RESUMO

Abstract The incorrect disposal of medicines and their environmental impact has been related to the health medicalization and the improper use of medication by society. In this sense, it is very important to know the profile of drug disposal for foster health policies. The aim was to identify the profile of disposal of medicines by the population, including the cost perspective. This is an inquiry descriptive study that began in September 2019. Medicine disposal health education program was carried out over six months in two University pharmacies. A questionnaire for sociodemographic and discarded medicines data collection was applied. Logistic regression analysis for variables association of correct disposal and the chi-square and t-student analysis for comparison between disposal programs were performed for a level of 5% and test power of 80%. Medicines weighed 23.3 kg and 28.5 kg, with the cost variation from US$ 13.5 to US$ 16.1 until the final treatment. The correct disposal was strongly associated with the disposal reason (p=0.013), source of information (p=0.006), prescription (p=0.03), form of use (p=0.01), acquisition source (p=0.001), cost with medication (p=0.0001), education (p=0.028) and age (p=0.05). The correct medicine disposal was associated with important features of the community related to education health.

RESUMO

Abstract The aim of this study was to identify and analyze the potential interactions between psychotropic drugs and alcohol and tobacco addiction. A cross-sectional study was carried out on secondary data collection in a Center for Psychosocial Care in Alcohol and Other Drugs. Subjects aged 18 years old and over, with alcohol and tobacco dependence, who were taking psychotherapies were included. Medical records with the most recent prescriptions were reviewed. Potential interactions between psychotropic drugs and alcohol and tobacco were analyzed using the Micromedex database and stratified according to clinical risks and mechanisms of action. The Pearson's Chi-square test was used to find significant associations between the variables of interest. The significance level was set at 5%. Between the 2010-2018 period, 2,322 subjects were treated at the care center. Of these, 1,020 fulfilled the inclusion criteria, out of whom 515 (50.5%) were dependent on alcohol and 310 (30.4%) were dependent on tobacco. We found 1,099 potential interactions between psychotropic drugs and alcohol and 160 potential interactions between psychotropic drugs and tobacco. In relation to alcohol dependence, psychotropic drugs interacted largely with moderate clinical risk, and pharmacokinetic mechanisms of action. In relation to tobacco dependence, high clinical risk interactions and pharmacodynamic mechanisms of action predominated.

RESUMO

Abstract The present study proposes and evaluates the test-retest reliability of indicators of the correct use of sodium alendronate in elderly patients. This is a test-retest reliability study for use of sodium alendronate. Six questions to evaluate the correct use of this medicine were elaborated after analysis of information in the literature. Data collection was performed through questionnaires in face-to-face in-home interviews by previously trained interviewers. The participants were initially interviewed (test) when they agreed to participate in the study, and secondly (retest), after a period of 7 to 14 days from the first interview. The reliability of the questions was evaluated by means of the agreement percentage and the Kappa coefficient. Fifty-seven pairs (test-retest) were obtained. The mean age was 69.3 (SD = 6.9) years, the majority (92.5%) completed elementary education, and declared themselves white (50.9%). All the questions presented high concordance ranging from 79.0% to 98.3%. The Kappa values ranged from 0.1 (low) to 0.83 (very good). The agreement percentage and the Kappa values suggest adequate reliability of the proposed questions. We suggest that they can be used as a simple and quick way to evaluate the quality of sodium alendronate use among the elderly.

RESUMO

INTRODUCTION: Operating room recording, via video, audio and sensor-based recordings, is increasingly common. Yet, surgical data science is a new field without clear guidelines. The purpose of this study is to examine existing published studies of surgical recording modalities to determine which are available for use in the operating room, as a first step towards developing unified standards for this field. METHODS: Medline, EMBASE, CENTRAL and PubMed databases were systematically searched for articles describing modalities of data collection in the operating room. Search terms included 'video-audio media', 'bio-sensing techniques', 'sound', 'movement', 'operating rooms' and others. Title, abstract and full-text screening were completed to identify relevant articles. Descriptive statistical analysis was performed for included studies. RESULTS: From 3756 citations, 91 studies met inclusion criteria. These studies described 10 unique data-collection modalities for 17 different purposes in the operating room. Data modalities included video, audio, kinematic and eye-tracking among others. Data-collection purposes described included surgical trainee assessment, surgical error, surgical team communication and operating room efficiency. CONCLUSION: Effective data collection and utilization in the operating room are imperative for the provision of superior surgical care. The future operating room landscape undoubtedly includes multiple modalities of data collection for a plethora of purposes. This review acts as a foundation for employing operating room data in a way that leads to meaningful benefit for patient care.

