RESUMEN Paciente primigesta de 27 años, sin antecedentes médicos de importancia y con un apropiado control prenatal, quien recibió atención por parto vaginal a las 39 semanas de gestación con anestesia epidural. Una hora después del parto, presentó cefalea holocraneana a predominio derecho, tratada con antiinflamatorios y relajantes muscular por indicación del servicio de neurología. Horas después de su alta, al tercer día posparto, presentó convulsiones tónico-clónicas bilaterales. Por un examen de orina con proteinuria (+) en tira reactiva y elevaciones discretas de la presión arterial, se solicitó un conteo de proteínas en 24 horas, con valores en 1094,5 mg (valor normal: 0-140). La resonancia magnética con contraste solicitada durante su admisión fue normal, recibiendo tratamiento con fenitoína y sulfato de magnesio durante su hospitalización. Fue dada de alta al quinto día, con controles posteriores por consultorio externo, sin cefalea, proteinuria y/o hipertensión.


ABSTRACT A 27-year-old primigravida patient without a relevant medical history and appropriate prenatal control received attention for vaginal delivery at 39 weeks of gestation. One -hour later, she experienced holocranial headache with right predominance, treated with anti-inflammatories and muscle relaxants by the indication of a neurologist. Hours after her discharge, on the third day post-partum, she developed bilateral tonic-clonic seizures.. Following a urine test in the emergency room with proteinuria (+) in a dipstick, we tested 24-hour protein count in 1094 mg (normal values 0-140). Magnetic resonance with contrast at admission was normal. She received Phenytoin and Magnesium Sulfate during her hospitalization. The evolution was favorable, and he was discharged at five days with ambulatory controls in the medical office without headache, proteinuria, and/or hypertension.

Introduction: The use of combined immunotherapy could increase non-severe and severe cardiac events in patients with cancer. To examine the occurrence of severe cardiac adverse events of combined immunotherapy compared to single immunotherapy, we analysed 4 electronic databases from inception to August 2021. Material and methods: We selected randomized controlled trials (RCTs) comparing combined versus single immunotherapy, for the treatment of melanoma, oesophagogastric cancer, renal cell carcinoma, and non-small cell lung cancer. Pre-defined combined immunotherapy included monoclonal antibodies against programmed cell death 1 (PD-1 inhibitors) plus against cytotoxic T lymphocyte antigen 4 (CTLA-4 inhibitors) or against programmed cell death ligand 1 (PD-L1 inhibitors) plus CTLA-4 inhibitors. The pooled risk ratios (RR) with their 95% confidence intervals (CI) were estimated using a random-effects model. Results: Four RCTs involving 1581 patients were included, with a follow-up time between 18 and 39 months. The use of combined immunotherapy in comparison with single immunotherapy was not associated with an increased risk of severe cardiac adverse events: acute coronary syndromes (RR = 1.76, 95% CI: 0.29-10.83, very low certainty of evidence (CoE)), myocardial infarction (RR = 3.93, 95% CI: 0.44-35.39, very low CoE), heart failure (RR = 2.99, 95% CI: 0.61-14.79, very low CoE), and atrial fibrillation (RR = 2.26, 95% CI: 0.62-8.16, very low CoE). Conclusions: Our meta-analysis shows that the risk of severe cardiac adverse events with combined immunotherapy seems similar to single immunotherapy, but the evidence is very uncertain. Therefore, more RCTs with longer follow-ups and adequately powered to assess cardiac adverse events are needed to confirm these findings.

Introduction: The long-term effects of SARS-CoV-2 are unclear, as are the factors influencing the evolution. Objective: to assess health-related quality of life 1 year after a hospital admission due to COVID-19 and to identify factors that may influence it. Materials and methods: Retrospective observational study in a tertiary hospital from March 2021 to February 2022. Inclusion criteria: ≥18 years old and admitted for SARS-CoV-2 infection. Exclusion criteria: death, not located, refusal to participate, cognitive impairment, and language barrier. Variables: demographic data, medical history, clinical and analytical outcomes during hospital admission, treatment received, and vaccination against SARS-CoV-2 following admission. Participants were interviewed by phone 1 year after admission, using the SF-36 quality of life questionnaire. Results: There were 486 included patients. The domains yielding the lowest scores were general health (median 65%, interquartile range [IQR] 45-80), vitality (median 65%, IQR 45-80), and mental health (median 73.5%, IQR 60-100). Multivariable analysis showed that female sex and fibromyalgia/fatigue had a negative influence on all domains. Obesity was associated with worse outcomes in physical functioning, physical role, bodily pain, and vitality. Other factors associated with worse scores were an older age in physical functioning and high age-adjusted Charslon comorbidity in physical functioning and general health. Age was associated with better results in emotional role and High C-reactive protein at admission on vitality. Conclusion: One year after admission for COVID-19, quality of life remains affected, especially the domains of general health, vitality, and mental health. Factors associated with worse outcomes are female sex, fibromyalgia/chronic fatigue, and obesity.

