Mesenchymal stromal cells (MSCs) have therapeutic potential due to their abilities of differentiation, immunomodulation, and migration to injured tissues, potentiating such effects when cells are activated. Guarana (Paullinia cupana) is a tropical plant species found in South America that is known for its antioxidant, stimulant, and cicatricial effects. The guarana extract is composed of many substances and caffeine is the main component. The objective was to evaluate the effects of guarana and caffeine on MSCs. After the initial characterization, MSCs were treated with Paullinia cupana (10, 100, and 1000 µg/mL) or caffeine (0.4, 4, and 40 µg/mL) for 24 h. MSCs treatment with 1000 µg/mL guarana increased cell polarity, viability, cell migration to chemoattractant, antioxidant potential, and liberation of extracellular vesicles (EVs), while it reduced the levels of autophagy. MSCs treated with 100 and 1000 µg/mL guarana or 40 µg/mL caffeine showed a decrease of cell proliferation. No treatment affected the cellular area and cell cycle of MSCs. The study shows in vitro evidence that guarana could be a promising alternative for activating MSCs to promote better cellular products for future clinical therapies.

PURPOSE: Literature has demonstrated an inverse relation between magnesium (Mg) consumption and development of type 2 diabetes mellitus (T2DM), hypertension (HT), and dyslipidemia. After bariatric surgery (BS), micronutrients deficiencies are common, it being important to ensure appropriate supplementation. There is no recommendation about Mg supplementation and to our knowledge, its effect has not been studied to date. Our aim was to evaluate the effect of Mg supplementation in cardio-metabolic risk factors on post-bariatric patients. MATERIALS AND METHODS: A retrospective observational study of patients with obesity who underwent BS was performed. Data was assessed preoperatively and yearly (4-year follow-up). RESULTS: A total of 3363 patients were included. In the first year of follow-up, 79.8% (n = 2123) of the patients were supplemented with Mg, with evidence of slightly decreased percentages in the following years. Mg deficiency (serum Mg < 1.52 mEq/L) was more common among patients who were not supplemented during each year of follow-up (p < 0.05). Among those who underwent Mg supplementation, the percentage of T2DM, HT, or low-density lipoprotein cholesterol (LDL-C) > 130 mg/dL was significantly lower. In the first year post-surgery, the supplementation group had a lower risk of T2DM (OR = 0.545, p < 0.0001), LDL-C > 130 mg/dL (OR = 0.612, p < 0.0001), and HT (OR = 0.584, p < 0.0001). The OR for having these metabolic comorbidities persisted lower during the 4 years' follow-up. Patients who had Mg deficiency had higher prevalence of T2DM and HT. CONCLUSION: Mg supplementation seems to have a protective effect on the development of T2DM, HT, and LDL-C > 130 mg/dL in post-bariatric patients.

Glioblastoma patients have a poor prognosis mainly due to temozolomide (TMZ) resistance. NRF2 is an important transcript factor involved in chemotherapy resistance due to its protective role in the transcription of genes involved in cellular detoxification and prevention of cell death processes, such as ferroptosis. However, the relation between NRF2 and iron-dependent cell death in glioma is still poorly understood. Therefore, in this study, we analyzed the role of NRF2 in ferroptosis modulation in glioblastoma cells. Two human glioblastoma cell lines (U251MG and T98G) were examined after treatment with TMZ, ferroptosis inducers (Erastin, RSL3), and ferroptosis inhibitor (Ferrostatin-1). Our results demonstrated that T98G was more resistant to chemotherapy compared to U251MG and showed elevated levels of NRF2 expression. Interestingly, T98G revealed higher sensitivity to ferroptosis, and significant GSH depletion upon system xc- blockage. NRF2 silencing in T98G cells (T98G-shNRF2) significantly reduced the viability upon TMZ treatment. On the other hand, T98G-shNRF2 was resistant to ferroptosis and reverted intracellular GSH levels, indicating that NRF2 plays a key role in ferroptosis induction through GSH modulation. Moreover, silencing of ABCC1, a well-known NRF2 target that diminishes GSH levels, has demonstrated a similar collateral sensitivity. T98G-siABCC1 cells were more sensitive to TMZ and resistant to Erastin. Furthermore, we found that NRF2 positively correlates with ABCC1 expression in tumor tissues of glioma patients, which can be associated with tumor aggressiveness, drug resistance, and poor overall survival. Altogether, our data indicate that high levels of NRF2 result in collateral sensitivity on glioblastoma via the expression of its pro-ferroptotic target ABCC1, which contributes to GSH depletion when the system xc- is blocked by Erastin. Thus, ferroptosis induction could be an important therapeutic strategy to reverse drug resistance in gliomas with high NRF2 and ABCC1 expression.

