OBJECTIVES: To examine the advanced practice nurse workforce in Swiss cancer care and how their roles are being implemented (eg, role structures, processes) to achieve optimal outcomes for patients and their families, care organizations, and the broader health care system. METHODS: A cross-sectional study was conducted. The sample included master-prepared advanced practice nurses in cancer care, who completed an online questionnaire from December 2021 to January 2022. Thirty-nine items assessed structures (eg, role characteristics, utilization), processes (eg, role activities, interventions), and perceived outcomes (eg, for patients, the health care system) of advanced practice nurses in Swiss cancer care. Data from closed questions were analyzed using descriptive statistics. Data from open-ended questions were organized and summarized into categories related to domains of advanced practice nursing and its reported frequency by the advanced practice nurses. RESULTS: The participating advanced practice nurses (n = 53), worked in half of the 26 Swiss cantons. Interventions were identified within nine categories, of which most were targeted to patients and their families (n = 7), followed by health care professionals (n = 2). Perceived positive outcomes were patient symptom management, length of hospital stay, and health care costs. Participants felt less confident in cancer care (eg, autonomous practice) and reported 15 professional development needs (eg, medical interventions, teaching). CONCLUSIONS: This study provides a comprehensive examination of 53 advanced practice nurses, detailing the characteristics of their roles and utilization across various jurisdictions and health care settings. The results highlight the diverse dimensions of advanced practice nursing and its potential to enhance cancer services and outcomes in Switzerland. Opportunities for role development support and expansion are identified. IMPLICATIONS FOR NURSING PRACTICE: More systematic health human resource planning is needed to expand the deployment of advanced practice nurses across jurisdictions, practice settings, and more diverse patient populations. Role development needs show the desire for specialized educational preparation in cancer care.

OBJECTIVES: Advanced practice nursing roles in cancer care are diverse and exist across the cancer care continuum. However, the titles used and the scope of practice differ across countries. This diversity is likely to be misleading to patients and influence nurses' contribution to health care. An understanding of the current state of advanced practice nursing roles in cancer care internationally is needed to inform opportunities for future role development and enhance cancer nursing career pathways. METHODS: This scoping review included a systematic search of four databases: MEDLINE, CINAHL, PsycINFO, and Academic Search Complete. Independent screening for papers meeting the review's inclusion criteria was undertaken using online screening software. Data extraction, coding, and mapping were undertaken in NVivo 12. RESULTS: Of the 13,409 records identified, 108 met the review's inclusion criteria. A variety of roles in cancer care settings were described. The United States and the United Kingdom had the most titles for advanced practice nursing roles. Tumor-specific roles were described and integrated into different phases of the cancer care continuum. Trends in continuing professional development for advanced practice nurses in cancer care included the rise in Fellowship programs in the United States and practice-based education in the United Kingdom. CONCLUSIONS: The differences in advanced practice nursing roles in cancer care allow regional and institutional variation to meet the needs of patient populations and health care system demands. However, a lack of clarity surrounding titles and roles results in confusion and underutilization of these nurses' highly specialized skill sets. IMPLICATIONS FOR NURSING PRACTICE: Incongruence in titles and scope of practice internationally will ultimately result in a merging of roles. There is a need for international agreement on education requirements for advanced practice nursing roles to promote career pathways.

INTRODUCTION: Quality of life (QoL) and patient satisfaction are major concerns in oncology. METHODS: The aim of this prospective observational study was to evaluate these parameters according to the mode of administration of anti-HER2 (subcutaneous [SC] versus intravenous [IV]), the place of administration (Home Hospitalization or HOD versus hospital) for patients supervised by an advanced practice nurse (APN). RESULTS: Between January 2022 and June 2023, 32 patients were included. They were statistically more satisfied with subcutaneous management (P=0.0004), a result explained by the speed of administration (43.5%), comfort during administration (26%) even though some expressed pain on injection and felt less anxiety (26%). Management by the APN seems more appropriate when anti-HER2 drugs were administered in HOD. In HOD, patients perceived an overall improvement in their quality of life, appetite and cognitive abilities, with a reduction in fatigue, pain and depression (P<0.05). However, the rate of outsourcing to HOD remained too low (30.4%), as 56.3% of patients would have liked to be cared for in HOD if they had had the opportunity. CONCLUSION: SC administration of anti-HER2 under the supervision of an APN has advantages for the patient, resulting in greatest satisfaction and improved patient QOL, preferably in HOD.

