RESUMO

The dental treatment of patients with oral cavity and oropharyngeal squamous cell carcinoma (OOPSCC) may be challenging for dentists. This study aimed to characterize systemic changes in patients with OOPSCC undergoing dental treatment prior to cancer therapy, with a specific focus on laboratory assessments. The primary objectives included identifying potential adverse events, such as infections or bleeding, resulting from dental procedures. Additionally, the study aimed to correlate baseline patient characteristics with treatment-related toxicities. This was a prospective cohort study that included 110 OOPSCC patients referred to the Dental Oncology Service at São Paulo State Cancer Institute, Brazil, between November/2019 and December/2020. Comorbidities, sociodemographic data, medication in use, cancer treatment-related toxicities, and altered laboratory tests results were correlated. The most common comorbidities and altered laboratory results were hypertension, dyslipidemia, diabetes, as well as elevated levels of C-reactive protein, hemoglobin, and hematocrit. Toxicities exhibited a progressive pattern over time, encompassing oral mucositis (OM), xerostomia, dysphagia, dysgeusia, trismus, and radiodermatitis. No correlation between comorbidities and cancer treatment-related toxicities, a positive correlation between medications in use and OM, and a negative correlation between medications and dysgeusia were found. OM was associated with altered thyroxine (T4) and free thyroxine (FT4), calcium, urea, creatinine, alkaline phosphatase, and syphilis. Family income and housing were OM predictors. Altered T4/FT4/urea/calcium/alkaline phosphatase/creatinine/syphilis may be useful clinical predictors of OM. Despite the elevated prevalence of comorbidities and abnormal laboratory findings, dental treatment prior to cancer treatment yielded no adverse events.

OBJECTIVES: To search for studies that address the efficacy of nonpharmacologic methods for pain relief in adults undergoing cardiac surgeries. DESIGN: A systematic review registered in the International Prospective Register of Systematic Reviews (PROSPERO) under number CRD42020168681. DATA SOURCE: PubMed, LILACS, CINAHL, the Web of Science, Scopus, and the Cochrane Central Register of Controlled Trials. REVIEW/ANALYSIS METHODS: The review used a PRISMA guideline that selected primary randomized controlled trials on the efficacy of nonpharmacologic pain relief therapies in patients undergoing cardiac surgery, with no time or language restrictions. The Joanna Briggs Institute Critical Appraisal Checklist for Randomized Clinical Trials was used to assess methodological rigor. RESULTS: After screening, 23 of the 140 studies found in the databases were selected. The studies examined the efficacy of 13 different nonpharmacologic therapies, as well as a combination of therapies, with massage therapy being the most commonly examined, followed by musical intervention and hypnosis. CONCLUSIONS: Some interventions, when combined with pharmacologic therapy, were effective in relieving postoperative pain after cardiac surgeries, according to the studies analyzed. However, most studies had significant methodological flaws, and further studies with high methodological quality are needed.

AIMS: Radiation caries (RC) is a highly prevalent and chronic complication of head and neck radiotherapy (HNRT) and presents a challenge for clinicians and patients. The present study aimed to assess the impact of RC on the morbidity and mortality outcomes of head and neck squamous cell carcinoma (HNSCC) patients. METHODS AND RESULTS: Patients were divided into three groups: (1) RC (n = 20), (2) control (n = 20), and (3) edentulous (n = 20). Information regarding the number of appointments, dental procedures, osteoradionecrosis (ORN), prescriptions, and hospital admissions were collected. Mortality outcomes were assessed through disease-free survival (DFS) and overall survival (OS) rates. RC patients required more dental appointments (p < .001), restorations (p < .001), extractions (p = .001), and antibiotic and analgesic prescriptions (p < .001). Kaplan-Meier subgroup analyses showed a significantly increased risk of ORN in RC compared to edentulous patients (p = .015). RC patients presented lower DFS rates (43.2 months) than the control and edentulous groups (55.4 and 56.1 months, respectively). CONCLUSIONS: RC impacts morbidity outcomes among cancer survivors due to increased demand for medication prescriptions, multiple specialized dental appointments, invasive surgical treatments, increased risk of ORN, and increased need for hospital admissions.

Objetivo: analisar os custos operacionais de um pronto-socorro relacionados ao atendimento de pacientes COVID-19 em 2020 e 2021. Método: estudo transversal, descritivo de abordagem quantitativa. A mensuração dos custos utilizou-se da perspectiva do gestor hospitalar, por meio de microcusteio por absorção. Custos diretos, indiretos e variáveis, foram avaliados de cima para baixo (top-down). Resultados: o perfil predominante foi de homens, com idades entre 61 e 70 anos, casados, brancos e moradores de Londrina (Paraná, Brasil). O tempo médio de internação para pacientes graves foi 12,20 dias e, para os demais, 8,38 dias. O desfecho principal foi a alta hospitalar. Os custos operacionais em 2020 foram de R$28.461.152,87, já em 2021 os valores encontrados foram R$43.749.324,61. O custo médio do paciente-dia foi de R$2.614,45 em 2020 para R$3.351,93 em 2021. Conclusão: verificou-se aumento dos custos no período estudado. Conhecer os custos operacionais do pronto-socorro, possibilita o planejamento financeiro institucional contribuindo para qualificar a tomada de decisões gerenciais.


