OBJECTIVE: To validate an educational material on diabetes through an expert consensus for its implementation into a web site. MATERIAL AND METHODS: An observational study was carried out in a group of health professionals, for which an educational material was developed for patients with diabetes. Topics included nutrition, physical exercise, control indicators, complications, pharmacological treatment, among others. The language, text and figures were focused on easy comprehension, additionally, a section of didactic activities to be answered by the patient with diabetes at the end of each module was included. To evaluate the educational material by health professionals, an instrument was designed and validated. Once all the educational material was available, each of the modules was sent by e-mail to at least three clinical experts in the assigned topic, as well as the instrument for the evaluation of the module. RESULTS: Thirty-seven experts were included in the study, 76% rated the educational modules evaluated as highly adequate, while only 24% rated them as adequate. The instrument used obtained a good level of internal consistency, with a Cronbach's alpha coefficient of 0.92. In the dimensions of the instrument, the lowest Cronbach's alpha score was that of "call-to-action", with a value of 0.71. CONCLUSION: The diabetes educational material was rated as highly appropriate by the clinical experts. The developed instrument has an adequate content validity, as well as a good level of internal consistency.

Magnetic antiperovskites, having chiral noncollinear antiferromagnetic ordering, have shown remarkable properties that range from negative thermal expansion to anomalous Hall effects. Nevertheless, details on the electronic structure, related to the oxidation states and the octahedral center's site effects, are still scarce. Here, we show a theoretical study, based on first-principles calculations in the framework of density-functional theory (DFT), on the electronic properties associated with the nitrogen site effects on the structural, electronic, magnetic, and topological degrees of freedom. Thus, we show that the nitrogen vacancy increases the value of the anomalous Hall conductivity and retains the chiral Γ4g antiferromagnetic ordering. Moreover, we reveal, based on the Bader charges and the electronic structure analysis, the negative and positive oxidation states of the Ni- and Mn-sites, respectively. This is in agreement with the expected A3α+Bß-Xδ- oxidation states to satisfy charge neutrality in antiperovskites, but the negative charge is rare for transition metals. Finally, we extrapolate our findings on the oxidation states to several Mn3BN compounds, showing that the antiperovskite structure is an ideal platform to encounter negative oxidation states for metals sitting at the corner B-sites.

INTRODUCTION: PrEP awareness in Latin America has been poorly characterized, with studies in Brazil, Mexico, and Peru highlighting awareness of 65% among gay, bisexual and other men who have sex with men (MSM). We assessed the association between higher risk of HIV infection, indicative of PrEP eligibility, and PrEP awareness among MSM from these countries. METHODS: This was a secondary analysis of a web-based survey advertised on social media platforms from March-June 2018 in Brazil, Mexico and Peru. Eligible individuals were cisgender MSM, ≥18 years old, HIV negative or of unknown status, who lived in these countries, and provided informed consent. Higher risk of HIV infection was defined as having 10 or more points in the HIV Risk Index for MSM (HIRI-MSM). We used multivariable Poisson regression models to calculate adjusted prevalence ratios (aPR) testing the association between higher risk for HIV and PrEP awareness. RESULTS: After exclusions, 19,457 MSM were included in this analysis. In Brazil, 53.8% were classified as higher risk for HIV, 51.9% in Mexico, and 54.2% in Peru. Higher risk for HIV was minimally associated with PrEP awareness among those in Brazil (aPR 1.04, 95% CI 1.01, 1.06), but no such association was observed in Mexico or Peru. Having more than a high school education, high income, daily use of geosocial networking (GSN) applications, and substance use were associated with PrEP awareness. CONCLUSION: Higher risk of HIV infection was associated with increased PrEP awareness in Brazil. However, this association was weak indicating that PrEP awareness could be strengthened with further prevention efforts. In the remaining countries, results were non-conclusive between risk and awareness. Interventions to increase PrEP awareness are paramount to increase PrEP willingness and uptake and in turn prevent new HIV infections. Social media platforms could play an important role to achieve this goal.

