Introducción: La revascularización mediante intervención coronaria percutánea con colocación de stent, o cirugía de derivación coronaria, alivia la isquemia miocárdica. Sin embargo, las pruebas de estrés no invasivas y la angiografía coronaria no siempre proporcionan la información adecuada sobre la importancia funcional de las estenosis en las arterias coronarias. Objetivo: Describir las recomendaciones actuales de la literatura médica con respecto a la fisiología coronaria en la cardiopatía isquémica. Desarrollo: El índice de reserva fraccional de flujo se considera el estándar de oro para detectar la isquemia miocárdica. Su naturaleza invasiva se equilibra con la resolución espacial inigualable y su relación lineal con el flujo sanguíneo máximo. Los resultados clínicos de pacientes cuya estrategia de revascularización se basa en mediciones de reserva fraccional de flujo son decisivos en varios subconjuntos de diferentes lesiones. En la última década se ha propuesto la evaluación de la gravedad de la estenosis coronaria mediante índices no hiperémicos. Sin embargo, la precisión de estos índices para distinguir correctamente la isquemia miocárdica es solo del 80 %. Conclusiones: Actualmente, la hiperemia máxima se recomienda para una óptima toma de decisiones sobre la revascularización.


Introduction: Revascularization by percutaneous coronary intervention with stenting, or coronary bypass surgery, alleviates myocardial ischemia. However, noninvasive stress testing and angiography do not always provide adequate information on the functional significance of coronary artery stenoses. Objective: To describe the recommendations of the medical literature regarding coronary physiology in ischemic heart disease. Development: The fractional flow reserve index is considered the gold standard for detecting myocardial ischemia. Its invasive nature is balanced by unmatched spatial resolution and its linear relationship to peak blood flow. The clinical outcomes of patients whose revascularization strategy is based on fractional flow reserve measurements are decisive in several subsets of other lesions. In the last decade, assessment of coronary stenosis severity by non-hyperemic indices has been proposed; however, the accuracy of these indices to correctly distinguish myocardial ischemia does not exceed 80%. Conclusions: Currently, maximal hyperemia is recommended for optimal revascularization decision making.

From chemistry design to clinical application, several approaches have been developed to overcome platinum drawbacks in antitumoral therapies. An in-depth understanding of intracellular signaling may hold the key to the relationship of both conventional drugs and nanoparticles. Within these strategies, first, nanotechnology has become an essential tool in oncotherapy, improving biopharmaceutical properties and providing new immunomodulatory profiles to conventional drugs mediated by activation of endoplasmic reticulum (ER) stress. Secondly, functional proteomics techniques based on microarrays have proven to be a successful method for high throughput screening of proteins and profiling of biomolecule mechanisms of action. Here, we conducted a systematic characterization of the antitumor profile of a platinum compound conjugated with iron oxide nanoparticles (IONPs). As a result of the nano-conjugation, cytotoxic and proteomics profiles revealed a significant improvement in the antitumor properties of the starting material, providing selectivity in certain tumor cell lines tested. Moreover, cell death patterns associated with immunogenic cell death (ICD) response have also been identified when ER signaling pathways have been triggered. The evaluation in several tumor cell lines and the analysis by functional proteomics techniques have shown novel perspectives on the design of new cisplatin-derived conjugates, the high value of IONPs as drug delivery systems and ICD as a rewarding approach for targeted oncotherapy and onco-immunotherapies.

BACKGROUND: Nowadays, nanoparticles (NPs) have evolved as multifunctional systems combining different custom anchorages which opens a wide range of applications in biomedical research. Thus, their pharmacological involvements require more comprehensive analysis and novel nanodrugs should be characterized by both chemically and biological point of view. Within the wide variety of biocompatible nanosystems, iron oxide nanoparticles (IONPs) present mostly of the required features which make them suitable for multifunctional NPs with many biopharmaceutical applications. RESULTS: Cisplatin-IONPs and different functionalization stages have been broadly evaluated. The potential application of these nanodrugs in onco-therapies has been assessed by studying in vitro biocompatibility (interactions with environment) by proteomics characterization the determination of protein corona in different proximal fluids (human plasma, rabbit plasma and fetal bovine serum),. Moreover, protein labeling and LC-MS/MS analysis provided more than 4000 proteins de novo synthetized as consequence of the nanodrugs presence defending cell signaling in different tumor cell types (data available via ProteomeXchanges with identified PXD026615). Further in vivo studies have provided a more integrative view of the biopharmaceutical perspectives of IONPs. CONCLUSIONS: Pharmacological proteomic profile different behavior between species and different affinity of protein coating layers (soft and hard corona). Also, intracellular signaling exposed differences between tumor cell lines studied. First approaches in animal model reveal the potential of theses NPs as drug delivery vehicles and confirm cisplatin compounds as strengthened antitumoral agents.

