INTRODUCTION: One of the major concerns with post-acute sequelae of COVID-19 (PASC) is the development of pulmonary fibrosis, for which no approved pharmacological treatment exists. Therefore, the primary aim of this open-label study was to evaluate the safety and the potential clinical efficacy of a prolonged-release pirfenidone formulation (PR-PFD) in patients having PASC-pulmonary fibrosis. METHODS: Patients with PASC-pulmonary fibrosis received PR-PFD 1800 mg/day (1200 mg in the morning after breakfast and 600 mg in the evening after dinner) for three months. Blood samples were taken to confirm the pharmacokinetics of PR-PFD, and adverse events (AEs) were evaluated monthly using a short questionnaire. Symptoms, dyspnea, and pulmonary function tests (spirometry, diffusing capacity for carbon monoxide, plethysmography, and 6-min walk test [6MWT]) were evaluated at baseline, and one and three months after having started the PR-PFD treatment. RESULTS: Seventy subjects with mild to moderate lung restriction were included. The most common AEs were diarrhea (23%), heartburn (23%), and headache (16%), for which no modifications in the drug study were needed. Two patients died within the first 30 days of enrolment, and three opted not to continue the study, events which were not associate with PR-PFD. Pulmonary function testing, 6MWT, dyspnea, symptoms, and CT scan significantly improved after three months of treatment with PR-PFD. CONCLUSION: In patients with PASC pulmonary fibrosis, three months' treatment with PR-PFD was safe and showed therapeutic efficacy. Still, it remains to be seen whether the pulmonary fibrotic process remains stable, becomes progressive or will improve.

BACKGROUND AND OBJECTIVE: Primary cutaneous lymphomas (PCL) are uncommon. Observations based on the first year of data from the Spanish Registry of Primary Cutaneous Lymphomas (RELCP, in its Spanish abbreviation) of the Spanish Academy of Dermatology and Venereology (AEDV) were published in February 2018. This report covers RELCP data for the first 5 years. PATIENTS AND METHODS: RELCP data were collected prospectively and included diagnosis, treatments, tests, and the current status of patients. We compiled descriptive statistics of the data registered during the first 5 years. RESULTS: Information on 2020 patients treated at 33 Spanish hospitals had been included in the RELCP by December 2021. Fifty-nine percent of the patients were men; the mean age was 62.2 years. The lymphomas were grouped into 4 large diagnostic categories: mycosis fungoides/Sézary syndrome, 1112 patients (55%); primary B-cell cutaneous lymphoma, 547 patients (27.1%); primary CD30+lymphoproliferative disorders, 222 patients (11%), and other T-cell lymphomas, 116 patients (5.8%). Nearly 75% of the tumors were registered in stage I. After treatment, 43.5% achieved complete remission and 27% were stable at the time of writing. Treatments prescribed were topical corticosteroids (1369 [67.8%]), phototherapy (890 patients [44.1%]), surgery (412 patients [20.4%]), and radiotherapy (384 patients [19%]). CONCLUSION: The characteristics of cutaneous lymphomas in Spain are similar to those reported for other series. The large size of the RELCP registry at 5 years has allowed us to give more precise descriptive statistics than in the first year. This registry facilitates the clinical research of the AEDV's lymphoma interest group, which has already published articles based on the RELCP data.

El envenenamiento debido a picadora de escorpiones es reportado en diferentes países de Suramérica, en algunos, como México, es un problema de salud pública. En Colombia el accidente por escorpiones no es de reporte obligatorio, por lo que hay subregistro de casos. Los accidentes por este tipo de animales ponzoñosos se asocian a manifestaciones locales, y en casos de gravedad moderada a severa, a problemas cardiovasculares y neurológicos. Sin embargo, es importante reconocer manifestaciones que, aunque reportadas con menos frecuencia, implican gran morbilidad, como la pancreatitis y las alteraciones renales, entre las que está la nefritis. Se reporta el caso de un varón de 46 años, que presentó un accidente por Tytus, con nefritis intersticial. Este es el primer reporte de esta complicación en Colombia. Nuestro objetivo principal es resaltar la importancia de una pronta identificación de casos y un oportuno inicio del tratamiento por parte del personal de la salud, que permitan disminuir la morbilidad y mortalidad de estos pacientes.


