RESUMO

As with Amazonian primates, mixed associations between species in the Atlantic Forest are also influenced by ecological factors. However, Atlantic Forest primates may face additional challenges, such as isolation pressures and fragmentation of forest habitats, which may increase the frequency of these arrangements. The main of this work is to report a sympatry with possible interaction between individuals of two species of primates of the Pitheciidae and Callitrichidae families: Callicebus nigrifrons (Spix 1823) and Callithrix aurita (É. Geoffroy Saint-Hilaire 1812) in an urban park in the south of the state of Minas Gerais. Individuals were observed interacting during foraging and displacement. The association of individuals of the two species can be explained by the low quality of the forest fragment, as it can increases the chances of obtaining food resources and configures a dilution strategy against predator attacks.


Assim como ocorre com os primatas amazônicos, as associações mistas entre espécies na Mata Atlântica também são influenciadas por fatores ecológicos. No entanto, os primatas da Mata Atlântica podem enfrentar desafios adicionais, como pressões de isolamento e fragmentação de habitats florestais, que podem aumentar a frequência desses arranjos. O objetivo deste trabalho é apresentar um relato de simpatia com possível interação entre indivíduos de duas espécies de primatas das famílias Pitheciidae e Callitrichidae: Callicebus nigrifrons (Spix 1823) e Callithrix Resumo aurita (É. Geoffroy Saint-Hilaire 1812) em um parque urbano no sul do estado de Minas Gerais. Foram observados indivíduos interagindo durante o forrageio e deslocamento. A associação de indivíduos das duas espécies pode ser explicada devido à baixa qualidade do fragmento florestal, pois pode aumentar as chances de obter recursos alimentares e configura uma estratégia de diluição de contra-ataques de predadores.

Purpose: This study aimed to evaluate racial disparities in medication use and associated factors among pregnant women receiving prenatal care at Brazilian Unified Health System primary care health units in the northeast region. Patients and Methods: A total of 1058 pregnant women in the NISAMI Cohort were interviewed between June 2012 and February 2014. Medicines used during pregnancy were classified according to the Anatomical Therapeutic Chemical (ATC) classification system and ANVISA pregnancy risk categories. Prevalence ratios (crude and adjusted) and 95% confidence intervals (CIs) were estimated using Poisson regression with robust error variance. All analyses were stratified by race (Asian, black, brown/mixed, Brazilian indigenous, and white). Results: Approximately 84% of the pregnant women used at least one medication, with a lower proportion among white women. The most reported medications were antianemic preparations (71.08%; 95% CI 68.27-73.72%), analgesics (21.74%; 95% CI 19.36-24.32%), and drugs for functional gastrointestinal disorders (18.81%; 95% CI 16.57-21.28%). Approximately 29% of women took potentially risky medications during pregnancy, with a higher prevalence among Asian and white women. Factors associated with medication use during pregnancy include a greater number of prenatal consultations, higher education levels, health problems, and smoking. In addition, maternal age above 25 years, smoking status, and two or more previous pregnancies were associated with potentially risky medication use during pregnancy. Conclusion: A high prevalence of medication use during pregnancy was found; however, this prevalence was lower among white women. Nonetheless, black and brown women used antianemic preparations less frequently. This finding suggests that race is a factor of inequity in prenatal care, demanding public policies to mitigate it.

INTRODUCTION: Despite advancements in the treatment of benign prostatic hyperplasia (BPH), the mechanisms underlying BPH development and progression remain elusive and lacks a one-size-fits-all therapeutic solution. Prostatic inflammation contributes to BPH and lower urinary tract symptoms (LUTS), but the initial trigger remains unknown. Current research suggests dysbiosis of the urinary microbiome as a potential culprit. This systematic review explores the emerging field of the male urinary and prostatic microbiome and its relationship with BPH/LUTS. METHODS: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines were followed. A systematic search in the Pubmed and Scopus databases was performed using specific terms. Inclusion criteria considered male non-neurogenic patients with LUTS due to BPH with analyses of urinary microbiome, concerning evaluation of English-language publications with relevance. RESULTS: Among seven articles involving 542 patients, there was an association between male LUTS/BPH and the urinary microbiome. Findings indicate a correlation between urinary microbiome dysbiosis and LUTS severity, with specific bacterial genera such as Streptococcus and Haemophilus linked to higher International Prostate Symptom Score (IPSS) scores and PSA levels. The fecal microbiome may be associated with LUTS, although contradictory findings are reported. The review also highlights methodological inconsistencies, small sample sizes, few negative controls and a lack of comprehensive clinical data as major limitations. CONCLUSIONS: While there is an undeniable correlation between the microbiome and LUTS/BPH, future research should aim to standardize sampling techniques and expand the score to include functional microbiome characterization, potentially leading to novel, microbiome-targeted therapeutic strategies for BPH.