BACKGROUND: During the COVID-19 pandemic, decentralised clinical trials incorporated self-monitoring, self-reporting, and telenursing tools to address health literacy and health empowerment of patients enrolled in clinical trials. We aimed to determine the impact of an educational intervention using telenursing consultations on health literacy, health empowerment, and health-related quality of life in cancer patients enrolled in clinical trials by measuring the level of satisfaction with the care received and assessing the views of healthcare professionals concerning the advanced practice nurse (APN) role in oncology clinical trials. METHODS: In this pilot analytical, descriptive, longitudinal, quasi-experimental, and pre-post test study, an educational intervention was conducted by 5 visits with an APN using synchronous teleconsultation in patients starting cancer treatment for the first time in a clinical trial (n = 60), and health professionals working with the APN (n = 31). A descriptive analysis of the samples and questionnaires were utilised along with statistical comparisons. RESULTS: After the intervention, patients' health literacy (31.7%), health empowerment (18.3%), and health-related quality of life (33.3%) increased (p < 0.05), with a decrease and trend towards resolution of care needs (p < 0.05). Satisfaction with the quality and care received in terms of perceived convenience, transition, and continuity of care showed positive results in 64.9 ± 20.7, 77.6 ± 19.5, and 72.1 ± 20.4 of respondents, respectively. On the overall assessment of the APN role, healthcare professionals expressed a high level of agreement with the statements related to their work performance. CONCLUSIONS: The data indicates that a clinical trial APN-led telenursing educational intervention results in an overall increase in health literacy, an improvement in health empowerment and health-related quality of life, and a decrease in care needs of oncology clinical trials patients. Patients stated that they received a high quality of care and health professionals indicated high levels of acceptance with APNs. Based on these results, we suggest that the APN role should gain more recognition in the Spanish healthcare system and their professional competencies should be aligned with those of other countries.

OBJETIVO: Sumarizar as evidências existentes acerca das ações de Prática Avançada em Enfermagem no âmbito da Atenção Primária à Saúde à nível mundial através de uma revisão sistemática. MÉTODO: Trata-se de um protocolo desenvolvido com base nas diretrizes do Preferred Reporting Items for Systematic Reviews and Meta-analysis Protocols (PRISMA-P). O protocolo foi registrado no International Prospective Register of Systematic Reviews (PROSPERO) sob o ID: CRD42022301785. Para condução da revisão, a busca sistemática será realizada nas bases de dados ScienceDirect, Scopus, Medical Literature Analyses and Retrieval System Online (MEDLINE/PubMed), Web of Science, Excerpta Medica Database (Embase) e Cumulated Index in Nursing and Allied Health Literature (CINAHL), sendo utilizada a seguinte expressão de busca: (("Advanced Practice Nursing" OR Nursing OR "Nursing specialities") AND "Primary Health Care" AND "Public Health"). Os critérios de inclusão incluem todos os estudos transversais publicados em inglês até outubro de 2022 que respondam à questão de pesquisa. Destes, serão excluídos: (i) estudos em animais; (ii) cartas, editoriais, revisões de literatura, revisões sistemáticas e metanálises.


