RESUMO

This review aims to identify and analyze the instruments employed for assessing the overall performance of workers exposed to fluoroscopy during surgical procedures and/or interventional procedures, using the PRISMA methodology. The studies were located through searches conducted on PubMed, Web of Science, Lilacs, ScienceDirect, B-ON, EBSCOhost, and EBSCO Discovery Service on March 27, 2023. Additional studies were identified using backward and forward citation techniques. The PEO strategic model was followed. The search spanned studies published between 2012 and 2022. The quality of the studies underwent assessment using the Joanna Briggs Institute checklist for analytical cross-sectional studies. Out of the 23 studies identified, encompassing 3604 individuals, 12 (52%) addressed the reliability and/or validity of the instruments, while 3 (13%) focused on the development and psychometric testing of the instruments. Only 5 instruments (23%), validated and reliable, exclusively evaluated occupational radiological protection. The predominant dimension covered was knowledge (82%, n = 19). The population was assessed in 18 studies, pre- and post-intervention in 2 studies, and 21 studies provided recommendations or tools for improvement. Individuals in the studies utilized passive dosimeters (ranging from 5% to 98%), thyroid shields (15% to 98%), and aprons or lead skirts/coats (28% to 99%). The evidence quality was moderate (6/8). This study underscores the imperative to enhance compliance with protective and monitoring equipment. Furthermore, additional information is warranted concerning the validity and reliability of the instruments used, as well as the development of instruments that are both valid and reliable.

O objetivo do trabalho é analisar o fluxo de acesso às Unidades Básicas de Saúde no município de Pelotas/RS no ano de 2022. Metodologia: Estudo qualitativo observacional, exploratório e descritivo realizado através da coleta de dados na Secretaria Municipal de Saúde, e busca nas bases de dados referenciais. Resultados: Em Pelotas/RS, no ano de 2022, possuía 51 Unidades Básicas contempladas por 100 equipes de saúde. Em relação ao horário de funcionamento, 90% das unidades atuavam em horário comercial e, apenas 10%, em horário estendido. Quanto à territorialização, encontrava-se em processo de adesão à política de população adscrita, tendo, no momento do estudo, 45 de suas unidades com a área populacional total de abrangência. Além disso, nos últimos anos foi implementado o sistema de acolhimento como forma de primeiro atendimento. Conclusões: No que tange ao objeto de estudo - o acesso - viu-se que a principal barreira está no horário de funcionamento das unidades. A modificação na forma de atendimento, implementado o acolhimento, foi visivelmente benéfico à população, permitindo maior porta de entrada à saúde. Evidenciou-se, ainda, algumas lacunas que podem impedir o acesso integral dos indivíduos, como a falta de alguns profissionais nas unidades e a fragmentação do sistema, esse último podendo ser solucionado com o incentivo e o investimento à tecnologia, a favor da integração dos serviços.

Abstract Objective To describe a cohort of placenta accreta spectrum (PAS) cases from a tertiary care institution and compare the maternal outcomes before and after the creation of a multidisciplinary team (MDT). Methods Retrospective study using hospital databases. Identification of PAS cases with pathological confirmation between 2010 and 2021. Division in two groups: standard care (SC) group - 2010-2014; and MDT group - 2015-2021. Descriptive analysis of their characteristics and maternal outcomes. Results During the study period, there were 53 cases of PAS (24 - SC group; 29 - MDT group). Standard care group: 1 placenta increta and 3 percreta; 12.5% (3/24) had antenatal suspicion; 4 cases had a peripartum hysterectomy - one planned due to antenatal suspicion of PAS; 3 due to postpartum hemorrhage. Mean estimated blood loss (EBL) was 2,469 mL; transfusion of packed red blood cells (PRBC) in 25% (6/24) - median 7.5 units. Multidisciplinary team group: 4 cases of placenta increta and 3 percreta. The rate of antenatal suspicion was 24.1% (7/29); 9 hysterectomies were performed, 7 planned due to antenatal suspicion of PAS, 1 after intrapartum diagnosis of PAS and 1 after uterine rupture following a second trimester termination of pregnancy. The mean EBL was 1,250 mL, with transfusion of PRBC in 37.9% (11/29) - median 2 units. Conclusion After the creation of the MDT, there was a reduction in the mean EBL and in the median number of PRBC units transfused, despite the higher number of invasive PAS disorders.


