RESUMO

Immunosenescence is a phenomenon caused by changes in the immune system, and part of these changes involves an increase in circulating immunological biomarkers, a process known as "Inflammaging." Inflammaging can be associated with many diseases related to older people. As the older population continues to grow, understanding changes in the immune system becomes essential. While prior studies assessing these alterations have been conducted in countries with Caucasian populations, this investigation marks a pioneering effort. The object of the study is to describe for the first time that the distribution of cytokines, chemokines, and growth factors serum levels, assessed by Luminex platform, has been examined in a Brazilian population-based study of older adult females and males by age. Blood samples from 2111 participants (≥50 years old) were analyzed at the baseline (2015/2016) of the ELSI-Brazil study. The exploratory variables considered in the study were age, sex, educational level, residence area, geographic region, alcohol and smoking consumption, physical activity, and self-reported medical diagnoses of hypertension, diabetes, asthma, arthritis, and cancer. The association between serum biomarker levels and age was assessed by a quantile regression model adjusted in the total population and stratified by sex. The significance level considered in the analysis was 0.05. The mean age of the participants was 62.9 years, with a slight majority of female (52.7 %). Differences were found between the sexes in the median circulating levels of the CCL11, CXCL10, and FGF biomarkers. Eight biomarkers showed significant associations with age, including the pro-inflammatory CXCL10, TNF-α, IL-6, IL-17, and IL-2; and type 2/regulatory CCL11 and IL-4, showing positive associations, and anti-inflammatory IL-1Ra showing a negative association. The results suggest similar associations between the sexes, revealing an inflammatory profile characterized by types 1 and 2. Remarkably, these findings reinforce the concept of the Inflammaging process in Brazilian population. These findings add novel insights to about the immunosenescence aspects in middle-income countries and help define biomarkers capable of monitoring inflammation in older adults.

Objetivos: Avaliar, por meio de uma revisão de literatura, a eficácia da associação da pentoxifilina e tocoferol (PENTO) para o tratamento da MRONJ. Revisão de literatura: Trata-se de uma revisão de literatura que foi realizada em maio de 2024 nas bases de dados PubMed (U.S. National Library of Medicine) e BVS (Biblioteca Virtual em Saúde), com lapso temporal de 10 anos. A estratégia de busca foi formulada pela conjugação dos descritores indexados no DeCS/MeSH "Pentoxifylline", "Tocopherols" e "Bisphosphonate-Associated Osteonecrosis of the Jaw", combinados ao operador booleano "AND", e adaptados de acordo com as especificidades de cada base de dados. Os critérios de seleção foram: estudos em humanos publicados nos últimos 10 anos, que atendessem à pergunta norteadora, com texto completo disponível em inglês, português ou espanhol, revisões de literatura, séries ou relatos de caso e estudos retrospectivos. A busca inicial resultou na identificação de 23 artigos. Destes, 06 artigos foram incluídos na pesquisa. Considerações finais: Os estudos demonstram que o mecanismo de ação do protocolo PENTO ainda não é totalmente esclarecido, mas parece estar relacionado à inibição da inflamação e diminuição da fibrose. Essa associação foi relatada como bem tolerada, com efeitos colaterais mínimos e não dispendiosos quando comparados com outras modalidades de tratamento não cirúrgico. O protocolo PENTO é eficaz no tratamento não invasivo da MRONJ. Mais estudos prospectivos são necessários na área.

ABSTRACT Objective: To analyze the spatiotemporal epidemiological dynamics of meningitis in Brazil, between 2010 and 2019. Methods: Descriptive ecological study with cases and deaths due to meningitis in Brazil (2010-2019) in the National Notifiable Diseases Information System (Sistema de Informações de Agravos de Notificação - SINAN). The following analyses were performed: (I) frequency analyses of cases and deaths, prevalence rates, mortality, lethality, Fisher's exact test, and chi-square test; (II) Prais-Winstein regression; and (III) Global, Local Moran's index, and Kernel density. Results: 182,126 cases of meningitis were reported in Brazil, of which 16,866 (9.26%) resulted in death, with prevalence rates of 9.03/100,000 inhabitants, mortality of 0.84/100,000 inhabitants, and lethality of 9.26%. There was a noted trend of decreasing prevalence rates (−9.5%, 95% confidence interval — 95%CI −13.92; −4.96, p<0.01) and mortality (−11.74%, 95%CI −13.92; −9.48, p<0.01), while lethality remained stable (−2.08%, 95%CI −4.9; 0.8; p<0.1941). The majority of cases were viral meningitis (45.7%), among 1-9 years old (32.2%), while the highest proportion of deaths was due to bacterial meningitis (68%), among 40-59 years old (26.3%). In the Moran and Kernel maps of prevalence and mortality rates, municipalities in the South, Southeast, and the capital of Pernambuco in the Northeast stood out with high rates; as for lethality, the North, Northeast, and Southeast coastal areas were highlighted. Conclusion: A decrease in meningitis cases and deaths was found in this study; however, the lethality rate was higher in areas with lower prevalence, emphasizing the need to enhance actions for identifying, monitoring, and providing health care for cases, as well as expanding vaccination coverage.


