The new demands for sustainable operation in the chemical industry due to increasing environmental regulations and agreements have generated the need to adapt existing processes to more intelligent production. The plastics sector is in a complex position due to its contribution to economic development and the climate crisis. Therefore, environmental assessment has become an important tool due to the benefits it provides by quantifying the environmental performance of processes, allowing it to balance operational and environmental needs. Polyvinyl chloride (PVC) is one of the most globally used polymers thanks to its resistance, flexibility, and cost-effectiveness. The polymer is synthetized by suspension polymerization, which is characterized by high productivity and controllability. However, it presents problems associated with intensive energy consumption and the emission of toxic substances and greenhouse gases. Therefore, an environmental assessment of the suspension PVC production process was performed using the waste reduction algorithm (WAR). The potential environmental impact (PEI) was quantified using the generation rate and the output velocity for four cases and three different fuels. It was found that the process transforms raw materials with high impacts, such as VCM, into substances with lower PEI, such as PVC. However, the process has a high generation of PEI due to the effects of energy consumption (-2860, -2410, 3020, and 3410 for cases 1-4, respectively). The evaluation of the toxicological impacts shows that the ATP category is the only one that presents a positive generation value (75 PEI/day); the product contributes to the formation and emission of impacts. The atmospheric categories showed that the energy consumption of the process is the most critical aspect with a contribution of 91% of the total impacts emitted. The AP and GWP categories presented the highest values. It was determined that the most suitable fuel is natural gas; it has lower impacts than liquid and solid fuels (coal). Additionally, it can be concluded that the PVC production process by suspension is environmentally acceptable compared to the polyethylene or polypropylene processes, with output impacts 228 and 2561 times lower, respectively.

With the objective of evaluating the quality parameters of raw milk in Ecuador between 2010 and 2020, a systematic review and meta-analysis of 73 studies on raw milk produced in different regions of Ecuador was performed. Under the random effects model, effect size and heterogeneity were determined vs. climatic region both among analyses and studies, with Cochran's Q, I2 and Tau (π) statistics. For all the variables, it was observed that there was great heterogeneity (I2 > 90%) among the studies; additionally, it was found that climatic region had an influence only among the variables arsenic, mercury, pH and total solids, and it was greater in the coastal region than the Inter-Andean region. The mean values of the physicochemical characteristics of the milk (titratable acidity, ash, cryoscopy, fat, lactose, pH, protein, non-fat solids and total solids) in the great majority of these studies were within the range allowed by Ecuadorian regulations. As for the hygienic quality of raw milk (total bacterial count, somatic cell count and presence of reductase), although the mean values were within those determined by local legislation, it should be noted that the range established by Ecuadorian regulations is relatively much higher compared to other regulations, which possibly means that there is a high presence of bacteria and somatic cells in raw milk. Finally, the presence of several adulterants (added water) and contaminants (AFM1, antibiotics and heavy metals) was confirmed in the milk, in addition to other substances such as eprinomectin, zearalenone and ptaquilosides, whose presence can be very dangerous, because they can be hepatotoxic, immunotoxic and even carcinogenic. In conclusion, there is great variability among the studies reviewed, with the physicochemical characteristics being the most compliant with Ecuadorian legislation; the hygienic characteristics, adulterants and contaminants of raw milk require greater attention by producers and local authorities, so that they do not harm the health of consumers and the profitability of producers in Ecuador.

Cyclic adenosine diphosphate (ADP)-ribose (cADPR) isomers are signaling molecules produced by bacterial and plant Toll/interleukin-1 receptor (TIR) domains via nicotinamide adenine dinucleotide (oxidized form) (NAD+) hydrolysis. We show that v-cADPR (2'cADPR) and v2-cADPR (3'cADPR) isomers are cyclized by O-glycosidic bond formation between the ribose moieties in ADPR. Structures of 2'cADPR-producing TIR domains reveal conformational changes that lead to an active assembly that resembles those of Toll-like receptor adaptor TIR domains. Mutagenesis reveals a conserved tryptophan that is essential for cyclization. We show that 3'cADPR is an activator of ThsA effector proteins from the bacterial antiphage defense system termed Thoeris and a suppressor of plant immunity when produced by the effector HopAM1. Collectively, our results reveal the molecular basis of cADPR isomer production and establish 3'cADPR in bacteria as an antiviral and plant immunity-suppressing signaling molecule.

