RATIONALE: Low-velocity penetrating head injury (PHI) is rare, comprising 0.2% to 0.4% of head traumas, but can be devastating and is associated with significant morbidity and mortality. No previous case of very-low-velocity PHI due to self-inflicted stabbing with a gimlet has been reported. PATIENT CONCERNS: A 62-year-old man was admitted to the hospital with bleeding head and abdominal wounds after stabbing his abdomen with a gimlet, and then hammering the same gimlet into his forehead and removing the gimlet himself. DIAGNOSES: Upon examination at admission, stab wounds were present on the forehead and the right upper quadrant. Computed tomography (CT) of the head revealed a bone defect in the left frontal bone and showed the intracranial path of the gimlet surrounded by mild hemorrhage and pneumocephalus. Magnetic resonance imaging (MRI) confirmed a small amount of hemorrhage with pneumocephalus but no vascular injury. INTERVENTIONS: Conservative treatment without surgery. OUTCOMES: Follow-up MRI on hospital day 58 showed no abscess or traumatic intracranial aneurysm. The patient achieved full recovery of motor and mental functions with conservative treatment and was discharged on hospital day 69. LESSONS: Very-low-velocity PHI might be successfully treated with conservative treatment.

Introdução:A violência autoprovocada é um importante problema de saúde pública. Esse agravo produz impactos no campo da saúde do indivíduo, da família eda coletividade com desdobramentos sociais e econômicos. Objetivo:Analisar a mortalidade por violência autoprovocada em mulheres em idade fértil no estado do Rio Grande do Norte, Brasil, entre os anos de 2012 e 2021. Metodologia:Trata-se de um estudo ecológico com abordagem quantitativa e utiliza-se como base o estado do Rio Grande do Norte. Os dados foram coletados do Departamento de Informática do Sistema Único de Saúde, por meio das Informações em Saúde,nas seções de estatísticas vitais e população residente com a seleção sexo feminino e faixa etária de 10 a 49 anos.Resultados:Entre os anos de 2012 a 2021, no estado do Rio Grande do Norte,foram registrados 213 óbitos de mulheres em idade fértil por lesões autoprovocadas. Considerando o início e o final desse período, é possível destacar que a faixa etária de maior ocorrência de suicídio foi de 30 a 39 anos em 2012 e de 40 a 49 anos em 2021. Observou-se, nos anos avaliados, que as mulheres eram em sua maioria solteiras, de raça parda/preta e que a própria residência da vítima foi o local predominante para o desfecho da lesão autoprovocada. No que se refere à escolaridade e à relação do óbito com período de gravidez ou puerpério é preciso ressaltar o alto índice de "Não informada" e "Ignorada" nos registros.A taxa média de mortalidade por lesões autoprovocadas em mulheres em idade fértil entre 2012 e 2021 foi de 2,0 óbitos por cada 100.000 habitantes. Conclusões:Assim, conclui-se que o cenário da mortalidade por violência autoprovocada em mulheres em idade fértil no Rio Grande do Norte necessita de estratégias para prevenção do suicídio nessa faixa etária (AU).


Introduction: Self-inflicted injury is a major public health problem that impacts the health, social, and economic areas of individuals, their families, and society. Aim: To analyze mortality by self-inflicted injury in fertile women from the Rio Grande do Norte state between 2012 and 2021.Methodology: This ecologic and quantitative study collected vital statistics of women aged between 10 and 49 years. Data were obtained from the Health Information Systems of the Brazilian Health Informatics Department.Results: A total of 213 deaths of fertile women by self-inflicted injury were registered between 2012 and 2021. Considering the age groups, most deaths occurred between 30 and 39 years in 2012 and between 40 and 49 years in 2021. In addition, women were mostly single andwith brown or black skin color, and most of the self-inflicted injuries happened at their houses. Regarding education level and the relationship of death with pregnancy or postpartum, most registries presented a high incidence of "Not informed" or "Ignored" answers. Last, the mean mortality by self-inflicted injury in this population was 2.0 per 100,000 inhabitants between 2012 and 2021.Conclusions: Strategies must be implemented to reduce the mortality by self-inflicted injury of fertile women from the Rio Grande do Norte state (AU).


