ABSTRACT OBJECTIVE To characterize complementary feeding and to analyze the influence of individual and contextual factors on dietary practices of low birth weight infants. METHODS This cross-sectional study included 2,370 low birth weight infants aged 6 to 12 months included in the Breastfeeding Prevalence Survey in Brazilian Municipalities (2008), which covered the 26 state capitals, the Federal District and 37 municipalities. Dietary practices were assessed using two indicators: I) dietary diversity, characterized by the consumption of five food groups: meat, beans, vegetables, fruit and milk; II) consumption of ultra-processed foods, characterized by the ingestion of at least one of the following foods on the day prior to the survey: soda, or processed juice, or cookie, cracker and crisps. The covariates of interest were the socioeconomic characteristics of infants, mothers and health services. The contextual factor was the "municipal prevalence of child undernutrition." The individualized effect of the study factors on outcomes was assessed by multilevel Poisson regression. RESULTS Approximately 59% of infants consumed ultra-processed foods, while 29% had diverse feeding. Mothers living in municipalities with child undernutrition prevalence below 10%, with higher education and working outside the home were more likely to offer dietary diversity. Consumption of ultra-processed foods was higher among infants living in municipalities with child undernutrition prevalence below 10%, whose mothers were younger and multiparous. CONCLUSIONS The low prevalence of diverse feeding combined with the high prevalence of ultra-processed food consumption characterizes the low quality of feeding of low birth weight Brazilian infants. Individual and contextual factors impact the feeding quality of this population, suggesting the need for effective strategies to increase the consumption of fresh and minimally processed foods and decrease the consumption of ultra-processed foods by this vulnerable population.


RESUMO OBJETIVO Caracterizar a alimentação complementar e analisar a influência de fatores individuais e contextuais sobre práticas alimentares de lactentes que nasceram com baixo peso. MÉTODOS Este estudo transversal incluiu 2.370 lactentes nascidos com baixo peso e com idade entre 6 e 12 meses incluídos na Pesquisa de Prevalência de Aleitamento Materno em Municípios Brasileiros (2008), que abrangeu as 26 capitais, o Distrito Federal e mais 37 municípios. As práticas alimentares foram avaliadas usando dois indicadores: i) diversidade alimentar, caracterizada pelo consumo dos cinco grupos alimentares: carnes, feijão, legumes e verduras, frutas e leite; ii) consumo de alimentos ultraprocessados, caracterizado pela ingestão de pelo menos um dos seguintes alimentos no dia anterior à pesquisa: refrigerante, ou suco industrializado, ou bolacha, biscoito e salgadinho. As covariáveis de interesse corresponderam às características socioeconômicas, dos lactentes, das mães e dos serviços de saúde. O fator contextual foi a "prevalência municipal de desnutrição infantil". O efeito individualizado dos fatores de estudo sobre os desfechos foi avaliado mediante regressão de Poisson com estrutura multinível. RESULTADOS Aproximadamente 59% dos lactentes consumiram alimentos ultraprocessados, enquanto 29% apresentaram diversidade alimentar. Mães que residiam em municípios com prevalência de desnutrição infantil inferior a 10%, com maior nível de escolaridade e que trabalhavam fora de casa foram mais propensas a oferecer diversidade alimentar. O consumo de alimentos ultraprocessados foi maior entre lactentes residentes em municípios com prevalência de desnutrição infantil inferior a 10%, cujas mães eram mais jovens e multíparas. CONCLUSÕES A baixa prevalência de alimentação diversa aliada à alta prevalência do consumo de alimentos ultraprocessados caracteriza a baixa qualidade da alimentação dos lactentes brasileiros com baixo peso ao nascer. Fatores individuais e contextuais impactam a qualidade da alimentação dessa população, sugerindo a necessidade de adoção de estratégias eficazes para aumentar o consumo de alimentos in natura e minimamente processados e diminuir o consumo de alimentos ultraprocessados por esta população vulnerável.

Abstract Objective: To analyze the prevalence of ultra-processed food intake among children under one year of age and to identify associated factors. Methods: A cross-sectional design was employed. We interviewed 198 mothers of children aged between 6 and 12 months in primary healthcare units located in a city of the metropolitan region of São Paulo, Brazil. Specific foods consumed in the previous 24 h of the interview were considered to evaluate the consumption of ultra-processed foods. Variables related to mothers' and children's characteristics as well as primary healthcare units were grouped into three blocks of increasingly proximal influence on the outcome. A Poisson regression analysis was performed following a statistical hierarchical modeling to determine factors associated with ultra-processed food intake. Results: The prevalence of ultra-processed food intake was 43.1%. Infants that were not being breastfed had a higher prevalence of ultra-processed food intake but no statistical significance was found. Lower maternal education (prevalence ratio 1.55 [1.08-2.24]) and the child's first appointment at the primary healthcare unit having happened after the first week of life (prevalence ratio 1.51 [1.01-2.27]) were factors associated with the consumption of ultra-processed foods. Conclusions: High consumption of ultra-processed foods among children under 1 year of age was found. Both maternal socioeconomic status and time until the child's first appointment at the primary healthcare unit were associated with the prevalence of ultra-processed food intake.