RESUMO Objetivo: Analisar a dinâmica epidemiológica espaçotemporal das meningites no Brasil, entre os anos de 2010 e 2019. Métodos: Estudo ecológico descritivo com os casos e óbitos por meningites no Brasil (2010-2019) no Sistema de Informações de Agravos de Notificação. Realizaram-se (I) análises de frequências dos casos e óbitos, taxas de prevalência, mortalidade, letalidade, testes de exato de Fisher e qui-quadrado; (II) regressão de Prais-Winsten; e (III) índice de Moran global, local e densidade de Kernel. Resultados: Notificaram-se 182.126 casos de meningites no Brasil, dos quais 16.866 (9,26%) evoluíram para óbito, com taxas de prevalência de 9,03/100.000/habitantes, mortalidade de 0,84/100.000/habitantes e letalidade de 9,26%. Destaca-se a tendência de decrescimento das taxas de prevalência (−9,5%, intervalo de confiança de 95% — IC95% −13,92; −4,96, p<0,01) e mortalidade (−11,74%, IC95% −13,92; −9,48, p<01,01), enquanto a letalidade se manteve estacionária (−2,08%, IC95% −4,9; 0,8; p<0,1941). A maioria dos casos foi de meningites virais (45,7%), entre 1 e 9 anos (32,2%), enquanto a maior parcela dos óbitos foi por meningites bacterianas (68%), entre 40 e 59 anos (26,3%). Nos mapas de Moran e Kernel das taxas de prevalência e mortalidade, destacaram-se com altas taxas os municípios do sul, sudeste e a capital de Pernambuco, no nordeste; já na letalidade, evidenciaram-se o norte, o nordeste e o litoral do sudeste. Conclusão: Encontrou-se decréscimo dos casos e óbitos por meningites neste estudo, entretanto a taxa de letalidade foi maior em áreas com menor prevalência, reforçando a necessidade do aprimoramento das ações de identificação, vigilância e assistência em saúde dos casos, bem como da ampliação da cobertura vacinal.

Resumo Este artigo teve como objetivo mapear os itinerários terapêuticos no cuidado em saúde em comunidades quilombolas rurais no norte de Minas Gerais, Brasil. Trata-se de um recorte de uma pesquisa qualitativa realizada em seis comunidades visitadas. Os dados foram produzidos por meio de 18 entrevistas individuais, analisados pelo referencial teórico-metodológico dos itinerários terapêuticos e organizados em três temas empíricos. As narrativas permitiram a compreensão dos percursos trilhados no cuidado em saúde pela população quilombola, a identificação dos componentes do subsistema popular (recursos naturais, o uso de chás e remédios caseiros), do subsistema familiar (transmissão de conhecimentos e herança cultural de cuidados), e do subsistema profissional (nível hospitalar, cuidados médicos, atenção primária e especializada). As dificuldades de acesso não decorrem apenas das distâncias geográficas, envolvem aspectos mais amplos da determinação social, como o racismo institucional, a baixa oferta de serviços, a necessidade de pagamento para deslocamentos e procedimentos médicos. Nesse sentido, fazem-se necessárias uma abordagem e intervenções das políticas públicas frente às desigualdades étnico-raciais, econômicas e de acesso aos serviços de cuidado em saúde.


Abstract This article aimed to map therapeutic itineraries in health care within rural Quilombola communities in the north of Minas Gerais, Brazil. This is a section of a qualitative research conducted in six visited communities. The data was collected through 18 individual interviews, analyzed using the theoretical-methodological framework of Therapeutic Itineraries, and organized into three empirical themes. The narratives allowed for understanding the paths taken in health care by the Quilombola population, identifying the components of the popular subsystem (natural resources, the use of teas and home remedies), the family subsystem (transmission of knowledge and cultural heritage of care), and the professional subsystem (hospital level, medical care, primary and specialized attention). The difficulties of access are not only due to geographical distances, but also broader aspects of social determination, such as institutional racism, low availability of services, the need for payment for transportation and medical procedures. In this sense, it is necessary to have an approach and interventions from public policies to address ethnic-racial, economic, and access inequalities in health care services.

