RESUMO

The synthesis and antiproliferative evaluation of novel d-glucopyranuronamide-containing nucleosides is described. Based on our previously reported anticancer d-glucuronamide-based nucleosides, new analogues comprising N/O-dodecyl or N-propargyl substituents at the glucuronamide unit and anomerically-N-linked 2-acetamido-6-chloropurine, 6-chloropurine or 4-(6-chloropurinyl)methyl triazole motifs were synthesized in 4-6 steps starting from acetonide-protected glucofuranurono-6,3-lactone. The methodologies were based on the access to N-substituted glycopyranuronamide precursors, namely 1,2-O-acetyl derivatives or glucuronoamidyl azides for further nucleobase N-glycosylation or 1,3-dipolar cycloaddition with N9 - and N7 -propargyl-6-chloropurines, respectively. N-Propargyl glucuronamide-based N9 -purine nucleosides were converted into (triazolyl)methyl amide-6,6-linked pseudodisaccharide nucleosides via cycloaddition with methyl 6-azido-glucopyranoside. A CuI/Amberlyst A-21 catalytic system employed in the cycloaddition reactions also effected conversion into 6-dimethylaminopurine nucleosides. Antiproliferative evaluation in chronic myeloid leukemia (K562) and breast cancer (MCF-7) cells revealed significant effects exhibited by the synthesized monododecylated purine-containing nucleosides. A N-propargyl 3-O-dodecyl glucuronamide derivative comprising a N9 -ß-linked 6-chloropurine moiety was the most active compound against MCF-7 cells (GI50 =11.9 µM) while a related α-(purinyl)methyltriazole nucleoside comprising a N7 -linked 6-chloropurine moiety exhibited the highest activity against K562 cells (GI50 =8.0 µM). Flow cytometry and immunoblotting analysis of apoptosis-related proteins in K562 cells treated with the N-propargyl 3-O-dodecyl glucuronamide-based N9 -linked 6-chloropurine nucleoside indicated that it acts via apoptosis induction.

O objetivo do presente estudo foi verificar o perfil do absenteísmo dos servidores públicos de um município do extremo sul catarinense. A pesquisa é de natureza observacional e transversal, utilizando-se de dados fornecidos pela prefeitura municipal estudada. Foram analisados 578 servidores públicos municipais que afastaram-se entre 2015 e 2020. Dentre os servidores em afastamento, aponta-se que 77,3% eram do sexo feminino, 31,5% tinham entre os 29 aos 38 anos de idade, 53,3% eram servidores estatutários, e 65,1% ocorreram nos primeiros 5 anos de trabalho. Os Auxiliares de Ensino de Educação, foram os profissionais que mais se afastaram, e entre as secretarias, foi a Secretaria de Educação. Como principais causas de afastamento foram identificadas as doenças causadas por transtornos mentais e comportamentais, doenças do sistema osteomuscular, doenças do sistema respiratório e fatores que influenciam o estado de saúde e o contato com os serviços de saúde. Conclui-se que as causas de afastamentos são preocupantes, e provocam alterações nos processos de trabalho. Dessa forma, é importante compreender essa dinâmica, contribui para planejar medidas para reduzir os índices de absenteísmo no município.

OBJECTIVE: To evaluate the value of positron emission tomography/computed tomography (PET/CT) in predicting no residual disease (NRD) after secondary cytoreductive surgery (SCS) compared with MSK criteria, the iMODEL, and the AGO score. METHODS: We analyzed 112 patients with platinum-sensitive ovarian carcinoma who underwent SCS. We excluded patients for whom PET/CT was not performed, those without sufficient data, and who received chemotherapy before SCS. Ultimately, 69 patients were included. RESULTS: Variables that correlated with NRD were peritoneal carcinomatosis index (odds ratio [OR]=0.91; 95% confidence interval [CI]=0.83-0.99; p=0.044), European Cooperative Oncology Group Performance Status (ECOG) 0 (OR=8.0; 95% CI=1.34-47.5; p=0.022), and ≤2 lesions by PET/CT (OR=4.36; 95% CI=1.07-17.7; p=0.039). Of the patients with ≤2 lesions by PET/CT, 48 (92.3%) underwent complete SCS. The sensitivity, positive predictive value, negative predictive value, and accuracy of PET/CT for NRD were 85.7%, 92.3%, 33.3%, and 81.2%, respectively. NRD was achieved after fulfilling the MSK criteria, iMODEL and AGO Score in 89.1%, 88.1% and 85.9%, respectively. The accuracy of the MSK criteria, iMODEL, and AGO score in predicting NRD was 87%, 83.3%, and 77.3%, respectively. The PET/CT findings agreed well with the AGO score and iMODEL. The addition of PET/CT to these models increased the NRD rates (92.2%, 91.8%, and 89.4% for MSK+PET/CT, iMODEL+PET/CT, and AGO+PET/CT, respectively), but lowered their accuracy. CONCLUSION: We observed NRD in 92.3% of patients with ≤2 lesions by PET/CT, with an accuracy of 81.2%. PET/CT did not increase the accuracy of the MSK criteria, iMODEL, or AGO score models.