With the aim of improving phototherapy for neonatal jaundice (hyperbilirubinemia), this study investigates the degradation of unconjugated bilirubin under irradiance by conventional light and by white, red, green, and blue LED sources in vitro. The absorption spectra of bilirubin under these different light sources are compared. The results demonstrate that white LED phototherapy promotes more efficient bilirubin degradation than conventional blue-light therapy. This study provides a basis for the design of novel phototherapy devices for the treatment of hyperbilirubinemia.

Introduction. Central pontine myelinolysis is characterized by the occurrence of acute demyelinating lesions of cells in the pons secondary to abrupt oscillations of serum osmolarity. Its exact incidence is not well defined, but studies show a prevalence of 0.25 to 0.5% in the general population, 2.5% in the intensive care unit, and up to 10% in patients with risk factors, such as chronic liver disease and hepatic transplantation, alcoholism, malnutrition, diuretic therapy, electrolyte imbalance, hypoglycemia, and hyperglycemia. Case Report. A 70-year-old white female with extranodal diffuse large B-cell non-Hodgkin's lymphoma (extensive mass on the left anterior chest wall), stage IVA, developed pontine myelinolysis secondary to hypovolemic acute hypernatremia, which occurred due to diarrhea caused by chemotherapy (rituximab, cyclophosphamide, doxorubicin, and vincristine). Discussion. Pontine myelinolysis occurs most often due to the rapid correction of chronic hyponatremia. But here, we describe a case of the disease secondary to the occurrence of hypovolemic acute hypernatremia in a patient with a hematological malignancy under treatment, who was on chronic treatment with thiazide diuretics and who presented with other electrolyte disturbances as risk factors for the development of pontine myelinolysis.

O objetivo deste trabalho foi estudar a prevalência de MG e MS e a filogenia das cepas circulantes, comparando-as com outras já descritas em poedeiras comerciais no Brasil. Foram coletados 140 suabes traqueais de poedeiras comerciais com sinais respiratórios em seis granjas da região centro-oeste de São Paulo. As amostras foram avaliadas por PCR, com posterior sequenciamento e análise filogenética das cepas identificadas. Das 140 amostras, 16,4% foram positivas para MG e 68,6% para MS. Houve diferença significativa nas frequências de MG e MS por granja, segundo o teste G de independência (P<0,05). Todas as cepas identificadas de MG e MS de granjas distintas apresentaram similaridade tanto pela lipoproteína para MG quanto pela região 16s rRNA para MS. Neste estudo, foi possível observar altas prevalências dos agentes estudados, sendo a de MS maior que a de MG. Foi detectada infecção mista por MG e MS em 11,4% das amostras e sabe-se que esses micoplasmas podem agir de forma sinérgica, agravando o quadro respiratório. As cepas circulantes identificadas, pela análise das regiões gênicas da lipoproteína para MG e 16S rRNA para MS, são similares em todas as granjas estudadas.(AU)


