RESUMO

BACKGROUND Despite the current global trend of reduction in the morbidity and mortality of neglected diseases, dengue’s incidence has increased and occurrence areas have expanded. Dengue also persists as a scientific and technological challenge since there is no effective treatment, vaccine, vector control or public health intervention. Combining bibliometrics and social network analysis methods can support the mapping of dengue research and development (R&D) activities worldwide. OBJECTIVES The aim of this paper is to map the scientific scenario related to dengue research worldwide. METHODS We use scientific publication data from Web of Science Core Collection - articles indexed in Science Citation Index Expanded (SCI-EXPANDED) - and combine bibliometrics and social network analysis techniques to identify the most relevant journals, scientific references, research areas, countries and research organisations in the dengue scientific landscape. FINDINGS Our results show a significant increase of dengue publications over time; tropical medicine and virology as the most frequent research areas and biochemistry and molecular biology as the most central area in the network; USA and Brazil as the most productive countries; and Mahidol University and Fundação Oswaldo Cruz as the main research organisations and the Centres for Disease Control and Prevention as the most central organisation in the collaboration network. MAIN CONCLUSIONS Our findings can be used to strengthen a global knowledge platform guiding policy, planning and funding decisions as well as to providing directions to researchers and institutions. So that, by offering to the scientific community, policy makers and public health practitioners a mapping of the dengue scientific landscape, this paper has aimed to contribute to upcoming debates, decision-making and planning on dengue R&D and public health strategies worldwide.

RESUMO

Analisa as possibilidades de aplicação do modelo gerencial derivado do enfoque de planejamento estratégico-situacional à gestão das organizaçoes de saúde. Analisa a experiência de mudança organizacional e implantação de um novo modelo gerencial da Fundação Santa Casa de Misericórdia do Pará. Os resultados revelam que a experiência imprimiu importantes mudanças na dinâmica organizacional, dentre as quais destacam-se: descentralização político-administrativa, introdução de uma prática de avaliação de desempenho institucional, gestão colegiada e baseada no processamento técnico-político de problemas e aumento da integração entre os setores administrativos e assistenciais. Revelam também que o enfoque estratégico situacional apresenta importantes contribuição para a gestão das organizaçoes de saúde, embora se revele insuficiente para enfrentar alguns problemas relacionados às especificidades destas organizaçoes.


The objective of this research is to analyse the perspectives of the aplication of the management model derived from the focus of the strategic-situational plan to the administration of the health organizations. Was developed trhough of the study-case, with the analysis of the experience of the organizational change and implementation of a new management model at the "Foundation Santa Casa de Misericórdia do Pará". The experience was able to make changes in the dynamic of the organization, wich we emphasize: political-administrative decentralization, the introduction of the avaliation of the institution performance, collegiate management, the political-tecnical processment of problems and the increase of the interaction between the administrative and assistance departments. The comparison between the management of Matus' model and of the Santa Casa shows important similarities and some diferences. Among the similarities we emphasize: the simultaneous modification of the key management systems, the emphasis on the decentralization, the rationalization of the director's agenda and the evaluation of the results. The Santa Casa model differs from the Matus' focus, first because it emphasizes the roles of the institutional actors, creating collective spaces of administration and internal negotiation foruns, with the objetive of decreasin the most important institutional conflicts. Second, for the constitution of model that tries to atend to the specificities and requirements of those organizations. The results of this research reveal that the focus's strategic-situational make important contributions to the health organizations, therefore, it is insuficient to face some of the problems related to these organizations. The greatest contributions to the Matus' model are: · the strategy of organizational change, through of the implantation of the key management systems; · The emphasis on the increase of institutional responsability with the introduction of a petition and service accoWlt system for the performance and based on the stablishment of comitments; · Emphasis on the decentralization of the political-administrative and the organizational flexibility; · The introduction of managerial dynamic based on processment of problem on all organizational levels; · The search for coherence between the basic organizational rules, that is, the direction, responsability and governability.
