Fat mass and obesity-related (FTO) gene single nucleotide polymorphisms (SNPs) interferes with food preferences that impact macronutrient intake. Few studies have investigated the relationship of this polymorphisms with the intake of micronutrients. Moreover, studies have shown multiple micronutrient deficiencies in patients with obesity. This work evaluated the effect of the FTO rs9939609 gene polymorphism on dietary nutritional quality and food intake of macronutrients and vitamins in of women with obesity candidates for metabolic surgery. The study included 106 women (24 to 60 years old) with BMIs of 36.1 to 64.8 kg/m2. A food frequency questionnaire validated for the local population was applied to obtain information about food intake. The Index of Nutritional Quality (INQ) was used to assess the adequacy of macronutrient and vitamin intake. Energy, protein and lipid intakes were higher in carriers of the A allele compared to TT in the younger age groups but were similar in the class of subjects aged ≥45 years. The INQ for protein was higher in carriers of the A allele than in carriers of the TT allele. The INQs for protein, carbohydrate, vitamins B2, B3 and B6 decreased, whereas the INQ for vitamin C increased with advancing age. The INQ for vitamin A was lower in AA than in TT, regardless of age, whereas vitamin E was higher in younger AA than in older AA. The INQ for vitamin B9 was higher in younger women than in older women. In conclusion, the FTO gene contributed to the intake of more energy, protein and lipids and interfered with the intake of vitamins B9, A and E. With the exception of vitamin A, the effect of the genotype was attenuated with ageing.

Rho GTPases are molecular switches regulating multiple cellular processes. To investigate the role of RhoA in normal intestinal physiology, we used a conditional mouse model overexpressing a dominant negative RhoA mutant (RhoAT19N) in the intestinal epithelium. Although RhoA inhibition did not cause an overt phenotype, increased levels of nuclear ß-catenin were observed in the small intestinal epithelium of RhoAT19N mice, and the overexpression of multiple Wnt target genes revealed a chronic activation of Wnt signaling. Elevated Wnt signaling in RhoAT19N mice and intestinal organoids did not affect the proliferation of intestinal epithelial cells but significantly interfered with their differentiation. Importantly, 17-month-old RhoAT19N mice showed a significant increase in the number of spontaneous intestinal tumors. Altogether, our results indicate that RhoA regulates the differentiation of intestinal epithelial cells and inhibits tumor initiation, likely through the control of Wnt signaling, a key regulator of proliferation and differentiation in the intestine.

ABSTRACT Objective Verify association between the perception of food insecurity and the diet quality of the Brazilian population, applying The Brazilian Food Insecurity Scale and the Brazilian Healthy Eating Index Revised. Methods Cross-sectional study using data from the Householder Budget Survey collected in 2017/18 with two 24-hour recalls. A multinomial regression model was used with Odds Ratio and a 95% confidence interval, with the final model being the insertion of variables according to the theoretical model of hierarchy adopted. Results A total of 57,920 households were analyzed, and of these, 39.22% lived with some degree of Food Insecurity. There was a significant difference between female heads of the household, mixed race and black race, households with adults and children and living in rural regions as the three levels of AI [Food insecure] (p=<0.001), with a greater chance of food insecurity in these households. The average Brazilian Healthy Eating Index Revised for the 46,152 individuals was 54.23 points for those who were not food insecure, and 54.11 points for those who experienced severe food insecurity. Conclusion It is concluded that there is an association between the perception of food insecurity and the nutritional quality of the diet of the Brazilian population, which can lead to malnutrition and obesity.


RESUMO Objetivo Verificar a associação entre a percepção de insegurança alimentar e a qualidade da dieta da população brasileira, aplicando a Escala Brasileira de Insegurança Alimentar e o Índice de Qualidade da Dieta Revisado a partir de dois recordatórios de 24 horas. Métodos Estudo transversal que utilizou dados da Pesquisa de Orçamentos Familiares de 2017-2018. Utilizou-se o modelo de regressão multinomial com Odds Ratio e intervalo de confiança de 95%, sendo o modelo final com inserção das variáveis de acordo com o modelo teórico de hierarquia adotado. Resultados Foram analisados 57.920 domicílios, e destes, 39,22% viviam com algum grau de Insegurança Alimentar. Verificou-se diferença significativa entre pessoas do sexo feminino como chefe do domicílio, raça pardo e preto, domicílios com adultos e crianças e residir na região rural como os três níveis de Insegurança Alimentar (p=<0,001), havendo uma maior chance de insegurança alimentar nesses domicílios. A média do Índice de Qualidade da Dieta Revisado para os 46.152 indivíduos foi de 54.23 pontos para os que não apresentavam insegurança alimentar, e 54.11 pontos para aqueles que vivenciaram insegurança alimentar grave. Conclusão Conclui-se que existe associação entre a percepção de insegurança alimentar e a qualidade nutricional da dieta da população brasileira, o que pode levar à desnutrição e obesidade.