The aim of this study was to evaluate the prevalence of MG and MS and the phylogeny of the circulating strains, comparing them with others already described in commercial laying hens from Brazil. A total of 140 tracheal swabs were collected from commercial laying hens with respiratory signs in six farms from the western region of São Paulo state. The samples were analyzed by PCR with subsequent sequencing and phylogenetic analysis of the identified strains. From the 140 samples, 68.6% were positive for MS and 16.4% for MG. There was a significant difference in the frequencies of MG and MS per farm according to G Test of independence (P<0.05). All strains identified as MG and MS from distinct farms presented similarity both by lipoprotein to MG and by 16s rRNA region to MS. In this study, it was possible to observe a high prevalence of MS compared to MG. Mixed MG and MS infection was detected in 11.4% of the samples. These mycoplasmas may act synergistically, worsening the respiratory signs. The circulating strains identified by analysis of the lipoprotein for MG and 16S rRNA for MS are similar on all poultry farms studied.(AU)

Apesar dos inúmeros benefícios da fluidoterapia transcirúrgica, sobrecarga de volume pode trazer efeitos deletérios, como a alteração de parâmetros hematimétricos. Dessa maneira, o presente trabalho teve como objetivo avaliar duas diferentes taxas de infusão de solução cristaloide no período transanestésico e seus efeitos até 24 horas pós-operatório. Foram utilizados dois grupos de seis animais cada, um deles recebeu solução de cloreto de sódio 0,9% a 10mL/kg/h (G10) e o outro a 5mL/kg/h (G5). Os valores de hematócrito foram avaliados no período de 24 horas em 10 diferentes momentos. Os resultados não apontaram diferenças significativas entre os grupos, porém foi observada redução significativa do hematócrito após indução anestésica. Foi ainda observada redução de hematócrito após o término da cirurgia em ambos os grupos, e o retorno aos valores basais de hematócrito ocorreu de forma significativa 12 horas após o procedimento cirúrgico em G10, e após oito horas em G5, mostrando uma tendência à hemodiluição mais persistente em G10.(AU)


Despite the beneficial goals of fluid therapy administered during surgery, volume overload can cause deleterious effects, such as alterations on hematimetric parameters. Thus, the objective of this paper was to evaluate two different cristaloid infusion rates during the surgical period and its effects on the 24-hour post-surgical period. Two groups of six animals each were used in the present study, one received 10mL/kg/h (G10) of 0,9% sodium chloride solution and the other 5mL/kg/h (G5) of the same solution. Packed cell volume (PCV) was evaluated in 10 different times during the 24 hours following surgery. The results did not show significant differences between groups, but they showed a major tendency of hemodilution in G10. A significant decrease of PCV was observed after induction of anesthesia. Decrease of PCV after the end of surgery in both groups was also observed, and the return to PCV basal values was observed 12 hours after the procedure in G10 and after eight hours in G5, showing a tendency of prolonged hemodilution in G10.(AU)

The former mine of Escádia Grande was active at the middle of 1900 and was exploited for Au and Ag. The mineralized quartz veins consist mainly of quartz, arsenopyrite, pyrite, rare chalcopyrite, galena, sphalerite, gold and argentite. The mine dumps and tailings were deposited close to a stream, and there is a river beach downstream used for recreational proposes. Two villages are also located close to the old mining area. Mine wastes contained up to 8090 mg/kg of As and 70.1 mg/kg of Sb. The waters of the stream that cross the mining area have circum-neutral pH values and contained elevated concentrations of As reaching up to 284 µg/L. However, geochemical speciation modeling (Phreeq C) revealed that As was mainly present as As (V). Arsenic concentrations in waters are attenuated throughout the stream, mainly by the iron-(hydro)-oxides adsorption upstream. However, at 2 km downstream of mine wastes in the river beach, the waters still exceeded 10 µg/L of As, the drinking water limit. The waters also have NO2-, Cu and Cd concentrations higher than drinking water limit. The stream sediments have As concentrations up to 45 times higher (3140 mg/kg) than the limit of the sediment guideline values of NWQMS (2000). The maximum arsenic concentrations in soils are also up to 27 times higher (5940 mg/kg) than the maximum concentrations in streams from FOREGS Geochemical Atlas of Europe. The use of river beach for recreational purposes causes cancer risk (4.48 × 10-6) higher than USEPA limit, mainly due to the arsenic exposure. Even for recreational purposes, stream sediments and soils in the old mining area have high non-carcinogenic effects (2.76 and 4.78, respectively) for children, also related to the arsenic exposure mainly by the ingestion pathway, and the risk is unacceptable according to the limits of USEPA. Moreover, the cancer risk resulting from exposure of adults to arsenic in soils also has unacceptable non-cancer risk (1.13). Arsenic is the main trace element that causes a human health concern in the Escádia Grande mining area.