Scorpion poisoning is reported in different countries in South America, especially in Mexico, where it has become a public health issue. In Colombia this type of accident is not mandatory to report, therefore, there is underreporting of cases. Accidents caused by these poisonous animals are mainly associated with local manifestations and, in cases of moderate to severe severity, to cardiovascular and neurological problems. Nevertheless, it is important to recognize other manifestations which, despite being reported less frequently, are associated with high morbidity such as pancreatitis and renal alterations, among which is nephritis. The case of a 46-year-old male patient is reported, who had an accident due to Tytus and presented interstitial nephritis. This is the first report of this complication in Colombia, and based on this, the main objective is to highlight the importance of prompt identification and treatment by health personnel, in order to reduce morbidity and mortality in these patients.

Na Colômbiao acidente por escorpiões não é de reporte obrigatório, porque há sub-registro de casos. Os acidentes por este tipo de animais peçonhentos se associam a manifestações locais, e em casos de gravidade moderada a severa, a problemas cardiovasculares e neurológicos. Entretanto, é importante reconhecer manifestações que, embora reportadas com menos frequência, implicam grande morbilidade, como a pancreatite e as alterações renais, entre as que está a nefrite. Se reporta o caso de um homem de 46 anos, que apresentou um acidente por Tytus, com nefrite intersticial. Este é o primeiro reporte desta complicação na Colômbia. Nosso objetivo principal é ressaltar a importância de uma breve identificação de casos e um oportuno início do tratamento por parte do pessoal da saúde, que permitam diminuir a morbilidade e mortalidade destes pacientes.

El presente trabajo es una recopilación de información de bases de datos Medline, PubMed, ScienceDirect, Google Scholar y SciELO. El objetivo fue recopilar recomendaciones de bioseguridad planteadas a nivel nacional e internacional, resaltando el uso del consentimiento informado en la práctica odontológica en el contexto de la pandemia por COVID-19 y realizar un análisis de los aspectos éticos que involucra la práctica profesional. Se incluyeron artículos de revisión sistemática, manuales, entrevistas, editoriales, páginas web y literatura gris, se discriminó la evidencia y refrendó la información con artículos publicados en revistas indexadas, relacionando los aspectos éticos y deontológicos de la práctica odontológica y criterios de bioseguridad. Se concluye que es necesario el uso del consentimiento informado, por el alto riesgo de infección cruzada, así como el refuerzo de varios aspectos de bioseguridad como el uso adecuado del equipo de protección personal (EPP), en beneficio de los pacientes y del personal.


This review compiles information from Medline, PubMed, ScienceDirect, Google Scholar, SciELO databases. Its objective is to provide biosecurity recommendations, high- lighting the use of informed consent in dental practice in the context of the COVID-19 pandemic, according to current Peruvian regulations. A systematic review of articles, manuals, interviews, editorials, web pages, grey literature included, evidence discredited and information endorsed with articles published in indexed journals, relating ethical and deontological aspects of dental practice in biosafety criteria. In conclusion, the use of informed consent is important because there is a high risk of cross-infection as the reinforcement of several aspects of biosafety like the appropriate use of personal protective equipment (PPE), for the benefit of patients and staff.

