OBJECTIVES: To evaluate the effect of a coaching intervention compared with control on physical activity and falls rate at 12 months in community-dwelling people aged 60+ years. DESIGN: Cluster randomised controlled trial. SETTING: Community-dwelling older people. PARTICIPANTS: 72 clusters (605 participants): 37 clusters (290 participants) randomised to the intervention and 35 (315 participants) to control. INTERVENTION: Intervention group received written information, fall risk assessment and prevention advice by a physiotherapist, activity tracker and telephone-based coaching from a physiotherapist focused on safe physical activity. Control group received written information and telephone-based dietary coaching. Both groups received up to 19 sessions of telephone coaching over 12 months. OUTCOMES: The co-primary outcomes were device-measured physical activity expressed in counts per minute at 12 months and falls rate over 12 months. Secondary outcomes included the proportion of fallers, device-measured daily steps and moderate-to-vigorous physical activity (MVPA), self-reported hours per week of physical activity, body mass index, eating habits, goal attainment, mobility-related confidence, quality of life, fear of falling, risk-taking behaviour, mood, well-being and disability. RESULTS: The mean age of participants was 74 (SD 8) years, and 70% (n=425) were women. There was no significant effect of the intervention on device-measured physical activity counts per minute (mean difference 5 counts/min/day, 95% CI -21 to 31), or falls at 12 months (0.71 falls/person/year in intervention group and 0.87 falls/person/year in control group; incidence rate ratio 0.86, 95% CI 0.65 to 1.14). The intervention had a positive significant effect on device-measured daily steps and MVPA, and self-reported hours per week of walking, well-being, quality of life, and disability. No significant between-group differences were identified in other secondary outcomes. CONCLUSION: A physical activity and fall prevention programme including fall risk assessment and prevention advice, plus telephone-based health coaching, did not lead to significant differences in physical activity counts per minute or falls rate at 12 months. However, this programme improved other physical activity measures (ie, daily steps, MVPA, hours per week of walking), overall well-being, quality of life and disability. Sarcopenia is a risk factor for adverse clinical outcomes in chronic kidney disease (CKD) patients, including mortality. Diagnosis depends on adopted consensus definition and cutoff values; thus, prevalence rates are generally heterogeneous. We conducted a systematic review and meta-analysis to investigate the global prevalence of sarcopenia and its traits across the wide spectrum of CKD. A systematic search was conducted using databases, including MEDLINE and EMBASE, for observational studies reporting the prevalence of sarcopenia. We considered sarcopenia according to the consensus definition of the European Working Group on Sarcopenia in Older People (EWGSOP), the Asian Working Group for Sarcopenia, the Foundation for the National Institutes of Health Sarcopenia Project, and the International Working Group on Sarcopenia (IWGS). Subgroup analyses by CKD stages, consensus, and gender were performed. Pooled prevalence was obtained from random-effect models. A total of 140 studies (42 041 patients) across 25 countries were included in this systematic review and meta-analyses. Global prevalence of sarcopenia was 24.5% [95% confidence interval (CI): 20.9-28.3) and did not differ among stages (P = 0.33). Prevalence varied according to the consensus definition from 11% to 30%, with no significant difference (P = 0.42). Prevalence of severe sarcopenia was 21.0% (95% CI: 11.7-32.0), with higher rates for patients on dialysis (26.2%, 95% CI: 16.6-37.1) compared to non-dialysis (3.0%, 95% CI: 0-11.1; P < 0.01). Sarcopenic obesity was observed in 10.8% (95% CI: 3.5-21.2). Regarding sarcopenia traits, low muscle strength was found in 43.4% (95%CI: 35.0-51.9), low muscle mass in 29.1% (95% CI: 23.9-34.5), and low physical performance in 38.6 (95% CI: 30.9-46.6) for overall CKD. Prevalence was only higher in patients on dialysis (50.0%, 95% CI: 41.7-57.4) compared to non-dialysis (19.6%, 95% CI: 12.8-27.3; P < 0.01) for low muscle strength. We found a high global prevalence of sarcopenia in the wide spectrum of CKD. Low muscle strength, the primary sarcopenia trait, was found in almost half of the overall population with CKD. Patients on dialysis were more prevalent to low muscle strength and severe sarcopenia. Nephrology professionals should be aware of regularly assessing sarcopenia and its traits in patients with CKD, especially those on dialysis.