Resumo: Objetivo: Analisar a prevalência do consumo de alimentos ultraprocessados entre crianças com menos de um ano e identificar os fatores associados. Métodos: Foi realizado um estudo transversal. Entrevistamos 198 mães de crianças com idades entre 6 e 12 meses em unidades de atenção primária à saúde localizadas em Embu das Artes, uma cidade da região metropolitana de São Paulo, Brasil. Alimentos específicos consumidos nas 24 horas anteriores à entrevista foram considerados para avaliar o consumo de alimentos ultraprocessados. As variáveis relacionadas às características das mães e crianças e as unidades de atenção primária à saúde foram agrupadas em três blocos de influência cada vez mais proximal com o resultado. Foi realizada uma análise de regressão de Poisson de acordo com um modelo estatístico hierárquico para determinar os fatores associados ao consumo de alimentos ultraprocessados. Resultados: A prevalência de consumo de alimentos ultraprocessados foi 43,1%. As crianças que não eram amamentadas apresentaram maior prevalência de consumo de alimentos ultraprocessados, porém não foi encontrada diferença estatística. Menor nível de escolaridade materna (RP 1,55 [1,08-2,24]) e o fato de a primeira consulta da criança na unidade de atenção primária à saúde acontecer na primeira semana de vida (RP 1,51 [1,01-2,27]) foram fatores associados ao consumo de alimentos ultraprocessados. Conclusões: Foi encontrado consumo elevado de alimentos ultraprocessados entre crianças com menos de um ano. A situação socioeconômica materna e o tempo da primeira consulta da criança na unidade de atenção primária à saúde foram associados à prevalência de consumo de alimentos ultraprocessados.

The objectives of this study were to estimate the age of complementary feeding introduction (CFI) and investigate the related health, demographic, and socio-economic factors. Analyses were based on 10,931 infants from the French national birth cohort ELFE, born in 2011. Health, demographic, and socio-economic data concerning infants and parents were collected at birth (face-to-face interviews and medical records) and 2 months (telephone interviews). Data on milk feeding and CFI practices were collected at birth and 2 months then monthly from 3 to 10 months using online or paper questionnaires. The associations between both health and social factors and CFI age were tested by multivariable multinomial logistic regressions. The mean CFI age was 5.2 ± 1.2 months; 26% of the infants started complementary feeding before 4 months of age (CF < 4 months), 62% between 4 and 6 months of age, and 12% after 6 months of age (CF > 6 months). CF < 4 months was more likely when mothers smoked, were overweight/obese, younger (<29 years), and used their personal experience as an information source in child caregiving and when both parents were not born in France. CF < 4 months was less likely when the infant was a girl, second-born, when the mother breastfed longer, and had attended at least one birth preparation class. Mothers of second-born infants and who breastfed their child longer were more likely to introduce CF > 6 months. Couples in which fathers were born in France and mothers were not born in France were less likely to introduce CF > 6 months. CF < 4 months occurred in more than 25% of the cases. It is important to continue promoting clear CFI recommendations, especially in smoking, overweight, young, not born in France, and nonbreastfeeding mothers.

Identificar os fatores associados à introdução de alimentos não recomendados no primeiro ano de vida, entre crianças residentes em municípios de baixo nível socioeconômico. Estudo multicêntrico transversal com 1.567 crianças de 12 a 59 meses de idade residentes em 48 municípios participantes do plano Brasil Sem Miséria da Região Sul do Brasil. Aplicou-se questionário estruturado aos responsáveis pelas crianças para a obtenção das informações sociodemográficas e idade na qual alimentos não recomendados foram introduzidos pela primeira vez na alimentação complementar. A prevalência de introdução de açúcar antes dos quatro meses de idade da criança foi de 35,5% (n = 497; IC95%: 33,1-38,0). As prevalências de introdução de biscoito doce/salgado, queijo petit suisse e gelatina antes do sexto mês de vida da criança foram de 20,4% (n = 287; IC95%: 18,3-22,3), 24,8% (n = 349; IC95%: 22,4-27,1) e 13,8% (n = 192; IC95%: 12,0-15,7), respectivamente. Identificou-se associação entre a menor escolaridade materna (RP = 1,25; IC95%: 1,03-1,51) e a menor renda mensal familiar (RP = 1,22; IC95%: 1,01-1,48) com a introdução de alimentos não recomendados. Verificou-se a introdução de alimentos não recomendados no primeiro ano de vida entre crianças residentes em municípios de alta vulnerabilidade socioeconômica da Região Sul do Brasil, e esta prática associou-se à menor escolaridade materna e menor renda familiar mensal.