BACKGROUND: The literature is scanty regarding the effect of radiation therapy (RT) on the mechanical properties of immature permanent teeth. AIM: To evaluate the effect of RT on the fracture resistance of simulated immature teeth submitted to different types of root reinforcement. DESIGN: Sixty-four human teeth simulating the Cvek stage 3 of root development were distributed into eight groups (n = 8), according to exposure or not to RT (70 Gy) and the root reinforcement method: Group NR (control)-no reinforcement/no RT; Group NR + RT (control)-no reinforcement/RT; Group PO-tricalcium silicate-based cement (TS) apical plug/canal obturation/no RT; Group PO + RT-TS apical plug/canal obturation/RT; Group TS-canal filling with TS/no RT; Group TS + RT-canal filling with TS/RT; Group FP-TS apical plug/fibreglass post/no RT; and Group FP + RT-TS apical plug/fibreglass post/RT. Fracture resistance was determined using a universal testing machine (0.5 mm/min). RESULTS: In the intergroup comparison, nonirradiated teeth had higher fracture resistance (p < .05). Groups FP and FP + RT had higher fracture resistance (p < .001). CONCLUSION: Radiotherapy affected the fracture resistance of simulated immature teeth. Reinforcement with fibreglass posts increased the fracture resistance, regardless of the radiation.

The habit of eating specific meals has been addressed in several studies, but the evaluation of meal patterns has received less attention. This study aimed to describe the meal patterns of the Brazilian population. A complex sampling design was used to select the 46,164 ≥ 10-year-old individuals examined in the Brazilian National Dietary Survey. Food consumption was assessed by two non-consecutive 24-hour recalls applied throughout a one-week period. The exploratory data analysis approach was used to determine the meal patterns, i.e., how individuals combined the main meals (breakfast, lunch, dinner) and snacks (morning, afternoon, evening/night) throughout the day. The most common meal patterns were three main meals + one snack, reported by 25.1% of the individuals, and three main meals + two snacks (24.6%). Other meal patterns identified were: three main meals + three snacks (18.5%); three main meals and no snacks (10.9%); one or two main meals + two snacks (7.4%); one or two main meals + one snack (6.9%); one or two main meals + three snacks (4.2%); and one or two main meals and no snacks (2.3%). Meal patterns varied according to gender and age group, and on typical versus atypical food consumption days. We found that eight patterns characterized the daily meal consumption in Brazil. Furthermore, around 80% of the population had three main meals every day and about 13% did not report having any snacks. The characterization of meal habits is important for tailoring and targeting health promotion actions.


Os hábitos de consumo de refeições específicas têm sido abordados em diversos estudos, no entanto, a avaliação dos padrões refeições tem recebido menos atenção. O objetivo deste estudo foi descrever os padrões de refeições da população brasileira. Um desenho amostral complexo foi utilizado para selecionar os 46.164 indivíduos de ≥ 10 anos examinados no Inquérito Nacional de Alimentação de 2017-2018. O consumo alimentar foi avaliado por meio de dois recordatórios de 24 horas não consecutivos, aplicados durante um período de uma semana. A análise exploratória de dados foi utilizada para determinar os padrões de refeições, ou seja, como os indivíduos combinam as principais refeições (café da manhã, almoço, jantar) e lanches (manhã, tarde, noite) ao longo do dia. Os padrões de refeições mais frequentes foram três refeições principais + um lanche, conforme relatado por 25,1% dos indivíduos, e três refeições principais + dois lanches (24,6%). Outros padrões de refeições identificados foram: três refeições principais + três lanches (18,5%); três refeições principais e nenhum lanche (10,9%); uma ou duas refeições principais + dois lanches (7,4%); uma ou duas refeições principais + um lanche (6,9%); uma ou duas refeições principais + três lanches (4,2%); e uma ou duas refeições principais e nenhum lanche (2,3%). Os padrões de refeições variaram de acordo com o sexo e a faixa etária, e nos dias típicos em comparação com os atípicos de consumo alimentar. Verificou-se que oito padrões caracterizaram o consumo diário de refeições no Brasil. Além disso, cerca de 80% da população realizava três refeições principais diárias e cerca de 13% reportaram não lanchar. A caracterização dos padrões de refeições é importante para adequar e direcionar ações de promoção da saúde.


Los hábitos alimenticios específicos se han abordado en varios estudios, sin embargo, poco se sabe sobre la evaluación de los patrones de alimentación. El objetivo de este estudio fue describir el patrón de alimentación de la población brasileña. Se utilizó un diseño de muestra complejo para seleccionar a 46.164 individuos de ≥ 10 años quienes participaron en la Encuesta Nacional de Alimentación 2017-2018. El consumo alimentario se evaluó mediante dos registros de 24 horas no consecutivos, aplicados durante una semana. Para determinar el patrón de alimentación, se aplicó el análisis exploratorio, es decir, cómo las personas combinan las comidas principales (desayuno, almuerzo, cena) y las meriendas (mañana, tarde, noche) a lo largo del día. Los patrones de alimentación más frecuentes fueron tres comidas principales + una merienda según informan el 25,1% de los individuos, y tres comidas principales + dos meriendas (24,6%). Otros patrones identificados destacaron tres comidas principales + tres meriendas (18,5%); tres comidas principales sin merienda (10,9%); una o dos comidas principales + dos meriendas (7,4%); una o dos comidas principales + una merienda (6,9%); una o dos comidas principales + tres meriendas (4,2%); y una o dos comidas principales sin merienda (2,3%). Los patrones de alimentación tuvieron una variación según el sexo y el grupo de edad, y en días típicos en comparación con los atípicos de consumo de alimentos. Se encontró que ocho patrones caracterizan el consumo diario de comidas en Brasil. Por lo tanto, aproximadamente el 80% de la población tienen tres comidas principales al día y aproximadamente el 13% informan que no tienen merienda. Es importante caracterizar los patrones de alimentación para adaptar y orientar las acciones de promoción de la salud.

