Background: Literature points towards the potential benefits of the application of Eye Movement and Desensitization Processing (EMDR)-therapy for patients in the medical setting, with cancer and pain being among the domains it is applied to. The field of applying EMDR-therapy for patients treated in the medical setting has evolved to such an extent that it may be challenging to get a comprehensive overview.Objective: This systematic literature review aims to evaluate the use and effectiveness of Eye Movement Desensitization and Reprocessing (EMDR) therapy in patients treated in the medical setting.Methods: We performed a literature search following the PRISMA guidelines. Studies were included if the effectiveness of EMDR-therapy was assessed in adult patients treated in a medical setting. Excluded were patients exclusively suffering from a mental health disorder, without somatic comorbidity. A risk of bias analysis was performed. This review was registered on PROSPERO (CRD42022325238).Results: Eighty-seven studies, of which 26 (pilot)-RCTs were included and categorized in 14 medical domains. Additionally, three studies focusing on persistent physical complaints were included. Most evidence exists for its application in the fields of oncology, pain, and neurology. The overall appraisal of these studies showed at least moderate to high risks of bias. EMDR demonstrated effectiveness in reducing symptoms in 85 out of 87 studies. Notably, the occurrence of adverse events was rarely mentioned.Conclusions: Overall, outcomes seem to show beneficial effects of EMDR on reducing psychological and physical symptoms in patients treated in a medical setting. Due to the heterogeneity of reported outcomes, effect sizes could not be pooled. Due to the high risk of bias of the included studies, our results should be interpreted with caution and further controlled high-quality research is needed.


First overview on the use of EMDR for adult patients treated in the medical setting.EMDR seems beneficial in improving psychological and physical symptoms.Given the heterogeneity of studies and high risk of bias, further controlled studies are needed in this field.

RESUMO

O objetivo deste estudo foi revisar sistematicamente a contribuição da família e da escolano desenvolvimento de habilidades para a vida(HV)no contexto de formação esportiva. As buscas foram conduzidas em sete bases de dado se por meio da pesquisa de referências. Foram seguidas as descrições do Prisma, identificando 51 estudos. Os resultados demonstraram uma predominância de investigações no Canadá e nos Estados Unidos. Os autores dos estudos têm utilizado diferentes instrumentos, variáveis e modelos teóricos para verificar a contribuição da escola e/ou da família no desenvolvimento de habilidades para a vida. Conclui-se que o envolvimento escolar e o familiar no esporte podem contribuir com a aquisição, refinamento e transferência de características que podem ser aplicadas na vida dos indivíduos. Por fim, existe a necessidade de construir instrumentos quantitativos específicos que avaliem em conjunto a contribuição da escola e da família neste processo (AU).


The aim of this study was to systematically review the contribution of family and school in the development of life skills(LS)in the context of sports training. The searches were conducted in sevendatabases and through the reference searches.Prisma descriptions were followed, identifying 51 studies. The results showed a predominance of investigations in Canada and the United States. The authors of the studies have used different instruments, variables and theoretical models to verify the contribution of the school and/or the family in the development of skills for life. It is concluded that school and family involvement in sport can contribute to the acquisition, refinement and transfer of characteristics that can be applied in the lives of individuals. Finally,there is a need to build specific quantitative instruments that jointly assess the contribution of the school and the family in this process (AU).


El objetivo de este estudio fue revisar sistemáticamente la contribución de la familia y la escuela en el desarrollo de habilidades para la vida(HV)en el contexto del entrenamiento deportivo. Las búsquedas se realizaron en siete bases de datos y mediante la búsqueda de referencias.Se siguieron las descripciones de Prisma, identificando 51 estudios. Los resultados mostraron un predominio de investigaciones en Canadá y Estados Unidos. Los autores de los estudios han utilizado diferentes instrumentos, variables y modelos teóricos paraverificar la contribución de la escuela y/o la familia en el desarrollo de habilidades para la vida. Se concluye que la implicación escolar y familiar en el deporte puede contribuir a la adquisición, perfeccionamiento y transferencia de características que pueden ser aplicadas en la vida de los individuos. Finalmente,existe la necesidad de construir instrumentos cuantitativos específicos que evalúen de manera conjunta la contribución de la escuela y la familia en este proceso (AU).

OBJECTIVE: Anemia is a common condition in end-stage renal disease (ESRD) patients. Erythropoiesis-stimulating agents (ESAs) are commonly used to treat anemia in these patients. However, concerns have been raised regarding their potential effects on blood pressure. This systematic review and meta-analysis aim to investigate the relationship between ESAs and changes in systolic and diastolic blood pressure in hemodialysis patients. METHOD: This study is a systematic review and meta-analysis based on clinical trial studies published in various databases, including Web of Science, Cochrane Library, Science Direct, PubMed, Embase, Scopus, and Google Scholar, between 1980 and the end of 2022. We evaluated the quality of articles using the Jadad scale checklist and analyzed the data using Stata 15 software. RESULTS: Our meta-analysis included 34 clinical trial studies. The results showed a significant increase in both systolic blood pressure (SBP) and diastolic blood pressure (DBP) after the consumption of ESAs compared to before consumption. The mean difference in SBP was 4.84mmHg (95% CI: 2.74-6.94; p-value<0.001) and in DBP was 4.69mmHg (95% CI: 2.67-6.71; p-value<0.001). No publication bias was observed. Our meta-regression analysis showed that sample size, quality assessment score, and geographical location of the study were significant factors related to observed heterogenicity in to mean difference of SBP (p-value≤0.20). For DBP, the sample size, quality assessment score and follow-up duration were significant variables (p-value≤0.20). CONCLUSION: Based on the findings of our study, it appears that receiving ESAs is associated with a significant increase in both SBP and DBP in hemodialysis patients, with an increase of about 5mmHg.