El estudio tuvo como fin identificar los factores asociados a la introducción de alimentos no recomendados durante el primer año de vida, entre niños residentes en municipios con un bajo nivel socioeconómico. Se trata de un estudio multicéntrico transversal con 1.567 niños de 12 a 59 meses de edad, residentes en 48 municipios participantes en el plan Brasil Sin Miseria de la región Sur de Brasil. Se aplicó un cuestionario estructurado a los responsables de los niños para la obtención de la información sociodemográfica y la edad en la que los alimentos no recomendados se introdujeron por primera vez en la alimentación complementaria. La prevalencia de introducción del aúcar, antes de los cuatro meses de edad del niño, fue de un 35,5% (n = 497; IC95%: 33,1-38,0). Las prevalencias de la introducción de galletas dulce/saladas, queso petit suisse y gelatina antes del sexto mes de vida del niño fueron de un 20,4% (n = 287; IC95%: 18,3-22,3), un 24,8% (n = 349; IC95%: 22,4-27,1) y un 13,8% (n = 192; IC95%: 12,0-15,7), respectivamente. Se identificó una asociación entre la menor escolaridad materna (RP = 1,25; IC95%: 1,03-1,51) y la menor renta mensual familiar (RP = 1,22; IC95%: 1,01-1,48), con la introducción de alimentos no recomendados. Se verificó la introducción de alimentos no recomendados durante el primer año de vida entre niños residentes en municipios de alta vulnerabilidad socioeconómica de la región Sur de Brasil, y esta práctica se asoció a una menor escolaridad materna y una menor renta familiar mensual.


The study aimed to identify factors associated with the introduction of inappropriate complementary feeding in the first year of life in children living in municipalities (counties) with low socioeconomic statusl. This was a cross-sectional multicenter study in 1,567 children 12 to 59 months of age in 48 municipalities participating in the Brazil Without Poverty plan in the South of Brazil. A structured questionnaire was applied to the children's parents to obtain socio-demographic information and the age at which inappropriate complementary foods were introduced for the first time in complementary feeding. Prevalence of introduction of sugar before four months of age was 35.5% (n = 497; 95%CI: 33.1-38.0). The prevalence rates for the introduction of cookies/crackers, creamy yogurt, and jelly before six months of age were 20.4% (n = 287; 95%CI: 18.3-22.3), 24.8% (n = 349; 95%CI: 22.4-27.1), and 13.8% (n = 192; 95%CI: 12.0-15.7), respectively. Associations were identified between low maternal schooling (PR = 1.25; 95%CI: 1.03-1.51) and low monthly family income (PR = 1.22; CI95%: 1.01-1.48) and the introduction of inappropriate complementary feeding. The study identified the introduction of inappropriate complementary feeding in the first year of life among children in municipalities with high socioeconomic vulnerability in the South of Brazil, associated with low maternal schooling and low monthly family income.

This study aimed to analyze the effect of type of breastfeeding on the nutritional status of children between 12-24 months of age. This cohort study included 435 children born in 2012 in a public hospital in Joinville, Santa Catarina State, Brazil. Two years after delivery the mothers and their children were contacted in their homes for a new investigation of demographic, economic, nutritional, and anthropometric data. In the unadjusted analysis, children who were not exclusively breastfed were more likely to be overweight (including obesity) at 2 years of age (OR = 1.6; p = 0.049) than exclusively breastfed children. After adjusting for several covariates, children who were not exclusively breastfed had a 12% higher risk of overweight including obesity compared to unadjusted analysis (OR = 2.6 vs. OR = 1.8; p = 0.043). In addition, birthweight was also an independent determinant of overweight including obesity (OR = 2.5; p = 0.002). The practice of exclusive breastfeeding can reduce the risk of overweight in children from developing countries such as Brazil.

BACKGROUND: Previous breastfeeding experience has been associated with subsequent infant feeding practices. However, few longitudinal studies have investigated formula-only feeding patterns or the full range of potentially associated characteristics. OBJECTIVE: This study aimed to determine the recurrence of infant feeding practices and maternal, birthing, and infant characteristics associated with recurrent formula-only feeding and changes between exclusive breastfeeding and formula-only feeding across subsequent births. METHODS: We conducted a population-based record-linkage study of 317 027 mothers, with a term singleton live-birth in 2007-2011, New South Wales, Australia. Infant feeding patterns were described using sequential birth pairs. For mothers with a first and second birth, robust Poisson regression was used to investigate the association between maternal, birthing, and infant characteristics and infant feeding patterns. Combined relative risks (RRs) were calculated for selected maternal characteristics. RESULTS: Across 69 994 sequential birth pairs, the recurrence rate of formula-only feeding was 71%, and 92% for exclusive breastfeeding. Maternal characteristics < 25 years old, being Australian born or single, smoking during pregnancy, and living in lower socioeconomic areas were most strongly associated with repeat formula-only feeding (RR, 22.1; 95% confidence interval [CI], 18.6-26.3), changing from exclusive breastfeeding to formula-only feeding (RR, 9.0; 95% CI, 7.4-10.7), and being less likely to change from formula-only feeding to exclusive breastfeeding (RR, 0.47; 95% CI, 0.38-0.59). CONCLUSION: Infant feeding practices were strongly recurrent, highlighting the importance of successful breastfeeding for first-time mothers. Additional support for young mothers from disadvantaged backgrounds accounting for infant feeding history, experiences, and common barriers could improve recurrent exclusive breastfeeding and positively affect infant and maternal health.