RESUMEN Fundamento las enfermedades no transmisibles han estado entre las primeras causas de morbi-mortalidad en el mundo y en Cuba desde hace más de 20 años. Entre ellas se destacan: la obesidad, la hipertensión arterial y la diabetes mellitus. Estas patologías aparecen desde la infancia y su prevención debe comenzar desde edades tempranas. Objetivo: determinar la obesidad, estratificar el riesgo cardiometabólico y promover estilos de vida saludable en escolares de Santa Clara. Métodos se realizó una investigación epidemiológica, descriptiva con diseño transversal a 959 escolares entre 8 y 12 años de Santa Clara y entre los años 2012 al 2020. Se desarrollaron programas computacionales, que permitieron estudiar la obesidad y calcular los índices de masa corporal, de cintura-cadera y de cintura-talla. Se incluyeron variables como: edad, sexo, color de la piel, peso, talla, circunferencia de la cintura, circunferencia de la cadera, tensión arterial, antecedentes patológicos personales y familiares, complementarios de laboratorio clínico, electrocardiogramas y ecocardiogramas. Se emplearon tablas de contingencia con doble entrada, para registrar la frecuencia de aparición de los valores de dos variables simultáneamente. Se aplicaron pruebas no paramétricas, como el Chi Cuadrado para independencia de factores y prueba de hipótesis de diferencia de proporción para grupos independientes. Resultados se detectaron un 17,94 % de escolares con sobrepeso y 15,95 % con obesidad. Fueron obesos moderados 4,48 % y 9,18 % severos. Aumentaron las medias de colesterol, triglicéridos y ácido úrico con el grado de obesidad. Se desarrollaron estimaciones de riesgo cardiometabólico en formatos Androide y WEB. Conclusiones existen altos porcentajes de obesos y sobrepesos. El índice cintura-cadera fue el mejor predictor de riesgo cardiometabólico en la investigación realizada en Santa Clara. Se requiere una mayor intervención clínica y educativa, que garantice la adopción de estilos de vida más saludable.


ABSTRACT Background: noncommunicable diseases have been among the leading causes of morbidity and mortality in the world and in Cuba for more than 20 years. These include: obesity, high blood pressure and diabetes mellitus. These pathologies appear from childhood and their prevention must begin from an early age. Objectives: to determine obesity, stratify cardiometabolic risk and promote healthy lifestyles in schoolchildren from Santa Clara. Methods: an epidemiological, descriptive investigation with a cross-sectional design was carried out on 959 schoolchildren between 8 and 12 years of age in Santa Clara and between 2012 and 2020. Computer programs were developed, which allowed studying obesity and calculating body mass indexes, waist -hip and waist-height. Variables were included such as: age, sex, skin color, weight, height, waist circumference, hip circumference, blood pressure, personal and family pathological history, clinical laboratory tests, electrocardiograms and echocardiograms. Double-entry contingency tables were used to record the frequency of appearance of the values ​​of two variables simultaneously. Non-parametric tests were applied, such as Chi Square for factor independence and proportion difference hypothesis test for independent groups. Results: 17.94 % of schoolchildren were overweight and 15.95 % obese. 4.48 % were moderately obese and 9.18 % were severely obese. The means of cholesterol, triglycerides and uric acid increased with the degree of obesity. Cardiometabolic risk estimates were developed in Android and WEB formats. Conclusions: there are high percentages of obese and overweight. The waist-hip ratio was the best predictor of cardiometabolic risk in the study conducted in Santa Clara. Greater clinical and educational intervention is required, which guarantees the adoption of healthier lifestyles.

Desde las perspectivas actuales, la hipertensión pulmonar es considerada un importante problema sanitario. El objetivo del artículo fue identificar las características epidemiológicas e historia natural de la hipertensión pulmonar en el contexto internacional y principalmente en la región americana. El conocimiento de la epidemiología de la hipertensión pulmonar ha experimentado un notable desarrollo con los resultados de los registros americanos, franceses, suizos, entre otros. Reportes iniciales consideraban que la enfermedad afectaba a pacientes jóvenes (edad promedio 36 años), siendo casi dos veces más frecuente en mujeres que en varones (1,7:1) y con una incidencia de 1-2 casos/1 000 000 habitantes/año y afecta a todos los grupos etarios. Las estimaciones actuales sugieren una prevalencia alrededor del 1 por ciento de la población mundial y aumenta hasta el 10 por ciento en los mayores de 65 años. La enfermedad auricular o ventricular izquierda y las enfermedades pulmonares son la causa más frecuente de hipertensión pulmonar. En Cuba no hay datos epidemiológicos disponibles sobre esta entidad. Los programas que ayuden a su conocimiento por la población médica se deben reforzar e impulsar un registro único de datos(AU)


From current perspectives, pulmonary hypertension is considered a major health problem. The present work was carried out to identify the epidemiological characteristics and natural history of pulmonary hypertension in the international context and mainly in the Americas. Knowledge of the epidemiology of pulmonary hypertension has undergone a remarkable development with the results of the American, French, and Swiss registries, among others. Initial reports considered that the disease affected young patients (average age 36 years), being almost twice more frequent in women than in men (1.7:1) and with an incidence of 1-2 cases/1,000,000 inhabitants/ year and affects all age groups. Current estimates suggest a prevalence around 1 percent of the world population and increases to 10 percent in those over 65 years of age. Left atrial or left ventricular disease and pulmonary diseases are the most common cause of pulmonary hypertension. In Cuba there are no epidemiological data available on this entity. The programs that help their knowledge by the medical population must be reinforced and promote a single data registry(AU)

