RESUMO

Mice navigate an odor plume with a complex spatiotemporal structure in the dark to find the source of odorants. This article describes a protocol to monitor behavior and record Ca 2+ transients in dorsal CA1 stratum pyramidale neurons in hippocampus (dCA1) in mice navigating an odor plume in a 50 cm x 50 cm x 25 cm odor arena. An epifluorescence miniscope focused through a GRIN lens imaged Ca 2+ transients in dCA1 neurons expressing the calcium sensor GCaMP6f in Thy1-GCaMP6f mice. The paper describes the behavioral protocol to train the mice to perform this odor plume navigation task in an automated odor arena. The methods include a step-by-step procedure for the surgery for GRIN lens implantation and baseplate placement for imaging GCaMP6f in CA1. The article provides information on real-time tracking of the mouse position to automate the start of the trials and delivery of a sugar water reward. In addition, the protocol includes information on using of an interface board to synchronize metadata describing the automation of the odor navigation task and frame times for the miniscope and a digital camera tracking mouse position. Moreover, the methods delineate the pipeline used to process GCaMP6f fluorescence movies by motion correction using NorMCorre followed by identification of regions of interest with EXTRACT. Finally, the paper describes an artificial neural network approach to decode spatial paths from CA1 neural ensemble activity to predict mouse navigation of the odor plume. SUMMARY: This protocol describes how to investigate the brain-behavior relationship in hippocampal CA1 in mice navigating an odor plume. This article provides a step-by-step protocol, including the surgery to access imaging of the hippocampus, behavioral training, miniscope GCaMP6f recording and processing of the brain and behavioral data to decode the mouse position from ROI neural activity.

Introducción: Los factores reproductivos se asocian con cáncer de mama. Actualmente se estudia el comportamiento según subtipos moleculares. Objetivo: Establecer la prevalencia de estos subtipos y su asociación con factores reproductivos en mujeres atendidas en centros del nororiente colombiano. Método: Estudio observacional de corte transversal, en mujeres con cáncer de mama subtipos luminales y HER2 durante 2012-2021. Se indagaron variables sociodemográficas, factores reproductivos y estadio tumoral. Resultados: En total, 347 pacientes cumplieron criterios de elegibilidad, correspondiendo a luminal A el 49,8% (intervalo de confianza del 95% [IC95%]: 44,5-55,1), a luminal B el 29,1% (IC95%: 24,3-33,9) y a HER2 el 15,5% (IC95%: 11,7-19,4). Las mujeres con tumores de mama luminal B tenían más riesgo de tener estadios localmente avanzados (odds ratio [OR]: 1,83; IC95%: 1,11-3,01; p = 0,02). Agrupando los subtipos luminales frente a HER2 se encontró que el 40,72% de las pacientes con subtipos luminales no habían lactado, frente al 69,71% con HER2 (diferencia estadísticamente significativa a favor de luminal A; OR: 1,91; IC95%: 1,02-3,53; p = 0,041). Conclusiones: La prevalencia de tumores luminales es del 84,5%. Existe asociación diferencial entre el antecedente de lactancia materna y la aparición de subtipos luminales, es decir, las mujeres que no lactaron se corresponden con mayor frecuencia con HER2. No se estableció asociación con otros factores estudiados.


Introduction: Stimulus-estrogenic factors are associated with breast cancer. Currently, the behavior according to molecular subtypes is being studied. Objective: To establish the prevalence of these subtypes and their association with reproductive factors in women attended in centers in northeastern Colombia. Method: Observational cross-sectional study in women with breast cancer subtypes luminal and HER2 during 2012 -2021. Sociodemographic variables, stimulus-estrogenic factors and tumor stage were investigated. Results: In total, 347 patients met eligibility criteria, corresponding to luminal A 49.8% (95% confidence interval [95%CI]: 44.5-55.1), luminal B 29.1% (95%CI: 24.3-33.9) and HER2 15.5% (95%CI: 11.7-19.4). Women with luminal B breast tumors were at higher risk of having locally advanced stages (odds ratio [OR]: 1.83; 95%CI: 1.11-3.01; p = 0.02). Grouping the luminal subtypes versus HER2 showed that 40.72% of patients with luminal subtypes had not lactated, compared to 69.71% HER2 (statistically significant difference in favor of luminal A; OR: 1.91; 95%CI: 1.02-3.53; p = 0.041). Conclusions: The prevalence of luminal tumors is 84.5%. There is a differential association between the history of breastfeeding and the appearance of luminal subtypes, i.e., women who did not breastfeed are more likely to have HER2. No association was established with other factors studied.

