RESUMO

OBJECTIVE: To evaluate changes in general and oral health-related quality of life (HRQoL) in patients with dentofacial deformity undergoing orthognathic surgery, and whether these changes vary according to type of deformity. MATERIAL AND METHODS: This is a prospective longitudinal multicenter study of patients with dentofacial deformities (n = 90). The Orthognathic Quality of Life Questionnaire (OQLQ), Oral Health Impact Profile (OHIP-14), and Short-Form Health Survey version2 (SF-36v2) were self-completed by patients before surgery, 3 and 6 months after orthognathic surgery. Change was tested using paired t-test, and compared between Class II and Class III of dentofacial deformity by unpaired t-test. The magnitude of change was examined estimating the standardized response mean (SRM). RESULTS: The OQLQ and OHIP-14 showed statistically significant improvements 6 months after surgery, compared with the pre-surgical evaluation, but the SF-36v2 only in the physical component summary. The SRM was large in OQLQ oral function (-1.11) and dentofacial facial aesthetics (-0.76) dimensions, and moderate in most of OHIP-14 dimensions. Differences in mean change between Class II and III were statistically significant for global scores of OQLQ (-10.08 vs -20.30, p = 0.0271) and OHIP-14 (-3.79 vs -10.56, p = 0.0144). CONCLUSIONS: A significant improvement was observed in oral HRQoL and in the physical component of general health in patients with dentofacial deformities Class II and III after orthognathic surgery. Improvement was greater among Class III than in Class II patients. CLINICAL RELEVANCE: These results provide patients, oral health care professionals, and planners with valuable information to make evidence-based decisions and facilitate shared clinical decision-making, taking into account the patients' perspective.

RESUMEN Introducción: La reconstrucción de las estructuras de la articulación temporomandibular (ATM) degenerada permanecen como un desafío clínico hasta el día de hoy, y su tratamiento se ha enfocado principalmente en el alivio del dolor y la eventual restauración de la función articular. Objetivo: Describir, mediante una revisión narrativa, las propiedades y características de diferentes células mesenquimales para el tratamiento regenerativo de patologías articulares. Material y método: Se realizó una búsqueda electrónica de la literatura en las bases de datos de PubMed, EBSCO, Web of Science, Scopus y The Cochrane Library a partir del año 2005, en idioma ingles y español, con énfasis en la regeneración articular de la rodilla y la ATM. Se excluyeron aquellos estudios donde la degeneración articular fuese producto de un trauma. Fueron evaluados dieciocho artículos. Resultados y discusión: Resaltan los diferentes protocolos utilizados con células madre mesenquimales derivadas de médula ósea, tejido adiposo y cordón umbilical humano, con resultados de regeneración articular en todos los casos a nivel radiográfico e histológico cuando fue posible. El tratamiento de las patologías articulares en la ATM ha sido poco descrito, con buenos resultados en reportes de caso y estudios observacionales.


ABSTRACT: Introduction: To date, temporomandibular joint (TMJ) reconstruction remains a clinical challenge, and its treatment has focused mainly on pain relief and the eventual restoration of joint function. Aim: To describe, through a narrative review, the properties and characteristics of different mesenchymal cells for the regenerative treatment of joint diseases. Material and method: An electronic search of the literature was carried out in the databases of PubMed, EBSCO, Web of Science, Scopus and The Cochrane Library since the year 2005, in English and Spanish, emphasizing joint regeneration of the knee and the TMJ. Studies where joint degeneration was the result of trauma were excluded. Eighteen articles were evaluated. Results and discussion: The reported literature highlights the different protocols used with mesenchymal stem cells derived from bone marrow, adipose tissue and human umbilical cord, with results of joint regeneration in all cases at radiographic and histological level when possible. The treatment of joint diseases in the TMJ has been poorly described, with good results in case reports and observational studies. Because of this, it is important to continue investigating this matter to establish clear treatment protocols in the temporomandibular joint.