Introducción: En los últimos años la utilización de la vía radial como forma de acceso para la realización de estudios diagnósticos e intervencionista ha cobrado mayor importancia e interés dentro de los servicios de cardiología intervencionista a nivel mundial. Nuestro país no escapa de este auge por la baja tasa de complicación y la mayor comodidad para el paciente. Objetivo: Caracterizar la intervención coronaria percutánea mediante la vía de acceso radial en pacientes atendidos en el Instituto de Cardiología y Cirugía Cardiovascular. Método: Estudio observacional, descriptivo, transversal. Muestra compuesta por 211 pacientes a los que se les realizó intervencionismo coronario percutáneo mediante la vía de acceso radial. Resultados: Predominó el sexo masculino (61,2 por ciento). La edad media fue 60,1 ±9,8 años. El Síndrome Coronario Crónico (76,3 por ciento ) fue el diagnóstico mayoritario, la hipertensión arterial el factor de riesgo más frecuente (73,9 por ciento ) y entre los antecedentes personales la cardiopatía isquémica (30,3 por ciento ). El intervencionismo de forma electiva se realizó en el 76,3 de los casos, utilizando la vía radial derecha en el 88,2 por ciento . Se demostró enfermedad de 2 y 3 vasos en el 45,1 por ciento de los pacientes. El proceder fue exitoso en el 96,7 por ciento de los pacientes. Las variables que demostraron relación estadísticamente significativa con el fracaso fueron: fracción de eyección del ventrículo izquierdo < 40 por ciento (p<0.0001), filtrado glomerular ≤ 60 ml/min (p=0.002), antecedente de cardiopatía isquémica (p= 0.016) y presencia de enfermedad coronaria de 3 vasos (p= 0.019). Conclusiones: La intervención coronaria percutánea mediante el acceso radial en el ICCCV es segura y eficaz, con una tasa de éxito elevada y escasas complicaciones(AU)


Introduction: In recent years the use of the radial approach as a form of access for diagnostic and interventional studies has become increasingly important and of interest in interventional cardiology services worldwide. Our country has not escaped this boom due to the low complication rate and greater patient comfort. Objective: To characterise percutaneous coronary intervention using the radial access route in patients attended at the Institute of Cardiology and Cardiovascular Surgery. Methods: Observational, descriptive, cross-sectional study. The sample consisted of 211 patients who underwent percutaneous coronary intervention via the radial access route. Results: Male sex predominated (61.2 percent). Mean age was 60.1 ±9.8 years. Chronic coronary syndrome (76.3 percent) was the most common diagnosis, hypertension the most frequent risk factor (73.9) percent and ischaemic heart disease (30.3 percent ). Elective intervention was performed in 76.3 percent of cases, using the right radial approach in 88.2 percent . Two- and three-vessel disease was demonstrated in 45.1 percent of patients. The procedure was successful in 96.7 percent of patients. Variables demonstrating statistically significant relationship with failure were: left ventricular ejection fraction < 40 percent (p(0.0001), glomerular filtration rate ≤ 60 ml/min (p=0.002), history of ischaemic heart disease (p= 0.016) and presence of 3-vessel coronary artery disease (p= 0.019). Conclusions: Percutaneous coronary intervention via radial access in ICCCV is safe and effective, with a high success rate and few complications(AU)

RESUMEN La trombosis muy tardía de stent es un fenómeno de baja frecuencia, pero de elevada morbilidad y mortalidad. Dentro de sus factores predisponentes se encuentran parámetros clínicos, anatómicos y relacionados con el procedimiento. Múltiples son los mecanismos fisiopatológicos que se plantean como responsables de la trombosis de stent. El tratamiento de esta complicación consiste en intentar restaurar el flujo del vaso lo antes posible. Se presenta el caso de un paciente con el diagnóstico de infarto agudo de miocardio con elevación del segmento ST secundario a trombosis muy tardía de stent metálico convencional (20 meses), tratado exitosamente mediante intervencionismo coronario percutáneo con dos stents liberadores de sirolimus.


ABSTRACT Very late stent thrombosis is a rare complication but with high morbidity and mortality. Predisposing factors include clinical, anatomical and procedure-related parameters. Many pathophysiological mechanisms are considered to be responsible for stent thrombosis. The treatment of this complication consists of attempting to restore blood flow as soon as possible. We present the case of an individual diagnosed with ST-segment elevation myocardial infarction after very late thrombosis of conventional bare metal stent (20 months). The patient was successfully treated by percutaneous coronary intervention with two sirolimus-eluting stents.

BACKGROUND: Smoking has been associated with poorer outcomes in relation to COVID-19. Smokers have higher risk of mortality and have a more severe clinical course. There is paucity of data available on this issue, and a definitive link between smoking and COVID-19 prognosis has yet to be established. METHODS: We included 5224 patients with COVID-19 with an available smoking history in a multicentre international registry Health Outcome Predictive Evaluation for COVID-19 (NCT04334291). Patients were included following an in-hospital admission with a COVID-19 diagnosis. We analysed the outcomes of patients with a current or prior history of smoking compared with the non-smoking group. The primary endpoint was all-cause in-hospital death. RESULTS: Finally, 5224 patients with COVID-19 with available smoking status were analysed. A total of 3983 (67.9%) patients were non-smokers, 934 (15.9%) were former smokers and 307 (5.2%) were active smokers. The median age was 66 years (IQR 52.0-77.0) and 58.6% were male. The most frequent comorbidities were hypertension (48.5%) and dyslipidaemia (33.0%). A relevant lung disease was present in 19.4%. In-hospital complications such sepsis (23.6%) and embolic events (4.3%) occurred more frequently in the smoker group (p<0.001 for both). All cause-death was higher among smokers (active or former smokers) compared with non-smokers (27.6 vs 18.4%, p<0.001). Following a multivariate analysis, current smoking was considered as an independent predictor of mortality (OR 1.77, 95% CI 1.11 to 2.82, p=0.017) and a combined endpoint of severe disease (OR 1.68, 95% CI 1.16 to 2.43, p=0.006). CONCLUSION: Smoking has a negative prognostic impact on patients hospitalised with COVID-19.