BACKGROUND: Cardiovascular diseases are the leading cause of death globally. There are six cardiovascular risk factors: diabetes, hypertension, hypercholesterolemia, overweight, sedentary lifestyle and smoking. Due to the low attendance of healthy people in the health system, the use of telemedicine can influence the acquisition of a heart-healthy lifestyle. OBJECTIVE: this systematic review and meta-analysis aimed to determine the effectiveness of telemedicine and e-health in reducing cardiovascular risk. METHODS: A systematic review and meta-analysis were carried out using the PubMed, Scopus, Cinhal and WOS databases. Randomized controlled studies between 2017 and 2022 in which telemedicine was used to reduce any of the risk factors were included. The methodological quality was assessed using the "PEDro" scale. RESULTS: In total, 763 studies were obtained; after the review, 28 target articles were selected and finally grouped as follows: 13 studies on diabetes, six on hypertension, seven on obesity and two on physical activity. For all of the risk factors, a small effect of the intervention was seen. CONCLUSIONS: although the current evidence is heterogeneous regarding the statistically significant effects of telemedicine on various cardiovascular risk factors, its clinical relevance is undeniable; therefore, its use is recommended as long as the necessary infrastructure exists.

Osteoporosis has been defined as the silent disease of the 21st century, becoming a public health risk due to its severity, chronicity and progression and affecting mainly postmenopausal women and older adults. Osteoporosis is characterized by an imbalance between bone resorption and bone production. It is diagnosed through different methods such as bone densitometry and dual X-rays. The treatment of this pathology focuses on different aspects. On the one hand, pharmacological treatments are characterized by the use of anti-resorptive drugs, as well as emerging regenerative medicine treatments such as cell therapies and the use of bioactive hydrogels. On the other hand, non-pharmacological treatments are associated with lifestyle habits that should be incorporated, such as physical activity, diet and the cessation of harmful habits such as a high consumption of alcohol or smoking. This review seeks to provide an overview of the theoretical basis in relation to bone biology, the existing methods for diagnosis and the treatments of osteoporosis, including the development of new strategies.

Introduction: The detection of the human papillomavirus (HPV) through the combination of the HPV test and other techniques such as cytology has impacted the detection and timely treatment of lesions associated with cervical cancer. Objective: To estimate the budgetary impact of the strategy of early detection of HPV with DNA test genotyping with reflex cytology versus conventional cytology in women aged 30 to 65 years attending the cervical cancer screening program at a health benefit managing entity in Colombia. Materials and methods: Using a decision tree and a Markov model, the clinical implications and direct costs of screening, diagnosis, and treatment were estimated in a cohort of women. The analysis considered two screening cycles and their annual costs. The data on the prevalence of clinical results and the costs were taken from the health managing entity. The information on the progression, persistence, and regression of the health states were taken from the ATHENA study. Results: The screening scheme with the HPV test, genotyping, and reflex cytology compared to conventional cytology was cost-saving. The average cost per screening cycle with the HPV test was estimated at COP $ 129,201,363 and with cytology at COP $ 186,309,952, i.e., a saving of COP $ 57,108,589 (30.7%). Conclusion: The implementation of the screening strategy under evaluation suggests prospective savings derived from the early detection of health states associated with the development of cervical cancer.


Introducción. La detección del virus del papiloma humano mediante la combinación de la prueba de HPV y otras técnicas como la citología, ha demostrado su eficacia en el diagnóstico y tratamiento oportuno de lesiones asociadas con el cáncer de cuello uterino. Objetivo. Estimar el impacto presupuestal de la estrategia de detección temprana del HPV mediante la prueba de genotipificación combinada con la citología en comparación con la citología convencional, en mujeres de 30 a 65 años participantes en el programa de tamizaje de cáncer de cuello uterino en una Entidad Administradora del Plan de Beneficios en salud (EAPB) en Colombia. Materiales y métodos. Utilizando un árbol de decisiones y un modelo de Markov, se estimaron las implicaciones clínicas y los costos directos anuales de dos ciclos de tamizaje, diagnóstico y tratamiento, en una cohorte de mujeres. Las prevalencias de los resultados clínicos y los costos se tomaron de la base de datos de una EAPB y la información de la progresión, persistencia y regresión de los estados de salud provinieron del estudio ATHENA. Resultados. El esquema de tamizaje con la prueba de HPV, la genotipificación y la citología resultó en un ahorro de costos comparado con la citología convencional. El costo promedio por ciclo de tamizaje con la prueba de HPV se estimó en COP $129'201.363 y con la citología en COP $186'309.952, es decir, un ahorro de COP $57'108.589 (30,7 %). Conclusión. La implementación de la estrategia de tamizaje evaluada sugiere que habría ahorros derivados de la detección temprana de los estados de salud asociados con el desarrollo de cáncer de cuello uterino.