INTRODUCTION: Pharmacogenetics evaluates how genetic variations influence drug responses. Nowadays, genetic tests have advanced, becoming more affordable, and its integration is supported by stronger clinical evidence. Guidelines such as those from CPIC (Clinical Pharmacogenetics Implementation Consortium) and resources like PharmGKB facilitate genotype-based prescribing; and organizations like the FDA promote genetic testing before initiating certain medications. Preventive pharmacogenetic panels seem promising, but further research on biomarkers and diverse populations is needed. The aim of this review is to analyze recent evidence on the genotype-drug response relationship to examine how the genetic profile of patients influences the clinical response to treatments, and analyze the areas of research that need further study to advance towards a genetic-based precision medicine. MATERIALS AND METHODS: A systematic search was conducted on PubMed to identify articles investigating the genotype-drug response relationship. The search strategy included terms such as "pharmacogenetics", "personalized treatment", "precision medicine", "dose adjustment", "individualizing dosing", "clinical routine", and "clinical practice." Clinical trials, observational studies, and meta-analyses published in English or Spanish between 2013 and 2023 were included. The initial search resulted in a total of 136 articles for analysis. RESULTS: 49 articles were included for the final analysis following review by 2 investigators. A relationship between genetic polymorphisms and drug response or toxicity was found for drugs such as opioids, GLP-1 agonists, tacrolimus, oral anticoagulants, antineoplastics, atypical antipsychotics, efavirenz, clopidogrel, lamotrigine, anti-TNFα agents, voriconazole, antidepressants, or statins. However, for drugs like metformin, quetiapine, irinotecan, bisoprolol, and anti-VEGF agents, no statistically significant association between genotype and response was found. CONCLUSION: The studies analyzed in this review suggest a strong correlation between genetic variability and individual drug responses, supporting the use of pharmacogenetics for treatment optimization. However, for certain drugs like metformin or quetiapine, the influence of genotype on their response remains unclear. More studies with larger sample sizes, greater ethnic diversity, and consideration of non-genetic factors are needed. The lack of standardization in analysis methods and accessibility to genetic testing are significant challenges in this field. As a conclusion, pharmacogenetics shows immense potential in personalized medicine, but further research is required.

INTRODUCTION: The increased risk of severe and life-threatening toxicity in patients with dihydropyridine dehydrogenase deficiency, under treatment with fluoropyrimidines, has been widely studied. An up-to-date overview of systematic reviews summarizing existing literature can add value by highlighting most relevant information and supports decision-making regarding treatment in dihydropyridine dehydrogenase deficient patients. The main objective of this overview is to identify published systematic reviews on the association between germline variations in the DPYD gene and fluoropyrimidine toxicity. METHODS AND ANALYSIS: This protocol was developed following the Preferred Reported Items for Systematic Review and Meta-analysis Protocols (PRISMA-P) checklist, and the overview of systematic reviews will be reported in accordance with the PRISMA statement. PubMed, Embase, Scopus and the Cochrane Library will be searched from inception to 2023. Systematic reviews irrespective of study designs that analyze the association between germline variations in the DPYD and fluoropyrimidine toxicity will be considered. Methodological quality will be assessed using AMSTAR2 checklist (Measurement Tool to Assess Systematic Reviews 2). Two independent investigators will perform the study selection, quality assessment and data collection. Discrepancies will be solved by a third investigator.

OBJECTIVE: To determine, through a systematic review, the effects of halo gravity traction in spinal deformity. METHODS: Prospective studies or case series of patients with scoliosis or kyphosis treated with cranial halo gravity traction (HGT) were included. Radiological outcomes were measured in the sagittal and/or coronal planes. Pulmonary function was also assessed. Perioperative complications were also collected. RESULTS: Thirteen studies were included. Congenital etiology was the most frequent etiology observed. Most studies provided clinically relevant curve correction values in the sagittal and coronal planes. Pulmonary values improved significantly after the use of HGT. Finally, there were a pool of 83 complications in 356 patients (23.3%). The most frequent complications were screw infection (38 cases). CONCLUSIONS: Preoperative HGT appears to be a safe and effective intervention for deformity that allows correction prior to surgery. However, there is a lack of homogeneity in the published studies.

ABSTRACT Objective: To identify the prevalence of errors that caused events supposedly attributable to vaccination or immunization. Method: Systematic literature review with meta-analysis carried out on the Medline, Cochrane Library, Cinahl, Web of Science, Lilacs, Scopus; Embase; Open Grey; Google Scholar; and Grey Lit databases; with studies that presented the prevalence of immunization errors that caused events or that provided data that allowed this indicator to be calculated. Results: We evaluated 11 articles published between 2010 and 2021, indicating a prevalence of 0.044 errors per 10,000 doses administered (n=762; CI95%: 0.026 - 0.075; I2 = 99%, p < 0.01). The prevalence was higher in children under 5 (0.334 / 10,000 doses; n=14). The predominant events were fever, local pain, edema and redness. Conclusion: A low prevalence of errors causing events was identified. However, events supposedly attributable to vaccination or immunization can contribute to vaccine hesitancy and, consequently, have an impact on vaccination coverage.