Resumo Objetivo Descrever uma coorte de casos do espectro do acretismo placentário (PAS) de uma instituição terciária e comparar os resultados maternos antes e depois da criação de uma equipa multidisciplinar (MDT). Métodos Estudo retrospectivo utilizando bancos de dados hospitalares. Identificação de casos de PAS com confirmação patológica entre 2010 e 2021. Divisão em dois grupos: grupo Standard Care (SC) - 2010-2014; e grupo MDT - 2015-2021. Análise descritiva de suas características e desfechos maternos. Resultados Durante o período do estudo, houve 53 casos de PAS (24 - grupo SC; 29 - grupo MDT). Grupo Standard Care: 1 placenta increta e 3 percretas; 12,5% (3/24) tiveram suspeita anteparto; 4 casos tiveram histerectomia periparto - uma eletiva devido à suspeita anteparto de PAS; 3 devido a hemorragia pós-parto. A média de perda hemática estimada (EBL) foi de 2.469 mL; transfusão de concentrado eritrocitário (PRBC) em 25% (6/24) - mediana 7,5 unidades. Equipa multidisciplinar: 4 casos de placenta increta e 3 percretas. A taxa de suspeita anteparto foi de 24,1% (7/29); foram realizadas 9 histerectomias, 7 eletivas por suspeita anteparto de PAS, 1 após diagnóstico intraparto de PAS e 1 após rotura uterina após interrupção da gravidez no segundo trimestre. A EBL média foi de 1.250 mL, com transfusão de PRBC em 37,9% (11/29) - mediana de 2 unidades. Conclusão Após a criação da MDT, houve redução na média de EBL e na mediana do número de unidades de PRBC transfundidas, apesar do maior número de PAS invasivos.

ABSTRACT Objective: To estimate the prevalence of allergic rhinitis symptoms and associated factors in six-year-old children. Methods: Cross-sectional epidemiological study involving 956 six-year-old schoolchildren from Palhoça, Santa Catarina, Brazil. Home interviews were conducted with mothers in which socio-demographic and house environmental conditions information were obtained, and the International Study of Athma and Allergies in Childhood (ISAAC) questionnaire for allergic rhinitis symptoms was applied. Bivariate and multivariate hierarchical analyses were performed using Poisson regression with a robust estimator. Results: The prevalence of allergic rhinitis symptoms was 21.7%. Children whose mothers had over 8 years of education, or who had air conditioning equipment in the house, or whose bedroom walls presented mold or moisture showed statistically significant and independent 5% higher prevalence of allergic rhinitis. Similarly, children of smoker mothers or those who lived with fur or feather animals indoors showed a 4% higher prevalence. Conclusion: Significant associations were observed between socio-demographic factors and environmental conditions in child's home and allergic rhinitis symptoms in children aged six years.


RESUMO Objetivo: Estimar a prevalência de sintomas de rinite alérgica e fatores associados em crianças de seis anos de idade. Métodos: Estudo epidemiológico de delineamento transversal envolvendo 956 escolares de seis anos de idade do município de Palhoça, Santa Catarina, Brasil. Foram realizadas entrevistas domiciliares com as mães sobre as informações sociodemográficas e as condições ambientais da casa e foi aplicado o questionário ISAAC para sintomas de rinite alérgica. Análises bivariada e multivariada hierarquizada foram realizadas por meio da Regressão de Poisson com estimador robusto. Resultados: A prevalência de sintomas de rinite alérgica foi de 21,7%. Crianças cujas mães apresentavam escolaridade maior que oito anos ou com presença de aparelho de ar condicionado ou mofo/umidade nas paredes do quarto em que dormiam apresentaram prevalência 5% maior, estatisticamente significativa e independente, de rinite alérgica. Da mesma forma, crianças com mães tabagistas ou que conviviam com animais de pelo ou pena dentro de casa apresentaram prevalência 4% maior. Conclusão: Foram observadas associações significativas entre fatores sociodemográficos e condições ambientais da casa da criança e sintomas de rinite alérgica em crianças de seis anos de idade.

