PURPOSE: To report cataract surgical coverage (CSC) and effective Cataract Surgical Coverage (eCSC) from four districts in Telangana, India. METHODS: Individuals aged ≥40 years were recruited from Adilabad, Mahbubnagar, Warangal and Khammam districts using two stage cluster sampling methodology. All participants had detailed eye examinations including visual acuity assessment using a standard Snellen chart at a distance of six meters, anterior segment examination and fundus imaging by trained professionals. CSC and eCSC were calculated for persons using a 6/12 visual acuity cut-off. RESULTS: Of 12,150 individuals enumerated from four districts 11,238 (92.5%) were examined. The mean (standard deviation) age of the participants was 54.1 (±11.2) years (range: 40 to 102 years) and 54.6% (n = 6137) were women. Overall, the CSC (%) was 54.2%. It ranged from a highest of 59% in Khammam followed by 57.5% in Adilabad, 51.7% in Warangal, and a least of 49.7% in Mahbubnagar district. Overall, the eCSC (%) was 39.9%. It ranged from a highest of 46.1% in Adilabad followed by 43.2% in Khammam, 36.2% in Warangal district and a least of 35.8% in the Mahbubnagar district. CONCLUSIONS: CSC and eCSC varied across the districts. A significant gap between CSC and eCSC is noted suggestive of challenges with quality of services. District-wise planning of cataract surgical services with a focus on quality care is recommended to improve coverage and contribute towards achieving the goal of universal eye health coverage in the Telangana State in India.

INTRODUCTION: Diabetes is a serious public health problem, with low- and middle-income countries (LMICs) bearing over 80% of the burden. Diabetic retinopathy (DR) is one of the most prevalent diabetic microvascular problems, and early diagnosis through eye screening programs for people with diabetes is critical to prevent vision impairment and blindness. Community-based treatments, including non-physician cadres have been recommended to enhance DR care. METHODS: The review protocol was determined and scoping review was conducted.The population, concept, and context were "cadre", "role of cadre in the management of DR", and LMICs". Data were collected from databases and searches, including grey literature. RESULTS: Cadre can motivate people to attend a diabetic eye screening event when the rate of eye examinations is about six times higher than before the start of the intervention. Health education is a possible area for task sharing, and the cadre reported could also perform the task of vision testing. The cadre could be a good supporter and a good reminder for society. However, several challenges have been faced in this study and inadequate infrastructure is the foremost challenge found in this study. Other challenges encountered in the studies include poverty, lack of community awareness, trust issues, and low education levels contributing to poor health. CONCLUSION: The current study highlighted significant gaps in the literature, which focus on the role of cadre as a community-based intervention in managing DR in LMICs. Further research is needed to develop evidence to support cost-effective screening services and cadre-related policy development in LMICs.

Purpose: To describe tele-retinal abnormality image findings from the Manhattan Vision Screening and Follow-up Study (NYC-SIGHT), which aims to investigate whether community-based eye health outreach strategies using telemedicine can improve visual outcomes among at-risk populations in Upper Manhattan. Methods: A 5-year prospective, cluster-randomized clinical trial was conducted. Eligible individuals aged 40 years and older were recruited from affordable housing developments and senior centers in New York City. Participants underwent on-site eye health screening (best-corrected visual acuity, intraocular pressure [IOP] measurements, and fundus photography). Fundus images were graded via telemedicine by a retina specialist. Multivariate logistic regression modeling was used to assess the factors associated with abnormal retinal findings requiring referral to ophthalmology. Results: Participants with a retinal abnormality on fundus photography (n = 157) were predominantly older adults, with a mean age of 68.4 ± 11.1 years, female (63.7%), African American (50.3%), and Hispanic (43.3%). A total of 32 participants in our study passed the vision and IOP screening but had an abnormal retinal image and ocular pathology that would have been missed without fundus photography. Individuals who self-identified as having preexisting glaucoma (odds ratio [OR] = 3.749, 95% confidence interval [CI] = 1.741-8.074, p = 0.0001) and had severe vision impairment (OR = 4.1034, 95% CI = 2.0740-8.1186, p = 0.000) at the screening had significantly higher odds of having an abnormal retinal image. Conclusion: This community-based study targeted populations at-risk for eye disease, improved access to eye care, detected a significant number of retinal image abnormalities requiring follow-up by using telemedicine, and provided evidence of the importance of fundus photography during eye health screenings. PURPOSE: The Manhattan Vision Screening and Follow-up Study aims to provide access to eye care for underserved populations, detect native rates of ocular pathology, and refer participants with eye disease to ophthalmology. This subanalysis describes the reasons for referral to ophthalmology and identifies risk factors associated with being referred. METHODS: Enrolled participants were aged ≥40 years, living independently in public housing developments and able to provide consent for eye health screenings. Those with habitual visual acuity 20/40 or worse, intraocular pressure (IOP) 23-29 mmHg, or an unreadable fundus image failed and were scheduled with the on-site optometrist. The optometric exam determined whether further referral to ophthalmology for a clinic exam was warranted. Those with an abnormal image or IOP ≥30 mmHg were referred directly to ophthalmology. Main outcome was factors associated with referral to ophthalmology. RESULTS: A total of 708 individuals completed the eye health screening over 15 months. A total of 468 participants were referred to ophthalmology (250 had an abnormal image and 218 were referred by the optometrist). Those referred were predominantly older adults (mean age 70.0 ± 11.4 years), female (66.7%), African American (55.1%) and Hispanic (39.5%). Seventy percent of participants had not had a recent eye exam. Stepwise multivariate logistic regression analysis showed that participants with pre-existing glaucoma (OR 3.14, 95% CI 1.62 to 6.08, p = 0.001), an IOP ≥23 mmHg (OR 5.04, 95% 1.91 to 13.28, p = 0.001), or vision impairment (mild) (OR 2.51, 95% CI 1.68 to 3.77, p = 0.001) had significantly higher odds of being referred to ophthalmology. CONCLUSION: This targeted community-based study in Upper Manhattan provided access to eye care and detected a significant amount of ocular pathology requiring referral to ophthalmology in this high-risk population.