OBECTIVE: To synthesize the existing evidence on advanced practice nursing interventions in primary health care settings worldwide through a systematic review. METHOD: A protocol was developed based on the Preferred Reporting Items for Systematic Reviews and Meta-analysis Protocols (PRISMA-P) guidelines. The protocol was registered in the International Prospective Register of Systematic Reviews (PROSPERO) under ID: CRD42022301785. To conduct the review, a systematic search will be performed in the databases ScienceDirect, Scopus, Medical Literature Analysis and Retrieval System Online (MEDLINE/PubMed), Web of Science, Excerpta Medica Database (Embase), and Cumulative Index to Nursing and Allied Health Literature (CINAHL) using the following search terms: (("Advanced Practice Nursing" OR Nursing OR "Nursing specialties") AND "Primary Health Care" AND "Public Health"). Inclusion criteria include all cross-sectional studies published in English through October 2022 that address the research question. The following will be excluded: (i) animal studies; (ii) letters, editorials, literature reviews, systematic reviews, and meta-analyses.

RESUMO Objetivo identificar quais os desfechos das inserções de dispositivos intrauterinos por enfermeiros em instituições de saúde. Métodos revisão integrativa realizada em oito bases de dados, com auxílio de um gerenciador de referências bibliográficas, utilizando o modelo de JBI para a análise do nível de evidência. Os dados foram organizados, categorizados e discutidos por meio de síntese descritiva. Resultados 10 estudos compuseram a amostra final, sendo dois apenas com enfermeiros e os demais com enfermeiros e médicos. Os principais desfechos avaliados foram perfuração e expulsão, sem diferenças significativas entre os profissionais que realizaram o procedimento. As taxas de sucesso, continuidade e satisfação foram semelhantes entre médicos e enfermeiros. Conclusão os desfechos das inserções de dispositivos intrauterinos por enfermeiros em instituições de saúde são similares aos realizados por médicos, com ampliação do acesso, sem aumentar as complicações relacionadas a esse método contraceptivo, contribuindo para a integralidade da assistência no campo do planejamento reprodutivo. Contribuições para a prática: os achados podem contribuir para o enfrentamento de barreiras pelos enfermeiros, servindo como subsídio para diretrizes e políticas de saúde que incentivem a inserção do dispositivo intrauterino por estes profissionais, principalmente, em contextos onde esta prática ainda não ocorre.


ABSTRACT Objective to identify the outcomes of intrauterine device insertions by nurses in healthcare institutions. Methods integrative review carried out in eight databases, with the help of a bibliographic reference manager, using the JBI model for analyzing the evidence level. The data was organized, categorized and discussed using descriptive summaries. Results 10 studies made up the final sample, two with nurses only and the others with nurses and physicians. The main outcomes assessed were perforation and expulsion, with no significant differences between the professionals who performed the procedure. Success rates, continuity and satisfaction were similar between physicians and nurses. Conclusion the outcomes of intrauterine device insertions by nurses in health institutions are similar to those carried out by physicians, with increased access, without increasing the complications related to this contraceptive method, contributing to comprehensive care in the field of reproductive planning. Contributions to practice: the findings could help nurses to tackle barriers and serve as a basis for guidelines and health policies that encourage the insertion of the intrauterine device by these professionals, especially in contexts where this practice does not yet take place.

ABSTRACT Objectives: to synthesize and analyze evidence on intrauterine device insertion by nurses in Primary Health Care. Methods: an integrative review, carried out in the BDENF, CINAHL, LILACS, SciELO, Scopus, PubMed and Web of Science databases in June 2022, delimiting the period from 1960 to 2022. Results: 141 articles were identified in the initial search, and 10 studies made up the final sample. Four (40%) were developed in the United States and one (10%) in Brazil, with publications from 1979 to 2021. The findings were grouped into three categories: Nurse training to insert an intrauterine device; Nurses' competency to insert an intrauterine device; and Women's access to intrauterine devices. Conclusions: nurse theoretical and practical training is a prominent element, consolidated in the favorable outcomes of insertions performed by nurses and satisfaction among women, a practice that has expanded access to the contraceptive method in Primary Health Care.