BACKGROUND: Human T-cell lymphotropic virus type 1 (HTLV-1), also denominated Human T-cell leukemia virus-1, induces immune activation and secretion of proinflammatory cytokines, especially in individuals with HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP). Regulatory T lymphocytes (Tregs) may control of inflammation through the production of regulatory cytokines, including IL10 and TGF-ß. In this study we determined the frequencies of CD4 + and CD8 + Tregs in a HAM/TSP population, compared to asymptomatic carriers and uninfected individuals, as well as investigated the profiles of regulatory and inflammatory cytokines. METHODS: Asymptomatic HTLV-1 carriers and HAM/TSP patients were matched by sex and age. The frequencies of IL10- and/or TGF-ß-producing Tregs were quantified by flow cytometry. Real-time reverse transcription polymerase chain reaction (RT-PCR) was used to quantify HTLV-1 proviral load and the mRNA expression of cytokines and cellular receptors in peripheral blood mononuclear cells. RESULTS: Total frequencies of CD4 + Tregs, as well as the IL10-producing CD4 + and CD8 + Treg subsets, were statistically higher in patients with HAM/TSP compared to asymptomatic HTLV-1-infected individuals. In addition, a positive correlation was found between the frequency of CD4 + IL10 + Tregs and proviral load in the HAM/TSP patients evaluated. A positive correlation was also observed between gene expression of proinflammatory versus regulatory cytokines only in HAM / TSP group. CONCLUSIONS: A higher frequencies of IL10-producing Tregs were identified in patients with HAM/TSP. Imbalanced production of IL10 in relation to TGF-ß may contribute to the increased inflammatory response characteristically seen in HAM/TSP patients.

INTRODUCTION: Lung cancer in never-smoker (LCINS) patients accounts for 20% of lung cancer cases, and its biology remains poorly understood, particularly in genetically admixed populations. We elucidated the molecular profile of driver genes in Brazilian LCINS. METHODS: The mutational and gene fusion status of 119 lung adenocarcinomas from self-reported never-smoker patients, was assessed using targeted sequencing (NGS), nCounter, and immunohistochemistry. A panel of 46 ancestry-informative markers determined patients' genetic ancestry. RESULTS: The most frequently mutated gene was EGFR (49.6%), followed by TP53 (39.5%), ALK (12.6%), ERBB2 (7.6%), KRAS (5.9%), PIK3CA (1.7%), and less than 1% alterations in RET, NTRK1, MET∆ex14, PDGFRA, and BRAF. Except for TP53 and PIK3CA, all other alterations were mutually exclusive. Genetic ancestry analysis revealed a predominance of European (71.1%), and a higher African ancestry was associated with TP53 mutations. CONCLUSION: Brazilian LCINS exhibited a similar molecular profile to other populations, except the increased ALK and TP53 alterations. Importantly, 73% of these patients have actionable alterations that are suitable for targeted treatments.

Alternative splicing is the process of generating different mRNAs from the same primary transcript, which contributes to increase the transcriptome and proteome diversity. Abnormal splicing has been associated with the development of several diseases including cancer. Given that mutations and abnormal levels of the RIPK2 transcript and RIP-2 protein are frequent in tumors, and that RIP-2 modulates immune and inflammatory responses, we investigated alternative splicing events that result in partial deletions of the kinase domain at the N-terminus of RIP-2. We also investigated the structure and expression of the RIPK2 truncated variants and isoforms in different environments. In addition, we searched data throughout Supraprimates evolution that could support the biological importance of RIPK2 alternatively spliced products. We observed that human variants and isoforms were differentially regulated following temperature stress, and that the truncated transcript was more expressed than the long transcript in tumor samples. The inverse was found for the longer protein isoform. The truncated variant was also detected in chimpanzee, gorilla, hare, pika, mouse, rat, and tree shrew. The fact that the same variant has been preserved in mammals with divergence times up to 70 million years raises the hypothesis that it may have a functional significance.

