La diverticulitis apendicular es una enfermedad poco frecuente, con una incidencia aproximada de 1%. Se define por la presencia de divertículos verdaderos o falsos en la pared del apéndice cecal. Durante la fase aguda posee una clínica indistinguible a la apendicitis, sin embargo en ocasiones presenta características clínicas particulares que la distinguen de la apendicitis aguda, tales como la presencia de dolor abdominal insidioso o intermitente y/o ausencia de sintomatología gastrointestinal (náuseas, vómitos o anorexia). En la diverticulitis apendicular las técnicas imagenológicas son de utilidad limitada al otorgar información inespecífica, por lo que el diagnóstico tiende a realizarse mediante el estudio anatomopatológico del apéndice posterior a una intervención quirúrgica en paciente con cuadro clínico compatible con apendicitis aguda. El tratamiento de elección corresponde a la apendicectomía, lo que permite evitar complicaciones futuras como por ejemplo perforación apendicular, neoplasias, entre otros. Se obtuvieron los datos de fuentes como Pubmed y Scielo. Específicamente la búsqueda de artículos originales y de revisiones sistemáticas, preferentemente menores a 15 años de publicación en revistas científicas de alto índice de impacto, con las palabras "diverticulitis apendicular", "diverticulosis" y "complicaciones diverticulares".


Appendiceal diverticulitis is a rare disease with an incidence of approximately 1%. It is defined by the presence of true or false diverticula in the wall of the cecal appendix. During the acute phase, it has symptoms that are indistinguishable from appendicitis, however, it sometimes presents particular clinical characteristics that distinguish it from acute appendicitis, such as the presence of insidious or intermittent abdominal pain and/or the absence of gastrointestinal symptoms (nausea, vomiting, or anorexia). In appendiceal diverticulitis, imaging techniques are of limited utility as they provide non-specific information, so the diagnosis tends to be made through the pathology study of the treatment after surgery in a patient with a clinical picture compatible with acute appendicitis. The treatment of choice corresponds to appendectomy, which allows avoiding future complications such as appendiceal perforation, neoplasms, among others. Data were obtained from sources such as Pubmed and Scielo, specifically searching for original articles and systematic reviews with the words "apendicular diverticulitis", "diverticulosis" and "diverticular complications". The criteria used were articles mainly under 5 years of publication in high-impact scientific journals.

The anesthetic-surgical stress response consists of metabolic, neuroendocrine, hemodynamic, immunological, and behavioral adaptations through chemical mediators such as the adrenocorticotropic hormone, growth hormone, antidiuretic hormone, cortisol, aldosterone, angiotensin II, thyroid-stimulating hormone, thyroxine, triiodothyronine, follicle-stimulating hormone, luteinizing hormone, catecholamines, insulin, interleukin (IL)-1, IL-6, tumor necrosis factor-alpha, and prostaglandin E-2. Behavioral changes include adopting the so-called prayer posture, altered facial expressions, hyporexia or anorexia, drowsiness, sleep disorders, restriction of movement, licking or biting the injured area, and vocalizations. Overall, these changes are essential mechanisms to counteract harmful stimuli. However, if uncontrolled surgical stress persists, recovery time may be prolonged, along with increased susceptibility to infections in the post-operative period. This review discusses the neurobiology and most relevant organic responses to pain and anesthetic-surgical stress in dogs and cats. It highlights the role of stress biomarkers and their influence on autonomous and demeanor aspects and emphasizes the importance of understanding and correlating all factors to provide a more accurate assessment of pain and animal welfare in dogs and cats throughout the surgical process.

El mixoma odontogénico es una neoplasia benigna, localmente agresiva, con una baja prevalencia (frecuencia del 2% al 5% de los tumores odontogénicos) en los maxilares. Este tumor a menudo se presenta como una lesión expansiva asintomática sin cambios nerviosos sensoriales. El objetivo del presente trabajo es detallar el caso de un paciente masculino de 42 años quien acude al servicio de Cirugía Bucal y Maxilofacial del Hospital Nacional Hipólito Unanue por presentar una tumoración de crecimiento lento en maxilar superior, que fue aumentando de tamaño con el tiempo y desplazando órganos dentarios, y que se extiende al paladar duro y parte de paladar blando, el diagnóstico anatomopatológico fue de mixoma odontogénico y recibió un tratamiento radical (hemimaxilectomia) por la naturaleza agresiva del tumor. En este trabajo discutimos la presentación del mixoma odontogénico en el maxilar superior, los enfoques de trata- miento; detallando la importancia de plantear alternativas de tratamiento que permitan la reconstrucción inmediata generando la menor morbilidad posible.