Mobile applications (Apps) may become effective aids in health care. Health Apps could reduce barriers such as access and costs and could be used to monitor symptoms, behaviors and even treatments. There is more evidence of their usefulness in nutrition, cardiovascular and mental health. Despite this, its current use is predominantly for information purposes. Healthcare App quality evaluation should consider both clinical and technological aspects since the evidence on its clinical effectiveness is still incipient and they have associated risks. In Chile, the use of mobile technology and Apps is increasing, but there are no regulations for their use. There are few national institutions oriented to the creation and development of Apps for healthcare, highlighting the Digital Transformation Committee, part of the Corporation for the Promotion of Production (CORFO) and the National Center for Health Information Systems (CENS). General recommendations for healthcare App development and use have been established. In this process, it would be beneficial to include actors involved in care. Given the progress of healthcare Apps worldwide and nationally, it is important that health professionals develop digital skills to maximize the potential benefit of these technologies.

AIM: To validate the new classification criteria for antineutrophil cytoplasmic antibody-associated vasculitis in a real-life Peruvian cohort of antineutrophil cytoplasmic antibody-associated vasculitis patients. METHODS: We reviewed medical records from a Peruvian tertiary care center from January 1990 to December 2019. Antineutrophil cytoplasmic antibody-associated vasculitis was diagnosed based on the 1990 American College of Rheumatology (ACR) criteria, the 2012 Chapel Hill Consensus Conference definitions, the European Medicines Agency (EMEA) algorithm, and the clinical acumen of the treating rheumatologists. We classified all patients using the "former criteria" (the 1990 ACR criteria for granulomatosis with polyangiitis [GPA] and eosinophilic GPA [EGPA] and the 1994 Chapel Hill Consensus Conference definition for microscopic polyangiitis [MPA]), the EMEA algorithm, and the "new criteria" (the 2017 ACR/European League Against Rheumatism Provisional Criteria). The level of agreement (using Cohen κ) was calculated using the clinical diagnosis as the criterion standard. RESULTS: We identified 212 patients, 12 of whom were excluded. One hundred fifty-four (77%) had MPA, 41 (20.5%) GPA, and 5 (2.5%) EGPA. The new criteria performed well for MPA (κ = 0.713) and EGPA (κ = 0.659), whereas the EMEA algorithm performed well for GPA (κ = 0.938). In the overall population, the new criteria showed better agreement (κ = 0.653) than the EMEA algorithm (κ = 0.506) and the former criteria (κ = 0.305). CONCLUSIONS: The 2017 ACR/European League Against Rheumatism Provisional Criteria showed better agreement for the clinical diagnosis of all the patients overall and had the best performance for MPA and EGPA. The EMEA algorithm had the best performance for GPA.

AIM: The aim of this study was to identify demographic and clinical risk factors for mortality in patients with antineutrophil cytoplasmic antibodies-associated vasculitides (AAVs) in a Peruvian tertiary referral hospital. METHODS: Medical records of patients with AAV according to classification criteria or diagnosed by an experienced rheumatologist, covering the period between January 1990 and December 2018, were reviewed. Granulomatosis with polyangiitis, microscopic polyangiitis, eosinophilic granulomatosis with polyangiitis, and renal-limited vasculitis were included. Potential predictors of mortality were demographic factors, clinical manifestations, antineutrophil cytoplasmic antibodies status, diagnosis, disease categorization, the 2009 Five Factor Score (FFS), and treatment. Cox regression models were used to determine the risk factors for mortality. Univariable and multivariable analyses using a backward selection method were performed. RESULTS: One hundred ninety-six patients were included; female-to-male ratio was 2:1. The median (interquartile range) age at diagnosis and follow-up were 60.0 (51.0-68.0) and 4.8 (1.3-11.6) years, respectively. One hundred forty-eight patients (75.5%) had microscopic polyangiitis, 37 (18.9%) granulomatosis with polyangiitis, 5 (2.6%) eosinophilic granulomatosis with polyangiitis, and 6 (3.0%) renal-limited vasculitis. Overall survival rates at 1, 5, and 10 years were 83.4%, 68.2%, and 51.7%, respectively. Ocular involvement was protective (hazards ratio [HR], 0.36; 95% confidence interval [CI], 0.17-0.74; p = 0.006), whereas renal (HR, 2.09; 95% CI, 1.33-3.28; p = 0.001) and lung involvement (HR, 2.07; 95% CI, 1.31-3.28; p = 0.002) and the 2009 FFSs were predictive of mortality (2009 FFS = 1: HR, 2.46; 95% CI, 1.50-4.04; p < 0.001; 2009 FFS = 2: HR, 3.07; 95% CI, 1.54-6.10; p = 0.001; 2009 FFS = 3: HR, 13.29; 95% CI, 3.69-47.88; p < 0.001). CONCLUSIONS: Ocular involvement was protective, whereas 2009 FFS ≥ 1 and renal and lung involvement were predictive factors of mortality in Peruvian AAV patients.

