OBJECTIVES: This study aimed to improve the performance and mode of administration of a glass-reinforced hydroxyapatite synthetic bone substitute, Bonelike by Biosckin® (BL®), by association with a dextrin-based hydrogel, DEXGEL, to achieve an injectable and moldable device named DEXGEL Bone. METHODS: Twelve participants requiring pre-molar tooth extraction and implant placement were enrolled in this study. BL® granules (250-500 µm) were administered to 6 randomized participants whereas the other 6 received DEXGEL Bone. After 6 months, a bone biopsy of the grafted area was collected for histological and histomorphometric evaluation, prior to implant placement. The performance of DEXGEL Bone and BL® treatments on alveolar preservation were further analyzed by computed tomography and Hounsfield density analysis. Primary implant stability was analyzed by implant stability coefficient technique. RESULTS: The healing of defects was free of any local or systemic complications. Both treatments showed good osseointegration with no signs of adverse reaction. DEXGEL Bone exhibited increased granule resorption (p = 0.029) accompanied by a tendency for more new bone ingrowth (although not statistically significant) compared to the BL® group. The addition of DEXGEL to BL® granules did not compromise bone volume or density, being even beneficial for implant primary stability (p = 0.017). CONCLUSIONS: The hydrogel-reinforced biomaterial exhibited an easier handling, a better defect filling, and benefits in implant stability. CLINICAL RELEVANCE: This study validates DEXGEL Bone safety and performance as an injectable carrier of granular bone substitutes for alveolar ridge preservation. In recent years, the effectiveness of antimicrobials in the treatment of Pseudomonas aeruginosa infections has gradually decreased. This pathogen can be observed in several clinical cases, such as pneumonia, urinary tract infections, sepsis, in immunocompromised hosts, such as neutropenic cancer, burns, and AIDS patients. Furthermore, Pseudomonas aeruginosa causes diseases in both livestock and pets. The highly flexible and versatile genome of P. aeruginosa allows it to have a high rate of pathogenicity. The numerous secreted virulence factors, resulting from its numerous secretion systems, the multi-resistance to different classes of antibiotics, and the ability to produce biofilms are pathogenicity factors that cause numerous problems in the fight against P. aeruginosa infections and that must be better understood for an effective treatment. Infections by P. aeruginosa represent, therefore, a major health problem and, as resistance genes can be disseminated between the microbiotas associated with humans, animals, and the environment, this issue needs be addressed on the basis of an One Health approach. This review intends to bring together and describe in detail the molecular and metabolic pathways in P. aeruginosa's pathogenesis, to contribute for the development of a more targeted therapy against this pathogen.

ABSTRACT: Analysis of locomotion is often used as a measure for impairment and recovery following experimental peripheral nerve injury. Compared to rodents, sheep offer several advantages for studying peripheral nerve regeneration. In the present study, we compared for the first time, two-dimensional (2D) and three-dimensional (3D) hindlimb kinematics during obstacle avoidance in the ovine model. This study obtained kinematic data to serve as a template for an objective assessment of the ankle joint motion in future studies of common peroneal nerve (CP) injury and repair in the ovine model. The strategy used by the sheep to bring the hindlimb over a moderately high obstacle, set to 10% of its hindlimb length, was pronounced knee, ankle and metatarsophalangeal flexion when approaching and clearing the obstacle. Despite the overall time course kinematic patterns about the hip, knee, ankle, and metatarsophalangeal were identical, we found significant differences between values of the 2D and 3D joint angular motion. Our results showed that the most apparent changes that occurred during the gait cycle were for the ankle (2D-measured STANCEmax: 157±2.4 degrees vs. 3D-measured STANCEmax: 151±1.2 degrees; P<.05) and metatarsophalangeal joints (2D-measured STANCEmin: 151±2.2 degrees vs. 3D-measured STANCEmin: 162 ± 2.2 degrees; P<.01 and 2D-measured TO: 163±4.9 degrees vs. 3D-measured TO: 177±1.4 degrees; P<.05), whereas the hip and knee joints were much less affected. Data and techniques described here are useful for an objective assessment of altered gait after CP injury and repairin an ovine model.