RESUMO

Endothelium-derived nitric oxide (NO)-induced vasodilation is impaired in pregnancy hypertension. However, the role of perivascular adipose tissue (PVAT)-derived hydrogen sulfide (H2S), as an alternative for counteracting vascular dysfunction, is incompletely clear in hypertensive disorders of pregnancy. Therefore, PVAT-derived H2S-induced vasodilation was investigated in pregnancy hypertension-induced endothelial dysfunction. Non-pregnant (Non-Preg) and pregnant (Preg) rats were submitted (or not) to the deoxycorticosterone (DOCA)-salt protocol and assigned as follows (n = 10/group): Non-Preg, Non-Preg+DOCA, Preg, and Preg+DOCA groups. Systolic blood pressure (SBP), angiogenesis-related factors, determinant levels of H2S (PbS), NO (NOx), and oxidative stress (MDA) were assessed. Vascular changes were recorded in thoracic aortas with PVAT and endothelium (intact and removed layers). Vasorelaxation responses to the substrate (L-cysteine) for the H2S-producing enzyme cystathionine-γ-lyase (CSE) were examined in the absence and presence of CSE-inhibitor DL-propargylglycine (PAG) in thoracic aorta rings pre-incubated with cofactor for CSE (pyridoxal-5 phosphate: PLP) and pre-contracted with phenylephrine. Hypertension was only found in the Preg+DOCA group. Preg+DOCA rats showed angiogenic imbalances and increased levels of MDA. PbS, but not NOx, showed increased levels in the Preg+DOCA group. Pre-incubation with PLP and L-cysteine elevated determinants of H2S in PVAT and placentas of Preg-DOCA rats, whereas no changes were found in the aortas without PVAT. Aortas of Preg-DOCA rats showed that PVAT-derived H2S-dependent vasodilation was greater compared to endothelium-derived H2S, whereas PAG blocked these responses. PVAT-derived H2S endogenously stimulated with the amino acid L-cysteine may be an alternative to induce vasorelaxation in endothelial dysfunction related to pregnancy hypertension.

AIMS: Pregnancy hypertension-induced endothelial dysfunction associated with impairment of nitric oxide (NO) bioavailability and hemodynamic derangements is a challenging for urgent procedures requiring maternal anesthesia. The volatile anesthetic isoflurane has demonstrated NO-associated protective effects. However, this isoflurane-induced effect is still unclear in pregnancy hypertension. Therefore, the present study examined the potential protective effects of isoflurane anesthesia on endothelial dysfunction and hemodynamic changes induced by hypertensive pregnancy associated with fetal and placental growth restrictions. MATERIALS AND METHODS: Animals were distributed into four groups: normotensive pregnant rats (Preg), anesthetized pregnant rats (Preg+Iso), hypertensive pregnant rats (HTN-Preg), and anesthetized hypertensive pregnant rats (HTN-Preg+Iso). Systolic and diastolic pressures, mean arterial pressure (MAP), heart rate, fetal and placental weights, vascular contraction, endothelium-derived NO-dependent vasodilation, and NO levels were assessed. The vascular endothelial growth factor (VEGF) levels and endothelial NO synthase (eNOS) Serine (1177) phosphorylation (p-eNOS) expression were also examined. KEY FINDINGS: Isoflurane produced more expressive hypotensive effects in the HTN-Preg+Iso versus Preg+Iso group, with respective reductions in MAP by 50 ± 13 versus 25 ± 4 mmHg (P < 0.05). Also, HTN-Preg+Iso compared to the HTN-Preg group showed (respectively) preventions against the weight loss of the fetuses (4.0 ± 0.6 versus 2.8 ± 0.6 g, P < 0.05) and placentas (0.37 ± 0.06 versus 0.30 ± 0.06 mg, P < 0.05), hyper-reactive vasocontraction response (1.8 ± 0.4 versus 2.8 ± 0.6 g, P < 0.05), impaired endothelium-derived NO-dependent vasodilation (84 ± 8 versus 50 ± 17 %, P < 0.05), reduced VEGF levels (147 ± 46 versus 25 ± 13 pg/mL, P < 0.05), and decreased p-eNOS expression (0.24 ± 0.07 versus 0.09 ± 0.05 arbitrary units, P < 0.05). SIGNIFICANCE: Isoflurane anesthesia protects maternal endothelial function in pregnancy hypertension, and possibly endothelium-derived NO is involved.

ABSTRACT Objective: To analyze the spatial patterns of leprosy in Pernambuco from 2011 to 2021. Methods: This is an ecological epidemiological study, carried out with data from the Notifiable Diseases Information System, based on new cases of leprosy among inhabitants of Pernambuco, between 2011-2021. An empirical Bayesian analysis of local and spatial dependence was performed with the global and local Moran indices. Results: 25,008 new cases of leprosy were registered with an annual case detection rate in the general population of 16.51 cases/100,000 inhabitants — which is considered high. Among those younger than 15 years of age, there were 5.16 cases/100,000 inhabitants (high) and 0.89/100,000 inhabitants with degree II of physical disability (low); there were also many high-risk cases with an overall Moran index of 0.33 (p<0.001), active transmission (0.26; p<0.001), and subsequent diagnosis of the disease (0.12; p<0.006), as well as distribution in macro-region 1 and macro-region 4. Conclusion: There was a heterogeneous spatial distribution in the state, showing two overviews, the first being the presence of municipalities with high risk of disease transmission and the second with clusters of silent municipalities, reinforcing the character of leprosy neglect as a major public health problem. This study brings reflections for leprosy control actions, due to the identification of priority areas to combat this disease in Pernambuco.