BACKGROUND: Patients in the immediate post-operative liver transplantation (LxT) period can develop respiratory and functional complications. In the postoperative months, these functions tend to improve. Nevertheless, there are few studies that evaluate precisely and specifically respiratory function in post-LxT long-term after surgery. The objective of the study was to describe the respiratory profile of patients 1 to 6 months after LxT, accompanied by LxT outpatients. METHODS: We included patients between 25 and 60 years old. We excluded patients with chronic renal or cerebrovascular impairment, severe heart disease, and history of lung surgery or liver re-transplantation. Evaluations were carried out on 3 occasions: 1 month, 3 months, and 6 months after LxT. The following evaluations were submitted: respiratory muscle strength (manuvacuometer), value flows and lung volumes (spirometer), and surface electromyography analyzing root mean square in the right (RMS-R) and left (RMS-L) diaphragm. We analyzed MELD (Model for End-Stage Liver Disease). After normality tests, we used the Friedman test (non-parametric values) and ANOVA (parametric values), P ≥ .5 with the use of SPSS 21.0. RESULTS: Patients (n = 15) had a mean age of 53.0 ± 7.5 years and 25.9 ± 4.6 MELD score. The statistically significant value obtained at the 3 occasions of evaluation was RMS-R, with a decline during periods of evaluation. This can be caused by removal of the liver, resulting in a denervation and reduction in compliance of this portion of the muscle. CONCLUSIONS: Patients between 1 and 6 months after transplantation have a specific respiratory profile, close to normal values. However, there are few studies on this subject, and we suggest that more research be done.

The aim of this study was to evaluate the phenolic and flavonoids contents and the antioxidant and antitumoral activity of leaf and calyx methanolic extracts from Hibiscus sabdariffa (roselle) cultivated with poultry litter and organosuper® under three modes of application. The total phenolic content in the each extract was determined using the Folin-Ciocalteu reagent and for aluminium chloride flavonoids. The antioxidant parameters were analyzed using a 2, 2-diphenyl-1-picrylhydrazyl (DPPH.) free radical scavenging assay. An antitumor colorimetric assay using sulforhodamine B. The highest contents of phenolic and flavonoids were observed in leaf extracts (389.98 and 104.52 mg g-1, respectively) and calyx extracts (474.09 and 148.35 mg g-1, respectively) from plants cultivated with organosuper®, although these values did not differ significantly from those observed for the other treatments. The average IC50 of leaves (43.48 µg mL-1) and calyces (37.15 µg mL-1) demonstrated that both have substances that may contribute to free radical scavenging action. The methanol extract from calyces showed significant selective activity against a leukemia line (K-562), with IC50 values of 0.12 mg mL-1 (organosuper®) and 1.16 mg mL-1 (poultry litter), with concentration-dependent, cytotoxic and cytocidal effects.