Objective: to analyze the operating costs of an emergency room related to the care of COVID-19 patients in 2020 and 2021. Method: cross-sectional, descriptive study with a quantitative approach. The measurement of costs was used from the perspective of the hospital manager, through absorption micro-costing. Direct, indirect and variable costs were evaluated from top to bottom (top-down). Results: the predominant profile was men, aged between 61 and 70 years, married, white and living in Londrina (Paraná, Brazil). The mean length of stay for critically ill patients was 12.20 days and for the others, 8.38 days. The main outcome was hospital discharge. Operating costs in 2020 were BRL 28,461,152.87, while in 2021 the values found were BRL 43,749,324.61. The average patient-day cost went from R$2,614.45 in 2020 to R$3,351.93 in 2021. Conclusion: costs increased in the study period. Be aware of the operational costs of emergency room, enablement or institutional financial planning, contributing to qualify management decision-making.

Los costos directos, indirectos y variables se evaluaron de arriba hacia abajo (top-down). Resultados: el perfil predominante fue el de hombres, con edad entre 61 y 70 años, casados, blancos y residentes en Londrina (Paraná, Brasil). La estancia media de internación de los pacientes críticos fue de 12,20 días y, de los demás, de 8,38 días. El resultado principal fue el alta hospitalaria. Los costos operacionales en 2020 fueron de R$ 28.461.152,87, mientras que en 2021 los valores encontrados fueron de R$ 43.749.324,61. Los costos medios del paciente/día aumentaron de R$2.614,45 en 2020 a R$3.351,93 en 2021. Conclusión: se observó un aumento de los costos en el periodo estudiado. Conocer los costos operativos de un servicio de urgencias posibilita la planificación financiera institucional, contribuyendo a calificar la toma de decisiones gerenciales.

OBJECTIVE: To evaluate the efficacy and outcomes of the surgical treatment for pelvic organ prolapse (POP) in stages III and IV by sacrospinous ligament fixation (SSLF) or uterosacral ligament suspension (USLS) by comparing anatomical and subjective cure rates and quality-of-life parameters (through the version validated for the Portuguese language of the Prolapse Quality of Life [P-QoL] questionnaire) under two definitions: genital prolapse Ba, Bp, and C < -1 (stage I) and Ba, Bp, and C ≤ 0 (stage II). MATERIALS AND METHODS: After we obtained approval from the Ethics Committee (under CAAE 0833/06) and registered the study in ClinicalTrials.gov (NCT01347021), 51 patients were randomized into two groups: the USLS group (N = 26) and the SSLF group (N = 25), with follow-up 6 and 12 months after the procedures. RESULTS: There was a significant improvement in the P-QoL score and anatomical measurements of all compartments in both groups after 12 months (p < 0.001). The anatomical cure rates in the USLS and SSLF groups, considering stage 1, were of 34.6% and 40% (anterior) respectively; of 100% both for groups (apical); and of 73.1% and 92% (posterior) respectively. The rates of adverse outcomes were of 42% (N = 11) and 36% (N = 11) for the USLS and SSLF groups respectively (p = 0.654), and those outcomes were excessive bleeding, bladder perforation (intraoperative) or gluteal pain, and urinary infection (postoperative), among others, without differences between the groups. CONCLUSION: High cure rates in all compartments were observed according to the anatomical criterion (stage I), without differences in P-QoL scores and complications either with USLS or SSLF for the surgical treatment of accentuated POP.


OBJETIVO: Avaliar a eficácia e os resultados do tratamento cirúrgico para prolapso de órgãos pélvicos (POP) nos estágios III e IV, por meio da técnica de fixação do ligamento sacroespinal (FLSE) ou suspensão do ligamento útero-sacro (SLUS), ao comparar os índices de cura anatômicos, subjetivos, e os parâmetros de qualidade de vida (por meio do questionário Prolapse Quality of Life [P-QoL] validado para a língua portuguesa) sob duas definições: prolapso genital Ba, Bp e C < −1 (estágio I) e Ba, Bp e C ≤ 0 (estágio II). MATERIAIS E MéTODOS: Após aprovação do Comitê de Ética (CAAE 0833/06) e registro no ClinicalTrials.gov (NCT01347021), 51 pacientes foram randomizadas em dois grupos: grupo SLUS (N = 26) e (2) grupo FLSE (N = 25), com seguimento de 6 e 12 meses. RESULTADOS: Houve melhora significativa nas pontuações no P-QoL e nas medidas anatômicas de todos os compartimentos em ambos os grupos após 12 meses (p < 0,001). As taxas de cura anatômica nos grupos SLUS e FLSE , considerando o estágio 1, foram de 34,6% e 40% (anterior), respectivamente; de 100% em ambos os grupos (apical); e de 73,1% e 92% (posterior), respectivamente. As taxas de resultados adversos foram de 42% (N = 11) e 36% (N = 11), respectivamente, nos grupos SLUS e FLSE (p = 0,654), e elas foram sangramento excessivo, perfuração da bexiga (intraoperatória) ou dor glútea, e infecção urinária (pós-operatória), entre outras, sem diferenças entre os grupos. CONCLUSãO: Altas taxas de cura em todos os compartimentos foram observadas segundo critério anatômico (estágio I), sem diferença quanto às pontuações no P-QoL e às complicações tanto com SLUS quanto com FLSE para o tratamento cirúrgico de POP acentuado.