Menopause is characterized by a reduction in sex hormones in women and is associated with metabolic changes, including fatty liver and insulin resistance. Lifestyle changes, including a balanced diet and physical exercise, are necessary to prevent these undesirable changes. Strength training (ST) has been widely used because of the muscle and metabolic benefits it provides. Our study aims to evaluate the effects of ST on hepatic steatosis and insulin resistance in ovariectomized mice fed a high-fat diet (HFD) divided into four groups as follows: simulated sedentary surgery (SHAM-SED), trained simulated surgery (SHAM-EXE), sedentary ovariectomy (OVX-SED), and trained ovariectomy (OVX-EXE). They were fed an HFD for 9 weeks. ST was performed thrice a week. ST efficiently reduced body weight and fat percentage and increased lean mass in OVX mice. Furthermore, ST reduced the accumulation of ectopic hepatic lipids, increased AMPK phosphorylation, and inhibited the de novo lipogenesis pathway. OVX-EXE mice also showed a better glycemic profile, associated with greater insulin sensitivity identified by the euglycemic-hyperinsulinemic clamp, and reduced markers of hepatic oxidative stress compared with sedentary animals. Our data support the idea that ST can be indicated as a non-pharmacological treatment approach to mitigate metabolic changes resulting from menopause.

BACKGROUND RARP is an established procedure in treatment of localized prostate cancer. Hemorrhagic complications in the postoperative period are rare, but sometimes life-threatening. Adequate monitoring and prompt intervention in these unusual scenarios rely on clinical judgement and blood and imaging studies. Prostatic fossa pseudoaneurysm formation after RARP is very rare and its etiology is not well known; it may be related to small vessel trauma. It becomes apparent with the development of hematuria 1-6 weeks after surgery. CASE REPORT A 58-year-old man underwent RARP with extended lymph node dissection for intermediate-risk prostate cancer, with bilateral preservation of neurovascular bundles and puboprostatic ligaments. He was discharged on day 2 without complications. In the following 4 weeks he came to the Emergency Department 3 times with hematuria and acute urinary retention. Four weeks after surgery, a pelvic CT angiogram showed a 20-mm pseudoaneurysm in the prostatic fossa, which was embolized by percutaneous angiography, with resolution of symptoms. He was discharged soon thereafter. CONCLUSIONS This case study describes a patient with prostatic fossa pseudoaneurysm after RARP. It was diagnosed 1 month after surgery and effectively managed by percutaneous embolization. Despite being a very rare condition, it must be kept in mind, especially when postoperative hematuria develops 1-6 weeks after surgery. Use of a management algorithm including serial blood tests, CT angiogram, and percutaneous angiography can lead to early detection and avoid life-threatening hemorrhage and overall postoperative morbidity.

INTRODUCTION: Bladder cancer (BC) is an increasingly frequent malignancy worldwide. Several variant histologies (VH) have been described in BC with a distinct clinical behavior. OBJECTIVES: This study aims to assess the prognostic impact of VH in BC, comparing its outcomes to pure urothelial carcinoma PUC in both non-muscle invasive (NMIBC) and muscle-invasive (MIBC) settings. METHODS: We included patients with primary BC, comparing those with VH with those with PUC, with an age and sex-matched proportion of 1:3, considering stage at diagnosis, recurrence-free, progression-free, and overall survival (OS). A total of 616 patients were included in the study, (460 UC and 151 VH). RESULTS: After first TURBT, MIBC was present in 99 (64.1%) of patients with VH, and 95 (20.6%) with UC (p<0.001). Concerning NMIBC, we observed higher rates of progression to MIBC amid patients with VH (p=0.009). Nodal involvement (p=0.020) and metastatic disease (p<0.001) were significantly higher within the VH group. A higher OS was observed among patients with NMIBC of PUC (p<0.001). There were no statistically significant differences of metastasis-free survival and OS between VH and UC groups within the MIBC setting. CONCLUSION: We confirmed that VH presents a more aggressive clinical course compared to PUC. An earlier radical treatment within the NMIBC setting could increase the oncological outcomes of the VH patients.

BACKGROUNDPhase 1 study of ATRinhibition alone or with radiation therapy (PATRIOT) was a first-in-human phase I study of the oral ATR (ataxia telangiectasia and Rad3-related) inhibitor ceralasertib (AZD6738) in advanced solid tumors.METHODSThe primary objective was safety. Secondary objectives included assessment of antitumor responses and pharmacokinetic (PK) and pharmacodynamic (PD) studies. Sixty-seven patients received 20-240 mg ceralasertib BD continuously or intermittently (14 of a 28-day cycle).RESULTSIntermittent dosing was better tolerated than continuous, which was associated with dose-limiting hematological toxicity. The recommended phase 2 dose of ceralasertib was 160 mg twice daily for 2 weeks in a 4-weekly cycle. Modulation of target and increased DNA damage were identified in tumor and surrogate PD. There were 5 (8%) confirmed partial responses (PRs) (40-240 mg BD), 34 (52%) stable disease (SD), including 1 unconfirmed PR, and 27 (41%) progressive disease. Durable responses were seen in tumors with loss of AT-rich interactive domain-containing protein 1A (ARID1A) and DNA damage-response defects. Treatment-modulated tumor and systemic immune markers and responding tumors were more immune inflamed than nonresponding.CONCLUSIONCeralasertib monotherapy was tolerated at 160 mg BD intermittently and associated with antitumor activity.TRIAL REGISTRATIONClinicaltrials.gov: NCT02223923, EudraCT: 2013-003994-84.FUNDINGCancer Research UK, AstraZeneca, UK Department of Health (National Institute for Health Research), Rosetrees Trust, Experimental Cancer Medicine Centre.