The Saanen is a highly productive breed, and for this reason, it has been raised in Brazil, but mostly under climate conditions completely different from where the breed originated. The objective of this study was to investigate variations in semen parameters and sperm membrane proteins from Saanen bucks (n = 7) raised in Northeastern Brazil, during dry season (September, October, and November) and rainy season (March, April, and May). We showed that during the dry season, sperm motility, concentration, and the percentage of normal sperm decreased as compared to the rainy season. Rectal temperatures of bucks had no significant (p > 0.05) variations during the dry and rainy seasons. However, temperatures of left and right skin testis were higher (p < 0.05) during the dry as compared to the rainy season. Expression of three proteins (lysine-specific demethylase 5D, adenosine triphosphate (ATP) synthase subunit d, and radial spoke head protein 9 homolog) in sperm membrane were more intense in rainy season and only one protein (cytosol aminopeptidase) had greater expression in the dry season of the year. Our results show that mechanisms of testicular thermoregulation of Saanen bucks did not prevent a decrease in seminal parameters during the dry season. This deterioration may be related to reduced expression of proteins associated with important functions in sperm membrane.

Estudou-se o efeito da categoria reprodutiva sobre a fertilidade de éguas inseminadas com sêmen asinino diluído, resfriado e armazenado. Os ciclos foram acompanhados por palpação retal e rufiação, sendo as inseminações realizadas às terças, quintas e sábados, a partir da detecção de um folículo de 3,0 a 3,5cm de diâmetro, em um dos ovários, até a ovulação. O sêmen de cinco jumentos da raça Pêga foi diluído nos diluidores de leite em pó desnatado-glicose ou glicina-gema de ovo, resfriado a 5ºC e armazenado por 12 horas, sendo a dose inseminante de 400 x 106 espermatozoides móveis (no momento da diluição final, pré-resfriamento) depositada no corpo do útero. O diagnóstico de gestação foi realizado por meio de palpação transretal, rufiação e ultrassonografia, realizada a cada 14 dias. Os resultados de 195 ciclos, referentes a 141 éguas, foram agrupados de acordo com a categoria reprodutiva a que pertenciam: potra, égua solteira, égua parida e no "cio do potro". As taxas de concepção, ao primeiro ciclo, foram de 60,00%, 48,28%, 75,00% e 47,17% e, após quatro ciclos, de 61,54%, 47,13%, 54,76% e 47,17%, na mesma ordem para as categorias descritas anteriormente. A categoria reprodutiva não teve efeito (P>0,05) sobre a fertilidade das éguas inseminadas com sêmen asinino resfriado, sendo as potras, éguas solteiras, éguas paridas e no "cio do potro" igualmente eficientes para o uso na reprodução...


The effect of the mare status on fertility of mares inseminated with diluted, cooled and stored jackass semen, was studied. The cycles were controlled by transrectal palpation and teasing, and mares were inseminated every Tuesday, Thursday and Saturday, from the detection of a follicle with 3.0 to 3.5cm diameter in one of the ovaries until ovulation. The semen of five Pêga jackasses was diluted in skim milk-glucose or in egg yolk-glycine extender and cooled at 5ºC for 12 hours, with the inseminate dose of 400 x 106 motile spermatozoa (at the moment of the final dilution, before cooling). The inseminations were carried out in the uterine body. Pregnancy was detected using transrectal palpation, teasing and ultrasound exams every 14 days. The results of 195 cycles of 141 mares were grouped according to the mare status: maiden, barren, lactation or in foal heat. Pregnancy rates for the first cycle were 60.00%, 48.28%, 75.00% and 47.17%, and after four cycles, the pregnancy rates/cycle were 61.54%, 47.13%, 54.76% and 47.17%, respectively for maiden, barren, lactation and in foal heat mares (P>0,05). The mare status did not affect pregnancy rates of mares inseminated with diluted and cooled jackass semen and were efficient to use on reproduction...