Odontogenic myxoma is a locally aggressive benign neoplasm with a low prevalence (frequency 2% to 5% of odontogenic tumors) in the jaws. This tumor often presents as an asymptomatic expansive lesion without sensory nerve changes. The aim of this study is to detail the case of a 42-year-old male patient who attends the Oral and Maxillofacial Surgery service of the Hipólito Unanue National Hospital due to a slow-growing tumor in the upper jaw, which increased in size over time and displacing dental organs, and extending to the hard palate and part of the soft palate, the pathological diagnosis was odontogenic myxoma and received radical treatment (hemimaxilectomy) due to the aggressive nature of the tumor. In this work we discuss the presentation of odontogenic myxoma in the maxilla, treatment approaches; detailing the importance of proposing treatment alternatives that allow immediate reconstruction generating the least possible morbidity.

AIMS: To report toxicity of a hypofractionated scheme of whole-breast (WB) intensity-modulated radiotherapy (IMRT) with simultaneous integrated boost (SIB) to the tumor bed (TB) using Tomotherapy® with Direct modality. METHODS: Patients with early breast cancer, undergoing radiotherapy (RT) in 15 daily fractions to WB (prescription dose 40.05 Gy) and SIB to the TB (48 Gy), between 2013 and 2017, was analyzed. Primary endpoint was acute and intermediate toxicity assessed at the end and within 6 months from RT, according to Radiation Therapy Oncology Group (RTOG) scale. Secondary endpoints included early chronic toxicity at 12-months follow-up, using the Late Effects Normal Tissue Task Subjective, Objective, Management, and Analytic (LENT-SOMA) scale, and cosmesis using Harvard criteria. RESULTS: The study population was of 287 patients. Acute and intermediate toxicity was collected among 183 patients with data available at the end of RT and within 6 months, 85 (46%) experienced G2 toxicity and 84 (46%) G1 toxicity, while 14 (8%) did not report toxicity at any time. A significant reduction of any grade toxicity was observed between the two time points, with the majority of patients reporting no clinically relevant toxicity at 6 months. At univariate analysis, age < 40 years, breast volume > 1000 cm3 and Dmax ≤ 115% of prescription dose were predictive factors of clinically relevant acute toxicity (G ≥ 2) at any time. At multivariable analysis, only age and breast volume were confirmed as predictive factors, with Relative Risks (95% Confidence Intervals): 2.02 (1.13-3.63) and 1.84 (1.26-2.67), respectively. At 12-month follow-up, 113 patients had complete information on any toxicity with 53% of toxicity G < 2, while cosmetic evaluation, available for 102 patients, reported a good-excellent result for 86% of patients. CONCLUSIONS: Hypofractionated WB IMRT with a SIB to the TB, delivered with TomoDirect modality, is safe and well-tolerated. Most patients reported no toxicity after 6 months and good-excellent cosmesis. Predictive factors of clinically relevant toxicity might be considered during treatment planning in order to further reduce side effects.

Mesenchymal stem cells (MSC) are modern tools in regenerative therapies of humans and animals owed to their immunomodulatory properties, which are activated in a pro-inflammatory environment. Different preconditioning strategies had been devised to enhance the immunomodulatory properties of MSC. In this research, we evaluated the immunological attributes of equine adipose MSC (eAMSC) before and after preconditioning in vitro with prostaglandin E2 (PGE2), substance P (SP), their combination and IFNγ. PGE2/SP was the best combination to keep or enhance the mesodermal lineage differentiation of eAMSC. Alongside with this, preconditioning of eMSC with PGE2 and SP did not affect expression of stemness MSC surface phenotype: CD90+, CD44+, MHC class I+, MHC class II- and CD45-, assessed by cytometry. Both naïve and preconditioned eAMSC expressed genes related with immune properties, such as MHC-I, PTGES, IL6, IL1A, TNFα and IL8 assessed by qPCR. Only TNFα was under expressed in treated cells, while the other markers were either overexpressed or not changed. In no cases MHC-II expression was detected. The antiproliferative effect of preconditioned eAMSC exposed to activated peripheral blood mononuclear cells (PBMC) showed that SP treatment significantly inhibited proliferation of LPS stimulated PBMC. When eAMSC were stimulated with Poly I:C, all the treatments significantly inhibited proliferation of stimulated PBMC (p < 0.05). Direct contact (coculture) between the preconditioned eAMSC and PBMC, induced a shift of significantly more (CD4/CD25/FOXP3)+ T-regulatory PBMC than naïve eAMSC. In the experiments of this research, we investigated the secreted proteomic profile of naïve and preconditioned eAMSC, 42 up-regulated and 40 down-regulated proteins were found in the proteomic assay. Our proteomic data revealed profound changes in the secretory pattern of MSC exposed to different treatments, compared to naïve eAMSC as well as among treatments. In overall, compared to naïve cells, the protein profile of preconditioned cells resembled the mesenchymal-epithelial transition (MET). Here we showed that the combined use of PGE2 and SP provoked in overall the highest expression of anti-inflammatory markers as well as lead to an increased acquisition of a T-regulatory phenotype in preconditioned eAMSC without affecting their "stemness".