Background: The bioactive peptides derived from snake venoms of the Viperidae family species have been promising as therapeutic candidates for neuroprotection due to their ability to prevent neuronal cell loss, injury, and death. Therefore, this study aimed to evaluate the cytoprotective effects of a synthetic proline-rich oligopeptide 7a (PRO-7a; <EDGPIPP) from Bothrops jararaca snake, on oxidative stress-induced toxicity in neuronal PC12 cells and astrocyte-like C6 cells. Methods: Both cells were pre-treated for four hours with different concentrations of PRO-7a, submitted to H2O2-induced damage for 20 h, and then the oxidative stress markers were analyzed. Also, two independent neuroprotective mechanisms were investigated: a) L-arginine metabolite generation via argininosuccinate synthetase (AsS) activity regulation to produce agmatine or polyamines with neuroprotective properties; b) M1 mAChR receptor subtype activation pathway to reduce oxidative stress and neuron injury. Results: PRO-7a was not cytoprotective in C6 cells, but potentiated the H2O2-induced damage to cell integrity at a concentration lower than 0.38 μM. However, PRO-7a at 1.56 µM, on the other hand, modified H2O2-induced toxicity in PC12 cells by restoring cell integrity, mitochondrial metabolism, ROS generation, and arginase indirect activity. The α-Methyl-DL-aspartic acid (MDLA) and L-NΩ-Nitroarginine methyl ester (L-Name), specific inhibitors of AsS and nitric oxide synthase (NOS), which catalyzes the synthesis of polyamines and NO from L-arginine, did not suppress PRO-7a-mediated cytoprotection against oxidative stress. It suggested that its mechanism is independent of the production of L-arginine metabolites with neuroprotective properties by increased AsS activity. On the other hand, the neuroprotective effect of PRO-7a was blocked in the presence of dicyclomine hydrochloride (DCH), an M1 mAChR antagonist. Conclusions: For the first time, this work provides evidence that PRO-7a-induced neuroprotection seems to be mediated through M1 mAChR activation in PC12 cells, which reduces oxidative stress independently of AsS activity and L-arginine bioavailability.(AU)

Objetivo:descrever a o impacto da histerectomia e a participação dos profissionais de saúde durante o processo cirúrgico. O problema de pesquisa parte do seguinte questionamento: que impacto tem a histerectomia na vida da mulher em idade reprodutiva? Que participação tem os profissionais de saúde no processo cirúrgico? Método: Estudo de natureza descritiva com abordagem qualitativa conforme os pressupostos de Minayo (2009) com método de pesquisa exploratória. Resultados:Foram entrevistadas 7 mulheres que foram submetidas a histerectomia, com idade acima de 18 anos, que responderam perguntas referentes ao impacto do diagnóstico, conhecimento sobre a cirugia e experiências sobre o acolhimento dos profissionais da saúde no processo cirúrgico. A entrevista foi realizada em ambiente virtual na plataforma Google Forms. Conclusão: As entrevistadas revelaram sentimentos de angústia e medo ao se depararem com o diagnóstico cirúrgico por ausência de conhecimento inicialmente, evidenciando insatisfação e ausência dos profissionais da saúde quanto à cirurgia. O estudo reforça a importância das equipes multidisciplinares em hospitais e/ou locais de cuidados de saúde, uma vez que o foco principal no trabalho com pessoas acometidas por uma enfermidade não é a doença em si, mas elas mesmas.