RESUMEN: Objetivo: Describir los cambios de la pirámide nasal al realizar una osteotomía Le Fort I y el manejo cosmético complementario a la cirugía ortognática. Material y método: Se realizó una búsqueda electrónica en las bases de datos PubMed, EBSCO, Scopus, The Cochrane Library, y en el metabuscador Epistemonikos a través de las palabras clave "Le Fort 1", "Orthognathic Surgery", "Maxillary Surgery", "Nasolabial Changes", "Nasal Change", "Nasolabial Esthetics", "Soft Tissue", "Nasal Profile", "Nose" y "Cosmetic", relacionadas entre sí con los términos booleanos AND y OR. Resultados: se seleccionaron 25 artículos: 15 estudios observacionales, 5 revisiones narrativas, 3 ensayos clínicos y 2 notas técnicas. Conclusión: se establece cierto patrón que permitiría predecir cambios que sufrirá la nariz: aumento en la proyección nasal, disminución del largo columelar y ensanchamiento de la base alar son uno de ellos. En otros aspectos como el ángulo nasolabial o la posición final de la punta nasal no se ha logrado definir un patrón común. Para evitarlos se han descrito muchas técnicas. Sin embargo, falta mayor evidencia.


ABSTRACT: Objective: To describe the changes of the nasal pyramid when performing a Le Fort I osteotomy and its cosmetic management complementary to the orthognathic surgery. Materials and Methods: An electronic search was made in PubMed, EBSCO, Scopus, The Cochrane Library and Epistemonikos using the keywords "Le Fort 1", "orthognathic surgery", "maxillary surgery", "nasolabial changes", "nasal change", "nasolabial esthetics", "soft tissue", "nasal profile", "nose" and "cosmetic" related to each other with the Boolean terms AND and OR. Results: 25 articles were selected; 15 observational studies, 5 narrative reviews, 3 clinical trials and 2 technical notes. Conclusion: Nasal changes tend to follow a certain pattern that allows to predict them when facing surgery: increase of nose projection, decrease in columellar length and widening of the alar base, among others. As for other aspects such as the nasolabial angle or the final position of the nasal tip, it has not been possible to define a common pattern. To avoid them, several techniques have been described. However, more evidence is needed.

RESUMEN: Introducción: La anomalía esqueletal clase II posee un 16 a 22,5% de prevalencia mundial. Cuando estos pacientes se someten al avance mandibular mediante osteotomía sagital de rama mandibular pueden presentar un grado de inestabilidad postoperatoria evidenciándose como recidiva de éste. Objetivo: Describir la estabilidad del avance mandibular mediante osteotomía sagital bilateral de rama mandibular en pacientes clase II esqueletal. Material y método: Se realizó una búsqueda electrónica en las bases de datos PubMed, EBSCO, The Cochrane Library, Tripdatabase y Scopus mediante las palabras clave "mandibular stability", "skeletal stability", "mandibular advancement", "sagittal split osteotomy", "sagittal split ramus osteotomy", "class II", "class III" y "distraction osteogenesis", relacionadas entre sí con los términos booleanos AND, OR y NOT. También se incluyeron los términos MeSH "mandibular advancement" y "Sagittal Split Ramus Osteotomy". Paralelamente se realizó una búsqueda manual en las revistas AJODO, BJOMS, JOMS y EJO. Resultados y discusión: Se seleccionaron 29 artículos: 24 estudios observacionales analíticos, 2 revisiones sistemáticas y 3 ensayos clínicos aleatorizados. El avance mandibular mediante osteotomía sagital de rama mandibular es estable. No obstante, se debe tener en cuenta la existencia de múltiples factores pre e intraquirúrgicos que podrían generar recidiva del tratamiento.