BACKGROUND: The aim of this study was to compare a commercial dosimetry workstation (PLANET® Dose) and the dosimetry approach (GE Dosimetry Toolkit® and OLINDA/EXM® V1.0) currently used in our department for quantification of the absorbed dose (AD) to organs at risk after peptide receptor radionuclide therapy with [177Lu]Lu-DOTA-TATE. METHODS: An evaluation on phantom was performed to determine the SPECT calibration factor variations over time and to compare the Time Integrated Activity Coefficients (TIACs) obtained with the two approaches. Then, dosimetry was carried out with the two tools in 21 patients with neuroendocrine tumours after the first and second injection of 7.2 ± 0.2 GBq of [177Lu]Lu-DOTA-TATE (40 dosimetry analyses with each software). SPECT/CT images were acquired at 4 h, 24 h, 72 h and 192 h post-injection and were reconstructed using the Xeleris software (General Electric). The liver, spleen and kidneys masses and TIACs were determined using Dosimetry Toolkit® (DTK) and PLANET® Dose. The ADs were calculated using OLINDA/EXM® V1.0 and the Local Deposition Method (LDM) or Dose voxel-Kernel convolution (DK) on PLANET® Dose. RESULTS: With the phantom, the 3D calibration factors showed a slight variation (0.8% and 3.3%) over time, and TIACs of 225.19 h and 217.52 h were obtained with DTK and PLANET® Dose, respectively. In patients, the root mean square deviation value was 8.9% for the organ masses, 8.1% for the TIACs, and 9.1% and 7.8% for the ADs calculated with LDM and DK, respectively. The Lin's concordance correlation coefficient was 0.99 and the Bland-Altman plot analysis estimated that the AD value difference between methods ranged from - 0.75 to 0.49 Gy, from - 0.20 to 0.64 Gy, and from - 0.43 to 1.03 Gy for 95% of the 40 liver, kidneys and spleen dosimetry analyses. The dosimetry method had a minor influence on AD differences compared with the image registration and organ segmentation steps. CONCLUSIONS: The ADs to organs at risk obtained with the new workstation PLANET® Dose are concordant with those calculated with the currently used software and in agreement with the literature. These results validate the use of PLANET® Dose in clinical routine for patient dosimetry after targeted radiotherapy with [177Lu]Lu-DOTA-TATE.

BACKGROUND: Double Crush Syndrome (DCS) is a clinical condition that involves multiple compression sites along a single peripheral nerve. The present study aims to describe the epidemiology of DCS and surgical results. METHODS: A retrospective observational analytic study included patients with clinical diagnosis of cervical radiculopathy and carpal tunnel syndrome who underwent surgery between January 2009 and January 2019. General demographic characteristics were noted, and 3 groups were distinguished: spinal surgery, carpal tunnel release, and bimodal decompression (BD); statistical differences were analyzed between them. RESULTS: The sample comprised 32 patients. DCS prevalence was 10.29%. Mean age at presentation was 59.25±10.98 years. There was female predominance (75%). Paresthesia was the main symptom (65.6%). Post-surgical results of BD showed significant improvement in sensory nerve conduction velocity, motor nerve conduction velocity (both P=0.008), and disability on Douleur Neuropathique 4 questions, Neck Disability Index, and Boston Carpal Tunnel Questionnaire (P=0.001, 0.004, 0.008, respectively). CONCLUSIONS: Diagnosis and management of DCS are a challenge. It is necessary to determine the site with maximal compression and risk of complications to decide on treatment. If first-line surgery is adequate, proximal and distal symptomatology can be improved. To maximize success, we recommend BD, according to the present results.