Lineage switching can induce therapy resistance in cancer. Yet, how lineage fidelity is maintained and how it can be lost remain poorly understood. Here, we have used CRISPR-Cas9-based genetic screening to demonstrate that loss of SMARCB1, a member of the SWI/SNF chromatin remodeling complex, can confer an advantage to clear cell renal cell carcinoma (ccRCC) cells upon inhibition of the renal lineage factor PAX8. Lineage factor inhibition-resistant ccRCC cells formed tumors with morphological features, but not molecular markers, of neuroendocrine differentiation. SMARCB1 inactivation led to large-scale loss of kidney-specific epigenetic programs and restoration of proliferative capacity through the adoption of new dependencies on factors that represent rare essential genes across different cancers. We further developed an analytical approach to systematically characterize lineage fidelity using large-scale CRISPR-Cas9 data. An understanding of the rules that govern lineage switching could aid the development of more durable lineage factor-targeted and other cancer therapies.

Abstract The aim of this study was to analyze the association between adherence to school meals and the co-occurrence of the regular consumption of healthy and unhealthy eating markers among Brazilian adolescents. Data from 67,881 adolescents in Brazilian public schools who participated in the 2015 National School Health Survey, were used. From the 7-day FFQ, the dependent variable was constructed, co-occurrence of regular consumption (≥ 5x/week) of healthy and unhealthy food markers, which was categorized as regular consumption of none, one or two, or three eating markers. We performed an ordinal logistic regression with adjustment for sociodemographic, eating habits outside of school, and school characteristics variables. The prevalence of the co-occurrence of the regular consumption of three healthy eating markers was 14.5%, and that of three unhealthy markers was 4.9%. High adherence to school meals (every day) was positively associated with regular consumption of healthy eating markers and inversely associated with regular consumption of unhealthy eating markers. The school meals provided by PNAE contribute to the promotion of healthy eating habits among Brazilian adolescents.


Resumo Este estudo tem como objetivo analisar a associação entre a adesão à alimentação escolar e a coocorrência do consumo regular de marcadores de alimentação saudável e não saudável entre adolescentes brasileiros. Foram avaliados 67.881 adolescentes de escolas públicas brasileiras participantes da Pesquisa Nacional de Saúde do Escolar (PeNSE) de 2015. A partir do QFA de sete dias, construiu-se a variável dependente, coocorrência do consumo regular (≥ 5x/semana) de marcadores de alimentação saudável e não saudável, que foi categorizada em consumo regular de nenhum; um ou dois; ou três marcadores de alimentação. Realizou-se regressão logística ordinal com ajuste para variáveis sociodemográficas, hábitos alimentares fora da escola e características da escola. A prevalência da coocorrência do consumo regular de três marcadores de alimentação saudável foi de 14,5%, e de três marcadores de alimentação não saudável foi de 4,9%. A alta adesão à alimentação escolar (todos os dias) foi positivamente associada ao consumo regular de marcadores de alimentação saudável e inversamente associada ao consumo regular de marcadores de alimentação não saudável. A alimentação escolar fornecida pelo PNAE contribui para a promoção de hábitos alimentares saudáveis entre os adolescentes brasileiros.

Considering the side effects in the oral cavity and dental structures of radiotherapy (RDT) for head and neck cancer, this study aimed to evaluate the effects of RDT on the root dentin concerning the obliteration of dentinal tubules, the inorganic composition of intra-radicular dentin, and the integrity of collagen fibers. Thirty human canines were selected from a biobank and randomly divided into two groups (n=15). The samples were sectioned buccolingually, and a hemisection was used for structural analysis by scanning electron microscopy (SEM) and energy-dispersive X-ray spectrometer (EDS). Low-vacuum SEM images were obtained at 2000-x magnification to observe the obliteration of the dentinal tubules. Moreover, compositional evaluation was performed using EDS. After RDT, the SEM and EDS analyses were repeated using the same methodology. RDT was applied fractionally at 2 Gy per day, 5 days per week, for 7 weeks, resulting in a total dose of 70 Gy. The collagen integrity of the irradiated and non-irradiated samples was analyzed using Masson's trichrome and picrosirius red staining polarization microscopy. Samples subjected to RDT exhibited dentinal tubule obliteration (p < 0.001); low integrity of type I and III collagen fibers (p < 0.05); compositional reduction of calcium (p = 0.012), phosphorus (p = 0.001), and magnesium (p < 0.001); an increased Ca/P ratio (p < 0.001). RDT affects the structure of dentinal tubules, the inorganic composition of intra-radicular dentin, and the collagen fiber integrity in the root dentin, which may interfere with the effectiveness and durability of dental procedures.