ABSTRACT Purpose: As digital devices are increasingly used at work, valid and reliable tools are needed to assess their effect on visual health. This study aimed to translate, cross-culturally adapt, and validate the Computer Vision Syndrome Questionnaire (CVS-Q©) into Portuguese. Methods: A 5-phase process was followed: direct translation, synthesis of translation, back-translation, consolidation by an expert committee, and pretest. To run the pretest, a cross-sectional pilot study was conducted with 26 participants who completed the prefinal Portuguese version of the CVS-Q© and were asked about difficulties, comprehensibility, and suggestions to improve the questionnaire. To evaluate the reliability and validity of the Portuguese version of the CVS-Q©, a cross-sectional validation study was performed in a different sample (280 workers). Results: In the pretest, 96.2% had no difficulty in completing it, and 84.0% valued it as clear and understandable. CVS-Q© in Portuguese (Questionário da Síndrome Visual do Computador, CVS-Q PT©) was then obtained. Validation revealed the scale's good internal consistency (Cronbach's alpha=0.793), good temporal stability (intraclass correlation coefficient=0.847; 95% CI 0.764-0.902, kappa=0.839), adequate sensitivity and specificity (78.5% and 70.7%, respectively), good discriminant capacity (area under the curve=0.832; 95% CI 0.784-0.879), and adequate convergent validity with the ocular surface disease index (Spearman correlation coefficient=0.728, p<0.001). The factor analysis provided a single factor accounting for 37.7% of the explained common variance. A worker who scored ≥7 points would have computer vision syndrome. Conclusions: CVS-Q PT© can be considered an intuitive and easy-to-understand tool with good psychometric properties to measure computer vision syndrome in Portuguese workers exposed to digital devices. This questionnaire will assist in making decisions on preventive measures, interventions, and treatment and comparing exposed populations in different Portuguese-speaking countries.


RESUMO Objetivos: À medida que a utilização de equipamentos digitais no emprego aumenta, a avaliação do seu efeito na saúde visual necessita de ferramentas válidas e robustas. Este estudo teve como objetivo traduzir, adaptar culturalmente e validar para português o Questionário da Síndrome Visual do Computador (CVS-Q©). Métodos: O procedimento foi realizado em 5 fases: tradução direta, síntese da tradução, tradução inversa, consolidação por um painel de especialistas, e pré-teste. Para fazer o pré-teste foi realizado um estudo piloto transversal aplicado a uma amostra de 26 participantes que completaram a versão pré-final da versão portuguesa do CVS-Q©, questionando por dificuldades, compreensão e sugestões de melhoria do questionário. Para avaliar a confiança e validade da versão portuguesa do CVS-Q© foi realizado um estudo transversal de validação em uma amostra diferente (280 funcionários). Resultados: No préteste, 96.2% dos participantes não apresentaram dificuldades no preenchimento do questionário, enquanto 84.0% indicaram que era claro e compreensível. Obteve-se, então, o CVS-Q© em português (Questionário da Síndrome Visual do Computador, CVS-Q PT©). A sua validação revelou uma boa consistência interna da sua escala (Cronbach's alpha=0.793), boa estabilidade tem poral (coeficiente de correlação interclasse=0.847; 95% CI 0.764-0.902, kappa=0.839), sensibilidades e especificidades adequadas (78.5% e 70.7%, respetivamente), boa capacidade de discriminação (área abaixo da curva=0.832; 95% CI 0.784-0.879), e uma adequada validade da convergência com o índice de doença da superfície ocular (ocular surface disease index - OSDI; coeficiente de correlação de Spearman=0.728, p<0.001). A análise fatorial revelou um único fator responsável por explicar a variância comum em 37.7%. Um funcionário com uma pontuação ≥7 pontos sofria de síndrome visual do computador. Conclusão: O CVS-Q PT© pode ser considerada uma ferramenta intuitiva, de fácil interpretação e com boas pro priedades psicométricas para avaliar a síndrome visual do computador em funcionários portugueses expostos a ecrãs digitais. Este questionário facilitará as decisões sobre medidas preventivas, intervenções e tratamento, e a comparação entre as populações expostas em diferentes países de língua portuguesa.

ABSTRACT This case report identified paracentral acute middle maculopathy as the cause of severe and irreversible vision loss after cataract surgery. Cataract surgeons should be aware of known risk factors for the development of paracentral acute middle maculopathy. In those patients, extra care regarding anesthesia, intraocular pressure, and some other aspects of cataract surgery must be taken. Paracentral acute middle maculopathy is currently understood as a clinical sign evident on spectral-domain optical coherence tomography, and it is probably evidence of deep ischemic insult to the retina. It should be a differential diagnosis in cases of marked low vision acuity associated with no fundus abnormalities in the immediate postoperative period, as demonstrated in the presented case.


RESUMO O presente relato de caso identificou a maculopatia média aguda paracentral como a causa de baixa de acuidade visual severa e irreversível após cirurgia de catarata. Existem fatores de risco bem estabelecidos para o desenvolvimento da maculopatia média aguda paracentral que devem ser conhecidos pelos cirurgiões de catarata. Nesse contexto cirúrgico, precauções extras no tocante a procedimentos anestésicos, pressão intraocular e alguns outros aspectos da cirurgia devem ser consideradas. A maculopatia média aguda paracentral é descrita como um sinal clínico observado no exame de tomografia de coerência óptica por domínio espectral e se trata, provavelmente, da evidência de um evento isquêmico no tecido vascular retiniano. Esse diagnóstico deve ser cogitado nos casos de perda de acuidade visual súbita no pós-operatório imediato associada com exame fundoscópico normal, como evidenciado no caso apresentado.