Resumen Introducción: En Chile, las enfermeras/os tienen respaldo legal para gestionar el cuidado, esto facilita el desarrollo de competencias para ejercer autonomía y liderazgo. Actualmente persisten barreras para transitar hacia un reconocimiento real de la independencia profesional. Objetivo: Develar la experiencia vivida de enfermeras con cargos en niveles de jefatura respecto al propio desarrollo para lograr el liderazgo y autonomía profesional. Metodología: Estudio cualitativo, descriptivo-interpretativo, desde la trayectoria fenomenológica de Edmund Husserl. Muestreo selectivo de caso homogéneo de tres enfermeras que desempeñaban rol de jefatura en instituciones de salud, región Valparaíso, Chile. Participación consentida. Entrevista no estructurada. Análisis del discurso utilizando el modelo de liderazgo integral de Ken Wilber. Resultados: Categorías: Conciencia del líder «ser inquieta...intelectualmente¼. Construyendo competencias «el liderazgo se aprende¼. Transitando hacia el ejercicio de la autonomía y el liderazgo, "para el posicionamiento hay que luchar". El peso de las estructuras organizacionales «tu rol es parte de un hospital¼. Interpretación: El ejercicio del liderazgo y la autonomía se fundamenta en competencias actitudinales por sobre las cognitivas y técnicas. Existe mayor apoyo y reconocimiento a nivel institucional, que al interior del equipo de salud. Persisten limitaciones para lograr máxima expresión del liderazgo y autonomía, por persistencia de modelos institucionales hegemónicos, centrados en decisiones médicas y desconocimiento del rol integrado. Conclusión: El líder se mueve en un escenario dual, entre un menor reconocimiento del equipo y un creciente empoderamiento del rol autónomo y visibilidad a nivel directivo institucional.


Abstract Introduction: In Chile, nurses have legal support to manage care, a situation which facilitates the development of competencies to perform with autonomy and leadership; however, barriers still persist while moving towards a real acknowledgement of professional independency. Objective: To unveil the lived experience of nurses in executive roles regarding their own development towards leadership and professional autonomy. Methodology: This is a descriptive and interpretative qualitative study framed by the Edmund Husserl phenomenological approach. The sample by homogeneous case included three nurses in executive roles in health institutions in Valparaiso, Chile. The participation was by consent. An unstructured interview was used. For the discourse analysis, the Ken Wilber integral leadership model was followed. Results: Categories: Leadership awareness <being inquiring… intellectually>… Building Competencies <leadership is learned>. Moving towards the practice of autonomy and leadership, "positioning requires striving". Weight of the organizational structures <your role is part of a hospital>. Interpretation: A practice of leadership and autonomy is based on attitude competencies over the cognitive and technical ones. Nevertheless, there still persist limitations to achieve a maximal expression of leadership and autonomy due to hegemonic institutional models which focus on medical decisions and neglect the integrated roles. Conclusion: The leader moves in a dual setting between a lower acknowledgement from the team, and a higher empowerment of the autonomous role and the visibility at the institutional directive level.

Interpretação: O Exercício da liderança e a autonomia fundamenta-se em competências atitudinais, sobre as cognitivas e técnicas. Existe maior apoio e reconhecimento a nível institucional, que no interior da equipe de saúde. Persistem limitações para atingir a máxima expressão de liderança e autonomia, por persistência de modelos institucionais hegemónicos, centrados em decisões médicas e desconhecimento do rolo integrado. Conclusão: O líder movimenta-se em um cenário dual, entre um menor reconhecimento da equipe e um empoderamento crescente do rolo autónomo e visibilidade a nível diretivo institucional.