Objetivo: Caracterizar la atención de accidentes de tránsito en la ciudad de Medellín para obtener una imagen general de la situación de salud en este contexto. Metodología: Estudio descriptivo observacional, con intención analítica, de fuentes secundarias de información, en pacientes con edad mayor o igual a 16 años, atendidos por el personal 123, módulo salud, víctimas de accidentes de tránsito en Medellín. Resultados: Un total de 3 829 individuos sufrieron accidente de tránsito entre octubre y diciembre del 2018 según los criterios del estudio. De estos, el 63.1% de sexo masculino, con edad media de 34 años. La zona con mayor demanda de atención prehospitalaria fue la comuna 10 de la ciudad (La Candelaria) y la mayoría de los afectados por los accidentes lo fueron en calidad de conductor, 64%, y en un 75% de los casos, el vehículo involucrado fue una motocicleta. Solo el 26.5% de los pacientes fueron dados de alta en la escena. Las extremidades fueron las zonas corporales de mayor compromiso, en un 78.2% de los episodios, seguidas por el cráneo y la cara, con un 26.8%. Conclusiones: Es evidente la necesidad de la ciudad de implementar y evaluar el impacto de estrategias dinámicas que permitan un manejo eficiente de los recursos, que priorice las zonas y grupos demográficos de mayor riesgo y la articulación de la disposición de pacientes dentro de la cadena de atención, según los tipos más frecuentes de lesiones.


Objective: To characterize the traffic accident attention in the city of Medellín in order to obtain a general image of the health situation in this context. Methodology: Observational descriptive study, with analytic intent, of secondary information sources in patients with age higher or equal to 16 years of age, served by the 123 personnel, health module, who were victims of traffic accidents in Medellín. Results: A total of 3 829 individuals suffered traffic accidents between October and December of 2018 according to the study criteria. Of these, 63.1% were males, with an average age of 34 years old. The zone with the highest demand of pre-hospital attention was the Comuna 10 of the city (known as La Candelaria) and most of the affected by the accidents were as drivers, 64%, and 75% of the cases, the vehicle involved was a motorcycle. Only 26.5% of the patients were discharged on the scene. The extremities were the body zones with the highest compromise in 78.2% of the episodes, followed by the cranium and the face, with 26.8%. Conclusions: It is evident that the city needs to implement and evaluate the impact of dynamic strategies that enable an efficient management of the resources that prioritizes the zones and demographic groups with the highest risk and the articulation of the disposition of patients within the attention chain, according to the most frequent types of injuries.

BACKGROUND AND OBJECTIVE: Primary cutaneous lymphomas are uncommon. This article describes the Primary Cutaneous Lymphoma Registry of the Spanish Academy of Dermatology and Venereology (AEDV) and reports on the results from the first year. PATIENTS AND METHODS: Disease registry for patients with primary cutaneous lymphoma. The participating hospitals prospectively recorded data on diagnosis, treatment, tests, and disease stage for all patients with primary cutaneous lymphoma. A descriptive analysis was performed. RESULTS: In December 2017, the registry contained data on 639 patients (60% male) from 16 university hospitals. The most common diagnoses, in order of frequency, were mycosis fungoides/Sézary syndrome (MF/SS) (348 cases, 55%), primary cutaneous B-cell lymphoma (CBCL) (184 cases, 29%), primary cutaneous CD30+ T-cell lymphoproliferative disorder (CD30+ CLPD) (70 cases, 11%), and other types of T-cell lymphoma (37 cases, 5%). In total, 105 (16.5%) of the cases recorded were incident cases. The most common diagnosis in the MF/SS group was classic MF (77.3%). Half of the patients with MF had stage IA disease when diagnosed, and the majority were either in partial remission (32.5%) or had stable disease (33.1%). The most widely used treatments were topical corticosteroids (90.8%) and phototherapy. The most common form of primary CBCL was marginal zone lymphoma (50%). Almost all of the patients had cutaneous involvement only and nearly half had stage T1a disease. Most (76.1%) were in complete remission. The main treatments were surgery (55.4%) and radiotherapy (41.9%). The most common diagnosis in patients with CD30+ CLPD was lymphomatoid papulosis (68.8%). Most of the patients (31.4%) had stage T3b disease and half were in complete remission. The most common treatments were topical corticosteroids (68.8%) and systemic chemotherapy (32.9%). CONCLUSION: The characteristics of patients with primary cutaneous lymphoma in Spain do not differ from those described in other series in the literature. The registry will facilitate clinical research by the AEDV's lymphoma group.