RESUMO Introdução: A visão é um dos sentidos que mais integra o indivíduo ao meio ambiente, pois capta a maior parte dos estímulos sensoriais aos quais o ser humano está exposto, e é justamente essa integração que permite a criação de memórias e a formação do conhecimento. A baixa visão na infância prejudica o aprendizado, a socialização e também a qualidade visual na idade adulta. Objetivo: Identificar a acuidade visual de crianças escolares, utilizando a tabela de Snellen e encaminhar e realizar intervenções necessárias naquelas com baixa visão. Métodos: Estudo transversal, com abordagem quantitativa. Utilizou-se a tabela de Snellen para determinar a acuidade visual de alunos do 1º ano do ensino fundamental da rede estadual de ensino. Resultados: Os participantes tem idade de 5 a 7 anos, sendo 40 do sexo feminino e 38 do sexo masculino. Destes, 23 apresentavam baixa acuidade visual e foram encaminhados para consulta oftalmológica. Conclusão: Com base nos resultados do presente estudo, sugere-se que a aplicação do teste de Snellen, por ser um teste de triagem de fácil treinamento e baixo custo, torne-se útil na promoção de saúde ocular. Sugere-se a importância da continuidade de estudos como este, visando na sensibilidade e na especificidade, com foco nas crianças escolares.


ABSTRACT Introduction: Vision is one of the senses that most integrates the individual with the environment, since it captures most of the sensory stimuli to which the human being is exposed, and it is precisely this integration that allows the creation of memories and the formation of the knowledge. Low vision in childhood impairs learning, socialization, and also determines visual quality in adulthood. Objective: to identify the visual acuity of school children using the Snellen Test and refer and carry out actions with those with low vision. Methods: Cross-sectional study with a quantitative approach. The Snellen Table was used to determine the visual acuity of students in the 1st year of elementary school in the state education network. Results: Participants are aged 5-7 years, 40 females and 38 males. Of these, 23 had low visual acuity and were referred to an ophthalmological consultation. Conclusion: Based on the results of the present study, the application of the Snellen Test is suggested, as it is an easy-to-train, low-cost screening test, so that it become useful in promoting eye health. Continuing studies like this one are important and target sensitivity and specificity, with a focus on schoolchildren.

Objetivo: Identificar los determinantes sociales relacionados a la salud ocular en pobladores adultos de una comunidad de Ica 2022. Materiales y métodos: Se desarrolló una investigación con enfoque cuantitativo de corte transversal, correlacional, donde se incluyó una muestra de 108 pobladores adultos del sector de Santa Bárbara del distrito de Tinguiña en Ica, obtenida por muestreo probabilístico; utilizando dos instrumentos elaborados por las autoras, cada uno fue sometido a validación (p: 0.031) y prueba de confiabilidad (α=0.81; KR-20: 0.83). Resultados: Se identificó como factor biológico sexo femenino en 69.4%, la edad promedio fue de 51.84 ±15.56; el 75% no refiere antecedentes familiares, dentro de los estilos de vida el 94% se expone a una pantalla electrónica, el 65% consume alimentos que protegen la visión y el 62% no toma en cuenta la distancia e iluminación adecuada para la lectoescritura; el 91% refirió recibir una atención sanitaria inadecuada, el 71% afirma que su situación económica no les permite atender su salud, 80% no accede a una consulta oftalmológica, ni cubren su tratamiento; asimismo, el 94% de los pobladores no cuidan su salud ocular. Conclusión: Los determinantes sociales de la salud ocular identificados corresponden a la deficiente situación socioeconómica (rs = 0.212; p: 0.027); el ingreso familiar deficiente para la atención de salud (rs = 0.25; p: 0.009); no cubrir la consulta oftalmológica (rs = 0.24; p: 0.012); ni el tratamiento que esta genera (rs = 0.213; p: 0.02). Objective: To identify the social determinants associated with eye health in adult residents of a community in Ica 2022. Materials and methods: An investigation with a cross-sectional quantitative approach was developed, correlational, which included a sample of 108adult residents of the Santa Bárbara sector of the Tinguiña district in Ica, obtained by probabilistic sampling; using two instruments developed by the authors, each one was subjected to validation (p: 0.031) and reliability test (α=0.81; KR-20: 0.83). Results: Female sex was identified as a biological factor in 69.4%, the average age was 51.84 ±15.56; 75% do not refer family history, within lifestyles, 94% are exposed to an electronic screen, 65% consume foods that protect vision; 65% consume foods that protect vision and 62% do not take into account the distance and adequate lighting for reading and writing; 91% reported receiving inadequate health care, 71% affirm that their economic situation does not allow them to take care of their health, 80% do not access an ophthalmological consultation, nor do they cover their treatment; Likewise, 94% of the inhabitants do not take care of their eye health. Conclusion: The social determinants of ocular health identified correspond to the deficient socioeconomic situation (rs = 0.212; p: 0.027); deficient family income for health care (rs = 0.25; p: 0.009) and does not cover ophthalmology consultation (rs = 0.24; p: 0.012); nor the treatment that it generates (rs = 0.213; p: 0.02). OBJECTIVE: As with any healthcare practice, elective surgeries had to be postponed since the start of the Covid-19 pandemic. This study aimed to examine the characteristics of ophthalmology outpatients and eye surgery admissions during the COVID-19 pandemic and also to compare the pandemic and pre-pandemic periods. METHODS: This retrospective study included patients admitted to the ophthalmology clinic of a tertiary hospital from April through June 2020. A control sample was formed using the registries from the same interval in the previous year. The primary endpoint was the difference between the number and distribution of types of surgical procedures in the pre-pandemic and pandemic period. Surgical procedures were classified as Group A; major special, Group B; special, Group C; major, Group D; medium, and Group E; minor surgeries. Also surgeries were also divided into 4 groups. Cataract and related surgeries were grouped as "Phaco", emergency surgeries for trauma patients as "Trauma", retina and related surgeries were grouped as "Retina", and eyelid and adnexal surgeries were grouped as "Eyelid". The secondary endpoint was the comparison between the pre-pandemic and pandemic period. RESULTS: A total of 116 operations were performed in 2020 (mean age: 42.3 ± 25.6 years, male: 63.8%). In 2019, 873 surgeries were performed in the same period of the year (mean age: 56.6 ± 20.2 years, male: 48.8%), indicating an 86.7% decrease during the pandemic period, and each surgery type reduced significantly. On the other hand, the proportion of Group A (10.3% in 2019 vs. 25.9% in 2020, p < 0.001), group B (5.4% in 2019-17.24% in 2020, p < 0.001), and group E (3.8% in 2019-8.6% in 2020, p < 0.001) surgeries among the total increased in the pandemic period. The proportion of trauma (3.1% in 2019-16.4% in 2020, p < 0.001) and retina (11.9% in 2019-37.1% in 2020, p < 0.001) surgeries also increased, whereas phaco and eyelid surgeries were recorded at a lesser rate in the pandemic period. CONCLUSION: This study showed a striking reduction in eye surgery during the early period of the Covid-19 pandemic. However, the rates of group A, B, and E surgeries increased significantly compared to the previous year.