Resultados: se identificaron 141 artículos en la búsqueda inicial y 10 publicaciones conformaron la muestra final. Cuatro (40%) fueron desarrollados en Estados Unidos y uno (10%) en Brasil, con publicaciones de 1979 a 2021. Los hallazgos se agruparon en tres categorías: Capacitación de enfermeras para insertar un dispositivo intrauterino; Competencia de las enfermeras para insertar un dispositivo intrauterino; y Acceso de las mujeres a los dispositivos intrauterinos. Conclusiones: la formación teórica y práctica de los enfermeros es un elemento destacado, consolidado en los resultados favorables de las inserciones realizadas por los enfermeros y la satisfacción de las mujeres, práctica que ha ampliado el acceso al método anticonceptivo en la Atención Primaria de Salud.

AIM(S): To understand the experiences of advanced practice nurses working in cancer care. DESIGN: Phenomenological qualitative study. METHODS: Three focus groups were held to collect qualitative data. Participants were recruited through theoretical non-probabilistic sampling of maximum variation, based on 12 profiles. Data saturation was achieved with a final sample of 21 oncology advanced practice nurses who were performing advanced clinical practice roles in the four centers from December 2021 to March 2022. An interpretative phenomenological analysis was performed following Guba and Lincoln's criteria of trustworthiness. The centers' ethics committee approved the study, and all participants gave written informed consent. Data analysis was undertaken with NVivo 12 software. RESULTS: Three broad themes emerged from the data analysis: the role performed, facilitators and barriers in the development of the role and nurses' lived experience of the role. CONCLUSION: Advanced practice nurses are aware that they do not perform their role to its full potential, and they describe different facilitators and barriers. Despite the difficulties, they present a positive attitude as well as a capacity for leadership, which has allowed them to consolidate the advanced practice nursing role in unfavourable environments. IMPLICATIONS FOR THE PROFESSION: These results will enable institutions to establish strategies at different levels in the implementation and development of advanced practice nursing roles. REPORTING METHOD: Reporting complied with COREQ criteria for qualitative research. PATIENT OR PUBLIC CONTRIBUTIONS: No patient or public contribution.

Introducción: se estima escasez de 5.9 millones de profesionistas de la salud a nivel mundial. La Enfermería de Práctica Avanzada (EPA), cuenta con conocimientos, competencias y habilidades clínicas específicas para actuar en situaciones de déficit de personal médico, tratando desde enfermedades agudas hasta crónico degenerativas, preservando tanto la salud mental como física. Objetivo: Determinar el desempeño de la Enfermería de Práctica Avanzada en el Sector Salud. Metodología: revisión sistemática sobre la EPA, que incluyó revisiones y artículos originales del periodo 2013-2022 en español e inglés, acerca de su desempeño: evolución, aceptación y reconocimiento. Para la identificación, cribado e inclusión de los artículos se utilizó el modelo PRISMA 2020. Resultados: de un total de 838 artículos identificados, se incluyeron 20 artículos y 7 de otras fuentes como citas y otros sitios web, obteniendo un total de 27 artículos de revisión, que abordan la evolución, la aceptación y reconocimiento. Se realizó análisis cualitativo. Conclusiones: el desempeño de EPA surge por las demandas poblacionales de atención primaria a la salud, diversos países ya cuentan con EPA, asistiendo a sus habitantes desde sus especialidades. La EPA es diversa y extensa, su profesionalización debe ser continua y permanente. En México falta camino para tomar la EPA como parte del equipo multidisciplinario