Agro-industrial residues have attracted attention for their applications in the field of biodegradable packaging. Recently, our research group has developed onion-based films with promising properties for this type of application due to their non-toxicity, biocompatibility and biodegradability. Therefore, in this study, we investigated the effect of Laponite clay concentration on the physicochemical and antioxidant properties of the onion-based films, which were prepared by a casting method. The XRD and FTIR data confirm the presence of the mineral clay in the onion-based films. These findings are consistent with those obtained from FE-SEM analysis, which revealed the presence of typical Laponite grains. In terms of wettability, the results show that the clay decreases the hydrophilic character of the material but slightly increases the water vapor permeation. Optical characterization revealed that the materials exhibited zero transmittance in the UV region and increased opacity in the visible region for composites containing 5% and 10% Laponite. Furthermore, the antioxidant test demonstrated higher antioxidant potential in the composites compared to the pure films. Consequently, these results suggest that the formation of Laponite and onion composites could be an essential strategy for developing natural polymers in the field of food contact packaging.

The incidence of cancer cases is increasing worldwide, and chemotherapy is often necessary as part of the treatment for many of these cases. Nature-based interventions have been shown to offer potential benefits for human well-being. OBJECTIVE: This study aims to investigate the outcome of nature images on clinical symptom management related to chemotherapy. METHODS: A randomized clinical trial was conducted in an outpatient cancer unit of a private hospital in Brazil, with 173 participants over the age of 18 who were undergoing chemotherapy and had signed an informed consent form. The intervention consisted of the presentation of a 12-min video featuring nature images categorized under the themes of Tranquility, Beauty, Emotions Up, or Miscellany. Images were sourced from the e-Nature Positive Emotions Photography Database (e-NatPOEM), a publicly available collection of affectively rated images. Sociodemographic and clinical data, as well as the participants' connection to nature, were investigated. The Positive Affect/Negative Affect Scale (PANAS) and the Edmonton Symptom Assessment System (ESAS) were applied pre- and post-intervention. RESULTS: Data showed very strong evidence of a reduction in negative affect for the intervention group (p < 0.001) and moderate evidence for the control group (p = 0.034). There was also a significant reduction in the intervention group for pain (p < 0.001), tiredness (p = 0.002), sadness (p < 0.001), anxiety (p < 0.001), and appetite (p = 0.001). The Beauty video had the best performance, while the Tranquility video showed no significant improvement in any of the symptoms evaluated. These findings suggest that images of nature may be a valuable tool to help control clinical and psychological symptoms in cancer patients undergoing chemotherapy.

Abstract Objective To compare the use of mirabegron with anticholinergics drugs for the treatment of overactive bladder (OB). Data Source Systematic searches were conducted in EMBASE, PUBMED, Cochrane, and LILACS databases from inception to September 2021. We included RCTs, women with clinically proven OB symptoms, studies that compared mirabegron to antimuscarinic drugs, and that evaluated the efficacy, safety or adherence. Data Collection RevMan 5.4 was used to combine results across studies. We derived risk ratios (RRs) and mean differences with 95% CIs using a random-effects meta-analytic model. Cochrane Collaboration Tool and GRADE was applied for risk of bias and quality of the evidence. Data Synthesis We included 14 studies with a total of 10,774 patients. Fewer total adverse events was reported in mirabegron group than in antimuscarinics group [RR 0.93 (0.89-0.98)]. The risk of gastrointestinal tract disorders and dry mouth were lower with mirabegron [RR 0,58 (0.48-0.68); 9375 patients; RR 0.44 (0.35-0.56), 9375 patients, respectively]. No difference was reported between mirabegron and antimuscarinics drugs for efficacy. The adherence to treatment was 87.7% in both groups [RR 0.99 (0.98-1.00)]. Conclusion Mirabegron and antimuscarinics have comparable efficacy and adherence rates; however, mirabegron showed fewer total and isolated adverse events.