OBJECTIVES: To evaluate the accuracy of an automated three-dimensional (3D) ultrasound technique for fetal intracranial measurements compared with manual acquisition. METHODS: This was a prospective observational study of patients presenting for routine anatomical survey between 18 + 0 and 22 + 6 weeks' gestation. After providing informed consent, each patient underwent two consecutive ultrasound examinations of the fetal head, one by a sonographer and one by a physician. Each operator obtained manual measurements of the biparietal diameter (BPD), head circumference (HC), transcerebellar diameter (TCD), cisterna magna (CM) and posterior horn of the lateral ventricle (Vp), followed by automated measurements of these structures using an artificial intelligence-based tool, SonoCNS® Fetal Brain. Both operators repeated the automated approach until all five measurements were obtained in a single sweep, up to a maximum of three attempts. The accuracy of automated measurements was compared with that of manual measurements using intraclass correlation coefficients (ICC) by operator type, accounting for patient and ultrasound characteristics. RESULTS: One hundred and forty-three women were enrolled in the study. Median body mass index was 24.0 kg/m2 (interquartile range (IQR), 22.5-26.8 kg/m2 ) and median subcutaneous thickness was 1.6 cm (IQR, 1.3-2.0 cm). Fifteen (10%) patients had at least one prior Cesarean delivery, 17 (12%) had other abdominal surgery and 78 (55%) had an anterior placenta. Successful acquisition of the automated measurements was achieved on the first, second and third attempts in 70%, 22% and 3% of patients, respectively, by sonographers and in 76%, 16% and 3% of cases, respectively, by physicians. The automated algorithm was not able to identify and measure all five structures correctly in six (4%) and seven (5%) patients scanned by the sonographers and physicians, respectively. The ICCs reflected good reliability (0.80-0.88) of the automated compared with the manual approach for BPD and HC and poor to moderate reliability (0.23-0.50) for TCD, CM and Vp. Fetal lie, head position, placental location, maternal subcutaneous thickness and prior Cesarean section were not associated with the success or accuracy of the automated technique. CONCLUSIONS: Automated 3D ultrasound imaging of the fetal head using SonoCNS reliably identified and measured BPD and HC but was less consistent in accurately identifying and measuring TCD, CM and Vp. While these results are encouraging, further optimization of the automated technology is necessary prior to incorporation of the technique into routine sonographic protocols. © 2020 International Society of Ultrasound in Obstetrics and Gynecology.

The study of plants adapted to an extreme environment with a high concentration of iron such as Río Tinto allowed the study of important elements for the development and control of plant growth including their localization, management, and storage. The absorption, transport, and accumulation of iron were studied in different species of dicotyledons (Sarcocornia pruinosa, Salicornia patula, Arthrocnemum macrostachyum, and Halogeton sativus of the Chenopodiaceae family) and monocotyledons (Imperata cylindrica, Cynodon dactylon, and Panicum repens from the Poaceae family), all obtained from the Río Tinto banks in different sample collection campaigns. The results clearly show that phytoferritin is not observed in the chloroplast of monocotyledons, an important difference from what is observed in dicotyledons. The presence of plastids with a high concentration of iron in the sieve tubes of monocotyledons strongly suggests their possible role in the transport and accumulation of iron in these plants.

BACKGROUND AND OBJECTIVE: Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a complex multisystemic severe drug hypersensitivity reaction whose diagnosis and management are troublesome. DRESS syndrome requires management by various specialists. The correct identification of the culprit drug is essential to ensure safe future therapeutic options for the patient. There are no previous Spanish guidelines or consensus statements on DRESS syndrome. Objective: To draft a review and guidelines on the clinical diagnosis, allergy work-up, management, treatment, and prevention of DRESS syndrome in light of currently available scientific evidence and the experience of experts from multiple disciplines. METHODS: These guidelines were drafted by a panel of allergy specialists from the Drug Allergy Committee of the Spanish Society of Allergy and Clinical Immunology (SEAIC), together with other medical specialists involved in the management of DRESS syndrome and researchers from the PIELenRed consortium. A review was conducted of scientific papers on DRESS syndrome, and the expert panel evaluated the quality of the evidence of the literature and provided grades of recommendation. Whenever evidence was lacking, a consensus was reached among the experts. RESULTS: The first Spanish guidelines on DRESS syndrome are now being published. Important aspects have been addressed, including practical recommendations about clinical diagnosis, identification of the culprit drug through the Spanish pharmacovigilance system algorithm, and the allergy work-up. Recommendations are provided on management, treatment, and prevention. Algorithms for the management of DRESS in the acute and recovery phases have been drawn up. Expert consensus-based stepwise guidelines for the management and treatment of DRESS syndrome are provided.