The aim is to present the technical feasibility and efficacy of multiple re-irradiation (re-EBRT) for local recurrence of prostate cancer (PCa) using retrospective analysis of an updated series of patients who received ablative re-EBRT with stereotactic image-guided technique for isolated local recurrence of PCa. Eight patients received three RT courses (2 re-RTs); of those 2 received 4 RT courses (3 re-RTs). Local relapse in the prostate was assessed by multiparametric magnetic resonance and/ or choline positron emission tomography. Before treatment planning, all patients had been evaluated for late toxicity from previous RT according to RTOG/EORTC. Biochemical control was assessed according to Phoenix definition. Mean age at the third RT course was 68 (standard deviation, SD: 7.2); all patients had a good performance status. At diagnosis, four cases were classified as high risk PCa, three as intermediate and one as low per NCCN 2017. Biochemical progression free interval after first and second RT-course were 74 (IQR: 59.3-133.6) months and 33 (IQR: 20.8-53.1) months, respectively. Biochemical and radiological response was registered in all patients. At present, seven out of eight patients are disease free. Overall toxicity profile was good; no severe acute or late genitourinary or gastrointestinal events were recorded. Multiple RT courses with high precision technology and image guidance can be proposed as a possible salvage therapy for locally recurrent, low-burden PCa recurrence in adequately selected patients. Deeper understanding of radiobiological effects of hypofractionation and larger series of patients are warranted to fully evaluate the applicability of multiple RT courses in the setting of locally recurrent PCa.

Children with ASD often exhibit early difficulties with action imitation, possibly due to low-level sensory or motor impairments. Impaired cortical rhythms have been demonstrated in adults with ASD during motor imitation. While those oscillations reflect an age-dependent process, they have not been fully investigated in youth with ASD. We collected magnetoencephalography data to examine patterns of oscillatory activity in the mu (8-13 Hz) and beta frequency (15-30 Hz) range in 14 adolescents with and 14 adolescents without ASD during a fine motor imitation task. Typically developing adolescents exhibited adult-like patterns of motor signals, e.g., event-related beta and mu desynchronization (ERD) before and during the movement and a postmovement beta rebound (PMBR) after the movement. In contrast, those with ASD exhibited stronger beta and mu-ERD and reduced PMBR. Behavioral performance was similar between groups despite differences in motor cortical oscillations. Finally, we observed age-related increases in PBMR and beta-ERD in the typically developing children, but this correlation was not present in the autism group. These results suggest reduced inhibitory drive in cortical rhythms in youth with autism during intact motor imitation. Furthermore, impairments in motor brain signals in autism may not be due to delayed brain development. In the context of the excitation-inhibition imbalance perspectives of autism, we offer new insights into altered organization of neurophysiological networks.

Nanotechnology involves manipulation of matter measuring 1-100 nm in at least one of its dimensions at the molecular level. Engineering and manipulation of matter at the molecular level has several advantages in the field of medicine (nanomedicine) since most of the biological molecules exist and function at a nanoscale. Though promising, questions still remain on how much of this will ultimately translate into achieving better patient care. Concerns of cost-effectiveness and nanotechnology safety still remain unclear. Orthopedics is an attractive area for the application of nanotechnology since the bone, and its constituents such as hydroxyapatite, Haversian systems, and the collagen fibrils are nanocompounds. The major orthopedic applications of nanotechnology involve around (i) effective drug delivery systems for antibiotics and chemotherapeutic agents, (ii) surface preparation of implants and prosthesis to improve osteointegration and reduce biofilm formation, (iii) controlled drug eluting systems to combat implant-related infections, (iv) tissue engineering for scaffolds preparation to deal with bone and cartilage defects, and (v) diagnostic applications in the field of oncology and musculoskeletal infections.

Adult stromal mesenchymal stem cells (MSCs) have been postulated as responsible for cell renewal in highly and continuously regenerative tissues such as the endometrium. MSCs have been identified in the endometrium of many species including humans, rodents, pets and some farm animals, but not in horses. The objective of this work was to isolate such cells from the endometrium of mares and to compare their main biological attributes with horse adipose-derived MSCs. Here we successfully isolated and characterized endometrial MSCs (eMSCs) from mares. Said cells showed fibroblast-like morphology, grew on plastic, had doubling population times of 46.4 ± 3.38 h, underwent tri-lineage (osteo, chondro and adipogenic) differentiation after appropriate inductions, migrated toward the attraction of fetal calf serum and displayed a pattern of surface markers commonly accepted for horse MSCs. All these are properties of MSCs. Some of these attributes were shared with equine adipose-derived MSCs, but the migration pattern of eMSC at 12 and 24 h after stimulation was reduced in comparison with adipose MSCs. Also, expression of CD44, CD90 and MHCI surface markers were dramatically down-regulated in eMSCs. In conclusion, equine-derived endometrial MSC share biological attributes with adipose MSC of this species, but displayed a different surface marker phenotype and an impaired migration ability. Conceivably, this phenotype is distinctive for MSC of this origin.