Objective: To estimate the prevalence of simultaneity of obesity, hypertension and/or diabetes and the association with sociodemographic and lifestyle-related behavior variables. Methods: This is a cross-sectional study with data from the 2013 National Health Survey. The presence of two/three diseases (obesity, hypertension, and diabetes) was considered multimorbidity of noncommunicable diseases (NCDs). Proportional odds logistic regression model, stratified by sex, was used to assess the sociodemographic and lifestyle factors associated with multimorbidity of NCDs, considering the sample complexity and sample weights. Results: The prevalence of multimorbidity of NCDs was 9.6%. The odds of multimorbidity are higher with increasing age, among blacks, but lower in the north region when compared with other regions of the country for both sexes. The odds of multimorbidity of NDCs increased among women who had lower levels of education and spent more time TV watching, and among men who lived with a partner and did not practice the recommended leisure-time physical activity. Conclusion: Sociodemographic factors and lifestyle-related behaviors were associated with the presence of obesity, hypertension, and/or diabetes.


Objetivo: Estimar la prevalencia de simultaneidad de obesidad, hipertensión y/o diabetes y la asociación con variables sociodemográficas y relacionadas con el estilo de vida. Métodos: Los datos fueron obtenido en la Encuesta Nacional de Salud Brasileña, una encuesta a nivel nacional, realizada en los hogares brasileños y llevada a cabo en 2013. La presencia de dos /tres enfermedades (obesidad, hipertensión y diabetes) se consideró multimorbilidad de las enfermedades no transmisibles (ENT). El modelo de regresión logística de probabilidades proporcionales, estratificado por sexo, se utilizó para evaluar los factores sociodemográficos y de estilo de vida asociados con la multimorbilidad de las ENT, considerando la complejidad de la muestra y los pesos de la muestra. Resultados: La prevalencia de multimorbilidad de las ENT fue del 9,6%. Las probabilidades de multimorbilidad son más altas con el aumento de la edad, entre los negros, pero más bajas en la región norte en comparación con otras regiones del país para los dos sexos. Las probabilidades de multimorbilidad de las ENT aumentaron entre las mujeres que tenían niveles más bajos de educación y pasaban más tiempo viendo la televisión, y entre los hombres que vivían con una pareja y no practicaban la actividad física recomendada para el tiempo libre. Conclusión: Los factores sociodemográficos y los comportamientos relacionados con el estilo de vida se asociaron con la presencia de obesidad, hipertensión y/o diabetes.

Objective: To evaluate the association between skipping breakfast and overweight among Brazilian adolescents. Methods: A total of 16,556 Brazilian students (aged 11-19 years) who participated in the National School Health Survey in 2015 were evaluated. The omission of breakfast (consumption frequency <5 days/week) was determined using the question, "Do you usually have breakfast"? The participants' weight and height were measured, and excess weight was classified according to the body mass index for sex and age. The covariates analyzed were sociodemographic and economic characteristics (sex, age group, socioeconomic status, skin color, maternal schooling, paid work, and living with parents); variables related to school (administrative dependence, study shift, and presence of a canteen); lifestyle (physical activity, alcohol consumption, smoking, sedentary behavior, consumption of industrialized foods, meals with parents, and school meals); and self-perception of body image. The association between skipping breakfast and being overweight was analyzed using a Poisson regression model with adjustment for covariates. Results: The prevalence of skipping breakfast and being overweight were 33.8% and 25.2%, respectively. After adjustment, skipping breakfast was associated with being overweight (OR:1.02; 95% CI:1.01; 1.04). Conclusion: Skipping breakfast is associated with being overweight among Brazilian adolescents. Thus, encouraging regular consumption of meals should be part of health promotion and healthy eating actions aimed at preventing diseases and chronic noncommunicable diseases.


Objetivo: Evaluar la asociación entre la omisión del desayuno y el sobrepeso entre adolescentes brasileños. Métodos: Fueron evaluados 16.556 estudiantes brasileños (de 11 a 19 años) que participaron en la Encuesta Nacional de Salud Escolar en 2015. La omisión del desayuno (frecuencia de consumo < 5 días/semana) se determinó mediante la pregunta "¿Desayuna habitualmente"? Se midió el peso y la talla de los participantes, y se clasificó el exceso de peso según el índice de masa corporal para el sexo y la edad. Las covariables analizadas fueron características sociodemográficas y económicas (sexo, grupo de edad, nivel socioeconómico, color de piel, escolaridad materna, trabajo remunerado y convivencia con los padres); variables relacionadas con la escuela (dependencia administrativa, turno de estudio y presencia de comedor); estilo de vida (actividad física, consumo de alcohol, tabaquismo, sedentarismo, consumo de alimentos procesados, comidas con los padres y alimentación escolar); y autopercepción de la imagen corporal. La asociación entre saltarse el desayuno y tener sobrepeso se analizó mediante un modelo de regresión de Poisson con ajuste por covariables. Resultados: La prevalencia de saltarse el desayuno y tener sobrepeso fue de 33,8% y 25,2%, respectivamente. Después del ajuste, saltarse el desayuno se asoció con sobrepeso (OR: 1,02; IC 95 %: 1,01; 1,04). Conclusión: Saltarse el desayuno está asociado con el sobrepeso entre los adolescentes brasileños. Por lo tanto, fomentar el consumo regular de comidas debe ser parte de las acciones de promoción de la salud y alimentación saludable dirigidas a la prevención de enfermedades y enfermedades crónicas no transmisibles.