Background: Repeated infection with Chlamydia trachomatis causes trachomatous trichiasis (TT). Surgery is the main and preferred method of treatment. However, many people decline surgery despite the availability of free services in nearby health facilities. Aimz: To identify the determinants of surgery refusal among TT patients in Ethiopia. Methods: This community-based, case-control study was conducted among 338 cases and 338 controls from 5 October to 17 December 2018. Using systematic random sampling, we selected people who had been operated on (controls) and those who refused surgery (cases) from registration documents and used a pre-tested, interviewer-administered, structured questionnaire for data collection. We used SPSS version 23 to analyse the data and used multivariate logistic regression to identify the determinants. Results: Having witnessed a poor surgical outcome [adjusted odds ratio (aOR): 3.51, 95% CI: 1.94-6.35] and lack of knowledge about TT (aOR: 1.77, 95% CI: 1.18-2.65) increased the refusal rate for surgery. Having trust in the surgeon (aOR: 0.26, 95% CI: 0.15-0.45), knowledge about eyelid surgery (aOR: 0.32, 95% CI: 0.16-0.64), long duration of trichiasis (aOR: 0.50, 95% CI: 0.31-0.79), decision-making via discussion with the family (aOR: 0.29, 95% CI: 0.13-0.64), frequent epilation (aOR: 0.31, 95% CI: 0.17-0.60), and receiving personal advice (aOR: 0.11, 95% CI: 0.04-0.28) reduced refusal rates. Conclusion: Refusing to have TT surgery was significantly related to knowledge about upper eyelid surgery, past surgical outcomes, decision-making capacity, and personal influences. Improved systems for upper eyelid surgery should be established in Ethiopia to better manage, and reduce unfavourable, surgical outcomes, and reduce surgery refusal.

RESUMO Objetivo Avaliar e caracterizar a qualidade da assistência oftalmológica ofertada em uma unidade de serviços ambulatoriais da Atenção Secundária do Sistema Único de Saúde na percepção dos usuários e/ou dos responsáveis por esses usuários. Métodos Foi realizado estudo seccional de prevalência analítica entre usuários assistidos no período de setembro de 2019 a março de 2020 em um dos seis serviços de assistência oftalmológica ofertado pelo Sistema Único de Saúde, no município de Vitória da Conquista (BA). Foram avaliadas as condições demográficas e socioeconômicas e a autopercepção da saúde. Também foram avaliadas as características e os níveis de qualidade da assistência à saúde oftalmológica. A coleta das informações ocorreu por meio de entrevista telefônica. Resultados Participaram do estudo 389 usuários e/ou responsáveis. Observaram-se elevados níveis de satisfação, considerando as perguntas que compõem o constructo do instrumento. Identificaram-se níveis de qualidade comprometidos em relação à liberdade de escolher seu prestador de cuidado ocular (154; 39,7%). Menores níveis de satisfação associaram-se com as variáveis sexo masculino (razão de chance de 1,66; IC95% 1,04-2,65; p=0,033) e raça autodeclada branca (razão de chance de 6,89; IC95% 4,00-11,86; p=0,000). Conclusão Os usuários relataram bons níveis de satisfação da assistência à saúde oftalmológica. Observaram-se maiores chances de menores níveis de satisfação entre homens e aqueles com raça autodeclarada branca.


ABSTRACT Objective To evaluate and characterize the quality of ophthalmic care offered in an outpatient service unit of the SUS secondary care according to the perception of users and/or those responsible for these users. Methods A cross-sectional study of analytical prevalence was carried out among users assisted from September 2019 to March 2020, in one of the six ophthalmic care services offered by the SUS, in the municipality of Vitória da Conquista (BA), Brazil. Demographic and socioeconomic conditions, as well as self-perception of health, were evaluated. The characteristics and quality levels of ophthalmic health care were also evaluated. Data collection took place through a telephone interview. Results A total of 389 users and/or guardians participated in the study. Participants reported high levels of satisfaction, considering the questions that make up the instrument's construct. Compromised quality levels were observed in relation to the freedom to choose their eye care provider (n=154; 39.7%). Lower levels of satisfaction were associated with the variables male gender (OR=1.66; 95%CI=1.04-2.65; p=0.033) and self-reported white race (OR=6.89; 95%CI=4, 00-11.86; p=0.000). Conclusion Users reported good levels of satisfaction with eye health care. There were greater chances of lower levels of satisfaction among men and among those with a self-reported white race.