Objetivo: Explorar la práctica pedagógica docente en la formación para el cuidado de enfermería en la Universidad Nacional Toribio Rodríguez de Mendoza de Amazonas. Material y método: Estudio cualitativo, exploratorio, descriptivo: la muestra la conformaron 21 informantes. La recolección de información fue por medio de entrevista semi estructurada. El análisis siguió un proceso de codificación abierta, axial y selectiva hasta obtener categorías emergentes. Resultados: Se identificaron las siguientes categorías: I) Convivencia y relaciones de cuidado/descuido en la formación de enfermería; II) (Des)articulación teórico- práctica en la enseñanza/aprendizaje del cuidado de enfermería: hacia la integración docente-asistente; III) Complementariedad docente/estudiante para la construcción de conocimiento y estrategias didácticas. Conclusiones: La pedagogía para el cuidado de enfermería se evidencia en un espacio de cuidado/descuido, demandan integración docencia-asistencia y se motivan para el fortalecimiento de competencias pedagógico didácticas, a través del aprendizaje basado en problemas, casos hipotéticos, laboratorios vivenciales y sociodramas e incluir como ejes trasversales el cuidado y la investigación.


Objective: To explore the pedagogical practice in nursing care formation among teachers of the Universidad Nacional Toribio Rodríguez de Mendoza de Amazonas. Method and materials: This is a qualitative, exploratory and descriptive study, with a sample of 21 informants. Data were gathered using semi-structured interviews. The analysis followed an open, axial, and selective coding process, until emerging categories were identified. Results: The following categories emerged: I) Co-living and care/negligence relationships in nursing formation; II) (Lack of) theory-practice articulation in teaching/learning nursing care: towards the teacher-assistant integration; III) Teacher/student complementarity in the construction of knowledge and didactical strategies. Conclusions: Pedagogy in nursing care is evidenced within a care/negligence space, and thus, it is necessary to strengthen the integration among teaching and assistance improving the pedagogical competencies through methodologies such as learning based on problem solving, presentation of hypothetical cases, use of laboratories, including those for high-fidelity simulation, among others.

PURPOSE: Personalized computational simulations of the heart could open up new improved approaches to diagnosis and surgery assistance systems. While it is fully recognized that myocardial fiber orientation is central for the construction of realistic computational models of cardiac electromechanics, the role of its overall architecture and connectivity remains unclear. Morphological studies show that the distribution of cardiac muscular fibers at the basal ring connects epicardium and endocardium. However, computational models simplify their distribution and disregard the basal loop. This work explores the influence in computational simulations of fiber distribution at different short-axis cuts. METHODS: We have used a highly parallelized computational solver to test different fiber models of ventricular muscular connectivity. We have considered two rule-based mathematical models and an own-designed method preserving basal connectivity as observed in experimental data. Simulated cardiac functional scores (rotation, torsion and longitudinal shortening) were compared to experimental healthy ranges using generalized models (rotation) and Mahalanobis distances (shortening, torsion). RESULTS: The probability of rotation was significantly lower for ruled-based models [95% CI (0.13, 0.20)] in comparison with experimental data [95% CI (0.23, 0.31)]. The Mahalanobis distance for experimental data was in the edge of the region enclosing 99% of the healthy population. CONCLUSIONS: Cardiac electromechanical simulations of the heart with fibers extracted from experimental data produce functional scores closer to healthy ranges than rule-based models disregarding architecture connectivity.