RESUMO: A análise da locomoção é frequentemente usada como uma medida para avaliar a disfunção e sua recuperação após lesão nervosa periférica experimental. Quando comparadas com os roedores, as ovelhas oferecem várias características atrativas como modelo experimental para o estudo da regeneração nervosa periférica. Não existem estudos acerca dos resultados da locomoção após lesão e reparação do nervo periférico no modelo ovino. No presente estudo, realizámos e comparámos a cinemática bidimensional (2D) e, pela primeira vez, tridimensional (3D) do membro pélvico durante a ultrapassagem de obstáculos no modelo ovino. Este estudo teve como objetivo obter dados cinemáticos para servir de modelo para uma avaliação objetiva do movimento articular do tornozelo em estudos futuros de lesão e reparação do nervo fibular comum (FC) no modelo ovino. A estratégia usada pelas ovelhas para elevar o membro pélvico sobre um obstáculo com uma altura moderada, fixado em 10% do seu comprimento, caracteriza-se por uma flexão pronunciada do joelho, tornozelo e metatarso-falangeana ao se aproximar e ultrapassar o obstáculo. Apesar dos padrões cinemáticos do quadril, joelho, tornozelo e metatarso-falangeano terem sido idênticos, foram encontradas diferenças significativas entre os valores do movimento angular das articulações em 2D e 3D. Os nossos resultados mostram que as mudanças mais aparentes que ocorreram durante o ciclo da marcha foram nas articulações do tornozelo (em 2DSTANCEmax: 157±2.4 graus vs. em 3D STANCEmax: 151±1.2 graus; P<.05) e metatarso-falangeana (em 2D STANCEmin: 151±2.2 graus vs. em 3D STANCEmin: 162 ± 2.2 graus; P<.01 e em 2D TO: 163±4.9 graus vs. em 3D TO: 177±1.4 graus; P<.05), enquanto as articulações do quadril e do joelho foram muito menos afetadas. É provável que os dados e técnicas descritas aqui sejam úteis para uma avaliação objetiva das alterações na marcha após lesão e reparação do PC no modelo ovino.

Chitosan, a natural polysaccharide, has been previously proposed as an elicitor in plants to prevent pathogen infections. The present study aimed to analyze the effect of chitosan solution and chitosan nanoparticles treatment applied on the grapevine variety Sousão with respect to the phenolic composition, antioxidant potential and antibacterial activity of its individual grape components. Grapevine plants of selected lines were sprayed with chitosan solution and chitosan nanoparticles, and ethanolic extracts of stems, seeds and skins were prepared from grapevines treated and not treated with chitosan. Total phenolic, anthocyanin and tannin contents were studied, and the identification of the individual phenolic compounds was performed by HPLC-DAD. The antimicrobial susceptibility method was performed using the Kirby-Bauer disc diffusion method against multidrug-resistant bacteria. Overall, there was small increase in the concentration of phenolic compounds, antioxidant and antimicrobial activities in grape components treated with chitosan solution. Seed extracts showed the highest antioxidant and antimicrobial activities. The studied individual components obtained from chitosan-treated grapevines could represent an added value due to the increased antioxidant and antibacterial potentials. The phenolic compounds found in components may be used in food and pharmaceutical industries as natural food preservers and antibiotic adjuvants.

Graphene and its derivatives, such as graphene oxide, represent the greatest potential materials in terms of biomaterials due to their excellent physical-chemical and biological properties. Thus, the present study has developed an integrative literature review in order to evaluate the capacity of graphene oxide to replace metal biomaterials currently used in Dentistry. For this purpose, LILACS, SciELO and PubMED databases were evaluated, with the following descriptors: graphene, biomaterials and Odontology, adapted for each database used. Firstly, the descriptors were searched separately and, later, the Boolean operator AND was used to define a search strategy. The articles were selected according to the following inclusion criteria: articles in Portuguese, English and Spanish, published and indexed in the databases, in the last ten years, with texts available in full and regarding the topic under study. A total of 14 scientific papers were found and 10 of them were selected for this review. After the critical reading of each article, it was possible to observe that graphene and its derivates present great biocompatibility, excellent mechanical, electrical and thermic properties, high flexibility, low density and a huge potential to be explored in health science. It is also important to highlight its potential application as a coating of metal biomaterials. (AU)