RESUMO Objetivo: Analisar os padrões espaciais da hanseníase em Pernambuco no período de 2011 a 2021. Métodos: Trata-se de um estudo epidemiológico ecológico, realizado com dados do Sistema de Informações de Agravos de Notificação com base nos casos novos de hanseníase em residentes de Pernambuco, entre 2011-2021. Foi realizada uma análise bayesiana empírica local e de dependência espacial com os índices de Moran global e local. Resultados: Foram registrados 25.008 casos novos de hanseníase com coeficiente médio anual de detecção de casos novos na população geral de 16,51 casos/100 mil habitantes — classificado como alto. Na população menor de 15 anos, ocorreu 5,16 casos/100 mil habitantes (muito alto) e 0,89/100 mil habitantes com grau II de incapacidade física (baixo) com aglomerados de alto risco para ocorrência com índice global de Moran de 0,33 (p<0,001), transmissão ativa (0,26; p<0,001) e diagnóstico tardio da doença (0,12; p<0,006), localizados na macrorregião 1 e na macrorregião 4. Conclusão: Houve uma distribuição espacial heterogênea no estado, evidenciando dois panoramas, o primeiro, a presença de municípios com alto risco de transmissão da doença e, o segundo, aglomerados de municípios silenciosos reforçando o caráter de negligência da hanseníase como grande problema de saúde pública. Este estudo traz reflexões para ações de controle da hanseníase, dada a identificação de áreas prioritárias para o enfrentamento dess/a doença em Pernambuco.

About 25% of the patients with the translocation t(11;19)(q23;p13.3)/KMT2A-MLLT1 present three-way or more complex fusions, associated with a worse prognosis, suggesting that a particular mechanism creates functional KMT2A fusions for this condition. In this work, we show a cryptic three-way translocation t(9;11;19). Interestingly, long-distance inverse polymerase chain reaction sequencing revealed a KMT2A-MLLT1 and the yet unreported out-of-frame SEC16A-KMT2A fusion, associated with low SEC16A expression and KMT2A overexpression, in an infant with B-acute lymphoblastic leukemia presenting a poor prognosis. Our case illustrates the importance of molecular cytogenetic tests in selecting cases for further investigations, which could open perspectives regarding novel therapeutic approaches for poor prognosis childhood leukemias.

OBJECTIVE: To analyze the prevalence of abdominal obesity and associated factors in quilombola communities in Northern Minas Gerais, Brazil. METHODS: This was a cross-sectional study conducted in 2019 through structured interviews and waist circumference measurement; Poisson regression was used, separated by gender, to calculate prevalence ratios (PR) of abdominal obesity adjusted by independent variables and 95% confidence interval (95%CI). RESULTS: 56.6% (95% CI 50.9;62.0) of the observed quilombolas presented abdominal obesity; in the adjusted analysis, among men, there was an association of the outcome with age ≥60 years old (60-69 years old: PR=2.52 - CI95% 1.33; 4.75), not being a smoker (PR=1.73 - 95%CI 1.17;2.55) and reported arterial hypertension (PR=1.42 - 95%CI 1.11;1.80), while in women, it was associated with age ≥50 years old (50-59 years old: PR=1.25 - 95% CI 1.01;1.54), smoking cessation (PR=1.26 - 95% CI 1.00; 1.58), consumption of chicken with skin (PR=1.09 - 95% CI 1.00;1.19) and hypertension (PR=1.22 - 95% CI 1.11;1.36). CONCLUSION: There was high prevalence of abdominal obesity among quilombolas. It was higher in the elderly, smokers, former smokers and those with hypertension.

KMT2A gene rearrangements represent the most frequent group of abnormalities in childhood leukemia (~70% of cases), with over 120 rearrangements described. The investigation of KMT2A rearrangements is still a vast field to be explored. Several studies have been characterizing different outcomes and leukemogenic mechanisms, depending on the translocation partner gene involved in childhood KMT2A-r leukemias. Therefore, the detection of the translocation partner gene, including in the context of complex rearrangements, may help to better delineate the disease. Here, we describe clinical and molecular cytogenetic data of a new complex variant translocation, involving chromosomes 9, 11, and 14, presenting a KMT2A gene extra copy and rearrangements, in an infant with de novo mixed-phenotype acute leukemia.