Resumen Introducción: La tiroiditis linfocítica crónica o tiroiditis de Hashimoto es la causa más frecuente de hipotiroidismo adquirido en la edad pediátrica. Se ha detectado una asociación entre concentraciones bajas de 25-hidroxivitamina D (25OH vitamina D) y el desarrollo de la enfermedad. El objetivo de este trabajo fue describir las concentraciones de 25OH vitamina D en pacientes de 5 a 18 años con diagnóstico de tiroiditis linfocítica crónica en tres centros de consulta externa de endocrinología pediátrica enMedellín,Colombia. Métodos: Se llevó a cabo un estudio observacional de corte transversal con recolección retrospectiva de la información. Se evaluaron características sociodemográficas, particularidades del diagnóstico, presencia de comorbilidad y frecuencia de deficiencia de vitamina D. Resultados: Se incluyeron 60 pacientes. La concentración de 25OH vitamina D fue suficiente en el 65% de los casos según los criterios de Institute of Medicine (IOM) y en el 10% de los casos según los criterios de la Endocrine Society. Los valores de calcio y fósforo sérico fueron normales en el 53% y el 45% de los pacientes, respectivamente. Todos los pacientes presentaron concentraciones normales de magnesio y paratohormona. No se encontraron diferencias en el análisis exploratorio al comparar la concentración de 25OH vitamina D, de anticuerpos tiroideos y el volumen tiroideo. Conclusiones: En esta población con tiroiditis linfocítica crónica no se encontró una mayor prevalencia de deficiencia de 25OH vitamina D según los criterios del IOM y de la Endocrine Society en comparación con datos previos de la población general. En el análisis exploratorio no se encontraron diferencias estadísticamente significativas.


Abstract Background: Chronic lymphocytic thyroiditis or Hashimoto's thyroiditis is the most frequent cause of acquired hypothyroidism in children. An association between low levels of 25-hydroxyvitamin D (25OH vitamin D) and the development of the disease have been detected. The aim of this study was to describe 25OH vitamin D levels in patients aged 5 to 18 years with a diagnosis of Hashimoto's thyroiditis in three pediatric endocrinology outpatient centers in Medellín, Colombia. Methods: We conducted a cross-sectional observational study with retrospective data collection. We evaluated the sociodemographic characteristics, diagnoses, presence of comorbidities, and frequency of vitamin D deficiency. Results: Sixty patients were included. The 25OH vitamin D levels were sufficient in 65% of the cases according to the Institute of Medicine (IOM) criteria and in 10% of the cases according to the Endocrine Society criteria. Serum calcium and phosphorus values were normal in 53% and 45% of the patients, respectively. All patients had normal magnesium and parathyroid hormone levels. No differences were found in the exploratory analysis when comparing 25OH vitamin D levels, thyroid antibody levels, and thyroid volume. Conclusions: In this chronic lymphocytic thyroiditis population, we did not find an increased prevalence of vitamin D deficiency according to IOM or the Endocrine Society criteria compared with previous data from the general population. No statistically significant differences were found in the exploratory analysis.

Resumen ANTECEDENTES: El cáncer de mama es la principal neoplasia en incidencia y mortalidad en mujeres. Los subtipos moleculares: luminal A, luminal B, HER2 y triple negativo tienen un pronóstico y tasas de supervivencia diferentes. En la bibliografía está demostrada la estrecha asociación entre los factores estímulo estrogénicos y el cáncer de mama en general, aunque las diferencias no son claras debido a los subtipos moleculares. OBJETIVO: Revisar la bibliografía reciente y describir la relación entre los subtipos moleculares del cáncer de mama y los factores reproductivos. METODOLOGÍA: Búsqueda en las bases de datos PubMed y LILACS con los términos MeSH y DeCS: neoplasias de la mama, subtipos moleculares, factores de riesgo, factores reproductivos. Se buscó la asociación entre los antecedentes de paridad, edad al primer embarazo y el antecedente de lactancia materna con los subtipos moleculares de cáncer de mama: luminal A, luminal B y HER2. RESULTADOS: Se obtuvieron 366 artículos y se eliminaron 352 por: duplicidad en títulos y resúmenes, sin pertinencia para el tema, protocolos de investigación que no estudiaran la asociación entre factores estímulo estrogénicos con los subtipos moleculares de cáncer de mama. Al final, el análisis se hizo con 14 artículos. CONCLUSIONES: Los tumores con receptores hormonales positivos se asociaron con: edad mayor al primer embarazo, mayor tiempo entre la menarquia y el primer embarazo a término y edad avanzada en el último embarazo. Factores protectores para tumores luminales y HER2 puro: lactancia materna y multiparidad.