Introducción: Se ha descrito una probable asociación entre la presencia de osteopenia/osteoporosis y el riesgo incrementado de cardiopatía isquémica. Objetivo: Determinar la posible asociación entre la presencia de síndrome coronario agudo y la densidad mineral ósea disminuida, así como la relación de ambas condiciones con algunos factores de riesgo cardiovascular y variables de la esfera reproductiva en mujeres en etapa de climaterio. Método: Se realizó un estudio transversal descriptivo con 72 mujeres (34 con síndrome coronario agudo y 38 sin síndrome coronario agudo), que fueron seleccionadas de bases de datos del Instituto de Cardiología y Cirugía Cardiovascular. La densidad mineral ósea se determinó mediante absorciometría dual de rayos X en columna lumbar. Las pruebas Chi cuadrado y U de Mann Whitney permitieron evaluar la posible relación entre variables. Resultados: El 55,9 por ciento de las pacientes con síndrome coronario agudo y el 60,5 por ciento de las mujeres sin síndrome coronario agudo tenían densidad mineral ósea disminuida. En las mujeres con densidad mineral ósea disminuida (n=42): 81 por ciento presentaron obesidad abdominal, 78,6 por ciento dislipoproteinemia, 83,3 por ciento hipertensión arterial y 76,2 por ciento refirieron el antecedente familiar de cardiopatía isquémica. Conclusiones: En las mujeres en etapa de climaterio estudiadas no se demostró asociación entre la presencia de síndrome coronario agudo y la densidad mineral ósea disminuida. Tampoco existió relación entre la presencia de síndrome coronario agudo y la densidad mineral ósea disminuida con factores de riesgo cardiovascular, ni con las variables de la esfera reproductiva(AU)


Introduction: A probable association has been described between the presence of osteopenia/osteoporosis and the increased risk of ischemic heart disease. Objective: To determine the possible association between the presence of acute coronary syndrome and decreased bone mineral density, as well as the relationship of both conditions with some cardiovascular risk factors and variables of the reproductive sphere in women during the climacteric stage. Method: A descriptive and cross-sectional study was carried out with 72 women (34 with acute coronary syndrome and 38 without acute coronary syndrome), who were selected from databases of the Institute of Cardiology and Cardiovascular Surgery. Bone mineral density was determined by dual lumbar spine X-ray absorptiometry. The chi-square and Mann Whitney U tests allowed to evaluate the possible relationship between variables. Results: 55.9 percent of the patients with acute coronary syndrome and 60.5 percent of the women without acute coronary syndrome had decreased bone mineral density. Among women with decreased bone mineral density (n=42), 81 percent had abdominal obesity, 78.6 percent had dyslipoproteinemia, 83.3 percent had arterial hypertension, and 76.2 percent had a family history of ischemic heart disease. Conclusions: In the women in the climacteric stage studied, no association was shown between the presence of acute coronary syndrome and decreased bone mineral density. There was no relationship either between the presence of acute coronary syndrome and decreased bone mineral density with cardiovascular risk factors, or with variables in the reproductive sphere(AU)

Abstract Recent studies have reported the occurrence of thrombotic phenomena or coagulopathy in patients with COVID-19. There are divergent positions regarding the prevention, diagnosis, and treatment of these phenomena, and current clinical practice is based solely on deductions by extension from retrospective studies, case series, observational studies, and international guidelines developed prior to the pandemic. In this context, the aim was to generate a group of recommendations on the prevention, diagnosis and management of thrombotic complications associated with COVID-19. Methods: A rapid guidance was carried out applying the GRADE Evidence to Decision (EtD) frameworks and an iterative participation system, with statistical and qualitative analysis. Results: 31 clinical recommendations were generated focused on: a) Coagulation tests in symptomatic adults with suspected infection or confirmed SARS CoV-2 infection; b) Thromboprophylaxis in adults diagnosed with COVID-19 (Risk scales, thromboprophylaxis for outpatient, in-hospital management, and duration of thromboprophylaxis after discharge from hospitalization), c) Diagnosis and treatment of thrombotic complications, and d) Management of people with previous indication of anticoagulant agents. Conclusions: Recommendations of this consensus guide clinical decision-making regarding the prevention, diagnosis, and treatment of thrombotic phenomena in patients with COVID-19, and represent an agreement that will help decrease the dispersion in clinical practices according to the challenge imposed by the pandemic.

There is limited information on the presenting characteristics, prognosis, and therapeutic approaches of young patients hospitalized for coronavirus disease 2019 (COVID-19). We sought to investigate the baseline characteristics, in-hospital treatment, and outcomes of a wide cohort < 65 years admitted for COVID-19. Using the international multicenter HOPE-COVID-19 registry, we evaluated the baseline characteristics, clinical presentation, therapeutic approach, and prognosis of patients < 65 years discharged (deceased or alive) after hospital admission for COVID-19, also compared with the elderly counterpart. Of the included 5746 patients, 2676 were < 65 and 3070 ≥ 65 years. All risk factors and several parameters suggestive of worse clinical presentation augmented through increasing age classes. In-hospital mortality rates were 6.8% and 32.1% in the younger and older cohort, respectively (p < 0.001). Among young patients, mortality, access to ICU and treatment with IMVwere positively correlated with age. Contrariwise, over 65 years of age this trend was broken so that only the association between age and mortality was persistent, while the rates of access to ICU and IMV started to decline. Younger patients also recognized specific predictors of case fatality, such as obesity and gender. Age negatively impacts on mortality, access to ICU and treatment with IMV in patients < 65 years. In elderly patients only case fatality rate keeps augmenting in a stepwise manner through increasing age categories, while therapeutic approaches become more conservative. Besides age, obesity, gender, history of cancer, and severe dyspnea, tachypnea, chest X-ray bilateral abnormalities, abnormal level of creatinine and leucocyte among admission parameters seem to play a central role in the outcome of patients younger than 65 years.