Abstract Introduction: About 5.9 millions healthcare professionals lack across the world. The Advanced Practice Nursing (APN), has specific knowledge, competences, and clinical abilities to act in medical absence, taking care from acute illness until chronic diseases, preserving both mental and physical health. Objective: To determine the Advanced Practice Nursing (APN) performance in the Health Sector. Methods: A systematic review about APN, including reviews and original articles from the period 2013-2022 in Spanish and English, about performance: evolution, acceptance and recognition. For the identification, screening, and inclusion of the articles the PRISMA 2020 model was used. Results: Of 838 total identified articles, 20 articles were included from the search and 7 from other sources such as citations and other websites, obtaining 27 total articles to be reviewed, which regard the evolution, acceptance and recognition. A qualitative analysis was performed. Conclusions: The APN performance arose because of demands for primary health care; several countries already have APN, providing care to inhabitants through their different specialties. APN is diverse and extensive, its professionalization must be continuous and permanent. In Mexico there is still some way to go to APN to take part in the multidisciplinary team.

OBJETIVO: Mapear na literatura como ocorre a assistência da prática avançada de enfermagem às pessoas com doenças crônicas não transmissíveis no contexto da atenção primária à saúde. MÉTODO: Scoping review, realizada em bases de dados e fontes de literatura cinzenta pelo cruzamento dos descritores "Prática Avançada de Enfermagem"; "Atenção Primária à Saúde"; "Resultados, efeitos, impactos, consequências" em inglês. RESULTADOS: a amostra foi de 24 artigos, 10 (41,60%) enfocam a assistência de enfermagem de prática avançada em pacientes com diabetes; seis (25,00%) no campo dos pacientes com doença cardiovascular; três (12,50%) no campo dos pacientes com câncer; três (12,50%) abordam o papel da prática avançada em mais de uma doença crônica não transmissível e dois (8,30%) abordam o desempenho em pacientes com doenças respiratórias. CONCLUSÃO: identificou-se a assistência da prática avançada de enfermagem às pessoas com doenças crônicas não transmissíveis e observou-se resultados positivos na atenção das quatro doenças crônicas estudadas.


OBJECTIVE: To describe how advanced practice nursing support is provided to individuals with noncommunicable chronic diseases in primary health care through a literature review. METHOD: A scoping review was conducted using databases and grey literature sources by intersecting the terms "advanced practice nursing"; "primary health care"; "outcomes, effects, impacts, consequences" in English. Results: The sample consisted of 24 articles, of which 10 (41.60%) focused on advanced nursing practice support in patients with diabetes; six (25.00%) in the area of patients with cardiovascular disease; three (12.50%) in the area of patients with cancer; three (12.50%) addressed the role of advanced practice in more than one noncommunicable chronic disease, and two (8.30%) addressed performance in patients with respiratory disease. CONCLUSION: Advanced nursing practice support for individuals with noncommunicable chronic diseases was identified, and positive outcomes were observed in the care of the four chronic diseases studied.

OBJETIVO: Refletir sobre as ações de práticas avançadas de enfermagem no contexto das Unidades de Terapia Intensiva. MÉTODO: Trata-se de um estudo teórico-reflexivo embasado em literatura internacional, cujos artigos apontem ações do enfermeiro de prática avançada no ambiente de terapia intensiva, além do guideline de Prática Avançada de Enfermagem do International Council of Nurses. RESULTADOS: A união de informações acerca das ações de práticas avançadas de enfermagem no ambiente de terapia intensiva permitiu a reflexão que gerou nove categorias de ações: educação continuada, educação em saúde, ser referência especializada em terapia intensiva, atividade de pesquisa, gerenciamento do cuidado, liderança de equipe, cuidado com a família, atuação em equipe multidisciplinar e assistência direta ao paciente. CONCLUSÃO: A reflexão evidenciou que educação, gestão e assistência são eixos de competências encontradas nas práticas avançadas de enfermagem na terapia intensiva.


OBJECTIVE: To reflect on the actions of advanced practice nursing in the context of intensive care units. METHOD: This is a theoretical-reflective study based on international literature whose articles refer to the actions of advanced practice nurses in the intensive care environment. It is also based on the International Council of Nurses' Advanced Practice Nursing Guideline. RESULTS: The union of information about the actions of advanced practice nursing in the intensive care environment allowed a reflection that generated nine categories of actions: continuing education, patient education, being a specialized reference in intensive care, research activity, care management, team leadership, family care, multidisciplinary teamwork, and direct patient care. CONCLUSION: The reflection showed that education, management, and care are the axes of competencies in advanced practice nursing in intensive care.