Estudou-se o efeito da idade sobre a fertilidade de éguas inseminadas com sêmen asinino diluído, resfriado e armazenado. Os ciclos foram acompanhados por palpação transretal e rufiação, sendo as inseminações realizadas às terças, quintas e sábados, a partir da detecção de um folículo de 3,0 a 3,5cm de diâmetro, em um dos ovários, até a ovulação. O sêmen de cinco jumentos da raça Pêga foi diluído nos diluidores de leite em pó desnatado-glicose ou glicina-gema de ovo, resfriado a 5ºC e armazenado por 12 horas, sendo a dose inseminante de 400 x 106 espermatozoides móveis (no momento da diluição final, pré-resfriamento). Os resultados de 195 ciclos estrais, referentes a 141 éguas, foram agrupados em classes, de acordo com a idade das éguas: 2,5 a 6 anos, 6,5 a 10 anos, 10,5 a 14 anos e 14,5 a 19 anos. As taxas de concepção, ao primeiro ciclo, foram de 68,42%, 50,75%, 46,88% e 52,17% e, após quatro ciclos, de 69,57%, 47,92%, 46,34% e 45,71% para as faixas etárias de 2,5 a 6, 6,5 a 10, 10,5 a 14 e 14,5 a 19 anos, respectivamente (P>0,05). A idade não teve efeito sobre a fertilidade das éguas inseminadas com sêmen asinino diluído e resfriado...


The effect of the mare age on fertility of mares inseminated with diluted, cooled and stored jackass semen was studied. The females were controlled by transrectal palpation and teasing, and inseminated every Tuesday, Thursday and Saturday, since the detection of a 3.0 to 3.5cm follicle diameter, in one of the ovaries, until ovulation. The semen of five Pêga jackasses was diluted in skim milk-glucose or in egg yolk-glicine extender and cooled at 5ºC for 12 hours, with the inseminate dose of 400 x 106 motile spermatozoa (at the moment of the final dilution, before cooling). The results of 195 cycles of 141 mares were grouped, in accordance with the age: 2.5 to 6 years, 6.5 to 10 years, 10.5 to 14 years and 14.5 to 19 years. O objetivo deste trabalho foi observar a influência do campo magnético (CM) de baixa frequência na membrana do saco vitelínico (MSV) e no desenvolvimento do embrião de codornas japonesas (Coturnix japonica) em 72 horas de incubação. Ovos fertilizados foram expostos a nove horas consecutivas de CM, sendo um grupo a partir das 48 horas e o outro a partir das 63 horas de incubação. A quantificação da vascularização da MSV foi determinada pela obtenção da dimensão fractal por meio dos métodos de box-counting e de dimensão de informação, enquanto o peso corporal e o percentual de comprimento cefálico dos embriões foram utilizados como parâmetros de desenvolvimento embrionário. The aim of this study was to observe the influence of the low frequency magnetic field (MF) on the yolk sac membrane (YSM) and embryonic development of Japanese quail (Coturnix japonica) in 72 hours of incubation. Fertilized eggs were exposed to 9 consecutive hours of MF, with a group from 48 hours and the second group from 63 hours of incubation. The evaluation of YSM vascularization was determined by the fractal dimensions obtained through box-counting method and information dimension, while body weight of the embryo and percentage of cephalic length were used as parameters for embryo development. GSTM1 (glutathione S-transferase mu 1) and GSTT1 (glutathione S-transferase theta 1) are critical enzymes for detoxification of endogenous and environmental carcinogens. Constitutive GST gene polymorphisms may be associated with increased risk for cancer development. We made an explorative study of a Brazilian population with malignant glioma to determine whether GSTM1 and GSTT1 genetic polymorphisms influence the response to intranasal administration of perillyl alcohol and the survival rate. Patients were stratified into groups according to clinical presentation, tumor classification, and tumor location. Circulating DNA was extracted from blood plasma or serum, and genotypes were detected by multiplex PCR. The cohort included 95 patients with recurrent malignant glioma included in a Phase I/II clinical trial with perillyl alcohol and 100 matched healthy control subjects. GSTM1 frequency was similar in patients with glioma (44%) and healthy controls (54%), but GSTT1 deletion was found in 11.5% patients, contrasting with 36% in controls. A longer survival rate was associated with a lack of GSTM1 deletion (31 weeks) and a deletion for GSTT1 (28 weeks). A poor survival rate was associated with GSTM1 deletion (23 weeks) and with a lack of a GSTT1 deletion (19 weeks). A significantly lower frequency of GSTT1 deletion in glioma patients compared to healthy controls indicates that GSTT1 deletion may exert a protective role against gliomagenesis, influence therapeutic response to intranasal perillyl alcohol treatment, and increase overall survival, especially considering tumor topography.