PURPOSE OF REVIEW: The aim of this review is to identify the key contextual and methodological differences in health impact assessments (HIA) of ambient air pollution performed for Europe. We limited our review to multi-country reviews. An additional aim is to quantify some of these differences by applying them in a HIA template in three European cities. RECENT FINDINGS: Several HIAs of ambient air pollution have been performed for Europe, and their key results have been largely disseminated. Different studies have, however, come up with substantial differences in attributed health effects. It is of importance to review the background contributing to these differences and to quantify their importance for decision makers who will use them. We identified several methodological differences that could explain the discrepancy behind the number of attributable deaths or years of life lost. The main differences are due to the exposure-response functions chosen, the ways of assessing air pollution levels, the air pollution scenarios and the study population. In the quantification part, we found that using risk estimates from the European Study of Cohorts for Air Pollution Effects (ESCAPE) instead of the American Cancer Society (ACS) study could nearly double the attributable burden of ambient air pollution. This study provides some insights into the differential results in previously published HIAs on air pollution in Europe. These results are important for stakeholders in order to make informed decisions.

BACKGROUND: Guidelines recommend treating actinic keratoses (AKs) as they are recognized as precursors of invasive squamous cell carcinoma. OBJECTIVE: The objective of this study was to collect real-world clinical data on the use of methyl aminolevulinate daylight photodynamic therapy (MAL DL-PDT) for the treatment of face and scalp AK in Europe. METHODS: A prospective, multicenter, non-interventional study was conducted in six European countries in patients receiving a single treatment of MAL DL-PDT for face and/or scalp AK. Patient-reported outcomes were assessed by patient questionnaires at baseline and at 3 months after treatment, efficacy was assessed at 3 months using a 6-point global improvement scale, and adverse events (AE) were recorded at each visit. RESULTS: Overall, 325 patients were enrolled from 52 investigational centres, 314 of whom attended the 3-month visit. Most patients had multiple lesions (58.4% had >10 lesions) with lesions mainly located on the scalp (60.0%) and/or forehead (54.2%). AKs were predominantly grade I (39.4%) or grade II (33.2%), and 10.5% of patients had grade III lesions. The proportions of patients and physicians that were overall satisfied to very satisfied with the MAL DL-PDT treatment were 80.4% and 90.3%, respectively. The vast majority of patients (90.0%) would consider using MAL DL-PDT again if needed. Physician-assessed efficacy at 3 months was at least much improved in 83.5% of patients, with 45.9% of patients requiring no retreatment. Related AEs were reported in 15% of patients. CONCLUSION: Use of MAL DL-PDT for multiple face and/or scalp AKs resulted in high levels of patient and physician satisfaction in clinical practice in Europe, reflecting the good efficacy and high tolerability of this convenient procedure.

OBJECTIVE: To assess quality of life of children and teenagers with recurrent respiratory papillomatosis, according to the evidence of infection by human papillomavirus types 6 and 11, compared with healthy volunteers and patients with chronic otitis media. METHOD: Participants and their parents completed the Pediatric Quality of Life Inventory 4.0. RESULTS: Patients with recurrent respiratory papillomatosis and their parents reported lower quality of life than healthy subjects (p < 0.01), but similar quality of life to patients with chronic otitis media. Those with human papillomavirus type 11 showed the lowest scores among all participants (p < 0.05). CONCLUSION: Young Mexican patients with recurrent respiratory papillomatosis and their parents perceive a poor quality of life, and they may experience limitations in interactions with their peers. Infection by human papillomavirus type 11 may increase the impact of the disease on quality of life.