Stem cells have been postulated as responsible for cell regeneration in highly and continuously regenerative tissues such as the endometrium. Few studies in cattle have identified and specified the presence of stem cells in the endometrium during the oestrous cycle. The aim of this study was to investigate the presence of mesenchymal stem cells (MSCs) in the bovine endometrium during the follicular phase (FP) of the oestrous cycle. Uterine tissue was collected in the time-frame comprising day 18 of the cycle and ovulation (day 0). We isolated, cultured and expanded four primary cell lines from endometrium and identified byRT-qPCR the expression of OCT4, SOX2 but not NANOG (undifferentiated/embryonic markers), CD44 (MSCs marker) and c-KIT (stem cell marker) genes; and the encoded Oct4, Sox2 and Cd44 proteins by Western blot or immunostaining of paraffin-embedded tissue in endometrium. We demonstrated that cells isolated from bovine endometrium displayed essentially the same gene expression pattern; however, at the protein level, Oct4 and Cd44 were not detected. Besides, they showed typical functional characteristics of MSCs such as fibroblast-like morphology, plastic adherence, high proliferative capacity, clone formation in vitro and the ability to differentiate into chondrogenic, osteogenic and adipogenic lineages. We obtained for the first time an extensive characterization of undifferentiated cells populations contained in the bovine endometrium during the FP of the oestrous cycle.

PURPOSE: Periodic cancer incidence estimates of Spain from all existing population-based cancer registries at any given time are required. The objective of this study was to present the current situation of cancer incidence in Spain. METHODS: The Spanish Network of Cancer Registries (REDECAN) estimated the numbers of new cancer cases occurred in Spain in 2015 by applying the incidence-mortality ratios method. In the calculus, incidence data from population-based cancer registries and mortality data of all Spain were used. RESULTS: In 2015, nearly a quarter of a million new invasive cancer cases were diagnosed in Spain, almost 149,000 in men (60.0%) and 99,000 in women. Globally, the five most common cancers were those of colon-rectum, prostate, lung, breast and urinary bladder. By gender, the four most common cancers in men were those of prostate (22.4%), colon-rectum (16.6%), lung (15.1%) and urinary bladder (11.7%). In women, the most common ones were those of breast (28.0%), colon-rectum (16.9%), corpus uteri (6.2%) and lung (6.0%). In recent years, cancer incidence in men seems to have stabilized due to the fact that the decrease in tobacco-related cancers compensates for the increase in other types of cancer like those of colon and prostate. In women, despite the stabilization of breast cancer incidence, increased incidence is due, above all, to the rise of colorectal and tobacco-related cancers. CONCLUSION: To reduce these incident cancer cases, improvement of smoking control policies and extension of colorectal cancer screening should be the two priorities in cancer prevention for the next years.

Introducción: La EPOC se caracteriza principalmente por una limitación crónica del flujo aéreo que altera la calidad de vida de los sujetos que la padecen. El objetivo de este estudio es evidenciar los efectos que tiene un programa de rehabilitación pulmonar (RP) diseñado en atención primaria y con escasos recursos en un grupo de pacientes EPOC. Pacientes y Método: Estudio de cohorte prospectivo de 13 pacientes (5 hombres, 8 mujeres) de 74,38 ± 10,12 ( X ¯ ± DS) años de edad, con diagnóstico de EPOC (Estadio GOLD I, II y III) que fueron sometidos durante 3 meses a un programa de RP con énfasis en el entrenamiento de la musculatura respiratoria y periférica, kinesiterapia respiratoria y actividades recreativas como inflar globos. Las variables medidas pre y post entrenamiento fueron: función pulmonar, test de marcha de 6 min (TM6), disnea a través de la escala modificada del Medical Research Council (mMRC), presión inspiratoria máxima (PIM), a volumen residual (VR) y a capacidad residual funcional (CRF), tiempo de resistencia a la fatiga de la musculatura inspiratoria (TRFMI), tiempo de resistencia a la fatiga de la musculatura espiratoria (TRFME), calidad de vida a través del COPD Assessment Test (CAT) y la circunferencia de los globos inflados (CGI) por cada paciente. El análisis estadístico se realizó con el programa SPSS versión 22, utilizando la prueba t de student para muestras relacionadas. Resultados: Hubo cambios significativos (p < 0,05) en: PIM CRF, PIM VR, TRFMI, TRFME, TM6, Disnea, CAT y CGI. Conclusiones: Nuestro programa de RP mejoró ¡a disnea, capacidad de ejercicio, calidad de vida y la fuerza y resistencia de la musculatura respiratoria lo que se reflejó en una mayor CGI por cada paciente post entrenamiento. Por lo tanto, afirmamos que sí es posible diseñar un programa de RP en atención primaria con pocos recursos y obtener excelentes resultados.