BACKGROUND: Knowledge of which physical activity programs are most effective for older adults in different sub-populations and contexts is limited. The objectives of this rapid review were to: 1) Overview evidence evaluating physical activity programs/services for older adults; and 2) Describe impact on physical activity, falls, intrinsic capacity (physical domain), functional ability (physical, social, and cognitive/emotional domains), and quality of life. METHODS: We conducted a rapid review of primary studies from 350 systematic reviews identified in a previous scoping review (March 2021: PEDro, MEDLINE, CINAHL, Cochrane Database). For Objective 1, we included intervention studies investigating physical activity programs/services in adults ≥ 60 years. Of these, we included good quality (≥ 6/10 PEDro scale) randomised controlled trials (RCTs) with ≥ 50 participants per group in Objective 2. RESULTS: Objective 1: Of the 1421 intervention studies identified from 8267 records, 79% were RCTs, 87% were in high income countries and 39% were good quality. Objective 2: We identified 87 large, good quality RCTs (26,861 participants). Overall activity promotion, structured exercise and recreation/sport had positive impacts (≥ 50% between-group comparisons positive) across all outcome domains. For overall activity promotion (21 intervention groups), greatest impacts were on physical activity (100% positive) and social outcomes (83% positive). Structured exercise (61 intervention groups) had particularly strong impacts on falls (91% positive), intrinsic capacity (67% positive) and physical functioning (77% positive). Recreation/sport (24 intervention groups) had particularly strong impacts on cognitive/emotional functioning (88% positive). Multicomponent exercise (39 intervention groups) had strong impacts across all outcomes, particularly physical activity (95% positive), falls (90% positive) and physical functioning (81% positive). Results for different populations and settings are presented. CONCLUSION: Evidence supporting physical activity for older adults is positive. We outline which activity types are most effective in different populations and settings.

BACKGROUND: This statistical analysis plan details the Coaching for Healthy AGEing (CHAnGE) trial analysis methodology. OBJECTIVE: To investigate the effect of a combined physical activity and fall prevention program on physical activity and falls compared to a healthy eating among people aged 60 years and over. METHODS: The CHAnGE trial is a pragmatic parallel-group cluster-randomised controlled trial with allocation concealment and blinded assessors. Clusters are allocated to either (1) a physical activity and fall prevention intervention or (2) to a healthy eating intervention. The primary outcomes are: objectively measured physical activity at 12 months post-randomisation, and self-reported falls throughout the 12-month trial period. Secondary outcomes include the proportion of participants reporting a fall, the proportion of participants meeting the Australian physical activity guidelines, body mass index, eating habits, mobility goal attainment, mobility-related confidence, quality of life, fear of falling, risk-taking behaviour, mood, well-being, self-reported physical activity, disability, and use of health and community services. ANALYSIS: We will follow the intention-to-treat principle. All analysis will allow for cluster randomisation using a generalised estimating equation approach. The between-group difference in the number of falls per person-year will be analysed using negative binomial regression models. For the continuously scored primary and secondary outcome measures, linear regression models adjusted for corresponding baseline scores will assess the effect of group allocation. Analyses will take into account cluster randomisation and will be adjusted for baseline scores. A subgroup analysis will assess differential effects of the intervention by baseline physical activity levels and history of falls.