Resumo Objetivo Comparar o uso de mirabegrom com anticolinérgicos para o tratamento da bexiga hiperativa (BH). Fonte de Dados Buscas sistemáticas foram realizadas nas bases de dados EMBASE, PUBMED, Cochrane e LILACS desde o início até setembro de 2021. Incluímos ECR, mulheres com sintomas de BH clinicamente comprovados, estudos que compararam mirabegrom a medicamentos antimuscarínicos e avaliaram a eficácia, segurança ou adesão. Coleta de Dados RevMan 5.4 foi usado para combinar os resultados entre os estudos. Derivamos razões de risco (RRs) e diferenças médias com intervalo de confiança (IC) de 95% usando um modelo meta-analítico de efeitos aleatórios. Cochrane Collaboration Tool e GRADE foi aplicado para risco de viés e qualidade da evidência. Síntese dos Dados Foram incluídos 14 estudos com um total de 10.774 pacientes. Menos eventos adversos totais foram relatados no grupo mirabegrom do que no grupo antimuscarínicos [RR: 0,93 (0,89-0,98)]. O risco de distúrbios do trato gastrointestinal e boca seca foram menores com mirabegrom [RR: 0,58 (0,48-0,68); 9.375 pacientes; RR: 0,44 (0,35-0,56), 9.375 pacientes, respectivamente]. Nenhuma diferença foi relatada entre mirabegrom e drogas antimuscarínicos para eficácia. A adesão ao tratamento foi de 87,7% em ambos os grupos [RR: 0,99 (0,98-1,00)]. Conclusão Mirabegrom e antimuscarínicos têm eficácia e taxas de adesão comparáveis, porém o mirabegrom apresentou menos eventos adversos totais e isolados.

Objetivo: Conhecer as percepções de mulheres sobre a autoimagem alterada após alopecia por quimioterapia antineoplásica. Método: Estudo descritivo, qualitativo, realizado em um hospital oncológico no Pará. Participaram 30 mulheres com alopecia por quimioterapia antineoplásica. Os dados foram produzidos no período de julho a agosto de 2021, por meio de entrevistas semiestruturadas. Analisaram-se os dados sob os preceitos da Análise de conteúdo associada com o software IRAMUTEQ. Resultados: Identificaram-se 17 mulheres com idade entre 44 e 56 anos (56,66%), com parceiro fixo (66,66%); católica (89,99%); com renda damiliar menor que um e até dois salários mínimos (63,33%). Dentre as cinco classes geradas pelo IRAMUTEQ, duas serão analisadas neste artigo: a percepção da mulher sobre a perda do cabelo e o cabelo como significado de feminilidade. Conclusão: As mulheres com autoimagens alteradas após a alopecia são protagonistas de enfrentamentos físicos e psicológicos que pode impactar negativamente as percepções sobre a aparência


Objective: To know the perceptions of women about the altered self-image after alopecia by antineoplastic chemotherapy. Method: Descriptive, qualitative study carried out in an oncology hospital in the state of Pará. Thirty women with alopecia from antineoplastic chemotherapy participated.Data were produced from July to August 2021, through individual interviews with a semi-structured script. Data were analyzed under the precepts of Content Analysis associated with the IRAMUTEQ software. Results: We identified 17 women aged between 44 and 56 years (56.66%), with a steady partner (66.66%); Catholic (89.99%); with household income below one and up to two minimum wages (63.33%). Among the five classes generated by IRAMUTEQ, two will be analyzed in this article "the woman's perception of hair loss" and "the hair as a meaning of femininity". Conclusion: Women with altered self-images after alopecia are protagonists of physical and psychological confrontations that can negatively impact perceptions about appearance.

Neoplasms of the exocrine pancreas are uncommon in domestic animals and rarely occur in wildlife. This article describes the clinical and pathological findings of one case of metastatic exocrine pancreatic adenocarcinoma in an 18-year-old giant otter (Pteronura brasiliensis) in captivity with a history of inappetence and apathy. Abdominal ultrasonography was inconclusive, and tomography revealed a neoplasm affecting the urinary bladder and hydroureter. During the anaesthesia recovery, the animal presented a cardiorespiratory arrest and died. Grossly, there were neoplastic nodules in the pancreas, urinary bladder, spleen, adrenal glands, and mediastinal lymph node. Microscopically, all nodules were composed of a malignant hypercellular proliferation of epithelial cells with acinar or solid disposition, supported by a sparse fibrovascular stroma. Neoplastic cells were immunolabeled with antibodies to Pan-CK, CK7, CK20, PPP and chromogranin A. Approximately 25% of the cells were positive for the presence of Ki-67 too. Pathological and immunohistochemical findings confirmed the diagnosis of metastatic exocrine pancreatic adenocarcinoma.