INTRODUCCIÓN: El estatus epiléptico superrefractario se define como convulsiones de 24 horas o más posterior al uso de anestésicos generales. Se presenta el siguiente caso con el fin de discutir el uso de anticonvulsivantes y la importancia del manejo multidisciplinario. PRESENTACIÓN DEL CASO: Escolar masculino de 8 años con antecedentes mórbidos de epilepsiades de los 2 meses en tratamiento con ácido valproico, lamotrigina y clobazam por recurrencia de crisis hipotónicas. Se hospitaliza por crisis atónicas frecuentes y compromiso de conciencia, se inicia levetiracetam y se retira lamotrigina. Electroencefalograma (EEG) muestra actividad epileptiforme muy frecuente sin variación ingresándose a unidad cuidados intensivos para administración de metilprednisolona por 5 días y manejo del estatus epiléptico superrefractario con midazolam en infusión continúa. Nuevo EEG severamente patológico compatible con status epiléptico eléctrico generalizado por lo que se induce coma barbitúrico con diferentes esquemas de tiopental y ketamina con persistencia del patrón de estallido supresión. Tras lo cual se modifica esquema a propofol y topiramato manteniendo antiepilépticos de base. A los 2 días de uso, se suspende propofol por mala respuesta, tras lo cual presenta 2 crisis convulsivas iniciándose fenobarbital. Evoluciona deforma favorable, sin crisis epilépticas clínicas, por lo que se decide alta con ácido valproico, levetiracetam, fenobarbital y topiramato. DISCUSIÓN: La tórpida evolución del caso expuesto y la necesidad de múltiples esquemas farmacológicos dejan en evidencia la necesidad de disponer y conocer el modo de uso de un amplio arsenal de fármacos anticonvulsivantes


INTRODUCTION: Super-refractory status epilepticus is defined as a 24 hours or more lasting seizure after the use of anaesthetics. The following case is shown in order to discuss the use of anticonvulsants and the importance of multidisciplinary management. CASE REPORT: 8 year old male with morbid history of epilepsy since 2 months old treated with valproic acid, lamotrigine and clobazam for recurrent hypotonic crisis. Is hospitalized for frequent atonicseizures and impaired consciousness, levetiracetam is initiated and lamotrigine removed. Electroencephalogram (EEG) shows persistent very frequent epileptiform activity. Patient is admitted to the intensive care unit for administration of methylprednisolone for 5 days and management with continuous infusion of midazolam for the super-refractory status epilepticus. New severely abnormal EEG compatible with generalized electrical status epilepticus deciding to induce a barbiturate coma with different schemes of ketamine and thiopental. Because of persistent suppression burst patter whereupon scheme is changed to propofol and topiramte maintaining chronic antiepileptic. After 2 days of use, propofol is suspended for poor response, after which the patient presents 2 seizures beginning the use of phenobarbital. He evolved favourably, without clinical seizures, so it is decided hospital discharge with valproic acid, levetiracetam, phenobarbital and topiramate. Las anomalías útero-vaginales o müllerianas son un grupo de patologías congénitas que se originan por un defecto del desarrollo, fusión o canalización de los conductos de Müller en la etapa embrionaria, excepcionalmente pueden deberse a una alteración adquirida. La mayoría se diagnostica en forma tardía, como resultado de estudios de infertilidad o complicaciones obstétricas tempranas o tardías. La resonancia magnética (RM) es un examen no invasivo, que no utiliza radiación ionizante, con una alta resolución de contraste, que permite la evaluación multiplanar de las malformaciones müllerianas, permitiendo su aproximación diagnóstica y caracterización, complementando otras técnicas de imágenes como la histerosalpingografía y la ecografía ginecológica. El radiólogo debe estar familiarizado con las alteraciones anatómicas de los genitales femeninos y su representación en RM permitiendo un diagnóstico diferencial apropiado, oportuno y en consecuencia, un tratamiento adecuado. Este artículo describe los hallazgos en este grupo de malformaciones según la clasificación de la American Fertility Society, la más utilizada en la literatura actual.