BACKGROUND: The question whether diets composed of local foods can meet recommended nutrient intakes in children aged 6 to 23 months living in low- and middle-income countries is contested. OBJECTIVE: To review evidence of studies evaluating whether (1) macro- and micronutrient requirements of children aged 6 to 23 months from low- and middle-income countries are met by the consumption of locally available foods ("observed intake") and (2) nutrient requirements can be met when the use of local foods is optimized, using modeling techniques ("modeled intake"). METHODS: Twenty-three articles were included after conducting a systematic literature search. To allow for comparisons between studies, findings of 15 observed intake studies were compared against their contribution to a standardized recommended nutrient intake from complementary foods. For studies with data on intake distribution, %< estimated average requirements were calculated. RESULTS: Data from the observed intake studies indicate that children aged 6 to 23 months meet requirements of protein, while diets are inadequate in calcium, iron, and zinc. Also for energy, vitamin A, thiamin, riboflavin, niacin, folate, and vitamin C, children did not always fulfill their requirements. Very few studies reported on vitamin B6, B12, and magnesium, and no conclusions can be drawn for these nutrients. When diets are optimized using modeling techniques, most of these nutrient requirements can be met, with the exception of iron and zinc and in some settings calcium, folate, and B vitamins. CONCLUSION: Our findings suggest that optimizing the use of local foods in diets of children aged 6 to 23 months can improve nutrient intakes; however, additional cost-effective strategies are needed to ensure adequate intakes of iron and zinc.

Resumo: Este estudo teve como objetivo avaliar a importância do tipo de aleitamento no risco de excesso de peso de crianças entre 12-24 meses de idade. Trata-se de um estudo de coorte que incluiu 435 crianças nascidas em 2012 em uma maternidade pública de Joinville, Santa Catarina, Brasil. Dois anos após o parto, as mães e seus filhos foram contatados nas residências para uma nova coleta de dados. Na análise não ajustada, crianças que não receberam aleitamento materno exclusivo apresentaram maior risco de desenvolver excesso de peso aos dois anos de idade (OR = 1,6; p = 0,049), quando comparadas às crianças amamentadas exclusivamente. Mesmo após o ajuste para diversas covariáveis, o risco das crianças não amamentadas exclusivamente apresentarem excesso de peso aumentou 12% em relação à análise não ajustada (OR = 2,6 vs. OR = 1,8; p = 0,043). Adicionalmente, o peso ao nascer também mostrou ser um determinante independente do risco de excesso de peso (OR = 2,5; p = 0,002). A prática do aleitamento materno exclusivo pode reduzir o risco de excesso de peso em crianças de países em desenvolvimento como o Brasil.


Resumen: Este estudio tuvo como objetivo evaluar la importancia del tipo de lactancia en el riesgo de exceso de peso de niños entre 12-24 meses de edad. Se trata de un estudio de cohorte que incluyó a 435 niños nacidos en 2012, en una maternidad pública de Joinville, Santa Catarina, Brasil. Tras dos años después del parto, se contactó con las madres y sus hijos en sus residencias para una nueva recogida de datos. En el análisis no ajustado, los niños que no recibieron exclusivamente el pecho materno presentaron mayor riesgo de desarrollar exceso de peso a los dos años de edad (OR = 1,6; p = 0,049), cuando se comparan con los niños amamantados exclusivamente. Incluso tras el ajuste para diversas covariables, el riesgo de que los niños no amamantados exclusivamente presentaran exceso de peso aumentó un 12%, en relación con el análisis no ajustado (OR = 2,6 vs. OR = 1,8; p = 0,043). Asimismo, el peso al nacer también mostró ser un determinante independiente del riesgo de exceso de peso (OR = 2,5; p = 0,002). La práctica de dar exclusivamente el pecho puede reducir el riesgo de exceso de peso en niños de países en desarrollo como Brasil.

BACKGROUND: The early introduction of complementary foods is common in Middle Eastern countries but little is known about the determinants of this practice in this region. OBJECTIVES: This prospective cohort study conducted from October 2007 to October 2008 investigated the determinants of the very early (before 17 weeks) introduction of complementary foods in Kuwait and compared rates of this practice against rates reported in the mid-1990s. METHODS: A total of 373 women were recruited from maternity hospitals in Kuwait City and followed to 26 weeks postpartum. Data on complementary feeding practices were available from 303 women. Multivariate logistic regression was used to estimate the association of very early introduction of complementary foods with infant sex and maternal characteristics including age, years of education, employment intentions at 6 months postpartum, parity, prepregnancy body mass index, and prepregnancy smoking status. RESULTS: All infants had received complementary foods by 26 weeks of age, with 30.4% receiving complementary foods before 17 weeks of age. Women born in other Arabic countries were less likely to introduce complementary foods before 17 weeks (adjusted odds ratio [adj OR] = 0.40; 95% confidence interval [CI], 0.22-0.73) than women born in Kuwait. Women who were exclusively formula feeding at 6 weeks postpartum were less likely to introduce complementary foods before 17 weeks (adj OR = 0.40; 95% CI, 0.23-0.71) than women who were still breastfeeding. CONCLUSION: Compared to the mid-1990s, fewer infants in Kuwait were receiving complementary foods before 17 weeks. Nevertheless, all infants had received complementary foods by 6 months of age.