Introducción: La violencia autoinfligida es un importante problema de salud pública. Este problema tiene impactos en la salud del individuo, la familia y la comunidad con consecuencias sociales y económicas.Objetivo: Analizar la mortalidad por violencia autoinfligida en mujeres en edad fértil en el estado de Rio Grande do Norte, Brasil, entre los años 2012 y 2021.Metodología: Se trata de un estudio ecológico con enfoque cuantitativo y utiliza como base el estado de Rio Grande do Norte. Los datos fueron recolectados del Departamento de Tecnologías de la Información del Sistema Único de Salud, a través de Información en Salud, en las secciones de estadísticas vitales y población residente con la selección del género femenino y rango de edad de 10 a 49 años. Resultados: Entre los años 2012 y 2021, en el estado de Rio Grande do Norte, se registraron 213 muertes de mujeres en edad fértil por lesiones autoinfligidas. Considerando el inicio y final de este periodo, es posible resaltar que el grupo etario con mayor incidencia de suicidio fue el de 30 a 39 años en 2012 y el de 40 a 49 años en 2021. Se observó, en los años evaluados, que las mujeres eran en su mayoría solteras, de raza mestiza/negra y la propia residencia de la víctima era el lugar predominante para la autolesión. En lo que respecta a la educación y la relación entre muerte y embarazo o puerperio, es necesario resaltar el alto índice de "No informados" e "Ignorados" en los registros. La tasa media de mortalidad por autolesiones en mujeres en edad fértil entre 2012 y 2021 fue de 2,0 muertes por 100.000 habitantes. Conclusiones: Así, se concluye que el escenario de mortalidad por violencia autoinfligida en mujeres en edad fértil en Rio Grande do Norte requiere estrategias para prevenir el suicidio en este rango de edad (AU).

BACKGROUND: Depressive disorder is a chronic mental illness characterized by persistent low mood as its primary clinical symptom. Currently, psychotherapy and drug therapy stand as the primary treatment modalities in clinical practice, offering a certain degree of relief from negative emotions for patients. Nevertheless, sole reliance on drug therapy exhibits a delayed impact on neurotransmitters, and long-term usage often results in adverse side effects such as nausea, drowsiness, and constipation, significantly impeding medication adherence. This study aims to investigate the impact of combining transcranial magnetic stimulation with sertraline on the cognitive level, inflammatory response, and neurological function in patients with depressive disorder who engage in non-suicidal self-injury (NSSI) behavior. METHODS: A total of 130 depressive patients NSSI behavior, who were admitted to our hospital from December 2020 to February 2023, were selected as the subjects for this research. The single-group (65 cases) received treatment with oral sertraline hydrochloride tablets, while the combination group (65 cases) underwent repetitive transcranial magnetic stimulation (rTMS) in conjunction with sertraline. The Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) was utilized to assess the depression status and cognitive function levels of both groups. Additionally, the enzyme-linked immunosorbent assay (ELISA) was employed to measure serum levels of inflammatory factors, including tumor necrosis factor-α (TNF-α), interleukin-1ß (IL-1ß), and interleukin-6 (IL-6). Furthermore, serum levels of neurotransmitters (norepinephrine (NE), dopamine (DA), 5-hydroxytryptamine (5-HT)) and neuro-cytokines (brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF), glial fibrillary acidic protein (GFAP)) were assessed. The clinical effects of the interventions on both groups were then evaluated. RESULTS: Following the treatment, the combination group exhibited significantly higher levels of immediate memory, delayed memory, attention, visual function, and language function compared to the single group, with statistically significant differences (p < 0.05). Additionally, the serum levels of TNF-α, IL-1ß, IL-6, and GFAP in the combination group were lower than those in the single group, while the levels of BDNF and NGF were higher in the combination group compared to the single group. These differences were also statistically significant (p < 0.05). Simultaneously, the total clinical effective rate in the combination group reached 95.38%, surpassing the 84.61% observed in the single group, and the disparity between the two groups was statistically significant (p < 0.05). CONCLUSIONS: The combined use of rTMS and sertraline in treating patients with depressive disorder exhibiting NSSI behavior has proven to be effective in enhancing cognitive function, mitigating inflammatory responses, and elevating levels of neurotransmitters and nerve cytokines in the patients.

OBJECTIVES: To explore the relationship between cumulative ecological risk and individual risky behavior and multiple forms of aggregated behaviors among adolescents, and examine the gender differences. METHODS: A large-scale, nationally representative, and students-based investigation was conducted in rural and urban areas of eight provinces in China from October to December 2021. A total of 22 868 adolescents with an average age of 14.64 years completely standardized questionnaire in which the sociodemographic characteristics, socio-ecological risk factors and risky behaviors were used to analyze. RESULTS: Of included students, 48.4% encountered the high level of social-ecological risk. The prevalence of breakfast intake not daily, alcohol use (AU), smoking, physical inactivity, prolonged screen time (ST) on weekdays and weekends, suicidal ideation, suicidal plan, suicidal attempt, and non-suicidal self-injury (NSSI) was 41.0%, 11.9%, 3.4%, 61.9%, 15.1%, 51.1%, 27.7%, 13.9%, 6.5% and 27.0% respectively. 22.2% of participants engaged in high-risk behaviors. All were significantly influences of increased cumulative ecological risk on individual behavior and low-risk clustering behaviors separately. The odds ratio of breakfast intake not daily, AU, smoking, physical inactivity, prolonged ST in weekday and weekend, suicidal ideation, suicidal plan, suicidal attempt, and NSSI for the adjusted model in low versus high level of cumulative ecological risk was respectively significant in both boy and girls, and the ratio of odds ratios (ROR) was separately 0.95 (p = 0.228), 0.67 (p < 0.001), 0.44 (p < 0.001), 0.60 (p < 0.001), 0.78 (p = 0.001), 0.83 (p = 0.001), 0.80 (p = 0.001), 0.83 (p = 0.022), 0.71 (p = 0.005), 0.75 (p = 0.001). Girls encountering a high level of cumulative ecological risk were more likely to engage in multiple forms of clustering risky behaviors than boys (RORs: 0.77, p = 0.001). CONCLUSIONS: Research and effective inventions at the social-ecological environment, based on the view of cumulative risk, are needed to promote the healthy development of behaviors in adolescence, and pay more attention to decreasing the occurrence of risky behaviours in girls than boys.

