.196 ± 0.52 s. En relación con el TRNSC, el grupo NP tuvo una media de 383.54 ± 67.96 ms vs. 1972.57 ± 3423.56 ms en el grupo P. 10 pacientes (25%) obtuvieron un TRNSC > 500 ms, de los cuales 100% requirieron marcapasos; de los 14 pacientes que requirieron marcapasos 10 (71.43%) tenían un TRNSC elevado (p ≤ 0.01). Conclusiones: El TRNSC tiene el potencial de ser una herramienta de pronóstico cuantitativo para la necesidad de futuros implantes de marcapasos en pacientes con disfunción del nodo sinusal, resultado de aleteo atrial pos-ablación. Esto podría ayudar a diagnosticar más temprano una disfunción del nodo sinusal, resultando en la reducción de la pérdida a largo plazo de la función cardíaca como efecto de la remodelación.

Según la OPS, los países desarrollados alcanzan un 11% de prevalencia de anemia, mientras que en los países en vías de desarrollo afecta al tercio poblacional, llegando a superar el 50% en el continente africano y asiático. El estudio tuvo como Objetivo: Analizar la efectividad del consumo de brownies de sangrecita sobre los niveles de hemoglobina en niños de la Institución Educativa Inicial Pública, Ica. Material y métodos: Para el desarrollo se utilizó la metodología de tipo cuasi experimental, nivel aplicativo y de enfoque cuantitativo. La población de estudio estuvo conformada por 100 niños, se realizó el descarte de anemia mediante el analizador de hemoglobina (hemo Q), micro cubetas, lancetas y demás implementos médicos. Para analizar los resultados se tuvo en consideración los valores normales emitidos por el Ministerio de Salud(MINSA) y se utilizaron fichas de control. Resultados: Se captó a 4 niños de la IEI Nº14 Mercedes Dibós de Camino al realizar el tamizaje inicial de anemia, quienes tenían un nivel de hemoglobina inferior a los valores normales de acuerdo a la normativa del MINSA; luego de consumir los brownies de sangrecita en un intervalo de 3 veces por semana durante 7 semanas se evidenció un incremento en sus niveles de hemoglobina respecto al tamizaje inicial. Conclusiones: El consumo de brownie de sangrecita es efectivo en el tratamiento de la anemia en los niños, incrementando su nivel de hemoglobina entre 1.9 g/dl a 2.3 g/dl


According to PAHO, developed countries reach an 11% prevalence of anemia, while in developing countries it affects one third of the population, reaching over 50% in the African and Asian continents. The study had the Objective: To analyze the effectiveness of the consumption of sangrecita brownies on hemoglobin levels in children of the Initial Educational Institution Public, Ica. Material and methods: A quasi-experimental methodology with a quantitative approach was used for the development of the study. The study population consisted of 100 children. Anemia was ruled out using the hemoglobin analyzer (hemoQ), micro cuvettes, lancets and other medical implements. To analyze the results, the normal values issued by MINSA were taken into consideration and control cards were used. Results:4 children from the IEI Nº14 Mercedes Dibós de Camino were caught during the initial screening for anemia, who had a hemoglobin level below the normal values according to MINSA regulations; but after consuming the blood brownies at an interval of 3 times per week for 7 weeks, an increase in their hemoglobin levels was evidenced with respect to the initial screening. Conclusion: The consumption of sangrecita brownie is effective in the treatment of anemia in children by increasing their hemoglobin level, the increase value was between 1.9 g/dl and 2.3 g/dl.Key Words:Anemia, sangrecita brownie, hemoglobin, fortification, children.

Objetivo: identificar factores asociados a severidad y mortalidad por COVID-19. Material y métodos: estudio observacional multicéntrico retrospectivo de pacientes ingresados por COVID-19 en centros de una ciudad de la República Dominicana. Se reportaron datos demográficos, clínicos, terapéuticos y de laboratorio para su comparación entre pacientes con enfermedad severa y no severa, fallecidos y dados de alta. Resultados: los factores asociados a mayor riesgo de severidad y mortalidad fueron el sexo masculino, la hipertensión arterial, la cantidad de comorbilidades, el uso de fármacos previos, niveles elevados de marcadores inflamatorios, linfopenia, neutrofilia, entre otros. Mientras que el uso de inhibidores de la enzima convertidora de angiotensina se asoció a menor mortalidad. Conclusiones: la identificación de los factores asociados a progresión severa y mortal por COVID-19 contribuye a la toma temprana de decisiones relacionadas a la hospitalización y a pautas de manejo médico diferencial en quienes conllevan mayor riesgo de desenlaces adversos.


