BACKGROUND: The efficacy of acupuncture at Zusanli (ST36) in alleviating lower-limb pain is widely acknowledged in clinical practice, while its underlying mechanism remains incompletely elucidated. Our previous research had revealed that the prompt analgesia induced by needling-ST36 was accompanied by expression alterations in certain exco-nucleotidases within the sciatic nerve. Building upon this finding, the current work focused on NTPDase1, the primary ecto-nucleotidase in the human body, which converts ATP into AMP. METHODS: A 20-min acupuncture was administered unilaterally at the ST36 on rats with acute ankle arthritis. The pain thresholds of the injured hind paws were determined. Pharmacological interference was carried out by introducing the corresponding reagents to the sciatic nerve. ATP levels around the excised nerve were measured using a luciferase-luciferin assay. Live calcium imaging, utilizing the Fura 2-related-F340/F380 ratio, was conducted on Schwann cells in excised nerves and cultured rat SCs line, RSC96 cells. RESULTS: The analgesic effect induced by needling-ST36 was impaired when preventing ATP degradation via inhibiting NTPDase1 activities with ARL67156 or Ticlopidine. Conversely, increasing NTPDase1 activities with Apyrase duplicated the acupuncture effect. Similarly, preventing the conversion of AMP to adenosine via suppression of NT5E with AMP-CP hindered the acupuncture effect. Unexpectedly, impeded ATP hydrolysis ability and diminished NTPDase1 expression were observed in the treated group. Agonism at P2Y2Rs with ATP, UTP, or INS365 resulted in anti-nociception. Contrarily, antagonism at P2Y2Rs with Suramin or AR-C 118925xx prevented acupuncture analgesia. Immunofluorescent labeling demonstrated that the treated rats expressed more P2Y2Rs that were predominant in Schwann cells. Suppression of Schwann cells by inhibiting ErbB receptors also prevented acupuncture analgesia. Finally, living imaging on the excised nerves or RSC96 cells showed that agonism at P2Y2Rs indeed led to [Ca2+]i rise. CONCLUSION: These findings strongly suggest that the analgesic mechanism of needling-ST36 on the hypersensation in the lower limb partially relies on NTPDase1 activities in the sciatic nerve. In addition to facilitating adenosine signaling in conjunction with NT5E, most importantly, NTPDase1 may provide an appropriate low-level ATP milieu for the activation of P2Y2R in the sciatic nerve, particularly in Schwann cells.

BACKGROUND: The activation of subcutaneous mast cells (MCs) helps to trigger the analgesic effect induced by acupuncture (AP), a traditional oriental therapy, that has been gradually accepted worldwide. This work aimed to reveal whether the serotonin (5-hydroxytryptamine, 5-HT) released from MCs plays an important role in this process, which has a controversial effect in the mechanism of pain. METHODS: In vivo tests, a 20-min session of AP was applied at Zusanli acupuncture point (acupoint) of acute ankle arthritis rats. Pain thresholds of the injured hindpaw were assessed to reflect the pain state, and the targeting substances in the interstitial space of the treated acupoint were sampled by microdialysis. In vitro experiments, exogenous 5-HT (exo-5-HT) was introduced to mediate adenosine triphosphate (ATP) release from cultured MCs. RESULTS: Needling promoted 5-HT accumulation at the Zusanli acupoint, which was prevented by sodium cromolyn. AP's analgesic effect was suppressed by the inhibition of 5-HT receptors at the acupoint, especially 5-HT1A subtype. In vitro tests, mechanical perturbation mimicking needling stimulation induced MCs to release 5-HT. 1 µM and 10 µM of exo-5-HT facilitated ATP release, which was restrained by blocking of 5-HT1 receptors rather than 5-HT3 receptors. As 5-HT, ATP and adenosine were also transiently accumulated in the treated acupoint during needling. Promoting ATP hydrolysis or activation adenosine A1 receptors duplicated AP analgesic effect. Finally, the inhibition of ATP receptors by suramin or pyridoxal phosphate-6-azo tetrasodium salt hydrate (PPADS) prevented AP analgesic effect. CONCLUSIONS: Our results suggest that MC-associated 5-HT release at acupoints contributes to AP analgesia, and the mediation of ATP secretion through 5-HT1A receptors might be the underlying mechanism at play. ATP could facilitate adenosine production or the propagation of needling signals.