Abstract BACKGROUND: Breast cancer represents the main neoplasia in incidence and mortality in women, it can be divided into molecular subtypes (luminal A, luminal B, HER2 and triple negative) having a differential prognosis and survival rates. There is literature that demonstrates the strong association between estrogenic stimulating factors and breast cancer, but the existing differences by molecular subtypes are not clear. OBJECTIVE: To review the recent literature and describe the relationship found between molecular subtypes of breast cancer and estrogenic stimulating factors. METHODOLOGY: Search in the PubMed and LILACS databases with the MeSH and DeCS terms: breast neoplasms, molecular subtypes, risk factors, reproductive factors, looking for the association between the antecedents of parity, age at first pregnancy and history of breastfeeding with molecular subtypes of breast cancer (luminal A, luminal B and HER2). RESULTS: A total of 366 results were obtained, excluding 352 articles when evaluating duplicity, titles and abstracts, articles without relevance to the topic, research protocols and articles that did not study the association of estrogenic stimulating factors with molecular subtypes of breast cancer, resulting in 14 articles. CONCLUSIONS: Hormone receptor-positive tumors were found to be associated with older age at first pregnancy, longer time between menarche and first term pregnancy and older age at last pregnancy. Breastfeeding and multiparity were found as protective factors for luminal tumors and pure Her2.

Resumen El cáncer de mama es la patología maligna más frecuente en la población femenina, su tratamiento ha evolucionado progresivamente en dirección a la conservación del seno a través del desarrollo de la cirugía Oncoplástica de seno, que consiste en manejo conservador con mastectomía parcial y reconstrucción inmediata. Las pacientes que presentan de manera concomitante cáncer de seno y macromastia, representan un reto mayor en el tratamiento debido a la dificultad de generar simetría con el seno contralateral y que además presentan sintomatología específica relacionada con la macromastia que disminuye su calidad de vida. El objetivo de este artículo es reportar el caso de una paciente que se maneja de manera exitosa y simultánea el cáncer de seno y macromastia, mediante cirugía oncoplástica de seno derecho y en el mismo tiempo quirúrgico, mamoplastia de reducción bilateral, obteniendo adecuado control oncológico y mejoría significativa en su calidad de vida medida por cuestionario BREAST-Q. Summary Breast cancer is the most frequent malignant pathology in the female population. Its treatment has progressively evolved in the direction of breast conservation through the development of oncoplastic breast surgery, which consists of conservative management with partial mastectomy and immediate reconstruction. Patients who present breast cancer concomitantly with macromastia, represent a greater challenge in treatment due to the difficulty of generating symmetry with the contralateral breast; they also present specific symptoms related to macromastia that diminishes their quality of life. The objective of this article, is to report the case of a patient with breast cancer and macromastia, who is successfully and simultaneously managed with oncoplastic surgery of the right breast and bilateral reduction mammoplasty, obtaining adequate oncological control and significant improvement in her quality of life measured by BREAST-Q questionnaire. Vagus nerve stimulation has shown many benefits for disease therapies but current approaches involve imprecise electrical stimulation that gives rise to off-target effects, while the functionally relevant pathways remain poorly understood. One method to overcome these limitations is the use of optogenetic techniques, which facilitate targeted neural communication with light-sensitive actuators (opsins) and can be targeted to organs of interest based on the location of viral delivery. Here, we tested whether retrograde adeno-associated virus (rAAV2-retro) injected in the heart can be used to selectively express opsins in vagus nerve fibers controlling cardiac function. Furthermore, we investigated whether perturbations in cardiac function could be achieved with photostimulation at the cervical vagus nerve. Viral injection in the heart resulted in robust, primarily afferent, opsin reporter expression in the vagus nerve, nodose ganglion, and brainstem. Photostimulation using both one-photon stimulation and two-photon holography with a GRIN-lens incorporated nerve cuff, was tested on the pilot-cohort of injected mice. Changes in heart rate, surface electrocardiogram, and respiratory responses were observed in response to both one- and two-photon photostimulation. The results demonstrate feasibility of retrograde labeling for organ targeted optical neuromodulation.