OBJETIVO: descrever a prática avançada do enfermeiro em cuidados paliativos que atua com a equipe Interdisciplinar em um projeto de extensão universitária nas favelas da Rocinha e Vidigal no Rio de Janeiro, Brasil. MÉTODO: Trata-se de um estudo descritivo do tipo relato de experiência, sobre a aplicabilidade de práticas avançadas de enfermagem, no contexto de cuidados paliativos em comunidades vulneráveis, Rocinha e Vidigal, 2019 e 2020. RESULTADOS: Enfermeiros líderes, por meio da micropolítica, buscam o alívio do sofrimento humano, mediante o controle de sinais e sintomas físicos, psicossociais e espirituais, através de raciocínio clínico, habilidade de resolução de problemas por intermédio da consulta de enfermagem e articulação junto a equipe interdisciplinar. CONCLUSÃO: A abordagem de cuidados paliativos sob a ótica do projeto de comunidade compassiva tem mostrado uma forte prática da autonomia do enfermeiro, assim como proporciona visibilidade para a vulnerabilidade social e fortalecimento da prática avançada do enfermeiro no Brasil.


OBJECTIVE: to describe the advanced practice of nurses in palliative care who work with the Interdisciplinary team in a university extension project in the favelas of Rocinha and Vidigal in Rio de Janeiro, Brazil. METHOD: This descriptive research shows an experience report on the applicability of advanced practice nursing in palliative care in vulnerable communities, Rocinha and Vidigal, 2019 and 2020. RESULTS: Through micropolitics, leading nurses seek to relieve human suffering by controlling physical, psychosocial, and spiritual signs and symptoms through clinical reasoning, problem-solving skills in nursing appointments, and articulation with the interdisciplinary team. COONCLUSION: From the perspective of the compassionate community project, the palliative care approach has shown a strong practice of nurse autonomy, provided visibility for social vulnerability, and strengthened the advanced practice of nurses in Brazil.

INTRODUCCIÓN: El desarrollo de la Enfermería de Práctica Avanzada se ha transformado en una tendencia global en las últimas décadas. Desde que esta formación de posgrado emergió en Estados Unidos ha trascendido a otros continentes y países. Si bien en la región de las Américas estos programas aún tienen un carácter incipiente, la Organización Panamericana de la Salud ha realizado esfuerzos constantes para relevar su aporte al logro del acceso universal de salud y al fortalecimiento de la atención primaria de salud y a la fuerza laboral de Enfermería. OBJETIVO: Este artículo presenta una reflexión acerca de la implementación de un programa de Práctica Avanzada en Chile, analizando facilitadores y desafíos del proceso. DESARROLLO: En respuesta a las necesidades del país y al contexto, la Escuela de Enfermería UC abrió su primer programa en 2021, específicamente en el área de la oncología, enfocado a abordar las brechas de acceso y cobertura en el área. Se plantean facilitadores y desafíos de la implementación que coinciden con lo que ocurre a nivel internacional CONCLUSIONES: Las fortalezas como los desafíos identificados proporciona una clara orientación para avanzar en la implementación y permiten proyectar estrategias basadas en evidencia y con pertinencia contextual, que consideren e integren las iniciativas exitosas publicadas y los logros obtenidos.