Background: Breakpoint cluster region-Abelson gene (BCR-ABL) tyrosine kinase inhibitors (TKIs) have revolutionized the treatment of patients with chronic myeloid leukemia (CML). However, concern has arisen about the cardiac safety profile of these drugs. Objectives: This study aims to compare long-term risks of adverse cardiovascular and cerebrovascular events (ACE), heart failure or left ventricular ejection fraction (LVEF) < 50%, and venous thromboembolic events (VTE) in patients with CML treated with BCR-ABL TKIs, using data from a large multinational network. Methods: Patients aged ≥ 18 years with CML treated with imatinib, dasatinib, or nilotinib without prior cardiovascular or cerebrovascular disease were included. We used propensity score matching to balance the cohorts. The 5-year cumulative incidences and hazard ratios were calculated. Results: We identified 3,722 patients with CML under treatment with imatinib (n = 1,906), dasatinib (n = 1,269), and nilotinib (n = 547). Patients with imatinib compared to dasatinib showed a higher hazard ratio (HR) for ACE (HR 2,13, 95% CI 1.15-3.94, p = 0.016). Patients with imatinib presented a lower HR than nilotinib for ACE (HR 0.50, 95% CI 0.30-0.83, p = 0.0074). In relation to heart failure or LVEF < 50%, patients with imatinib had a higher HR than dasatinib (HR 9.41, 95% CI 1.22-72.17, p = 0.03), but no significant difference was observed between imatinib and nilotinib (HR 0.48, 95% CI 0.215-1.01, p = 0.064). Conclusion: In this retrospective study with a large number of patients with CML, those treated with nilotinib had a higher 5-year ratio of ACE, while patients with dasatinib showed a lower ratio than patients with imatinib. The ratio of heart failure was higher in patients with imatinib than in patients with dasatinib, but not when compared to nilotinib.

Erysipelas is often related to lymphedema, which can occur in up to 60% of cases, with advanced age, radiotherapy, tumor extension, surgical approach, and infections as risk factors. The aim of this study was to present and discuss a series of cases of erysipelas after breast cancer surgery treated in a private mastology clinic over the past ten years. This is a retrospective horizontal cohort study in which we selected all cases of erysipelas after breast cancer surgery from 2009 to 2019. The following were evaluated: number of patients treated with a diagnosis of breast carcinoma with axillary approach, age, surgery performed, adjuvant treatment and treatment of erysipelas, presence of lymphedema, and measurement of circumferences between both arms and associated diseases. A total of 12 cases of breast cancer were treated. In 66.66% of cases, a radical axillary lymphadenectomy was performed, and in 16.66% of cases, only a sentinel lymph node investigation was performed. The average age was 67.6 years. Erysipelas appeared, on average, 43 months after cancer diagnosis. Two deaths were reported due to severe erysipelas leading to sepsis. More studies are still needed on the subject. Of the 12 cases in this study, eight (66.66%) were associated with lymphedema. Only two (16.66%) of the patients in this group who developed erysipelas were not submitted to axillary dissection. The treatment for 50% of the participants in this research was with penicillin G benzathine. There were three relapses, and two patients died during the research period