Objetivou-se levantar a produção científica na literatura sobre violência nas aulas de EF escolar nos últimos 5 anos. Desenvolveu-se uma revisão sistemática sobre a temática de violência nas áreas da Educação e EF através do método PRISMA. As bases consultadas foram Scielo, Lilacs, Scopus e Google Acadêmico, durante setembro/2022- outubro/2022. Foram encontrados 1028 estudos e, após aplicados os filtros de seleção, chegou-se a 14 trabalhos. Percebeu- se uma ascendência de pesquisas relacionadas à violência escolar e EF de cunho qualitativo e predominantes nas regiões Sudeste e Sul do Brasil. Os dados mostram que os cursos de formação de professores de EF ainda carecem de informações sobre o fenômeno da violência escolar, questão que afeta a vivência pedagógica dos docentes, uma vez que a postura do professor pode gerar e/ou intensificar atos de violência ao corpo docente e discente, tendo a escola, muitas vezes, um papel omisso frente à problemática.


The objective was to survey the scientific production in the literature on violence in PE classes at school in the last 5 years. A systematic review was developed on the theme of violence in the areas of Education and PE using the PRISMA method. The databases consulted were Scielo, Lilacs, Scopus and Google Scholar, during September/2022-October/2022. 1028 studies were found and, after applying the selection filters, 14 works were reached. An ascendance of research related to school violence and PE of a qualitative nature and predominant in the Southeast and South regions of Brazil was noticed. The data show that the training courses for PE teachers still lack information about the phenomenon of school violence, an issue that affects the pedagogical experience of teachers, since their attitude can generate and/or intensify acts of violence against teachers and students, with the school often having a silent role in the face of the problem.


El objetivo fue relevar la producción científica en la literatura sobre la violencia en las clases de EF en la escuela en los últimos 5 años. Se desarrolló una revisión sistemática sobre el tema de la violencia en las áreas de Educación y EF utilizando el método PRISMA. Las bases de datos consultadas fueron Scielo, Lilacs, Scopus y Google Scholar, durante septiembre/2022-octubre/2022. Se encontraron 1028 estudios y, después de aplicar los filtros de selección, se llegó a 14 trabajos. Se notó un ascenso de investigaciones relacionadas con la violencia escolar y la EF de carácter cualitativo y predominantes en las regiones Sureste y Sur de Brasil. Los datos muestran que los cursos de formación de docentes de EF aún carecen de información sobre el fenómeno de la violencia escolar, tema que afecta la experiencia pedagógica de los docentes, ya que la actitud del profesor puede generar y/o intensificar actos de violencia contra docentes y alumnos, teniendo la escuela muchas veces un papel silencioso frente al problema.

ABSTRACT Objective: The aim of this study was to synthesize the evidence on the prevalence of latent Mycobacterium tuberculosis infection (LTBI) among undergraduate health care students. Methods: A systematic review of prevalence with meta-analysis was conducted. Prospective and retrospective cohorts and cross-sectional studies involving probable exposure to M. tuberculosis during undergraduate education, along with the tuberculin skin test (TST) or interferon-γ release assay (IGRA) for investigation of latent tuberculosis were searched. Searches were conducted in MEDLINE, CINAHL, EMBASE, LILACS, Scopus, and Web of Science databases. Independent reviewers were responsible for the selection and inclusion of studies. Data were extracted, critically appraised, and synthesized using the JBI approach. PRISMA was used to report the study. Results: Twenty-two studies were analyzed. The overall prevalence in healthcare undergraduate students was 12.53%. Conclusion: The prevalence of LTBI in undergraduate health students was high for such a highly educated population. Screening with TST and/or IGRA and chemoprophylaxis, when necessary, should be provided to undergraduate health students when in contact with respiratory symptomatic patients.


RESUMO Objetivo: O objetivo deste estudo foi sintetizar as evidências sobre a prevalência de infecção de Mycobacterium tuberculosis (ILTB) entre estudantes de graduação da área da saúde. Método: Foi realizada uma revisão sistemática de prevalência com metanálise. Coortes prospectivas e retrospectivas e estudos transversais envolvendo provável exposição a M. tuberculosis durante a graduação, juntamente com o teste tuberculínico (TT) ou ensaio de liberação de interferon-γ (IGRA) para investigação de tuberculose latente foram pesquisados. As buscas foram realizadas nas bases de dados MEDLINE, CINAHL, EMBASE, LILACS, Scopus e Web of Science. Revisores independentes foram responsáveis pela seleção e inclusão dos estudos. Os dados foram extraídos, avaliados criticamente e sintetizados utilizando a abordagem JBI. PRISMA foi usado para relatar o estudo. Resultados: Vinte e dois estudos foram analisados. A prevalência geral em estudantes de graduação da área da saúde foi de 12,53%. Conclusão: A prevalência de ILTB em estudantes de graduação em saúde foi alta para uma população com alto nível de escolaridade. Triagem com TT e/ou IGRA e quimioprofilaxia, quando necessária, deve ser fornecida aos estudantes de graduação da área da saúde quando em contato com pacientes sintomáticos respiratórios.