La enfermedad infecciosa causada por el nuevo coronavirus-2019 (COVID-19), produce un síndrome respiratorio agudo con implicaciones significativas en el sistema cardiovascular tanto en los pacientes con enfermedad cardiovascular preexistente (ECV), que poseen un riesgo de empeoramiento de su enfermedad y muerte, como en los que la infección se ha asociado con múltiples complicaciones directas e indirectas cardiovasculares. Las complicaciones cardiovasculares asociadas con más frecuencia a esta entidad incluyen lesión aguda del miocardio, miocarditis, síndromes coronarios agudos, arritmias y tromboembolismo venoso. Por otra parte, las terapias empleadas para el tratamiento de la enfermedad por COVID-19 pueden tener interacciones medicamentosas con los fármacos cardiovasculares ensombreciendo el pronóstico de los pacientes. El artículo resalta las distintas recomendaciones expuestas por expertos en la literatura médica respecto a las complicaciones cardiovasculares relacionadas con el nuevo coronavirus (COVID-19)(AU)


The infectious disease caused by the new coronavirus-2019 (COVID-19), produces an acute respiratory syndrome with significant implications for the cardiovascular system both in patients with pre-existing cardiovascular disease (CVD), who have a risk of worsening their disease and death, such as those in which the infection has been associated with multiple direct and indirect cardiovascular complications. The cardiovascular complications most frequently associated with this entity include acute myocardial injury, myocarditis, acute coronary syndromes, arrhythmias, and venous thromboembolism. Furthermore, the therapies used to treat COVID-19 disease may have drug interactions with cardiovascular drugs, darkening the prognosis of the patients. The present work attempts to highlight the different recommendations made by experts in the medical literature regarding cardiovascular complications related to the new coronavirus (COVID-19)(AU)

Objective: to share our experience in the radial distal approach to perform coronary angiography and / or dilation of coronary stenosis. The initial results are exposed in the access of the distal radial artery or in the so-called anatomical "snuff box". Methods: The left or right radial distal artery was used as an access site in 6 patients admitted to the Institute of Cardiology and Cardiovascular Surgery for coronary angiography and dilatation of coronary stenosis between May 6 and June 6 of 2019. All with pulse present in their distal radial artery. In the laboratory, they had the access arm flexed with respect to the homolateral shoulder so that the hand was placed on their contralateral groin. In all cases, the operator was placed on the right side of the patient, to perform coronary angiography and / or dilation of coronary stenosis. During the hospital stay, the main demographic characteristics and complications were recorded. Results: the average age of the patients was 72 +/- 10 years and 83.3 percent were men. We use the Judkins and Amplatz 6 and 5 French catheters for the procedures. Five admitted with the diagnosis of angina and one with severe aortic valve stenosis. In 5 patients, distal transradial coronary angiography was successfully performed. Only one patient experienced distal radial spasm, using in this case the ipsilateral proximal radial access, without making it necessary to approach the femoral artery. In total, 4 patients had coronary intervention, performing successfully in the 4 patients. The right coronary artery was the artery that required most of the intervention (3 patients). There were no cases of occlusion of the radial arteries, no hematoma of the hand, of the B.A.R.C scale greater than 1 was observed in any patient. Numbness of the hand was documented in none. The radial introducer was removed at the end of the procedure. Hemostasis was achieved with manual compression. Conclusion: the distal radial approach is feasible in a selected group of patients as a procedure for coronary angiography and dilatation of coronary stenosis(AU)


Objetivo: compartir nuestra experiencia en el abordaje radial distal para realizar la angiografía coronaria y/o la dilatación de la estenosis coronaria. Se exponen los resultados iniciales en el acceso de la arteria radial distal o en la llamada "tabaquera" anatómica. Métodos: se utilizó la arteria radial distal izquierda o derecha como sitio de acceso en 6 pacientes ingresados en el Instituto de Cardiología y Cirugía Cardiovascular para angiografía coronaria y dilatación de estenosis coronaria del 6 de mayo al 6 de junio de 2019. Todos los pacientes tenían pulso en la arteria radial distal. En el laboratorio, se les flexionó el brazo de acceso con respecto al hombro homolateral de modo que la mano descansara sobre la ingle contralateral. En todos los casos, el técnico se situó a la derecha del paciente para realizar la angiografía coronaria y/o dilatación de la estenosis coronaria. Durante la estancia de los pacientes en el hospital, se registraron sus principales características demográficas y complicaciones. Resultados: la edad promedio de los pacientes fue de 72 +/- 10 años y 83,3 por ciento eran hombres. Usamos catéteres franceses Judkins y Amplatz 6 y 5 para los procedimientos. Cinco pacientes habían sido ingresados con un diagnóstico de angina y uno con estenosis valvular aórtica severa. La angiografía coronaria transradial distal fue exitosa en 5 pacientes. Solo un paciente experimentó un espasmo distal radial, usándose en ese caso el acceso radial proximal ipsilateral sin que fuera necesario abordar la arteria femoral. Un total de 4 pacientes se sometieron a intervención coronaria, la que fue exitosa en los 4. La arteria coronaria derecha fue la que requirió la mayor parte de la intervención (3 pacientes). No hubo ningún caso de oclusión de las arterias radiales ni de hematoma de la mano. Tampoco se observó un valor de la escala BARC mayor de 1 en ningún paciente. No se documentó entumecimiento de la mano en ningún paciente. El introductor radial se retiró al final del procedimiento. La hemostasia se alcanzó mediante compresión manual. Conclusión: el abordaje radial distal es factible en un grupo seleccionado de pacientes como procedimiento para la angiografía coronaria y la dilatación de la estenosis coronaria(AU)