RESUMO

BACKGROUND: Physical activity and sleep are behavioral risk factors for cancer that may be influenced by environmental exposures, including built and natural environments. However, many studies in this area are limited by residence-based exposure assessment and/or self-reported, time-aggregated measures of behavior. METHODS: The Nurses' Health Study 3 (NHS3) Mobile Health Substudy is a pilot study of 500 participants in the prospective NHS3 cohort who use a smartphone application and a Fitbit for seven-day periods, four times over a year, to measure minute-level location, physical activity, heart rate, and sleep. RESULTS: We have collected data on 435 participants, comprising over 6 million participant-minutes of heart rate, step, sleep, and location. Over 90% of participants had five days of ≥600 minutes of Fitbit wear-time in their first sampling week, and this percentage dropped to 70% for weeks 2 to 4. Over 819 sampling weeks, we observed an average of 7,581 minutes of heart rate and step data [interquartile range (IQR): 6,651-9,645] per participant-week, and >2 million minutes of sleep in over 5,700 sleep bouts. We have recorded location data for 5,237 unique participant-days, averaging 104 location observations per participant-day (IQR: 103-107). CONCLUSIONS: This study describes a protocol to incorporate mobile health technology into a nationwide prospective cohort to measure high-resolution objective data on environment and behavior. IMPACT: This project could provide translational insights into interventions for urban planning to optimize opportunities for physical activity and healthy sleep patterns to reduce cancer risk.See all articles in this CEBP Focus section, "Modernizing Population Science."

RESUMO

Inpatient falls are among the most common adverse events threatening patient safety. Although many studies have developed predictive models for fall risk, there are some drawbacks. First, most previous studies have relied on an incident-reporting system alone to identify fall events. Thus, it has been found that falls are more likely to be underreported. Second, there has been a controversy on how to select accurate representative values for patient status data across multiple times and various data sources in electronic health records. Given this background, this study used nurses' progress notes as a complementary data source to detect fall events. In addition, we developed criteria including coverage, currency, and granularity in order to integrate electronic health records data documented at multiple times in various data types and sources. Based on this methodology, we developed three models, logistic regression, Cox proportional hazard regression, and decision tree, to predict risk of patient falls and evaluate the predictive performance of these models by comparing the results to results from the Hendrich II Fall Risk Model. The findings of this study will be used in a clinical decision support system to predict risk of falling and provide evidence-based tailored recommendations in the future.

RESUMO

The US Food and Drug Administration's 2013 Unique Device Identification System Rule requires manufacturers to label devices with unique identifiers. Implantable devices are now shipped with unique identifiers, and many electronic health records have fields to incorporate them. Health policy changes have prompted hospital systems to assess implementation of implant barcode scanning systems to capture unique device identifiers. Project aims were to assess predictors of operating room nurses' acceptance of a new implant barcode scanning system, describe operating room nurses' perceptions of the system value, and identify operating room nurses' perceived gaps in system implementation. An online survey was disseminated to operating room nurses, and focus groups were conducted with orthopedic operating room nurses in an academic medical center that had recently implemented an implant barcode scanning system in surgical services. Predictors of barcode scanning acceptance included perceived usefulness for patient care, perceived ease of use, and perceived usefulness (self). Nurses perceived the system to be more accurate and valuable for patient safety. Perceived gaps in system implementation related to communication, completeness of the system, consistency in process, and training. Understanding nurse perceptions of new barcode scanning systems and engaging them in the implementation process are key areas for success and optimization of these systems.

RESUMO

Abstract Aims: Recently, high-intensity training methods have become popular, integrating the cardiovascular and neuromuscular training in a single training session, among these methods is CrossFit®. The objective of this study was to analyze the superficial thermal response to CrossFit® exercise in men and women, in order to use this knowledge to prevent overuse injuries. Methods: Nineteen volunteers involved in CrossFit® exercise for more than 6-month (12 males and 7 females) were recruited. The acquisition of the thermal images was performed in a climatized room in two moments, at rest (before exercise), and after one CrossFit® training session. The training session lasted 45min, comprising warm-up (10-min), accessory work (15-20min), and workout of the day (15-20-min). Before the first image acquisition, volunteers were acclimated for 15 min. The Wilcoxon signed-rank test was used to compare the skin temperature between pre- and post-exercise. Results: Temperatures rose significantly pre- to post-exercise in the forearm and anterior thigh regions, while it decreased in the anterior thorax and dorsal lower back regions. These results were found both, in the overall sample, and the male volunteers, but not when the female results were isolated. Conclusion: It can be concluded that superficial thermal response to one CrossFit® training session was characterized and was different for men and women. The superficial thermal responses were aligned with the physiological alterations promoted by other modalities, such as resistance training, cycling, and running.