The uterus-vaginal or Müllerian anomalies are a group of congenital diseases that are caused by a developmental defect, merger or channeling of Müllerian ducts in the embryonic stage, exceptionally may be due to an acquired disorder. Most belatedly diagnosed as a result of studies of infertility or early or late obstetric complications. Magnetic resonance imaging (MRI) is a noninvasive test that does not use ionizing radiation, with high contrast resolution, which allows multiplanar evaluation of Müllerian malformations, allowing its diagnostic approach and characterization, complementing other imaging techniques such as hysterosalpingography and gynecological ultrasound. The radiologist should be familiar with the anatomical abnormalities of the female genitalia and its representation in MRI, allowing an appropriate and timely differential diagnosis, and consequently, an appropriate treatment.

This review was designed as a handbook of metabolomic markers of high significance for a wide range of human diseases. This is the first report to collate results from recent studies in a format that allows ready identification of key metabolites by cross-comparisons of results from one disease to another. All the data presented in this work were obtained by previous research carried out exclusively during clinical trials in humans. Also, discussion of the pathophysiological pathways linked to the markers described is provided. The clinical assays focused on non-targeted or targeted metabolomics and metabolite profiling (focused assays which only refer to a limited array of known biomarkers, applying discriminatory and bioinformatic tools to them) as well as predictive modelling based on clinical trials. The data also highlight pathways and biological compounds that are disrupted at early stages of the diseases, in order to help elucidate target compounds and the pathophysiology of the considered diseases for early prognosis and diagnosis using noninvasive samples (saliva, sputum, serum, plasma, blood, urine, tissue, faecal water or faeces). In the tables, the candidate metabolites for biomarkers of diagnosis, or the biomarkers themselves, are detailed, indicating the type of sample in which they were detected and their up- or down-regulation (if calculated). The metabolites derived from each study have been filtered carefully, according to the analytical platform, and biostatistical discriminant analyses developed. Among the pool of data provided, those reaching a level of significance of p=0.05-0.0001, according to the Bonferroni correction, Steel-Dwass t- or Wilcoxon matched pair tests, are shown.

El trauma es la principal causa de muerte en los pacientes pediátricos mayores de 1 año, siendo el trauma abdominal responsable del 10 por ciento de las causas de muerte. Las lesiones de vísceras huecas es inferior al 1 por ciento , sin embargo, su mortalidad es del 20 por ciento en el caso de perforación intestinal. La tomografía computada es el método de elección en la identificación y cuantificación de las lesiones en trauma abdominal, dado su excelente rendimiento para lesiones de vísceras sólidas, con menor sensibilidad en lesiones de vísceras huecas, por lo que en estas últimas es importante una alta sospecha clínica y análisis minucioso de las imágenes por parte de los radiólogos. Se realizó una revisión retrospectiva de los hallazgos en pacientes pediátricos referidos a tomografía computarizada, con historia de trauma abdominal contuso. Los principales hallazgos en tomografía computada sugerentes de lesión de víscera hueca fueron: aire extraluminal, extravasación de medio de contraste, presencia de líquido libre intraperitoneal, coágulo centinela adyacente al asa comprometida y engrosamiento de la pared intestinal.


Trauma is the leading cause of death in pediatric patients older than 1 year, with abdominal trauma accounting for 10% of causes of death. Hollow viscera injuries are less than 1%, however its mortality is 20% in the case of intestinal perforation. Computed tomography is the method of choice for the identification and quantification of abdominal trauma injuries, given its excellent performance for solid viscera injuries, with less sensitivity in hollow visceral injuries, so that in the latter a high clinical suspicion and thorough analysis of the images by radiologists is important. A retrospective review was conducted of the findings in pediatric patients, with a history of blunt abdominal trauma, referred to computed tomography. The main findings on computed tomography suggestive of hollow visceral injury were: extraluminal air, extravasation of contrast medium, the presence of free intraperitoneal fluid, sentinel clot adjacent to the affected loop and thickening of the bowel wall.

