RESUMO

Pikamilone is an original drug of Russian produce having a central (vegetotropic and vascular) and peripheral (vasodilating) mechanisms of action. The drug was tried as monotherapy in elderly patients with benign prostatic hyperplasia who had symptoms of the lower urinary tracts to estimate its effect on local and general mechanisms of nervous and vascular regulation of detruzor function in infravesical obstruction. A positive trend in the clinical symptoms was observed (by IPSS scale), especially irritative symptoms. Urodynamics has also improved: maximal and mean urinary flow rate and total amount of urination increased. However, the residual urine volume remained the same. Further placebo-controlled and randomized studies are needed for precise estimation of pikamilone effects. Meanwhile, the drug can be used as a component of combined therapy of benign prostatic hyperplasia.

RESUMO

The paper presents the technique and results of electromyography of the penis (EMP) for 68 males divided into 3 groups: 21 volunteers with erectile disorders, 20 patients with nonvasculogenic erectile dysfunction and no history of pelvic operations, 27 patients with erectile abnormalities as a result of pelvic operations (after cystectomy -11, cystoprostatectomy -1, rectal resection -7, rectal extirpation -8 patients, groups 1, 2, 3 and 4, respectively). Autonomic innervation of the penis is primarily characterized by such EMP parameters as amplitude, synchronism and frequency of potentials. Neurogenic penile disorder was identified in 3 patients of group 2 (1 patient with intervertebral disk hernia, 2 patients with lumbosacral osteochondrosis), in all patients of group 3 (electric silence was recorded in 22 patients). EMP should be included in the algorithm of examination of patients with erectile dysfunction as providing a reliable assessment of autonomic penile innervation.

RESUMO

Water intoxication remains a serious complication of transurethral resection (TUR) occurring more frequently in patients with large-size benign prostatic hyperplasia (BPH) and in those who was operated for more than 1 hour. An advanced irrigation system employing mechanical valve "Floval" and active aspiration provides controlled irrigation of the bladder preventing spontaneous rise of intravesical pressure in conducting TUR in BPH patients.

The authors employed transurethral radiofrequency thermodestruction (TRT) in 27 BPH patients with infravesical obstruction and grave somatic status excluding radical surgery. 17 of them had cystostomic drainage. A single procedure was not long and ran without general anesthesia. In patients able to urinate objective and subjective improvement was recorded, in those with cystostomic drainage suprapubic fistula was eliminated in 10 patients. In some cases TRT produced the results comparable to transurethral resection of the prostate. TRT complications comprise mainly infection and inflammation (11.1%). By efficacy and safety TRT occupies an intermediate position between thermotherapy and prostatic transurethral resection. This method is alternative in BPH patients with cystostoma and severe associated diseases.

Urinary tract infection (UTI) in females occurs significantly more frequently than in males because of specific anatomical and functional features of female urinary system, sequelae of pregnancy, delivery, gynecological diseases. Much controversy still exists as to pathogenesis of UTI and UTI-induced urinary inflammation. We have examined 233 females of different age with UTI and obtained evidence which shows participation of such factors as early and intensive sex, ignorance of sex hygiene, multiple pregnancies, deliveries, abortions, inflammatory gynecological diseases, anogenital infection in its pathogenesis. These factors were registered 2-4 times more frequently in UTI females than in controls without UTI. Bacteriological urinary and genital findings coincide in 80% of cases in terms of an infective agent. This suggests that it is essential to detect urogenital infection in girls and females as early as possible and to treat it adequately with antibacterial and other drugs. The leading role of an ascending urinogenic route in urinary tract infection from local sources in anogenital zone, sexual factor and the absence of relevant hygienic habits proved most contributing to UTI pathogenesis. This concept serves the basis for UTI prevention in females.
