RESUMEN: Objetivo: Describir los cambios de la pirámide nasal al realizar una osteotomía Le Fort I y el manejo cosmético complementario a la cirugía ortognática. Material y método: Se realizó una búsqueda electrónica en las bases de datos PubMed, EBSCO, Scopus, The Cochrane Library, y en el metabuscador Epistemonikos a través de las palabras clave "Le Fort 1", "Orthognathic Surgery", "Maxillary Surgery", "Nasolabial Changes", "Nasal Change", "Nasolabial Esthetics", "Soft Tissue", "Nasal Profile", "Nose" y "Cosmetic", relacionadas entre sí con los términos booleanos AND y OR. Resultados: se seleccionaron 25 artículos: 15 estudios observacionales, 5 revisiones narrativas, 3 ensayos clínicos y 2 notas técnicas. Conclusión: se establece cierto patrón que permitiría predecir cambios que sufrirá la nariz: aumento en la proyección nasal, disminución del largo columelar y ensanchamiento de la base alar son uno de ellos. En otros aspectos como el ángulo nasolabial o la posición final de la punta nasal no se ha logrado definir un patrón común. Para evitarlos se han descrito muchas técnicas. Sin embargo, falta mayor evidencia.


ABSTRACT: Objective: To describe the changes of the nasal pyramid when performing a Le Fort I osteotomy and its cosmetic management complementary to the orthognathic surgery. Materials and Methods: An electronic search was made in PubMed, EBSCO, Scopus, The Cochrane Library and Epistemonikos using the keywords "Le Fort 1", "orthognathic surgery", "maxillary surgery", "nasolabial changes", "nasal change", "nasolabial esthetics", "soft tissue", "nasal profile", "nose" and "cosmetic" related to each other with the Boolean terms AND and OR. Results: 25 articles were selected; 15 observational studies, 5 narrative reviews, 3 clinical trials and 2 technical notes. Conclusion: Nasal changes tend to follow a certain pattern that allows to predict them when facing surgery: increase of nose projection, decrease in columellar length and widening of the alar base, among others. As for other aspects such as the nasolabial angle or the final position of the nasal tip, it has not been possible to define a common pattern. To avoid them, several techniques have been described. However, more evidence is needed.

RESUMEN Introducción: La reconstrucción de las estructuras de la articulación temporomandibular (ATM) degenerada permanecen como un desafío clínico hasta el día de hoy, y su tratamiento se ha enfocado principalmente en el alivio del dolor y la eventual restauración de la función articular. Objetivo: Describir, mediante una revisión narrativa, las propiedades y características de diferentes células mesenquimales para el tratamiento regenerativo de patologías articulares. Material y método: Se realizó una búsqueda electrónica de la literatura en las bases de datos de PubMed, EBSCO, Web of Science, Scopus y The Cochrane Library a partir del año 2005, en idioma ingles y español, con énfasis en la regeneración articular de la rodilla y la ATM. Se excluyeron aquellos estudios donde la degeneración articular fuese producto de un trauma. Fueron evaluados dieciocho artículos. Resultados y discusión: Resaltan los diferentes protocolos utilizados con células madre mesenquimales derivadas de médula ósea, tejido adiposo y cordón umbilical humano, con resultados de regeneración articular en todos los casos a nivel radiográfico e histológico cuando fue posible. El tratamiento de las patologías articulares en la ATM ha sido poco descrito, con buenos resultados en reportes de caso y estudios observacionales.


ABSTRACT: Introduction: To date, temporomandibular joint (TMJ) reconstruction remains a clinical challenge, and its treatment has focused mainly on pain relief and the eventual restoration of joint function. Aim: To describe, through a narrative review, the properties and characteristics of different mesenchymal cells for the regenerative treatment of joint diseases. Material and method: An electronic search of the literature was carried out in the databases of PubMed, EBSCO, Web of Science, Scopus and The Cochrane Library since the year 2005, in English and Spanish, emphasizing joint regeneration of the knee and the TMJ. Studies where joint degeneration was the result of trauma were excluded. Eighteen articles were evaluated. Results and discussion: The reported literature highlights the different protocols used with mesenchymal stem cells derived from bone marrow, adipose tissue and human umbilical cord, with results of joint regeneration in all cases at radiographic and histological level when possible. The treatment of joint diseases in the TMJ has been poorly described, with good results in case reports and observational studies. Because of this, it is important to continue investigating this matter to establish clear treatment protocols in the temporomandibular joint.

La sepsis es una enfermedad potencialmente mortal que puede afectar al neonato. En tal sentido, se expone la información necesaria para brindar una atención adecuada al recién nacido pretérmino con sepsis de inicio precoz: concepto, epidemiologia, patogenia, manifestaciones clínicas, diagnóstico, tratamiento y prevención. Asimismo, se abordan los factores que aumentan la posibilidad de infección en este recién nacido, las condiciones predisponentes en la madre y la corioamnionitis (clínica e histológica). Dicha infección puede prevenirse mediante un adecuado control prenatal y una correcta atención durante el parto, lo cual disminuirá la morbilidad y la mortalidad, así como tendrá gran impacto en la salud pública del país.


The sepsis is a potentially mortal disease that can affect the newborn. In such a sense, the necessary information is exposed to offer an appropriate care to the preterm newborn with sepsis of early beginning: concept, epidemiology, pathogenesis, clinical signs, diagnosis, treatment and prevention. Also, some factors that increase the infection possibility in this newborn, the predisposing conditions in the mother and the chorioamnionitis (clinical and histological) are approached. This infection can be prevented by means of an appropriate prenatal control and a correct care during the childbirth, which will diminish the morbidity and mortality, as well as will have great impact in the public health of the country.

PURPOSE: To compare 3 types of intermaxillary fixation (IMF) and their behavior when subjected to tension forces in 3 study models with a palatine fracture feature. MATERIALS AND METHODS: An experimental study of 3 identical acrylic models was performed. All had the same palatine fracture pattern on the maxillary midline. All were reduced with different IMF methods (ie, direct interdental wiring, Erich arch bars, and self-tapping screws). Tension forces were applied to the study models to observe the fracture line behavior. RESULTS: IMF with direct interdental wiring did not cause significant separation of the fracture feature in the anterior or posterior sector. IMF with the Erich arch bars caused a 2-mm separation in the anterior sector and 0 mm in the posterior sector. The IMF with self-tapping screws caused a 3-mm separation in the anterior sector and 1 mm in the posterior sector. CONCLUSIONS: IMF using self-tapping screws resulted in the greatest separation of the fracture compared with the results with Erich arch bars and direct interdental wiring. IMF with self-tapping screws tended to displace the fracture lines by application of the vector furthest from the center of resistance.