ABSTRACT After 2 years of the COVID-19 pandemic, the protocols used to control infection lack attention and analysis. We present data about deposits of complete genomic sequences of SARS-CoV-2 in the Global Initiative on Sharing All Influenza Data (GISAID) database made between January 2021 and May 31, 2022. We build the distribution profile of SARS-CoV-2 variants across South America, highlighting the contribution and influence of each variant over time. Monitoring the genomic sequences in GISAID illustrates negligence in the follow up of infected patients in South America and also the discrepancies between the number of complete genomes deposited throughout the pandemic by developed and developing countries. While Europe and North America account for more than 9 million of the genomes deposited in GISAID, Africa and South America deposited less than 400 000 genome sequences. Genomic surveillance is important for detecting early warning signs of new circulating viruses, assisting in the discovery of new variants and controlling pandemics.


RESUMEN Tras dos años de pandemia del COVID-19, los protocolos empleados para controlar la infección carecen de atención y análisis. En este artículo se presentan datos sobre depósitos de secuencias genómicas completas del SARS-CoV-2 en la base de datos de secuenciación GISAID, la Iniciativa mundial para intercambiar todos los datos sobre la gripe aviar, realizadas entre enero del 2021 y el 31 de mayo del 2022. Se creó el perfil de distribución de las variantes del SARS-CoV-2 en América del Sur, en el que se destacaron la contribución y la influencia de cada variante a lo largo del tiempo. El monitoreo de las secuencias genómicas en GISAID ilustra la negligencia en el seguimiento de los pacientes infectados en América del Sur, así como las discrepancias entre el número de genomas completos depositados a lo largo de la pandemia por parte de los países desarrollados y los países en desarrollo. Mientras que Europa y América del Norte han depositado más de 9 millones de genomas en GISAID, África y América del Sur han aportado menos de 400 000 secuencias genómicas. La vigilancia genómica es importante para detectar los primeros signos de alerta de virus nuevos en circulación, ayudar en el descubrimiento de nuevas variantes y controlar las pandemias.


RESUMO Após 2 anos da pandemia de covid-19, os protocolos usados para controlar a infecção necessitam maior atenção e análise. Apresentamos dados sobre as sequências genômicas completas do SARS-CoV-2 depositadas no banco de dados do a iniciativa internacional para o intercâmbio de dados sobre os vírus da influenza (GISAID) entre janeiro de 2021 e 31 de maio de 2022. Construímos o perfil de distribuição das variantes do SARS-CoV-2 na América do Sul, destacando a contribuição e a influência de cada variante ao longo do tempo. O monitoramento das sequências genômicas do GISAID ilustra a negligência no acompanhamento de pacientes infectados na América do Sul e as discrepâncias entre os países desenvolvidos e em desenvolvimento com relação ao número de genomas completos depositados ao longo da pandemia. Enquanto a Europa e a América do Norte respondem por mais de 9 milhões dos genomas depositados no GISAID, a África e a América do Sul depositaram menos de 400 000 sequências genômicas. A vigilância genômica é importante para detectar sinais de alerta precoces de novos vírus circulantes, auxiliar na descoberta de novas variantes e controlar pandemias.