Las búsquedas se realizaron en las bases de datos MEDLINE, CINAHL, EMBASE, LILACS, Scopus y Web of Science. Revisores independientes fueron responsables de la selección e inclusión de los estudios. Los datos se extrajeron, se evaluaron críticamente y se sintetizaron utilizando el enfoque JBI. Se utilizó PRISMA para informar el estudio. Resultados: Se analizaron veintidós estudios. La prevalencia global en estudiantes universitarios en salud fue del 12,53%. Conclusión: La prevalencia de LTBI en estudiantes universitarios de salud fue alta para una población con un nivel educativo tan alto. Se debe proporcionar tamizaje con TST y/o IGRA y quimioprofilaxis, cuando sea necesario, a los estudiantes universitarios en salud cuando estén en contacto con pacientes sintomáticos respiratorios.

Con el incremento en el acceso y uso de las redes sociales en Colombia, es de esperar que prácticas y discursos violentos se trasladen a estos espacios digitales. Sin duda, hoy en día las redes sociales son parte de los ecosistemas de información y comunicación de los colombianos en los que las culturas de violencia y paz se expanden y transforman. En este contexto, es esencial explorar las formas en las que investigamos la violencia en redes sociales, ya que entender este fenómeno permite responder mejor a las necesidades y experiencias de las comunidades del país. Este artículo busca explorar cómo se ha estudiado la violencia en las redes sociales en Colombia mediante una revisión sistemática de la literatura. Resultados muestran un complejo trayecto de investigación en el país que comprende variados objetivos, como entender las diferentes formas en las que la violencia es ejercida a través de redes sociales o sobre cómo nos permite conocer más contextos de violencia. Igualmente, los resultados muestran que las investigaciones están principalmente enfocadas en dos plataformas: Facebook y Twitter. Finalmente, se encontró que los artículos están principalmente orientados a investigar tres tipos de violencias: violencia de género, violencia relacionada con conflicto armado y violencia escolar. Los hallazgos de esta revisión sistemática de la literatura permiten, por un lado, valorar previos estudios que han demostrado el complejo panorama de la violencia mediática en el país y, por otro lado, descubrir posibles vacíos en la literatura que podrían ser respondidos por investigadoras e investigadores en el futuro.


With the increase in the access and use of social media platforms in Colombia, it is expected that violent practices and discourses transfer to these digital spaces. Certainly, today's social media are part of the information and communication ecosystems of Colombians, where cultures of violence expand and transform. In this context, it is essential to explore the ways in which we examine and research social media violence, since understanding this phenomenon allows us to strengthen a culture of peace that better responds to the needs and experiences of the country's communities. Thus, this article seeks to explore how social media violence has been studied in Colombia, through a systematic literature review. Results show a complex trajectory of research in the country that includes various objectives, such as understanding the different ways in which violence is exercised through social media, or how it allows us to learn more about contexts of violence. Likewise, the results show that research is mainly focused on two platforms: Facebook and Twitter. Finally, the articles were found to be mainly focused on three types of violence: gender-based violence, violence related to armed conflict, and school violence. Overall, findings of this systematic literature review allow, on the one hand, to assess previous studies that have demonstrated the complex landscape of social media violence in the country, and, on the other hand, to discover possible gaps in the literature that could be addressed by researchers in the future.

Introducción: La espirometría es una prueba de función pulmonar usada en la valoración de programas de rehabilitación para evaluar exposiciones a tóxicos y alérgenos, en estudios epidemiológicos y en el desarrollo de ecuaciones de referencia en poblaciones específicas; estos valores pueden variar de acuerdo con la altura. Objetivo: Establecer las diferencias entre los valores de referencia de espirometría forzada en población adulta residentes en alturas mayores y menores a 1500 metros sobre el nivel del mar. Métodos: Revisión sistemática, se encontraron 536 estudios, se evaluaron 66 en texto completo, 33 en calidad metodológica con las listas de chequeo Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies y Quality Assessment of Case-Control Studies; 21 estudios fueron seleccionados para la revisión y 12 surtieron metanálisis. Resultados: Se contó con 48 923 participantes de alturas entre 2,4 y 4440 m s. n. m. Hay diferencias iniciales al relacionar la altura (+/-1500 m s. n. m) con el VEF1 (hombres: DM 0,29; IC: 0,03-0,55; mujeres: DM 0,27; IC: -0,07-0,60) y los valores de referencia con el sexo: CVF (DM 1,31; IC: 1,24-1,37) y VEF1 (DM: 1,03; IC: 0,95-1,11). Se reportó alta heterogeneidad y riesgo de sesgo de publicación. Discusión: Estas diferencias se dan en función de aspectos antropométricos y fisiológicos como la aclimatación y el envejecimiento pulmonar. Los mecanismos que influyen en estos cambios son la adaptación genética, molecular, fisiológica y anatómica, que permiten compensar los efectos de la hipoxia aguda o crónica, lo cual aumenta la ventilación alveolar y los valores espirométricos. Conclusiones: Los valores de referencia de espirometría varían de acuerdo con la altura (mayores en alturas > 1500 m s. n. m) y el sexo (más elevados en hombres). Es necesario contar con evidencias más amplias y contundentes en la temática.