Objetivo: Analisar a percepção dos gestores dos consórcios intermunicipais de saúde e das comissões intergestores regionais do sul de Santa Catarina sobre a incorporação das centrais de regulação nos procedimentos eletivos de média e alta complexidade, inseridos nos consórcios intermunicipais de saúde do estado. Métodos: Trata-se de um estudo descritivo como abordagem mista, realizado com representantes legais de consórcios intermunicipais de saúde e gestores municipais representantes da Comissão Intergestores Regional do sul de Santa Catarina, totalizando 6 participantes. A coleta de dados se deu através de entrevista semiestruturada. A análise ocorreu por método de condensação de significados com auxílio do programa SPSS versão 23.0. Resultados: Foi possível obter a percepção dos representantes legais e gestores acerca dos mecanismos de regulação, legislação vigente, sua eficácia e seu conhecimento prévio a respeito dos mesmos. Além disso, conseguiu-se traçar as características do perfil profissional dos respectivos indivíduos. Conclusões: Os resultados obtidos apontam uma percepção positiva dos gestores acercada regulação e o auxílio que a mesma trouxe aos municípios em relação à continuidade na linha de cuidado e a garantia do acesso aos pacientes com critérios clínicos mais graves. Evidenciou-se também a necessidade de melhorias na eficiência e fiscalização das legislações vigentes, além da necessidade dos gestores receberem capacitações acerca de características pontuais dos mecanismos legais.

No Sistema Único de Saúde (SUS) os Agentes Comunitários de Saúde (ACS) estão em constante contato com o território em que atuam, pela sua função e papel de morador, resultando em consequências ambíguas no estabelecimento de suas ações. Assim, o presente estudo busca compreender o papel dos Agentes Comunitários de Saúde, sua relação com o território, unidade e profissionais, e os processos de saúde e adoecimento no desempenho da função. Trata-se de um estudo qualitativo no formato de pesquisa participante, por meio de entrevista semiestruturada com 6 ACS, que também foram acompanhados em suas atividades diárias. A análise de conteúdo dos dados obtidos por diário de campo e entrevista possibilitou a identificação de 3 categorias: Papel dos ACS; Relação com a Equipe, e Saúde e Sofrimento dos ACS. O sofrimento dos ACS decorre da vigilância sobre seu trabalho, dificuldades no relacionamento interpessoal com a equipe, adversidades no território e sobrecarga de trabalho. Identifica-se que são profissionais muitas vezes subestimados e incompreendidos


In the Unified Health System, Community Health Agents (CHA) are in constant contact with the territory in which they operate, due to their role and role as residents, resulting in ambiguous consequences in the establishment of their actions. Thus, the present study seeks to understand the role of Community Health Agents, their relationship with the territory, unit and professionals, and the processes of health and illness in the performance of the function. This is a qualitative study in the form of participatory research, through a semi-structured interview with 6 CHA, who were also monitored in their daily activities. The content analysis of the data obtained by field diary and interview enabled the identification of 3 categories: Role of the CHA; Relationship with the Team, and Health and Suffering from the CHA. The CHA's suffering stems from surveillance over their work, difficulties in interpersonal relationships with the team, adversities in the territory and work overload. It is identified that they are professionals often underestimated and misunderstood.

BACKGROUND: Risk-reducing operations are an important part of the management of hereditary predisposition to cancer. In selected cases, they can considerably reduce the morbidity and mortality associated with cancer in this population. OBJECTIVES: The Brazilian Society of Surgical Oncology (BSSO) developed this guideline to establish national benchmarks for cancer risk-reducing operations. METHODS: The guideline was prepared from May to December 2021 by a multidisciplinary team of experts to discuss the surgical management of cancer predisposition syndromes. Fourteen questions were defined and assigned to expert groups that reviewed the literature and drafted preliminary recommendations. Following a review by the coordinators and a second review by all participants, the groups made final adjustments, classified the level of evidence, and voted on the recommendations. RESULTS: For all questions including risk-reduction bilateral salpingo-oophorectomy, hysterectomy, and mastectomy, major agreement was achieved by the participants, always using accessible alternatives. CONCLUSION: This and its accompanying article represent the first guideline in cancer risk reduction surgery developed by the BSSO, and it should serve as an important reference for the management of families with cancer predisposition.