The SARS-CoV-2 virus infection led to millions of deaths during the COVID-19 pandemic. Hundreds of workers from several other Brazilian cities, as well as from other countries, arrive daily in Macaé to work in the oil supply chain, making this city a putative hotspot for the introduction of new viral lineages. In this study, we performed a genomic survey of SARS-CoV-2 samples from Macaé during the first outbreak of COVID-19, combined with clinical data and a molecular integrative analysis. First, phylogenomic analyses showed a high occurrence of viral introduction events and the establishment of local transmissions in Macaé, including the ingression and spread of the B.1.1.28 lineage in the municipality from June to August 2020. Second, SARS-CoV-2 mutations were identified in patients with distinct levels of COVID-19 severity. Third, molecular interactions of the mutated spike protein from three B.1.1.33 local samples and human ACE2 showed higher interactions than that of the wild-type spike protein from the ancestral virus. Altogether, these results elucidate the SARS-CoV-2 genomic profile in a strategic Brazilian city and further explore the functional aspects of SARS-CoV-2 with a characterization of emerging viral mutations associated with clinical data and the potential targets for drug development against SARS-CoV-2.

We present a case report of a fetal diagnosis of cystic fibrosis after ultrasound abnormalities. After delivery, a type 3A intestinal atresia was diagnosed. Segmental enterectomy with end-to-end anastomosis was performed. This case report highlights the diagnosis complexity of a fetal intestinal atresia associated with cystic fibrosis.

Esta pesquisa teve como objetivo compreender as atitudes significativas de homens alcoolistas e suas expressões de sentimentos diante das interações familiares e comunitárias em tempos de pandemia da COVID-19. Trata-se de um estudo qualitativo, descritivo, do tipo exploratório, fundamentado no método da História Oral de Vida. Os participantes desta pesquisa foram cinco homens que vivenciam o alcoolismo, residentes em um município do estado da Bahia, acompanhados pelo Centro de Atenção Psicossocial álcool e outras drogas. Foi utilizada a entrevista semiestruturada para a coleta de informações, no período de março a abril de 2021, mediante videoconferências pela Plataforma Digital Google Meet. O conteúdo dos relatos orais foi analisado por meio da técnica de análise de conteúdo. Observou-se que as atitudes significativas do homem alcoolista foram impactadas pelos desdobramentos da pandemia, principalmente devido à adoção das medidas sanitárias, como o distanciamento social. Identificou-se ainda algumas expressões de sentimentos do homem alcoolista diante das interações familiares e comunitárias na pandemia, a exemplo de medo, desconfiança e insegurança afetiva, e exclusão social, além de expressões como fé, gratidão e esperança. Os participantes expressaram em suas falas atitudes de mudanças no convívio social e nos hábitos nesse período pandêmico, incluindo o uso de máscaras e álcool em gel além da diminuição no consumo do álcool para a maioria deles. Afirma-se então, que as interações simbólicas provenientes do cenário pandêmico promoveram atitudes de proteção que englobam o autocuidado e o cuidado coletivo, além de interações marcadas por conflitos familiares.


This study aimed to understand the meaningful attitudes of alcoholic men and their expressions of feelings in the face of family and community interactions in times of the COVID-19 pandemic. This is a qualitative, descriptive, exploratory study, based on the Oral History of Life method. The participants of this study were five men who experience alcoholism, living in a municipality in the state of Bahia, accompanied by the Psychosocial Care Center for alcohol and other drugs. A semi-structured interview was used to collect information, from March to April 2021, through videoconferences through the Google Meet Digital Platform. The content of the oral reports was analyzed using a content analysis technique. It was observed that the significant attitudes of the alcoholic man were impacted by the unfolding of the pandemic, mainly due to the adoption of sanitary measures, such as social distancing. Some expressions of feelings were also identified of these men facing family and community interactions during the pandemic, such as fear, distrust, and affective insecurity, and social exclusion, in addition to expressions such as faith, gratitude, and hope. The participants' discourses expressed attitudes of changes in social life and habits during this pandemic period, including the use of masks and gel alcohol in addition to the decrease in alcohol consumption for most of them. Therefore, the symbolic interactions arising from the pandemic scenario promoted protective attitudes that encompass self-care and collective care, in addition to interactions marked by family conflicts.