Entomopathogenic fungi, widely available biological agents used to control agricultural pests, are sporadically reported to cause focal or disseminated infection in reptiles and mammals, including humans. This study summarizes the clinical presentation, histopathological and molecular findings by panfungal polymerase chain reaction and sequencing of four cases of hypocrealean fungal infections in captive common green iguanas (Iguana, iguana). One case of granulomatous pneumonia, hepatitis and serositis was related to Metarhizium flavoviride complex infection. Two disseminated fungal infection cases, with scarce inflammatory cell infiltration, were caused by Beauveria bassiana while there was one case of multifocal granulomatous and necrotizing pneumonia by Purpureocillium spp. To the best of our knowledge, this is the first report of fatal mycosis infection due to entomopathogenic fungi in captive common green iguanas.

Background: In Brazil, 5870 new cases of malignant central nervous system tumors (MCNST) were estimated for men and 5220 for women for each year of the 2020-2022 triennium. The objective of this study was to analyze incidence rate trends and compare demographic characteristics of new MCNST cases according to tumor topographies in Brazil from 2000 to 2015. Methods: This study comprises an analytical cross-sectional assessment of secondary databases extracted from the Brazilian National Cancer Institute (INCA) website. Data comprised new neoplasm cases of meninges (C70), brain (C71), spinal cord, cranial nerves, and other central nervous system parts (C72) retrieved from 23 population-based cancer registries. A descriptive analysis was performed. Crude and age-adjusted incidence rates were calculated. Linear trends were calculated using a linear least squares regression for adjusted incidence rates versus time. Results: A total of 24 986 new MCNST cases were recorded. The main topography was the brain (91.5%). Except for meninges tumors, where 62.4% of the cases were observed in women, MCNST cases were more frequent among men concerning the other evaluated topographies. All 3 topographies occurred predominantly in adult patients aged from 40- to 64-year-old. Between 2000 and 2015, incidence rates ranged from 5.12 to 4.95 (a 1.4% increase of per year; 95% CI -4.0 to 6.8; P = .584) in men and from 4.35 to 3.61 (a 3.1% increase per year; 95% CI -1.7 to 8.0; P = .189). Conclusions: The most frequent topography was the brain. Incidence rates of MCNST remained relatively stable over time in both sexes.

OBJECTIVE: To identify information needs of patients with coronary artery disease and develop and validate the content of educational messages for mobile phones for these patients. METHOD: The study was carried out in three phases: 1) Identification of information needs in relation to coronary artery disease of patients hospitalized for an acute coronary event; 2) Development of templates containing text and pictures about the disease and treatment; 3) Content validity analysis of template evidence through the assessment of 10 experts. Templates were considered validated when the Content Validity Ratio (CVR) was equal to or greater than 0.80. RESULTS: A total of 67 patients were included, and all the information that emerged about the disease was classified as important to very important. Thirty templates were developed (heart function, recommendations on nutrition and exercise, treatments and medications, and clinical signs related to the disease and risk factor control), and the CVR obtained was greater than 0.80. CONCLUSION: All information needs were categorized by patients as important or very important. The templates were developed and validated considering content and design.

OBJECTIVE: To analyze the epidemiological profile and the specific survival of patients diagnosed with tongue squamous cell carcinoma at the National Cancer Institute (INCA). MATERIALS AND METHODS: Hospital Cancer Registry System Data and Mortality Information from 2007 to 2009 were retrieved in a retrospective cohort study of patients diagnosed with tongue squamous cell carcinoma. Specific survival was estimated using the Kaplan-Meier method. The association between independent variables and the risk of death was explored in a Cox proportional hazards regression model. RESULTS: A total of 346 patients were eligible, mostly male (77.5%), smokers (87.6%), with alcohol consumption (80.9%), with low education (65.6%), advanced staging at the time of diagnosis (71.1%), and presenting a high mortality rate (72.5%). In total, 44.5% of patients underwent a surgical approach alone or associated with another treatment modality, of which 85.1% of patients underwent neck dissection and 90.1% had free surgical margins. Specific survival was 40.6% in two years and 31.2% in five years. CONCLUSION: The 5-year specific survival was considered worse in individuals over 60 years, and who did not undergo surgical treatment or had surgery associated with another treatment, compared to patients undergoing isolated surgery.

OBJECTIVE: The aim of this study was to describe, from a historical perspective, the relevance, resilience and outcomes of vaginal hysterectomy (VH) in gynecology in the age of technological scenario. METHODS: The authors searched records from January 2011 to January 2021 on the following databases: Medline, EMBASE, and CENTRAL (The Cochrane Library) for combinations of the terms "vaginal hysterectomy," "outcomes" AND "history"; and before that period, if the search had historical relevance. INCLUSION CRITERIA: randomized clinical trials; hysterectomy performed for benign gynecological conditions; and VH outcomes compared with Abdominal Hysterectomy (AH), Laparoscopic Hysterectomy (LH) or Robotic Hysterectomy (RH). RESULTS: The VH combines sequences of reproducible techniques which have been developed over the years to safely and effectively overcome the limitations of difficult cases of vaginal extirpation from the uterus. CONCLUSION: The authors support endoscopic surgical approaches in complex surgery for benign indications, urogynecology, and gynecologic oncology when appropriate. However, what makes the gynecological surgeon different from the general surgeon is the vaginal access. It is essential to continue to train residents in vaginal surgical skills and provide safe and cost-effective patient care. The art of technology is the resilience of keeping only the patient at the center of innovation.