In the challenge of making children and young people more physically active, possibilities of unit-ing knowledge between digital games and physical activity and, as a product, the exergames have emerged. Using this method as a possibility of behavior change, the present study aimed to present a prototype of an exergame that stimulates the increase of physical activity levels in adolescents. In order for the user to be able to use the exergame as a mobile application, it is necessary to register initially with personal and anthropometric data, establish goals by time, and then start using it. The proposal is to make the application available to schools and students for research, evaluating the in-fluence of the exergame on the level of habitual physical activity, displacement, participation in phys-ical education classes, motivation and interest for changing behavior of physical activity through-out the day, among other parameters. Equipping Physical Education professionals with these new healthcare tools is necessary, and they can be pedagogical for the paradigm shift on how to contribute to a healthier aging and with more quality of life


No desafio de tornar crianças e jovens mais fisicamente ativas, surgem possibilidades de união de conhe-cimentos entre jogos digitais e atividade física e como produto os exergames. Utilizando este método como possibilidade de mudanças de comportamento, o presente estudo teve como objetivo apresentar um protótipo de exargame que estimule o aumento dos níveis de atividade física em adolescentes. Para que o usuário possa utilizar o exergame em formato de aplicativo móvel, se faz necessário fazer inscrição inicial com seus dados pessoais e antropométricos estabelecimento de objetivos por tempo para na sequência possa iniciar a utilização. A proposta é disponibilizar o aplicativo para as escolas e estudantes para realização de pesquisas, avaliando a influência do exergame sobre o nível de atividade física habitual, de deslocamento, participação nas aulas de educação física, motivação e interesse para a mudança de comportamento de atividade física durante todo o dia, entre outros parâmetros. Instrumentalizar os professores de Educação Física sobre o cuidado com a saúde, através destas novas ferramentas é necessário e podem ser pedagógicas para mudança de paradigmas sobre como contribuir para um envelhecimento mais saudável e com mais qualidade de vida

Breast cancer is the most frequent among women in the world and in Brazil. New treatment strategies are considerably increasing survival rates in the context of Breast cancer, making it important to study the physical, social, and emotional effects of the disease and its treatments. In this context, integrative medicine emerges as a strategy based on scientific evidence, along with conventional therapy, with a mind-body approach with the use of natural products and lifestyle changes. The aim of this study was to carry out a brief literature review on integrative medicine and lifestyle in women who survived Breast cancer. This is an integrative review carried out with studies indexed in PubMed. Eight search strategies were carried out using the keywords: "survivorship," "breast cancer," "lifestyle," "nutrition," "physical activity," "alcohol," "tobacco," "sleep," "distress," and "relationship," respecting the period between 2015 and 2021. In all, 166 articles were found. Studies that considered other types of cancer and did not focus on the lifestyle of cancer survivors were excluded from the analysis. The remaining 28 articles referring to the proposed theme were read and analyzed in full. The results were described according to the six pillars of a healthy lifestyle proposed by the American College of Lifestyle Medicine, being addressed as follows: (1) nutrition, (2) physical activity, (3) stress, (4) substance abuse (alcohol and tobacco), (5) sleep, and (6) healthy relationships (marital relationships and social support), showing the importance of training health services and professionals in cancer survival programs to provide better guidance to patients with Breast cancer on how to use integrative therapies properly and what lifestyle changes can help optimize various aspects of your health, reducing the risk of recurrence or a new cancer.

Objetivo: refletir sobre o acesso na Atenção Primária à Saúde (APS) através de analogias entre dois programas televisivos brasileiros. Método: trata-se de uma reflexão teórica sustentada em elementos da cultura televisiva popular brasileira, a partir dos programas: a porta da esperança e a porta dos desesperados. Resultados: o acesso à saúde no âmbito da APS pode ser analisado na perspectiva de três portas: esperança, na solução de suas necessidades; desespero, voltada aqueles que não conseguem encontrar soluções as suas demandas; e a prioritária, pautada nas atribuições que lhe constitui e no direito à saúde. Conclusão: a APS precisa superar a tipologia de porta da esperança e dos desesperados para efetivação da sua atribuição ordenadora da rede de atenção à saúde.


Objective: to reflect on access in Primary Health Care (PHC) through analogies of two Brazilian television shows. Method: this is a theoretical reflection based on elements of Brazilian popular television culture, based on the shows: the door of hope and the door of the desperate. Results: the access to health within the scope of PHC can be analyzed from the perspective of three doors: hope, in the solution of their needs; despair, aimed at those who cannot find solutions to their demands; and the priority, based on the attributions that constitute it and the right to health. Conclusion: PHC needs to overcome the typology of the door of hope and of the desperate for the achievement of its ordering attribution of the health care network.