La esclerosis sistémica sin escleroderma es una forma no común de compromiso sistémico de esta enfermedad, con ausencia de manifestaciones cutáneas. Presentamos el caso de una mujer de 71 años con índice de masa corporal (IMC) en 16,9 kg/m2, con historia de hipotiroidismo y estreñimiento crónico, quien acude a emergencia por dolor y distensión abdominal, asociado a náuseas. Se realizó una tomografía abdomino-pélvica encontrándose cámara gástrica colapsada, asas intestinales con algunos segmentos distendidos, con signos de fecalización de asa delgada. La colonoscopía describió dolicocolon severo. Se observaron placas induradas en codo derecho y en ambos glúteos. Los exámenes auxiliares: ANA 1/640 patrón centromérico, anticuerpos anticentrómero = 227 U/mL (valor normal - VN: 0-25), Anti SCL-70 = 1,9 U/mL (VN 0-25) y anti RNP = 0,1 U/mL (VN 0-25). La capilaroscopía de borde ungueal a 200x mostró megacapilares. El diagnóstico clínico fue esclerosis sistémica sin esclerodermia. Se inició tratamiento con dieta rica en fibra y nutrición parenteral por 1 semana. La evolución de la paciente fue favorable.


Systemic sclerosis sine Scleroderma is an uncommon form of systemic compromise of this disease, in the abscense of cutaneous manifestations. We present the case of a 71 years-old woman, body mass index (BMI) in 16.9 kg/m2, with history of hypothyroidism and chronic constipation, who goes to the emergency with pain, abdominal bloating, and nausea. An abdominopelvic tomography was made, reporting a collapse of the gastric chamber, some segments of intestinal loops distended with signs of fecalization in the small intestine. A colonoscopy described severe dolicocolon. We observed indurated plaques in left elbow and in both buttocks. Lab tests: ANA 1/640 with anticentromeric pattern, anticentromere antibodies = 227 U/mL (0-25), Anti-SCL-70 = 1.9 U/mL (0-25), anti-RNP = 0.1 U/mL (0-25). Peri-ungueal cappilaroscopy 200x showed mega capillaries. Clinical diagnosis was systemic sclerosis sine scleroderma. We initiated treatment with a high-fiber diet and parenteral nutrition for a week. The patient evolution was favorable.

BACKGROUND: Due to an increase in life expectancy, the prevalence of chronic diseases is also on the rise. Clinical practice guidelines (CPGs) provide recommendations for suitable interventions regarding different chronic diseases, but a deficiency in the implementation of these CPGs has been identified. The PITeS-TiiSS (Telemedicine and eHealth Innovation Platform: Information Communications Technology for Research and Information Challenges in Health Services) tool, a personalized ontology-based clinical decision support system (CDSS), aims to reduce variability, prevent errors, and consider interactions between different CPG recommendations, among other benefits. OBJECTIVE: The aim of this study is to design, develop, and validate an ontology-based CDSS that provides personalized recommendations related to drug prescription. The target population is older adult patients with chronic diseases and polypharmacy, and the goal is to reduce complications related to these types of conditions while offering integrated care. METHODS: A study scenario about atrial fibrillation and treatment with anticoagulants was selected to validate the tool. After this, a series of knowledge sources were identified, including CPGs, PROFUND index, LESS/CHRON criteria, and STOPP/START criteria, to extract the information. Modeling was carried out using an ontology, and mapping was done with Health Level 7 Fast Healthcare Interoperability Resources (HL7 FHIR) and Systematized Nomenclature of Medicine Clinical Terms (SNOMED CT; International Health Terminology Standards Development Organisation). Once the CDSS was developed, validation was carried out by using a retrospective case study. RESULTS: This project was funded in January 2015 and approved by the Virgen del Rocio University Hospital ethics committee on November 24, 2015. Two different tasks were carried out to test the functioning of the tool. First, retrospective data from a real patient who met the inclusion criteria were used. Second, the analysis of an adoption model was performed through the study of the requirements and characteristics that a CDSS must meet in order to be well accepted and used by health professionals. The results are favorable and allow the proposed research to continue to the next phase. CONCLUSIONS: An ontology-based CDSS was successfully designed, developed, and validated. However, in future work, validation in a real environment should be performed to ensure the tool is usable and reliable.