Hybrid peripheral nerve sheath tumors show combined features of more than one type of conventional benign peripheral nerve sheath tumors. There are few cases reported of hybrid peripheral nerve sheath tumors in the head and neck region. A 68-year-old female patient was referred for evaluation of an oral swelling lasting five years. Intraoral examination revealed a small mobile nodule located in the lower vestibule. The patient underwent excisional biopsy and microscopic evaluation showed typical features of neurofibroma enclosing areas with palisading nuclei compatible with Antoni A pattern, which are seen in schwannomas. These regions showed strong and diffuse immunoreactivity for S100 protein and moderate positivity in the neurofibroma area. CD34 was positive in the neurofibroma area and entrapped axons were positive for neurofilament. The final diagnosis was oral hybrid neurofibroma-schwannoma tumor. Hybrid peripheral nerve sheath tumors, although extremely rare, may arise within the oral cavity. To the best of our knowledge, this is the first neurofibroma-schwannoma tumor reported in the oral cavity. Recognizing hybrid peripheral nerve sheath tumors as a distinct clinicopathological entity is important because they may also be associated with syndromic disorders.

ABSTRACT Objective: to assess the prevalence of sleep disturbances and factors associated with sleep quality in patients with Acute Coronary Syndrome. Method: this is a cross-sectional study, carried out in the Cardiology Unit of a public university hospital, from October 2021 to December 2022, with patients hospitalized for Acute Coronary Syndrome over 18 years old. Sociodemographic and clinical data were collected, and sleep quality was measured by the Pittsburgh Sleep Quality Index. Statistical association tests were performed, considering a value of p<0.05 as significant. Results: a total of 96 patients were included, the majority being male, married and with a mean age of 63 years. The most prevalent comorbidities were hypertension, dyslipidemia and diabetes. It was identified that 92% had alteration in sleep quality and that the number of hours slept (p.<0.01), time to start sleep (p.0.03), sleep latency (p.<0.01), sleep duration (p.<0.01), habitual sleep efficiency (p.<0.02) and daytime sleepiness and daytime dysfunction (p.0.01) were significantly associated with sleep quality. There was a weak but significant correlation between age (r-0.22, p.0.02) and the presence of obstructive coronary lesions (r-0.23; p.0.02) with the Pittsburgh Sleep Quality Index score. Conclusion: most patients with acute coronary syndrome were classified as poor sleepers, therefore educational interventions to promote sleep should be performed in this population to reduce cardiovascular risk.


RESUMEN Objetivo: evaluar la prevalencia de trastornos del sueño y factores asociados a la calidad del sueño en pacientes con Síndrome Coronario Agudo. Método: estudio transversal, realizado en la Unidad de Cardiología de un hospital universitario público, de octubre de 2021 a diciembre de 2022, con pacientes hospitalizados por Síndrome Coronario Agudo mayores de 18 años. Se recogieron datos sociodemográficos y clínicos, y la calidad del sueño se midió mediante el Índice de Calidad del Sueño de Pittsburgh. Se realizaron pruebas de asociación estadística, considerando significativo un valor de p<0,05. Resultados: se incluyeron 96 pacientes, la mayoría hombres, casados ​​y con una edad media de 63 años. Las comorbilidades más prevalentes fueron hipertensión arterial sistémica, dislipidemia y diabetes. Se identificó que el 92% presentaba cambios en la calidad del sueño y que la cantidad de horas dormía (p.<0,01), tiempo de inicio del sueño (p.0,03), latencia del sueño (p.<0,01), duración del sueño (p.<0,01), eficiencia del sueño (p.<0,02) y somnolencia diurna y disfunción diurna (p.0,01) se asociaron significativamente con la calidad del sueño. Hubo una correlación débil pero significativa entre la edad (r-0,22, p.0,02) y la presencia de lesiones coronarias obstructivas (r-0,23; p.0,02) con el puntaje del Pittsburgh Sleep Quality Index. Conclusión: la mayoría de los pacientes con Síndrome Coronario Agudo fueron clasificados como insomnes, por lo que se deben realizar intervenciones educativas para promover el sueño en esta población para reducir el riesgo cardiovascular.