Objetivo: reflexionar sobre el acceso en la Atención Primaria de Salud (APS) a través de analogías entre dos programas de televisión brasileños. Método: se trata de una reflexión teórica a partir de elementos de la cultura popular televisiva brasileña, a partir de los programas: la puerta de la esperanza y la puerta de los desesperados. Resultados: el acceso a la salud en el ámbito de la APS puede analizarse desde la perspectiva de tres puertas: la esperanza, en la solución de sus necesidades; la desesperación, dirigida a quienes no encuentran solución a sus demandas; y la prelación, con base en las atribuciones que la constituyen y el derecho a la salud. Conclusión: la APS necesita superar la tipología de la puerta de la esperanza y del desesperado para cumplir su tarea de organización de la red de atención a la salud.

O objetivo deste estudo foi avaliar o nível de satisfação dos usuários da Atenção Primária em Saúde de um município de grande porte. Trata-se de um estudo transversal, do tipo censitário e com amostra por conveniência, visto que foram incluídas todas as unidades de saúde e entrevistados os usuários que estavam na sala de espera no dia da coleta e aceitaram participar da pesquisa. Toda a coleta de dados foi realizada por meio de questionários, e,posteriormente,tabulada e analisada no software Stata 14.0. Os usuários relataram satisfação com a unidade e equipe, e com serviços assistenciais. Entretanto, usuários que manifestaram que trocariam de unidade caso isto fosse possível, tinham razões atribuídas à organização do cuidado e agendamento de consultas. Concluímos que a satisfação com os cuidados exercidos pelos profissionais e com o acolhimento de informações de cunho afetivo foram evidentemente altas, ao passo que a insatisfação com a organização do cuidado é aparente.


This study aimed to assess the level of satisfaction of users of PrimaryHealth Care in a large city. This is a cross-sectional, census-type study with a convenience sample, as all health units were included and users who were in the waiting room on the day of collection and who agreed to participate in the survey were interviewed. All data collection was performed through questionnaires, and later tabulated and analyzed using the Stata 14.0 software. Users reported satisfaction with the unit and team, and with care services. However, users who stated that they would change units if this was possible had reasons attributed to the organization of care and appointment scheduling. We conclude that satisfaction with the care provided by professionals and with the reception of information of an affective nature were evidently high, while dissatisfaction with the organization of care is apparent.

Abstract Objective To study the anatomy of the medial coracoclavicular ligament and assess the contribution of the acromioclavicular, coracoclavicular and medial coracoclavicular ligaments to the stability of the acromioclavicular joint. Methods Twenty-six shoulders from 16 fresh cadavers were dissected after placement in dorsal recumbency with a 15-cm cushion between the shoulder blades. An extended deltopectoral approach was performed proximally and medially, followed by plane dissection and ligament identification. The acromioclavicular and coracoclavicular distances were measured using points previously marked with a millimeter caliper. Six of these specimens were submitted to a biomechanical study. The acromioclavicular ligament, the coracoclavicular ligament and the medial coracoclavicular ligament were sectioned sequentially, and a cephalic force of 20 N was applied to the lateral clavicle. The acromioclavicular and coracoclavicular distances were measured in each of the ligament section stages. Results The right medial coracoclavicular ligament presented, on average, 48.9 mm in length and 18.3 mm in width. On the left side, its mean length was 48.65 mm, with a mean width of 17.3 mm. Acromioclavicular, coracoclavicular and medial coracoclavicular ligament section resulted in a statistically significant increase in the coracoclavicular distance and posterior scapular displacement. Conclusion The medial coracoclavicular ligament is a true ligamentous structure found in all dissected shoulders. Our results showed that the scapular protraction relaxed the medial coracoclavicular ligament, while scapular retraction tensioned it; in addition, our findings demonstrate that this ligament contributes to the vertical and horizontal stability of the acromioclavicular joint.