PURPOSE: The strict lockdown implemented due the COVID-19 pandemic is generating a great impact on wellbeing and health-related quality of life (HRQoL) in people with cancer. We aimed to evaluate the efficacy and feasibility of an online home-based exercise intervention performed during a lockdown period analysing its effects on body composition, physical fitness, and HRQoL in breast cancer survivors. METHODS: Fifteen women with breast cancer receiving hormonal therapy (55.5 ± 6.7 years) were included in the study. The exercise intervention consisted of two weekly sessions of remotely supervised functional training (60 min per day) and two weekly sessions of unsupervised aerobic training (20-30 min/session; 60-85% of maximum heart rate) for a total of 16 weeks. DXA absorptiometry was used for the assessment of body composition. Functional assessment included cardiorespiratory fitness (CRF) by Rockport walking test, upper and lower body strength (grip strength, arm curl test, and chair stand test), walking speed (brisk walking test), and agility (8-foot up-and-go test). The HRQoL was evaluated with the QLQ-BR23 questionnaire. The adherence to the intervention was measured as the percentage of online classes attended. RESULTS: Rate of adherence for the online exercise intervention was 90 ± 17%. The exercise intervention induced significant (p < 0.05) improvements in physical fitness: CRF (+ 9%), right arm and lower limb strength (+ 10% and + 18%, respectively) and lower limbs lean mass (+ 2% and + 3.5% for left and right leg, respectively). CONCLUSION: This feasibility study suggests that an online home-based exercise intervention during COVID-19 lockdown could improve physical fitness and body composition in breast cancer survivors even in a context of heightened concern for future health.

BACKGROUND: Sheehan's syndrome is a hypopituitarism secondary to necrosis of the pituitary gland caused by massive postpartum bleeding. The presentation is frequently amenorrhea and agalactia; hypoglycemia as a solitary presentation is uncommon. CASE REPORT: 68-year-old female with a history of postpartum hemorrhage 35 years ago. She had two episodes of hypoglycemia. During her hospital stay, panhypopituitarism was detected and the empty sella was confirmed by magnetic resonance imaging. CONCLUSIONS: It is recommended that medical personnel suspect Sheehan´s syndrome in any woman with nonspecific hypoglycemia and no history of chronic degenerative disease.


ANTECEDENTES: El síndrome de Sheehan es un hipopituitarismo secundario a necrosis de la glándula hipófisis causado por una hemorragia posparto. La presentación frecuentemente es como amenorrea y agalactia; la hipoglucemia como presentación solitaria es infrecuente. CASO CLÍNICO: Mujer de 68 años con antecedente de hemorragia posparto hace 35 años. Cursó con dos episodios de hipoglucemia. Durante su hospitalización se detecta panhipopituitarismo y se corrobora por resonancia magnética la silla turca vacía. CONCLUSIONES: Es recomendable que el personal médico sospeche un síndrome de Sheehan ante cualquier mujer con hipoglucemia inespecífica y sin antecedente de enfermedad crónica degenerativa.

OBJECTIVES: This study investigated the associations of chronic diseases with changes in lifestyle and health behaviours in older people following the coronavirus disease 2019 (COVID-19) lockdown in Spain and compared the differences in changes over time. METHODS: 1,092 participants (80.3±5.6 years; 66.5% female) from 2 Spanish cohorts were included. Telephone-based questionnaires were conducted to evaluate lifestyle and health risk behaviours at the end of lockdown and 7 months post-lockdown. Participants were classified as having physician-diagnosed chronic diseases based on self-reported data. Cox proportional models adjusted for major confounders were used. RESULTS: Compared to those without the corresponding chronic diseases, older people with hypertension were less likely to report increased alcohol consumption (hazard ratio [HR], 0.73; 95% confidence interval [CI], 0.55 to 0.99). Pulmonary diseases were associated with lower risks of increased sedentary time (HR, 0.58; 95% CI, 0.39 to 0.86) and worsened sleep quality (HR, 0.56; 95% CI, 0.36 to 0.87), while cardiovascular diseases were associated with a lower risk of decreased sedentary time (HR, 0.58; 95% CI, 0.38 to 0.88). Depression was linked to a higher likelihood of improved diet quality (HR, 1.53; 95% CI, 1.00 to 2.36). Cancer pacients were less likely to have worsened sleep quality (HR, 0.44; 95% CI, 0.22 to 0.89) but more likely to have reduced their frequency of social contact (HR, 2.05; 95% CI, 1.05 to 3.99). CONCLUSIONS: Older people with chronic diseases showed beneficial changes in lifestyle and health risk behaviours after the COVID-19 lockdown. In particular, older people with hypertension, pulmonary disease, and cancer tended to make beneficial lifestyle and health behaviour changes. However, older people with cardiovascular disease and depression engaged in more health risk behaviours.