ABSTRACT Monoclonal antibodies are a useful tool for laboratory diagnosis and an instrument used in the treatment of various diseases and represent one of the most important groups of new drugs for the treatment of cancer. The revolution in the world occured in 1975 when Milstein and Köhler discovered monoclonal antibodies in Cambridge. Objective To review the use of monoclonal antibodies in medicine and in the treatment of cancer. To provide a generalized vision of the concept of monoclonal antibody to explain its therapeutic applicability, and to approach an economic, health-care approach to obtaining and accessing new therapies. Method In the characterization of the research phenomenon, the descriptive study, the collection of documentary data and the correlation between the different sources were used. Discussion However, the costs for both the patient and the public health systems are still high, and the cost-effectiveness assessment must be optimized so that cost-effectiveness and access to time for patients can be compatible. And the challenge of developing new mAbs aimed at new targets, improving the safety profile, avoiding, or reducing adverse immune reactions and achieving lower production costs through improvements in biotechnology, is left open.(AU)

RESUMO

BACKGROUND: Automated hand hygiene monitoring systems (AHHMS) are being developed to supplement direct observations of hand hygiene (HH). We compared compliance rates generated by direct observations and by a badge-based AHHMS. METHODS: Observations of HH compliance were conducted in a surgical intensive care unit (SICU) and a general medical ward (GMW) during a 7-month baseline period and a 13-week intervention period, when compliance was also estimated using an AHHMS. Compliance rates were analyzed using time-series analysis. RESULTS: During the entire pre- and postintervention period, univariate analysis of observations revealed significant improvement in overall HH compliance in the SICU (P = .001) and the GMW (P = .03), beginning before implementation of the AHHMS. Initiation of the AHHMS was associated with a transient drop in entry and exit compliance on both units. During the intervention period, observations suggested that compliance on entry and exit continued to increase in the SICU, but not in the GMW. Time series analysis of AHHMS data revealed that entry and exit compliance did not change significantly in the SICU but decreased significantly in the GMW (P = .001). The average accuracy of the AHHMS was 60%. CONCLUSIONS: Based on observations, HH compliance increased during the baseline period. Observations and the AHHMS yielded different trends in compliance. The AHHMS's accuracy of HH events and health care personnel location were suboptimal.

RESUMO

The ability to determine high-quality, artefact-free structures is a challenge in micro-crystallography, and the rapid onset of radiation damage and requirement for a high-brilliance X-ray beam mean that a multi-crystal approach is essential. However, the combination of crystal-to-crystal variation and X-ray-induced changes can make the formation of a final complete data set challenging; this is particularly true in the case of metalloproteins, where X-ray-induced changes occur rapidly and at the active site. An approach is described that allows the resolution, separation and structure determination of crystal polymorphs, and the tracking of radiation damage in microcrystals. Within the microcrystal population of copper nitrite reductase, two polymorphs with different unit-cell sizes were successfully separated to determine two independent structures, and an X-ray-driven change between these polymorphs was followed. This was achieved through the determination of multiple serial structures from microcrystals using a high-throughput high-speed fixed-target approach coupled with robust data processing.

RESUMO

Harnessing the anomalous signal from macromolecular crystals with volumes of less than 10 000 µm3 for native phasing requires careful experimental planning. The type of anomalous scatterers that are naturally present in the sample, such as sulfur, phosphorus and calcium, will dictate the beam energy required and determine the level of radiation sensitivity, while the crystal size will dictate the beam size and the sample-mounting technique, in turn indicating the specifications of a suitable beamline. On the EMBL beamline P13 at PETRA III, Mesh&Collect data collection from concanavalin A microcrystals with linear dimensions of ∼20 µm or less using an accordingly sized microbeam at a wavelength of 1.892 Š(6.551 keV, close to the Mn edge at 6.549 keV) increases the expected Bijvoet ratio to 2.1% from an expected 0.7% at 12.6 keV (Se K edge), thus allowing experimental phase determination using the anomalous signal from naturally present Mn2+ and Ca2+ ions. Dozens of crystals were harvested and flash-cryocooled in micro-meshes, rapidly screened for diffraction (less than a minute per loop) and then used for serial Mesh&Collect collection of about 298 partial data sets (10° of crystal rotation per sample). The partial data sets were integrated and scaled. A genetic algorithm for combining partial data sets was used to select those to be merged into a single data set. This final data set showed high completeness, high multiplicity and sufficient anomalous signal to locate the anomalous scatterers, and provided phasing information which allowed complete auto-tracing of the polypeptide chain. To allow the complete experiment to run in less than 2 h, a practically acceptable time frame, the diffractometer and detector had to run together with limited manual intervention. The combination of several cutting-edge components allowed accurate anomalous signal to be measured from small crystals.