The Brazilian strategy to overcome the spread of COVID-19 has been particularly criticized due to the lack of a national coordinating effort and an appropriate testing program. Here, a successful approach to control the spread of COVID-19 transmission is described by the engagement of public (university and governance) and private sectors (hospitals and oil companies) in Macaé, state of Rio de Janeiro, Brazil, a city known as the National Oil Capital. In 2020 between the 17th and 38th epidemiological week, over two percent of the 206,728 citizens were subjected to symptom analysis and RT-qPCR testing by the Federal University of Rio de Janeiro, with positive individuals being notified up to 48 h after swab collection. Geocodification and spatial cluster analysis were used to limit COVID-19 spreading in Macaé. Within the first semester after the outbreak of COVID-19 in Brazil, Macaé recorded 1.8% of fatalities associated with COVID-19 up to the 38th epidemiological week, which was at least five times lower than the state capital (10.6%). Overall, considering the successful experience of this joint effort of private and public engagement in Macaé, our data suggest that the development of a similar strategy countrywise could have contributed to a better control of the COVID-19 spread in Brazil. Quarantine decree by the local administration, comprehensive molecular testing coupled to scientific analysis of COVID-19 spreading, prevented the catastrophic consequences of the pandemic as seen in other populous cities within the state of Rio de Janeiro and elsewhere in Brazil.

RESUMO

59-year-old women with permanent atrial fibrillation and previous haemorrhagic stroke was admitted for percutaneous left atrial appendage occlusion. Shortly after, AmplatzerTM device migration into the left ventricular outflow tract was confirmed. Unsuccessful transcatheter mobilization led to an emergent surgery for device retrieval and LAA closure. No events at 6-month follow-up.

OBJECTIVES: We aim to report our experience on venoarterial extracorporeal membrane oxygenation (VA-ECMO) for postcardiotomy shock (PCS). METHODS: Single center, retrospective study of all patients on VA-ECMO for PCS, from November 2006 to July 2019. Pediatric and adult patients were analysed separately. Primary outcomes were survival to discharge and one-year survival. RESULTS: Twenty-nine patients were included. Pediatric group (group PED) (62%, n=18): mean age 1,3±2,1 years and 39% male. Adults (group AD) (38%, n=11): mean age 55,6±15,9 years and 64% male. Indications in group PED were complex congenital heart surgery (94%) and heart transplant (6%), with 27% being reoperations; in group AD valvular surgery (45%), aortic surgery (21%), coronary artery bypass grafting (18%) and pulmonary endarterectomy (9%); 45% were reoperations. ECMO support was initiated intraoperatively due to failure to wean from cardiopulmonary bypass in 28% of group PED and 73% of group AD. Central cannulation was performed in all pediatric patients and 82% adults. Bleeding was the most common complication in both groups (group PED 39%, group AD 45%). Mean ECMO support time was respectively 6,2±4,9 and 6,2± 3,6 days for group PED and group AD. Weaning rate was 44% in group PED (with 2 patients bridged to LVAD) and 45% in group AD. Survival to discharge as well as one-year survival were both 28% in group PED and 18% in group AD. CONCLUSION: Despite low survival and high complication rates, VA ECMO support provides a survival benefit in refractory cases, with a dismal prognosis, that would otherwise die.

RESUMO

The endometrial stromal sarcoma (EES) is a rare uterine malignancy and its intracardiac metastasis are exceedingly rare. We report a case of a 53-year-old female patient diagnosed with a metastatic tumor of a ESS in the right side of the heart, who underwent successful surgical resection and initiated chemotherapy with docetaxel and gemcitabine. At a 9-month follow-up, the patient was in New York Heart Association-Class I, without any further complications.

OBJECTIVES: At Santa Marta Hospital, Ross Surgery was performed for the first time in 1999. Twenty years later, we feel it is desirable to evaluate the mid and long-term results of our experience, as well as estimate the future of this procedure. METHODS: Between March 1999 and June 2016, 23 Ross procedures were performed at our institution. We did a retrospective analysis of the patients´ data, results of the surgery, complications, freedom from reoperation and mortality. RESULTS: The majority (36,4%) of the patients had aortic stenosis, 22,7% had aortic regurgitation and 27,3% had aortic stenosis and regurgitation. Sub-valvular stenosis was present in 13,6% of patients. The mean follow-up is 15 years. The overall mortality was 9%, without early mortality. In our series, 83% of the patients are free from reoperation. Eighty percent (n=16) of the survivors are in NYHA class I, with the remaining 20% (n=4) in class II. CONCLUSION: Ross surgery has strict indications and in this group of patients the advantages are undeniable and the outcomes, according to our results (with 83% of patients free from reoperation, at a mean follow-up of 15 years), are positive and encouraging.