AIM: The aim of this study was to identify the demographic and clinical features of patients with ANCA-associated vasculitides (AAVs) in a Peruvian tertiary referral hospital. METHODS: Medical records of patients with AAV according to classification criteria or diagnosed by an experienced rheumatologist, and covering the period between January 1990 and December 2019, were reviewed. Granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), eosinophilic granulomatosis with polyangiitis (EGPA), and renal-limited vasculitis (RLV) were included. Demographic factors (age at diagnosis, sex), disease duration, clinical manifestations (per organ involvement), creatinine level at diagnosis (milligram per deciliter), ANCA status, diagnosis, 2009 Five Factor Score, disease categorization, and treatment were recorded. RESULTS: Two hundred twelve patients were included. Their female-to-male ratio was 1.9:1 (139 [65.6%]/73 [34.4%]), and their mean (SD) age at diagnosis was 59.2 (12.5) years. One hundred fifty-eight patients (74.5%) had MPA, 42 (19.8%) GPA, 7 (3.3%) RLV, and 5 (2.4%) EGPA. Neurological, lung, and renal involvements were the most frequently affected systems. Myeloperoxidase preferentially occurred in MPA (82.5%), whereas proteinase 3 did occur in GPA (79.5%). Microscopic polyangiitis patients were older (61.1 [11.5] years). Female sex predominated in MPA and RLV (2.4:1 and 6:1, respectively), but the opposite was the case for EGPA (1:4). Ear-nose-throat and ocular involvement were more frequent in GPA (both p's < 0.001), and neurological and cardiovascular involvement were more frequent in EGPA (p < 0.001 and p = 0.002, respectively). CONCLUSIONS: This is one of the largest series of AAV patients in Latin America. Overall, female sex predominated. Microscopic polyangiitis was the most frequent AAV, and myeloperoxidase-ANCA was the most frequent antibody in Peruvian AAV population.

RESUMO

SARS-CoV-2 pandemic generated a profound impact on people's health, emphasizing the relevance of healthy lifestyles. Recommendations on how to maintain adequate physical activity, diet, sleep and social connection have been issued. However, it is worth expanding our look to other possible elements related to lifestyles such as the relationship with technology, nature, pets and music. These areas should be included in the assessment and intervention from this perspective. To achieve changes, the values, beliefs, intentions, motivations, risk/benefit balances, capacity for self-regulation, previous history of changes and the person's sense of competence in relation to the possible changes that are being suggested, should be assessed. Individualized and contextualized suggestions that increase the intention of change should be made, avoiding confrontation and generalizations. Although there are still areas of uncertainty in this approach, particularly in relation to dosage and mechanisms of action, its development should be encouraged, given its great potential in terms of cost-effectiveness.

Objetivos: Estimar el efecto de los antimaláricos (AM) sobre los diferentes dominios del índice de daño SLICC (SDI). Métodos: Se estudiaron pacientes con diagnóstico clínico reciente (≤2 años) de lupus eritematoso sistémico (LES) de la cohorte GLADEL. Variable de estudio: aumento en los dominios del SDI desde el ingreso a la cohorte. Variables independientes: características sociodemográficas, clínicas, laboratorio y tratamientos. El efecto de los AM, como variable dependiente del tiempo, sobre los dominios más frecuentes del SDI (ajustado por factores de confusión) fue examinado con un modelo de regresión de Cox multivariado. Resultados: De 1466 pacientes estudiados, 1049 (72%) recibieron AM con un tiempo medio de exposición de 30 meses (Q1-Q3: 11-57) y 665 pacientes (45%) presentaron daño durante un seguimiento medio de 24 meses (Q1-Q3: 8-55); 301 eventos fueron cutáneos, 208 renales, 149 neuropsiquiátricos, 98 musculoesqueléticos, 88 cardiovasculares y 230 otros. Después de ajustar por factores de confusión, el uso de AM se asoció a un menor riesgo de daño renal (HR 0,652; IC 95%: 0,472-0,901) y en el límite de la significancia estadística (HR 0,701, IC 95%: 0,481-1,024) para el dominio neuropsiquiátrico. Conclusión: En GLADEL, el uso de AM se asoció independientemente a un menor riesgo de daño acumulado renal.