Objective: to describe the impact of hysterectomy and the involvement of healthcare professionals during the surgical process. The research problem originates from the following questions: What impact does hysterectomy have on the life of a woman of reproductive age?What role do healthcare professionals play in the surgical process? Method:A descriptive study with a qualitative approach according to Minayo's (2009) assumptions using an exploratory research method. Results:Seven women who underwent hysterectomy, aged over 18 years, were interviewed. They answered questions regarding the impact of the diagnosis, knowledge about the surgery, and experiences of healthcare professionals' support during the surgical process. The interviews were conducted in a virtual environment using the Google Forms platform. Conclusion:The interviewees revealed feelings of anguish and fear when faced with the surgical diagnosis due to initial lack of knowledge, showing dissatisfaction and absence of healthcare professionals regarding the surgery. The study reinforces the importance of multidisciplinary teams in hospitals and/or healthcare facilities, as the main focus in working with people affected by an illness is not the disease itself, but the individuals themselves


Objetivo: describir el impacto de la histerectomía y la participación de los profesionales de la salud durante el proceso quirúrgico. El problema de investigación se basa en la siguiente pregunta: ¿qué impacto tiene la histerectomía en la vida de las mujeres en edadreproductiva? ¿Qué papel juegan los profesionales de la salud en el proceso quirúrgico? Método:Se trata de un estudio descriptivo con enfoque cualitativo según los supuestos de Minayo (2009) con un método de investigación exploratorio. Resultados:Se entrevistó a 7 mujeres mayores de 18 años sometidas a histerectomía, quienes respondieron preguntassobre el impacto del diagnóstico, conocimientos sobre cirugía y experiencias sobre la recepción de profesionales de la salud en el proceso quirúrgico. La entrevista se realizó en un entorno virtual en la plataforma Google Forms. Conclusión:Los entrevistados revelaron sentimientos de angustia y miedo frente al diagnóstico quirúrgico debido a la falta de conocimiento inicial, evidenciando insatisfacción y ausencia de profesionales de la salud con respecto a la cirugía. El estudio refuerza la importancia de los equipos multidisciplinarios en los hospitales y/o en los entornos sanitarios, ya que el foco principal en el trabajo con personas afectadas por una enfermedad no es la enfermedad en sí, sino ellos mismos.