Objective. To examine the nature and energy contribution of complementary feeding in breastfed infants in their sixth month of life, and the prevalence of the use of bottles as a delivery method. Materials and methods. We recruited 156 breastfeeding infants at a health clinic in metropolitan Quetzaltenango, Guatemala. A previous-day recall was performed. Results. Sixty nine mothers (44%) reported offering items other than breastmilk. The median contribution of energy from complementary foods among infants with mixed feeding (n=66) was 197 kcal/day (interquartile range [IQR] 49-353). The median energy contribution of formula or cow's milk among consumers (n=39) was 212 kcal/day (IQR 84-394). Bottles were used on the previous day by 55 (80%) of the 69 mothers not offering exclusive breastfeeding. Conclusions. Premature introduction of non-breastmilk items is commonly practiced in feeding Guatemalan infants. Adherence to the internationally recognized guidelines for early infant feeding should be an intervention priority for this population.


Objetivos. Examinar la naturaleza y contribución energética de la alimentación complementaria en infantes lactantes en el sexto mes de vida y la prevalencia del uso de biberones como método de alimentación. Material y métodos. Se seleccionaron 156 infantes lactantes en un centro de salud en la ciudad de Quetzaltenango, Guatemala. Se solicitó una rememoración de la alimentación del día anterior. Resultados. Sesenta y nueve madres (44%) reportaron dar alimentos adicionales a la leche materna. La mediana de contribución energética de alimentos complementarios en infantes con alimentación mixta (n=66) fue 197 kcal/día (rango intercuartil [RIC] 43-353). La mediana de contribución energética de leche de vaca entre consumidores (n=39) fue 212 kcal/día (RIC 84-394). De 69 madres que ofrecían alimentación mixta, 55 (80%) proporcionaron biberones. Conclusiones. Las madres guatemaltecas practican comúnmente la introducción prematura de la alimentación complementaria. La adhesión a las guías internacionalmente reconocidas para alimentación de infantes debe ser una prioridad de intervención en esta población.

This study aimed to analyze time trends in complementary feeding practices among children under one year of age in the city of Rio de Janeiro, Brazil, from 1998 to 2008. Based on comparable questions on food consumption in five surveys conducted in representative randomized samples of children in 1998 (n = 3,762), 2000 (n = 3,670), 2003 (n = 4,305), 2006 (n = 3,686), and 2008 (n = 2,621), 16 complementary feeding indicators were constructed. The percentage frequency of all indicators was estimated for each year of the study. Linear trend analyses were also conducted by means of multivariate regression models. All indicators of early complementary feeding decreased from 1998 to 2008. In the 6-11.9 month age group, there was a significant downward trend in the proportion of children receiving food with adequate consistency for their age, consumption of iron-rich foods, and overall adequacy of the diet. A significant upward trend in fruit consumption was observed. There was a decrease in the early complementary feeding, and generally a worsening of complementary feeding practices in children aged 6-11.9 months in Rio de Janeiro during the period studied.


Este estudo objetivou analisar as tendências temporais das práticas de alimentação complementar entre crianças < 1 ano de idade na cidade do Rio de Janeiro, Brasil, de 1998 a 2008. Com base em questões comparáveis sobre o consumo de alimentos de cinco inquéritos realizados em amostras probabilísticas de crianças em 1998 (n = 3.762), 2000 (n = 3.670), 2003 (n = 4.305), 2006 (n = 3.686) e 2008 (n = 2.621), foram construídos 16 indicadores de alimentação complementar. A frequência percentual dos indicadores foi estimada para cada ano. Análises de tendências lineares foram realizadas por meio de modelos de regressão multivariada. Os indicadores de alimentação complementar precoce diminuíram de 1998 a 2008. No grupo de 6 a 11,9 meses de idade, houve tendência significativa do aumento no consumo de frutas e de queda na proporção de crianças que recebiam preparações com consistência adequada para sua idade, que consumiam alimentos ricos em ferro e que recebiam alimentação adequada. Durante o período estudado, houve queda da alimentação complementar precoce e, em geral, pioraram as práticas de alimentação complementar em crianças de 6 a 11,9 meses no Rio de Janeiro.


Se analizaron las tendencias temporales de las prácticas de alimentación complementaria en niños < 1 año de edad en la ciudad de Río de Janeiro, Brasil, de 1998 a 2008. En base a preguntas comparables acerca del consumo de alimentos en cinco encuestas, basadas en muestras aleatorias de niños en 1998 (n = 3.762), 2000 (n = 3.670), 2003 (n = 4.305), 2006 (n = 3.686) y 2008 (n = 2.621), se construyeron 16 indicadores de alimentación complementaria. Se calculó para cada año la frecuencia porcentual de los indicadores. Los análisis de tendencia lineal se realizaron por medio de modelos de regresión multivariable. Los indicadores de alimentación complementaria precoz disminuyeron. En niños con 6 a 11,9 meses de edad, hubo una tendencia significativa de aumento del consumo de frutas y una caída en la proporción de los que recibieron papillas con una consistencia adecuada para su edad, que consumieron alimentos ricos en hierro y que recibieron una alimentación adecuada. Durante el período de estudio, hubo una disminución de la alimentación complementaria precoz y, en general, un deterioro de las prácticas de alimentación complementaria en niños de 6 a 11,9 meses de edad en Río de Janeiro.