Objective: To assess the effects of antimalarials (AM) over the items of the SLICC Damage Index (SDI). Methods: Patients with recent (≤2 years) diagnosis of systemic lupus erythematosus (SLE) from the GLADEL cohort were studied. End-point: increase in items SDI since cohort entry. Independent variables (socio-demographic, clinical, laboratory and treatment) were included. The effect of AM as a time dependent variable on most frequent SDI items (adjusting for potential confounders) was examined with a multivariable Cox regression model. Results: Of the 1466 patients included in this analysis, 1049 (72%) received AM with a median exposure time of 30 months (Q1-Q3: 11-57). Damage occurred in 665 (45%) patients during a median follow-up time of 24 months (Q1-Q3: 8-55). There were 301 integument, 208 renal, 149 neuropsychiatric, 98 musculoskeletal, 88 cardiovascular and 230 others less frequently represented damages. After adjusting for potential confounders at any time during follow-up, a lower risk of renal damage (HR 0.652; 95% CI: 0.472-0.901) and borderline for neuropsychiatric damage (HR 0.701, 95% CI: 0.481-1.024) was found. Conclusion: In the GLADEL cohort, after adjustment for possible confounding factors, AM were independently associated with a reduced risk of renal damage accrual.

OBJECTIVE: To define whether Amerindian genetic ancestry correlates with clinical and therapeutic variables in admixed individuals with rheumatoid arthritis (RA) from Latin America. METHODS: Patients with RA (n = 1347) and healthy controls (n = 1012) from Argentina, Mexico, Chile, and Peru were included. Samples were genotyped for the Immunochip v1 using the Illumina platform. Clinical data were obtained through interviews or the clinical history. RESULTS: Percentage of Amerindian ancestry was comparable between cases and controls. Morning stiffness (p < 0.0001, OR 0.05), rheumatoid factor (RF; p < 0.0001, OR 0.22), radiographic changes (p < 0.0001, OR 0.05), and higher number of criteria were associated with lower Amerindian ancestry after Bonferroni correction. Higher Amerindian ancestry correlated only with weight loss (pBonferroni < 0.0001, OR 2.85). Increased Amerindian ancestry correlated with higher doses of azathioprine (p < 0.0001, OR 163.6) and sulfasalazine (p < 0.0001, OR 48.6), and inversely with methotrexate (p = 0.001, OR 0.35), leflunomide (p = 0.001, OR 0.16), and nonsteroidal antiinflammatory drugs (pBonferroni = 0.001, OR 0.37). Only the presence of RF and weight loss were modified after confounders adjustment. CONCLUSION: Amerindian ancestry protects against most major clinical criteria of RA, but regarding the association of RF with increased European ancestry, age, sex, and smoking are modifiers. Ancestry also correlates with the therapeutic profiles.

Intraoperative electron radiation therapy (IOERT) involves irradiation of an unresected tumour or a post-resection tumour bed. The dose distribution is calculated from a preoperative computed tomography (CT) study acquired using a CT simulator. However, differences between the actual IOERT field and that calculated from the preoperative study arise as a result of patient position, surgical access, tumour resection and the IOERT set-up. Intraoperative CT imaging may then enable a more accurate estimation of dose distribution. In this study, we evaluated three kilovoltage (kV) CT scanners with the ability to acquire intraoperative images. Our findings indicate that current IOERT plans may be improved using data based on actual anatomical conditions during radiation. The systems studied were two portable systems ("O-arm", a cone-beam CT [CBCT] system, and "BodyTom", a multislice CT [MSCT] system) and one CBCT integrated in a conventional linear accelerator (LINAC) ("TrueBeam"). TrueBeam and BodyTom showed good results, as the gamma pass rates of their dose distributions compared to the gold standard (dose distributions calculated from images acquired with a CT simulator) were above 97% in most cases. The O-arm yielded a lower percentage of voxels fulfilling gamma criteria owing to its reduced field of view (which left it prone to truncation artefacts). Our results show that the images acquired using a portable CT or even a LINAC with on-board kV CBCT could be used to estimate the dose of IOERT and improve the possibility to evaluate and register the treatment administered to the patient.