Introducción: el melanoma representa el 4% de los tumores malignos de la piel, siendo responsable del 80% de las muertes por cáncer de piel. Su incidencia ha ido en ascenso en las últimas décadas, convirtién dose en un problema de salud pública. El PET/CT (Positron Emission Tomography/ComputedTomography) ha demostrado ser útil en la detección de metástasis, con una sensibilidad de 87% y un VPP (valor predictivo positivo) de 90%. El PET/CT puede mostrar resultados inesperados hasta en un 15% de los casos, incluyendo la detección de metástasis,hallazgo que puede modificar el tratamiento. Objetivos: El objetivo deeste estudio es la descripción del compromiso secundario y eventuales asociaciones con la localización del tumor y su profundidad. Metodología: Estudio descriptivo retrospectivo de los PET/CT realizados entremarzo del 2011 y mayo del 2013. Se seleccionaron los casos de melanoma cutáneo, considerando variables demográficas, características del melanoma y metástasis. Resultados: De un total de 1510 PET/CT realizados, 105 exámenes (67 pacientes), correspondieron a casos demelanoma (6,9%). De ellos, el 63% correspondían a hombres. El promedio de edad del grupo fue de 54,8 años (DS 9,9 años)...


Introduction: melanoma represent 4 percent of malignant skin tumors, accounting for 80 percent of deaths from skin cancer. Its incidence has been increasing in recent decades, posing a public health problem. The PET / CT (Positron Emission Tomography/ Computed Tomography) has proven useful in detecting metastases, with a sensitivity of 87percent and a PPV ( positive predictive value) of 90 percent, and may show unexpected results in up to 15 percent of cases that may modify treatment. Objectives: Describe metastases distribution and association with tumour location and depth. Methodology: Retrospective descriptive study of PET / CT performed between March 2011 and May 2013. Cases of cutaneous melanoma were identified, demographic variables, and characteristics of melanoma metastases were extracted from electronic health reports. Results: From a total of 1510 PET / CT performed, 105 exams (67patients) were related to melanoma cases (6.9 percent) . 63 percent of these were male subjects. Control and triathlete volunteers (n=8 and n=15, respectively) were given 400 mL and 200 mL of aronia-citrus juice (AC-juice), respectively. The 24h urine samples were hydrolysed to determine the flavanones concentration by UPLC-QqQ-MS/MS. The flavanones metabolites in both groups of volunteers were glucuronides, sulfates, and sulfo-glucuronides, and the total excretion of flavanones increased fivefold in the triathletes compared with the control volunteers. The increase of ninefold in the homoeriodictyol of triathletes compared to control volunteers may suggest the overactivation of the microbiota metabolism caused by physical exercise. No differences concerning the bioavailability were detected between men and women in controlboth groups. The AC-juice could provide synergistic effects on health due to the increase in the bioavailability of flavanones, avoiding the deleterious effects caused by the overdosage of nutritional supplements.

Several treatments for skin whitening are available today, but few of them are completely adequate, especially owing to the carcinogenic potential attributed to classical drugs like hydroquinone, arbutin and kojic acid. To provide an alternative and safer technology for whitening, we developed two botanical compounds originated from Brazilian biodiversity, an extract of Schinus terebinthifolius Raddi and a linoleic acid fraction isolated from Passiflora edulis oil. The whitening effect of these compounds was assessed using biochemical assays and in vitro models including cellular assays and equivalent skin. The results showed that S. terebinthifolius Raddi extract is able to reduce the tyrosinase activity in vitro, and the combination of this extract with linoleic acid is able to decrease the level of melanin produced by B16 cells cultured with melanocyte-stimulating hormone. Furthermore, melanin was also reduced in human reconstituted epidermis (containing melanocytes) treated with the compounds. The combination of the compounds may provide a synergistic positive whitening effect rather than their isolated use. Finally, we demonstrated that the performance of these mixed compounds is comparable to classical molecules used for skin whitening, as kojic acid. This new natural mixture could be considered an alternative therapeutic agent for treating hyperpigmentation and an effective component in whitening cosmetics.