Objective: To identify factors associated with severity and mortality by COVID-19. Material and methods: Retrospective multicenter observational study of patients admitted with COVID-19 in health centers in a city of the Dominican Republic. Demographic, clinical, therapeutic and laboratory data were reported for comparison between patients with severe and non-severe disease, deceased and discharged. Results: Factors significantly associated with higher risk of severity and mortality were male sex, hypertension, number of comorbidities, drugs' used previously, high levels of inflammatory markers, lymphopenia, neutrophilia, among others. While the use of angiotensin converting enzyme inhibitors was associated with lower mortality. Conclusions: Identification of factors associated with severe and fatal progression of COVID-19 contributes to early clinical decision-making related to hospitalization and differential medical management in those at increased risk of adverse outcomes.

ABSTRACT: The rising prevalence of opioid use disorder and injection drug use has resulted in an increasing incidence of chronic hepatitis C virus (HCV) infection. Although older adults historically have represented the bulk of HCV infections in the United States, demographics have shifted and most new infections are presenting in younger patients. As a result, screening guidelines for HCV have evolved, moving toward a near-universal screening paradigm. Rates of screening and linkage to care remain low, attributed to the fact that underserved populations are disproportionately affected and often have limited access to specialty care. Collaborative models to treat HCV using primary care providers have been proposed to facilitate linkage to care and reduce transmission.

PURPOSE: Aurora Kinase A (AKA) inhibition with gemcitabine represents a potentially synergistic cancer treatment strategy via mitotic catastrophe. The feasibility, safety, and preliminary efficacy of alisertib (MLN8237), an oral AKA inhibitor, with gemcitabine was evaluated in this open-label phase I trial with dose escalation and expansion. METHODS: Key inclusion criteria included advanced solid tumor with any number of prior chemotherapy regimens in the dose escalation phase, and advanced pancreatic adenocarcinoma with up to two prior chemotherapy regimens. Four dose levels (DLs 1-4) of alisertib (20, 30, 40, or 50 mg) were evaluated in 3 + 3 design with gemcitabine 1000 mg/m2 on days 1, 8, and 15 in 28-day cycles. RESULTS: In total, 21 subjects were treated in dose escalation and 5 subjects were treated in dose expansion at DL4. Dose-limiting toxicities were observed in 1 of 6 subjects each in DL3 and DL4. All subjects experienced treatment-related adverse events. Grade ≥ 3 treatment-related adverse events were observed in 73% of subjects, with neutropenia observed in 54%. Out of 22 subjects evaluable for response, 2 subjects (9%) had partial response and 14 subjects (64%) had stable disease. Median PFS was 4.1 months (95% CI 2.1-4.5). No significant changes in pharmacokinetic parameters for gemcitabine or its metabolite dFdU were observed with alisertib co-administration. CONCLUSIONS: This trial established the recommended phase 2 dose of alisertib 50 mg to be combined with gemcitabine. Gemcitabine and alisertib are a feasible strategy with potential for disease control in multiple heavily pre-treated tumors, though gastrointestinal and hematologic toxicity was apparent.

Introducción: durante la actual situación de confinamiento surgida por el COVID-19, la población ha estado expuesta a estados emocionales donde su calidad de sueño se ha visto afectada. Por esta razón, esta investigación se ha propuesto determinar el impacto del aislamiento social por el COVID-19 en la calidad del sueño en adultos de 18 a 60 años en la República Dominicana. Materiales y métodos: este estudio observacional de corte transversal fue realizado en la República Dominicana en el 2020, específicamente en adultos de 18 a 60 años. La encuesta fue esparcida virtualmente vía Google Forms. Las variables utilizadas fueron categóricas (sexo, estado civil, zona de residencia, nivel académico, consumo de sustancias) y continuas (edad, calidad de sueño, nivel de estrés). De igual forma, se utilizaron encuestas aprobadas internacionalmente, como el Índice de Calidad de Sueño de Pittsburgh. Resultados: la población del estudio consistió en 821 participantes (edad media 30.94 años, 74.3 % mujeres). La prevalencia de una mala calidad del sueño fue de 80.7 %. La puntuación media del PSQI fue de 8.56 ± 4.20. Por otro lado, se destacó la relación del estrés con una mala calidad de sueño, obteniendo una p=0.00. Los factores independientes relacionados con la mala calidad del sueño incluyeron la edad y el sexo femenino. Conclusiones: el presente estudio muestra una visión general de las diferentes causas relacionadas a alteraciones en la calidad de sueño. Cabe destacar, la exposición a altos niveles de estrés durante el aislamiento social fruto de la pandemia causada por el COVID-19, asociado con una pobre calidad de sueño. Además, el sexo femenino mostró mayor tendencia a presentar trastornos de sueño, por igual los consumidores de alcohol.