This study aimed to assess the occurrence of arbuscular mycorrhizal fungi (AMF) in annatto (Bixa orellana L.) cultivars and their response to AMF inoculation using biometric parameters. The occurrence surveys were conducted in annatto fields in three municipalities from Pernambuco Forest Zone: Lagoa de Itaenga, Gloria de Goitá, and Vitoria de Santo Antão, and in four cultivars (Red Piave, Green Piave, Red Peruvian Paulista, and Green Peruvian Paulista). In a greenhouse, biometric parameters of annatto seedlings of Red Piave, Red Peruvian Paulista, Embrapa-36, and Embrapa-37 cultivars inoculated with AMF isolated from annatto fields. The Red Piave cultivar exhibited greater root colonization than the Green Peruvian Paulista in the Lagoa de Itaenga and Vitoria de Santo Antão municipalities. The cultivar Red Piave showed a more beneficial association with AMF in plants and soil than cultivar Green Peruvian Paulista did, in both Lagoa de Itaenga and Vitoria de Santo Antão. AMF inoculation was effective in promoting the growth of annatto plants, particularly those inoculants with S. heterogama and C. etunicatum.

Objetivo: Analisar a prevalência de obesidade abdominal e fatores associados em comunidades quilombolas do norte de Minas Gerais, Brasil. Métodos: Estudo transversal realizado em 2019, a partir de entrevistas estruturadas e mensuração do perímetro da cintura; empregou-se regressão de Poisson, separada por sexo, para calcular razões de prevalência (RP) da obesidade abdominal ajustadas pelas variáveis independentes e intervalo de confiança de 95% (IC95%). Resultados: 56,6% (IC95% 50,9;62,0) dos quilombolas observados apresentaram obesidade abdominal; na análise ajustada, entre homens, verificou-se associação do desfecho com idade de ≥;60 anos (60-69 anos: RP=2,52 - IC95% 1,33;4,75), não ser tabagista (RP=1,73 - IC95% 1,17;2,55) e referir hipertensão arterial (RP=1,42 - IC95% 1,11;1,80), enquanto, nas mulheres, associou-se com idade ≥50 anos (50-59 anos: RP=1,25 - IC95% 1,01;1,54), ex-tabagismo (RP=1,26 - IC95% 1,00;1,58), consumo de frango com pele (RP=1,09 - IC95% 1,00;1,19) e hipertensão (RP=1,22 - IC95% 1,11;1,36). Conclusão: Encontrou-se prevalência de obesidade abdominal alta entre os quilombolas, maior nos idosos, hipertensos, fumantes e ex-fumantes.


Objetivo: Analizar la prevalencia de obesidad abdominal y factores asociados en comunidades quilombolas del norte de Minas Gerais, Brasil. Métodos: Estudio transversal realizado en 2019 a partir de entrevistas y medición de la circunferencia de la cintura; se utilizó la regresión de Poisson para calcular las razones de prevalencia (RP) de la obesidad abdominal ajustadas por variables independientes y un intervalo de confianza del 95% (IC95%). Resultados: 56,6% (IC95% 50,9;62,0) tenía obesidad abdominal; entre los hombres hubo una asociación del resultado con la edad ≥60 años (60-69 años: RP=2,52 - IC95% 1,33;4,75), no fumadores (RP=1,73 - IC95% 1,17;2,55) e hipertensión (RP=1,42; IC95% 1,11;1,80); en las mujeres, se asoció con la edad ≥50 años (50-59 años: RP=1,25 - IC95% 1,01;1,54), exfumadoras (RP=1,26 - IC95% 1,00;1,58), consumo de pollo con piel (RP=1,09 - IC95% 1,00;1,19) e hipertensión (RP=1,22 - IC95% 1,11;1,36). Conclusión: La prevalencia de obesidad abdominal fue alta entre quilombolas, y fue mayor en ancianos, hipertensos, fumadores y exfumadores.


Objective: To analyze the prevalence of abdominal obesity and associated factors in quilombola communities in Northern Minas Gerais, Brazil. Methods: This was a cross-sectional study conducted in 2019 through structured interviews and waist circumference measurement; Poisson regression was used, separated by gender, to calculate prevalence ratios (PR) of abdominal obesity adjusted by independent variables and 95% confidence interval (95%CI). Results: 56.6% (95% CI 50.9;62.0) of the observed quilombolas presented abdominal obesity; in the adjusted analysis, among men, there was an association of the outcome with age ≥60 years old (60-69 years old: PR=2.52 - CI95% 1.33; 4.75), not being a smoker (PR=1.73 - 95%CI 1.17;2.55) and reported arterial hypertension (PR=1.42 - 95%CI 1.11;1.80), while in women, it was associated with age ≥50 years old (50-59 years old: PR=1.25 - 95% CI 1.01;1.54), smoking cessation (PR=1.26 - 95% CI 1.00; 1.58), consumption of chicken with skin (PR=1.09 - 95% CI 1.00;1.19) and hypertension (PR=1.22 - 95% CI 1.11;1.36). Conclusion: There was high prevalence of abdominal obesity among quilombolas. It was higher in the elderly, smokers, former smokers and those with hypertension.