Introduction: Spirometry is a lung function test used in the evaluation of rehabilitation programs to evaluate exposures to toxics and allergens in epidemiological studies and in the development of reference equations in specific populations; these values may vary according to height. Objective: To establish the differences between the reference values of forced spirometry in the adult population living at altitudes greater than and less than 1,500 meters above sea level. Methods: Systematic review a total of 536 studies were found; 66 were assessed in full text and 33 were assessed for methodological quality using the Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies and Quality Assessment of Case-Control Studies checklists; 21 studies were selected for the review and 12 provided meta-analyses. Results: There were 48,923 participants from heights between 2.4 and 4,440 m.a.s.l. There are initial differences when relating height (+/-1,500 m.a.s.l.) with FEV1 (men: MD 0.29; CI: 0.03-0.55; women: MD 0.27; CI: -0.07-0 .60); and the reference values with gender: FVC (MD 1.31, CI: 1.24-1.37) and FEV1 (MD: 1.03, CI: 0.95-1.11). High heterogeneity and risk of publication bias are reported. Discussion: These differences occur based on anthropometric and physiological aspects such as acclimatization and lung aging. The mechanisms that influence these changes are genetic, molecular, physiological and anatomical adaptations that allow compensation for the effects of acute or chronic hypoxia, which increases alveolar ventilation and spirometric values. Conclusions: The spirometry reference values vary according to height (higher at heights >1,500 m.a.s.l.) and sex (higher in men). It is necessary to have broader and more convincing evidence on the subject.

Introduction: Postoperative nausea and vomiting (PONV) are common complications in surgical patients undergoing general anesthesia, and multiple strategies have been suggested to prevent them. Objective: To describe the available evidence on the effectiveness of pharmacological and non-pharmacological strategies for preventing PONV in adults undergoing surgery under general anesthesia, as reported in previous meta-analyses and systematic reviews. Methodology: An overview of systematic reviews and meta-analyses was conducted. Searches were performed in PubMed, EBSCO, EMBASE, Cochrane Database, Science Direct, and Scopus, without restrictions as to gender, clinical condition, or date of publication, including articles in Spanish, French, and English only. Two reviewers independently and in duplicate did the screening, data extraction, quality evaluation, and risk of bias assessment according to AMSTAR-2. The PRISMA and PRIOR statements were followed for reporting. PROSPERO registration number CRD42021251999. Results: Out of 80 candidate articles, three were viable for meta-analysis. 1.5 mg to 18 mg doses of Dexamethasone showed a significant reduction in the risk of PONV, with a RR of 0.48 (95 % CI 0.41-0.57; p<0.001), I2=63 % (p=0.07), and a NNTc of 5 and 7. Other effective strategies included the use of acoustic stimulation/acupuncture/acupressure, 5HT3 antagonists, NK1 antagonists, gabapentinoids, haloperidol, droperidol, metoclopramide, midazolam, mirtazapine, among others. The risk of publication bias was low. Conclusion: Different strategies are effective for PONV prophylaxis in surgeries under general anesthesia. Dexamethasone shows the best available evidence at the moment. The documented methodological quality suggests the need for better studies to establish the effectiveness of the strategies.


Introducción: Las náuseas y el vómito posoperatorios (NVPO) son comunes en pacientes quirúrgicos bajo anestesia general y se han planteado múltiples estrategias para prevenirlos. Objetivo: Describir la evidencia disponible sobre la efectividad de las estrategias farmacológicas y no farmacológicas para prevenir las NVPO en adultos sometidos a cirugía bajo anestesia general, según lo descrito en metaanálisis y revisiones sistemáticas previas. Metodología: Se realizó una metarrevisión de revisiones sistemáticas y metaanálisis. Se ejecutaron búsquedas en PubMed, EBSCO, Embase, Cochrane Database, ScienceDirect y Scopus, sin restricción por sexo, condición clínica ni fecha de publicación, solo de artículos en español, francés e inglés. Dos revisores llevaron a cabo tamizaje, extracción de datos, evaluación de calidad y riesgo de sesgo según AMSTAR-2, de manera independiente y en duplicado. Se siguieron las declaraciones PRISMA y PRIOR para el reporte, previo registro en Prospero CRD42021251999. Resultados: De 80 artículos candidatos, se seleccionaron tres viables para realización de metaanálisis. La dexametasona entre 1,5 mg y 18 mg mostró un RR=0,48 (IC95 % [0,41-0,57]; p<0,001), I2=63 % (p=0,07) y un NNTc 5 y 7. Otras estrategias efectivas incluyen el uso de acuestimulación/acupuntura/acupresión, antagonistas 5HT3, antagonistas NK1, gabapentinoides, haloperidol, droperidol, metoclopramida, midazolam, mirtazapina, entre otras. El riesgo de sesgo de las publicaciones fue bajo. Conclusión: Diferentes estrategias son efectivas para profilaxis NVPO en cirugías con anestesia general. Dexametasona presenta la mejor evidencia disponible al momento. La calidad metodológica documentada sugiere la necesidad de realizar mejores trabajos para determinar la efectividad de las estrategias.