BACKGROUND: Bipolar disorders (BD) in youth are associated with a high risk of self-harm behaviors. Childhood trauma (CT) is a relevant environmental stressor that is related to both BD diagnosis and self-harm in adulthood. It is not yet established whether CT may impact self-harm risk in youth. Therefore, the aim of this study was to investigate the distribution patterns of CT in youth BD with and without self-harm. METHODS: We assessed 273 participants (aged 13-25 years), 96 youths with BD according to DSM-5 criteria and 177 healthy controls (HC). History of CT was obtained using the Childhood Trauma Questionnaire (CTQ). The association between CT and self-harm was tested using multivariate statistical models. RESULTS: Over 45% of participants with BD reported lifetime self-harm. The BD Self-harm group reported more emotional abuse, emotional neglect, sexual abuse, and physical abuse than HC. The BD No-Self-harm group reported more emotional abuse than HC. The BD Self-harm group reported more emotional abuse and neglect than the BD No-Self-harm group. The BD Self-harm group also reported separated parents, hospitalizations, smoking, use of antiepileptics, antipsychotics and lithium. Emotional abuse was an independent predictor of self-harm in youths with BD. CONCLUSION: Findings support the importance of assessing CT, in particular emotional abuse, in youth with BD at risk for self-harm.

INTRODUCTION: Alterations in inflammatory processes have previously been reported in impulsive and unstable disorders, as well as in other psychiatric conditions. In order to investigate transdiagnostic biomarkers associated with various phenotypic features of these disorders, this study is designed to identify biomarkers of inflammatory and oxidative endophenotypes related to autolytic behavior. METHODS: Peripheral blood mononuclear cells were collected from 35 patients with borderline personality disorder (BPD), 29 patients with restrictive eating disorder (rED), 21 patients with purging eating disorder (pED) and 23 control subjects. Plasma levels of different inflammatory and oxidative factors were measured by ELISA and the expression of selected proteins was by Western Blot. Principal component analysis (PCA) was performed to categorize the different inflammatory factors. Additionally, Ancova was performed to observe the differences in the principal components among the different groups and logistic regression analysis was conducted to assess the predictive capacity of these components for autolytic behaviors. RESULTS: We found two inflammatory/oxidative components were associated with BPD, characterized by high levels of JNK and ERK and low levels of GPx, SOD and Keap1; and two other inflammatory/oxidative components were linked to pED, associated with more JNK, TBARS and TNF-α and less GPx and SOD. Two components, with more JNK and ERK and less GPx, SOD and Keap1, predicted non-suicidal self-injury and three components, with higher JNK, TBARS and TNF-α levels and lower GPx, SOD and iNOS levels, predicted suicide attempts. CONCLUSIONS: These results strongly support the endophenotypic characterization of impulsivity and the identification of transdiagnostic inflammatory/oxidative biomarkers relevant to autolytic behavior in impulsive and unstable disorders. These dates lay the groundwork for developing of screening tests for these biomarker components to rapidly detect biological risk factors for specific impulse control disorders and future self-injurious behaviors.

Este estudo teve como objetivo compreender a autolesão na adolescência e investigar sua relação com o funcionamento familiar em um Centro de Atenção Psicossocial Infantojuvenil por meio de um estudo de caso. A autolesão é caracterizada como um comportamento intencional de ferir o próprio corpo, sem a intenção de morrer, podendo evidenciar o desamparo e o convite ao olhar do outro, diante de sua dor. A pesquisa adotou uma abordagem clínico-qualitativa, com análise e discussão dos conteúdos à luz da Psicanálise de Casal e Família. Os resultados revelaram que a adolescente inscreve no corpo um sofrimento não verbalizado, relacionado à crise cultural e à fragilidade dos suportes simbólicos necessários para o desenvolvimento psíquico. Na família, evidenciou-se falhas das funções continentes do Eu-pele familiar, como a dificuldade em perceber os cortes e compreender as queixas da filha. Conclui-se que a autolesão pode servir como uma defesa contra o sofrimento psíquico, comunicando falhas ambientais. Destaca-se a importância da família no desenvolvimento psíquico durante a adolescência, fornecendo apoio emocional diante das transformações e lutos exigidos ao longo da vida.