INTRODUCTION: Atrial fibrillation (AF) contributes to increased morbidity and mortality. Pharmacological and percutaneous catheter therapies are unsatisfactory, with potential serious adverse effects. Cox-Maze III/IV surgery, with higher rates of success, has not been widely adopted because of the associated complexity of the procedure. METHODS: We performed a retrospective analysis of the first patients submitted to surgical ablation of AF with occlusion of the left atrial appendage with a totally videothoracoscopic (VATS) approach in our institution. We describe the surgical technique and our results, including duration of surgery, hospital stay, complications and maintenance of sinus rhythm after surgery, at 6, 12 and 18 months of follow-up. RESULTS: We studied 15 patients (ages ranging from 39 to 75 years old; 54,5% female gender). Mean time since the diagnosis of AF was 5,75 years. All had been submitted to prior catheter ablation (mean of 2 attempts). Mean diameter and volume of the left atrium was 42 mm (M-mode) and 70 ml (43 ml/m2), respectively. Mean duration of surgery was 2 hours and 22 minutes. In one patient we had to convert the surgery to median sternotomy. Mean hospital stay was 4,8 days. Mean time of follow-up was 12 months. During follow-up, 91%, 90% and 80% of the patients were in sinus rhythm at 6, 12 and 18 months, respectively. CONCLUSION: This surgical approach represents a real benefit for those patients with multiple attempts of catheter ablation without success. However, a larger sample of patients with a longer period of follow-up is necessary for further conclusions.

OBJECTIVES: Jatene surgery or arterial switch is performed at our institution since the late nineties. We reviewed our results to identify the main causes of reoperation and, more importantly, to determine what variables predict the need for reoperation. METHODS: In this retrospective analysis were included all the 91 patients with d-TGA who underwent an arterial switch operation at our institution between 1995 and 2016. RESULTS: Mean follow-up was 10 years (range 5-25 years). Seventy-one percent of patients had simple TGA and 29% had complex TGA. The need of reoperation was 21% (n=19 patients). Right ventricle outflow tract obstruction was the main indication for reoperation (58%). The overall mortality was 9.9%. The gender (P= 0.8), diagnosis (simple or complex TGA) (P= 0,5) or the existence of palliative surgeries (P=0.9) were unable to predict the need for reoperation. The presence of anomalous coronary pattern was the only variable reaching statistical significance (P < 0.05), both in univariate and multivariate analysis. CONCLUSIONS: In our series, the main indication for reoperation after arterial switch operation was right ventricle outflow tract obstruction and the only predictive variable was the presence of anomalous coronary pattern.

OBJECTIVE: To investigate the association between periodontitis and self-reported halitosis among young adults, and whether there is an interaction between the effects of smoking and periodontitis on halitosis. METHODS: Data from the 1982 Pelotas birth cohort, Brazil, were used. The controlled direct effect of periodontitis on self-reported halitosis, not mediated by tongue coating, was estimated using marginal structural modeling. In addition, an interaction between the effects of smoking and periodontitis on halitosis was also tested. Confounders comprised sociodemographic information, obesity, diabetes, and oral hygiene habits. RESULTS: The controlled direct effect of periodontitis on halitosis not mediated by tongue coating showed that individuals with periodontitis had 90% higher risk of self-reporting halitosis (RR 1.90) compared to healthy individuals. Individuals with mild periodontitis had twice the risk of reporting halitosis than periodontally healthy individuals (RR 2.31). We also found an interaction between the effects of smoking and periodontitis on halitosis, as noted among smokers with mild (RR 2.91) and moderate-to-severe periodontitis (RR 5.84). CONCLUSION: There is a controlled direct effect of periodontitis on halitosis not mediated by tongue coating. Additionally, an interaction between the effects of smoking and periodontitis on halitosis was also detected.