RESUMO

The aim of this special article is to summarize and discuss, from an anesthesia perspective, the network meta-analysis on drugs used for the prevention of postoperative nausea and vomiting after general anesthesia, in agreement with the Cochrane Colombia collaboration and within the framework of the Cochrane Corners strategy. Through the combination of indirect comparisons and based on the evidence, the use of aprepitant, ramosetron, granisetron, dexamethasone and ondansetron is recommended with a high degree of certainty for the reduction of postoperative nausea and vomiting.


Este artículo especial tiene el objetivo de resumir y discutir desde la perspectiva de la anestesiología, el metaanálisis en red sobre fármacos para prevenir náuseas y vómito posoperatorio luego de anestesia general, en acuerdo con la colaboración Cochrane Colombia y en el marco de la estrategia Cochrane Corners. Mediante la combinación de la evidencia y el uso de comparaciones indirectas, se ha recomendado con alto grado de certeza el uso de aprepitant, ramosetrón, granisetrón, dexametasona y ondansetrón para la reducción de náuseas y vómito posoperatorio.

Introducción: Las aplicaciones informáticas en la práctica médica han permitido el desarrollo de novedosas formas de comunicación en la atención del Sistema de Salud. La estandarización representa un elemento necesario para la sostenibilidad y escalabilidad del software desarrollado. En sistemas para la salud existen diversas aplicaciones del estándar HL7. Objetivo: Realizar una Revisión Sistemática de la Literatura (RSL) sobre el estándar de interoperabilidad HL7. Materiales y métodos: Para la definición y ejecución del presente estudio se empleó la propuesta metodológica establecida por Petersen la cual plantea cinco pasos secuenciales: definición de las preguntas de investigación; definición de los criterios de búsqueda; definición de los criterios de selección de estudios primarios; determinación del esquema de clasificación; y extracción de datos y mapeo de estudios. Resultados: Se obtuvieron como resultado preliminar 428 569 entradas posibles. Aplicando el primer filtro de inclusión y exclusión sobre el título, resumen y palabras clave, quedaron 864 documentos, de los cuales solo 276 cumplieron con haber sido publicados en el período que se analiza (2017-2022). De ellos sólo 72 permitían acceso completo a la documentación y al ser analizados se descartan 51 por no ofrecer información acorde al tema de investigación. Resultaron un total de 21 artículos para un análisis a mayor profundidad. Conclusiones: A partir de la revisión sistemática se pudo constatar que en la optimización de los procesos comunicativos es preciso el uso de estándares que armonicen el intercambio de información y provean un lenguaje común para todos los agentes involucrados.


Introduction: Computer applications in medical practice have allowed the development of novel forms of communication in the Health Care System. Standardization represents a necessary element for the sustainability and scalability of the developed software. In health systems there are various applications of the HL7 standard. Objective: To perform a Systematic Review of Literature (RSL) on the HL7 interoperability standard. Methods: The methodological proposal established by Petersen, which proposes five sequential steps such as: definition of research questions, definition of search criteria, definition of the selection criteria of primary studies, determination of the classification scheme, and data extraction and mapping study, was used for the definition and implementation of this study. Results: A total of 428,569 possible entries were obtained as a preliminary result. Applying the first inclusion and exclusion filter on the title, abstract and keywords, 864 documents remained, of which only 276 complied with having been published in the period under analysis (2017-2022). Only 72 of them allowed full access to the documentation and when analyzed, 51 were excluded for not offering information according to the research topic. A total of 21 articles were selected for further analysis. Conclusions: Based on the systematic review, it was possible to verify that in the optimization of communication processes, standards must be used to harmonize the exchange of information and provide a common language for all the agents involved.

Los bebés exhiben una amplia gama de comportamientos prosociales, que incluyen ayudar, compartir, consolar y cooperar. En la infancia, estos comportamientos se vuelven más sofisticados y socialmente apropiados. El objetivo de esta investigación es identificar y revisar sistemáticamente la literatura que ha incorporado el estudio de la figura paterna y su impacto en el desarrollo de la conducta prosocial en los niños/as entre los 0 y 5 años. Método, utilizando la metodología PRISMA, se realiza una búsqueda y análisis de artículos científicos en las bases de datos Scopus y Web of Science, considerando criterios de inclusión y exclusión establecidos. Resultados, se seleccionaron 28 artículos y el análisis de sus resultados indica que el padre tiene un impacto en el desarrollo de la conducta prosocial de los niños entre los 0 y 5 años. El ejercicio del rol paterno y el apoyo entregado por el padre, favorece que el niño/a manifieste conductas prosociales desde la infancia temprana, en la relación con sus figuras significativas y con terceros, de forma directa e indirecta, en los distintos contextos de socialización.


Infants exhibit a wide range of prosocial behaviors, including helping, sharing, comforting, and cooperating. In infancy, these behaviors become more sophisticated and socially appropriate. The aim of this research is to systematically identify and review the literature that has incorporated the study of the father figure and its impact on the development of prosocial behavior in children aged 0-5 years. Method, using the PRISMA methodology, a search and analysis of scientific articles in the Scopus and Web of Science databases was carried out, considering established inclusion and exclusion criteria. Results, 28 articles were selected and the analysis of their results indicates that fathers have an impact on the development of prosocial behavior in children between 0 and 5 years of age. The exercise of the paternal role and the support provided by the father, favors the child to manifest prosocial behaviors from early childhood, in the relationship with their significant figures and with third parties, directly and indirectly, in the different socialization contexts.