Epithelioid hemangioendothelioma (EHE) is a rare vascular tumor of intermediate malignancy, often with indolent behavior. Though most cases have a characteristic WWTR1-CAMTA1 gene fusion, a subtype of EHE with YAP1-TFE3 fusions and a distinct morphology has recently been described histologically, but no cases of YAP1-TFE3 EHE have been described in the cytology literature. We herein report on a case of YAP1-TFE3 fusion associated EHE diagnosed on fine-needle aspiration and core biopsy of a liver mass in an 18-year-old male patient who presented with synchronous lung and liver involvement. We also discuss the differential diagnosis of EHE on cytology specimens.

RESUMO

Stent embolization is a rare complication of percutaneous coronary intervention. It is encountered when a stent unintentionally dislodges from the balloon. Its consequences vary from bypass surgery to death if not treated adequately. To promote a successful outcome, the "twisted wire technique" is a rapid and effective method to recover the lost stent. Its practice can attenuate the non-wanted consequences. .

BACKGROUND: Although specific interventions previously demonstrated benefit in patients with ARDS, use of these interventions is inconsistent, and patient mortality remains high. The impact of variability in center management practices on ARDS mortality rates remains unknown. RESEARCH QUESTION: What is the impact of treatment variability on mortality in patients with moderate to severe ARDS in the United States? STUDY DESIGN AND METHODS: We conducted a multicenter, observational cohort study of mechanically ventilated adults with ARDS and Pao2 to Fio2 ratio of ≤ 150 with positive end-expiratory pressure of ≥ 5 cm H2O, who were admitted to 29 US centers between October 1, 2016, and April 30, 2017. The primary outcome was 28-day in-hospital mortality. Center variation in ventilator management, adjunctive therapy use, and mortality also were assessed. RESULTS: A total of 2,466 patients were enrolled. Median baseline Pao2 to Fio2 ratio was 105 (interquartile range, 78.0-129.0). In-hospital 28-day mortality was 40.7%. Initial adherence to lung protective ventilation (LPV; tidal volume, ≤ 6.5 mL/kg predicted body weight; plateau pressure, or when unavailable, peak inspiratory pressure, ≤ 30 mm H2O) was 31.4% and varied between centers (0%-65%), as did rates of adjunctive therapy use (27.1%-96.4%), methods used (neuromuscular blockade, prone positioning, systemic steroids, pulmonary vasodilators, and extracorporeal support), and mortality (16.7%-73.3%). Center standardized mortality ratios (SMRs), calculated using baseline patient-level characteristics to derive expected mortality rate, ranged from 0.33 to 1.98. Of the treatment-level factors explored, only center adherence to early LPV was correlated with SMR. INTERPRETATION: Substantial center-to-center variability exists in ARDS management, suggesting that further opportunities for improving ARDS outcomes exist. Early adherence to LPV was associated with lower center mortality and may be a surrogate for overall quality of care processes. Introducción: el síndrome coronario agudo (SCA) es una serie de signos y síntomas que se refieren a isquemia miocárdica repentina. Estas condiciones clínicas son Síndrome Coronario agudo sin Elevación del Segmento ST (SCASEST) y Síndrome Coronario Agudo con Elevación del Segmento ST (SCACEST). A nivel mundial hay hallazgos controversiales con respecto a la diferencia de presentación de SCA en hombres y mujeres. Objetivo: determinar las diferencias en presentación del síndrome coronario agudo según el género en República Dominicana. Métodos: en este estudio observacional retrospectivo unicéntrico, 3,548 pacientes con SCA llevados al laboratorio de cateterismo fueron observados durante el período de agosto de 2016 a septiembre de 2019. Resultados: la edad promedio de presentación fue mayor en las mujeres, con una media de 63 años (DE ± 12.2). El SCACEST fue más frecuente en hombres (69.35 %) y la Angina Inestable en mujeres (50.14 %, p <0.0001). Angiográficamente, la enfermedad coronaria multivascular fue más común en hombres y la angiografía coronaria normal en mujeres (41.30 % y 35.37 %, p < 0.0001). Conclusión: dentro de la población dominicana, la mujer tiende a ser mayor al momento de presentación de SCA, con menor tendencia a someterse a intervención coronaria terapéutica.


