RESUMO

PURPOSE: To evaluate the possibility of implanting a drug-eluting coronary stent into the canaliculus of a human cadaver. METHODS: The punctum and canaliculus of an embalmed human cadaver were identified and dilated using a punctum dilator and Bowman probes. At this stage, the integrity of the lacrimal drainage system was assessed by dacryoendoscopy. A drug-eluting coronary stent, which was collapsed around a balloon at the tip of a catheter, was inserted into the canaliculus. The balloon was inflated to expand and lock the spring-like stent into position. The balloon catheter was then deflated and removed from the canaliculus. Dacryoendoscopy was used once again to assess the position of each stent after implantation. RESULTS: The four canaliculi of one human cadaver were successfully identified, dilated, and intubated using drug-eluting coronary stents. Dacryoendoscopy confirmed that each stent achieved a satisfactory position within the canaliculi. The seamless integration of the stent with the surrounding tissues resulted in a significant dilation of the canaliculi. The procedure was deemed short and simple, with the time required to implant a stent into the canaliculus and asses its position being less than a minute. CONCLUSIONS: Canalicular obstructions can often be a source of therapeutic challenges. Our pilot study shows that a drug-eluting coronary stent can be implanted with precision into the canaliculus of a human cadaver. We propose that at least some canalicular obstructions could be treated using a novel rigid mesh tube similar to drug-eluting coronary stents.

Basal cell carcinoma metastases are rare. We report a case of an 83 year-old male with recurrent cutaneous infiltrative basal cell carcinoma on the right vertex with tumor aggregates of infiltrative basal cell carcinoma in the lymphatic vessels of the superficial dermis strongly suspected on pathology slide evaluation during Mohs micrographic surgery. D2-40 immunohistochemical stain allows the detection of lymphatic invasion by highlighting endothelial cells. Further histologic review and D2-40 stains performed on the paraffin-embedded sections did not reveal an endothelial cell lining in favor of lymphatic invasion. What was identified as lymphatic invasion was the result of a peculiar basal cell carcinoma stroma. Correct identification of basal cell carcinoma stroma as a mimicker of lymphatic invasion would prevent potentially invasive investigation as well as associated costs. This is of great importance as the prognostic and therapeutic implications can be of significant value for the patient.

RESUMO

The bilobed flap is most often used for the reconstruction of small cutaneous defect of the nose. Under certain circumstances, it can also be useful in the reconstruction of much larger defects of the face. Our objective is to report a case of bilobed flap reconstruction in a patient with a large defect of the zygomatic cheek following a Mohs micrographic surgery for lentigo maligna.

RESUMO

Oral mucoceles are lesions arising mainly from the minor salivary glands and are thought to occur after excretory duct trauma. We report a case of multiple superficial oral mucoceles on a child's labial mucosa after Mycoplasma pneumoniae mucositis. Mucoceles can mimic persistent or recurrent stomatitis and lead to potential errors in management.

BACKGROUND: Calciphylaxis is a well-known entity in end-stage kidney disease and after renal transplant. Cases of nonuremic calciphylaxis (NUC) have also been reported, but data on this rare condition are mainly empirical. OBJECTIVE: We discuss a case of NUC secondary to spontaneous tumour lysis syndrome in a patient who had chemotherapy for non-Hodgkin lymphoma and present a review of the literature to better characterize malignancy-associated NUC. METHODS: We identified 12 published cases of malignancy-associated NUC. RESULTS: This systematic review of malignancy-associated NUC did not show a relationship between the type of malignancy, the distribution of skin lesions, or mortality. However, distal more than proximal involvement seems to be more frequently associated with calcium phosphate imbalance. CONCLUSION: Clinicians must maintain a high index of suspicion for calciphylaxis when evaluating patients with cutaneous lesions developed in the setting of malignancy.

BACKGROUND: Sarcoidosis is a noncaseating granulomatous disease that affects a large variety of organs and tissues. Skin sarcoidosis is commonly found in scar and tattooed tissues. OBJECTIVE: To report this particular case of sarcoidosis following electrolysis hair removal. METHOD: We report the case of a woman who developed sarcoidosis years after axillary electrolysis hair removal. RESULTS: The diagnosis of cutaneous sarcoidosis was suggested by the clinical manifestations and confirmed by histopathologic findings. CONCLUSION: Sarcoidosis should be considered during the investigation of skin lesions occurring in scar or traumatized tissue.