The aim of this study was to understand self-injury in adolescence and investigate its relationship with family functioning in a Child and Adolescent Psychosocial Care Center through a case study. Self-injury is characterized as an intentional behavior of hurting one's own body, without the intention of dying, and can show helplessness and an invitation to the gaze of others in the face of their pain. The research adopted a clinical-qualitative approach, analyzing and discussing the content in the light of Couple and Family Psychoanalysis. The results revealed that the teenager inscribes non-verbalized suffering on her body, related to the cultural crisis and the fragility of the symbolic support necessary for psychic development. In the family, there were failures in the continental functions of the family Self-skin, such as the difficulty in perceiving the cuts and understanding the daughter's complaints. We conclude that self-injury can serve as a defense against psychic suffering, communicating environmental failures. The importance of the family in psychic development during adolescence is highlighted, providing emotional support in the face of the transformations and mourning required throughout life.


El objetivo de este estudio fue comprender la autolesión en la adolescencia e investigar su relación con el funcionamiento familiar en un Centro de Atención Psicosocial a Niños y Adolescentes a través de un estudio de caso. La autolesión se caracteriza por ser una conducta intencional de dañar el propio cuerpo, sin intención de morir, y puede ser una evidencia de indefensión y una invitación a la mirada de los demás ante su dolor. La investigación adoptó un abordaje clínico-cualitativo, analizando y discutiendo el contenido a la luz del Psicoanálisis de Pareja y Familia. Los resultados revelaron que la adolescente inscribía en su cuerpo un sufrimiento no verbalizado, relacionado con la crisis cultural y la fragilidad del soporte simbólico necesario para el desarrollo psíquico. En la familia, hubo fallas en las funciones continentales de la Auto-piel familiar, como la dificultad para darse cuenta de los cortes y comprender las quejas de la hija. Concluimos que la autolesión puede servir como defensa contra el sufrimiento psíquico, comunicando fallas ambientales. Se destaca la importancia de la familia en el desarrollo psíquico durante la adolescencia, proporcionando apoyo emocional frente a las transformaciones y duelos necesarios a lo largo de la vida.

OBJECTIVE: To investigate the associations of OPRM1 gene rs179971, OPRK1 gene rs6473797 and DCC gene rs8084280 polymorphisms with non-suicidal self-injury (NSSI) characteristics and motivations in adults. MATERIAL AND METHODS: A pilot sample included 28 adult patients with history of NSSI (89.3% (n=25) women, median age (Q1-Q3) - 23 (21.25-25) years). Most patients (78.6%, n=20) had a diagnosis of bipolar disorder. NSSI characteristics and motivations were assessed using the Inventory of Statements about Self-Injury (ISAS) scale. The Childhood Trauma Questionnaire (CTQ) was used to control for childhood trauma - one of the most important environmental factors associated with NSSI. The Baratt Impulsivity Scale (BIS) and the Buss-Perry Aggression Questionnaire (BPAQ) were also used to assess impulsivity and aggression, respectively. RT-PCR was used for genotyping, a genetic effect was assessed using the dominant model. Mann-Whitney U-test, Pearson χ2-test and multiple linear regression were used for statistical analysis. RESULTS: Carriers of the minor G allele of OPRM1 gene rs1779971 had a higher level of aggression assessed by BPAQ (p=0.02). The minor C allele of OPRK1 gene rs6473797 was associated with an increase of the subjective importance of «Affect regulation¼ (B=2.23; CI 95% [0.39-4.06]; p=0.022) and «Anti-dissociation¼ (B=3.31; CI 95% [0.18-6.44]; p=0.039) motivations, whereas the minor T allele of DCC gene rs8084280, on the contrary, was associated with a decrease of the importance of «Affect regulation¼ (B=-1.74; CI 95% [-3.30 - -0.18]; p=0.032). Moreover, this effect was found after adjusting for diagnosis, sex, age, and the presence of childhood trauma. CONCLUSIONS: To our knowledge, this is the first study on the association of genetic markers with NSSI motivations. The results of this pilot study demonstrate that OPRK1 and DCC gene polymorphisms can determine differences in motivations for self-harm, however, these results require confirmation in large samples.

OBJECTIVE: Data from South Asia indicate that for 15%-20% of suicide attempts, pesticides are purchased from shops; otherwise, pesticides are obtained from an individual's house or nearby environment. We aimed to investigate the difference between individuals who directly purchase pesticides from shops for suicide attempts and suicide deaths versus those related to accessing the pesticides from an individual's house or nearby environment. METHODS: We conducted two comparative studies in rural Sri Lanka: (1) non-fatal shop cases (n = 50) were survivors of self-poisoning with pesticides who ingested the pesticides after purchasing them from a shop; non-fatal domestic cases (n = 192) were survivors who accessed pesticides from their house or nearby environment. (2) fatal shop cases (n = 50) were individuals who died after ingesting pesticides they purchased for the act; fatal domestic cases (n = 102) were patients who died after ingesting pesticides they accessed at house or nearby environment. Logistic regression analysis was used to assess the characteristics which distinguished between the shop and domestic cases. RESULTS: Data indicate that 20.7% and 32.9% of individuals who used pesticides for suicide attempts and suicide deaths had purchased them from shops, respectively. Being a non-farmer was the main distinguishing characteristic of shop cases: adjusted odds ratios (AOR) 8.9, 95% confidence intervals (CI) 3.2-24.4 for non-fatal shop cases, and AOR 4.0, 95% CI 1.5-10.6 for fatal shop cases. Non-fatal shop cases also had higher suicide intent (AOR 3.0, CI 1.0-8.9), and ingesting an insecticide (AOR 4.8, CI 1.8-1.0-8.9) than non-fatal domestic cases. CONCLUSION: A high suicide intent of individuals who purchase pesticides for the event explains the high proportion of such fatal cases. Such high suicide intent makes the prevention implications difficult to spell out for those individuals who purchase pesticides for self-poisoning. However, our findings are valuable for clinicians to assess pesticide poisoning cases in hospitals.