Foi identificado que 92% tinham alteração na qualidade do sono e que a quantidade de horas dormidas (p.<0,01), o tempo para iniciar o sono (p.0,03), a latência do sono (p.<0,01), duração do sono (p.<0,01), eficiência do sono (p.<0,02) e sonolência diurna e disfunção diurna (p.0,01) apresentaram associação significativa com a qualidade do sono. Houve correlação fraca, porém, significativa entre a idade (r-0,22, p.0,02) e a presença de lesões obstrutivas coronarianas (r-0,23; p.0,02) com o escore do Pittsburgh Sleep Quality Index. Conclusão: a maioria dos pacientes com Síndrome Coronariana Aguda foram classificados como maus dormidores, portanto intervenções educativas para promoção do sono devem ser realizadas nesta população para a redução no risco cardiovascular.

Abstract Objective To evaluate the efficacy and outcomes of the surgical treatment for pelvic organ prolapse (POP) in stages III and IV by sacrospinous ligament fixation (SSLF) or uterosacral ligament suspension (USLS) by comparing anatomical and subjective cure rates and quality-of-life parameters (through the version validated for the Portuguese language of the Prolapse Quality of Life [P-QoL] questionnaire) under two definitions: genital prolapse Ba, Bp, and C< −1 (stage I) and Ba, Bp, and C ≤ 0 (stage II). Materials and Methods After we obtained approval from the Ethics Committee (under CAAE 0833/06) and registered the study in ClinicalTrials.gov (NCT 01347021), 51 patients were randomized into two groups: the USLS group (N = 26) and the SSLF group (N = 25), with follow-up 6 and 12 months after the procedures. Results There was a significant improvement in the P-QoL score and anatomical measurements of all compartments in both groups after 12 months (p< 0.001). The anatomical cure rates in the USLS and SSLF groups, considering stage 1, were of 34.6% and 40% (anterior) respectively; of 100% both for groups (apical); and of 73.1% and 92% (posterior) respectively. The rates of adverse outcomes were of 42% (N = 11) and 36% (N = 11) for the USLS and SSLF groups respectively (p= 0.654), and those outcomes were excessive bleeding, bladder perforation (intraoperative) or gluteal pain, and urinary infection (postoperative), among others, without differences between the groups. Conclusion High cure rates in all compartments were observed according to the anatomical criterion (stage I), without differences in P-QoL scores and complications either with USLS or SSLF for the surgical treatment of accentuated POP.


Resumo Objetivo Avaliar a eficácia e os resultados do tratamento cirúrgico para prolapso de órgãos pélvicos (POP) nos estágios III e IV, por meio da técnica de fixação do ligamento sacroespinal (FLSE) ou suspensão do ligamento útero-sacro (SLUS), ao comparar os índices de cura anatômicos, subjetivos, e os parâmetros de qualidade de vida (por meio do questionário Prolapse Quality of Life [P-QoL] validado para a língua portuguesa) sob duas definições: prolapso genital Ba, Bp e C< −1 (estágio I) e Ba, Bp e C ≤ 0 (estágio II). Materiais e Métodos Após aprovação do Comitê de Ética (CAAE 0833/06) e registro no ClinicalTrials.gov (NCT 01347021), 51 pacientes foram randomizadas em dois grupos: grupo SLUS (N = 26) e (2) grupo FLSE (N = 25), com seguimento de 6 e 12 meses. Resultados Houve melhora significativa nas pontuações no P-QoL e nas medidas anatômicas de todos os compartimentos em ambos os grupos após 12 meses (p< 0,001). As taxas de cura anatômica nos grupos SLUS e FLSE , considerando o estágio 1, foram de 34,6% e 40% (anterior), respectivamente; de 100% em ambos os grupos (apical); e de 73,1% e 92% (posterior), respectivamente. As taxas de resultados adversos foram de 42% (N = 11) e 36% (N = 11), respectivamente, nos grupos SLUS e FLSE (p= 0,654), e elas foram sangramento excessivo, perfuração da bexiga (intraoperatória) ou dor glútea, e infecção urinária (pós-operatória), entre outras, sem diferenças entre os grupos. Conclusão Altas taxas de cura em todos os compartimentos foram observadas segundo critério anatômico (estágio I), sem diferença quanto às pontuações no P-QoL e às complicações tanto com SLUS quanto com FLSE para o tratamento cirúrgico de POP acentuado.