Introduction: COPD is characterized by a chronic limitation to airflow that alters the quality of life of the people suffering this illness. The objective of this study is to demonstrate the effects that has a pulmonary rehabilitation program (PRP) designed in primary care using limited resources in a group of COPD patients. Method: Prospective study of 13 patients (5 men, 8 women), age: 74.38 ± 10.12 ( X ¯ ± SD) years-old with COPD, GOLD stage I, II and III, which were submitted for 3 months to a pulmonary rehabilitation program with emphasis on the training of the respiratory muscles and recreational activities like inflating balloons. The variables measured pre and post training were: lung function, six-minute walk test (6MWT), dyspnea using the modified scale from Medical Research Council (mMRC), maximal inspiratory pressure (PIMax) at functional residual capacity (FRC), PIMax at residual volume (RV), time of fatigue resistance of the inspiratory and expiratory muscles (TFRIM and TFREM respectively), quality of life through the COPD assessment Test (CAT) and the circumference of the inflated balloons (CIB) for each patient. Results: Significant changes (p < 0.05) were found in: dyspnea, distance of 6MWT, PIMax at FRC, PIMax at RV, CAT, TFRIM, TFREM and CIB. Conclusions: Our pulmonary rehabilitation program improved the dyspnea, exercise capacity, quality of life and the strength and endurance of the respiratory muscles which was reflected by a greater circumference of the inflated balloons by each patient post-training. Therefore we affirm that it is possible to design a pulmonary rehabilitation program in primary care using limited resources and getting excellent results.

RESUMO

La lesión de Dieulafoy es una causa poco frecuente de sangrado gastrointestinal alto, pero es una de las causas más frecuentes relacionadas con sangrado oculto y recurrente. La ubicación extragástrica de la lesión de Dieulafoy es rara. Por su localización la lesión de Dieulafoy duodenal es de difícil diagnóstico y manejo. La terapia endoscópica, combinada con inyección de adrenalina más terapia mecánica, reduce el riesgo de resangrado. En este artículo se presenta el caso de un paciente tratado en la Clínica Universitaria Colombia, así como la revisión del tema


Dieulafoy’s lesions do not usually cause upper gastrointestinal bleeding, but they are one of the most common causes of hidden and recurrent bleeding. An extra-gastric Dieulafoy lesion is rare, and, because of their location, Dieulafoy’s lesions in the duodenum are difficult to diagnosis and treat. Endoscopic injection therapy combined with adrenaline injections and mechanical therapy reduce the risk of rebleeding. This article describes the case of a patient treated at the Clínica Universitaria Colombia and reviews the topic of Dieulafoy’s lesions

BACKGROUND: Pet is a toxin from the family of Serine Protease Autotransporters of Enterobacteriaceae which was initially identified in Enteroaggregative Escherichia coli strains. This protease exhibits enterotoxin properties, damages the cell cytoskeleton and induces intestinal epithelium alterations, which are associated with a severe inflammatory process. An in-vitro study was conducted to evaluate the effect of Pet on the migration of human peripheral blood monocytes-derived macrophages and its participation in the activation of the early inflammatory response and cytokine expression. RESULTS: In the macrophage migration activation assay, Pet produced a similar effect to that induced by opsonized zymosan (ZAS). Regarding the cytokine expression, an increase of IL-8, TNF-α (pro-inflammatory) and IL-10 (anti-inflammatory) was identified. In addition to the above results, the nuclear translocation of NF-kB pp65 was also identified. These events are probably related to the inflammatory response identified in the histological examination of intestine rat samples inoculated with Pet during a ligated loop assay. CONCLUSION: The results showed that Pet participates as an immunostimulant molecule for macrophages, which activates both their mobility and cytokine expression. These observations suggest that the toxin participates in the inflammatory process that is observed during the host infection by EAEC Pet producing.