Objective: To know the most frequent acute cardiovascular complications in a Peruvian population of oncologic patients. Materials and methods: Retrospective, descriptive study of oncologic patients treated at Clinica Delgado between January 2014 and December 2019, from which the subgroup with the seven most prevalent cancers at the national level was selected according to information from Globocan 2018. Additionally, we evaluated the epidemiology of patients with cardiovascular complications that conditioned their hospitalization or were detected during this, calculating their cardiovascular risk according to Hermann and SCORE risk scales. Results: Forty-four patients had complications; 27 (61.4%) were hospitalized due to acute cardiovascular causes. The mean age of this subgroup was 69.88 years (SD 12.77), and 22 (81.5%) were older than 60 years. Fourteen (51.9%) were male. According to the Hermann scale, 33.3% had intermediate-risk and 14.9% had a high or very high risk. According to the SCORE scale, 62.97% had an intermediate-risk and 7.40% high risk. The most common acute cardiovascular complications were deep vein thrombosis and ischemic stroke (66.65%). One patient (3.7%) reported previous cardiovascular disease. Four patients (14.8%) had a fatal outcome during hospitalization. The median length of hospitalization was five days. Conclusions: We present the cases of acute cardiovascular complications in a population of oncologic patients and their vascular risk according to Hermann and SCORE scales. The most common complications were deep vein thrombosis (48.14%), stroke (18.51%), and myocardial infarction (14.81%).

BACKGROUND: Physical fitness and body composition are important health indicators; nevertheless, their combined pattern interrelationships and their association with mortality are poorly investigated. METHODS: This longitudinal study is part of the Spanish EXERNET-Elder project. Person-months of follow-up were calculated from the interview date, performed between June 2008 and November 2009, until the date of death or censoring on March 2018 (whichever came first). In order to be included, participants had to fulfill the following criteria: (a) be older than 65 years, (b) live independently at home, (c) not suffer dementia and/or cancer, and (d) have a body mass index above 18.5. Body fat and weight were assessed by a bioelectrical impedance analyzer. Fitness was measured with the Senior Fitness and the one-leg static balance tests. The Spanish Death Index was consulted for the death's identification. Cluster analysis was performed to identify Fat-Fit patterns and traditional cut-points and percentiles to create the Fat-Fit groups. Cox proportional hazards regression models were used to calculate the hazard ratios (HRs) of death in clustered Fat-Fit patterns and in traditional Fat-Fit groups. RESULTS: A total of 2299 older adults (76.8% of women) were included with a baseline mean age of 71.9 ± 5.2 years. A total of 196 deaths (8.7% of the sample) were identified during the 8 years of follow-up. Four clustered Fat-Fit patterns (Low fat-Fit, Medium fat-Fit, High fat-Unfit, and Low fat-Unfit) and 9 traditional Fat-Fit groups emerged. Using the Low fat-Fit pattern as the reference, significantly increased mortality was noted in High fat-Unfit (HR: 1.68, CI: 1.06-2.66) and Low fat-Unfit (HR: 2.01, CI: 1.28-3.16) groups. All the traditional Fit groups showed lower mortality risk when compared to the reference group (obese-unfit group). CONCLUSION: Physical fitness is a determinant factor in terms of survival in community-dwelling older adults, independently of adiposity levels.