Abstract Frailty is characterized as a set of factors related to the body structure that lead the subject to a process of physical vulnerability, increasing their dependence. The study aims to investigate the aggregation of factors related to physical frailty (PF) in elderly residents of a city with a low Human Development Index (HDI). This is a cross-sectional study carried out in the city of Ibicuí, state of Bahia, Brazil, and including a random 270 elderly people aged ≥ 60 years. The physical frailty condition was identified according to the criteria proposed by Fried and collaborators. In the data analysis, descriptive statistics, cluster analysis, and multinominal logistic regression procedures were used. The highest prevalence of aggregation was identified when the four risk factors were combined: weight loss, strength, walking speed and physical activity levels (O/E = 4.36; CI = 4.04 - 4.68). It was identified that older people (80 years old or more) with a lower level of education (unlettered) were more likely to have three or more risk factors for physical frailty (p <0.05). As for sociodemographic variables, those who were older and had lower levels of education were more likely to have three or more risk factors. The development of actions that encourage a healthier lifestyle to favor the prevention and treatment of physical frailty, as well as to increase health literacy and knowledge, may reduce the problems related to this condition in older adults, mainly thinking about the next generations.


Resumo A fragilidade é caracterizada como um conjunto de fatores relacionados à estrutura corporal que levam o sujeito a um processo de vulnerabilidade física, aumentando sua dependência. O estudo tem como objetivo investigar a agregação de fatores relacionados à fragilidade física (FP) em idosos residentes em um município com baixo Índice de Desenvolvimento Humano (IDH). Trata-se de um estudo transversal realizado na cidade de Ibicuí, estado da Bahia, Brasil, e incluiu aleatoriamente 270 idosos com idade ≥ 60 anos. A condição de fragilidade física foi identificada de acordo com os critérios propostos por Fried e colaboradores. Na análise dos dados, foram utilizadas estatísticas descritivas, análise de cluster e procedimentos de regressão logística multinominal. A maior prevalência de agregação foi identificada quando os quatro fatores de risco foram combinados: perda de peso, força, velocidade de caminhada e níveis de atividade física (O/E = 4,36; IC = 4,04 - 4,68). Identificou-se que idosos (80 anos ou mais) com menor escolaridade (analfabetos) apresentaram maior probabilidade de apresentar três ou mais fatores de risco para fragilidade física (p <0,05). Quanto às variáveis ​​sociodemográficas, aqueles que eram mais velhos e com menor escolaridade tinham maior chance de apresentar três ou mais fatores de risco. O desenvolvimento de ações que estimulem um estilo de vida mais saudável para favorecer a prevenção e o tratamento da fragilidade física, bem como aumentar a alfabetização e o conhecimento em saúde, pode reduzir os problemas relacionados a essa condição nos idosos, principalmente pensando nas próximas gerações.

Patients with childhood acute myeloid leukemia (AML) with complex karyotypes (CKs) have a dismal outcome. However, for patients with a KMT2A rearrangement (KMT2A-r), the prognosis appears to depend on the fusion partner gene rather than the karyotype structure. Thus, a precise characterization of KMT2A-r and the fusion partner genes, especially in CKs, is of interest for managing AML. We describe the clinical and molecular features of a child who presented with a large abdominal mass, AML, and a new CK, involving chromosomes 11, 16, and 19 leading to a KMT2A-MLLT1 fusion and 2 extra copies of the ELL gene, thus resulting in the concurrent overexpression of MLLT1 and ELL. Molecular cytogenetic studies defined the karyotype as 47,XY,der(11)t(11;16)(q23.3;p11.2),der(16)t(16;19)(p11.2;p13.3),der(19)t(11;19)(q23.3;p13.3),+der(19)t(16;19)(16pter→p11.2::19p13.3→19q11::19p11→19p13.3::16p11.2→16pter). Array CGH revealed a gain of 30.5 Mb in the 16p13.3p11.2 region and a gain of 18.1 Mb in the 19p13.3p12 region. LDI-PCR demonstrated the KMT2A-MLLT1 fusion. Reverse sequence analysis showed that the MLLT1 gene was fused to the 16p11.2 region. RT-qPCR quantification revealed that ELL and MLLT1 were overexpressed (4- and 10-fold, respectively). In summary, this is a pediatric case of AML presenting a novel complex t(11;16;19) variant with overexpression of ELL and MLLT1.