BACKGROUND: This study provides information on cancer incidence and mortality in a Colombian population during 2010-2014, based on the data and methodology of the Population-based Cancer Registry of Antioquia to facilitate the implementation of cancer control strategies. METHODS: This is a descriptive study of cancer incidence and mortality in a population, residing in the urban area of the municipality of Medellin. The cancers included in the study are those prioritized in the cancer control plan for Colombia (PDCC-cancers). The collection, processing and systematization of the data were performed in accordance with internationally standardized parameters for population cancer registries. Incidence and mortality rates were calculated by gender, age and tumor location. RESULTS: During 2010-2014 there were 22,379 new cancer cases recorded in the urban area of the municipality of Medellin, of which 43.5% corresponded to the PDCC-cancers. During the same period, 14,922 cancer deaths were reported, 23.5% related to the PDCC-cancers, 53.5% in women. Prostate cancer and breast cancer were the principal cause of morbidity in men and women, respectively, and lung cancer was the principal cause of death for both sexes. CONCLUSION: Cancer is a health problem for the population of Medellin. It is necessary to emphasize research and monitor risk factors, the health response and the capacity of the health provider network when facing the growing demand caused by this epidemic.


ANTECEDENTES: Es un estudio sobre la incidencia y mortalidad por cáncer en una población colombiana (2010-2014), a partir de los datos y la metodología del Registro Poblacional de Cáncer (RPC) de Antioquia para facilitar la implementación de estrategias para el control del cáncer. MÉTODOS: Estudio descriptivo, cuya población objeto reside en el área urbana del municipio de Medellín. Se incluyeron para el análisis los cánceres priorizados por el plan de control de cáncer de Colombia (cánceres-PDCC). La recolección, el procesamiento y la sistematización de los datos se realizaron de acuerdo con los parámetros estandarizados internacionalmente para los RPC. Las tasas de incidencia y mortalidad se calcularon por sexo, edad y ubicación del tumor. RESULTADOS: En el periodo 2010-2014 se registraron 22,379 casos nuevos de cáncer en el área urbana del municipio de Medellín, 43.5% corresponde a los cánceres-PDCC. Para el mismo periodo se reportaron 14,922 muertes por cáncer, 23.5% pertenecen a cánceres-PDCC, 53.5 %, ocurrieron en mujeres. El cáncer de próstata y el cáncer de mama fueron la primera causa de morbilidad por cáncer en hombres y mujeres respectivamente, y el cáncer de pulmón fue la principal causa de muerte por cáncer para los dos sexos. CONCLUSIÓN: El cáncer es un problema de salud para la población del municipio de Medellín. Se requiere hacer énfasis en las investigaciones y monitoreo de los factores de riesgo, así como hacer seguimiento a la respuesta sanitaria y la capacidad de la red de prestadores frente a la demanda creciente de esta epidemia.

Incidence and mortality rates were calculated by gender, age and tumor location. Results: During 2010-2014 there were 22,379 new cancer cases recorded in the urban area of the municipality of Medellin, of which 43.5% corresponded to the PDCC-cancers. During the same period, 14,922 cancer deaths were reported, 23.5% related to the PDCC-cancers, 53.5% in women. Prostate cancer and breast cancer were the principal cause of morbidity in men and women, respectively, and lung cancer was the principal cause of death for both sexes. Conclusion: Cancer is a health problem for the population of Medellin. It is necessary to emphasize research and monitor risk factors, the health response and the capacity of the health provider network when facing the growing demand caused by this epidemic.


Resumen Antecedentes: Es un estudio sobre la incidencia y mortalidad por cáncer en una población colombiana (2010-2014), a partir de los datos y la metodología del Registro Poblacional de Cáncer (RPC) de Antioquia para facilitar la implementación de estrategias para el control del cáncer. Métodos: Estudio descriptivo, cuya población objeto reside en el área urbana del municipio de Medellín. Se incluyeron para el análisis los cánceres priorizados por el plan de control de cáncer de Colombia (cánceres-PDCC). La recolección, el procesamiento y la sistematización de los datos se realizaron de acuerdo con los parámetros estandarizados internacionalmente para los RPC. Las tasas de incidencia y mortalidad se calcularon por sexo, edad y ubicación del tumor. Resultados: En el periodo 2010-2014 se registraron 22,379 casos nuevos de cáncer en el área urbana del municipio de Medellín, 43.5% corresponde a los cánceres-PDCC. Para el mismo periodo se reportaron 14,922 muertes por cáncer, 23.5% pertenecen a cánceres-PDCC, 53.5 %, ocurrieron en mujeres. El cáncer de próstata y el cáncer de mama fueron la primera causa de morbilidad por cáncer en hombres y mujeres respectivamente, y el cáncer de pulmón fue la principal causa de muerte por cáncer para los dos sexos. Conclusión: El cáncer es un problema de salud para la población del municipio de Medellín. Se requiere hacer énfasis en las investigaciones y monitoreo de los factores de riesgo, así como hacer seguimiento a la respuesta sanitaria y la capacidad de la red de prestadores frente a la demanda creciente de esta epidemia.