RESUMO Objetivo Avaliar a percepção e as atitudes da população brasileira a respeito dos cuidados e da atenção com a saúde ocular. Métodos Realizou-se um estudo transversal. A amostra foi aleatória e calculada para ser representativa da população brasileira. A coleta dos dados foi feita por empresa especializada em levantamentos populacionais, por meio de contato telefônico, por equipe treinada. O questionário foi estruturado com perguntas objetivas sobre os hábitos com a saúde ocular dos entrevistados. Todas as entrevistas foram gravadas e revisadas. Resultados A população de estudo foi de 2.132 pessoas. O perfil da amostra foi semelhante ao da população brasileira. Entre os entrevistados, 11,4% nunca tinham ido ao oftalmologista, 35% só o procuravam quando havia algum sintoma ocular ou visual, e 29,5% foram ao oftalmologista há mais de 2 anos. Pouco mais da metade dos entrevistados (55,8%) declarou ter algum problema de visão, e a miopia foi autodeclarada por quase metade dos entrevistados (43,2%), com proporção ainda maior nos jovens e naqueles com maior escolaridade. Dos entrevistados, 11,0% relataram já ter comprado óculos de grau sem prescrição médica, e 9,8% faziam ou já tinham feito uso de colírios sem prescrição médica, com proporção mais elevada nos autodeclarados indígenas e naqueles com menor escolaridade. Catarata e glaucoma apareceram como os problemas visuais mais frequentes na população acima de 50 anos. O tabagismo foi o principal fator de risco para o autodeclarado problema visual. Conclusão Obteve-se, nesta pesquisa, um retrato dos hábitos com relação à saúde ocular da população brasileira, que aponta para uma realidade em que parte dos brasileiros nunca foi ao oftalmologista e fazia uso de óculos e colírios sem a devida prescrição médica. Identificaram-se, ainda, os grupos populacionais mais propensos para problemas visuais e as populações mais vulneráveis e com maior dificuldade de acesso ao oftalmologista.


ABSTRACT Objective To evaluate the perception and attitudes of the Brazilian population regarding eye health care. Methods A cross-sectional study was carried out. The sample was random and calculated to be representative of the Brazilian population. Data collection was carried out by a specialized company, through telephone calls, by trained staff. The questionnaire was structured with objective questions about the eye health habits of the interviewees. All interviews were recorded and reviewed. Results The study population was 2132 people. The sample profile was similar to that of the Brazilian population. Among those interviewed, 11.4% had never been to an ophthalmologist, 35% only went to see one when facing an eye or visual symptom, and 29.5% had been to an ophthalmologist for more than 2 years. Just over half of the interviewees (55.8%) declared that they had a vision problem and myopia was self-declared by almost half of the interviewees (43.2%), with an even higher proportion among young people and those with higher education. Of those interviewed, 11.0% reported having already purchased prescription glasses without a medical prescription and 9.8% use or have used eye drops without a medical prescription, with a higher proportion among self-declared indigenous people and those with less education. Cataracts and glaucoma appeared as the most frequent visual problems in the population over 50 years old. Smoking was the main risk factor for self-declared visual problems. Conclusion In this research, we obtained a portrait of the eye health habits and attitudes of the Brazilian population, which points to a reality in which a great number of Brazilians have never been to the ophthalmologist and used glasses and eye drops without a proper medical prescription. The population groups most prone to visual problems and the most vulnerable populations with greater difficulty in accessing the ophthalmologist were also identified.

Objetivo: avaliar os fatores clínicos preditores para o ressecamento ocular e para o diagnóstico de enfermagem Risco de ressecamento ocular em pacientes no pós-operatório em Unidade de Recuperação Pós-Anestésica. Método: estudo transversal realizado entre maio e agosto de 2017. Amostra foi de 157 pacientes adultos submetidos a procedimentos cirúrgicos eletivos. Os dados foram analisados mediante estatística descritiva e inferencial. Resultados: o diagnóstico clínico de ressecamento ocular prevaleceu em 85,35% dos pacientes, e o diagnóstico de enfermagem Risco de ressecamento ocular em 14,65%. Observou-se diferenças estatísticas entre a hiperemia, lacrimejamento excessivo e teste de Schirmer insuficiente com o diagnóstico de ressecamento ocular no olho direito e com o diagnóstico Risco de ressecamento ocular. No olho esquerdo o teste de Schirmer foi insuficiente. Conclusão: a hiperemia, o lacrimejamento excessivo e o teste de Schirmer insuficiente são fatores clínicos preditores relevantes no perioperatório para promover medidas preventivas e/ou detectar precocemente o ressecamento ocular.


Objetivo: evaluar los predictores clínicos de ojo seco y el diagnóstico de enfermería Riesgo de ojo seco en pacientes postoperados en la Unidad de Recuperación Postanestésica. Material y método: estudio transversal realizado entre mayo y agosto de 2017. La muestra fue de 157 pacientes adultos sometidos a procedimientos quirúrgicos electivos. Los datos se analizaron mediante estadística descriptiva e inferencial. Resultados: el diagnóstico clínico de ojo seco predominó en el 85,35% de los pacientes, y el diagnóstico de enfermería Riesgo de ojo seco en el 14,65%. Se observaron diferencias estadísticas entre la hiperemia, el lagrimeo excesivo y el test de Schirmer insuficiente con el diagnóstico de sequedad ocular en el ojo derecho y con el diagnóstico Riesgo de ojo seco. En el ojo izquierdo el test de Schirmer fue insuficiente. Conclusión: la hiperemia, el lagrimeo excesivo y el test de Schirmer insuficiente son predictores clínicos perioperatorios relevantes para promover medidas preventivas y/o la detección precoz del ojo seco.