Objetivo: Determinar factores asociados a mortalidad en pacientes con enfermedad grave por SARS-CoV-2. Material y métodos: Estudio retrospectivo en pacientes mayores de 18 años hospitalizados por enfermedad grave (saturación periférica de Oxígeno < 93% o compromiso radiológico pulmonar > 30%) y prueba molecular positiva en hisopado nasofaríngeo; en el hospital Rebagliati entre marzo y mayo de 2020. Se revisó historia clínica electrónica, resultados de análisis (de laboratorio y radiológicos) al ingreso y tratamiento recibido. Se realizó estadística bivariada y multivariada utilizando regresión de cox. Resultados: Se incluyeron 337 pacientes, de los cuales 215 fallecieron (63.8%). Fueron de sexo masculino 72.1%, edad promedio 63.6 años (DS 15.3), tiempo de enfermedad 7.4 días, estancia hospitalaria 10.3 días y 48.8% ingresó a ventilación mecánica invasiva. El análisis multivariado encontró significancia en edad > 60 años (HR 1.76, IC95% 1.26-2.45), antecedente de diabetes mellitus tipo 2 (HR 1.63, IC95% 1.14-2.32), compromiso radiológico pulmonar > 75% (HR 1.87, IC95% 1.37-2.56), leucocitosis > 14000 x mm3 (HR 1.59, IC95% 1.15-2.20), albúmina sérica < 4.0 g/dL (HR 2.49, IC95% 1.30-4.75) y lactato > 1.5 mmol/L (HR: 1.766 IC95% 1.25-2.49). Conclusiones: La edad mayor de 60 años, diabetes mellitus tipo 2, compromiso pulmonar severo, leucocitosis, albúmina baja y lactato elevado al ingreso hospitalario, son factores asociados a mortalidad en pacientes con enfermedad grave por SARS-CoV-2.

Materials and methods: Retrospective study in patients older than 18 years hospitalized with severe disease (peripheral oxygen saturation <93% or radiological lung involvement > 30%) and a positive RT-PCR nasopharyngeal swab; in the Rebagliati hospital between March and May 2020. We reviewed electronic medical record, results of admission studies (laboratory and radiological) and treatment received. Bivariate and multivariate analysis were performed using cox regression. Results: 337 patients were studied, 215 of them died (63.8%). 72.1% were male, mean age was 63.6 years (SD 15.3), time of disease evolution was 7.4 days, hospital stay 10.3 days, and 48.8% were on mechanical ventilation. Multivariate analysis found significance in age > 60 years (HR 1.76, IC95% 1.26-2.45), history of type 2 Diabetes (HR 1.63, IC95% 1.14-2.32), pulmonary involvement > 75% (HR 1.87, IC95% 1.37-2.56), leukocytes > 14000 x mm3 (HR 1.59, IC95% 1.15-2.20), serum albumin < 4.0 g/dL(HR 2.49, IC95% 1.30-4.75) and lactate > 1.5 mmol/L(HR: 1.766 IC95% 1.25-2.49). Conclusions: Age older than 60 years, type 2 diabetes mellitus, severe pulmonary involvement, leukocytosis, low albumin, and high lactate at admission are factors associated with mortality in patients with severe SARS-CoV-2 disease.

Introducción: Un adecuado abastecimiento de medicamentos en los establecimientos de salud aumentará la posibilidad de un adecuado control de la hipertensión y la diabetes. Objetivo: Determinar el desabastecimiento de antidiabéticos y antihipertensivos a nivel nacional en el contexto de la crisis por COVID-19. Material y métodos: Análisis del "Sistema integrado de suministro de medicamentos e insumos médicos quirúrgicos" (SISMED) entre el 13 de junio y 15 de julio del 2020, de acuerdo a la lista del "Petitorio nacional único de medicamentos de medicamentos esenciales" (PNUME) del Ministerio de Salud. Resultados: Entre el 16 % y 24% de las regiones están totalmente desabastecidos en al menos un antidiabético y 4 % y 96 % en al menos un antihipertensivo. El antidiabético más desabastecido fue la Metformina de 500 mg y los antihipertensivos más desabastecidos fueron Labetalol 5 mg/ml iny, Atenolol 50 mg tab y Carvedilol 6.25 mg tab. El desabastecimiento fue porcentualmente mayor en hospitales y los institutos especializados en comparación con centros y puestos de salud según región geográfica. Conclusiones: Existe un desabastecimiento de antihipertensivos y antidiabéticos en los establecimientos de salud, el cual es heterogéneo en las diferentes regiones del Perú.


Introduction: An adequate supply of medicines in health establishments will increase the possibility of adequate control of hypertension and diabetes. Objective: To determine the supply of antidiabetic and antihypertensive drugs at the national level in the context of the crisis by COVID-19. Material y methods: Analysis of the "Sistema Integrado de Suministro de Medicamentos e Insumos Médicos Quirúrgicos" (SISMED) Database, between June 13th and July 15th, 2020, according to the "National list for medicines of essential medicines" (PNUME) of Ministry of health. Results: Between 16% and 24% of the departments have a total shortage of at least one antidiabetic, and 4% and 96% of at least one antihypertensive. The most depleted antidiabetic was Metformin 500 mg, and the most depleted antihypertensive drugs were Labetalol 5 mg / ml iny, Atenolol 50 mg tab and Carvedilol 6.25 mg tab. The percentage of distribution was higher in hospitals and specialized institutes in comparison with primary health facilities. Conclusions: There is a shortage of antihypertensive and antidiabetic drugs in health centers in Peru.