Objetivo: analizar la correlación entre el tiempo en rango y la hemoglobina glicosilada de personas que viven con diabetes mellitus y realizan la monitorización continua de la glucemia o el automonitoreo de la glucemia capilar Método: revisión sistemática de etiología y riesgo basada en las directrices del JBI e informada según los Preferred Reporting Items for Systematic Reviews and Meta-Analyses, abarcando seis bases de datos y la literatura gris. La muestra incluyó 16 estudios y la calidad metodológica fue evaluada utilizando las herramientas del JBI. Protocolo registrado en Open Science Framework, disponible en https://doi.org/10.17605/OSF.IO/NKMZB. Resultados: tiempo en rango (70-180 mg/dl) mostró una correlación negativa con la hemoglobina glicosilada, mientras que el tiempo por encima del rango (>180 mg/dl) mostró una correlación positiva. Los coeficientes de correlación variaron entre -0,310 y -0,869 para el tiempo en rango, y entre 0,66 y 0,934 para el tiempo por encima del rango. Un estudio se realizó en una población que hacía el automonitoreo. Conclusión: hay una correlación estadísticamente significativa entre el tiempo en rango y el tiempo por encima del rango con la hemoglobina glicosilada. Cuanto mayor sea la proporción en el rango glucémico adecuado, más cerca o por debajo del 7% estará la hemoglobina glicosilada. Se necesitan más estudios que evalúen esta métrica con datos del automonitoreo de la glucemia.


Objective: to analyze the correlation between time on target and glycated hemoglobin in people living with diabetes mellitus and carrying out continuous blood glucose monitoring or self-monitoring of capillary blood glucose. Method: systematic review of etiology and risk based on JBI guidelines and reported according to Preferred Reporting Items for Systematic Reviews and Meta- Analyses, covering six databases and grey literature. The sample included 16 studies and methodological quality was assessed using JBI tools. Protocol registered in the Open Science Framework, available at https://doi.org/10.17605/OSF.IO/NKMZB. Results: time on target (70-180 mg/dl) showed a negative correlation with glycated hemoglobin, while time above target (>180 mg/dl) showed a positive correlation. Correlation coefficients ranged between -0.310 and -0.869 for time on target, and between 0.66 and 0.934 for time above target. A study was carried out on a population that performed self-monitoring. Conclusion: there is a statistically significant correlation between time on target and time above target with glycated hemoglobin. The higher the proportion in the adequate glycemic range, the closer to or less than 7% the glycated hemoglobin will be. More studies are needed to evaluate this metric with data from self-monitoring of blood glucose.

Objetivo: analizar los efectos de las tecnologías educativas en la prevención y tratamiento de la úlcera diabética. Método: revisión sistemática realizada en siete bases de datos, un índice bibliográfico, una biblioteca electrónica y literatura gris. La muestra estuvo compuesta por 11 ensayos clínicos controlados aleatorizados. La síntesis de los resultados fue descriptiva y mediante metaanálisis. Resultados: las tecnologías educativas predominantes fueron la capacitación y la orientación verbal, se destacaron las tecnologías blandas-duras. En comparación con la atención habitual, las tecnologías educativas demostraron ser un factor protector para prevenir la incidencia de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) y la evaluación de certeza de evidencia fue baja. Las tecnologías educativas también demostraron ser un factor protector para prevenir la incidencia de amputación en miembros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) y la certeza de evidencia fue muy baja. Conclusión: tecnologías educativas blandas-duras, como orientación verbal estructurada, juegos educativos, clase expositiva, capacitación teórico-práctica, video educativo, folder, rotafolio educativo y dibujos lúdicos, y tecnologías duras, como calzado terapéutico, plantillas, termómetro infrarrojo digital, kits para el cuidado de los pies, aplicación de telemedicina y teléfono móvil, resultaron efectivas para la prevención y el tratamiento de la úlcera diabética, sin embargo, es necesario que se realicen estudios más robustos.


Objective: to analyze the effects of educational technologies in the prevention and treatment of diabetic ulcers. Method: a systematic review conducted in seven databases, a bibliographic index, an electronic library and the Gray Literature. The sample consisted of 11 randomized controlled clinical trials. The synthesis of the results was descriptive and through meta-analysis. Results: the predominant educational technologies were training sessions and verbal guidelines, with soft-hard technologies standing out. When compared to usual care, the educational technologies presented a protective factor to prevent the incidence of diabetic ulcers (RR=0.40; 95% CI=0.18-0.90; p=0.03) and the certainty of the evidence assessment was low. The educational technologies also had a protective factor to prevent the incidence of lower limb amputations (RR=0.53; 95% CI=0.31-0.90; p=0.02) and certainty of the evidence was very low. Conclusion: soft-hard educational technologies such as structured verbal guidelines, educational games, lectures, theoretical-practical training sessions, educational videos, folders, serial albums and playful drawings, and hard technologies such as therapeutic footwear, insoles, infrared digital thermometer, foot care kits, Telemedicine app and mobile phone use, were effective for the prevention and treatment of diabetic ulcers, although more robust studies are required.