Samples subjected to RDT exhibited dentinal tubule obliteration (p < 0.001); low integrity of type I and III collagen fibers (p < 0.05); compositional reduction of calcium (p = 0.012), phosphorus (p = 0.001), and magnesium (p < 0.001); an increased Ca/P ratio (p < 0.001). RDT affects the structure of dentinal tubules, the inorganic composition of intra-radicular dentin, and the collagen fiber integrity in the root dentin, which may interfere with the effectiveness and durability of dental procedures.


Resumo Considerando os efeitos colaterais na cavidade oral e estruturas dentárias da radioterapia (RDT) para o câncer de cabeça e pescoço, este estudo teve como objetivo avaliar os efeitos da RDT sobre a dentina radicular quanto à obliteração dos túbulos dentinários, a composição inorgânica da dentina intra-radicular e a integridade das fibras colágenas. Trinta caninos humanos foram selecionados de um biobanco e divididos aleatoriamente em dois grupos (n=15). As amostras foram seccionadas no sentido vestíbulo-lingual, e uma hemissecção foi utilizada para análise estrutural por microscopia eletrônica de varredura (MEV) e espectrômetro de raios X de dispersão de energia (EDS). Imagens de MEV de baixo vácuo foram obtidas com aumento de 2500x para observar a obliteração dos túbulos dentinários. Além disso, a avaliação composicional foi realizada usando EDS. Após a RDT, as análises em MEV e EDS foram repetidas usando a mesma metodologia. A RDT foi aplicada fracionada a 2 Gy por dia, 5 dias por semana, durante 7 semanas, resultando em uma dose total de 70 Gy. A integridade do colágeno das amostras irradiadas e não irradiadas foi analisada usando microscopia de polarização de coloração com tricrômio de Masson e picrosirius red. As amostras submetidas a RDT apresentaram obliteração dos túbulos dentinários (p < 0,001); baixa integridade das fibras colágenas tipo I e III (p < 0,05); redução composicional de cálcio (p = 0,012), fósforo (p = 0,001) e magnésio (p < 0,001); e uma relação Ca/P aumentada (p < 0,001). A RDT afeta a estrutura dos túbulos dentinários, a composição inorgânica da dentina intra-radicular e a integridade das fibras colágenas na dentina radicular, o que pode interferir na eficácia e durabilidade dos procedimentos odontológicos.

OBJECTIVES: Palliative sedation for existential suffering (PS-ES) is considered a controversial practice to control refractory suffering at the end of life. This study aims to explore Belgian palliative care physicians' perceptions regarding the ethics of PS-ES. METHODS: This nationwide qualitative study follows a Grounded Theory approach. We conducted semistructured interviews with 25 palliative care physicians working in 23 Belgian hospitals and hospices (Flanders, Brussels, Wallonia). We analysed the data using the Qualitative Analysis Guide Of Leuven and we followed the Consolidated Criteria for Reporting Qualitative Research guidelines. RESULTS: The data revealed that Belgian palliative care physicians have difficulty characterising ES and distinguishing it from other types of suffering. They express mixed attitudes towards PS-ES and employ a wide range of ethical arguments in favour and against it, which are mainly linked to the four principles of biomedical ethics. CONCLUSION: Since there is a lack of consensus among Belgian palliative care physicians about PS-ES, further research is needed to clarify their attitudes. A better understanding of physicians' underlying presuppositions can provide more insight into their perceptions of ES and PS-ES and provide further insight for establishing good practices in ES management at the end of life.

This study investigated the effect of radiation timing on the bond strength of resin cement to intraradicular dentine. Fifty human teeth were distributed into 5 groups (n = 10): Control (nonirradiated teeth), Before-RCT (teeth irradiated before root canal treatment), After-CH (teeth irradiated after canal preparation and placement of calcium hydroxide intracanal dressing), After-RCT (teeth irradiated after completion of root canal treatment) and After-FPL (teeth irradiated after luting of a glass fibre post). Each tooth received 70 Gy irradiation. The roots were sectioned for push-out strength testing. After-RCT and After-FPL groups had significantly lower push-out strength than the control at the middle third (p < 0.05). Control and After-CH groups had a higher percentage of cohesive dentine failure. Radiotherapy after root canal obturation and post luting adversely affected the adhesiveness of resin cement to intraradicular dentine. Teeth irradiated before root canal treatment and after placement of calcium hydroxide had the best performance.