RESUMEN Objetivo Caracterizar los modelos teóricos que han fundamentado las investigaciones empíricas sobre salud mental positiva desde el momento en que aparece el concepto en el campo de la salud hasta la actualidad. Métodos Se realizó un proceso sistemático de búsqueda de literatura, publicada en las bases de datos PubMed, Ebsco (Academic Search Complete, ERIC, Fuente Académica, MasterFILE premier, MedicLatina, Medline y Psychology and Behavioral Sciences Collection), Science Direct, Psicodoc, Springer Link, Taylor and Francys, Wiley Online Library, DOAJ (Directory of Open Access Journals), Redalyc, Scielo, Ovid, Embase y Proquest (Psychology Journals, Nursing and Allied Health Source, Health and Medical Complete y Social Science Journals). Se utilizó como criterio de búsqueda el descriptor “salud mental positiva”. Resultados De los 51 estudios consultados, 84% presentan un enfoque cuantitativo y también 84% están publicados en inglés y fueron realizados entre los años 2000 y 2014. Se identificaron cinco categorías en la utilización del concepto salud mental positiva: la ausencia de enfermedad como indicador de salud mental positiva, el modelo de Jahoda, el desarrollo de la escala de Lluch, la utilización del concepto de bienestar como sinónimo de salud mental positiva y un reciente interés por diseñar escalas de medición. Conclusiones La salud mental positiva no debe ser entendida como la antinomia del trastorno mental, la ausencia de enfermedad o la simple suma de atributos personales. Es importante avanzar en el desarrollo de modelos conceptuales que servirán como fundamento para el abordaje de la salud mental desde un enfoque centrado en la promoción de la salud.


ABSTRACT Objective Characterize the theoretical models that have underpinned empirical research on the concept of positive mental health from the time it first emerged in the field of health up to the present. Methods A systematic search of the literature was conducted in PubMed, EBSCO (including Academic Search Complete, ERIC, Academic Source, MasterFILE Premier, MedicLatina, MEDLINE, and the Psychology and Behavioral Sciences Collection), Science Direct, Psicodoc, Springer Link, Taylor & Francis, Wiley Online Library, Directory of Open Access Journals (DOAJ), Redalyc, SciELO, Ovid, Embase, and ProQuest (including Health and Medical Complete, the Nursing and Allied Health Source, Psychology Journals, and Social Science Journals). The search criterion was the descriptor “positive mental health.” Results Of 51 studies consulted, 84% used a quantitative approach; 84% were published in English; and the same percentage were conducted between 2000 and 2014. The concept of positive mental health has been applied in essentially five different ways: as the absence of disease; as the subject of the Jahoda model; as a combination of factors on the Lluch scale; as a synonym of well-being; and as part of more complex scales of measurement. Conclusions Positive mental health should not be viewed as the opposite of a mental disorder, the absence of disease, or the sum of a given set of personal conditions. It is important to move forward in the development of conceptual models that will serve as a basis for approaching mental health from the perspective of health promotion.

Growing evidence suggests that the main olfactory epithelium contains a subset of olfactory sensory neurons (OSNs) responding to pheromones. One candidate subpopulation expresses the calcium activated cation channel TRPM5 (transient receptor potential channel M5). Using GFP driven by the TRPM5 promoter in mice, we show that this subpopulation responds to putative pheromones, urine, and major histocompatibility complex peptides, but not to regular odors or a pheromone detected by other species. In addition, this subpopulation of TRPM5-GFP+ OSNs uses novel transduction. In regular OSNs, odorants elicit activation of the cyclic nucleotide-gated (CNG) channel, leading to Ca2+ gating of Cl- channels; in TRPM5-GFP+ OSNs, the Ca2+ -activated Cl- ANO2 (anoctamin 2) channel is not expressed, and pheromones elicit activation of the CNG channel leading to Ca2+ gating of TRPM5. In conclusion, we show that OSNs expressing TRPM5 respond to pheromones, but not to regular odors through the opening of CNG channels leading to Ca2+ gating of TRPM5.