Resultados: as tecnologias educativas predominantes foram os treinamentos e as orientações verbais, destacando-se as tecnologias leve-duras. Na comparação com o cuidado usual, as tecnologias educativas apresentaram fator de proteção para prevenção da incidência de úlcera diabética (RR=0,40; IC 95%=0,18-0,90; p=0,03) e a avaliação de certeza da evidência foi baixa. As tecnologias educativas também tiveram fator de proteção para prevenção da incidência de amputação em membros inferiores (RR=0,53; IC 95%=0,31-0,90; p=0,02) e a certeza da evidência foi muito baixa. Conclusão: as tecnologias educativas leve-duras, como orientações verbais estruturadas, jogos educativos, aula expositiva, treinamentos teórico-práticos, vídeo educativo, folder, álbum seriado e desenhos lúdicos, e as tecnologias duras, a exemplo do calçado terapêutico, palmilhas, termômetro digital de infravermelho, kits de cuidados com os pés, aplicativo de telemedicina e telefone móvel, foram efetivas para prevenção e tratamento da úlcera diabética, porém, estudos mais robustos são necessários.

Objetivo: evaluar el efecto de la intervención educativa que realizan los enfermeros para controlar la presión arterial en personas con hipertensión arterial, en comparación con los cuidados habituales. Método: revisión sistemática con metaanálisis de ensayos clínicos aleatorizados realizada en seis bases de datos. Se incluyeron estudios en los cuales el enfermero llevó a cabo la intervención educativa en la persona con hipertensión arterial. El riesgo de sesgo se evaluó mediante la herramienta Risk of Bias Tool, el metaanálisis se hizo utilizando el software Review Manager y la certeza de la evidencia se calculó usando el sistema Grading of Recommendations Assessment, Development and Evaluation. Resultados: se encontraron 1692 estudios revisados por pares y se incluyeron ocho artículos en el metaanálisis. El metaanálisis se calculó para el resultado presión arterial sistólica y presión arterial diastólica, subagrupados por tiempo y tipo de implementación de la intervención. Para la intervención educativa presencial, realizada d forma individual combinada con actividad grupal, la estimativa del efecto fue -12,41 mmHg (Intervalo de Confianza 95%, -16,91 a -7,91, p<0,00001) para la presión sistólica y -5,40 mmHg (Intervalo 95% Confianza, -7,98 a -2,82, p<0,0001) para la presión diastólica, con certeza de evidencia alta. Conclusión: a intervención educativa realizada por el enfermero, de forma individual combinada con la actividad grupal, tiene efecto clínico y estadísticamente significativo. Objetivo: to assess the effect of an educational intervention performed by nurses for blood pressure control in people with arterial hypertension, when compared to usual care. Método: a systematic review with meta-analysis of randomized clinical trials, conducted in six databases. The studies included were those in which an educational intervention was performed by nurses on people with arterial hypertension. The risk of bias was assessed by means of the Risk of Bias Tool, the meta-analysis was performed in the Review Manager software and certainty of the evidence was calculated in the Grading of Recommendations Assessment, Development and Evaluation system. Resultados: a total of 1,692 studies were found, which were peer-reviewed, including eight of them in the meta-analysis. The meta-analysis was calculated for the "systolic blood pressure" and diastolic blood pressure" outcomes, in subgroups by time and by intervention performance type. For the in-person educational intervention, performed individually combined with a group activity, the effect estimate was -12.41 mmHg (95% Confidence Interval: from -16.91 to -7.91, p<0,00001) for systolic pressure and -5.40 mmHg (95% Confidence Interval: from -7.98 to -2.82, p<0,00001) for diastolic pressure, with high certainty of evidence. Conclusión: the educational intervention performed by nurses, individually and combined with a group activity, presents a statistically significant clinical effect. Objetivo: avaliar o efeito da intervenção educativa realizada por enfermeiros para controle da pressão arterial em pessoas com hipertensão arterial, comparada com cuidado habitual. Método: revisão sistemática com metanálise de ensaios clínicos randomizados realizada em seis bases de dados. Foram incluídos estudos em que a intervenção educativa foi realizada pelo enfermeiro à pessoa com hipertensão arterial. O risco de viés foi avaliado pela Risk of Bias Tool, a metanálise no software Review Manager e a certeza da evidência no sistema Grading of Recommendations Assessment, Development and Evaluation. Resultados: foram encontrados 1692 estudos, revisados por pares, e oito artigos foram incluídos na metanálise. A metanálise foi calculada para o desfecho pressão arterial sistólica e pressão arterial diastólica, em subgrupo por tempo e por tipo de execução da intervenção. Para a intervenção educativa presencial, realizada de modo individual combinada com atividade de grupo, a estimativa de efeito foi de -12.41 mmHg (Intervalo de Confiança 95%, -16.91 a -7.91, p<0.00001) para pressão arterial sistólica e -5.40 mmHg (Intervalo de Confiança 95%, -7.98 a -2.82, p<0.0001) para pressão arterial diastólica, com certeza da evidência alta. Conclusão: a intervenção educativa realizada pelo enfermeiro, de modo individual combinada com atividade de grupo, apresenta efeito clínico e estatisticamente significativo.