INTRODUCTION: The development of Advanced Practice Nursing has become a global trend in recent decades. Since this postgraduate training emerged in the United States, it has spread to other continents and countries. Although in the region of the Americas these programs are still incipient, the Pan American Health Organization has made constant efforts to highlight their contribution to the achievement of universal health access and to the strengthening of primary health care and the nursing workforce. OBJECTIVE: This article presents a reflection on the implementation of an Advanced Practice program in Chile, analyzing facilitators and challenges of the process. DEVELOPMENT: In response to the needs of the country and the context, the UC School of Nursing opened its first program in 2021, specifically in oncology, focused on addressing the gaps in access and coverage in the area. Facilitators and challenges of implementation are raised that coincide with what is happening internationally CONCLUSIONS: The strengths as well as the challenges identified provide clear guidance to advance implementation and allow projecting strategies based on evidence and with contextual relevance, which consider and integrate the successful initiatives published and the achievements obtained.

PURPOSE: The growing complexity of cancer treatments requires changes in how care is organized and who provides it. The incorporation of advanced practice nursing roles within multidisciplinary teams can improve care in cancer patients. This study aims to understand the lived experience of cancer patients and multidisciplinary professionals in relation to the care provided by advanced practice nurses (APN). METHODS: Phenomenological qualitative study. Data were collected through in-depth interviews and a field diary. Participants were recruited through convenience sampling; until theoretical data saturation was achieved. An interpretative phenomenological analysis was performed, following Guba and Lincoln's criteria for trustworthiness. RESULTS: Interviews were performed with 18 professionals and 11 patients, from high-complexity public hospitals between March-December 2021. The main themes that emerged were: Advanced practice nurse role and competencies, Benefits provided by the APN, and Relevant aspects of nursing care. CONCLUSION: Advanced practice nurses play a fundamental role in cancer care, making positive contributions to the patient experience and to the multidisciplinary team's work. Elucidating the contribution of advanced practice nurses in oncology will facilitate the definition of their specific competencies and, in turn, the implementation of training and management strategies to consolidate this figure in specialized centers.

OBJECTIVE: To identify the competency profile of advanced practice nurses involved in the care process of cancer patients. METHODS: Cross-sectional and descriptive study. The study included all nurses involved in the cancer patient care process in a tertiary hospital in Barcelona. Competence profile data were collected using the instrument for defining the role of the advanced practice nurse (APRD), as well as sociodemographic and occupational variables. Sociodemographic and occupational data were compared against the performance of advanced practice activities. RESULTS: A total of 29 (82.9%) nurses participated with a mean age of 42.6±12.54 years. 9 (31%) nurses were identified as meeting the standard in all 6 domains on the APRD scale to be considered advanced practice nurses. Of these 9 (31%) nurses, 7 (24.1%) met the training standards required by the International Council of Nurses (ICN) with an official master's degree and 2 (6.9%) with a PhD. CONCLUSIONS: There are nurses who carry out their activity in the oncology field of the hospital analyzed with the EPA profile. The identification of advanced practice nurses (APNs) in our health system is essential to be able to recognize the competencies of these professionals and create specific positions that help to address chronicity, patients' quality of life, their survival, and the optimization of health resources. Our study highlights the importance of chronicity and cancer as areas for the development of the APNs.

BACKGROUND: Advanced practice nurse (APN) roles bring great added value to health systems. However, their integration into the health workforce and the sustainability of the role depend on contextual factors surrounding their implementation. AIM: To explore the contextual factors that influence the organization, implementation, and performance of clinical practice among oncology APNs in Catalonia (Spain). METHODS: This is a descriptive qualitative study. A framework of contextual factors was applied to explore the perspectives of 14 oncology APNs in public hospitals in Catalonia by means of semistructured interviews. Data were analyzed according to the thematic analysis approach. The COREQ checklist was used to report the study. RESULTS: APNs in cancer care strongly depend on the hospital environment where they are introduced. Recognition by the multidisciplinary team, the existence of mentoring experiences, and networking between APNs are critical factors that can help or hinder the development and autonomy of the APNs. Likewise, support from nursing managers and directors is decisive in defining the professional profile, establishing accountability mechanisms, and securing financial resources, including economic recognition. Factors related to the external environment can also contribute, including a standardized national APN model and scientific societies. CONCLUSIONS: Contextual factors around clinical practice, institutional structures, and professional networks are crucial determinants for adequately integrating APNs at the health system level. IMPLICATIONS FOR NURSING POLICY: Professional bodies and national nursing organizations should lay the groundwork for defining standards of practice and advocate for specific regulations. In addition, financial recognition and accountability mechanisms to assess the impact of their contribution should be a priority to ensure sustainability and APN satisfaction.