Introducción: la cirrosis es la condición médica final de diversas enfermedades hepáticas de carácter progresivo, con una prevalencia variable de un país a otro; en la actualidad, el abuso de alcohol, el hígado graso no alcohólico y las hepatitis crónicas virales son mencionados como sus principales causas. En nuestros centros no se dispone de estudios clínicos en relación con esta enfermedad. Materiales y métodos: estudio retrospectivo y descriptivo de pacientes con diagnóstico de cirrosis hepática entre el 1 de enero del 2010 al 31 de marzo del 2014. Se realizó revisión de historias clínicas. Resultados: la serie incluyó 419 pacientes, 50,1% mujeres y 49,9% hombres, con una edad promedio de diagnóstico de cirrosis de 63 años; el 73% de los pacientes presentaban hallazgos de hepatopatía crónica en el examen físico y el 27% tenía un examen físico normal. Las principales etiologías en esta serie fueron: esteatohepatitis no alcohólica (25,5%), alcohólica (14,8%), infección por virus C (14,6%), autoinmunidad (10%), esteatohepatitis no alcohólica más alcohol (6,7%), y otras (14,6%). La clasificación de Child-Pugh se pudo calcular en 394 pacientes y de estos, el 59,1%, 32,4% y 8,3% eran A, B y C, respectivamente. En relación con las descompensaciones, las principales fueron ascitis (36,1%), sangrado variceal (28,4%) y hepatocarcinoma (15,3%). Conclusiones: el comportamiento epidemiológico local no difiere en relación con el mundial. Llama la atención su detección en fases tempranas y la NASH como factor etiológico principal.


Introduction: Cirrhosis is the final medical condition of various progressive liver disease. Its prevalence varies from one country to another. Currently alcohol abuse, non-alcoholic fatty liver disease and chronic viral hepatitis are mentioned as the main causes. In our centers we have no clinical studies regarding this disease. Materials and Methods: This is a retrospective, descriptive study of the clinical histories of patients who were diagnosed with liver cirrhosis between January 1, 2010 to March 31, 2014. Results: The study included 419 patients, 50.1% of whom were women and 49.9% of whom were men. The average age at diagnosis of cirrhosis 63 years. 73% of patients had physical findings of chronic liver disease and 27% had normal physical examinations. The main etiologies in this series were nonalcoholic steatohepatitis (25.5%), alcoholic cirrhosis (14.8%), hepatitis C infection (14.6%), autoimmune cirrhosis (10%), nonalcoholic steatohepatitis plus alcohol (6.7%), and others (14.6%). The Child-Pugh classification could be calculated in 394 patients. Of these 59.1% were classified A, 32.4% were classified B, and 8.3% were classified C. The primary reasons for decompensation ascites (36.1%), bleeding varices (28.4%) and hepatocellular carcinoma (15.3%). Conclusions: Local epidemiological behavior does not differ from those found elsewhere in the world. Attention needs to be paid to detection in early stages and NASH as the main etiological factor.

El cáncer constituye la segunda causa de muerte en Chile y las infecciones constituyen uno de los principales gatillantes del deceso. El objetivo principal de esta investigación es describir las características clínicas y demográficas de los pacientes oncológicos del Hospital Base Valdivia (HBV) que presentaron complicaciones infecciosas entre el periodo enero-julio de 2013. Materiales y métodos: Se realizó un estudio de serie de casos que incluyó a todos los pacientes oncológicos hospitalizados que presentaron complicaciones infecciosas entre enero-julio de 2013, excluyendo a quienes rechazaran el consentimiento informado o tuvieran fichas clínicas incompletas. Se aplicaron instrumentos para determinar estado nutricional y nivel socioeconómico. Análisis computacional con estadística descriptiva. Resultados: Se pesquisaron 38 pacientes con un total de 52 infecciones. Dieciséis eran hombres y 22 mujeres, edad promedio 59 años. La mayoría pertenecía a estratos socioeconómicos bajos y poseía estudios básicos. Las neoplasias subyacentes más frecuentes fueron el cáncer de mama y cervicouterino. Dieciséis pacientes se encontraban en etapa IV y cerca de 2/3 estaban desnutridos. De las 52 infecciones 31 fueron localizadas, mientras 21 tuvieron respuesta sistémica, 2 pacientes fallecieron debido a la infección. Los sitios más frecuentes de afección fueron: árbol bronquial, tracto genitourinario y cavidad oral. Clínicamente se determinó que 44 de los cuadros fueron bacterianos, 3 fúngicos, 3 virales y 2 combinados. Discusión: Llama la atención el predominio de pacientes provenientes de niveles socioeconómicos y educacionales bajos. Si bien las infecciones pueden corresponder a cuadros potencialmente fatales en pacientes oncológicos, la mayoría de nuestros casos correspondieron a cuadros leves.