Objective: was to evaluate the influence of mentoplasty on esthetic perception of dental surgeons, dental students and lay persons. In addition, to determine whether the outstanding chin affects the perceived desire for orthognathic surgery. Material and Methods: An image of an ideal silhouette was created. This image was manipulated to create another six images with different degrees of retrogenia and progenia in increments, advancing and receding the chin on a scale of 5 mm (from +15 to -15 mm) in the anteroposterior and vertical directions. Once the images were obtained, a questionnaire was created, which was evaluated by lay persons, dental surgeons and dental students (n = 50). The evaluators used a visual analog scale to analyze the images, establishing a score ranging from 0 to 10. Associated with the scale, the evaluators were asked whether they wished to have some surgical correction performed if the image in question were a representation of their own facial profile. Results: demonstrated that image H (0 mm) was ranked as the most attractive by the dental surgeons, while the dental students and lay persons scored B (-5 mm) as the best image. In general, images A (+15 mm) and D (-15 mm) were those that most induced a desire for corrective surgery. Conclusion: the prognathic and retrognathic facial profiles were unpleasant and perceptible to the dentist, dental student and lay person. The more projected and retracted the chin was, the greater was the desire to have orthognathic surgery performed. (AU)

Associado a escala os avaliadores foram questionados se eles teriam vontade de realizar alguma correção cirúrgica caso aquela imagem representasse o seu próprio perfil facial. Resultados: demostraram que a imagem H (0 mm) foi ranqueada como a mais atratativa pelos cirurgiões dentistas, enquanto os estudantes de odontologia e pessoas leigas pontuaram a B (-5 mm) como a melhor. De forma geral, as imagens A (+15 mm) e D (-15 mm) foram as que mais induziram o desejo por cirurgia corretiva. Conclusão: o perfil facial prognata e retrognata é desagradável e perceptível para o dentista, o estudante de odontologia e o leigo. Quanto mais projetado e retruido for o mento maior o desejo pela realização de cirurgia ortognática. (AU)

Introdução: O crescimento da população idosa aumentou a incidência de doenças crônicas não transmissíveis, como por exemplo, a hipertensão arterial. Objetivo: Verificar o efeito da atividade física sobre a reatividade vascular em idosas hipertensas. Métodos: A coleta de dados foi realizada de outubro a novembro de 2016. Foi efetuado um programa de exercícios de treino de força e aeróbico realizados durante 4 semanas, com duração de 40 minutos. Após os exercícios realizou-se o Cold Pressor Test (CPT), em seguida foi aferida a pressão arterial e a frequência cardíaca imediatamente após o estímulo e aos 5, 10, 15 e 20 minutos subsequentes. Resultados: Mostraram que ambas as intervenções são efetivas para uma menor resposta de reatividade vascular das idosas, quando submetidas ao estresse. Conclusão: Conclui-se que as intervenções são efetivas como um tratamento não farmacológico para a hipertensão arterial, além de promover uma diminuição significativa da pressão arterial pós CPT.


Introduction: The growth of the elderly population has increased the incidence of chronic non communicable diseases, such as hypertension. Objective: To verify the effect of physical activity on vascular reactivity in hypertensive elderly women. Methods: Data were collected from October to November 2016. A program of strength training and aerobic exercises was carried out during 4 weeks, with duration of 40 minutes. After the exercises, the Cold Pressor Test (CPT) was performed, then blood pressure and heart rate were measured immediately after the stimulus and at 5, 10, 15 and 20 minutes thereafter. Results: The results showed that both interventions are effective for a lower vascular reactivity response of the elderly, when submitted to stress. Conclusion: It is concluded that the interventions are effective as a non-pharmacological treatment for hypertension, in addition to promoting a significant decrease in AP (arterial pressure) after Cold Pressor Test.

BACKGROUND: Myelodysplastic syndrome (MDS) is rare in the pediatric age group and it may be associated with inheritable bone marrow failure (BMF) such as Fanconi anemia (FA). FA is a rare multi-system genetic disorder, characterized by congenital malformations and progressive BMF. Patients with FA usually present chromosomal aberrations when evolving to MDS or acute myeloid leukemia (AML). Thus, the cytogenetic studies in the bone marrow (BM) of these patients have an important role in the therapeutic decision, mainly in the indication for hematopoietic stem cell transplantation (HSCT). The most frequent chromosomal alterations in the BM of FA patients are gains of the chromosomal regions 1q and 3q, and partial or complete loss of chromosome 7. However, the significance and the predictive value of such clonal alterations, with respect to malignant progress, are not fully understood and data from molecular cytogenetic studies are very limited. CASE PRESENTATION: A five-year-old boy presented recurrent infections and persistent anemia. The BM biopsy revealed hypocellularity. G-banding was performed on BM cells and showed a normal karyotype. The physical examination showed to be characteristic of FA, being the diagnosis confirmed by DEB test. Five years later, even with supportive treatment, the patient presented severe hypocellularity and BM evolution revealing megakaryocyte dysplasia, intense dyserythropoiesis, and 11% myeloblasts. G-banded analysis showed an abnormal karyotype involving a der(9)t(9;11)(p24;q?22). The FISH analysis showed the monoallelic loss of ATM and KMT2A genes. At this moment the diagnosis was MDS, refractory anemia with excess of blasts (RAEB). Allogeneic HSCT was indicated early in the diagnosis, but no donor was found. Decitabine treatment was initiated and well tolerated, although progression to AML occurred 3 months later. Chemotherapy induction was initiated, but there was no response. The patient died due to disease progression and infection complications. CONCLUSIONS: Molecular cytogenetic analysis showed a yet unreported der(9)t(9;11)(p24;q?22),der(11)t(9;11)(p24;q?22) during the evolution from FA to MDS/AML. The FISH technique was important allowing the identification at the molecular level of the monoallelic deletion involving the KMT2A and ATM genes. Our results suggest that this chromosomal alteration conferred a poor prognosis, being associated with a rapid leukemic transformation and a poor treatment response.