Introduction: Acute Coronary Syndrome (ACS) is a series of signs and symptoms referring to sudden myocardial ischemia. These clinical conditions are: Non-ST-Acute Coronary Syndromes (NST-ACS) and ST-segment Elevation Myocardial Infarction (STEMI). Globally there are controversial findings regarding the difference in SCA presentation in both men and women. Objective: Determine differences in presentation of acute coronary syndrome by gender in the Dominican Republic. Methods: In this retrospective single-centered observational study 3,548 patients with ACS taken to the catheterization laboratory underwent observation, during the time frame of August 2016 to September 2019. Results: The average age of presentation was higher in females, with a mean of 63 years old (SD ± 12.2). STEMI was greater in males (69.35%) and females presented more with UA (50.14%, p<0.0001). Angiography findings showed that multi-vascular coronary artery disease was more common in males and normal coronary angiography more frequent in females (41.30% and 35.37%, p<0.0001). Conclusion: Within the Dominican population, females tend to be older at the time of appearance of ACS with a lower tendency of undergoing coronary intervention.

Introducción: la diabetes tipo 1 es una enfermedad crónica de alto impacto económico con gran capacidad de ser controlada, la misma no tiene ninguna descripción local previa. Su principal causa de mortalidad es los eventos cardiovasculares y el manejo adecuado la disminuye considerablemente. Objetivo: determinar el riesgo cardiovascular en pacientes adultos con diabetes tipo 1 en la ciudad de Santiago de los Caballeros, República Dominicana. Método: se realizó un estudio descriptivo transversal multicéntrico con 39 pacientes en el período de junio a noviembre de 2019. La calculadora "Steno T1 Risk Engine" se utilizó para estimar el riesgo cardiovascular. Resultados: se obtuvo una relación significativa entre la albuminuria (p = 0.0127), presión arterial sistólica (p = 0.0002), tiempo de diagnóstico (p = 0.0037) y nivel de riesgo cardiovascular. La hemoglobina glucosilada (p = 0,7884) y la actividad física (p = 0.706) no mostraron una relación significativa con el riesgo cardiovascular. Conclusión: el nivel de riesgo cardiovascular promedio es bajo, con probabilidades <10 % de un evento cardiovascular agudo dentro de los 10 años. Esta herramienta permite incluir una evaluación cardiovascular rutinaria con datos que perfilen el tratamiento orientado a disminuir complicaciones vasculares, mortalidad y aumentar adherencia al tratamiento.


Introduction: Type 1 diabetes is a chronic condition with a high economic impact but potentially controllable. There is no previous local description of this condition. Its main fatality cause is due to cardiovascular events and its proper management can diminish it. Objective: This study aimed to determine the cardiovascular risk in adult patients with type 1 diabetes mellitus in the city of Santiago de los Caballeros, Dominican Republic. Method: A descriptive multicenter cross-sectional study was done on 39 patients in the period of June-November 2019. "Steno T1 Risk Engine" calculator estimated the cardiovascular risk. Results: A significant relationship was obtained between albuminuria (p = 0.0127), systolic blood pressure (p = 0.0002), diagnosis time (p = 0.0037) and cardiovascular risk level. Glycated hemoglobin (p = 0.7884) and physical activity (p = 0.7063) did not show a significant relationship with cardiovascular risk. Conclusion: Average cardiovascular risk level is low with <10% probabilities of an acute cardiovascular event within 10 years. This tool could lead to quick cardiovascular risk evaluations to guide the treatment lowering vascular complications, mortality and increasing treatment adherence.

Histiocyte-rich rhabdomyoblastic tumor is a recently described skeletal muscle neoplasm of uncertain malignant potential, characterized by slow growth, a fibrous capsule containing peripheral lymphoid aggregates, spindle-to-epithelioid cells with a rhabdomyoblastic immunophenotype, and a dense histiocytic infiltrate. It most commonly arises within the muscles of the lower legs and trunk in young-to-middle-aged men, and initial reports suggest indolent behavior. In this paper, we present two additional cases of histiocyte-rich rhabdomyoblastic tumor with similar clinicopathologic features and discuss the differential diagnosis including its overlap with inflammatory leiomyosarcoma.

We present a case detailing a 70-year-old female with a history of triple-negative breast carcinoma (TNBC) of the left breast and contralateral stage pT2a nodular melanoma of the right upper arm who underwent sentinel lymph node biopsy of the right axilla demonstrating a metastatic epithelioid tumor that was strongly positive for S-100 protein and SOX10. The tumor cells were negative for HMB-45 and Melan-A and positive for CK7 and other breast markers (GCDFP15, mammaglobin, and GATA3). While concerning for metastatic melanoma based on clinical history and initial immunohistochemistry, tumor morphology and subsequent immunohistochemistry was supportive of metastatic breast adenocarcinoma. This case demonstrates a rare but perilous diagnostic pitfall of triple-negative breast carcinomas that strongly and diffusely express S-100 protein and SOX10 mimicking melanoma.