Objective: to evaluate the complementary feeding practices for infants, focusing on the main pureed baby foods, and verify adherence to the guidelines adopted in Brazil. Methods: through cross-sectional study, aspects of complementary feeding of 404 healthy infants between 4 and 9 months of age (São Paulo, Curitiba and Recife) were evaluated. Socio-demographic data, history and food habits were collected. Mothers described three recipes (preparations) usually used in key baby foods. The findings were compared with those recommended by the Brazilian Society of Pediatrics. Results: the average age was 6.9±1.6 months. Among infants, 241/404 (59.6%) were still breastfeeding. Among those who received another type of milk, 193/368 (52.4%) received whole cow's milk, while 151/368 (41.0%) drank infant or follow-on formulas. Regarding baby food recipes salted reported by mothers, it was seen that 30% and 60% contained meat and vegetables, respectively. The percentages less suitable for feeding in general were observed for use of cow's milk and added sugar, chocolate and cereal in feeding bottles; 79% and 80.5% of the families interviewed would adopted such practices. Conclusion: the early termination of exclusive/predominant breastfeeding and the practice of an inadequate transition diet have shown a picture of quantitatively and qualitatively inadequate feeding, with the risk of causing serious nutritional problems in later ages, such as anemia and vitamin A deficiency, or excess of nutrients, leading to obesity, diabetes and dyslipidemias. .


Objetivo: avaliar as práticas relacionadas à alimentação complementar de lactentes, com ênfase nas papas principais, e verificar a concordância com as recomendações atualmente adotadas no Brasil. Métodos: por meio de estudo transversal, foram avaliados os aspectos da alimentação complementar de 404 lactentes saudáveis entre 4 e 9 meses de vida (São Paulo, Recife e Curitiba). Coletaram-se dados sociodemográficos de antecedentes e hábitos alimentares. As mães descreveram três receitas (preparações) que costumam utilizar nas papas principais. Os achados foram comparados ao preconizado pela Sociedade Brasileira de Pediatria. Resultados: a média de idade foi de 6,9±1,6 meses. Cerca de 241 lactentes (59,6%) permaneciam em aleitamento materno. Entre os que recebiam outro tipo de leite, 193 dos 368 (52,4%) recebiam leite de vaca integral, e 151 dos 368 (41%), fórmulas infantis ou de seguimento para lactentes. Em relação às receitas de papas salgadas relatadas pelas mães, foi possível observar que 30 e 60% delas continham leguminosas e carnes, respectivamente. Os piores percentuais de adequação na alimentação, em geral, foram observados para uso de leite de vaca e adição de açúcar, achocolatado e cereais em mamadeiras; 79 e 80,5% das famílias entrevistadas adotavam essas práticas. Conclusão: o abandono precoce do aleitamento materno exclusivo/predominante e a prática de uma dieta de transição inadequada têm mostrado um quadro de consumo quantitativa e qualitativamente inapropriado, com riscos de acarretar graves problemas nutricionais nas faixas etárias posteriores, como anemia e hipovitaminose A, ou excessos de nutrientes, como obesidade, diabete e dislipidemias. .

Data on the residue levels of persistent organic pollutants (POPs) in baby food samples are scarce. This is the first study to explore current contamination status and exposure assessment of organochlorines (OCs), including polychlorinated biphenyls (PCBs) and organochlorine pesticides (OCPs), in baby food from Korea. In this study, the concentrations of OCs were determined in homemade baby food samples (n=100) collected from 6-, 9-, 12- and 15-month-old infant groups. The average concentrations of PCBs, dichloro-diphenyl-trichloroethanes (DDTs), hexachlorocyclohexanes (HCHs) and chlordanes (CHLs) in baby food samples were 37.5, 96.6, 26.0, and 13.2 pg/g fresh weight, respectively. The major compounds were CBs 28, 153, 52, and 33 for PCBs and p,p'-DDE, p,p'-DDT and ß-HCH for OCPs. The contribution of DDTs to the total OC concentrations increased from 30% (6-month-old infants) to 67% (15-month-old infants) with increasing infant age, while the concentrations of PCBs, HCHs and CHLs gradually decreased with increasing infant age, suggesting that highest priority for risk reduction of DDTs. The estimated daily intakes (EDIs) of OCs in Korean infants from baby food consumption were lower than the thresholds proposed by the United States Environmental Protection Agency and Health Canada, implying limited potential health risks. However, considering simultaneous exposure from baby food and breast milk consumption, chlordanes and heptachlor epoxide posed potential health risks. Considering the importance of early development and the vulnerability of infants, it is essential to perform systematic monitoring and management programs of OCs in baby food for risk reduction in Korean infants.