ABSTRACT: Introduction: The class II skeletal anomaly has a 16-22,5% prevalence worldwide. When class II patients undergo mandibular advancement through Bilateral Sagittal Split Osteotomy (BSSO), they can present a postoperative instability, evidenced as a relapse. Objective: To describe the stability of mandibular advancement through BSSO in skeletal class II patients. Materials and method: An electronic search was performed in the databases PubMed, EBSCO, The Cochrane Library, Tripdatabase and Scopus using the keywords "mandibular stability", "skeletal stability", "mandibular advancement", "sagittal split osteotomy", "sagittal split ramus osteotomy", "class II", "class III" and "distraction osteogenesis", related to each other with the Boolean terms AND, OR and NOT. Also "mandibular advancement" and "Sagittal Split Ramus Osteotomy" MeSH terms were included. In parallel, a manual search in the journals AJODO, BJOMS, JOMS and EJO was performed. Results and discussion: 29 articles were selected: 24 analytic observational studies, 2 systematic reviews and 3 randomized clinical trials. Mandibular advancement through BSSO is stable. However, multiple pre and intraoperative factors that could cause a treatment relapse must be taken into account.

OBJECTIVE: The aim of this study was to develop a Spanish version of the Orthognathic Quality of Life Questionnaire (OQLQ) that is conceptually equivalent to the original questionnaire, as well as acceptable, reliable, valid, and responsive for use in Chilean patients with dentofacial deformities. MATERIAL AND METHODS: The recommended standard methodology, with direct and back-translation, was used. A prospective longitudinal multicenter study of adult patients diagnosed with dentofacial deformity was carried out, self-administrating OQLQ, OHIP-14 (Oral Health Impact Profile), and SF-36 (Short Form 36 Health Survey) during the presurgery visit in order to examine construct validity. To evaluate reproducibility, questionnaires were re-administered 4 weeks later to subjects with a stable dental condition. Responsiveness was assessed among subjects followed up until 3 months after surgery. RESULTS: Of the 230 patients under presurgical orthodontic treatment included in the study, 216 completed the questionnaire, 142 formed the reliability sub-sample, and 30 were evaluated 3 months after surgery. Cronbach's alpha ranged from 0.78 to 0.94 and test-retest intraclass correlation coefficients ranged from 0.84 to 0.91 (p = 0.001) by dimension. The correlation matrix between OQLQ dimensions and SF-36 and OHIP-14 confirmed most of the associations previously hypothesized as moderate (rs > 0.4). Confirmatory factor analysis supported the same structure as the original instrument, considering four dimensions. Responsiveness was demonstrated by the large improvement observed in the global score 3 months after surgery: mean change ±SD = -15.1 ± 18.05 and standard response mean = -0.84 (p < 0.001). CONCLUSIONS: The Spanish version of OQLQ has demonstrated good levels of reliability, validity, and responsiveness - similar to those of the original questionnaire.

La población chilena se caracteriza por ser una población muy heterogenea, debido a la gran mezcla exístente entre las distintas etnias y razas a lo largo del tiempo. Esto supone una gran diversidad de formas, tamaños y grosores de estructuras anatómicas, dentro de las cuales ubicamos a los grosores faciales. Si bien existen bases de datos de los grosores tisulares faciales en la población chilena, éstas han sido realizadas a través de técnicas como la ultrasonografía y punción, sin existir estudios realizados en Chile utilizando técnicas radiológicas para medir, como es la telerradiografía de perfil. Los objetivos fueron: 1. Medir los grosores tisulares a nivel de GB y NA, 2. Evaluar correlación de grosores según distribución por sexo y biotipo, 3. Comparar grosores entre distintos grupos, 4. Creación de una base de datos de referencia de la población chilena sobre el espesor del tejido blando a nivel de GB y Nay 5. Comparar los resultados obtenidos con otros estudios de grosores realizados en la población chilena. Se escogió al azar a 99 alumnos de odontología entre 20 a 27 años que necesitaban tratamiento de ortodoncia, a los cuales se les solicita una telerradiografía de perfil, en donde se mide los grosores de tejidos blandos a nivel de glabela y nasion, se clasifican a los pacientes según sexo y biotipo utilizando el análisis de Roth-Jaraback. Al analizar los resultados según sexo se observó siempre un grosor mayor en hombre que en mujer, con diferencia significativa a nivel de glabela y nasion. Al evaluar los resultados según biotipo no se observa diferencias siginificativas entre un biotipo y otro. Los hombres presentan un mayor grosor de tejido blando a nivel de glabela y nasion, con diferencia significativa en ambos. Se propone una nueva clasificación según biotipo, pero no se encuentra resultados concluyentes.