La osteoporosis es la enfermedad ósea más común en el mundo globalizado. Produce un alto impacto en el costo de la Salud Pública debido al incremento en forma progresiva en el correr del tiempo de vida del número de personas que la padecen, originada entre otros factores por la mayor expectativa de vida poblacional y disponer de nuevos procedimientos de diagnóstico, los que han conseguido gran penetración en las atenciones de salud. Las fracturas óseas son la principal consecuencia de esta enfermedad, las que originan dolor crónico, discapacidad y pérdida de la calidad de vida, pudiendo llevar inclusive a la muerte. Existen guías clínicas internacionales vigentes que se basan en evidencias de estudios clínicos aleatorizados y que dan pautas para el tratamiento de la osteoporosis y en especial de la forma posmenopáusica, ofreciendo tanto acciones farmacológicas como no farmacológicas. A raíz de resultados de estudios de seguimiento en gran número de pacientes, se discute en la actualidad aspectos de soporte terapéuticos tradicionales, como el uso y dosis de suplementos de calcio, sosteniéndose que en los que tienen una dieta adecuada de calcio existe un aparente no beneficio en su ingesta y es posible que un grupo de ellos puedan desarrollar eventos adversos como complicaciones, tales como constipación crónica, diverticulitis, litiasis renal o enfermedades cardiovasculares. Del mismo modo se reportan estudios que plantean la sugerencia de un uso con tiempo límite de bifosfonatos, sucediendo además de la pérdida de eficacia, la posibilidad de desarrollar eventos adversos como fracturas femorales atípicas y osteonecrosis de mandíbula. Este último evento adverso también es descrito con el uso de denosumab. O, por otro lado, en los que reciben teriperatida existe la infrecuente posibilidad de desarrollar neoplasias como el osteosarcoma, por lo que la recomendación es que su uso no debe superar los dos años. Finalmente, a raíz de las investigaciones sobre las concentraciones séricas de la vitamina D y su relación con los síntomas y signos que pueden dar su deficiencia, en las personas, y los posibles beneficios con acciones sobre la autoinmunidad, algunas neoplasias y una leve mejora de la masa ósea, da pie a sostener que el suplemento de vitamina D3 debe ser una parte importante en la terapia.


Osteoporosis is the most common bone disease in the globalized world, with high impact on health care costs due to progressive increase in life expectancy and the existence of new diagnostic procedures. Bone fractures are the main consequence of this disease, which cause chronic pain, disability and loss of quality of life, and can even lead to death. Existing international clinical guidelines based on evidence from randomized trials give standards for the treatment of osteoporosis, especially its post-menopausal form, offering both pharmacological and non-pharmacological actions. Based on results from follow-up studies in a large number of patients, we currently discuss aspects of traditional therapeutic support, such as the use and dosage of calcium supplements. It is proposed that there may exist an apparent non-benefit from its intake among those with an adequate level of dietary calcium; it is possible that some of them may develop adverse events such as chronic constipation, diverticulitis, kidney stones or cardiovascular disease. Similarly, some studies suggest to use biphosphonates for a limited period of time, which associates with loss of efficiency and the possibility of adverse events, such as atypical femoral fractures and osteonecrosis of the jaw bone. This last adverse event is also described with the use of denosumab. What is more, among those who receive teriperatide there exists the infrequent possibility of developing neoplasms such as osteosarcoma, so that the recommendation is that its use cannot exceed 2 years. Finally, as a result of research on serum concentrations of vitamin D and their relationship with signs and symptoms that may present in its deficiency, as well as the possible benefits from vitamin D on autoimmunity, some neoplasms and a mild improvement in bone mass, we can affirm that the supplementation with vitamin D3 must be an important part in its therapy.