ABSTRACT High Intensity Interval Training (HIIT) promotes similar adaptations and in some cases, superior to continuous aerobic training of moderate intensity, even when performed at all-out intensities. In this condition, it tends to have the greatest physiological disturbance and accumulation of metabolites compared to submaximal intensities, being one of the main factors associated with the negative response of pleasure in training. Objective: To verify the relationship between physiological and psychophysiological responses in order to repeat the training of two HIIT protocols of mesm volume load and different periods of work in trained adults of both sexes. Methods: In a randomized experiment, the sample consisted of 9 individuals aged 28±5 years, body mass 69.6±14.2 kg, height 169±0.1cm, BMI 24.1±2.3 kg/m2 and body fat percentage 20.2±7.9% practitioners of structured aerobic physical exercises for at least six months. Two different HIIT protocols were performed on an exercise bike, being (I) protocol 1:0.5, with prescribed intensity (80-90% of MAX HR), (II) protocol 1:2 with prescribed intensity (all-out) with total duration of effort of 20 minutes and interval of at least 48 hours between each session. Heart rate (HR), systemic blood pressure (SBP), subjective perception of exertion (SPE), visual analog scale (VAS), affective response (AR), fun (PACES) and intention to repeat the session were analyzed. The statistical analysis used was ANOVA with Tukey's post-hoc (p<0.05). Results: The analyses of HR, SBP, VAS, PACES and SPE showed no significant difference between the training protocols. The affective response (AR and intention to repeat) showed a decline in the protocol 1:0.5, but without significance (p>0.05). Conclusion: The data allow us to conclude that the 1:2 protocol caused greater physiological disturbances during exertion and presented greater positive affective response and intention to repeat the exercise session, variables that may contribute to adherence to regular physical exercise. RESUMEN El entrenamiento de intervalos de alta intensidad (HIIT) promueve adaptaciones similares y, en algunos casos, superiores al entrenamiento aeróbico continuo de intensidad moderada. En esta condición, tiende a tener una mayor alteración fisiológica y acumulación de metabolitos en comparación con las intensidades submáximas, siendo uno de los principales factores asociados con la respuesta negativa del placer en el entrenamiento. Objetivo: Verificar la relación entre las respuestas fisiológicas y psicofisiológicas para repetir el entrenamiento de dos protocolos HIIT de carga del mismo volumen y diferentes períodos de trabajo en adultos entrenados de ambos sexos. Métodos: En un experimento aleatorizado, la muestra estuvo constituida por 9 individuos de 28±5 años, masa corporal 69,6±14,2 kg, altura 169±0,1cm, IMC 24,1±2,3 kg/m2 y porcentaje de grasa corporal 20,2±7,9% practicantes de ejercicios físicos aeróbicos estructurados durante al menos seis meses. Se realizaron dos protocolos HIIT diferentes en bicicleta estática, siendo protocolo 1: 0,5, con intensidad prescrita (80-90% de la FC MÁXIMA), protocolo 1: 2 con intensidad prescrita (total) con duración total del esfuerzo de 20 minutos e intervalo de al menos 48 horas entre cada sesión. Se analizaron la frecuencia cardíaca (FC), presión arterial sistémica (PAS), percepción subjetiva de esfuerzo (PSE), escala analógica visual (EVA), respuesta afectiva (FS), diversión (PACES) e intención de repetir la sesión. El análisis estadístico utilizado fue ANOVA con post-hoc de Tukey (p<0,05). Resultados: Los análisis de HR, PAS, EVA, PACES y PSE no mostraron diferencias significativas entre los protocolos de entrenamiento. La respuesta afectiva (FS e intención de repetir) mostró una disminución en el protocolo 1:0,5, pero sin significancia (p>0,05). Conclusión: Los datos permiten concluir que el protocolo 1:2 causó mayores alteraciones fisiológicas durante el esfuerzo y presentó mayor respuesta afectiva positiva e intención de repetir la sesión de ejercicio, variables que pueden contribuir para la adhesión al ejercicio físico. RESUMO O Treinamento Intervalado de Alta Intensidade (HIIT) promove adaptações semelhantes e em alguns casos, superiores ao treinamento aeróbico contínuo de intensidade moderada, mesmo quando executado em intensidades all out. Nesta condição, tende a maior perturbação fisiológica e acúmulo de metabólitos comparado a intensidades submáximas, sendo um dos principais fatores associados a resposta negativa de prazer no treinamento. Objetivo: Verificar qual relação entre respostas fisiológicas e psicofisiológicas na intenção de repetir o treinamento de dois protocolos HIIT de mesmo volume load e distintos períodos de trabalho, em adultos treinados de ambos os sexos. Métodos: Em experimento randomizado, a amostra foi composta por 9 indivíduos com idade entre 28±5 anos, massa corporal 69,6±14,2 kg, estatura 169±0,1cm, IMC 24,1±2,3 kg/m2 e percentual de gordura corporal 20,2±7,9% praticantes de exercícios físicos aeróbicos estruturados por pelo menos seis meses. Realizaram dois diferentes protocolos de HIIT em bicicleta ergométrica, sendo (I) protocolo 1:0,5, com intensidade prescrita (80-90% da FCmáx), (II) protocolo 1:2 com intensidade prescrita (all-out) com duração total de esforço de 20 minutos e intervalo de pelo menos 48 horas entre cada sessão. Foram analisados frequência cardíaca (FC), pressão arterial sistêmica (PAS), percepção subjetiva de esforço (PSE), escala visual analógica (EVA), resposta afetiva (FS), divertimento (PACES) e intenção de repetir a sessão. A análise estatística usada foi ANOVA com post-hoc de Tukey (p<0,05). Resultados: As análises da FC, PAS, EVA, PACES e PSE não apresentaram diferença significativa entre os protocolos de treinamento. As respostas afetivas (FS e intenção de repetir) apresentaram maior declínio no protocolo 1:0,5, porém sem significância (p>0,05). Conclusão: Os dados permitem concluir que o protocolo 1:2 provocou maiores perturbações fisiológicas durante esforço e apresentou maior resposta afetiva positiva e intenção de repetir a sessão de exercício, variáveis que podem vir a contribuir na aderência à prática regular de exercícios físicos. ABSTRACT Objective: Analyze the effects of high-intensity interval training (HIIT) on cardiometabolic parameters, and cardiorespiratory fitness to compile the most used HIIT training types in adults with spinal cord injury (SCI). Methods: This is a systematic review of searches performed in the electronic databases PubMed / Medline, Science Direct, and Google Scholar. Studies included I) needed to apply HIIT training II) adults with SCI to analyze III) cardiometabolic aspects and cardiorespiratory fitness. Two independent reviewers selected the articles for inclusion, extracted their data, and assessed their methodological quality. Results: 654 studies were found. Thus, 12 studies, 11 pre- and post-intervention, and one control group (CG) with 106 participants were analyzed. Pre- and post-HIITT intervention results revealed significant improvement in cardiorespiratory fitness and cardiometabolic aspects (VO2peak, LDH, HDL, insulin resistance). In addition, GC results revealed significant improvement in cardiorespiratory fitness observed in the intervention group (HIIT) compared to the moderate-low intensity (GC) group. Seven studies used the arm ergometer as the primary exercise modality. Two studies described functional electrical stimulation (FES) performed with the arm ergometer plus electrical stimulation in the lower limbs. None reported heart rate dynamics during the study period. Conclusion: High-intensity interval training improves physical fitness and cardiometabolic health in adults with SCI. RESUMEN Objetivo: Analizar los efectos del entrenamiento interválico de alta intensidad (HIIT) sobre los parámetros cardiometabólicos, fitness cardiorrespiratorio y recopilar los tipos de HIIT más utilizados en el entrenamiento en adultos con lesión medular (LME). Métodos: Se trata de una revisión sistemática, para lo cual se realizaron búsquedas en bases de datos electrónicas PubMed/Medline, Science Direct y Google Scholar. Se incluyeron estudios que I) necesitaban aplicar entrenamiento HIIT en II) adultos con SCI y analizar III) aspectos cardiometabólicos y aptitud cardiorrespiratoria. Dos revisores independientes seleccionaron los artículos para su inclusión, extrajeron sus datos y evaluaron su calidad metodológica. Resultados: De los 654 estudios encontrados, se analizaron 12 estudios, 11 pre y post intervención y 1 grupo control (GC) con un total de 106 participantes. Los resultados previos y posteriores a la intervención HIIT revelaron una mejora significativa en la aptitud cardiorrespiratoria y los aspectos cardiometabólicos (VO2pico, LDH, HDL, resistencia a la insulina). Los resultados de GC revelaron una mejora significativa en la aptitud cardiorrespiratoria observada del grupo de intervención (HIIT) en comparación con el grupo de intensidad moderada-baja (GC). Siete estudios utilizaron el ergómetro de brazo como la modalidad principal de ejercicio. Dos estudios describieron la estimulación eléctrica funcional (EEF) realizada con el ergómetro de brazo más la estimulación eléctrica en los miembros inferiores. Ninguno informó la dinámica de la frecuencia cardíaca durante el período de estudio. Conclusiones: El entrenamiento intervalos de alta intensidad mejora la condición física y la salud cardiometabólica en adultos con LME. Evidencia de nivel II; Revisión sistemática de estudios de nivel II.