Continued breastfeeding at year 1 is one of the core indicators for assessing global infant and young child feeding practices. The study aims to determine the frequency of breastfeeding after 12 months (long-term breastfeeding) according to a number of infant and maternal characteristics and to investigate the effects of long-term breastfeeding on the nutritional habits and growth status of children as seen in the national data. The sample included 1666 children aged 12 to 35 months from the 2003 Turkey Demographic and Health Survey. Only 55.9% of children were breastfed beyond 12 months. The rates of long-term breastfeeding were higher in the presence of the following characteristics: high birth order, long preceding birth interval, religious marriage ceremony, usage of traditional contraceptive methods, mothers aged 30-34 years and overweight mothers. Rates were lower for mothers with tobacco exposure and for bottle-fed infants. Long-term breastfeeding did not affect the consumption of plain yogurt, solid foods and semi-solid foods; however, it was associated with a decreased intake of bottled milk and fruit juice. The rates of long-term breastfeeding were similar in the case of undernourished children and of those with normal growth status. Long-term breastfeeding was related to certain maternal and infant characteristics; however, it did not affect the consumption of complementary food. Breastfeeding promotion programs should include targeted interventions for younger, primiparous and smoking mothers.

OBJETIVO: avaliar o consumo, frequência e tipo de leite ingerido por crianças brasileiras menores de 60 meses. MÉTODOS: estudo transversal baseado em dados secundários da Pesquisa Nacional de Demografia e Saúde de 2006/7. Foram utilizados dados de 4.718 crianças menores de 60 meses. Todas as análises foram realizadas com amostra expandida. O inquérito dietético representou o consumo do dia anterior e foi estimado por meio de análise univariada apresentada em percentual. RESULTADOS: consumiram leite materno no dia anterior à investigação 91% das crianças menores de seis meses e 34,8% das crianças de 13 a 24 meses. Entre as crianças que receberam outros leites, o leite de vaca foi consumido por 62,4% das crianças menores de seis meses, por 74,6% das crianças de 6 a 12 meses e por aproximadamente 80% das crianças maiores de doze meses. O consumo de fórmulas infantis foi de 23% em crianças menores de seis meses, 9,8% na idade de 6 a 12 meses e menor 1% nas demais idades. O consumo de leite de soja variou de 14,6% a 20% nas idades investigadas. O consumo de leite não materno antes de seis meses de idade foi mais frequente em crianças residentes nas Regiões Nordeste e Sul. CONCLUSÃO: os resultados deste estudo mostram que a maior parte das crianças recebeu leite de vaca em substituição ao leite materno. Enfatiza-se a importância do fomento às políticas públicas de promoção, proteção e apoio ao Aleitamento Materno em todas as regiões brasileiras para reversão do cenário observado.


OBJECTIVE: to assess the intake, frequency, and type of milk consumed by Brazilian children younger than 60 months of age. METHODS: this was a cross-sectional study, which used secondary data from the National Demographic and Health Survey of 2006-2007. Data from 4,817 children under the age of 60 months were used. All analyses were performed with expanded samples. The dietetic survey assessed the previous day's consumption, and estimates were made through univariate analysis, presented as a percentage. RESULTS: on the day prior to the survey, breast milk was consumed by 91% of the children younger than six months of age, by 61.5% of the children aged 6 to 12 months, and by 34.8% of the children aged 13 to 24 months. Among the children who had received other types of milk, cow's milk was consumed by 62.4% of the children younger than six months, by 74.6% of the children aged 6 to 12 months, and by approximately 80% of the children older than 12 months. Infant formulas were consumed by 23% of the children younger than six months of age, by 9.8% of the children aged 6 to 12 months, and by less than 1% of the older children. Soy milk consumption varied from 14.6% to 20% among the investigated ages. Non-breast milk consumption before the age of six months was more frequent in children living in the Northea CONCLUSION: the results of the present study demonstrated that most children received cow's milk prematurely as a substitute for breast milk, highlighting the importance of the development of public policies to promote, protect, and support breastfeeding in all regions of Brazil, aiming at reversing the observed scenario.

Poland faces the same problem of obesity epidemics as other European countries. Polish contribution to the EU research projects concerning overweight and obesity is a direct consequence of recognition of this problem and scientific activity of Polish researchers. At present time early prevention seems to be the only effective approach to decrease obesity and obesity-related chronic diseases in adulthood. Three studies described here consider early prevention of obesity - by decreasing protein intake in infancy (CHOP and EARNEST study) or by behavioural approach in preschoolers (TOYBOX). The Children's Memorial Health Institute is participating in all these projects as a recruiting centre of the subjects studied and as a central laboratory for the CHOP and EARNEST studies. The CHOP study proved in a randomized trial that high protein intake in infancy increases the risk of obesity. The results of the CHOP studies have already indicated the need for protein reduction in infant formulas. This was reduced to 1.8 g/100kcal in the 2006 EU Directive. The results from the TOYBOX project which was started in 2010 have not yet been published.