ABSTRACT Objective Our study analyses differences in performance between sexes, and changes in performance between age groups at Olympic distance during the ITU Duathlon World Championships, held between 2005 and 2016. During this period, a total of 9,772 duathletes were analysed (6,739 men and 3,033 women). Methods Two-way analyses of variance (ANOVA) were used to examine sex- and age-related differences in performance (time, percentage of time and performance ratio) in the first running and cycling legs, the second running leg, and total race for the top 10 male and female athletes in each age group at the Duathlon World Championships. Results The age group with the highest participation, in both male and female categories, was 40-44 years, and it was found that the mean age of female finisher participants across all age groups was 23.5±12. With regards to performance, the best results for total race time and the cycling segment were achieved in the 30-34-year age group, for both male and female athletes. With regards to performance in the first and third segments (running legs), the best times were achieved in the 25-29 and 30-34 age groups, for men and women respectively. Conclusion According to the results of our study, the best results in the professional career of a duathlete are achieved at between 30 and 35 years, therefore the athlete should incorporate this factor into their training plan. RESUMO Objetivo Nosso estudo analisa as diferenças de desempenho entre os sexos e as mudanças do desempenho entre as faixas etárias quanto à distância olímpica durante o Campeonato Mundial de Duatlo da ITU, realizado entre 2005 e 2016. Nesse período, foram analisados 9.772 duatletas (6.739 homens e 3.033 mulheres). Métodos As análises de variância bidirecional (ANOVA) foram usadas para examinar as diferenças de desempenho relacionadas com sexo e idade (tempo, porcentagem de tempo e coeficiente de desempenho) na primeira corrida, ciclismo, segunda corrida e competição total dos 10 melhores atletas do sexo masculino e feminino em cada faixa etária no Campeonato Mundial de Duatlo. Resultados A faixa etária com maior participação, tanto dos homens quanto das mulheres foi de 40 a 44 anos, e verificou-se que a média de idade das participantes do sexo feminino que concluíram a competição em todas as faixas etárias foi de 23,5 ± 12. Com relação ao desempenho, os melhores resultados para o tempo total de competição e o segmento de ciclismo foram na faixa etária de 30 a 34 anos, tanto em atletas masculinos quanto femininos. Com relação ao desempenho no primeiro e terceiro segmentos (corridas), o melhor desempenho foi alcançado nas faixas de 25 a 29 e 30 a 34 anos, em homens e mulheres, respectivamente. Conclusão De acordo com os resultados obtidos neste estudo, os melhores resultados na carreira profissional de um duatleta poderiam ser alcançados entre 30 e 35 anos, portanto o atleta deve incorporar esse fator em seu plano de treinamento. En ese período, fueron analizados 9.772 duatletas (6.739 hombres y 3.033 mujeres). Métodos Los análisis de variancia bidireccional (ANOVA) fueron usados para examinar las diferencias de desempeño relacionadas con sexo y edad (tiempo, porcentaje de tiempo y coeficiente de desempeño) en la primera carrera, ciclismo, segunda carrera y competición total de los 10 mejores atletas de sexo masculino y femenino en cada grupo de edad en el Campeonato Mundial de Duatlón. Resultados El grupo de edad con mayor participación, tanto de los hombres como de las mujeres, fue de 40 a 44 años y se verificó que la edad promedio de las participantes femeninas que concluyeron la competición en todos los grupos de edad fue de 23,5 ± 12,5. Con relación al desempeño, los mejores resultados para el tiempo total de competición y el segmento de ciclismo fueron en el grupo de edad de 30 a 34 años, tanto en atletas masculinos como femeninos. Con relación al desempeño en el primer y tercer segmentos (carreras), el mejor desempeño fue alcanzado en los grupos de 25 a 29 y 30 a 34 años, en hombres y mujeres, respectivamente. Conclusión De acuerdo con los resultados obtenidos en este estudio, los mejores resultados en la carrera profesional de un duatleta podrían ser alcanzados entre 30 y 35 años, por lo tanto el atleta debe incorporar ese factor en su plan de entrenamiento. RESUMEN Fundamento: la hipertensión arterial y la obesidad en la edad pediátrica exhiben altos niveles de prevalencia a nivel mundial, incluyendo Cuba, lo cual fundamenta su diagnóstico precoz para disminuir sus efectos en estas edades y su impacto en la adultez. Objetivo: elaborar una estrategia educativa encaminada al perfeccionamiento de la labor docente para fomentar una adecuada cultura en salud entre los escolares. Métodos: se desarrolló un estudio epidemiológico descriptivo y prospectivo de carácter transversal, que incluyó el cálculo de la obesidad y otros factores de riesgo, en escolares de varias escuelas primarias. Se emplearon métodos teóricos: análisis-síntesis, inducción-deducción y sistémico-estructural; empíricos: análisis documental y encuestas a escolares, docentes y familiares y los matemático-estadístico para los valores analizados. Resultados: en el diagnóstico clínico se encontró elevada prevalencia de factores de riesgo cardiovascular, en cifras relacionadas con hipertensión arterial, obesidad, sobrepeso, sedentarismo y otros; mientras el diagnóstico educativo permitió conocer carencias en la preparación docente y en la familia de los escolares sobre temas de hábitos y estilos de vida saludable, por lo que se elaboró y aplicó una estrategia pedagógica para ofrecer solución a la problemática. Conclusiones: la estrategia permitió dotar a los docentes de los conocimientos y medios necesarios para promover estilos de vida saludables, desde lo curricular y extracurricular donde se fortaleció el trabajo metodológico integrando estos temas al proceso docente educativo, lo cual propició desarrollar una cultura de salud en los escolares.