Our and in vitro studies had confirmed that mechanosensitive ATP release and accumulation in acupoints was elicited by acupuncture (AP), which might be a pivotal step for triggering AP analgesia. But to date, the dynamics of extracellular ATP (eATP) in the interstitial space during AP process was poorly known, mainly due to the low temporal resolution of the current detection approach. This study attempted to capture rapid eATP signals in vivo in the process of needling, and further explored the role of this eATP mobilization in initiating AP analgesic effect. Ipsilateral 20-min needling was applied on Zusanli acupoint (ST36) of complete Freund's adjuvant (CFA)-induced ankle arthritis rats. Pain thresholds were assessed in injured-side hindpaws. eATP in the interstitial space was microdialyzed and real-time quantified by luciferin-luciferase assay at 1-min interval with the aid of the microfluid chip. We revealed in behavioral tests that modulation of eATP levels in ST36 influenced AP analgesic effect on ankle arthritis. A transient eATP accumulation was induced by needling that started to mobilize at 4 min, climbed to the peak of 11.21 nM within 3.25 min and gradually recovered. Such AP-induced eATP mobilization was significantly impacted by ankle inflammation, needling depth, needle manipulation, and the presence of local ecto-nucleotidases. This work reveals that needling elicits a transient eATP mobilization in acupoints, which contributes to initiating AP analgesia. This study will help us better understand the peripheral mechanism of AP analgesia and guide clinicians to optimize the needle manipulations to improve AP efficacy.

INTRODUCTION: Diabetic peripheral neuropathy (DPN) is one of the most common complications of diabetes that strongly impact the patients' quality of life and working ability. Evidence indicated that low level light therapy (LLLT)/photobiomodulation might be effective for neuropathy. However, the effect of LLLT for DPN is not clear. The objective of this systematic review and meta-analysis is to determine the effects and safety of LLLT/photobiomodulation for DPN, in comparison with other methods such as sham light, no treatment, other active treatment and LLLT as an additional treatment compared with another treatment alone. METHODS AND ANALYSIS: We will search eight databases from their inception to the date before the review submission. Randomised controlled trials (RCTs) will be included. Two reviewers will independently extract data using a structured data extraction method and assess the risk of bias in the included studies. Data will be synthesised using standardised mean difference or risk ratio with 95% CIs for continuous and dichotomous data, respectively. The primary outcome will be change in pain and secondary outcomes will include global symptom improvement, functional impairment and disability, impairment of sensation, quality of life, nerve conduction, and adverse events. Sensitivity and subgroup analysis will be employed to explore the influence of possible clinical and methodological characteristics. Publication bias will be assessed using funnel plot. We will conduct meta-analysis with RevMan V.5.4 and evaluate quality of the evidence using GRADE approach. ETHICS AND DISSEMINATION: This study does not require ethics approval. Our findings will be disseminated in the peer-reviewed publications. PROSPERO REGISTRATION NUMBER: CRD42021276056.

As an ancient analgesia therapy, acupuncture has been practiced worldwide nowadays. A good understanding of its mechanisms will offer a promise for its rational and wider application. As the first station of pain sensation, peripheral sensory ganglia express pain-related P2X receptors that are involved in the acupuncture analgesia mechanisms transduction pathway. While the role of their endogenous ligand, extracellular ATP (eATP), remains less studied. This work attempted to clarify whether acupuncture modulated eATP levels in the peripheral sensory nerve system during its analgesia process. Male Sprague-Dawley rats underwent acute inflammatory pain by injecting Complete Freund's Adjuvant in the unilateral ankle joint for 2 days. A twenty-minute acupuncture was applied to ipsilateral Zusanli acupoint. Thermal hyperalgesia and tactile allodynia were assessed on bilateral hind paws to evaluate the analgesic effect. eATP of bilateral isolated lumbar 4-5 dorsal root ganglia (DRGs) and sciatic nerves were determined by luminescence assay. Nucleotidases NTPDase-2 and -3 in bilateral ganglia and sciatic nerves were measured by real-time PCR to explore eATP hydrolysis process. Our results revealed that acute inflammation induced bilateral thermal hyperalgesia and ipsilateral tactile allodynia, which were accompanied by increased eATP levels and higher mechano-sensitivity of bilateral DRGs and decreased eATP levels of bilateral sciatic nerves. Acupuncture exerted anti-nociception on bilateral hind paws, reversed the increased eATP and mechanosensitivity of bilateral DRGs, and restored the decreased eATP of bilateral sciatic nerves. NTPDase-2 and -3 in bilateral ganglia and sciatic nerves were inconsistently modulated during this period. These observations indicate that eATP metabolism of peripheral sensory nerve system was simultaneously regulated during acupuncture analgesia, which might open a new frontier for acupuncture research.

