RESUMO

Creation of a system of palliative care in the Russian Federation is a pressing problem of public health, which at present is going through the first stage of development. According to WHO estimations, at least 75% cancer patients need palliative care. The system of medical care to cancer patients functioning in countries with well-developed economy (United Kingdom, USA, Canada, Australia) intends equal distribution of resources between radical treatment and palliative care (including analgesia) and reflects the actual requirements of patients. The scope of care should be extended from symptomatic treatment to comprehensive (medical, psychological, social, and spiritual) care of patients and their families.

In the anterior horns of the rats exposed to a 14 day space mission and 8-11 hrs of earth gravity environments postflight there occurred morphologic changes pointing to the motoneuron hypofunction which was expressed to a greater extent in motoneurons at C3-C4 level than at L4-L5 level. As the time of the rat stay under Earth gravity conditions following 14-day space mission is increased up to 48-51 hrs the structural changes in the motoneuron-glia capillary system at the L4-L5 level is indicative of an activation of spinal cord motoneurons. The 14 day head-down suspension (HDS) of the rats results in the morphologic changes in anterior horns which are an indication of hyperfunction of motoneurons in cervical enlargement and hypofunction of motoneurons in lumbar enlargement of the spinal cord. An increase of HDS time up to 32 days intensifies the morphologic changes in lumbar enlargement motoneurons. A lesser extent of morphologic changes manifestation in the motoneuron-glia-capillary system at L4-L5 level in the rats following a 14-day HDS as compared to the rats exposed to 14-day space mission is indicative of more slower development of motoneuron hypofunction under Earth gravity conditions as opposed to space environments.

RESUMO

The article generalizes 17-year experience in the organization of surgical service for patients with disturbed rhythm and conductivity of the heart in Leningrad and the northwest region of the USSR. The work of the city center of surgery of arrhythmias is analysed. Prospective calculations were made. Basically new approaches to determining the regular number of persons on the staff and the number and type or operative interventions are suggested.

RESUMO

Under analysis were results of the outpatient aid to patients in the center of surgery during 5 years. Every year the consultative and ambulatory aid was given to 1200 patients. A conclusion was made that organization of the city consultative-diagnostic center for patients requiring surgical treatment of arrhythmia with a special staff and equipment for performing electrophysiological and other investigations, organization of the zone consultation units in large polyclinics for electrophysiological investigations will improve the quality and efficiency of treatment of patients with complex disorders of the heart rhythm and conduction.