OBJECTIVE: To identify factors associated with exclusive breast-feeding and breast-feeding during the first year of life among Norwegian infants. DESIGN: Data on breast-feeding practices were collected by a semi-quantitative FFQ. SETTING: In 2006-2007 about 3000 infants were invited to participate in a population-based prospective cohort study in Norway. SUBJECTS: A total of 1490 mothers/infants participated at both 6 and 12 months of age. RESULTS: Exclusive breast-feeding at 4 months was associated with parental education, parity and geographical region, while exclusive breast-feeding at 5·5 months was associated only with maternal age. At both ages, a negative association with exclusive breast-feeding was observed for maternal smoking. Breast-feeding at 6 months was associated with parental education, maternal age and marital status. Breast-feeding at 12 months was associated with maternal education, maternal age and number of children. At both ages, negative associations with breast-feeding were observed for maternal smoking and descending birth weight. At 12 months, a negative association was also observed for having day care by other than the parents. CONCLUSIONS: Even though Norway has an extensive and positive breast-feeding tradition and a maternal leave system that supports the possibility to breast-feed, factors like maternal education, maternal age and maternal smoking are strongly associated with duration of exclusive breast-feeding and breast-feeding. Research to better understand the reasons for inequalities in breast-feeding is needed to facilitate the development of more effective breast-feeding promotion strategies. This again may improve compliance with recommendations and reduce inequalities in infant feeding practices.

BACKGROUND: A plant-based diet protects against chronic oxidative stress-related diseases. Dietary plants contain variable chemical families and amounts of antioxidants. It has been hypothesized that plant antioxidants may contribute to the beneficial health effects of dietary plants. Our objective was to develop a comprehensive food database consisting of the total antioxidant content of typical foods as well as other dietary items such as traditional medicine plants, herbs and spices and dietary supplements. This database is intended for use in a wide range of nutritional research, from in vitro and cell and animal studies, to clinical trials and nutritional epidemiological studies. METHODS: We procured samples from countries worldwide and assayed the samples for their total antioxidant content using a modified version of the FRAP assay. Results and sample information (such as country of origin, product and/or brand name) were registered for each individual food sample and constitute the Antioxidant Food Table. RESULTS: The results demonstrate that there are several thousand-fold differences in antioxidant content of foods. Spices, herbs and supplements include the most antioxidant rich products in our study, some exceptionally high. Berries, fruits, nuts, chocolate, vegetables and products thereof constitute common foods and beverages with high antioxidant values. CONCLUSIONS: This database is to our best knowledge the most comprehensive Antioxidant Food Database published and it shows that plant-based foods introduce significantly more antioxidants into human diet than non-plant foods. Because of the large variations observed between otherwise comparable food samples the study emphasizes the importance of using a comprehensive database combined with a detailed system for food registration in clinical and epidemiological studies. The present antioxidant database is therefore an essential research tool to further elucidate the potential health effects of phytochemical antioxidants in diet.

To describe the infant feeding practices in the general population in Uganda, and to assess the impact of maternal HIV status on these practices, a questionnaire was administered to women attending the follow-up clinics for child vaccination. Among the mothers who were still breastfeeding at the time of interview (N=838), 61.4% of the HIV-infected women had planned to breastfeed for a maximum of 6 months, compared with 12.1% of the HIV-uninfected women (p<0.001). Among the women who were not breastfeeding at the time of interview (N=108), 82.5% of the HIV-infected women had stopped breastfeeding within 3 months, compared with 23.5% of the HIV-uninfected women (p<0.001). Only 2.1% of HIV-infected women seen up to 14 weeks postnatally practised mixed feeding, compared with 23.6% of HIV-uninfected women (p<0.001). After 6 months, however, 30% of the HIV-infected women and 55% of the HIV-uninfected mothers were using mixed feeding, with no significant differences. Programmes for the prevention of mother-to-child transmission of HIV should re-enforce counseling activities to address the issue of early weaning by HIV-infected women, and to support safe breastfeeding up to 6 months.

OBJECTIVES: To evaluate the relationship between breastfeeding and the prevalence of obesity and metabolic syndrome in a group of obese children and adolescents. MATERIAL AND METHODS: We performed a retrospective study in obese children and adolescents treated at the Endocrinology and Nutrition Service of the Hospital de Getafe (Madrid). The variables studied were age, sex, height, weight, body mass index (BMI), waist circumference, waist-height ratio, blood pressure, triglycerides, high-density lipoprotein and fasting glucose. Information was also collected on food received in the first months of life. Breastfeeding was defined as feeding with maternal milk for at least 3 months. The SPSS v.15 statistical package was used. RESULTS: A total of 126 patients with obesity were recruited (71 boys and 55 girls) with a mean age of 11.94 +/- 3.12 years. Of these, 117 (92.86%) were morbidly obese (BMI > 97th percentile for age and sex). All patients had a waist circumference > 90th percentile for age and sex. Of the 126 patients evaluated, 36.8% were breastfed for more than 3 months and 63.2% were fed with artificial milk only. Compared with patients fed with artificial milk, those who were breast fed had a lower BMI (31.53 +/- 5.77 vs 32.08 +/- 6.78) and lower waist circumference (95.02 +/- 3.4 vs. 95.69 +/- 3.2 cm), although this difference was not statistically significant. The prevalence of metabolic syndrome in the study population was 19.8%. Within this group, 64% had not been breast fed compared with 36% who had been fed with artificial milk. CONCLUSIONS: Breast feeding for at least 3 months was associated with lower levels of obesity, smaller waist circumference and fewer complications related to metabolic syndrome in childhood and adolescence. Sixty-four percent of children with complete metabolic syndrome had received artificial feeding. Further studies are needed to ascertain the impact of breastfeeding on the development of obesity and cardiometabolic risk.