Resumen: Objetivo: Comparar la eficacia clínica y la seguridad de la terapia de microfracturas (MF) versus implantación de condrocitos autólogos en membrana (MACI) en el manejo de lesiones condrales de rodilla ≥ 3 cm2 y el seguimiento a 12 meses postratamiento. Material y métodos: Se realizó un estudio de cohorte retrospectiva, de Enero de 2016 a Diciembre de 2017. Se incluyeron pacientes con una o varias lesiones condrales en rodilla ≥ 3 cm2 para comparar la terapia MF versus MACI para la reparación de lesión condral. Se realizaron valoraciones clínicas y funcionales previas al tratamiento quirúrgico y 12 meses posteriores, con medición de los arcos de movimiento, escala EVA, Oxford e índice de Lequesne. Resultados: Se incluyeron 12 pacientes en MF y 12 pacientes en MACI. La lesión más frecuente se localizó en la patela en ocho pacientes (67%). Se demostró incremento en los arcos de movimiento, así como mejoría en la comparación entre el nivel basal y en el seguimiento a 12 meses: en EVA, MF mostró 48.4% y MACI 57.5% (p ≤ 0.05); escala de Oxford: MF 32.65% y MACI 51.04% (p ≤ 0.05); índice de Lequesne: MF 40.12% y MACI 50%. Se presentaron dos casos de derrame articular en MACI, que se resolvieron con la realización de artrotomías. Conclusión: En este estudio se demostró mejoría significativa en MACI con alivio del dolor, funcionalidad y arcos de movimiento en comparación con el tratamiento de MF en lesiones ≥ 3 cm2 del cartílago articular de rodilla después de un año de seguimiento.


Abstract: Objective: To compare the clinical efficacy and safety of microfracture therapy (MF) versus implantation of autologous chondrocytes (MACI) in the management of chondral lesions of the knee ≥ 3 cm2 and follow up to 12 months post treatment. Material and methods: A retrospective cohort study was conducted from January 2016 to December 2017. Patients with one or more chondral lesions in knee ≥ 3 cm2 were included to compare MF versus MACI therapy for the repair of chondral lesion. Clinical and functional evaluations were carried out prior to the surgical treatment and 12 months later, with measurement of the range of motion, EVA, Oxford scale and Lequesne index. Results: Twelve patients were included in MF and 12 patients in MACI. The most frequent lesion was located in the Patella in eight patients (67%). It showed an increase in the arcs of motion, as well as improvement in the comparison between baseline and follow-up at 12 months: in EVA, MF demonstrated 48.4% and MACI 57.5% (p ≤ 0.05); Oxford scale: MF 32.65% and MACI 51.04% (p ≤ 0.05); index of Lequesne: MF 40.12% and MACI 50%. Two cases of joint effusion were presented in MACI, which were resolved with the realization of arthrotomies. Conclusion: In this study, significant improvement was demonstrated in MACI with pain relief, functionality, and range of motion compared to the treatment of MF in lesions ≥ 3 cm2 of the articular cartilage of the knee after one year of follow-up.

Resumen: Introducción: En México se realizó el primer trasplante de menisco en 2002. El objetivo fue describir la evolución clínica de pacientes con dolor asociado al síndrome postmeniscectomía que fueron sometidos a trasplante de menisco alogénico (MAT), con bloques óseos vía artroscópica con seguimiento a un año. Material y métodos: Se realizó un estudio de cohorte retrospectiva con seguimiento a un año; se incluyeron pacientes con dolor en interlínea articular secundario a meniscectomía parcial o total, entre 15 y 55 años de edad, de ambos sexos, que fueron sometidos a MAT. La evolución clínica se evaluó con EVA, SF-36 y escala de Lysholm; los resultados se describen en frecuencias, porcentajes y valor de p significativa < 0.05. Resultados: Se incluyeron nueve pacientes; ocho hombres y una mujer con edad de 31.3 años (16-52), el seguimiento fue de 16.6 meses (12-23). La EVA prequirúrgica fue de 8.5 (6-10) y al año de seguimiento el promedio de esta escala fue de 1.25 (0-5) (p < 0.001). En el seguimiento a un año posterior al MAT, la escala de Lysholm fue en promedio de 91 (68-100) y la SF-36 fue en promedio de 79.17 (65-92.92); tres pacientes regresaron a su práctica deportiva previa a la lesión. Conclusión: Éste es el primer reporte en México del seguimiento a un año del MAT; se demostró mejoría clínica significativa (p < 0.001) con remisión del dolor en la rodilla con meniscectomía previa y en algunos casos el retorno a la práctica deportiva.