Resumo Objetivo Estudar a anatomia do ligamento coracoclavicular medial e avaliar a contribuição do ligamento acromioclavicular, coracoclaviculares e coracoclavicular medial na estabilidade da articulação acromioclavicular. Métodos Foram dissecados 26 ombros de 16 cadáveres frescos, posicionados em decúbito dorso-horizontal, com um coxim de 15 cm de altura entre as escápulas. Realizou-se uma via deltopeitoral estendida proximal e medialmente. Realizou-se dissecção por planos e identificação dos ligamentos. Realizou a medida da distância acromio-clavicular e coracoclavicular usando pontos previamente demarcados com paquímetro milimetrado. Em seis dessas amostras foi realizado estudo biomecânico. Seccionando, nesta ordem, o ligamento acromioclavicular, os coracoclaviculares e o ligamento coracoclavicular medial com uma força cefálica de 20N foi aplicada na clavícula lateral. Foi medida a distância acromio-clavicular e coracoclavicular em cada uma das etapas de secção dos ligamentos. Resultados A média de comprimento do ligamento coracoclavicular medial foi de 48,9mm e a média de largura, de 18,3mm no lado direito. No esquerdo, a média de comprimento foi de 48,65mm e a média da largura, 17,3mm. Após a secção dos ligamentos acromioclaviculares, coracoclaviculares, com a secção do ligamento coracoclavicular medial houve aumento estatisticamente significativo da distância córaco-clavicular e um deslocamento posterior da escápula. Conclusão O ligamento coracoclavicular medial é uma estrutura ligamentar verdadeira, presente em todos os ombros dissecados. Nossos resultados demonstraram que o ligamento coracoclavicular medial encontra-se relaxado com a escápula em protração e tenso com a escápula em retração e segundo nossos resultados participa tanto da estabilidade vertical quanto da estabilidade horizontal da articulação acromioclavicular.

RESUMO

Abstract Mechanical ventilation in prone position is an alternative strategy for patients with acute respiratory discomfort syndrome (ARDS) to improve oxygenation in situations when traditional ventilation modalities have failed. However, due to the significant increase in ARDS cases during the SARS-CoV-2 pandemic and the experimental therapeutic use of potentially arrhythmogenic drugs, cardiopulmonary resuscitation in this unusual position could be needed. Therefore, we will review the available scientific evidence of cardiopulmonary resuscitation in prone position.

RESUMO

Theranostics describes the pairing of diagnostic biomarkers and therapeutic agents with common specific targets. Nuclear medicine is the greatest theranostics protagonist, relying on radioactive tracers for imaging biologic phenomena and delivering ionizing radiation to the tissues that take up those tracers. The concept has gained importance with the growth of personalized medicine, allowing customized management for diseases, refining patient selection, better predicting responses, reducing toxicity, and estimating prognosis. This work provides an overview of the general concepts of the theranostics approach in nuclear medicine discussing its background, features, and future directions in imaging and therapy.

Cancer demands precise evaluation and accurate and timely assessment of response to treatment. Imaging must be performed early during therapy to allow adjustments to the course of treatment. For decades, cross-sectional imaging provided these answers, showing responses to the treatment through changes in tumor size. However, with the emergence of immune checkpoint inhibitors, complex immune response patterns were revealed that have quickly highlighted the limitations of this approach. Patterns of response beyond tumor size have been recognized and include cystic degeneration, necrosis, hemorrhage, and cavitation. Furthermore, new unique patterns of response have surfaced, like pseudoprogression and hyperprogression, while other patterns were shown to be deceptive, such as unconfirmed progressive disease. This evolution led to new therapeutic evaluation criteria adapted specifically for immunotherapy. Moreover, inflammatory adverse effects of the immune checkpoint blockade were identified, many of which were life threatening and requiring prompt intervention. Given complex concepts like tumor microenvironment and novel therapeutic modalities in the era of personalized medicine, increasingly sophisticated imaging techniques are required to address the intricate patterns of behavior of different neoplasms. Fluorine 18-fluorodeoxyglucose PET/CT has rapidly emerged as one such technique that spans both molecular biology and immunology. This imaging technique is potentially capable of identifying and tracking prognostic biomarkers owing to its combined use of anatomic and metabolic imaging, which enables it to characterize biologic processes in vivo. This tailored approach may provide whole-body quantification of the metabolic burden of disease, providing enhanced prediction of treatment response and improved detection of adverse events. ©RSNA, 2020.