The analgesic role of the adenosine A1 receptor is thought to involve the modulation of the spinal N-methyl D-aspartate receptor-mediated nociceptive pathway, which is suggested to be an underlying mechanism in chronic pain. Knee osteoarthritis is a degenerative condition accompanied by chronic pain. We have demonstrated that 10.6-µm laser irradiation has an antinociceptive effect in the monosodium iodoacetate -induced knee osteoarthritis in rats. However, its mechanism of action has yet to be explored. In the present work, we investigate the mechanism of 10.6-µm laser irradiation mediated antinociception in the monosodium iodoacetate -induced knee osteoarthritis. Results showed that the 10.6-µm laser significantly reversed the monosodium iodoacetate -induced nociceptive behaviors for up to 28 days. Moreover, the up-regulation of the A1 receptor and the down-regulated phosphorylation of the N-methyl D-aspartate receptor 1 subunit of the N-methyl D-aspartate receptor were observed in the spinal cord dorsal horn in the monosodium iodoacetate injected rats treated by laser irradiation. Intrathecal injection of 8-cyclopentyl-1,3-dipropylxanthine markedly reversed the effects of laser irradiation, as evidenced both by behavioral pain tests and by levels of spinal phosphorylation of N-methyl D-aspartate receptor 1. These results suggest that the spinal A1 receptor contributes to the antinociceptive effects of 10.6-µm laser, at least in part by inhibiting phosphorylation of N-methyl D-aspartate receptor 1 in the monosodium iodoacetate -induced knee osteoarthritis pain.

BACKGROUND: Knee osteoarthritis (OA) is a major cause of disability among the older adults. Few treatments are safe and effective. Moxibustion is commonly used in treating knee OA in Chinese medicine (CM). CO2 Laser moxibustion device is a substitute for traditional moxibustion, which mimics the effects of traditional moxibustion. More data are needed to support its application in knee OA. OBJECTIVE: ObjectiveThe trial aims to assess the effect and safety of CO2 laser moxibustion in patients with knee osteoarthritis compared with a sham control. METHODS: This is a protocol for a multicenter, randomized, double-blind, placebo-controlled trial. A total of 392 participants were recruited and assigned to the CO2 laser moxibustion group and sham laser moxibustion group with a 1:1 ratio at 6 outpatient clinics in Shanghai, China. Participants in both groups received treatment at the affected knee(s) at the acupuncture point Dubi (ST 35) and an Ashi point. There were 3 sessions per week for 4 weeks, and an additional 20-week follow-up. Primary outcomes were changes in the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain scores at week 4. Secondary outcomes were WOMAC function score, stiffness score and overall score, VAS pain, Short-Form heath survey (SF-36), and patients' global assessment. The serum levels of cytokines involved in progress of knee OA were explored. Safety was assessed during the whole trial. DISCUSSION: CO2 laser moxibustion device, designed as a substitute for CM moxibustion, is easy to use and control with no choking smoke and smell, and is a plausible method for double-blind research. This study would provide rigorous evidence for the effect and safety of CO2 laser moxibustion in treating knee OA (Trial registration No.: ISRCTN15030019).