OBJETIVO: analizar la evolución de la unidad de pago por capitación (UPC), los métodos y las variables existentes para su definición dentro del Sistema General de Seguridad Social en Salud colombiano, luego de su implementación entre 1995 y el 2011. METODOLOGIA: estudio de tipo observacional descriptivo de corte longitudinal. La población de referencia la constituyeron todos los documentos consultados, entre libros, artículos de revista, bases de datos y presentaciones oficiales, que contuvieran los términos claves. Todos los documentos analizados se clasificaron tomando como referencia una escala de tipo Likert con cinco niveles. RESULTADOS: se encontró que la UPC se valora como elemento indispensable para mantener el equilibrio financiero del sistema. Desde 1995 hasta el 2011, los métodos para definirla fueron variados, pasando por una definición basada solo en el equilibrio financiero y la disponibilidad de recursos hasta configurar un estudio técnico con herramientas estadísticas que la ajustara anualmente. CONCLUSIONES: el origen de la UPC puede atribuirse a la orientación del sistema hacia un esquema de mercado y a la confluencia de tres teorías básicas. Las variaciones de la UPC siempre han referido un valor similar año tras año; por ello, aunque cambie la metodología para su definición, esta seguirá en proporciones iguales si se compara con años anteriores.


OBJECTIVE: to analyze the evolution of the Capitation Payment (UPC for its Spanish name "unidad de pago por capitación") as well as the methodologies and variables for defining it that have existed in the Colombian Social Security System in Health since its implementation between 1995 and 2011. METHODOLOGY: An observational, longitudinal descriptive study. The reference population was made up of all the papers reviewed, including books, journal articles, databases, and official presentations containing the key concepts. RESULTS: It was found that the Capitation Payment (UPC) is valued as an essential element to maintain the financial balance of the health system. From 1995 to 2011 there were a number of methodologies for defining this payment. They ranged from a definition based only on the financial balance and the availability of resources, to a technical study using statistical tools to annually adjust the value of the UPC. CONCLUSIONS: The origin of the UPC can be attributed to the system’s orientation toward a market scheme and to the convergence of three basic theories. The variations in the value of the UPC have always resulted in a similar value year after year; this is why even if the methodology for defining it changed, the proportions will be the same when compared to previous years.

Although it is now established that sensory neurons in both the main olfactory epithelium and the vomeronasal organ may be activated by both general and pheromonal odorants, it remains unclear what initiates sampling by the vomeronasal organ. Anterograde transport of wheat germ agglutinin-horseradish peroxidase was used to determine that adequate intranasal syringing with zinc sulfate interrupted all inputs to the main olfactory bulb but left intact those to the accessory olfactory bulb. Adult male treated mice were frankly anosmic when tested with pheromonal and non-pheromonal odors and failed to engage in aggressive behavior. Treated juvenile females failed to show puberty acceleration subsequent to exposure to bedding from adult males. Activation of the immediate early gene c-Fos and electrovomeronasogram recording confirmed the integrity of the vomeronasal system in zinc sulfate-treated mice. These results support the hypothesis that odor detection by the main olfactory epithelium is required to initiate sampling by the vomeronasal system.

An unusual property of the olfactory system is that sensory input at the level of the first synapse in the olfactory bulb takes place at two mirror-image glomerular maps that appear identical across the axis of symmetry. It is puzzling how two identical odor maps would contribute to sensory function. The functional units in these maps are the glomeruli, ovoid neuropil structures formed by axons from olfactory sensory neurons expressing the same olfactory receptor. Here we find that the genetically identified P2 glomeruli are asymmetric across the axis of symmetry in terms of responsiveness to urine volatiles and neuroanatomical structure. Furthermore, P2 asymmetry is modified by sensory deprivation and abolished by decreased BDNF levels. Thus, while mirror odor maps show symmetry at the macroscopic level in maps encompassing the entire surface of the olfactory bulb, they display asymmetry at the level of the single glomerulus.

The olfactory and immune systems must perform optimally in the task of recognizing thousands of molecules to ensure survival. A particularly intriguing link between these systems is that animals can smell differences in the major histocompatibility complex (MHC), a cluster of highly polymorphic genes found on human chromosome 6 and mouse chromosome 17. Two different sets of compounds found in urine have been postulated to convey information on MHC haplotype: volatile compounds (odortypes) and MHC peptides. Here we argue for complementary roles for these chemosignals.