Total intravenous anesthesia (TIVA) has been gaining ground in the routine of small animals. This study aimed to evaluate the hemodynamic effects produced by continuous infusion of propofol isolated or associated with ketamine, S-ketamine, or remifentanil in dogs submitted to video laparoscopic ovariectomy. Thirty-two female dogs were randomly assigned to 4 groups (n = 8): G,1 propofol (0.6 mg/kg/min); G2. ketamine (2 mg/kg followed by 100 µg/kg/min) and propofol (0.4 mg/kg/min); G3, S-ketamine (1 mg/kg followed by 50 µg/kg/min) and propofol (0.4 mg/kg/min); and G4, remifentanil (1 µg/kg followed by 0.2 µg/kg/min) and propofol (0.4 mg/kg/min). All dogs were submitted to the same pre-anesthetic protocol with acepromazine (0.1 mg/kg) and meperidine (4 mg/kg) intramuscularly, followed by anesthetic induction with propofol (4 mg/kg). All animals were mechanically ventilated. Heart rate (HR), respiratory rate (f), SpO2, systolic, diastolic and mean arterial blood pressures (SAP, DAP and MAP, respectively), EtCO2, cardiac output (CO), blood glucose and rectal temperature were evaluated in 7 time-points (M0-M7). HR increased throughout the anesthesia in all groups, except for G4, which showed inferior values. In all groups, EtCO2 increased from M1 to M7. SAP was higher in G1 in relation to G2 in M2 and M3, and G4 in all time points. G4 also obtained the lower values for DAP and MAP, although not inferior to 60 mmHg. CO was unchanged through time and among groups. No groups had hyperglycemia, although glucose levels varied with time. It was concluded that all TIVA protocols showed satisfactory results and hemodynamic stability.

Hyperlipidemia causes lipotoxicity which prompts an inflammatory response linked to the development of cardiovascular diseases. Natural compounds have been receiving special attention for its potential to treat diseases, inexpensiveness, and safety. Guarana (Paullinia cupana) has demonstrated notable anti-inflammatory and antioxidant effects, which may prevent chronic diseases caused by changes in lipid profile. Thus, this study aims to evaluate the effect of guarana powder (Paullinia cupana) in the purine metabolism and inflammatory profile in lymphocytes and serum of rats with Poloxamer-407-induced hyperlipidemia. Pretreatment with guarana 12.5, 25, and 50 mg/kg/day or caffeine (0.2 mg/kg/day) by gavage was applied to adult male Wistar rats for a period of 30 days. As a comparative standard, we used simvastatin (0.04 mg/kg) post-induction. Hyperlipidemia was acutely induced with intraperitoneally injection of Poloxamer-407 (500 mg/kg). Guarana powder and caffeine increased the activity of the E-NTPDase (ecto-apyrase), and all pretreatments decreased the E-ADA (ecto-adenosine deaminase) activity, reducing the inflammatory process caused by lipotoxicity. In hyperlipidemic rats, ATP levels were increased while adenosine levels were decreased, guarana and caffeine reverted these changes. Guarana powder, caffeine, and simvastatin also prevented the increase in INF-γ and potentiated the increase in IL-4 levels, promoting an anti-inflammatory profile. Guarana promoted a more robust effect than caffeine. Our results show that guarana powder and caffeine have an anti-inflammatory as seen by the shift from a proinflammatory to an anti-inflammatory profile. The effects of guarana were more pronounced, suggesting that guarana powder may be used as a complementary therapy to improve the lipotoxicity-associated inflammation.