Resumo: O setor de saúde constitui um dos maiores campos de desenvolvimento econômico, social, científico e tecnológico do Brasil e tem sido arena de disputa entre os interesses capitalistas, que entendem a saúde como bem de mercado, e os defensores do acesso universal, que a entendem como bem essencial. No Brasil, a Constituição Federal de 1988 determinou que a saúde é direito de todos e dever do Estado, e, desde então, o país avança com a política pública, mas sofre recuos e bloqueios pela condição de país de economia periférica, historicamente subordinado aos interesses dos países centrais, que o veem como um amplo mercado consumidor. Esses interesses externos, associados à burguesia interna, têm obtido vantagens do Estado brasileiro desde a década de 1960, quando a formação de grupos empresariais se expandiu, dominando diversos segmentos da saúde, sobretudo a partir de políticas neoliberais dos anos de 1990. Esses aspectos são muito explorados nas publicações do campo da Saúde Coletiva, mas este texto busca uma nova abordagem, utilizando a Teoria Marxista da Dependência como referência para analisar, ainda que de forma preliminar, a situação de dependência política, econômica e tecnológica que tem distanciado a política de saúde dos ideais de um sistema público e universal, defendido na Reforma Sanitária brasileira.


Abstract: The health sector is one of the major fields of economic, social, scientific, and technological development in Brazil, and has been the arena of dispute between capitalist interests that regard health as a commodity and advocates of universal access who regard health as an essential good. The Brazilian Federal Constitution of 1988 determined that health is a fundamental right and the State's responsibility, and since then the country has made progress with public policy, but has suffered setbacks and blockages due to its status as a peripheric country, historically subordinated to the interests of core countries, which see Brazil as a broad consumer market. These external interests associated with the internal bourgeoisie have been taking advantage of the Brazilian State since the 1960s, when the foundation of business groups expanded, dominating various health segments, especially since the neoliberal policies of the 1990s. These aspects are much explored in publications in Public Health, but this text seeks a new approach, using the Marxist Theory of Dependency as a reference to analyze, albeit in a preliminary way, the situation of political, economic, and technological dependence that has distanced health policy from the ideals of a public and universal system, defended in the Brazilian Health Reform.

Resumo Introdução: São restritos os estudos que apresentam estimativas com representatividade nacional sobre omissão do café da manhã entre adolescentes brasileiros. Objetivo: Estimar a prevalência da omissão do café da manhã entre estudantes adolescentes brasileiros, segundo características sociodemográficas, nos anos de 2012 e 2015. Método: Análise realizada com os dados da Pesquisa Nacional de Saúde do Escolar, com escolares do 9º, ano nos anos de 2012 e 2015. Foi estimada a prevalência de omissão do café da manhã (consumo por menos de cinco dias por semana), segundo sexo, faixa etária, dependência administrativa da escola e região do país, considerando o desenho complexo da amostra. Resultados: A prevalência de omissão do café da manhã verificada foi de 38,1% em 2012 e 35,6% em 2015, sendo mais elevada entre meninas e estudantes de escolas privadas. A omissão dessa refeição foi mais elevada entre os escolares das regiões Sul, Sudeste e Centro-Oeste. As capitais brasileiras pertencentes às regiões Sul e Centro-Oeste apresentaram as maiores prevalências de omissão do café da manhã. Conclusões: Não houve alteração significativa da prevalência de omissão de café da manhã entre os dois inquéritos realizados, sendo maior a sua omissão entre as meninas, estudantes de escolas públicas e de regiões mais desenvolvidas.


Abstract Background: There is limited research presenting nationally representative estimates regarding breakfast omission among Brazilian adolescents. Objective: The objective of this study was to estimate the prevalence of breakfast omission among Brazilian adolescent students, considering sociodemographic characteristics, in the years 2012 and 2015. Method: An analysis was conducted using data from the National School Health Survey, including 9th grade students in the years 2012 and 2015. The prevalence of breakfast omission (defined as consumption on fewer than 5 days per week) was estimated based on gender, age group, school administrative dependence, and region of the country, considering the complex sample design. Results: The prevalence of breakfast omission observed was 38.1% in 2012 and 35.6% in 2015, with higher rates among girls and students from private schools. Omission of this meal was more common among students in the South, Southeast, and Central West regions. Brazilian capitals located in the South and Central West regions exhibited the highest prevalence of breakfast omission. Conclusions: There was no significant change in the prevalence of breakfast omission between the two surveys, and its omission was higher among girls, students from public schools, and in more developed regions.

ABSTRACT Objective To evaluate the consumption of free sugars and associated factors in the diet of university students entering full-time courses at a public university in Mato Grosso. Methods Cross-sectional study with students aged 16 to 26 years in the first semester of 2016, 2017, and 2018. Average sugar consumption was estimated using a 24-hour diet recall, evaluating the percentage of free sugars in total energy intake and the prevalence of consumption greater than 10.0% of total energy intake, according to socioeconomic and lifestyle variables, stratified by sex, and the foods that most contributed to its consumption. Results A total of 1,063 students were evaluated, the percentage of free sugars in the students' diet was on average 14.3% among men and 15.4% among women, with the prevalence of consumption above 10.0% also being higher among women (72.6 vs. 66.6%). There was a direct relationship between the participation of free sugars in the highest tertiles of energy intake, and the main food groups sources of sugar were desserts, juices, soft drinks, and coffee. There was a significant association between courses outside the health sciences, tobacco use, and unsatisfactory meal consumption profile with higher consumption of free sugars among women. Among men, sugar consumption was higher among those who were not overweight compared to those who were overweight. Conclusion The consumption of free sugars is high among university students, being associated with higher energy consumption, especially for sweetened beverages.