The purpose of this study was to observe the characteristics of infrared radiation spectrum of Zigong acupoint (EX-CA1) within the menstrual cycle of healthy female. We used highly sensitive infrared radiation spectrum detection system and phase-locked amplification technology to detect and analyse the infrared radiation spectrum from 1.5µm to 18µm of 32 healthy female before, during, and after menstruation at EX-CA1 and control points. The results showed that the total radiation intensity of left EX-CA1 was significantly higher than that of left control point (P <0.05) at the whole menstrual cycle, and the difference between right EX-CA1 and right control points was statistically significant before and after menstruation (P <0.05), no statistical difference during menstruation. Previous studies found that the radiation near 15µm was correlated with glucose metabolism. The results of this study showed that there were statistical differences in 10 wavelengths between left EX-CA1 and left control point from 14µm to 18µm, and there were statistical differences in 3 wavelengths on the right side (P <0.05). The left side is more prominent than the right side. The infrared radiation intensity of EX-CA1 decreased gradually with the change of cycle rhythm within menstrual cycle, but there was no statistical difference in this trend. There was no statistical difference in total radiation intensity between the right and left side of EX-CA1. Compared with the control points, the number of different wavelengths between left EX-CA1 and left control point during menstruation was significantly larger than that between right EX-CA1 and right control point (P <0.001). The results indicated that the energy of EX-CA1 was higher than control points. There was no difference in the radiation intensity between the right and left sides of EX-CA1 but there was acupoint laterality compared with nonacupoints. There was no significant rhythmic change in infrared radiation intensity of EX-CA1 during the menstrual cycle.

Although acupuncture, especially electroacupuncture (EA) is widely used to treat pain, its mechanisms have not been completely understood. In the present paper, we review the development of researches on the underlying mechanisms of EA in relieving inflammatory pain in recent years from a)peripheral inflammation-mediated immune response and neuro-immune interaction of receptors on primary nociceptors, b) crosstalk of neurotransmitters or neuromodulators, cellular signaling pathways, other related bioactive molecules, as well as glial activation in the dorsal horns of spinal cord, and c) supraspinal modulation of both sensory and affective components of pain. Inflammatory pain involves complex neuro-immune networks of neurons and non-neurons, various inflammatory mediators, neurotransmitters, neuromodulators and cellular signaling molecules in the peripheral and central nervous systems. Therefore, the analgesic mechanism of acupuncture still needs to be studied in depth at multi-levels and multi-targets, for instance, the reciprocal actions of peripheral opioid peptide, adenosine and TRPV 1 (which have been demonstrated to be involved in EA analgesia individually), the roles of spinal cord adenosine A 1 receptor (A 1 R) and A 2 R (for which fewer studies have been conducted), the interactions of classical neurotransmitters/neuromodulators/neuropeptides and their receptors, changes of intracellular molecules at transcriptional and translation levels, etc. during acupuncture analgesia. Furthermore, the mechanisms underlying reciprocal actions of neurotransmitters, neuromodulators and their receptors complicated in acupuncture-induced relief of pain affection in the higher brain regions also need to be explored further.

OBJECTIVE: To analyze the distinctive pathological characteristics in the spectrums of spontaneous infrared radiation at the Taiyuan (LU 9) acupoint in patients with asthma. METHODS: A highly sensitive infrared spectrum detecting device was used to detect the spectrums of spontaneous infrared radiation at Taiyuan (LU 9) in 37 asthma patients and 34 healthy volunteers. RESULTS: Asthma patients had significantly lower infrared intensity than that of the healthy volunteers (P>0.01). Asthma patients had significantly lower overall infrared radiation intensity at the left Taiyuan (LU 9) than that of healthy volunteers (P > 0.05), but there was no significant difference between healthy volunteers and asthma patients at the right Taiyuan (LU 9) (P > 0.05). The infrared radiation intensity of 17 wavelength spots at the left Taiyuan (LU 9) and 4 wavelength spots at the right Taiyuan (LU 9) in asthma patients were significantly lower than those of healthy volunteers (P > 0.05). At 2 microm, the infrared radiation intensity of asthma patients was significantly stronger than that of healthy volunteers (P > 0.05). At 19 wavelength spots in the healthy volunteers and at 4 wave-length spots in the asthma patients, the left Taiyuan (LU 9) showed a significantly stronger intensity than that of the right Taiyuan (LU 9) (P > 0.05S). By Pearson's chi2 test, healthy volunteers had more wavelength spots that were significantly different between the left and right Taiyuan (LU 9) than the asthma patients (P > 0.01). CONCLUSION: Changes in the infrared spectrum at the Taiyuan (LU 9) acupoint in asthma patients may reflect distinct pathological changes. Certain acupuncture points may be related to specific organs.