The aim of this study was to evaluate the rapid tests currently used for canine parvovirus (CPV) diagnosis: hemagglutination test (HA), enzyme immunoassay (EIA) and polymerase chain reaction (PCR). A total of 112 fecal samples collected from diarrheic puppies up to one year of age were tested. The EIA was able to detect CPV antigen in 44 samples. By HA, 32 samples tested highly positive with titers >128, eight tested weakly positive (titers 32 and 64) and 72 were negative (titers <16). Using PCR, 57 samples were found positive including 13 EIA-negative and 19 HA-negative samples. The best correlation was observed between EIA and PCR (88.4%). These tests were able to detect all types of CPV, including CPV-2c. Considering that 23%-33% of dogs presenting enteritis did not show infection by EIA nor HA, negative results from the antigen detection tests should be confirmed through molecular methods.


Avaliaram-se os métodos rápidos rotineiramente utilizados para diagnóstico da infecção por parvovírus canino (CPV): teste de hemaglutinação (HA), ensaio imunoenzimático (EIE) e reação em cadeia pela polimerase (PCR). Um total de 112 amostras fecais de cães diarreicos com até um ano de idade foi testado. O EIE foi capaz de detectar o antígeno do CPV em 44 amostras. Por HA, 32 amostras foram consideradas fortemente positivas com títulos >128, oito fracamente positivas (títulos 32 e 64) e 72 negativas (títulos <16). Por PCR, 57 amostras foram positivas incluindo 13 EIE-negativas e 19 HA-negativas. A melhor correlação foi observada entre EIE e PCR (88,4%). Os testes foram capazes de detectar todos os tipos de CPV, incluindo o CPV-2c. Considerando-se que em 23%-33% dos filhotes com enterite a infecção por CPV não foi diagnosticada pelos testes de EIE e HA, os resultados negativos nos testes de detecção de antígeno devem ser confirmados por meio de métodos moleculares.

OBJECTIVE: To evaluate the effect of early fetoscopic tracheal occlusion (FETO) (22-24 weeks' gestation) on pulmonary response and neonatal survival in cases of extremely severe isolated congenital diaphragmatic hernia (CDH). METHODS: This was a multicenter study involving fetuses with extremely severe CDH (lung-to-head ratio < 0.70, liver herniation into the thoracic cavity and no other detectable anomalies). Between August 2010 and December 2011, eight fetuses underwent early FETO. Data were compared with nine fetuses that underwent standard FETO and 10 without fetoscopic procedure from January 2006 to July 2010. FETO was performed under maternal epidural anesthesia, supplemented with fetal intramuscular anesthesia. Fetal lung size and vascularity were evaluated by ultrasound before and every 2 weeks after FETO. Postnatal therapy was equivalent for both treated fetuses and controls. Primary outcome was infant survival to 180 days and secondary outcome was fetal pulmonary response. RESULTS: Maternal and fetal demographic characteristics and obstetric complications were similar in the three groups (P > 0.05). Infant survival rate was significantly higher in the early FETO group (62.5%) compared with the standard group (11.1%) and with controls (0%) (P < 0.01). Early FETO resulted in a significant improvement in fetal lung size and pulmonary vascularity when compared with standard FETO (P < 0.01). CONCLUSIONS: Early FETO may improve infant survival by further increases of lung size and pulmonary vascularity in cases with extremely severe pulmonary hypoplasia in isolated CDH. This study supports formal testing of the hypothesis with a randomized controlled trial.