RESUMEN Objetivo Identificar el fenómeno procoagulante en pacientes SARS-CoV- 2 y proponer orientación terapéutica sostenible para países de bajos ingresos. Método Se realizó una revisión sistemática que identificó cinco estudios observacionales de un escrutinio a partir de 78 resultados. Se examinaron 712 pacientes y los resultados fueron agrupados según mortalidad y severidad. La comparación de los grupos se interpretó mediante estadística descriptiva. Resultado Los valores del dímero D se asociaron significativamente en todas las observaciones a mayor severidad y mortalidad. La protrombina se asoció, en algunas observaciones, a mayor mortalidad; en cuanto a severidad, los resultados fueron inconclusos. Conclusión El COVID-19 tiene importante actividad procoagulante y su tratamiento oportuno puede alterar el pronóstico. La evidencia explorada avala métodos sostenibles. Se necesita más evidencia para mejorar el manejo. Se recomienda un abordaje sistemático temprano de los pacientes con medidas terapéuticas sostenibles a la medida del sistema de salud.


ABSTRACT Objective To identify the procoagulant phenomenon in SARS-CoV-2 patients and propose sustainable therapeutic guidance for low-income countries. Methods A systematic review was conducted. It identified 5 observational studies from a scrutiny from 78 results. 712 patients were examined and the results were grouped according to mortality and severity. The comparison of the groups was interpreted using descriptive statistics. Results D-dimer values were significantly associated with greater severity and mortality. Prothrombin was associated in some observations with higher mortality, but in terms of severity it was inconclusive. Conclusion COVID-19 disease has significant procoagulant activity and its timely treatment can alter the prognosis. The explored evidence supports sustainable methods. More evidence is needed to improve management. An early systematic approach to patients with sustainable therapeutic measures tailored to the health system is recommended.

We report 21 cases of a distinctive and unique vascular tumor which we propose to be a pure lymphatic-type angiosarcoma characterized by architectural and growth characteristics of angiosarcoma, cytologic, and immunohistochemical features of lymphatic differentiation, a prominent lymphocytic infiltrate, and variable nuclear grade. Patients included 12 males and 9 females with a median age of 65 years (range: 32 to 95 y). Tumors involved the head and neck (n=11), lower extremities (n=5), trunk (n=4), and upper extremity (n=1) and were located superficially in the dermis and/or subcutis. Tumors were designated "low grade" (n=10) when the nuclear grade was low, and vascular channel formation was evident throughout but with multilayering of endothelium within the vessels. Cases were designated "high grade" (n=11) when nuclei appeared higher grade with more rounded contours and prominent nucleoli and when solid areas predominated over vascular channel formation. A striking feature of both groups was the presence of a dense, lymphocytic infiltrate with occasional germinal center formation. All cases strongly and diffusely expressed at least 1 lymphatic marker (21/21) with podoplanin (17/19) and Prox-1 (11/11) more commonly expressed than LYVE-1 (5/10). No consistent molecular alteration was identified. Follow-up on 17 patients (median: 41 mo, mean: 54 mo) showed 10 patients were alive without disease, 5 were alive with disease, 1 died of other cause, and 1 died of disease. Local recurrence developed in 9 cases and metastasis in 2 cases, although neither correlated with grade as defined. On the basis of clinical follow-up to date, the natural history of lymphatic-type angiosarcoma appears to be more favorable than other forms of cutaneous angiosarcoma.

Primary cardiac tumors are rare, and most are benign. Intimal sarcomas are among the rarest of the malignant cardiac tumors; they are aggressive and associated with a poor prognosis. Whereas transesophageal echocardiography has been instrumental in evaluating cardiac masses, other imaging methods, such as cardiac magnetic resonance, have proved invaluable in accurately characterizing these masses. We present the case of a 49-year-old woman in whom we diagnosed a primary intimal sarcoma of the left atrial appendage, and we discuss the importance of multimodal imaging in the evaluation and diagnosis of cardiac masses.

Recent work has revealed SMARCB1/INI1 loss by immunohistochemistry in a subset of epithelioid schwannomas and explored the significance of cytologic atypia and increased mitotic activity in these tumors. Additional studies have evaluated the utility and limitations of histone H3K27 trimethylation in diagnosis of high-grade and low-grade malignant peripheral nerve sheath tumors. New terminology regarding nerve sheath tumors in neurofibromatosis type 1 patients was proposed during a 2016 conference to better define guidelines for classification of this group of tumors. This review highlights novel findings and practical applications relating to these topics in peripheral nerve sheath tumors.