OBJECTIVE: To study the effects of three different laser treatments (650 nm alone, 10.6 µm alone and combined laser of 650 nm and 10.6 µm) on experimental osteoarthritis of the knees in C57 black mice. METHODS: Sixty C57 black mice were divided randomly into 6 groups. Ten mice were assigned to a normal control group (no intervention) and the other 5 groups were subjected to a forced running regimen to induce osteoarthritis. One group was set as the model control group. The other 4 groups were given 90 s of a 650 nm laser, 90 s of a 10.6 µm laser, 90 s of a combined laser, or a sham treatment on acupoint Dubi (ST35) of the rear left leg 3 times per week for 4 weeks. The modified Mankin score was used to evaluate the degree of cartilage degradation. Immunohistochemical staining for heat shock protein (HSP) 70 was conducted. RESULTS: Mankin scores of the model control group and the sham control group were significantly higher than that of the normal control group (P<0.01). Mankin score of the combined laser group was significantly lower than that of the model control group (P<0.01). Compared with the normal control group, there was a significant induction of HSP70 in the arthritic chondrocytes of the combined laser group. CONCLUSION: The arthritic cartilage induced in C57 black mice improved significantly after combined laser treatment of 650 nm and 10.6 µm lasers. This effect may be related to the induction of HSP70 in the arthritic chondrocytes. The two different lasers appear to have a synergistic effect.

RESUMO

Infrared radiation temperature and characteristics of infrared spectrum of meridians and acupoints are summarized in this article. The infrared radiant track along the running course of meridians might be a ubiquitous phenomenon of the vital activity. The high temperature bands below the meridian lines at the cuticular layer are often taken as the manifestation of the tract. It could be induced by various stimulations on acupoints with warm-needling as the best causative method. High temperature and low resistance are considered as 2 features of the acupoint zone. The adenosine-triphosphate energy metabolism of the acupoint zone is higher than the non-point zone, which indicates that essential physiological and pathological information are carried by infrared radiation temperature of acupoint and infrared spectrum of acupoint. However, it is realized that study on characteristics of infrared radiation only is far from enough to reveal the essence of meridians and acupoints. And much still remains to be done in strengthening basic studies of characteristics of infrared radiation, structure pattern of meridians and acupoints as well as volt-ampere characteristics.

RESUMO

OBJECTIVE: To explore the effects of combined laser on patients with knee osteoarthritis (OA) of yang deficiency and cold coagulation type. METHODS: Forty-one cases with knee OA of yang deficiency and cold coagulation type were randomly divided into a combined laser group (n = 22) and a red light group (n = 19), with combined laser and red light irradiation on Dubi (ST 35) and Neixiyan (EX-LE 4), respectively. They were treated for 6 weeks. The scores of Western Ontario and McMaster Universities' Osteoarthritis Index (WOMAC VA3. 1) were used for assessment of therapeutic effects. RESULTS: After treatment for 2 weeks and 6 weeks, the WOMAC scores significantly decreased in the two groups as compared with those before treatment (P < 0.05, P < 0.01 and P < 0.001). The mean improvement rate of WOMAC scores in the combined laser group was better than those in the red light group. CONCLUSION: Combined laser irradiation can improve the pain, stiffness and functional limitation of patients with knee OA of yang deficiency and cold coagulation type.

OBJECTIVE: To compare differences of surface infrared radiation spectrums between Danzhong (CV 17) A high-sensitive PHE and non-acupoint control point in the patient of hyperplasia of mammary glands. METHODS: 201 surface infrared spectrograph was used to detect infrared radiation spectrums of Danzhong (CV 17) and non-acupoint control point at 1.5-16.0 microm wave band in the patient of hyperplasia of mammary glands. RESULTS: The shape of the infrared spectrums of Danzhong (CV 17) was similar to that of the non-acupoint control point, but with lower radiation intensity. Of the 59 wavelength spots detected, 13 (6.75-8.25 microm, 9.00 microm, 9.25 microm, 9.75 microm, 13.25-13.75 microm) had significant differences in infrared radiation intensity between Danzhong (CV 17) and non-acupoint control point (P<0.05). CONCLUSION: The intensity of infrared radition of Danzhong (CV 17) is lower and Qimen (LR 14) is higher than that of the non-acupoint control point in the patient with hyperplasia of mammary glands, showing that different channels are at different states of deficiency and excess under pathological condition of hyperplasia of mammary glands.