Históricamente, las personas con discapacidad han constituido un sector al que, sobre prejuicios anclados en el binomio normalidad/anormalidad, se le ha discriminado. Sin embargo, en diferentes partes del mundo está comenzando a emerger, de manera más visible, un movimiento de personas con discapacidad que en su proceso de afianzamiento como un sujeto social también está realizando aportes a la comprensión de formas de opresión que hasta muy recientemente no se consideraban como tales. En este contexto, el presente documento, que forma parte de una investigación amplia sobre discapacidad y colonialidad realizada en el sur de México, explora, a partir del uso de la teoría fundamentada, la ruta que ha seguido la transformación de las nociones sobre las personas con discapacidad como consecuencia de los movimientos sociales contemporáneos, llegando a la conclusión de que las nociones tradicionales han llevado a la exclusión de ese sector, entendiendo la exclusión como la "muerte social".


Historically, people with disabilities have been part of a sector that, over prejudices, rooted in the binomial normality / abnormality, has suffered discrimination. However, thanks to awareness processes that gradually have taken place in different parts of the world, a movement of people with disabilities is emerging in a more visible way, movement that, in its process of consolidation as a social subject, is also making contributions to the understanding of forms of oppression that, until recently, were not considered as such. In this context, this document, that is part of a research on disability and colonialism carried out in the south of Mexico, explores, with the use of the grounded theory, the path that the transformation of the notions about of the persons with disability has followed as a consequence of the social contemporary movements, leading to the conclusion that the traditional notions have generated the exclusion of this sector, understanding the exclusion as "social death".

El entrenamiento en Informática Biomédica es fundamental para enfrentar los desafíos de un mundo globalizado. Sin embargo, el desarrollo de programas de entrenamiento e investigación en posgrado en esta área son escasos en América Latina. A través del QUIPU: Centro Andino de Investigación y Entrenamiento en Informática para la Salud Global, se ha desarrollado el primer programa de Diplomado y Maestría en Informática Biomédica en la Región Andina. El objetivo de este artículo es describir la experiencia del programa. A la fecha han participado 51 alumnos de Perú, Chile, Ecuador, Colombia y Venezuela; procedentes de ministerios de salud, hospitales, universidades, centros de investigación, colegios profesionales y empresas privadas. Diecisiete cursos se impartieron con la participación presencial y virtual de profesores de Argentina, Chile, Colombia, Estados Unidos, México y Perú. Este programa está ya institucionalizado en la Facultad de Salud Pública y Administración de la Universidad Peruana Cayetano Heredia.


Training in Biomedical Informatics is essential to meet the challenges of a globalized world. However, the development of postgraduate training and research programs in this area are scarce in Latin America. Through QUIPU: Andean Center for Training and research in Iformatics for Global Health, has developed the first Certificate and Master’s Program on Biomedical Informatics in the Andean Region. The aim of this article is to describe the experience of the program. To date, 51 students from Peru, Chile, Ecuador, Colombia and Venezuela have participated; they come from health ministries, hospitals, universities, research centers, professional associations and private companies. Seventeen courses were offered with the participation of faculty from Argentina, Chile, Colombia, USA, Mexico and Peru. This program is already institutionalized at the School of Public Health and Administration from the Universidad Peruana Cayetano Heredia.

El advenimiento del uso de terapias biológicas en Reumatología ha modificado significativamente el pronóstico de pacientes portadores de artritis reumatoide (AR), artritis juvenil (AJ), espondilitis anquilosante (EA), entre otras enfermedades. A diferencia de las terapias convencionales estos productos biológicos se dirigen a los llamados blancos terapéuticos ya sea estas una línea celular, un mediador inflamatorio o un receptor de superficie. Estos compuestos son producidos por células vivas mediante la tecnología del ADN recombinante. Estos compuestos pueden tener componentes humano y animal [quiméricos (Xi), humanizados (Zu)], o completamente humanos (H) lo cual se reconoce por las letras que se incluyen en el nombre de cada uno. En el campo de la Reumatología, el primer compuesto utilizado fue el etanercept (anti-factor de necrosis tumoral o anti-TNF) aprobado en 1998, pero otros anti-TNF han demostrado su beneficio en AR, como en EA y AJ. Los inhibidores de Interleucina (IL-1) casi no se usan en AR actualmente, pero si los inhibidores de IL-6, así como los agentes contra las células B y los agonistas de CTLA-4 (Cytotoxic T lymphocyte antigen). Existe asimismo un compuesto dirigido al BLyS (B-lymphocyte stimulator) el cual se usa en lupus eritematoso sistémico y otro dirigido al receptor activador del factor nuclear κB (RANKL, receptor activator of nuclear factor-κB ligand) que se usa en osteoporosis. Con el avance en el conocimiento de la patogenia de las enfermedades reumáticas, se vienen reconociendo otra blancos terapéuticas. En los años venideros, este campo ha de expandirse en proporciones geométricas.