RESUMO

Microalgae, photosynthetic microorganisms, are rich in lipids, polyunsaturated fatty acids, carbohydrates, proteins, vitamins, as well as carotenoids, which are antioxidants that may protect human body from various diseases including obesity, cardiovascular disease, vision-related diseases such as macular degeneration and certain types of cancer. These natural pigments have applications in the pharmaceutical (nutraceutical), food (coloring, functional food, and supplements), and cosmetics industries (e.g. sunscreen), as well as in aquaculture (animal feed). The Dunaliella salina microalga can synthesize 10% of dry weight in ß-carotene (orange pigment, pro-vitamin A activity) under high light intensity and nitrogen and phosphorus limitation, among other stress conditions. The first chapter of this thesis presents a review focused on microalgae carotenoids: culture systems, mode of operation, and applications. In this bibliographic survey, the advantages of microalgae cultivation in relation to traditional sources (higher plants) were discussed, as well as a discussion of the main cultivation systems and their importance in cell growth. This review presented a critical analysis of the different operational regimes like batch, fed-batch, semi-continuous and continuous. Relevant information on the most important world producers of microalgae carotenoids were presented. Chapter II presents the development of a modified method of dispersive liquid-liquid microextraction (DLLME) for rapid extraction of ß-carotene from Dunaliella salina cultivated in tubular photobioreactor, with subsequent development of a rapid chromatographic screening method using a C4 column for separation of geometric isomer of ß-carotene. The use of benzene as extraction solvent and water with 50% acetone as dispersant provided the best condition for the extraction of this carotenoid. In HPLC (High Performance Liquid Chromatography), employing mobile phase composed of methanol and water (95:5, v/v), it was possible to detect/quantify ß-carotene at 14 min (retention time). Besides the short analysis time (<20 min), by the miniaturized extraction (< 10 mL organic waste) this method abide by green chemistry analytical principles. It is known that nitrogen, phosphorus, as well as carbon and vitamins are vital elements for the growth of microalgae, also determining the biochemical composition of biomass. In this sense, Chapter III presents the study of the influence of different amounts of sodium nitrate (1N = 75 mg L-1; 1.5N = 112.5 mg L-1, and 3N = 225 mg L-1) and phosphate monobasic dehydrate (1P = 5.65 mg L-1, 1.5P = 8.47 mg L-1, and 3P = 16.95 mg L-1) in seawater-based f/2 medium on the growth of Dunaliella salina and ß-carotene biosynthesis, by continuous process with different replenishment proportions (R = 20% and 80%). Best results of cell productivity were obtained by semicontinuous process (mean values of Px up to 6.7 x 104 cells mL-1 d-1 with medium 1N:1P; R =20%) in comparison with batch process cultivation. Maximum cell density (Xm) obtained in this work was not dependent of R, but the best results were obtained when using medium 1.5N:1.5P (mean values up to 5.6 x 105 cells mL-1 with R =80%) instead of 1N:1P. The content of ß-carotene in the cells, in general, was higher in cells grown in medium 1N:1P (mean yield values up to 57.5 mg g-1 with R =80%) in comparison with medium 1.5N:1.5P. The cultivation of D. salina with media 3N:3P led to a long lag phase, followed by decrease in cell density and cell lysis. The use of a tubular photobioreactor contributed to successfully cultivate this microalga without contamination by protozoa. The cultivation of Dunaliella salina in tubular photobioreactor with the use of 12:12 photoperiod was appropriate, as well as to induce carotenogenesis, in the second stage, by increasing the light intensity and absence of pH control

This study investigated maternal depression and perception of marital adjustment of young mothers, to understand its impact on the baby's psychofunctional symptoms. It is a transversal contrasting cases study, in which two mothers (20 and 21years), whose babies were 8 and 11 months old, from Porto Alegre/RS, were interviewed. R-DAS, EPDS and Symptom Check List were also applied to evaluate, respectively, marital adjustment, maternal depression and baby's psychofunctional symptoms. Data were integrated in the case analysis. The results suggest that perception of poor marital adjustment and presence of maternal depression may contribute to baby's psychofunctional symptoms, through a spillover process, i.e., the overflow of marital maladjustment on parental relationship. These findings are relevant to clinical practice, in searching for psychological interventions that promote the mental health of couples and babies.(AU)