OBJECTIVES: The aim of this study was to evaluate the effects of dipyrone and tramadol, used for 5 days, on postoperative pain, hematological and biochemical parameters, and oxidative markers on erythrocytes. METHODS: Twenty-eight healthy cats underwent ovariohysterectomy and were randomly allocated to four groups (each n = 7), according to the postoperative treatment administered intravenously: control (saline 1 ml q8h), DIP1 (dipyrone 25 mg/kg q24h), DIP2 (dipyrone 25 mg/kg q12h) and DIP3 (dipyrone 25 mg/kg q8h). All animals received tramadol (2 mg/kg q8h). Pain was assessed by visual analog (VAS), multidimensional UNESP and Glasgow pain scales for cats preoperatively and at 3, 6, 12, 24, 36 and 48 h after extubation. Venous blood was collected daily for 5 days, and on day 10, to perform a complete blood count (CBC) and determine the percentage of Heinz bodies (HBs). Serum biochemistry was evaluated preoperatively and on days 5 and 10; superoxide dismutase (SOD), catalase (CAT), myeloperoxidase (MPO) and lipoperoxidation were evaluated preoperatively and on days 3, 5 and 10. RESULTS: Control cats had higher pain scores than DIP3 cats by UNESP (P = 0.0065), and DIP2 (P = 0.0035) and DIP3 cats (P = 0.0108) by VAS 3 h postoperatively. Rescue analgesia was required by two animals in the control group and one each in the DIP1 and DIP2 groups. There was no difference in SOD or CAT among groups. On day 5, MPO was more active in DIP2 than in DIP3 cats (P = 0.0274). No difference in lipoperoxidation among treatment and control cats was found. CBC remained constant and without statistical difference among groups. Control, DIP2 and DIP3 cats presented a similar percentage of HBs on day 10. Biochemical variables were similar among groups and times. CONCLUSIONS AND RELEVANCE: The administration of dipyrone in cats, when used in combination with tramadol, did not ensure better analgesia than tramadol alone. Dipyrone did not significantly affect biochemical variables and oxidative markers, despite minimal, clinically irrelevant, hematological differences between groups.

Abstract Background: There is a comprehensive understanding of the role of dietary fiber as a nutrient and its function during digestion in cattle. On the other hand, the role of fiber digestion in goats has not received similar attention. Objective: To evaluate the effects of different ratios of forage neutral detergent fiber (fNDF) and non-fibrous carbohydrates (NFC) on ruminal fermentation products, and in vitro neutral detergent fiber (NDF) digestibility in goats. Methods: A 3 × 5 factorial arrangement in a completely randomized design with three fNDF levels (100, 190, and 290 g/kg) and five NFC levels (350, 400, 450, 500, and 550 g/kg) was used. The experiment was performed in vitro. Two ruminally fistulated goats were used as rumen liquid donors. Results: The ratios between fNDF and NFC affected all ruminal parameters (p<0.05). Increasing NFC levels in diets containing 100 and 290 g/kg fNDF resulted in linear increase (p<0.05) in ammonia concentration after 48 h fermentation. There was no adjustment of linear models (p>0.05) for pH values. Total volatile fatty acids (VFA) and their individual molar proportions were affected (p<0.05) at all fNDF and NFC levels. The NDF digestibility was not affected (p>0.05) by fNDF or NFC levels, except for diets containing 290 g/kg fNDF, which were fit to a quadratic model (p<0.05). Conclusion: The relationship between fNDF and NFC concentrations affect the fermentation end products and in vitro NDF digestibility. NFC plays a more consistent role than fNDF in the ruminal microbial ecosystem of goats during in vitro fermentation.


Resumen Antecedentes: Aunque hay una amplia comprensión del papel que juega la fibra dietética como nutriente y su función durante la digestión en el ganado vacuno, el papel de la digestión de la fibra en las cabras no ha recibido una atención similar. Objetivo: Evaluar los efectos de diferentes proporciones de fibra detergente neutra proveniente del forraje (fNDF) y carbohidratos no fibrosos (NFC) sobre los productos de la fermentación ruminal y la digestibilidad in vitro de la fibra detergente neutra (NDF) en cabras. Métodos: Se utilizó un arreglo factorial de 3 × 5, distribuidos en un diseño completamente aleatorizado con tres niveles de fNDF (100, 190 y 290 g/kg) y cinco niveles de NFC (350, 400, 450, 500 y 550 g/kg). El experimento se realizó in vitro. Se utilizaron dos cabras fistuladas en rumen como donantes de líquido ruminal. Resultados: La relacion entre fNDF y NFC afectó todos los parámetros ruminales estudiados (p<0,05). El aumento de los niveles de NFC en dietas con 100 y 290 g/kg de fNDF resultó en aumento lineal (p<0,05) de la concentración de amoníaco después de 48 h de fermentación. No hubo ajuste de modelos lineales (p>0,05) para valores de pH. Los ácidos grasos volátiles totales y sus proporciones molares individuales se afectaron (p<0,05) en todos los niveles de fNDF y NFC. La digestibilidad de NDF no fue afectada (p>0,05) por los niveles de fNDF ni NFC, con excepción de las dietas con 290 g/kg fNDF, que se ajustaron a un modelo cuadrático (p<0,05). Conclusión: La relación entre las concentraciones de fNDF y NFC afecta los productos finales de fermentación y la digestibilidad in vitro de NDF. Los niveles de NFC juegan un papel más consistente que fNDF en el ecosistema microbiano ruminal de la cabra durante la fermentación in vitro.