INTRODUCTION: Suicide claims many lives globally, each year. For every person that dies by suicide, multitudes more attempt it. A national shortage of psychiatrists may prevent many individuals from receiving timely mental health care. For many individuals, the primary entry point into the healthcare system is through the emergency department. The trauma service frequently treats patients with severe self-inflicted injuries and for many this is not the first time. This represents an opportunity for intervention to disrupt the cycle and prevent future death. METHODS: We conducted a retrospective chart review of all patients with self-inflicted injuries, admitted to the trauma surgery service between 2012 and 2021. All patients above 10 years old were included. RESULTS: Four hundred forty-one patients were admitted due to self-injurious behavior in the period under study. The majority of patients (71.9%) had a pre-existing mental health disorder. Fifty six patients suffered fatal injuries; the majority were White (78.6%), males (80.3%), and were inflicted by gunshot (71.4%). Nearly one third of patients with self-inflicted injuries had a history of self-injurious behavior with the average number of attempts being 2.7 (SD: ±3.8). CONCLUSIONS: We need interdisciplinary and innovative solutions for this public health crisis. Perhaps telemedicine can be used to buttress the access to adequate mental health care. More research needs to be done to better identify the barriers individuals encounter in accessing mental health care, both pre- and post-crisis. The goal is that, by identifying the gaps, we can collaboratively bridge them to prevent a preventable death.

BACKGROUND: The increase in tobacco/conventional cigarette (CC) and electronic cigarette (EC) usage among Chinese youth has become a growing public health concern. This is the first large-scale study to compare the impact of CC and EC usage on risk for nonsuicidal self-injury (NSSI) and suicidality in cis-heterosexual and sexual and gender minority (SGM) youth populations in China. OBJECTIVE: This study examines the CC and EC risks for NSSI and suicidality among Chinese youth and compares the extent to which SGM and cis-heterosexual youth's risks for NSSI and suicidality are influenced by their CC and EC usage and dependence. METHODS: A total of 89,342 Chinese participants completed a cross-sectional self-report survey in 2021. Sociodemographic information, sexual orientations, gender identities, CC and EC usage, CC and EC dependence, and risks for suicidality and NSSI were assessed. The Mann-Whitney U test and chi-square test were performed for nonnormally distributed continuous variables and categorical variables, respectively. The multivariable linear regression model was used to examine both the influence of CC and EC usage and CC and EC dependence on NSSI and suicidality as well as the interaction effects of CC and EC usage and CC and EC dependence on NSSI and suicidality by group. RESULTS: The prevalence of CC usage (P<.001) and dependence (P<.001) among SGM participants was lower than that among their cis-heterosexual counterparts. However, the prevalence of EC usage (P=.03) and EC dependence (P<.001) among SGM participants was higher than that among their cis-heterosexual counterparts. The multivariable linear regression model showed that CC dependence and EC dependence had a unique effect on NSSI and suicidality (CCs: B=0.02, P<.001; B=0.09, P<.001; ECs: B=0.05, P<.001; B=0.14, P<.001, respectively). The interaction effects of (1) CC usage and group type on NSSI and suicidality (B=0.34, P<.001; B=0.24, P=.03, respectively) and dual usage and group type on NSSI and suicidality (B=0.54, P<.001; B=0.84, P<.001, respectively) were significant, (2) CC dependence and group type on NSSI were significant (B=0.07, P<.001), and (3) EC dependence and group type on NSSI and suicidality were significant (B=0.04, P<.001; B=0.09, P<.001, respectively). No significant interaction effect was observed between EC usage and group type on NSSI and suicidality (B=0.15, P=.12; B=0.33, P=.32, respectively) and between CC dependence and group type on suicidality (B=-0.01, P=.72). CONCLUSIONS: Our study shows evidence of intergroup differences in NSSI and suicidality risks between SGM and cis-heterosexual youth related to CC and EC usage. These findings contribute to the growing literature on CC and EC in cis-heterosexual and SGM populations. Concerted efforts are necessary at a societal level to curb the aggressive marketing strategies of the EC industry and media coverage and to maximize the impact of educational campaigns on EC prevention and intervention among the youth population.