Objective: to evaluate the clinical predictors for dry eye and the nursing diagnosis Risk of dry eye in postoperative patients in the Post-Anesthesia Care Unit. Method: cross-sectional study conducted between May and August, 2017. The sample was 157 adult patients undergoing elective surgical procedures. Data were analyzed using descriptive and inferential statistics. Results: the clinical diagnosis of dry eye prevailed in 85.35% of patients, and the nursing diagnosis Risk of dry eye in 14.65%. Statistical differences were observed between hyperemia, excessive tearing and insufficient Schirmer test with the diagnosis of ocular dryness in the right eye and with the diagnosis Risk of dry eye. In the left eye the Schirmer test was insufficient. Conclusão: hyperemia, excessive tearing and insufficient Schirmer test are relevant perioperative clinical predictors to promote preventive measures and/or early detection of dry eye.

ABSTRACT Purpose: To analyze the epidemiological profiles of evisceration and enucleation cases in the ophthalmologic emergency department of a Brazilian tertiary hospital. Methods: Patients treated in the ophthalmologic emergency department of Hospital São Paulo (Universidade Federal de São Paulo) during the period 2013 to 2018 were retrospectively evaluated. Urgent cases of evisceration or enucleation surgery were included, and elective cases were excluded. The following information was extracted from the patients' medical records: demographic data, immediate and associated reasons for the surgical procedure, informed visual acuity, symptom duration until ophthalmologic care, complications, distance from the residence to the tertiary hospital, and time of hospitalization. Results: In total, 61 enucleations and 121 eviscerations were included in this study. The patients had a mean age of 63.27 ± 18.68 years. Of the patients, 99 were male (54.40%), and 83 were female (45.60%). The indications for evisceration or enucleation were corneal perforation with (44.50%) and without (23.63%) signs of infection, endophthalmitis (15.38%), ocular trauma (14.29%), neoplasia (0.55%), burn accident (1.10%), and phthisis bulbi (0.55%). The self-reported visual acuity was no light perception (87.36%) or light perception (1.10%). However, 3.30% of the patients did not cooperate with the examination, and no information on visual acuity was available for the remaining 8.24%. The mean symptom duration before ophthalmologic care was sought was 18.32 days. Two patients had sympathetic ophthalmia after evisceration. Conclusions: More eviscerations than enucleations were performed throughout the study period. The most common demographic characteristics were age >60 years and male sex. The main indications for urgent evisceration and enucleation procedures were corneal perforation with and without infection, endophthalmitis, and ocular trauma. The findings from this study could guide clinicians in performing preventive measures to avoid destructive eye procedures.


RESUMO Objetivo: Analisar o perfil epidemiológico dos casos de evisceração e enucleação no pronto-socorro oftalmológico de um hospital terciário brasileiro. Métodos: Análise retrospectiva dos casos tratados no pronto-socorro oftalmológico do Hospital São Paulo (Universidade Federal de São Paulo) entre os anos de 2013 a 2018. Os casos urgentes de evisceração e enucleação foram incluídos e os casos eletivos foram excluídos. A análise dos prontuários médicos foi baseada em: dados demográficos, causas imediatas e associadas ao procedimento, acuidade visual informada, duração dos sintomas antes do atendimento oftalmológico, complicações, distância da residência até o hospital e tempo de hospitalização. Resultados: 61 enucleações e 121 eviscerações foram incluídas no estudo. Os pacientes tinham uma média de idade de 63,27 ± 18,68 anos; 99 eram do sexo masculino (54,50%) e 83 do sexo feminino (45,60%). As indicações de evisceração e enucleação foram: perfuração corneana com (44,50%) e sem (23,63%) sinais infecciosos, endoftalmite (15,38%), trauma ocular (14,29%), neoplasia (0,55%), queimadura (1,10%) e phthisis bulbi (0,55%). A acuidade visual informada foi de ausência de percepção luminosa (87,36%), percepção luminosa (1.10%), ausência de colaboração (3,30%) e sem dados informados (8,24%). A média de tempo até a busca pelo serviço oftalmológico foi de 18,32 dias. Houve 2 casos de oftalmia simpática após evisceração. Conclusões: Eviscerações foram predominantemente realizadas em comparação a enucleações em todo o período de estudo. As características demográficas mais comuns foram idade >60 anos e sexo masculino. As principais indicações para procedimentos urgentes de evisceração e enucleação foram perfuração corneana com e sem infecção, endoftalmite e trauma ocular. Este estudo poderia guiar medidas preventivas para evitar procedimentos oculares destrutivos.

RESUMO As intervenções de saúde ocular e a prevenção da cegueira na escola são desenvolvidas desde o século passado no Brasil, acompanhando mudanças de modelos de atenção e de saúde na escola. Trata-se de uma pesquisa qualitativa da técnica de análise documental que teve por objetivo analisar os documentos normativos sobre a temática da saúde ocular no espaço escolar no Brasil, especialmente no âmbito do Programa Saúde na Escola (PSE), e a trajetória histórica das políticas de saúde ocular. Para tratamento dos dados, adotou-se a análise de conteúdo, sendo o corpus do estudo constituído de 48 atos normativos. Entre estes foi possível identificar decretos presidenciais, consultas públicas, portarias ministeriais, e portarias interministeriais. Evidenciou-se que a saúde ocular integral e coordenada ainda é um desafio para o Sistema Único de Saúde (SUS), dado pela interrupção de iniciativas que garantem acesso a cuidados e pelo pouco incentivo à realização dessas práticas, em detrimento de outras ações do PSE. Constatou-se ainda que o debate sobre ações de saúde ocular no ambiente escolar é datado anteriormente à Política Nacional de Atenção em Oftalmologia, revelando pioneirismo no trabalho da temática na Atenção Primária à Saúde.


ABSTRACT Eye healthcare interventions and prevention of blindness at school have been developed since the last century in Brazil, following changes in models of care and health at school. This is a qualitative research of documentary analysis that aimed to analyze the normative documents on the subject of eye health in schools in Brazil, especially within the scope of the School Health Program (PSE) and the historical trajectory of eye healthcare policies. For data treatment, content analysis was adopted, and the corpus of the study consisted of 48 normative acts. Among the normative acts, it was possible to identify Presidential Decrees, Public Consultations, Ministerial Decrees, and Interministerial Decrees. It became evident that a comprehensive and coordinated eye healthcare is still a challenge for the Unified Health System (SUS) due to the interruption of initiatives that guarantee access to healthcare and the little incentive to carry out these practices in detriment of other PSE actions. It was also found that the debate on eye healthcare actions in the school environment dates back to the National Policy of Care in Ophthalmology, revealing a pioneering work on the subject in Primary Health Care.

