BACKGROUND: For elderly people, the benefit of minimally invasive cardiac surgery (MICS) is unclear, so we evaluated the safety, recovery, and long-term survival in elderly MICS patients.Methods and Results: 63 propensity score-matched pairs of 213 consecutive patients (≥70 years old) who underwent mitral and/or tricuspid valve surgery between 2010 and 2020 (121 right mini-thoracotomies vs. 92 full sternotomies) were compared. The primary outcome was safety (composite endpoint of in-hospital death or major complication). Secondary outcomes were early ambulation and discharge to home. There were no differences between the groups for in-hospital death (3.2% vs. 0.0%, P=0.157) and primary outcome (14.3% vs. 17.5%, P=0.617). The rate of early ambulation (73.0% vs. 55.6%, P=0.048) and discharge to home (66.7% vs. 49.2%, P=0.034) were significantly higher in the mini-thoracotomy group. Major complication was an independent negative predictor of early ambulation for mini-thoracotomy but not for a conservative approach. Survival was 87.8±4.4% vs. 86.8±4.7% at 5 years, which was not significantly different. CONCLUSIONS: Similar safety but better recovery were observed for mini-thoracotomy, and long-term survival was comparable between groups. Major complication was a negative predictor of early ambulation after mini-thoracotomy. Careful preoperative risk stratification would enhance the benefits of MICS in elderly patients.

RESUMO

The keys to successful repeat mitral valve repair are careful selection of eligible patients and the use of high-quality surgical techniques. Regarding patient selection, patients with soft leaflet tissue, redundant rough-zone areas, and posterior mitral leaflet regions are good candidates for repeat repair. Autologous pericardial patch reinforcement for sutured dehiscence line, triangular resection for residual excess tissue, and sophisticated neo-chord reconstruction are high-quality techniques that increase the durability of repeat repair.

BACKGROUND: Open surgery can be required or even fatal if incomplete deployment of stent graft (SG) occurs. We herein report the first case in which a snare was successfully used to perform endovascular therapeutic troubleshooting on the proximal portion of a Zenith Alpha thoracic endovascular graft proximal component that showed incomplete deployment. CASE PRESENTATION: The patient was an 80-year-old woman. She underwent thoracic endovascular aortic repair (TEVAR) for subacute phase Stanford type B ulcer-like projection aortic dissection. Although the ulcer-like projection disappeared, a follow-up computed tomography angiogram (CTA) obtained approximately 1 year postoperatively showed type Ia and Ib endoleaks. Since there is a high risk of rupture as the aneurysm diameter increases, we determined that an additional SG was indicated. An attempt was made to place the SG in Zone 3, but as the lesser curvature side of the proximal portion stopped in a position that was perpendicular to the vascular wall (downward facing), the SG proximal portion did not completely expand. A guiding sheath was inserted into the aortic arch via the left brachial artery, and, using a snare that we inserted via the femoral artery, we grasped the guiding sheath. A catheter and guidewire (GW) were inserted via the guiding sheath and then rotated under the lesser curvature of the SG proximal portion; the GW was then passed through the loop of the snare. This allowed us to insert the hard loop structure under the SG proximal portion, which in turn allowed successful repair of the incomplete deployment of the SG. Type Ia and Ib endoleaks remained but were less than those before additional TEVAR. One week later, she was discharged. One year later, CT showed no interval change in the size of aortic aneurysm with dissection, and she has been followed on an outpatient basis. CONCLUSIONS: When the endovascular diameter of the proximal aortic arch is large, incomplete deployment of the proximal portion of a Zenith Alpha thoracic endovascular graft can occur, but bailout is possible through the use of the snare technique as endovascular therapy.

A 72-year-old man presented with recurrent constrictive pericarditis, which developed 6 months after pericardiectomy, and pericardial substitution with an expanded polytetrafluoroethylene membrane. Re-pericardiectomy was performed. A new thick membranous structure had grown under the expanded polytetrafluoroethylene membrane anterior to the right ventricle, and was firmly adhered to the epicardium. This new structure exhibited collagenous fiber-based fibrotic thickening, and resembled a foreign body reaction. It was surmised that recurrence of constrictive pericarditis may have been induced by the expanded polytetrafluoroethylene membrane. Heart failure resolved after the operation; however, the patient died of respiratory failure on postoperative day 6.

The serine/threonine kinase Akt lies at a critical signaling node downstream of phosphatidylinositol-3-kinase and is important in promoting cell survival and inhibiting apoptosis. An Akt inhibitor may be particularly useful for cancers in which increased Akt signaling is associated with reduced sensitivity to cytotoxic agents or receptor tyrosine kinase inhibitors. We evaluated the effect of a novel allosteric Akt inhibitor, MK-2206, in combination with several anticancer agents. In vitro, MK-2206 synergistically inhibited cell proliferation of human cancer cell lines in combination with molecular targeted agents such as erlotinib (an epidermal growth factor receptor inhibitor) or lapatinib (a dual epidermal growth factor receptor/human epidermal growth factor receptor 2 inhibitor). Complementary inhibition of erlotinib-insensitive Akt phosphorylation by MK-2206 was one mechanism of synergism, and a synergistic effect was found even in erlotinib-insensitive cell lines. MK-2206 also showed synergistic responses in combination with cytotoxic agents such as topoisomerase inhibitors (doxorubicin, camptothecin), antimetabolites (gemcitabine, 5-fluorouracil), anti-microtubule agents (docetaxel), and DNA cross-linkers (carboplatin) in lung NCI-H460 or ovarian A2780 tumor cells. The synergy with docetaxel depended on the treatment sequence; a schedule of MK-2206 dosed before docetaxel was not effective. MK-2206 suppressed the Akt phosphorylation that is induced by carboplatin and gemcitabine. In vivo, MK-2206 in combination with these agents exerted significantly more potent tumor inhibitory activities than each agent in the monotherapy setting. These findings suggest that Akt inhibition may augment the efficacy of existing cancer therapeutics; thus, MK-2206 is a promising agent to treat cancer patients who receive these cytotoxic and/or molecular targeted agents.

We have identified and characterized the sequence and expression of two Group B Sox genes in the acorn worm, Ptychodera flava. One sequence represents a Group B1 Sox gene and is designated Pf-SoxB1; the other is a Group B2 Sox gene and is designated Pf-SoxB2. Both genes encode polypeptides with an HMG domain in the N-terminal half. Whole-mount in situ hybridization to embryonic and larval stages of P. flava shows that the two genes are expressed in rather similar patterns at these stages. Expression is first detected in the cells of the blastula and subsequently localizes to the ectoderm during gastrulation. As the mouth forms, expression becomes concentrated in the stomodeum region. During morphogenesis of the tornaria larva, expression in the stomodeal ectoderm remains prominent around the mouth and under the oral hood. Later the genes are prominently upregulated in the ciliary bands and the apical organ. These results provide additional evidence that genes playing essential roles in the formation of the chordate dorsal central nervous system function in the development of the ciliary bands and apical organ, neural structures of this non-chordate deuterostome larva.