ABSTRACT: The field of transgender health has grown exponentially since the early 2010s. While this increased visibility has not been without controversy, there is growing acknowledgement of the needs of transgender, nonbinary, and gender expansive (TNG) patients and the health disparities they experience compared to the cisgender population. There is also increased interest among clinicians and trainees in providing gender-affirming care in all medical specialties. This is particularly relevant in psychiatry as mental health disparities in TNG patients have been well-documented. TNG patients experience significant minority stress and higher rates of psychiatric illness, self-harm, suicidality, and psychiatric hospitalization compared to their cisgender peers. In this review, we will cover potential interactions and side effects relevant to psychiatric medication management for the three most common medication classes prescribed as part of gender-affirming hormone therapy (GAHT): gonadotropin-releasing hormone receptor agonists, estradiol, and testosterone. Although no studies directly examining the efficacy of psychiatric medications or their interactions with GAHT for TNG patients have been published yet, we have synthesized the existing literature from both cisgender and TNG patients to shed light on health care disparities seen in TNG patients. Since clinicians' lack of comfort and familiarity with gender-affirming care contributes significantly to these disparities, we hope this narrative review will help psychiatric prescribers provide TNG patients with the same quality of care that cisgender patients receive.

Comportamentos autolesivos na adolescência se tornaram um problema de saúde pública mundial. Frequentemente, esses comportamentos compõem o quadro sintomático de doenças psiquiátricas, sendo conceitualizados como estratégias disfuncionais de regulação emocional. Diante disso, este estudo investiga a relação entre comportamentos autolesivos, percepção de suporte familiar e sintomas ansiosos e depressivos em adolescentes da rede estadual de ensino do Recife, assim como estima a prevalência do fenômeno (n=501). Também buscou-se estimar a prevalência do fenômeno e sua relação com variáveis sociodemográficas. Os instrumentos utilizados foram o Protocolo Health Behaviour in School-Aged Children, a Spence Children's Anxiety Scale e o Inventário de Depressão Infantil, todos adaptados ao português brasileiro. Foram realizadas análises da consistência interna, qui quadrado, teste t, correlação e descritivas. Os resultados apontaram prevalência de 31,3% para comportamentos autolesivos. Adolescentes do sexo feminino apresentaram 2,26 vezes mais chance de engajar na prática de comportamentos autolesivos. Verificou-se diferenças significativas entre participantes que se autolesionavam e aqueles que não se autolesionavam no tangente à percepção de sintomas depressivos, sintomas ansiosos e suporte familiar. Nossos achados enfatizam a importância dos vínculos familiares no combate aos comportamentos autolesivos. Assim, esperamos contribuir para a construção de estratégias preventivas contra comportamentos de risco, ansiedade e depressão.


Self-injurious behaviors in adolescence have become a public health problem worldwide. Often these behaviors are symptoms of psychiatric diseases, conceptualized as dysfunctional strategies of emotional regulation. Therefore, this study investigates the relation between self-injurious behaviors, family support perception and anxiety and depressive symptoms within adolescents from the state education network in Recife. It also estimates the prevalence of the phenomenon (n=501). Another goal was to estimate the prevalence of the phenomenon and its relation with sociodemographic variables. The instruments used were the Health Behavior in School-Aged Children Protocol, the Spence Children's Anxiety Scale and the Children's Depression Inventory, all adapted to Brazilian Portuguese. Internal consistency, chi-square, t-test, correlation and descriptive analyzes were performed. The results showed a prevalence of 31.3% for self-injurious behaviors, with female adolescents being 2.26 times more likely to engage in self-injurious behaviors. There were significant differences between participants who engaged in self-injurious behaviors and those who did not due to their perceptions of depressive symptoms, anxious symptoms and family support. Our findings emphasize the importance of family bonds in combating self-injurious behaviors. Thus, we hope to contribute to the creation of preventive strategies against risk behaviors, anxiety and depression.

Estos comportamientos componen el cuadro sintomático de enfermedades psiquiátricas, siendo conceptualizados como estrategias disfuncionales de regulación emocional. Este estudio investiga la relación entre las conductas autolesivas, la percepción del apoyo familiar y los síntomas de ansiedad y depresión entre adolescentes de la red estatal de educación en Recife, además de estimar la prevalencia del fenómeno (n=501). También se buscó estimar la prevalencia del fenómeno y su relación con variables sociodemográficas. Los instrumentos utilizados fueron el protocolo Health Behaviour in School-Aged Children, la Spence Children's Anxiety Scale y el Invetario de Depresión Infantil, todos adaptados al portugués brasileño. Se realizaron análisis de consistencia interna, chi cuadrado, prueba t, correlación y descriptivos. Los resultados apuntaron para la prevalencia de 31,3% para comportamientos autolesivos. Adolescentes del sexo femenino presentaron 2,26 veces más chances de involucrarse en la práctica de comportamientos autolesivos. Se verificaron diferencias significativas entre participantes que se autolesionaban y aquellos que no se autolesionaban frente a la percepción de síntomas depresivos, síntomas ansiosos y apoyo familiar. Esperamos contribuir con la construcción de estrategias preventivas contra conductas de riesgo, ansiedad y depresión.