RESUMO Objetivo Avaliar o consumo de açúcar livre e os fatores associados na alimentação de universitários ingressantes em cursos de período integral em uma universidade pública de Mato Grosso. Métodos Estudo transversal com estudantes de 16 a 26 anos de idade no primeiro semestre de 2016, 2017 e 2018 (n=1.063). O consumo médio de açúcar foi estimado por meio de recordatório de 24 horas, sendo avaliada a participação do açúcar livre na ingestão energética total e a prevalência de participação do consumo superior a 10,0% da ingestão energética, segundo variáveis socioeconômicas, de estilo de vida e fatores dietéticos, , estratificadas por sexo. Resultados A participação do açúcar livre na dieta foi, em média, de 14,3% entre os homens e 15,4% entre as mulheres, sendo a prevalência de consumo acima de 10,0% também maior entre as mulheres (72,6 vs. 66,6%). Foi maior a participação do açúcar livre nos tercis mais elevados de ingestão energética, sendo verificado que os principais grupos de alimentos fontes foram sobremesas, sucos, refrigerantes e café. Entre as mulheres, verificou-se maior consumo de açúcar livre entre aquelas que faziam uso de tabaco e que apresentavam perfil de consumo de refeições insatisfatório. Por outro lado, menor consumo de açúcar livre foi observado entre as estudantes da área da saúde. Entre os homens, o consumo de açúcar livre foi maior entre aqueles sem excesso de peso, comparados aos com excesso. Conclusão O consumo de açúcar livre foi elevado entre estudantes universitários, sendo maior entre os estudantes com maior consumo energético. Verificou-se que as bebidas adoçadas estiveram entre as principais fontes.

Both pregnancy and obesity can influence significant changes in the immune system. On this basis, the present study proposes to evaluate the humoral immune response of overweight pregnant mares in response to a commercial vaccine. Thirty pregnant Crioulo mares were separated according to body condition score (BCS) into overweight (BCS≥7/9) or lean-control (BCS= 5-6/9). In each group, the animals were subdivided into vaccinated and controls. The mares were vaccinated against EHV-1 in two doses spaced 21 days apart and had their blood collected monthly, for five months, for antibody evaluation. Both vaccinated groups had an increase in specific neutralizing antibodies after the vaccine. However, after the second dose, there was no increase in antibodies in any of the groups. Vaccinated overweight and lean-control mares did not differ at any time point. Therefore, this study demonstrated that obesity does not influence the humoral immune response in pregnant Crioulo mares.(AU)


Tanto a gestação quanto a obesidade podem influenciar o desenvolvimento de alterações significativas no sistema imune, portanto, o presente estudo teve como objetivo avaliar a resposta imune humoral de éguas gestantes com sobrepeso em resposta a uma vacina comercial. Trinta éguas Crioulas gestantes foram separadas de acordo com o escore de condição corporal (ECC) em éguas com sobrepeso (ECC≥7/9) e éguas controles (ECC=5-6/9) e, ainda, em cada grupo, os animais também foram separados em vacinados e controles. As éguas foram vacinadas contra o EHV-1 em duas doses com intervalo de 21 dias, sendo realizadas coletas de sangue mensalmente durante cinco meses para avaliação de anticorpos neutralizantes. Ambos os grupos vacinados tiveram aumento de anticorpos neutralizantes específicos após a vacina, porém, após a segunda dose, não foi observado aumento de anticorpos em nenhum dos grupos. Nenhuma diferença foi observada entre éguas vacinadas com sobrepeso e as éguas controles em nenhum momento. Assim, este estudo demonstrou que a obesidade não é um fator que influencia a resposta imune humoral de éguas Crioulas gestantes.(AU)

Objetivo: Este estudo visou avaliar o desempenho da razão cintura-estatura (RCE) como indicador complementar do estado nutricional na primeira fase da adolescência. Métodos: Trata-se de estudo transversal com 148 adolescentes (10 a 13 anos de idade) de duas escolas de Macaé, RJ, realizado em 2016/2017. Foram coletadas informações de maturação sexual, peso, estatura e perímetro da cintura (PC). Para verificar como as classificações do índice de massa corporal-para-idade (IMC/I), PC e RCE dialogam em termos de triagem de risco em saúde, foi feito o teste Kappa. Os limites máximos de sensibilidade e especificidade da RCE segundo o IMC/I foram analisados pela curva ROC (Receiver Operating Characteristics). Resultados: Dentre os avaliados, 51,4% eram meninas e mais de 60% encontravam-se nos dois primeiros estágios de maturação sexual. A prevalência de excesso de peso (sobrepeso+obesidade) foi 31,8%, obesidade 17,6% e RCE elevada 20,3%, sem diferença segundo sexo e maturação sexual. A RCE apresentou boa concordância com excesso de peso (Kappa=0,707) e obesidade (Kappa=0,780). Já a concordância entre IMC/I e PC foi pobre. O valor de 0,45 da RCE foi o ponto de corte mais adequado para identificar os adolescentes com excesso de peso. Conclusões: Este trabalho sugere que a RCE apresenta melhor desempenho que o PC como indicador complementar do estado nutricional no início da adolescência. A RCE agrega informação sobre a gordura central ponderada pela estatura, não requer curva de comparação e apresenta ponto de corte, o que facilita ações de triagem nos serviços de saúde e em estudos epidemiológicos.