BACKGROUND: To determine the effect of atrial pacing on the rate of post-operative atrial fibrillation (POAF) following coronary artery bypass grafting. METHODS: After a systematic literature search, randomized clinical trials (RCTs) comparing any combination of no pacing (NP), bi-atrial (BiA) pacing, left-atrial (LA) pacing and right-atrial (RA) pacing were included. Pairwise and network meta-analyses were performed using the generic inverse variance method. The primary outcome was POAF incidence. Secondary outcomes were postoperative bleeding, infection, and operative mortality. Leave-one-out and meta-regression were done. RESULTS: Fourteen RCTs were included with a total of 1727 patients. Compared with NP, any form of atrial pacing was significantly associated with lower incidence of POAF (odds ratio [OR]: 0.49; 95% confidence interval [CI]: 0.35-0.69). BiA pacing was associated with the larger risk reduction (OR: 0.36; 95% CI: 0.20-0.64 vs. 0.59; 95% CI: 0.34-1.02 for LA and 0.64; 95% CI: 0.38-1.07 for RA). Secondary outcomes were similar between the no pacing and pacing groups. On meta-regression, age and the use of continuous monitoring were associated with lower reduction of the incidence of POAF. In the network meta-analysis, BiA pacing ranked the best strategy for the prevention of POAF (OR: 0.34; 95% CI: 0.21-0.55). CONCLUSIONS: Compared to other pacing modalities, BiA pacing is associated with lower rates of POAF following CABG.

BACKGROUND: Conduits used in coronary artery bypass artery grafting (CABG) have different properties and flow profiles. We compared intraoperative mean graft flow (MGF) between arterial and venous conduits, off-pump CABG (OPCABG) and on-pump CABG (ONCABG) procedures, skeletonized and pedicled internal mammary artery (IMA) grafts, and pulsatility index (PI) between OPCABG and ONCABG, in pairwise meta-analyses. METHODS: Following a systematic literature search, all studies comparing MGF in arterial and venous grafts, were included. The primary endpoint was comparison of pooled MGF between arterial and venous grafts. Secondary endpoints were comparisons of pooled MGF in OPCABG vs ONCABG, anastomosed skeletonized vs pedicled IMA grafts, free skeletonized vs pedicled IMA grafts and PI in OPCABG versus ONCABG. RESULTS: A total of 25 studies with 4443 patients were included. Compared with venous grafts, arterial grafts had lower MGF (standardized mean difference [SMD], -0.28; 95% confidence interval [CI, -0.34; -0.22]; P < .001). OPCABG was associated with significantly lower MGF compared to ONCABG (SMD, -0.29; 95%CI, -0.50; -0.08]; P = .01). No differences were found in MGF between skeletonized vs pedicled IMA after anastomosis (SMD, 0.32; 95%CI [-0.08; 0.71]; P = .11) or in free flow (SMD, 0.76; 95%CI [-0.14; 1.65]; P = .10). No difference was found in PI between OPCABG and ONCABG. At meta-regression, age was associated with higher MGF, while OPCABG was associated with lower MGF. CONCLUSIONS: Intraoperative flow of venous conduits is higher than that of arterial grafts. Compared to OPCABG surgery, graft flow is higher in ONCABG. In skeletonized and pedicled IMA conduits, no difference in flow profiles was found.