O grafeno e seus derivados, como o óxido de grafeno, representam atualmente o maior potencial em termos de biomateriais, devido às suas excelentes propriedades físico-químicas e de biocompatibilidade. Assim, o presente estudo teve como premissa conduzir uma revisão integrativa da literatura de modo a verificar a capacidade do óxido de grafeno em substituir os biomateriais metálicos atualmente utilizados na Odontologia. Para isto, foram utilizadas as bases de dados Lilacs, SciELO e Pubmed, com os seguintes descritores: grafeno, biomateriais e Odontologia, adaptados para cada base de dados utilizada. Primeiramente, os descritores foram pesquisados isoladamente e, posteriormente, o operador booleano AND foi utilizado de modo a definir uma estratégia de busca. Os artigos foram selecionados de acordo com os critérios de inclusão: artigos em Português, Inglês e Espanhol, publicados e indexados nas referidas bases de dados, nos últimos dez anos, com texto disponível na íntegra e que retratassem a temática em estudo. Foram encontrados um total de 14 artigos científicos e, dez destes artigos foram selecionados para compor a revisão. A partir da leitura crítica de cada artigo, foi possível observar que o grafeno e seus derivados apresentam uma alta biocompatibilidade, notáveis propriedades mecânicas, elétricas e térmicas, alta flexibilidade, baixa densidade de massa e um enorme potencial a ser explorado para beneficiar a área da saúde. Deve-se, também, destacar sua potencial aplicação como revestimento de materiais metálicos implantáveis. (AU)

BACKGROUND: Cardiac surgeries are complex procedures aiming to re-establish coronary flow and correct valvular defects. Oxidative stress, caused by inflammation and ischemia-reperfusion injury, is associated with these procedures, increasing the risk of adverse outcomes. N-acetylcysteine (NAC) acts as an antioxidant by replenishing the glutathione stores, and emerging evidence suggests that NAC may reduce the risk of adverse perioperative outcomes. We conducted a systematic review and meta-analysis to investigate the addition of NAC to a standard of care among adult patients undergoing cardiac surgery. METHODS: We searched four databases (PubMed, EMBASE, CENTRAL, LILACS) from inception to October 2018 and the grey literaure for randomized controlled trials (RCTs) investigating the effect of NAC on pre-defined outcomes including mortality, acute renal insufficiency (ARI), acute cardiac insufficiency (ACI), hospital length of stay (HLoS), intensive care unit length of stay (ICULoS), arrhythmia and acute myocardial infarction (AMI). Reviewers independently screened potentially eligible articles, extracted data and assessed the risk of bias among eligible articles. We used the GRADE approach to rate the overall certainty of evidence for each outcome. RESULTS: Twenty-nine RCTs including 2,486 participants proved eligible. Low to moderate certainty evidence demonstrated that the addition of NAC resulted in a non-statistically significant reduction in mortality (Risk Ratio (RR) 0.71; 95% Confidence Interval (CI) 0.40 to 1.25), ARI (RR 0.92; 95% CI 0.79 to 1.09), ACI (RR 0.77; 95% CI 0.44 to 1.38), HLoS (Mean Difference (MD) 0.21; 95% CI -0.64 to 0.23), ICULoS (MD -0.04; 95% CI -0.29 to 0.20), arrhythmia (RR 0.79; 95% CI 0.52 to 1.20), and AMI (RR 0.84; 95% CI 0.48 to 1.48). LIMITATIONS: Among eligible trials, we observed heterogeneity in the population and interventions including patients with and without kidney dysfunction and interventions that differed in route of administration, dosage, and duration of treatment. This observed heterogeneity was not explained by our subgroup analyses. CONCLUSIONS: The addition of NAC during cardiac surgery did not result in a statistically significant reduction in clinical outcomes. A large randomized placebo-controlled multi-centre trial is needed to determine whether NAC reduces mortality. REGISTRATION: PROSPERO CRD42018091191.

ABSTRACT BACKGROUND: A high-quality electronic search is essential for ensuring accuracy and comprehensiveness among the records retrieved when conducting systematic reviews. Therefore, we aimed to identify the most efficient method for searching in both MEDLINE (through PubMed) and EMBASE, covering search terms with variant spellings, direct and indirect orders, and associations with MeSH and EMTREE terms (or lack thereof). DESIGN AND SETTING: Experimental study. UNESP, Brazil. METHODS: We selected and analyzed 37 search strategies that had specifically been developed for the field of anesthesiology. These search strategies were adapted in order to cover all potentially relevant search terms, with regard to variant spellings and direct and indirect orders, in the most efficient manner. RESULTS: When the strategies included variant spellings and direct and indirect orders, these adapted versions of the search strategies selected retrieved the same number of search results in MEDLINE (mean of 61.3%) and a higher number in EMBASE (mean of 63.9%) in the sample analyzed. The numbers of results retrieved through the searches analyzed here were not identical with and without associated use of MeSH and EMTREE terms. However, association of these terms from both controlled vocabularies retrieved a larger number of records than did the use of either one of them. CONCLUSIONS: In view of these results, we recommend that the search terms used should include both preferred and non-preferred terms (i.e. variant spellings and direct/indirect order of the same term) and associated MeSH and EMTREE terms, in order to develop highly-sensitive search strategies for systematic reviews.