Objetivo: Avaliar os efeitos de diferentes relações da fibra em detergente neutro advindo de forragem (fNDF) e carboidratos não-fibrosos (NFC) sobre produtos da fermentação ruminal e digestibilidade in vitro da fibra em detergente neutro (NDF) em caprinos. Métodos: Utilizou-se um esquema fatorial de 3 × 5 distribuídos em um delineamento inteiramente casualizado com três níveis de fNDF (100, 190 e 290 g/kg) e cinco níveis de NFC (350, 400, 450, 500 e 550 g/kg). O experimento foi realizado in vitro. Duas cabras fistuladas no rúmen foram utilizadas como doadoras de líquido ruminal. Resultados: As relações entre fNDF e NFC impactaram todos os parâmetros ruminais estudados (p<0,05). O aumento dos níveis de NFC em dietas contendo 100 e 290 g/kg de fNDF resultou em um aumento linear (p<0,05) na concentração de amônia após 48 h de fermentação. Não houve ajuste de modelos lineares (p>0,05) para valores de pH. Os ácidos graxos voláteis totais e suas proporções molares individuais foram afetados (p<0,05) em todos os níveis de fNDF e NFC. A digestibilidade NDF não foi afetada (p>0,05) pelos níveis de fNDF e NFC, com exceção de dietas experimentais contendo 290 g/kg fNDF, que foram ajustadas a um modelo quadrático (p<0,05). Hyperlipidemia is associated with endothelial dysfunction and inflammatory disorders. Adenosine and adenosine deaminase (ADA) modulate immune responses and lipid metabolism. Curcumin and rutin are polyphenols with antioxidant, anti-inflammatory, and hypolipidemic effects. We evaluated the action of rutin and curcumin in the lipid levels and inflammation, as well as their effect on ADA activity in serum, lymphocytes, platelets, and neutrophils of hyperlipidemic rats. Adult male Wistar rats pretreated with curcumin and/or rutin for 30 days were submitted to Poloxamer-407- induced hyperlipidemia. Biochemical, hematological, and oxidative stress parameters, as well as serum and extracellular ADA activity, were performed 36h post-induction. Hyperlipidemia was confirmed by the increase in total cholesterol (TC) and triglycerides (TG). Hematological alterations, elevated reactive oxygen species (ROS) levels, and increased myeloperoxidase (MPO) and ADA activities were observed in hyperlipidemic rats. Curcumin and the curcumin/rutin association decreased TG and increased high-density lipids (HDL) levels. The pretreatments prevented changes in the hematological parameters, decreased the activities of MPO in plasma and ADA in serum and cells. Cholesterol and ROS levels were not altered by the pretreatments. Our results show that pretreatments with rutin and/or curcumin prevent the hyperlipidemia-induced inflammation. Pretreatments with curcumin and/or rutin are potential complementary therapies in the prevention of hypertriglyceridemia and inflammation.