As atividades exercidas pelas figuras parentais, que visam a sobrevivência e o desenvolvimento dos filhos, sofrem interferência de diferentes determinantes, entre eles a personalidade parental. Investigar essa relação pode contribuir com informações sobre as características individuais mais propensas a fornecer os cuidados e aquelas associadas a fatores de risco. O objetivo desse artigo foi revisar os resultados de estudos sobre personalidade parental e parentalidade. A busca foi realizada nas bases de dados Capes Periódicos, PubMed e BVS-Psi com os descritores "parenting" e "parental personality. Foram encontrados 137 estudos e 18 satisfizeram os critérios de inclusão. Encontrou-se apenas estudos internacionais e verificou-se que há uma tendência de os estudos utilizarem o termo "parental personality" sem diferenciar nos resultados os aspectos relacionados ao pai e à mãe. O modelo teórico predominante foi o Cinco grandes fatores da personalidade. As dimensões da personalidade extroversão e amabilidade se relacionaram à uma parentalidade calorosa e responsiva; o neuroticismo à rigidez ou permissividade; a abertura à experiência à flexibilidade; e a conscienciosidade à imposição de limites na educação. Concluiu-se que as características de personalidade de pais e mães podem ressoar em diferentes comportamentos em relação à parentalidade. Recomenda-se para pesquisas futuras a investigação de aspectos relacionados apenas ao pai e à mãe, que diferenciem o tipo de interação pais-filhos em relação ao gênero da criança.(AU)

Investigar esa relación puede contribuir con informaciones sobre las características individuales más propensas a fortalecer los cuidados y aquellas asociadas a los factores de riesgo. El objetivo de este artículo fue examinar los resultados de estudios sobre personalidad parental y parentalidad. La búsqueda fue realizada en las bases de datos Capes Periódicos, PubMed e BVS-Psi con los descriptores "parenting" e "parental personality". Fueron encontrados 137 estudios y apenas 18 satisficieron los criterios de inclusión. Se encontraron apenas estudios internacionales e se corroboró que hay una tendencia a usar la categoría "parental personality" sin que los resultados diferencien los aspectos relacionados con las madres o los padres. El modelo teórico predominante fue el de los cinco grandes factores de la personalidad. Las dimensiones de la personalidad extroversión y amabilidad se relacionaron con una parentalidad cálida y responsable; el neuroticismo a la rigidez o permisividad; la apertura a la experiencia y a la flexibilidad; y la responsabilidad a la imposición de límites en la educación. Se concluyó que las características de personalidad de padres y madres pueden repercutir en diferentes comportamientos en relación con la parentalidad. Se recomienda a investigaciones futuras el estudio de aspectos circunscritos al padre y a la madre, que distingan el tipo de interacción padres-hijos, de acuerdo con el género del niño.(AU)


The activities carried out by parental that aim at the survival and the development of the children suffer interference from different factors, between them the parental personality. Investigating this relationship may contribute with information about the individual characteristics that are most likely to provide care and those associated with risk factors. The objective of this article was to review the results of studies about parental personality and parenting. The search was performed in Capes Periódicos, PubMed and BVS-Psi databases with the keywords: "parenting" and "parental personality". Among the 137 studies found, 18 have met the inclusion criteria. It has been found only international papers and it was noted a trend to use the term "parental personality" was shown, without differentiating the aspects related to the father and the mother in the results. The predominant theoretical model was the Big Five Factor Model. The personality dimensions of extroversion and amability were related to warm and responsive parenting; neuroticism to stiffness or permissiveness; openness to experience to flexibility; and conscientiousness to the imposition of limits in education. It was concluded that the personality characteristics of fathers and mothers can have repercussions on different behaviors in relation to parenthood. It is recommended for future research the investigation of aspects related only to the father and the mother that differentiate the type of interaction parents-children with respect to the gender of the child.(AU)