Objective: This study evaluated the performance of the Waist-to-Height Ratio (WHR) as an additional indicator of nutritional status in the first phase of adolescence. Methods: This is a cross-sectional study, developed in 2016/2017, with 148 adolescents (10 to 13 years old) from two public schools of Macaé, a municipality in Rio de Janeiro, Southeast Brazil. We collected information on sexual maturation, weight, height, and waist circumference (WC). The Kappa Test was performed to verify the accordance among Body Mass Index-for-Age (BMI/A), WC, and WHR in relation to health risk screening. The maximum limits of sensitivity and specificity of WHR according to BMI/A were analyzed by ROC curve (Receiver Operating Characteristics). Results: Among the participants, 51.4% were girls, and more than 60% were in the first two stages of sexual maturation. The prevalence of excess weight (overweight+obesity) was 31.8%, obesity 17.6%, and high WHR 20.3%, with no difference according to sex and sexual maturation. WHR showed good agreement with excess weight (Kappa=0.707) and obesity (Kappa=0.780). The agreement between BMI/A and WC was poor. The value 0.45 was the most appropriate WHR cutoff point to identify adolescents with excess weight. Conclusions: This study suggests that WHR performs better than WC as an additional indicator of nutritional status in early adolescence. WHR brings information on central adiposity weighted by height, does not require a comparison curve, and has a cutoff point, which may facilitate screening in health services and epidemiological studies.

Metabolic reprogramming is critical for tumor initiation and progression. However, the exact impact of specific metabolic changes on cancer progression is poorly understood. Here, we integrate multimodal analyses of primary and metastatic clonally-related clear cell renal cancer cells (ccRCC) grown in physiological media to identify key stage-specific metabolic vulnerabilities. We show that a VHL loss-dependent reprogramming of branched-chain amino acid catabolism sustains the de novo biosynthesis of aspartate and arginine enabling tumor cells with the flexibility of partitioning the nitrogen of the amino acids depending on their needs. Importantly, we identify the epigenetic reactivation of argininosuccinate synthase (ASS1), a urea cycle enzyme suppressed in primary ccRCC, as a crucial event for metastatic renal cancer cells to acquire the capability to generate arginine, invade in vitro and metastasize in vivo. Overall, our study uncovers a mechanism of metabolic flexibility occurring during ccRCC progression, paving the way for the development of novel stage-specific therapies.

Brazilian cattle production is mostly carried out in pastures, and the need to mitigate the livestock's greenhouse gas (GHG) emissions and its environmental footprint has become an important requirement. The adoption of well-suited breeds and the intensification of pasture-based livestock production systems are alternatives to optimize the sector's land use. However, further research on tropical systems is necessary. The objective of this research was to evaluate the effect of Holstein (HO) and Jersey-Holstein (JE x HO) crossbred cows in different levels of pasture intensification (continuous grazing system with low stocking rate-CLS; irrigated rotational grazing system with high stocking rate-RHS), and the interaction between these two factors on GHG mitigation. Twenty-four HO and 24 JE x HO crossbred dairy cows were used to evaluate the effect of two grazing systems on milk production and composition, soil GHG emissions, methane (CH4) emission, and soil carbon accumulation (0-100 cm). These variables were used to calculate carbon balance (CB), GHG emission intensity, the number of trees required to mitigate GHG emission, and the land-saving effect. The number of trees necessary to mitigate GHG emission was calculated, considering the C balance within the farm gate. The mitigation of GHG emissions comes from the annual growth rate and accumulation of C in eucalyptus trees' trunks. The CB of all systems and genotypes presented a deficit in carbon (C); there was no difference for genotypes, but RHS was more deficient than CLS (-4.99 to CLS and -28.72 to RHS ton CO2e..ha-1.year-1). The deficit of C on GHG emission intensity was similar between genotypes and higher for RHS (-0.480 to RHS and -0.299 to CLS kg CO2e..kg FCPCmilk-1). Lower GHG removals (0.14 to CLS higher than 0.02 to RHS kg CO2e..kg FCPCmilk-1) had the greatest influence on the GHG emission intensity of milk production. The deficit number of trees to abatement emissions was higher to HO (-46.06 to HO and -38.37 trees/cow to JE x HO) and to RHS (-51.9 to RHS and -33.05 trees/cow to CLS). However, when the results are expressed per ton of FCPCmilk, there was a difference only between pasture management, requiring -6.34 tree. ton FCPCmilk-1 for the RHS and -3.99 tree. ton FCPCmilk-1 for the CLS system. The intensification of pastures resulted in higher milk production and land-saving effect of 2.7 ha. Due to the reservation of the pasture-based dairy systems in increasing soil C sequestration to offset the GHG emissions, especially enteric CH4, planting trees can be used as a mitigation strategy. Also, the land-save effect of intensification can contribute to the issue, since the area spared through the intensification in pasture management becomes available for reforestation with commercial trees.