OBJETIVO: Avaliar o custo operacional do Centro de Tratamento de Queimados (CTQ) e calcular o custo paciente-dia. MÉTODO: Estudo transversal, com análise retrospectiva, quantitativa. Optou-se pelo sistema de microcusteio por absorção como método de apuração de custos através do sistema de informação de Compras Gerenciamento de Recursos Materiais e Serviço de Arquivo Médico no CTQ nos anos de 2019 e 2020. RESULTADOS: 824 internações incluindo enfermaria e Unidade Terapia Intensiva, com uma média de taxa de ocupação 73,4% e 91,4%, respectivamente. O perfil predominante foi do sexo masculino, de 0-9 anos, a maior taxa de mortalidade foi entre 60-80 anos, custo operacional R$15.547.698,81 e R$ 15.114.274,03, que gera um custo operacional mensal da unidade de R$ 1.277.582,21; os custos diretos representaram 58% e 54% nos anos de 2019 e 2020, respectivamente. Sendo assim, em média, o paciente-dia custou R$ 3.274,46 e uma taxa média de permanência de 13 dias de internação, portanto, pode-se fazer uma projeção de custos do tratamento de R$ 42.567,98, incluindo os custos indiretos e diretos. CONCLUSÕES: O custo operacional médio mensal foi de R$ 1.277.582,21 e paciente-dia teve um custo médio de R$ 3.274,46 para 13 dias de internação, projeta-se o custo total do tratamento R$ 42.567,98, incluindo os custos diretos e indiretos.


OBJECTIVE: To evaluate the operating cost of the Burn Treatment Center (CTQ) and to calculate the patient-day cost. METHODS: Cross-sectional study, with retrospective, quantitative analysis. The micro absorption costing system was chosen as a method of calculating costs through the Purchasing information system Material Resource Management and Medical Archive Service at the CTQ in 2019 and 2020. RESULTS: 824 hospitalizations including ward and Therapy Unit Intensive, with an average occupancy rate of 73.4% and 91.4%, respectively. The predominant profile was male, aged 0-9 years, the highest mortality rate was between 60-80 years, operating cost R$15,547,698.81 and R$15,114,274.03, which generates a monthly operating cost of unit of BRL 1,277,582.21, direct costs represented 58% and 54% in 2019 and 2020, respectively. Therefore, on average, the patient-day cost BRL 3,274.46 and an average hospital stay of 13 days, therefore, a treatment cost projection of BRL 42,567.98, including indirect and direct costs. CONCLUSIONS: The average monthly operating cost was R$ 1,277,582.21 and patient-day had an average cost of R$ 3,274.46 for 13 days of hospitalization, the total cost of treatment is projected to be R$ 42,567.98 including direct and indirect costs.

ABSTRACT The COVID-19 pandemic has imposed a health crisis around the world. Health professionals are frequently exposed to stressors that put them at high risk for the development or progression of disabling mental disorders, including posttraumatic stress disorder (PTSD). To understand how pandemic stressors have affected the mental health of healthcare workers, our group conducted a longitudinal and nationwide survey. We investigated the occurrence of traumatic events related to the COVID-19 pandemic and the association between exposure to these events and PTSD symptomatology. Importantly, we also investigated factors that might increase or decrease the risk for PTSD. Depression symptoms were also investigated. The results of the first wave of the project were published in a series of three articles, each focused on different risk or protective factors. The results showed that female sex, young age, a lack of adequate personal protective equipment, social isolation and a previous history of mental illness were consistent predictors of PTSD symptoms. Healthcare workers who reported high levels of peritraumatic tonic immobility, a defensive response that is involuntary, reflexive, and evoked by an intense and inescapable threat, also exhibited an increase in the probability of being diagnosed with PTSD. On the other hand, professional recognition had a negative relationship with PTSD and depression symptoms, emerging as a significant protective factor for psychological health. The identification of protective and risk factors in these situations is crucial to guide the adoption of long-term measures in work environments that will enhance the psychological health of these professionals.


RESUMO A pandemia de COVID-19 impôs uma crise de saúde no mundo. Profissionais de saúde foram frequentemente expostos a estressores que os colocam em alto risco para o desenvolvimento ou progressão de transtornos mentais incapacitantes, incluindo o transtorno de estresse pós-traumático (TEPT). Para entender como esses estressores afetaram a saúde mental desses profissionais, nosso grupo realizou um estudo longitudinal nacional. Investigamos a ocorrência de eventos traumáticos especificamente relacionados à pandemia e a associação entre a exposição a esses eventos traumáticos e a sintomatologia do TEPT. É importante ressaltar que também investigamos fatores que podem aumentar ou diminuir o risco de TEPT. Sintomas de depressão também foram investigados. Os resultados da primeira onda do projeto foram publicados em uma série de três artigos, cada um focado em diferentes fatores de risco ou proteção. Os resultados mostraram que sexo feminino, idade jovem, falta de equipamento de proteção individual adequado, isolamento social e história prévia de doença mental foram preditores consistentes de sintomas de TEPT. Os profissionais de saúde que relataram altos níveis de imobilidade tônica, uma resposta peritraumática involuntária e reflexa evocada em situações de ameaça intensa e inescapável, exibiram aumento na probabilidade de um provável diagnóstico de TEPT. Por outro lado, reconhecimento profissional teve relação negativa com sintomas de TEPT e depressão, emergindo como importante fator de proteção para a saúde mental. Identificar fatores protetores ou de risco nessas situações é fundamental para orientar a adoção de medidas de longo prazo nos ambientes de trabalho que melhorem a saúde mental desses profissionais.