RESUMO

ABSTRACT This work presents the design, analysis and implementation of a mechatronic System focused on helping to improve Rehabilitation therapies to patients affected by some Radial Nerve Injuries (SYRR-NERI). The device was developed with the aim of finding simple solutions for the different subsystems that form the SYRR-NERI. The mechatronic system basically consists of two coupled four-bar mechanisms with a Proportional-Integral-Derivative (PID) controller in their respective drive motors to regulate the rotational velocity. In addition, each mechanism has touch sensors in order to limit its movement. The implementation of the control algorithm and the logic to perform the synchronized movements is performed by a microcontroller. The result is an affordable and functional prototype which needs to be tested in real situations with the help of physiotherapists and patients to obtain the necessary information to improve its performance.


RESUMEN Este trabajo presenta el diseño, análisis e implementación de un sistema mecatrónico enfocado en ayudar a mejorar las terapias de rehabilitación para pacientes afectados por algunas lesiones del nervio radial. El dispositivo fue desarrollado tratando de proponer soluciones sencillas para los diferentes subsistemas que integran el dispositivo. El sistema mecatrónico consiste básicamente en dos mecanismos acoplados de cuatro barras con un controlador Proporcional-Integral-Derivativo (PID) en sus respectivos motores impulsores para regular la velocidad angular. Además, cada mecanismo tiene sensores táctiles para limitar el movimiento. La implementación del algoritmo de control y de la lógica para realizar los movimientos sincronizados se lleva a cabo mediante un microcontrolador. El resultado es un dispositivo económicamente accesible y funcional que tiene por objetivo ser probado en situaciones reales y, con la ayuda de fisioterapeutas y pacientes, obtener la información necesaria para mejorar su desempeño.

RESUMO

En este trabajo se evaluó el efecto de las bacterias persistentes presentes en un inóculo de alta densidad de una cepa autóctona de Escherichia coli sobre la eficacia de enrofloxacina y ciprofloxacina mediante ensayos in vitro de curvas de muerte bacteriana y de determinación de la concentración preventiva de mutantes. En las curvas de muerte realizadas sobre inóculos de alta densidad, ningún antibiótico presentó actividad bactericida y solo permitieron la sobrevida de bacterias persistentes. En el ensayo para determinar la concentración preventiva de mutantes, sobre la superficie del agar de las placas con elevadas concentraciones de enrofloxacina y ciprofloxacina, las bacterias persistentes permanecieron viables sin desarrollar colonias y adoptando morfología filamentosa como una forma de adaptación y supervivencia. Se discute la utilidad clínica de las concentraciones preventivas de mutantes de enrofloxacina y ciprofloxacina sobre E. coli ya que, estas elevadas concentraciones permitirían la sobrevida de una sub-población de bacterias persistentes originando un reservorio biológico que podría dar origen a infecciones crónicas y a favorecer la emergencia de mutantes resistentes.


This work evaluated the effect of persister cells present in a high inocula size of a wild strain of Escherichia coli on the efficacy of enrofloxacin and ciprofloxacin by in vitro time-kill curve assays and mutant prevention concentration testing. In time-kill curves performed with high inocula size, no antibiotics showed bactericidal activity, but only allowed the survival of persister cells. In the assay to determine the mutant prevention concentration, on the surface of agar plates containing high enrofloxacin and ciprofloxacin concentrations, persister cells remained viable and without bacterial colonies development and adopting filamentous morphology as a form of adaptation and survival. The clinical usefulness of mutant prevention concentrations of enrofloxacin and ciproflocxacin against Escherichia coli is discussed, as these high concentrations would allow the survival of a sub-population of persister cells originating a biological reservoir that could give rise to chronic infections and favor the emergence of resistant mutants.