Introducción: Un adecuado abastecimiento de medicamentos en los establecimientos de salud aumentará la posibilidad de un adecuado control de la hipertensión y la diabetes. Objetivo: Determinar el desabastecimiento de antidiabéticos y antihipertensivos a nivel nacional en el contexto de la etapa inicial de la pandemia por la COVID-19 en Perú. Material y métodos: Análisis del "Sistema integrado de suministro de medicamentos e insumos médicos quirúrgicos" (SISMED) entre el 13 de junio y 15 de julio del 2020, de acuerdo a la lista del "Petitorio nacional único de medicamentos de medicamentos esenciales" (PNUME) del Ministerio de Salud. Resultados: Entre el 16 % y 24% de las regiones están totalmente desabastecidos en al menos un antidiabético y entre 4 y 96 % en al menos un antihipertensivo. El antidiabético más desabastecido fue la Metformina de 500 mg y los antihipertensivos más desabastecidos fueron Labetalol 5 mg/ml iny, Atenolol 50 mg tab y Carvedilol 6,5 mg tab. El desabastecimiento fue porcentualmente mayor en hospitales y los institutos especializados en comparación con centros y puestos de salud según región geográfica. Conclusiones: Existe un desabastecimiento de antihipertensivos y antidiabéticos en los establecimientos de salud, el cual es heterogéneo en las diferentes regiones del Perú.

The most undersupplied antidiabetic was Metformin 500 mg and the most undersupplied antihypertensives were Labetalol 5 mg/ml injection, Atenolol 50 mg tab and Carvedilol 6.5 mg tab. Stock-outs were percentage-wise higher in hospitals and the specialized institutes compared to health centers and health posts according to geographic region. Conclusions: There is a shortage of antihypertensive and antidiabetic drugs in health facilities, which is heterogeneous in the different regions of Peru.

BACKGROUND: Physical fitness levels and the amount of accumulated adipose tissue (fatness) relate to current and future individuals' heath status. Nevertheless, the interrelationships of their combined patterns with polypharmacy and the types of medications consumed have not been sufficiently investigated. METHODS: This cross-sectional study was carried out in six Spanish regions between 2008 and 2009 with a sample of older community-dwelling adults (≥65 years old) without dementia or cancer. Fitness was measured with one-leg balance and senior fitness tests, as well as by measuring weight and fat mass with a bioelectrical impedance analyzer. Polypharmacy was defined as the use of five or more medications. An analysis of variance was performed for comparisons between the physical fitness and fatness patterns and the medication consumed. RESULTS: A total of 1709 elders were included in the study (72.1 ± 5.2 years). The two unfit patterns were those with the highest drug consumption. The High-Fat-Unfit pattern was the one that had the most significant consumption and had the highest percentage of polymedicated subjects. The Low-Fat-Fit pattern had a significantly lower percentage of people that did not consume any medications. The highest percentages of drug consumption in 7 of the 10 groups that were included were concentrated in the two unfit patterns. CONCLUSIONS: This study highlights the importance of fitness in older adults, as it is at least as important as the avoidance of accumulation of excess fat with respect to the consumption of a smaller number of medicines.

We used data from 3041 participants in four cohorts of community-dwelling individuals aged ≥65 years in Spain collected through a pre-pandemic face-to-face interview and a telephone interview conducted between weeks 7 to 15 after the beginning of the COVID-19 lockdown. On average, the confinement was not associated with a deterioration in lifestyle risk factors (smoking, alcohol intake, diet, or weight), except for a decreased physical activity and increased sedentary time, which reversed with the end of confinement. However, chronic pain worsened, and moderate declines in mental health, that did not seem to reverse after restrictions were lifted, were observed. Males, older adults with greater social isolation or greater feelings of loneliness, those with poorer housing conditions, as well as those with a higher prevalence of chronic morbidities were at increased risk of developing unhealthier lifestyles or mental health declines with confinement. On the other hand, previously having a greater adherence to the Mediterranean diet and doing more physical activity protected older adults from developing unhealthier lifestyles with confinement. If another lockdown were imposed during this or future pandemics, public health programs should specially address the needs of older individuals with male sex, greater social isolation, sub-optimal housing conditions, and chronic morbidities because of their greater vulnerability to the enacted movement restrictions.