Signaling mediated by purines is a widespread mechanism of cell-cell communication related to vasomotor responses and the control of platelet function in the vascular system. However, little is known about the involvement of this signaling as well as the role of reactive oxygen species (ROS) in the development of hypothyroidism. Therefore, the present study investigates changes in the purinergic system, including enzyme activities and expression in platelets, and oxidative profiles in patients with post-thyroidectomy hypothyroidism. The nucleoside triphosphate diphosphohydrolase 1 (NTPDase/CD39) expression in patients increased by 40%, and the adenosine triphosphate (ATP) or adenosine diphosphate (ADP) hydrolyzing activity increased by 82% and 70%, respectively. The activities of ecto-5´-nucleotidase and adenosine deaminase (ADA) also significantly enhanced (39% and 52%, respectively), which correlates with a 45% decrease in adenosine concentration. Furthermore, these patients demonstrated an increased production of ROS (42%), thiobarbituric acid reactive substances (TBARS) (115%), carbonyl protein (30%) and a decreased glutathione S-transferase (GST) activity (20%). This study demonstrates that hypothyroidism interferes with adenine nucleoside and nucleotide hydrolysis and this is correlated with oxidative stress, which might be responsible for the increase in ADA activity. This increase causes rapid adenosine deamination, which can generate a decrease in their concentration in the systemic circulation, which can be associated with the development of vascular complications.

Abstract Objective: To investigate the association between postpartum depression and the occurrence of exclusive breastfeeding. Method: This is a cross-sectional study conducted in the states of the Northeast region, during the vaccination campaign in 2010. The sample consisted of 2583 mother-child pairs, with children aged from 15 days to 3 months. The Edinburgh Postnatal Depression Scale was used to screen for postpartum depression. The outcome was lack of exclusive breastfeeding, defined as the occurrence of this practice in the 24 h preceding the interview. Postpartum depression was the explanatory variable of interest and the covariates were: socioeconomic and demographic conditions; maternal health care; prenatal, delivery, and postnatal care; and the child's biological factors. Multivariate logistic regression analysis was conducted to control for possible confounding factors. Results: Exclusive breastfeeding was observed in 50.8% of the infants and 11.8% of women had symptoms of postpartum depression. In the multivariate logistic regression analysis, a higher chance of exclusive breastfeeding absence was found among mothers with symptoms of postpartum depression (OR = 1.67; p < 0.001), among younger subjects (OR = 1.89; p < 0.001), those who reported receiving benefits from the Bolsa Família Program (OR = 1.25; p = 0.016), and those started antenatal care later during pregnancy (OR = 2.14; p = 0.032). Conclusions: Postpartum depression contributed to reducing the practice of exclusive breastfeeding. Therefore, this disorder should be included in the prenatal and early postpartum support guidelines for breastfeeding, especially in low socioeconomic status women.


Resumo Objetivo: Verificar a associação entre a depressão pós-parto e a ocorrência do aleitamento materno exclusivo. Método: Estudo de corte transversal feito nos estados da Região Nordeste, durante a campanha de vacinação de 2010. A amostra consistiu de 2.583 binômios mães-crianças entre 15 dias e três meses. Usou-se a Escala de Depressão Pós-Parto de Edimburgo para rastrear a depressão pós-parto. O desfecho consistiu da ausência de aleitamento materno exclusivo nas 24 horas que antecederam a entrevista. A depressão pós-parto foi variável explanatória de interesse e as covariáveis foram: condições socioeconômicas e demográficas, assistência pré-natal, ao parto e pós-natal e fatores da criança. Fez-se análise de regressão logística multivariada com o objetivo de controlar possíveis fatores de confusão. Resultados: A amamentação exclusiva foi observada em 50,8% das crianças e 11,8% das mulheres apresentaram sintomatologia indicativa de depressão pós-parto. Na análise de regressão logística multivariada foi verificada uma maior chance de ausência do aleitamento materno exclusivo entre as mães com sintomas de depressão pós-parto (OR = 1,67; p < 0,001). Conclusões: A depressão pós-parto contribuiu para redução da prática do aleitamento materno exclusivo. Assim, esse transtorno deveria ser incluído nas orientações de apoio desde o pré-natal e nos primeiros meses pós-parto, especialmente em mulheres de baixo nível socioeconômico.