Chilean population is characterized for being heterogeneous, due to a large variety and mix of different ethnicities and races. This includes diversity in shapes, sizes and thickness of anatomic structures, part of which is facial thickness. Even though there are databases of facial tissue thickness for Chilean population, these have been carried out through techniques such as ultrasonography and puncture, without taking into consideration radiologic techniques, such as profile teleradiography. The aims of the study were: 1. Measure tissue thickness at GB and NA level, 2. Evaluate correlation in thickness according to distribution per sex and biotype. 3. Compare thickness among the groups. 4. Create a database of soft tissue thickness at GB and NA level in Chilean population, 5. Compare results with other studies regarding tissue thickness in Chilean population. Ninety-nine dentistry students between 20 and 27 years of age, candidates for orthodontic treatment, were randomly chosen. Lateral teleradiography was requested from all individuals, soft tissue thickness was measured at GB and NA level, classifying patients according to Roth-Jaraback analysis. When analysing the results according to sex, larger thickness was observed in men each time, with a significant difference at GB and NA levels. When evaluating biotype differences no significant difference was observed. Men showed thicker soft tissue at NA and GB level than women, with a significant difference between both. A new classification is proposed according to biotype, but no concluding data is found.

Una de las principales complicaciones al realizar la osteotomía sagital de rama, es la presencia de alteraciones sensitivas que se producen a nivel del nervio alveolar inferior. La parestesia luego de esta técnica reporta una incidencia entre el 85-87 por ciento, de los cuales entre el 0-24 por ciento queda con parestesia permanente. Dentro de los tratamientos utilizados para recuperar las alteraciones sensitivas se encuentra el uso de láser de baja frecuencia; el cual se utiliza para acelerar la recuperación sensitiva y la reparación neuronal, disminuir el dolor y restaurar el funcionamiento normal del nervio injuriado. Los objetivos de esta investigación fueron realizar una revisión bibliográfica sobre los resultados obtenidos en pacientes sometidos a osteotomía sagital de rama que presentaron alteraciones sensitivas y en cuyo tratamiento se utilizó el láser, y crear un protocolo de tratamiento con láser en pacientes sometidos a osteotomía sagital de rama madibular. Se realizó una revisión de artículos entre los años 1990-2010, utilizando buscadores como: EBSCO, Cochcrane, TripDataBase, Medline, Lilacs, Pubmed y Decs, además de una búsqueda en revistas científicas. Los resultados de los artículos seleccionados señalan el efecto beneficioso que conlleva el uso del láser, demostrando su eficacia en la recuperación precoz de la sensibilidad postoperatoria. El uso del láser de baja frecuencia es una alternativa favorable para pacientes con alteraciones sensitivas que fueron sometidos a cirugía sagital de rama, debido a su pronta, mejor y mayor recuperación de la sensibilidad...


A major complication that occurs at the sagittal ramus osteotomy, is the presence of damage occurring at the inferior alveolar nerve. An incidence of paresthesia after this technique reported at 85-87 percent, of which between 0-24 percent are left with permanent paresthesia. Among the treatments used to restore sensitivity is the use of low level laser therapy, which is used to speed recovery and repair sensitive neurons, reduce pain, and restore normal functioning to injured nerve. The aims were to undertake a literature review of empiric research on the results obtained in patients under going sagittal ramus osteotomy, who has sensory disturbances and in wich laser treatment was used, and create a laser treatment protocol in patients undergoing sagittal ramus osteotomy. A review of articles between the years 1990-2010, using search engines such as EBSCO, Cochcrane, Tripdatabase, Medline, Lilacs, Pubmed and Decs. The review also included a search in scientific journals. The results of the reviewed literature showed the beneficial effect of the use of laser, demonstrating its effectiveness in the early recovery of postoperative sensitivity. The use of low level laser therapy is a favorable alternative for patients with sensory impairment who underwent sagittal ramus osteotomy due to its rapid and greater sensitivity recovery...