Métodos: Trata-se de revisão sistemática, para a qual foram realizadas pesquisas nas bases de dados eletrônicas PubMed / Medline, Science Direct e Google Scholar. Foram incluídos estudos em que I) o treinamento HIIT era aplicado em II) adultos com LME e analisaram III) os aspectos cardiometabólicos e aptidão cardiorrespiratória. Dois revisores independentes selecionaram os artigos para a inclusão, extraindo seus dados e avaliarando a sua qualidade metodológica. Resultados: 654 estudos foram encontrados. Desses, 12 estudos, 11 pré e pós intervenção e 1 grupo controle (GC) com um total de 106 participantes foram analisados. Introduction: Pheochromocytoma is a rare neoplasia arising from the adrenal medulla that secretes catecholamines. Those afflicted by this condition can present a wide range of symptoms. One of the most common is paroxysmic hypertension. Interestingly, although rare, some patients present with shock. We describe two cases of pheochromocytoma in which the initial presentation was shock. Case 1. 49 year-old woman, with a history of resistant hypertension, presented to the emergency department with thoracic pain and fever. EKG, echocardiogram (ECC), and myocardial necrosis markers were compatible with Takotsubo syndrome (TS). CT demonstrated a staghorn calculus, hydronephrosis, and signs compatible with xanthogranulomatous pyelonephritis in the right kidney. Additionally, and incidentally, it revealed a 60 mm nodule on the right adrenal gland. Piperacillin/tazobactam was started immediately, and the patient was submitted to urgent upper urinary tract drainage. This procedure was complicated by a cardiorespiratory arrest that was treated with adrenaline administration. The patient was admitted to the ICU due to multifactorial shock and started alpha and, posteriorly, beta blockage. Biochemical adrenal incidentaloma endocrinologic study was negative (under hemodialysis). Multiorgan failure progressively improved. After 2 weeks, the patient was submitted to a laparoscopic transperitoneal right adrenalectomy. No complications were reported. Histological analysis revealed a pheochromocytoma. Case 2. 28-year-old woman presented to the emergency department with headaches and nausea. Vitals were compatible with shock. CT revealed an incidental 72 mm mass on the right adrenal. EKG, ECC, and myocardial necrosis markers were compatible with TS. The patient was started on alpha and, posteriorly, beta blockage. Adrenal incidentaloma endocrinological study demonstrated high urinary catecholamines. Right transperitoneal adrenalectomy was performed. No complications were noted. Histological analysis revealed a pheochromocytoma. Conclusion: Pheochromocytoma can present with complex, enigmatic, and rare clinical pictures. Clinicians should be wary of the possibility of this diagnosis when managing adrenal masses.