El presente estudio tuvo como objetivo general determinar el grado de afectación que presenta la prueba Griess (a y b), al interior del cañón del arma de fuego tipo escopeta, marca Benelli, modelo Súper Nova, con la trasmisión de los contaminantes aceite, agua lluvia, diésel y gasolina. Surge la necesidad de indagar si los contaminantes establecidos pueden afectar la realización del procedimiento descrito en el numeral 4.1 de la guía 2DCGU-0024 "realizar procedimientos balísticos a las armas (de fuego, neumáticas, de gas, fisto, detonadoras, eléctricas, entre otras) e ingreso a los sistemas de identificación balística". La metodología utilizada fue de tipo experimental, para ello se realizó una cantidad importante de ensayos, se manipularon las soluciones A y B del reactivo de GRIESS. Como resultado se evidenció que los agentes contaminantes transferidos al interior del cañón (aceite 4 tiempos, diésel y gasolina) no afectan los resultados para la identificación de nitritos producto de la combustión durante el fenómeno del disparo, caso contrario ocurre con el agua lluvia, ya que se observa que esta afecta la elaboración de la prueba, teniendo en cuenta que sin entrar en contacto con la pólvora deflagrada, arroja como resultado una coloración rosada tenue, que indica de forma preliminar que es positiva para la presencia de nitritos.


This study's general objective was to determine the degree of impact a GRIESS test (a and b) may have on the inside of a Benelli Supernova shotgun barrel by transmitting contaminants, such as oil, rain water, diesel and gasoline. The need arose to research whether or not the established contaminants can affect performing the procedure described in clause 4.1 of guide 2DC-GU-0024 on "performing ballistic testing on weapons (firearms, pneumatic, gas, black powder, detonating, electrical, etc.) and entering ballistic identification systems." The methodology was experimental. A high number of tests were carried out, manipulating components A and B of the GRIESS reagent. As a result, it was observed that contaminating agents transferred within a barrel (4-stroke engine oil, diesel and gasoline) do not affect the results of identifying nitrates resulting from combustion during firearm discharge. The opposite occurs with rain water, since it was observed that is affects preparing evidence, taking into account that it yields a light pink coloration as a result without coming into contact with combusted gunpowder, which preliminarily indicates that it tested positive for the presence of nitrites.


O objetivo geral do presente estudo foi determinar o grau de afetação que o teste de GRIESS (a e b) apresenta, dentro do cano da arma de fogo tipo espingarda, marca Benelli, modelo Super Nova, com a transmissão dos contaminantes: óleo, água da chuva, diesel e gasolina. Surge a necessidade de investigar se os contaminantes estabelecidos podem afetar o desempenho do procedimento descrito no numeral 4.1 do guia 2DC-GU-0024 "realizar procedimentos balísticos em armas (armas de fogo, pneumáticas, gás, fisto, detonantes, elétricos, entre outros) e acesso a sistemas de identificação balística". A metodologia utilizada foi experimental, nesse sentido, foi realizado um número significativo de testes, e as soluções A e B do reagente de GRIESS foram manipuladas. Como resultado, ficou evidente que os agentes contaminantes transferidos para o interior do cano (óleo 4 tempos, diesel e gasolina) não afetam os resultados para a identificação de nitritos produto da combustão durante o fenômeno de queima. O contrário ocorre com a água da chuva, pois observa-se que isso afeta o preparo do teste, visto que, sem entrar em contato com a pólvora deflagrada, produz uma leve coloração rosa, o que preliminarmente indica que é positivo para a presença de nitritos.

OBJECTIVES: To assess the influence of muscle power and adiposity on all-cause mortality risk and to evaluate the 'fat but powerful' (F+P) (or 'fat but fit') paradox in older adults. METHODS: A total of 2563 older adults (65‒91 years old) from the EXERNET multicentre study were included. Adiposity (body mass index (BMI), waist circumference, body fat percentage (BF%) and fat index), allometric and relative power (sit-to-stand muscle power test) and various covariates (age, sex, hypertension, smoking status and walking and sitting times per day) were registered at baseline. All-cause mortality was recorded during a median follow-up of 8.9 years. Participants were classified into four groups: lean and powerful (L+P), F+P, lean but weak and fat and weak (F+W). Cox proportional hazard regression models and adjusted HRs were calculated. RESULTS: According to BMI and waist circumference, all-cause mortality risk was reduced in the F+P (HR=0.55 and 0.63, p=0.044 and 0.049, respectively) and L+P (HR=0.57 and 0.58, p=0.043 and 0.025, respectively) groups. According to BF%, all-cause mortality decreased in the L+P group (HR=0.53; p=0.021), and a trend for a reduction was reported in the F+P group (HR=0.57; p=0.060). According to fat index, a survival benefit was only noted in the L+P group (HR=0.50; p=0.049). Higher levels of relative power reduced all-cause mortality risk among older people (HR=0.63 and 0.53, p=0.006 and 0.011, respectively). CONCLUSION: Powerful older people exhibited a reduced 9-year all-cause mortality regardless of BMI, waist circumference and BF%. Obesity according to fat index blunted the survival benefits of being powerful.