Objetivo: traçar o padrão de consumo de álcool entre trabalhadores de um colégio técnico-agrícola de uma cidade nordestina brasileira. Método: pesquisa quantitativa, transversal e descritiva com 30 trabalhadores, com amostragem censitária. Elencou-se como critérios de elegibilidade: ser servidor público concursado na instituição e com vínculo empregatício de, pelo menos, seis meses. A coleta ocorreu por meio da aplicação do Teste para Identificação de Problemas relacionados ao Uso de Álcool e os dados foram analisados por meio de estatística descritiva. Resultados: verificou-se que 73% dos trabalhadores consumiram álcool nos últimos 12 meses; que o maior padrão de consumo de risco e nocivo ocorreu entre homens, com idades entre 30 e 39 anos, casados, com pósgraduação completa, com renda entre três e cinco salários mínimos. Conclusões: o padrão de consumo de álcool identificado foi de que os participantes consumiram bebidas alcoólicas nos últimos 12 meses com padrão de risco e nocivo.


Objective: to draw the pattern of alcohol consumption among workers of a technical-agricultural college in a Brazilian northeastern city. Method: a quantitative, transversal and descriptive research with 30 workers, with census sampling. It was listed as eligibility criteria: to be a public servant who has been a bankrupt in the institution and who has been employed for at least six months. The collection took through the application of the Test for Identification of Problems related to the Use of Alcohol and the data were analyzed by means of descriptive statistics. Results: it was found that 73% of the workers have used alcohol in the last 12 months, above the national average. That the highest pattern of risk and harmful consumption occurred among men, aged between 30 and 39 years, married, with a full postgraduate studies, with income between three and five minimum wages. Conclusion: the pattern of alcohol consumption identified was that the participants consumed alcoholic beverages in the last 12 months with a risk and harmful pattern

Método: se realizó una investigación cuantitativa, transversal y descriptiva con 30 trabajadores, con muestreo censal. Se definió como criterios de elegibilidad: ser servidor público concursado en la institución y con vínculo laboral de al menos seis meses. La recolección ocurrió mediante la aplicación de la prueba para Identificación de Problemas relacionados con el consumo de alcohol y los datos fueron analizados mediante estadística descriptiva. Resultados: se encontró que el 73% de los trabajadores han usado alcohol en los últimos 12 meses, por encima de la media nacional; el mayor patrón de consumo de riesgo y perjudicial ocurrió entre los hombres, de edades comprendidas entre 30 y 39 años de edad, casados, con estudios de postgrado completo, con ingreso mensual entre tres y cinco salarios mínimos. Conclusión: el patrón de consumo de alcohol identificado fue que los participantes consumieron bebidas alcohólicas en los últimos 12 meses con un patrón de riesgo y perjudicial.

Dengue is an important mosquito-borne disease and a global public health problem. The disease is caused by dengue virus (DENV), which is a member of the Flaviviridae family and contains a positive single-stranded RNA genome that encodes a single precursor polyprotein that is further cleaved into structural and non-structural proteins. Among these proteins, the non-structural 3 (NS3) protein is very important because it forms a non-covalent complex with the NS2B cofactor, thereby forming the functional viral protease. NS3 also contains a C-terminal ATPase/helicase domain that is essential for RNA replication. Here, we identified 47 NS3-interacting partners using the yeast two-hybrid system. Among those partners, we highlight several proteins involved in host energy metabolism, such as apolipoprotein H, aldolase B, cytochrome C oxidase and glyceraldehyde-3-phosphate dehydrogenase (GAPDH). GAPDH directly binds full-length NS3 and its isolated helicase and protease domains. Moreover, we observed an intense colocalization between the GAPDH and NS3 proteins in DENV2-infected Huh7.5.1 cells, in NS3-transfected BHK-21 cells and in hepatic tissue from a fatal dengue case. Taken together, these results suggest that the human GAPDH-DENV NS3 interaction is involved in hepatic metabolic alterations, which may contribute to the appearance of steatosis in dengue-infected patients. The interaction between GAPDH and full-length NS3 or its helicase domain in vitro as well as in NS3-transfected cells resulted in decreased GAPDH glycolytic activity. Reduced GAPDH glycolytic activity may lead to the accumulation of metabolic intermediates, shifting metabolism to alternative, non-glycolytic pathways. This report is the first to identify the interaction of the DENV2 NS3 protein with the GAPDH protein and to demonstrate that this interaction may play an important role in the molecular mechanism that triggers hepatic alterations.