Objetivo: Establecer el estado de salud mental de las víctimas de la violencia armada en Bojayá. Método: Descripción de trastornos mentales asociados con la experiencia vivida el 2 de mayo de 2002 en Bojayá, donde en un enfrentamiento armado murieron 119 personas y 98 resultaron heridas. Estudio de tipo analítico de casos y controles. Muestra no aleatoria de 40 civiles adultos, promedio de 35 años; 21 permanecían desplazados en Quibdó (Chocó) y 19 retornaron a Bojayá. Grupo casos conformado por 15 personas expuestas con trastorno de estrés postraumático (TEPT), y grupo control con 25 sin TEPT. Se empleó la mini international neuropsychiatric interview (MINI) un año después de los hechos. Resultados: 37% de los participantes presentó TEPT (todos retornaron a Bojayá). Los trastornos que mostraron mayor asociación con el TEPT (grupo casos) fueron la fobia social (p=0,001, RD 26,29) y trastornos por angustia (TA): TA actual (p=0,008, RD 15,33), TA alguna vez (p=0,019, RD 8,25) y agorafobia sin historia de angustia (p=0,024, RD 11,50). Sin diferencias significativas entre los grupos, el porcentaje fue alto en depresión mayor (DM) (37% casos y 38% controles), riesgo de suicidio (37% y 62%, respectivamente), trastorno de ansiedad generalizada (TAG) (47% y 38%, respectivamente). Conclusiones: Son frecuentes el TEPT, el TAG, el DM y el riesgo suicidio en las víctimas de violencia armada. Los trastornos por angustia y la fobia social pueden considerarse factores de riesgo al TEPT.


Purpose: To describe the mental health status of victims of an armed combat in the state of Chocó (Colombia). Method: A description of the mental disorders associated with a traumatic experience lived on May 2nd 2002 (Bojayá township- Colombia), as a conse– quence of an armed confrontation, resulting in 119 persons dead and 98 injured. Analytic level methodology, with case control design, was used. A non random sample of 40 civilian adults, mean age 35 years, was studied. Of these, 19 persons returned to their place of origin and 21 persons remained in Quibdó (state capital) were evaluated. The case group consisted of a total of 15 persons with Post– traumatic Stress Disorder (PTSD) and the control group included 25 persons without PTSD diagnosis. The instrument used for evaluation was the structured interview pro– tocol M.I.N.I applied one year after the tragic event. Results: Disorders that showed greater association with PTSD were: Social Phobia (p=0.001, OR 26.29), Actual Panic Disorder (p=0.008, OR 15.33), Occasional Panic Disor– der (p=0.019, OR 8.25), and Agoraphobia without Panic History (p=0.024, OR=11.50). The prevalence of was high for the following disorders, without significant differences between groups: Major Depression (MD) (37% cases and 38% controls), Suicide Risk (37% y 62% respectively), Generalised Anxiety Disorder (GAD) (47% and 38% respectively). Conclusions: Victims of armed violence who suffer PTSD are at high risk of presenting other anxiety disorders such as social pho- bias, GAD, and panic attacks.

Odor quality is thought to be encoded by the activation of partially overlapping subsets of glomeruli in the olfactory bulb (odor maps). Mouse genetic studies have demonstrated that olfactory sensory neurons (OSNs) expressing a particular olfactory receptor target their axons to a few individual glomeruli in the bulb. While the specific targeting of OSN axons provides a molecular underpinning for the odor maps, much remains to be understood about the relationship between the functional and molecular maps. In this article, we ask the question whether intensive training of mice in a go/no-go operant conditioning odor discrimination task affects odor maps measured by determining c-fos up-regulation in periglomerular cells. Data analysis is performed using a newly developed suite of computational tools designed to systematically map functional and molecular features of glomeruli in the adult mouse olfactory bulb. This suite provides the necessary tools to process high-resolution digital images, map labeled glomeruli, visualize odor maps, and facilitate statistical analysis of patterns of identified glomeruli in the olfactory bulb. The software generates odor maps (density plots) based on glomerular activity, density, or area. We find that training up-regulates the number of glomeruli that become c-fos positive after stimulation with ethyl acetate.

OBJECTIVE: The authors examined the feasibility of using olfactory receptor neurons from living patients to test whether calcium signaling is altered in a neuronal cell population in bipolar disorder. METHOD: Ratiometric fluorescence photomicroscopy was used to assess basal and stimulus-induced changes in intracellular calcium levels in biopsy-derived olfactory receptor neurons from seven euthymic patients with bipolar disorder who were medication-free, 10 euthymic patients with bipolar disorder who were treated with mood stabilizers, and 17 age- and sex-matched comparison subjects without bipolar disorder. RESULTS: Olfactory receptor neurons from the seven medication-free patients responded to stimuli predominantly with decreases in intracellular calcium, unlike those from the seven matched healthy subjects. Olfactory receptor neurons from patients treated with mood stabilizers were less likely to respond to stimulation than olfactory receptor neurons from medication-free patients. CONCLUSIONS: This study demonstrates the feasibility of using olfactory receptor neurons to examine alterations in intracellular signaling in neuronal cells from living patients. Our results, although based on a small number of subjects, suggest that altered intracellular calcium signaling in olfactory receptor neurons may be a trait of bipolar disorder.