OBJECTIVE: To probe the diagnostic value of the infrared radiation spectrum of acupoint for ulcerative colitis (UC). METHODS: A high sensitivity PHE 201 infrared spectrum instrument was used to determine the infrared radiation spectrum of Hegu (LI 4) and Shangjuxu (ST 37) in 34 cases of UC. RESULTS: Of 59 waves detected, there were significant differences in infrared radiation intensity of 28 different waves between the healthy people and the patients with UC in right Hegu (LI 4) (P < 0.05 or P < 0.01) and 13 waves in left Hegu (LI 4) (P < 0.05); there were significant differences in 16 different waves in right Shangjuxu (ST 37) (P < 0.05 or P < 0.01) and in 17 waves in left Shangjuxu (ST 37) (P < 0.05 or P < 0.01); there was a significant difference in 18 waves between right and left Hegu (LI 4) of the patients (P < 0.05 or P < 0.01) and 7 waves between right and left Hegu (LI 4) of the healthy people (P < 0.05). There was a significant difference in 4 waves between right and left Shangjuxu (ST 37) of the patients and one wave between right and left Shangjuxu (ST 37) of the healthy people (P < 0.01). CONCLUSION: Both Hegu (LI 4) and Shangjuxu (ST 37) show changes of infrared radiation spectrum when the intestine gets lesion, and Hegu (LI 4) can better show the change.

OBJECTIVE: To observe the effect of infrared laser stimulation on Neiguan (PC 6) for bradycardia of rabbits induced by injection of pituitrin. METHODS: 10.6 microm CO(2) infrared laser, 650 nm semiconductor red laser and the compound laser of their aggregation were sent out by a compound laser sanatory instrument to stimulate Neiguan (PC 6) respectively for the purpose of analyzing the differences of the effects exerted by infrared laser and red laser as well as the changes of the effects after their aggregation. RESULTS: After 30 min and 40 min of the injection of pituitrin, the heart rate showed no significant difference between 10.6 microm CO(2) infrared laser group and the normal control group (P>0.05). However, the heart rate showed significant difference between the 650 nm semiconductor red laser group and the normal control group (P<0.05). After 30 min of the injection of pituitrin, the heart rate of the compound laser group were significantly different to that of the untreated group (P<0.05), and it was not significantly different to that of the normal control group (P>0.05). However, the heart rate of non-acupoint stimulated by the compound laser group showed no significant difference to that of the untreated group (P>0.05), and it was significantly different to that of the normal control group (P<0.05). CONCLUSION: Stimulation on Neiguan with 10.6 microm CO(2) infrared laser can evidently improve the bradycardia induced by injection of pituitrin, while the 650 nm semiconductor red laser has no such function, but they have coordinating effect. The thermal effect produced by CO(2) infrared laser stimulating acupoint is probably beneficial for semiconductor laser penetrating skin as well as the biochemical reactions it carries. The improvement of heart rate produced by the laser stimulation on Neiguan (PC 6) has relative specificity.

The objective of this paper was to observe the effect of low-level combined- or single-laser irradiation on bradycardia produced by pituitrin in rabbits. A combined-laser apparatus was made. A 10.6-microm CO2 laser and a 650-nm semiconductor laser, transmitted by different optical fibers, converged to output and irradiate on the Neiguan (PC6) acupuncture point in rabbits with bradycardia produced by pituitrin. Thirty minutes after the model was set, the heart rates of the combined-laser Neiguan group made quicker recoveries than those of the model control group, the laser-control group, or the single-laser Neiguan group (P<0.05), and the heart rates of the single-CO2-laser Neiguan group were similar to those of the normal group (P>0.05). However, there were significant differences between the 650-nm-laser Neiguan group and the normal control group (P<0.05). The combined-laser irradiation certainly has a curative effect on bradycardia produced by pituitrin. A single CO2 laser could accelerate the recovery from bradycardia, while single 650-nm-laser irradiation on the Neiguan acupoint does not produce such an effect.