La actividad antibacteriana de los sueros de bovino y de búfalo fue evaluada in vitro de manera indirecta con dos reacciones de inmunohemólisis: (i) un ensayo de hemólisis-hemoaglutinación sobre eritrocitos no sensibilizados de conejo y (ii) un ensayo de cinética de eritrocitos no sensibilizados de conejo frente a concentraciones crecientes de suero. La actividad bactericida de los sueros fue evaluada in vitro cuantificando la reducción de desarrollo de un inóculo de Escherichia coli en medio de cultivo enriquecido con suero de ambas especies. En el ensayo de hemólisis-hemoaglutinación, la hemólisis máxima se observó hasta la dilución 1:16 del suero de ambas especies. El mayor porcentaje de hemólisis se obtuvo con el suero de búfalo (84,7 ± 9,71%) respecto del suero bovino (71,0 ± 5,05%). Sin embargo no se hallaron diferencias en las diluciones de suero necesarias para obtener el 50% de la hemólisis total, siendo estas de 10,2 ± 2,48 para el suero de bovinos y de 11,8 ± 2,18 para el suero de búfalo. El suero de búfalo redujo el desarrollo bacteriano en un 69,8% respecto del 47,2% obtenido por el suero de bovino. No obstante estos resultados, se necesitarán estudios adicionales para corroborar estos hallazgos in vivo.


The antibacterial activity of serum of bovine and buffaloes was evaluated in vitro indirectly by two immunohemolysis assays: (i) an unsensitized rabbit blood cells hemolysis-hemagglutination assay and (ii) and a hemolytic-kinetic of unsensitized rabbit blood cells in function of increasing serum concentrations. The serum bactericidal activity was evaluated in vitro by quantifying the reduction of the development of an Escherichia coli inoculum in a culture medium enriched with serum of both species. In the hemolysis-hemagglutination assay, the máximum haemolysis was observed until a 1:16 serum dilution for both species. The highest percentage of hemolysis was obtained with buffalo serum (84.7 ± 9.71%) with respect to bovine serum (71.0 ± 5.05%). However, no differences were found in the serum dilutions necessary to obtain 50% of the total hemolysis, being these 10.2 ± 2.48 for the serum of cattle and 11.8 ± 2.18 for the serum of buffalo. Buffalo serum reduced bacterial growth by 69.8% compared to 47.2% obtained by bovine serum. Despite these results, additional studies will be needed to corroborate these findings in vivo.

OBJECTIVE: To describe our experience in first-trimester screening for trisomies 21 and 18 firstly by the combined test alone and secondly by cell-free (cf) DNA testing contingent on the results from a previously performed combined test. METHODS: Women with singleton pregnancies attending Torrejon University Hospital in Madrid, Spain, from November 2011 to January 2016, were screened for trisomy (T)21 and T18 by the combined test at 11-13 weeks. Before the introduction of cfDNA testing, women at high risk (>1 in 250) were offered invasive testing (IT) and from January 2015 they were offered cfDNA test as well as IT. RESULTS: Combined test was performed in 6011 pregnancies. The risk was high in 202 (3.4%) cases. There was complete follow-up for 5507 (91.6%) pregnancies. Detection rate (DR) for T21 was 83.3% (15/18) and 100% (4/4) for T18. Additionally, 2/2 (100%) cases of T13 and 2/2 (100%) cases of triploidy were also detected. False positive rate (FPR) was 3.2% (174/5488). The introduction of this contingent model was followed by a 73% reduction on the IT rate in the high-risk group, from 76.3% to 20.8%. CONCLUSION: Contingent screening for trisomies 21 and 18 by cfDNA testing at 11-13 weeks is feasible and has a lower IT rate than combined testing alone.