The advent of biologic therapies in Rheumatology has modified significantly the prognosis of patients with rheumatoid arthritis (RA), juvenile arthritis (JA), ankylosing spondylitis (AS), among others. In contrast to the conventional therapies, these biological therapies are directed at specific targets being those a cell line, an inflammatory mediator, or a surface receptor. These compounds are produced by live cells using recombinant DNA technology. They can have human and animal components [chimerics (Xi), humanized (Zu)] or be completely human (H); this is recognized by the letters included in their names. The first compound used in Rheumatology was etanercept (antitumor necrosis factor) which was approved by the FDA in 1998; however other anti-TNF compounds have proven to be beneficial in RA as well as in AS and JA. The interleukin-1 (IL-1) inhibitors are rarely used in RA at thepresent time, but IL-6 inhibitors and agents directed against CTLA-4 (Cytotoxic T lymphocyte antigen) are. There is also a compound against BLyS (B-lymphocyte stimulator) which is used in systemic lupus erythematosus andother directed at RANKL (receptor activator of nuclear factor-κB ligand) which is used in osteoporosis. With the advances made in the understanding of the pathogenesis of the rheumatic diseases, new therapeutic targets are being recognized. In the years to come this field will expand in geometric proportions.

Objetivos. El objetivo del presente trabajo es evaluar la calidad de páginas web en español, que contengan información sobre los síntomas del embarazo. Materiales y métodos. Se realizó un estudio transversal y analítico, empleando la metodología previamente validada por Curró et al. para evaluar la calidad de páginas web; estas páginas fueron recuperadas a través de búsquedas en Google, MSN.com y Yahoo.com. La evaluación de la calidad incluyó tres componentes: contenido médico, responsabilidad y usabilidad de la página web. El contenido médico fue contrastado con la información disponible en las guías nacionales de Atención Integral de la Salud Sexual y Reproductiva, del Ministerio de Salud del Perú. Resultados. Se evaluaron 99 páginas web con contenido en español. El 26,3% presentó media adherencia al patrón de oro. En el 32,3% se encontraron los tres factores evaluados de las guías nacionales; 37,4% eran páginas web de mala calidad. El 24,2% eran blogs los cuales estaban asociados, como factor protector, a páginas web de mala calidad (OR: 0,1, IC 95%: 0,02-0,5). Conclusiones. La mayoría de páginas web evaluadas en español, relacionadas con síntomas del embarazo, son de mala calidad. Estos resultados indican que se requiere educar adecuadamente a los pacientes y al público en general así como evaluar la calidad de información que se encuentra en Internet.


Objectives. The objective of this study is to evaluate the information quality about pregnancy symptoms in Spanish available in the internet. Materials and methods. This is an analytic, cross-sectional study. In order to evaluate the selected websites, we used a methodology previously validated by Curró et al. The websites were selected from Google, MSN.com and Yahoo.com. The quality evaluation included three components: medical contents, accountability and usability of the website. The medical content was compared with the data available in the national guidelines for Comprehensive Sexual and Reproductive Health, from the Peruvian Ministry of Health. Results. Ninety nine websites in Spanish were evaluated. From them, 26.3% had medium adherence to the gold standard document. In 37.4% the websites were of low quality. A total of 24.2% were blogs, they were associated (as a protrective factor) to sites of poor quality (OR: 0.1, 95% CI: 0.02 to 0.5). Conclusions. The majority of websites in Spanish related to pregnancy symptoms have low quality. These findings indicate the need to properly educate patients and general public, and a critical evaluation of the information quality found at the Internet as well.

RESUMO

New drug classes, biologics, have been developed over the past 10 years based on human or chimeric antibodies against cytokines or receptors with pivotal roles in the inflammatory pathways of immune-mediated inflammatory disease. Anti-tumor necrosis factor agents carry the largest infection risk of all the biologics, predisposing patients to mycobacterial infections. Patients receiving biologics are at higher risk for developing tuberculosis. New cases of tuberculosis or reactivation of latent tuberculosis infections may occur during the course of treatment, so a high level of vigilance is highly recommended.