Introducción: El uso de los videojuegos, por la extensión que ha llegado a alcanzar durante la pandemia de la COVID-19, es una variable relevante de estudio, especialmente por sus interacciones con aspectos de la salud mental y visual. Objetivo: predecir el nivel de Síndrome informático visual a partir de un índice optimizado sobre el nivel de adicción en estudiantes universitarios de dos poblaciones: española y china. Material y Métodos: Se administró un cuestionario online con tres instrumentos validados: un cuestionario para evaluar el juego con videojuegos (CHCVI), un cuestionario para evaluar la adicción a los videojuegos (CERV) y un cuestionario para detectar el síndrome visual por ordenador (CSQ). Los tres cuestionarios se aplicaron a una muestra de 253 estudiantes, tanto de universidades chinas como españolas. Para establecer las predicciones, se construyeron índices robustos basados en el análisis factorial de los instrumentos administrados. Finalmente, se aplicó una regresión logística para obtener un modelo matemático útil para predecir el Síndrome Informático Visual. Resultados: Los resultados mostraron un mayor síndrome informático visual y apetito por los videojuegos en los estudiantes españoles, y menores puntuaciones de síndrome informático visual pero una mayor alteración de la vida cotidiana en los estudiantes chinos debido a este tipo de ocio. Además, se comprobó que los estudiantes de la muestra china tenían un menor riesgo de padecer el síndrome informático visual, y que tener mayores niveles de adicción implicaba 1,4 veces más probabilidades de sufrir dicho síndrome. Conclusiones: Los presentes hallazgos demuestran una relación hasta ahora inexplorada entre la adicción a los videojuegos y los síntomas visuales relacionados con el abuso del ocio electrónico.


Introduction: The use of video games, due to the extent that it has reached during the COVID-19 pandemic, is a relevant study variable especially because of its interactions with aspects of mental and visual health. Objective: to predict the occurrence of computer vision syndrome according to the level of addiction to video games in university undergraduates during a particular period of uncertainty due to health and mobility restrictions imposed by governments as a result of the COVID-19 pandemic. Material and Methods: To accomplish this objective, an online questionnaire was administered with three validated instruments: a questionnaire to assess playing video games (CHCVI), a questionnaire to evaluate video games addiction (CERV), and a questionnaire to detect computer vision syndrome (CSQ). The three questionnaires were applied to a sample of 253 students from both Chinese and Spanish universities. To establish the predictions, robust indexes were constructed based on the Factor Analysis of the instruments administered. Finally, logistic regression was applied to predict computer vision syndrome. Results: The results showed greater computer vision syndrome and appetite for video games in Spanish students, and lower computer vision syndrome scores but a greater alteration of daily life in chinese students due to this type of leisure. Moreover, it was found that students from the Chinese sample entailed a lower risk of suffering from computer vision syndrome, and that having higher levels of addiction involved 1,4 times more likelihood of suffering from such syndrome. Conclusions: The present findings demonstrate a previously unexplored relationship between video games addiction and visual symptoms related to screen exposure.

ABSTRACT Purpose: To report the distribution of referral reasons for children from a pediatric glaucoma outpatient clinic in a tertiary eye care service. Methods: The medical records of patients aged <18 years who were referred to a pediatric glaucoma center in the city of São Paulo, Brazil, between 2012 and 2018 were retrospectively reviewed. The data collected included the referral reasons, age, hospital of origin, and who detected the ocular alteration. For defining the diagnosis, the Childhood Glaucoma Research Network classification was used. Results: Five hundred sixty-three eyes of 328 patients were included in the study. Glaucoma diagnosis was confirmed in 162 children (49%). In 83 patients (25%), the glaucoma diagnosis was ruled out, and 83 (25%) continued outpatient follow-up for suspected glaucoma. The main referral reasons were a cup-to-disc ratio >0.5 or an asymmetry ≥0.2 (24%), intraocular pressure >21 mmHg (21%), and corneal opacity (15%). In the neonatal period, the referral reasons were corneal opacity, buphthalmos, tearing, and photophobia. After the neonatal period, besides these external changes, other signs were also reasons for referral, such as cup-to-disc ratio >0.5 or asymmetry ≥0.2, intraocular pressure >21 mmHg, and myopic shift. The referrals were made by health professionals in 69% and parental concern in 30% of the cases. In 97% of the primary congenital glaucoma cases, the parents were the first to identify the change and to seek for health care. Conclusions: The referral reasons of the children to a tertiary glaucoma clinic were differed between the age groups and diagnoses. We suggest that awareness with these findings is important to avoid and postpone diagnosis, identify their impacts on prognosis, and avoid spending important resources for the management of diseases with inaccurate referrals.