Introduction: Psychoactive substances abuse is considered a problematic social factor due its likelihood to cause harmful, self-destructive behaviors to the subjects and the overall society. Stress in an individual's early life may also be a contributing factor to substance abuse as well as suicide attempts. There is a lack of studies examining these factors in people with substance-use disorder. Aim: to identify the relationship between early-life stress and suicide attempts in drug-dependent adults. Methods: This is a predictive correlational study with a cross-sectional approach. The convenience sample consisted of 105 individuals treated at an outpatient unit for addiction treatment. The participants were assessed using the Mini-international Neuropsychiatric Interview, a Childhood Trauma Questionnaire to measure the severity of the different types of early life stress; the Beck Scale for Suicide Ideation was also used. The data were analyzed using descriptive and inferential statistics through univariate and multivariate logistic regression. Results: The analyzed sample included 33 (31.4%) individuals who attempted suicide and were significantly more likely to suffer from emotional, physical, or sexual abuse than those who had never attempted suicide (p <0,05). Conclusions: Different forms of early-life stress are related to attempted suicide in people with substance-use disorder. Further studies are needed to understand the effects of early-life stress on suicide attempts in drug-dependent people.


Introducción: El abuso de sustancias psicoactivas se considera un factor social problemático cuando se producen conductas autodestructivas perjudiciales para el sujeto y la sociedad. El estrés en la vida temprana de una persona también puede ser un factor que contribuya a las situaciones de abuso de sustancias, así como a los intentos de suicidio. Hay una falta de estudios que examinen estos factores en personas con trastorno por uso de sustancias. Objetivo: Identificar la relación entre estrés en la vida temprana y los intentos de suicidio en personas adultas con trastorno por uso de sustancias. Métodos: Estudio correlacional, predictivo, transversal. La muestra de conveniencia consistió en 105 personas tratadas en una unidad ambulatoria para el tratamiento de adicciones. Las personas se evaluaron por medio de la Mini-Entrevista Neuropsiquiátrica Internacional, el Cuestionario de Trauma Infantil para medir la gravedad de los diferentes tipos de estrés en la vida temprana y la Escala de Beck para Ideación Suicida. Los datos se analizaron mediante estadística descriptiva e inferencial mediante regresión logística univariante y multivariante. Resultados: De la muestra analizada, 33 (31,4%) habían intentado suicidarse y tenían una exposición significativamente mayor al abuso emocional, físico y sexual (p<0,05) en comparación con las personas que nunca habían intentado suicidarse. Conclusiones: Diferentes formas de estrés en la vida temprana están relacionadas con el intento de suicidio en personas con trastorno por consumo de sustancias. Se necesitan más estudios para comprender los efectos del estrés en la vida temprana sobre los intentos de suicidio en una persona con dependencia.

O estresse no início da vida de um indivíduo também pode ser um fator que contribui para situações de abuso de substâncias, bem como tentativas de suicídio. Há uma falta de estudos examinando esses fatores em pessoas com transtorno por uso de substâncias. Objetivo: Identificar a relação entre o estresse no início da vida e as tentativas de suicídio em adultos com transtorno de uso de substâncias. Métodos: Estudo correlacional preditivo com abordagem transversal. A amostra por conveniência foi composta por 105 indivíduos atendidos em uma unidade ambulatorial para tratamento de dependências e foi avaliada por meio do Mini-International Neuropsychiatric Interview, do Childhood Trauma Questionnaire para medir a gravidade de diferentes tipos de estresse precoce e da Escala de Beck para Ideação Suicida. OBJECTIVES: Ecological momentary assessment (EMA) has seen an explosion in popularity in recent years; however, an improved understanding of how to minimise (selective) non-adherence is needed. METHODS: We examined a range of respondent characteristics predictors of adherence (defined as the number of EMA surveys completed) in the D2M EMA study. Participants were a sample of n = 255 individuals drawn from the longitudinal z-proso cohort who completed up to 4 EMA surveys per day for a period of 2 weeks. RESULTS: In unadjusted analyses, lower moral shame, lower self-control, lower levels of self-injury, and higher levels of aggression, tobacco use, psychopathy, and delinquency were associated with lower adherence. In fully adjusted analyses with predictors selected using lasso, only alcohol use was related to adherence: beer and alcopops to higher adherence and spirits to lower adherence. CONCLUSIONS: These findings provide potential insights into some of the psychological mechanisms that may underlie adherence in EMA. They also point to respondent characteristics for which additional or tailored efforts may be needed to promote adherence.