BACKGROUND: A clinical decision support system (CDSS ) has been designed to predict the outcome (overall survival) by extracting and analyzing information from routine clinical activity as a complement to clinical guidelines in lung cancer patients. MATERIALS AND METHODS: Prospective multicenter data from 543 consecutive (2013-2017) lung cancer patients with 1167 variables were used for development of the CDSS. Data Mining analyses were based on the XGBoost and Generalized Linear Models algorithms. The predictions from guidelines and the CDSS proposed were compared. RESULTS: Overall, the highest (> 0.90) areas under the receiver-operating characteristics curve AUCs for predicting survival were obtained for small cell lung cancer patients. The AUCs for predicting survival using basic items included in the guidelines were mostly below 0.70 while those obtained using the CDSS were mostly above 0.70. The vast majority of comparisons between the guideline and CDSS AUCs were statistically significant (p < 0.05). For instance, using the guidelines, the AUC for predicting survival was 0.60 while the predictive power of the CDSS enhanced the AUC up to 0.84 (p = 0.0009). In terms of histology, there was only a statistically significant difference when comparing the AUCs of small cell lung cancer patients (0.96) and all lung cancer patients with longer (≥ 18 months) follow up (0.80; p < 0.001). CONCLUSIONS: The CDSS successfully showed potential for enhancing prediction of survival. The CDSS could assist physicians in formulating evidence-based management advice in patients with lung cancer, guiding an individualized discussion according to prognosis.

BACKGROUND: Few studies have evaluated the causes of physical inactivity in rural teachers from South America. Determining the causes and effects of physical inactivity in rural teachers could be the base for future intervention studies aiming to improve physical activity (PA) engagement in this population. OBJECTIVES: To examine the levels of PA in rural teachers and evaluate the associations between their jobs, interest in PA, body composition and practice of PA. METHODS: Body mass index (BMI) and body fat % (BF) of rural teachers from the Valparaiso Region in Chile were measured; interviews regarding PA habits, interests and PA practice were conducted. The relationship between BF% and interest in PA and the practice of sports was analysed stratifying by sex. RESULTS: Around 71% were overweight/obese and 80% of the teachers reported not practicing any PA. These figures are higher than the Chilean national average for adults (64.5% overweight/obese). Moreover, females presented a higher BF% than male teachers (35.98% vs. 26.44%, p < 0.05). The main causes of not performing PA were: firstly, the lack of time (51.8%), and secondly, fatigue after work and/or finishing work late. In addition, rural teachers claim that in their free time they continue to finish tasks that they were not able to complete during their working hours. CONCLUSIONS: Rural Chilean teachers show a high prevalence of obesity and low PA levels. Moreover, rural teachers have a work overload stemming from their jobs, and this could be an important link to an overall sedentary lifestyle. Further studies should focus on the relationship between teacher obesity, physical activity, nutrition and work overload.

El compromiso cardiovascular de los pacientes con COVID-19, especialmente en la subpoblación con factores de riesgo cardiovasculares, se asocia a la severidad de la enfermedad e inclusive a una mayor mortalidad en algunas series de casos. Los mecanismos fisiopatológicos descritos con mayor claridad son la injuria miocárdica directa y el proceso asociado a la inflamación sistémica.Las manifestaciones cardiovasculares son diversas e incluyen injuria miocárdica, miocarditis, miocardiopatías, insuficiencia cardíaca, síndromes coronarios agudos, arritmias, eventos tromboembólicos. Adicionalmente, debemos considerar los efectos adversos de los medicamentos utilizados para tratar la infección viral. (AU)


Cardiovascular compromise among COVID-19 patients, especially in the subpopulation with cardiovascular risk factors, is associated with severity of disease and a higher mortality in some case-series. The most clearly described pathophysiological mechanisms are direct myocardial injury and the process associated with systemic inflammation. Cardiovascular manifestations are diverse and include myocardial injury, myocarditis, cardiomyopathies, heart failure, acute coronary syndromes, arrhythmias, thromboembolic events. Moreover, we must consider the adverse effects of medications used to treat the viral infection. (AU)