Cancer is the second leading cause of death in Chile and infections are one of the main-triggering of death. The main objective of this research is to describe clinical and demographic characteristics of oncological patients of Hospital Base Valdivia with ongoing infectious complications between January-July 2013. Material and methods: series of cases, we reviewed the medical records of all oncological patients who had infectious complications between January-July 2013, excluding those who rejected the invitation or had incomplete medical records. Also, tools for determining nutritional status and socioeconomic status were applied. Data went through computational analysis with determination of descriptive statistics. Result: 38 patients were included with total of 52 infections. Sixteen were men and 22 women, average age was 59 years. Most belonged to lower socioeconomic strata and only had basic education. The most common underlying neoplasms were breast and cervical cancer. Sixteen patients were in stage IV and about two-thirds were malnourished. 31 of the 52 infections were localized, while in 21 there was systemic response, 5 progressed to sepsis and 2 to septic shock, 2 patients died because of their infections. The most common sites of infection were: bronchia, genitourinary tract and oral cavity. Clinically it was determined that 44 were bacterial infections, 3 fungal, 2 viral and 2 combined. Discussion: it draws our attention the predominance of patients from low socioeconomic and educational levels. While infections may correspond to potentially fatal complications in cancer patients, the majority of our cases were mild.

La Enfermedad de Hirschsprung (EH) es una enfermedad poco frecuente que engloba un sinnúmero de problemáticas tanto para el cirujano infantil como para el paciente y sus familiares. Hay numerosas técnicas quirúrgicas descritas para su resolución y con distintos resultados. Objetivo Analizar el resultado postquirúrgico de nuestra casuística evaluando funcionalidad. Metodología Se realiza estudio retrospectivo de pacientes operados por EH entre el 2000 y 2013 en nuestro centro. Se analizan datos según las distintas técnicas de descenso. Se excluyen, para la evaluación funcional 3 pacientes sin resolución definitiva(pendiente reconstitución y/o descenso) y 3 pacientes sin registro clínico postoperatorio. Resultados y Discusión: se analizan datos de 42 pacientes con biopsias compatibles con EH. La edad al momento del diagnóstico corresponde a una media de 30.5 d y 255 d al momento del descenso. El tipo de descenso utilizado principalmente fue el de Georgeson en 17 pacientes (40.4 por ciento), De La Torre en 9 (21.4 por ciento) y Rehbein en 7 (16.6 por ciento). El 45 por ciento debutaron complicados de los cuales el 63.1 por ciento fue ostomizado previo al descenso. El 35.7 por ciento presenta complicaciones precoces. Los resultados funcionales se evalúan al control con cirujano y por encuesta al cuidador aplicando score PICSS. Los pacientes operados con la técnica de Georgeson según nuestra revisión presentaron menos complicación asociadas a largo plazo, teniendo mejor funcionalidad en un 75 por ciento posterior al año del descenso. Entre las complicaciones tardías 33.3 por ciento presento enterocolitis, 14.3 por ciento soiling y un 28.5 por ciento constipación. 57.1 por ciento de nuestra serie presenta funcionalidad adecuada al último control. Se comparan resultados con encuesta telefónica realizada. Conclusiones El poder aplicar protocolos de evaluación funcional en pacientes operados de EH nos permite objetivar mejor los resultados de nuestras cirugías.

Endometrial stem cells have been identified in humans, mice and pigs. This study was designed to determine whether the uterine endometrium of cycling cows contains such cells, to identify markers of stemness and ultimately to isolate putative stem/progenitor cell and evaluate their capability to differentiate into mesodermal derivatives. Uteri from healthy cows in the early (days 1-5) and late luteal phases (days 13-18) of the oestrous cycle were collected. Total RNA and proteins were isolated and searched for gene markers of embryonic (OCT4, NANOG, SOX2) and mesenchymal (CD44, STAT3, CD-117) stem cells and for protein markers (Oct4, Sox2, Cd44) in Western blots or immunostaining of paraffin-embedded tissue. Primary cell cultures were isolated; characterized in terms of morphology, colony formation and gene/protein expression; and induced osteogenic and chondrogenic differentiation. We identified expression of embryonic (OCT4 and SOX2, but not NANOG) and mesenchymal (STAT3, CD44 and c-KIT) gene markers in the endometrium of cycling cows and the encoded proteins (Oct4, Sox2 and Cd44) in both stages of the oestrous cycle. Derived cell lines displayed essentially the same gene expression pattern; however, at the protein level, Oct4 was not detected. No clear influence of the stage of the oestrous cycle was found. Cell lines from late luteal phase displayed osteogenic and chondrogenic differentiation potential upon chemical stimulation. In this research, we demonstrated the presence of mesenchymal progenitor cell populations of apparently mesenchymal origin in the endometrium of cycling cows, in both the early and late phases of the oestrous cycle. The cells isolated from the late luteal phase were more acquiescent to differentiate into mesodermal derivatives than cells in the early luteal phase. Our findings might have implications for the understanding of uterine stem cell biology in cows and other farm animal species.