The specialty of oncology nursing has been evolving in North America for nearly a century, keeping pace with the rapid and dynamic developments in cancer care. This narrative review outlines the history and development of oncology nursing in North America with a focus on the United States and Canada. The review highlights the important contributions that specialized oncology nurses have made to the care of people affected by cancer from time of diagnosis through treatment, follow-up and survivorship care, as well as palliative, end-of-life, and bereavement care. Keeping pace with the rapid evolution of cancer treatments throughout the last century, nursing roles have similarly evolved to meet the need for more specialized training and education. This paper discusses the growth of nursing roles, including advanced practice and navigator roles. In addition, the paper outlines the development of professional oncology nursing organizations and societies that have been established to help guide the profession with best practices, standards, and competencies. Finally, the paper discusses new challenges and opportunities regarding the access, availability, and delivery of cancer care that will shape future development of the specialty. Oncology nurses will continue to be integral to the provision of high-quality, comprehensive cancer care as clinicians, educators, researchers, and leaders.

PURPOSE: Exploring the persisting presence of underlying processes, dynamics, experienced barriers and facilitators of Advanced Practice Nurses (APN) in oncology and Oncology Nurse Navigators (ONN) during their role integration in an interprofessional team over a research period of seven years. METHODS: A qualitative study based on the principles of Grounded Theory, in which individual semi-structured interviews and focus groups were conducted with 51 ONN and APN from 11 university and local hospitals in Belgium between 2011-2018. Data were analyzed to develop a framework that consists of different themes. RESULTS: ONN and APN experienced a lonely journey during role integration. They were searching for partners, medical knowledge and acknowledgement from the interprofessional team. ONN and APN had a watchful attitude and tried to make themselves visible to ensure they were involved in the team and to preserve their role and responsibilities. An unclear role description to the interprofessional team, and a lack of coaching and guidance were influencing factors in the experience of ONN and APN. CONCLUSION: ONN and APN in oncology feel difficulties to integrate their role in existing interprofessional teams. A lack of role clarity, the interprofessional team environment and a desire for coaching and guidance are influencing factors to implement APN roles. Coaching and mentorship of novice ONN/APN and their interprofessional team and healthcare managers are needed to address these issues. These findings could form the basis for a mentorship program for ONN/APN and their interprofessional team to enhance role integration.

Care provided by an advanced practice nurse: Experiences of patients with sarcoma and family members. A qualitative study Abstract. Background: Sarcomas are a rare, heterogeneous group of malignant tumors with different trajectories, which cause significant burden to patients and families. Due to the complex nature of treatment, an interprofessional team at the sarcoma center of a Swiss university hospital provides care to affected individuals. This interprofessional team includes an advanced practice nurse (APN) who cares for patients and family members throughout the trajectory of the disease. To date, there are limited descriptions within literature of APN care from the perspective of patients with sarcoma and their family members. Aim: To investigate how patients with sarcoma and their family members experienced APN care. Methods: The study was guided by the qualitative research methodology "Interpretive Description". Individual interviews with seven patients and five family members were conducted and analyzed in an iterative process. Results: For patients and family members, the time from diagnosis to therapy and follow-up was very stressful. They experienced the APN's care during this time as a great support and described her as a compassionate, trustworthy and continuous contact person who expertly provided information and advice while acting as a coordinator. Conclusions: Patients with sarcoma and their family members require continuous contact with a person who is compassionate, while also professional, confident and competent. Providing APN care can meet all of these essential requirements.