RESUMO Objetivos: Relatar a distribuição dos motivos de encaminhamento de crianças para ambulatório de glaucoma infantil em um serviço oftalmológico terciário. Métodos: Dados médicos de pacientes menores que 18 anos encaminhados para ambulatório de glaucoma infantil na cidade de São Paulo, Brasil, de 2012 a 2018 foram retrospectivamente analisados. Os dados incluíram os motivos de encaminhamento, a idade, a origem e quem detectou a alteração ocular. Para definição diagnóstica, a classificação do Childhood Glaucoma Research Network foi usada. Resultados: 563 olhos de 328 pacientes foram incluídos no estudo. O diagnóstico de glaucoma foi confirmado em 162 crianças (49%). 83 (25%) pacientes tiveram diagnóstico de glaucoma descartado, e 83 (25%) continuaram em acompanhamento como glaucoma suspeito. Os principais motivos de encaminhamento foram relação escavação-disco >0,5 ou assimetria ≥0,2 (24%), pressão intraocular >21 mmHg (21%) e opacidade corneana (15%). No período neonatal, os motivos de encaminhamento foram opacidade corneana, buftalmo, lacrimejamento e fotofobia. Após o período neonatal, além desses sinais externos, outros sinais foram também motivos de encaminhamento, como escavação-disco >0,5 ou assimetria ≥0,2, pressão intraocular >21 mmHg e miopização. Os encaminhamentos ocorreram por profissionais de saúde em 69% e preocupação dos pais em 30%. Os pais foram os primeiros a identificar as alterações e procurar cuidado médico em 97% dos casos de glaucoma congênito primário. Conclusões: Os motivos de encaminhamento de crianças para um serviço de glaucoma de glaucoma terciário foram determinados e diferem em diferentes faixas etárias e grupos. Os autores reforçam a necessidade de alertar diferentes grupos para os sinais mais comuns, a fim de evitar, não só o adiamento do diagnóstico, o que impacta no prognóstico, mas também despender recursos importantes direcionados ao enfrentamento dessas doenças, com encaminhamentos imprecisos.

Introducción: El incremento del empleo de los dispositivos electrónicos en las actividades de estudio, trabajo, entretenimiento e interacción social desde las plataformas digitales deriva numerosos riesgos y causas de problemas visuales y defectos refractivos. La prevalencia de estos puede conllevar a considerarlos como problemas de salud a nivel global. Objetivo: Describir los riesgos y las repercusiones del uso excesivo de los dispositivos electrónicos en la salud visual a cualquier edad. Métodos: El método de revisión bibliográfica y la sistematización de resultados investigativos de diversos autores posibilitan identificar los riesgos y la repercusión del uso excesivo de los dispositivos electrónicos sobre la salud visual, así como las recomendaciones para afrontar esa realidad. Resultados: La sistematización permite demostrar cómo incide el uso de los dispositivos electrónicos en la elevada prevalencia de trastornos oculares y visuales, como el síndrome visual informático y la miopía en la población mundial actualmente. Conclusiones: Entre los riesgos y las repercusiones del uso excesivo de los dispositivos electrónicos sobre la salud visual, señalados por varios autores, se describieron factores ópticos y ambientales que inciden en la aparición de trastornos como la visión tubular, el síndrome visual informático, además de los defectos refractivos como la miopía, considerados problemas de salud a nivel global en la actualidad(AU)


Introduction: The increase in the use of electronic devices in study, work, entertainment and social interaction activities from digital platforms derives numerous risks and causes of visual problems and refractive defects. The prevalence of these can lead to consider them as global health problems. Objective: To describe the risks and repercussions of excessive use of electronic devices on visual health to prevent eye problems at any age. Methods: The bibliographic review method and the systematization of research results from various authors make it possible to identify the risks and repercussions of the excessive use of electronic devices on visual health, as well as the recommendations to face this reality. Results: The systematization allows us to demonstrate how the use of electronic devices affects the high prevalence of eye and visual disorders, such as computer vision syndrome and myopia in the world population today. Conclusions: The excessive use of electronic devices has caused visual problems and refractive errors globally. An act of responsibility is imposed to inform and raise awareness about the damage to eye health and its harmful effects(AU)

Métodos: Se realizó una revisión bibliografía y documental sobre diabetes mellitus y salud visual, estudios y documentos publicados por instituciones nacionales e internacionales, temas vinculados a la diabetes mellitus y la salud visual, se accedió a artículos desde diferentes fuentes, revistas indexadas, metaanálisis y fuentes primarias no publicadas, tesis de maestría, y doctorados. Resultados: Para conservar la salud visual es necesario llevar a cabo medidas que conlleven a prevenir la pérdida de la visión por catarata, glaucoma, retinopatia y edema macular diabético, que incluyen no sólo un buen control de los parámetros metabólicos y las revisiones periódicas al oftalmólogo, sino también el cumplimiento de la educación diabetológica por un equipo multidisciplinario y actualización sobre este tema de los prestadores de salud. La disminución visual que provoca la catarata y otras enfermedades oculares puede conducir a padecer discapacidades como, pérdida de autonomía para administrarse los medicamentos, limitar las actividades de la vida cotidiana e incapacidad laboral y social, con la consiguiente afectación de la calidad de vida Conclusiones: Acudir al oftalmólogo con frecuencia, mantener el control glucémico, del colesterol y presión arterial representan el pilar fundamental para preservar la salud visual en el paciente diabético(AU)


Introduction: Diabetes mellitus requires multifactorial and multisectoral medical attention. Diabetic ophthalmopathy demands integrated health systems that facilitate the prevention, detection and treatment of complications at the primary, secondary and tertiary care levels. Objective: To show the theoretical and practical concepts and current trends on diabetes mellitus and visual health. Methods: A bibliographic and documentary review on diabetes mellitus and visual health was carried out. Studies and documents published by local and international institutions, articles, topics related to diabetes mellitus and visual health were retrieved from different sources, indexed journals, meta-analysis and unpublished primary sources, research degree and master's, theses. Results: To preserve visual health, it is necessary to carry out reading that lead to preventing vision loss due to cataract, glaucoma, retinopathy and diabetic macular edema, which include not only good control of metabolic parameters and regular check-ups with the ophthalmologist , but also compliance with diabetes education by a multidisciplinary team and updating the health providers on this subject. The visual decrease caused by cataract and other eye diseases can lead to disabilities such as loss of autonomy to manage medications, limit activities of daily life and work and social incapacity, with the consequent impact on quality of life. Conclusions: Frequently going to the ophthalmologist, maintaining glycemic, cholesterol and blood pressure control represent the fundamental pillar to preserve visual health in diabetic patients(AU)