ABSTRACT Background: high blood pressure and obesity in the pediatric age exhibit high levels of prevalence worldwide, including Cuba, which bases its early diagnosis to reduce its effects at these ages and its impact on adulthood. Objective: to develop an educational strategy aimed at improving the teaching work to promote an adequate health culture among schoolchildren. Methods: a cross-sectional descriptive and prospective epidemiological study was developed that included the calculation of obesity and the test of sustained weight in schoolchildren of several primary schools. Theoretical methods were used: analysis-synthesis, induction-deduction and systemic-structural; empirical ones: documentary analysis and surveys to schoolchildren, teachers and family; and the mathematical-statistic for the analyzed values. Results: in the clinical diagnosis a high prevalence of cardiovascular risk factors was found, in figures related to high blood pressure, body overweight, obesity, and cardiovascular hyperactivity; while the educational diagnosis allowed knowing deficiencies in the teaching preparation and in the family of schoolchildren on topics as healthy habits and lifestyles, so an educational strategy was developed and applied to offer a solution to the problem. Conclusions: the strategy allowed teachers to provide the necessary knowledge and means to promote healthy lifestyles, inside the curriculum and out of it where methodological work was strengthened by integrating these issues into the teaching-learning process, which led to the development of a necessary health culture in the students.

Abstract Aim: To characterize the Tpeak-Tend, the Tpeak-Tend dispersion and Tpeak-Tend/QT in children and its relationship with clinical variables. Methods: Cross-sectional study in 126 children between 9 and 12 years of the Camilo Cienfuegos School in Santa Clara, Cuba. Clinical and anthropometric variables were obtained to determine their relationship with electrocardiographic parameters: Tpeak-Tend V5, Tpeak-Tend dispersion and Tpeak-Tend/QT ratio V5. In addition, laboratory tests were conducted. Results: Age and systolic blood pressure are associated with an increased probability of having values of Tpeak-Tend/QT V5 ≥75 percentile for both sexes (OR: 1.72, CI 95%: 1.02-2.91; p= 0.043), (OR: 1.08, CI 95%: 1.01-1.16; p= 0.017) respectively. The body mass index and systolic blood pressure are linearly and significantly correlated with the Tpeak-Tend/QT V5 (r= 0.224; p= 0.012) and (r= 0.220; p= 0.014) respectively. Conclusions: The age of the patients and the systolic blood pressure figures are factors that increase the probability of having values of the Tpeak-Tend/QT V5 ≥75 percentile. There was a significant linear correlation between the Tpeak-Tend/QT V5 with the body mass index and the systolic blood pressure.


Resumen Objetivo: Caracterizar el Tpico-Tfinal, la dispersión del Tpico-Tfinal y el Tpico-Tfinal/QT en niños y su relación con variables clínicas. Métodos: Estudio transversal en 126 niños entre 9 y 12 años de la escuela Camilo Cienfuegos en Santa Clara, Cuba. Se obtuvieron variables clínicas y antropométricas para determinar su relación con los parámetros electrocardiográficos: el Tpico-Tfinal V5, la dispersión del Tpico-Tfinal y el Tpico-Tfinal/QT V5. Además, se realizaron pruebas de laboratorio. Resultados: La edad y la presión arterial sistólica se asocian con una mayor probabilidad de tener valores del Tpico-Tfinal/QT V5 ≥75 percentil para ambos sexos (OR: 1.72, IC 95%: 1.02-2.91; p= 0.043), (OR: 1.08, IC 95%: 1.01-1.16; p= 0.017) respectivamente. El índice de masa corporal y la presión arterial sistólica están correlacionados de manera lineal y significativa con el Tpico-Tfinal/QT V5 (r= 0.224; p= 0.012) y (r= 0.220; p= 0.014) respectivamente. Conclusiones: La edad de los pacientes y las cifras de presión arterial sistólica son factores que aumentan la probabilidad de tener valores del percentil Tpico-Tfinal/QT V5 ≥75. Existe correlación lineal significativa entre el Tpico-Tfinal/QT V5 con el índice de masa corporal y la presión arterial sistólica.