BACKGROUND: Self-inflicted gunshot wounds (SIGSWs) remain a leading, preventable cause of death in the United States. The present study evaluated patient demographics, operative characteristics, in-hospital outcomes, and resource utilization between patients with SIGSW and other GSW. METHODS: The 2016-2020 National Inpatient Sample was queried for patients ≥16 years old admitted following gunshot wounds. Patients were categorized as SIGSW if they were injured through self-harm. Multivariable logistic regression was used to evaluate the association of SIGSW on outcomes. The primary endpoint was in-hospital mortality with complications, costs, and length of stay secondarily considered. RESULTS: Of an estimated 157,795 surviving to hospital admission, 14,670 (9.30%) were SIGSW. Self-inflicted gunshot wounds were more commonly female (18.1 vs 11.3%), insured by Medicare (21.1 vs 5.0%), and white (70.8 vs 22.3%) (all P < .001) compared to non-SIGSW. Psychiatric illness was more prevalent in SIGSW (46.0 vs 6.6%, P < .001). Additionally, SIGSW more frequently underwent neurologic (10.7 vs 2.9%) and facial operations (12.5 vs 3.2%) (both P < .001). After adjustment, SIGSW was associated with greater odds of mortality (AOR: 12.4, 95% CI: 10.4-14.7). Length of stay (ß: +1.5 days, 95% CI: .8-2.1) and costs (ß: +$3.6 K, 95% CI: 1.4-5.7) were significantly greater in SIGSW. CONCLUSIONS: Self-inflicted gunshot wounds are associated with increased mortality compared to other GSW, likely due to the increased proportion of injuries in the head and neck region. This lethality, coupled with the high prevalence of psychiatric illness in this population, indicates that efforts must be made to intervene through primary prevention, including enhanced screening and weapon safety considerations for those at risk.

Self-injurious behaviors are repetitive, persistent actions directed toward one's body that threaten or cause physical harm. These behaviors are seen within a broad spectrum of neurodevelopmental and neuropsychiatric conditions, often associated with intellectual disability. Injuries can be severe and distressing to patients and caregivers. Furthermore, injuries can be life-threatening. Often, these behaviors are challenging to treat and require a tiered, multimodal approach which may include mechanical/physical restraints, behavioral therapy, pharmacotherapy, or in some cases, surgical management, such as tooth extraction or deep brain stimulation. Here, we describe a series of 17 children who presented to our institution with self-injurious behaviors in whom botulinum neurotoxin injections were found helpful in preventing or lessening self-injury.

BACKGROUND: Psychological distress following a cancer diagnosis potentially increases the risk of intentional, nonfatal self-injury. The purpose of this work is to evaluate and compare rates of nonfatal self-injury among individuals in Ontario diagnosed with cancer against matched controls with no history of cancer. METHODS: Adults in Ontario diagnosed with cancer from 2007 to 2019 were matched to 2 controls with no history of cancer, based on age and sex. We calculated the absolute and relative difference in rates of nonfatal self-injury in the 5 years before and after the index date (date of cancer diagnosis and dummy date for controls). We used crude difference-in-differences methods and adjusted Poisson regression-based analyses to examine whether the change in rates of nonfatal self-injury before and after index differed between cancer patients and controls. RESULTS: The cohort included 803 740 people with cancer and 1 607 480 matched controls. In the first year after diagnosis, individuals with cancer had a 1.17-fold increase in rates of nonfatal self-injury (95% confidence interval [CI] 1.03-1.33) compared with matched controls, after accounting for pre-existing differences in rates of nonfatal self-injury and other clinical characteristics between the groups. Rates of nonfatal self-injury remained elevated in the cancer group by 1.07-fold for up to 5 years after diagnosis (95% CI 0.95-1.21). INTERPRETATION: In this study, incidence of nonfatal self-injury was higher among individuals diagnosed with cancer, with the greatest impact observed in the first year after diagnosis. This work highlights the need for robust and accessible psychosocial oncology programs to support mental health along the cancer journey.

Increased risk of violence and self-harm means prisoners are a vulnerable population with complex health needs. They account for a small proportion of patients with burn injuries; however, present a unique set of challenges. This study investigates the incidence, pattern and outcomes of burn injuries in prison population. Prisoners referred from 2010 to 2021 were identified using the International Burn Injury Database (iBID). Patient demographics, burn injury characteristics and outcomes were collected. Patients were then stratified based on mechanism of injury, treatment modality (surgery/conservative), hospital admission (inpatient/outpatient), and compliance with outpatient follow-up, for subgroup analyses. Sixty-eight prisoners sustained burns during the study period, with a median age of 28.5 years and TBSA of 3%. The majority were male (98.5%) and required hospital admission (75%). Scalds were the most common injury type (77.9%) and assault the most frequent cause of burns (63.2%). Eighteen patients (26.5%) underwent a surgical procedure and there were two mortalities. Of patients for whom follow-up was planned, 22% attended no appointments, with a further 49% of prisoners missing at least one appointment. Relative to patients managed nonoperatively, prisoners undergoing surgery had a longer stay and all attended outpatient follow-up appointments. Prisoners represent a unique population with exceptional challenges. Attention should be given to protecting vulnerable patients at risk of assault, education of prison staff around burn prevention and first aid, and ensuring that prisoners are able to access burns follow-up to minimize long-term sequelae. Opportunities exist to aid this such as the adoption of telemedicine.