An evaluation of the inflammatory enzymatic interactions related to pulmonary function can help identify biomarkers for interventions or prophylactic measures to improve patient prognosis. This study aimed to determine the effect of epoxide hydrolase inhibition by GSK2256294 in different pulmonary inflammation models. A secondary search was performed using Medline/PubMed, Web of Science, SciELO, Cochrane Library, Embase, Academic Google, and gray literature by two independent reviewers, who analyzed the methodological quality and consistency of the data. Different variables were compared using a meta-analysis. A total of 86 studies were found, 4 of which were selected from the gray literature. Based on the eligibility criteria, two clinical and one preclinical studies were evaluated. GSK2256294 inhibited the soluble epoxide hydrolase enzyme in both clinical and preclinical models, exhibiting greater effectiveness in clinical studies and contributing to the anti-inflammatory activity mediated by the eicosatrienoic pathway by reducing the levels of dihydroxyeicosatrienoic acids and leukotoxin-diol. Overall, GSK2256294 was identified as a promising drug for controlling the deleterious manifestations of lung inflammation. Further clinical and preclinical studies are required to ensure consistency among the evidence and identify other biological activities mediated by GSK2256294.

RESUMO

OBJECTIVE: To develop and validate an instrument to assist in the systematization of perioperative nursing care in robotic surgery. METHODS: Methodological study developed in four phases: content survey; textual elaboration; content validation by the group of expert judges and target audience; and elaboration of the electronic instrument layout. RESULTS: Eleven expert judges and seven evaluators of the target audience participated. For validation, the Content Validity Index (CVI) was used with a 0.78 cutoff point. The instrument total CVI after evaluation was 0.90 by the expert judges and 0.88 by the target audience. CONCLUSION: The tool built was proved satisfactory for the systematization of perioperative nursing care. The instrument construction was based on the updated scientific literature and validated by the expert judges and target audience.

ABSTRACT Purpose: This study aimed to analyze the diagnostic accuracy of dynamic and static ultrasound (DSUS) in detecting vesicoureteral reflux (VUR) and renal scarring in a cohort of children with neurogenic bladder (NB). Materials and Methods: A retrospective, longitudinal, observational study was conducted using the Reporting Diagnostic Accuracy Studies guideline. The DSUS (index test) data were compared with voiding cystourethrography (VCUG) and renal scintigraphy 99mTc-dimercaptosuccinic (reference tests). Overall performance for predicting VUR and renal scarring was assessed using renal pelvic diameter (RPD)/distal ureteral diameter and renal parenchymal thinning on DSUS, respectively. Results: A total of 107 patients (66 girls, median age 9.6 years) participated. Seventeen patients (15.9%) presented VUR, eight bilateral. For overall reflux grade, the AUC was 0.624 for RPD and 0.630 for distal ureteral diameter. The diagnostic performance for detecting high-grade VUR was slightly better for DSUS parameters. The AUC was 0.666 for RPD and 0.691 for distal ureteral diameter. The cut-offs of 5 mm for RPD and 6.5 mm for distal ureteral diameter presented the best diagnostic odds ratio (DOR) to identify high-grade VUR. The increase of RPD during detrusor contractions showed an accuracy of 89.2%. The thinness of renal parenchyma presented an accuracy of 88% for renal scarring. Conclusion: DSUS predicts VUR and renal scarring in children with NB with fair to good accuracy, and all measurements exhibited a high negative predictive value (NPV). The increase in RPD during voiding or detrusor contractions proved to be the most accurate parameter for indicating the presence of VUR in this study.

Background: Falcine meningiomas account for 5% of intracranial meningiomas. They may involve the eloquent cortex as well as vascular structures. Gross-total resection with additional margins has been shown to be beneficial to patients and has been associated with a reduction in disease recurrence. Case Description: A 57-year-old patient presented with recurrent frontal headaches that worsened when lying down. Magnetic resonance imaging showed a large lesion with homogeneous enhancement attached to the anterior third of the falx cerebri at the right frontal lobe, causing significant compression, and suggesting a meningioma. Conclusion: This operative video highlights the application of 1st-time, exposure, bone removal, arachnoid, irrigation, and reconstruction principles for safely and effectively removing a large falcine meningioma using the "grade zero" concept for maximal resection.