Introducción: El virus de papiloma humano per se no es capaz de desarrollar todas las transformaciones neoplásicas en el cérvix uterino, de manera que factores de riesgo como los genéticos, ambientales, estilo de vida sexual y el desbalance oxidativo podrían contribuir a la enfermedad. Objetivo: Determinar el comportamiento del sistema enzimático antioxidante en mujeres con atipia de células escamosas de significado indeterminado y neoplasia intraepitelial cervical grado I. Métodos: Se conformaron tres grupos de estudio: el primero incluyó 30 mujeres con diagnóstico de atipias, el segundo se constituyó con 40 mujeres con neoplasia intraepitelial grado I y el tercero consistió en 30 mujeres con citología negativa tomadas como control, provenientes de la consulta de Patología de Cuello del Agustín Gómez Lubián de Santa Clara. Mediante métodos espectrofotométricos se determinaron los niveles de actividad enzimática superóxido dismutasa y catalasa así, como las concentraciones de glutatión reducido. Las comparaciones se realizaron con el programa SPSS, versión 18. Resultados: En el grupo de atipias aunque los tres parámetros tuvieron una tendencia a la disminución no hubo diferencias significativas con respecto al control. Mientras que el grupo de neoplasia grado I evidenció disminución significativa de los tres indicadores estudiados al ser comparados con el grupo control. Conclusiones: Se constató afectación del sistema antioxidante enzimático en el grupo de neoplasia grado I, lo cual podría considerarse un cofactor importante en la progresión de las lesiones en el cérvix uterino(AU)


Introduction: Human papillomavirus per se can not carry out all the neoplastic transformations occurring in the uterine cervix. Genetic and environmental risk factors as well as sexual behavior and oxidative imbalance may also play a role. Objective: Determine the behavior of the enzymatic antioxidant system in women with squamous cell atypia of indeterminate significance and grade I cervical intraepithelial neoplasia. Methods: Three study groups were formed. The first group included 30 women diagnosed with atypia, the second group was made up of 40 women with grade I intraepithelial neoplasia, and the third or control group consisted of 30 women with negative cytology from Agustín Gómez Lubián Cervical Pathology service in Santa Clara. Spectrophotometric methods were used to determine the levels of superoxide dismutase and catalase enzymatic activity, as well as the concentrations of reduced glutathione. Comparisons were made with the SPSS software, version 18. Results: In the atypia group the three parameters showed a decreasing tendency, but differences with respect to the control group were not significant. In the grade I neoplasia group, however, a significant reduction was found of the three study indicators when compared with the control group. Conclusions: Damage to the enzymatic antioxidant system was observed in the grade I neoplasia group. This could be considered to be an important cofactor in the progress of uterine cervix lesions(AU)

HYPOTHESIS: The distribution of antioxidants (AOs) in O/W emulsions depends on two partition constants, that between oil-interfacial (POI), and that between the aqueous-interfacial (PWI) regions, that need to be determined in the unbroken emulsion to prevent disruptions of the present equilibria. Prediction of the effects of temperature on AO partitioning is challenging because may change both POI and PWI in a different extent and the control of their interfacial concentrations is crucial to optimize their antioxidant efficiency. Such effects can be analyzed in the intact emulsions with the aid of a pseudophase kinetic model. EXPERIMENTS: Here we estimated the partition constants of the food-grade antioxidants -propyl (PG), octyl (OG) and lauryl (LG) gallates- in intact corn oil-in-water emulsions and their interfacial concentrations by employing a kinetic methods, and carried out a thermodynamic analysis of the transfer parameters controlling their partitioning. FINDINGS: Results show that the Gibbs free energy for the transfer of gallates to the interfacial region is spontaneous and the transfer process is enthalpy driven. An increase in T favors their incorporation into the interfacial region in an extent that depends on AO hydrophobicity. For any of AOs, the effective interfacial concentrations are much higher (15-170 fold) than the stoichiometric concentration. Results are basic to get a deeper knowledge on the driving force that partitions the AOs in lipid-based foods and to select the best AO to minimize the oxidation of lipids.

RESUMEN Mujer de 72 años de edad, con antecedentes de angina de esfuerzo estable, a quien tras presentar un síndrome coronario agudo sin elevación del segmento ST, se le realizó angiografía coronaria convencional (invasiva), donde se evidenció enfermedad arterial coronaria multivaso con estenosis significativa del tronco coronario izquierdo y se decidió tratamiento quirúrgico. A los 70 minutos presentó dolor en el flanco izquierdo y región lumbar ipsilateral, náuseas y vómitos. Al examen físico inicial se evidenció masa palpable, dolorosa, en flanco izquierdo. La tomografía demostró la presencia de un hematoma renal subcapsular izquierdo. Al cuadro de abdomen agudo se sumó choque hipovolémico, por lo que la paciente fue intervenida quirúrgicamente, donde se comprobó el diagnóstico tomográfico y se realizó nefrectomía. Este caso tiene la curiosidad de presentar factores predisponentes para causa espontánea y traumática; sin embargo, en base a los hallazgos en el estudio anatomopatológico se concluyó como hematoma renal subcapsular de origen traumático.


ABSTRACT A 72-year-old woman, with a history of stable effort angina, who was performed, after presenting a non-ST segment elevation acute coronary syndrome, a conventional coronary angiography (invasive) that evidenced the multivessel coronary artery disease with significant stenosis of the left main coronary artery, therefore, a surgical treatment was decided. At 70 minutes she presented pain in the left side and ipsilateral lumbar region, nausea and vomiting. The initial physical examination evidenced a palpable, painful mass in the left side. The tomography showed the presence of a left subcapsular renal hematoma. To the acute abdomen pattern was added a hypovolemic shock, thus, the patient underwent surgery, confirming the tomographic diagnosis and performing a nephrectomy. This particular case presented predisposing factors for spontaneous and traumatic cause; however, based on the findings in the anatomopathological study, the conclusion was a renal subcapsular hematoma of traumatic origin.
