RESUMO

Skin cancer treatment is a core aspect of dermatology that relies on accurate diagnosis and timely interventions. Teledermatology has emerged as a valuable asset across various stages of skin cancer care including triage, diagnosis, management, and surgical consultation. With the integration of traditional dermoscopy and store-and-forward technology, teledermatology facilitates the swift sharing of high-resolution images of suspicious skin lesions with consulting dermatologists all-over. Both live video conference and store-and-forward formats have played a pivotal role in bridging the care access gap between geographically isolated patients and dermatology providers. Notably, teledermatology demonstrates diagnostic accuracy rates that are often comparable to those achieved through traditional face-to-face consultations, underscoring its robust clinical utility. Technological advancements like artificial intelligence and reflectance confocal microscopy continue to enhance image quality and hold potential for increasing the diagnostic accuracy of virtual dermatologic care. While teledermatology serves as a valuable clinical tool for all patient populations including pediatric patients, it is not intended to fully replace in-person procedures like Mohs surgery and other necessary interventions. Nevertheless, its role in facilitating the evaluation of skin malignancies is gaining recognition within the dermatologic community and fostering high approval rates from patients due to its practicality and ability to provide timely access to specialized care.

Teledermatology (TD) is a remote medical care tool that experienced a great boom during the COVID19 pandemic, resulting in an increase in both, the number of remote consultations and the number of dermatologists who practice it. Several studies have reported a high degree of satisfaction among patients attended by this method. However, few studies have done so in dermatologists. We set out to investigate the experience of Chilean dermatologists with the tool by means of an online survey. Results. A total of 156 surveys were completed. The number of dermatologists using TD increased from 19.9% to 80.8% during the pandemic. They cited the absence of the physical examination-dermatoscopy as the main limitation to developing TD and reported feeling more comfortable with some pathologies. Dermatologists feel moderately comfortable and confident with the tool, however, they experienced varying degrees of stress and frustration. Only 40.8% plan to continue using TD in the future. Conclusion. Synchronous TD has clear limitations, however, these should emerge as an opportunity to consider in its development and its platforms, in order to generate a more comfortable tool for more dermatologists and thus extend its use, as an important tool for equal access to health care in Chile. RESUMO

The Janus kinase-signal transducer and activator of transcription (JAK-STAT) intracellular signaling pathway is implicated in the pathogenesis of a number of inflammatory dermatoses. Clinical trials and other studies have demonstrated the efficacy of JAK inhibitors in the treatment of a variety of dermatologic conditions. Here we review JAK inhibitors currently under investigation for the treatment of alopecia areata, vitiligo, sarcoidosis, necrobiosis lipoidica, granuloma annulare, and systemic lupus erythematosus with a special emphasis on safety and the implications of JAK inhibitors during the novel coronavirus 2019 pandemic.

With recent advances in genetic engineering technology, gene therapy is now being considered as a treatment not only for congenital diseases but also acquired diseases, such as cancer. Gene therapeutic agents for hereditary immune disorders, haemophilia, retinal diseases, neurodegenerative diseases, and lymphoma have been approved in the United States and Europe. In the field of dermatology, clinical trials of gene therapy have been conducted, because the skin is an easily accessible organ that represents an attractive tissue for gene therapy. In recent years, gene therapy has been attempted for a variety of skin diseases, such as genodermatoses (including epidermolysis bullosa and Netherton syndrome), cutaneous lymphoma, and malignant melanoma. As a result, it is difficult to grasp the current status of gene therapy in dermatology. This review focuses on each of the gene-transfer techniques currently in use and describes the current status of gene therapy for skin diseases using each technology.

BACKGROUND: The annual conference of the European Academy of Dermatology and Venereology is one the largest dermatology conferences worldwide. OBJECTIVES: Network analysis can be used for in-depth insight into trending topics and underlying trends at the congress. MATERIALS & METHODS: Network analysis was employed to assess the entirety of the submitted abstracts to the congress in 2019. The data were processed, analysed, and visualised using easy-to-understand network graphs. Topics were then compared to their respective global burden (Disease Adjusted Life Years [DALYs]) and the number of respective publications on PubMed in the year 2018. RESULTS: Overall, 1,280 lecture titles and 1,941 poster titles were included in the final analysis. The most frequently used terms were "patients" (n = 473), "treatment" (n = 301), and "psoriasis" (n = 335). Relative to DALYs, "psoriasis" (+21.9%) among others, was rather over-represented, while "fungal skin diseases" (-7.6%) and "urticaria" (-6.4%) were under-represented. Compared to the relative number of PubMed publications in 2018, "psoriasis" (+20.3%), "acne" (+7.9%), and "alopecia" (+3.1%) were over-represented, while "melanoma" (-22.5%), "dermatitis" (-4.2%) and "pruritus" (-3.4%) were rather under-represented. CONCLUSION: The network analysis showed that the congress was a patient and therapy-centred event. An explanation for the particular focus on chronic inflammatory skin diseases and melanoma would be the introduction of new therapies at the congress. To delineate trends over time, a longitudinal network analysis including several congresses should be conducted and could be used to determine additional topics to be included in future events.

BACKGROUND: Port-wine stain (PWS) is a congenital vascular malformation affecting 0.3–0.5% of normal population. These characteristic lesions arise due to the interplay of vascular, neural, and genetic factors. Treatment options include lasers, cosmetic tattooing, electrotherapy, cryosurgery, derma-abrasion, and skin grafting; however, none of these treatment alternatives appears to be satisfactory and is unable to provide consistent, satisfactory responses or even complete cures. Currently, laser is the treatment of choice, as it is comparatively safe and more effective than other procedures. The most commonly used modality is pulsed dye laser (PDL). The literature research includes peer-reviewed articles (clinical trials or scientific reviews). Studies were identified by searching electronic databases (MEDLINE and PubMed) to January 2020 and reference lists of respective articles. Only articles published in English language were included. BACKGROUND: Psychodermatology is an emerging subspeciality of dermatology. Psychodermatology clinics use a multidisciplinary approach to deal with psychological or psychiatric elements related to skin disease. Two previous studies in 2004 and 2012 highlighted the deficiency of psychodermatology services in the UK, despite the evidence that these services have high demand and are cost-effective. AIMS: To reassess psychodermatology service provision in the UK and outline the developments that have been made. METHODS: In conjunction with BBC Radio 5 Live, a survey questionnaire was distributed via email to the UK membership of the British Association of Dermatologists (BAD) and Psychodermatology UK. The survey consisted of 13 questions asking about the availability of psychodermatology services. RESULTS: Basic percentages were used to analyse quantitative data, and content analysis was used for qualitative data. Our results showed that less than a quarter of the respondents (24%) have access to a nearby dedicated psychodermatology service. Additionally, the psychodermatology units do not have a unified configuration and clinical provision model differs nationally. Only around 5% of the clinicians have access to a clinic that provides psychology-dermatology-oncology service, and even fewer have access to a paediatric psychodermatology (4.8%). Engagement in psychodermatology research was reported by around 12% of the participants. CONCLUSIONS: The psychocutaneous services in the UK have improved to some extent over the past decade; the service has become more widely available nationally, and the investment in research is promising. However, it is still insufficient and unable to fulfil patient demand, especially for vulnerable individuals such as children and dermato-oncology patients.

RESUMO

This Perspective briefly reviews the relationship between UV-induced mutations in habitually sun-exposed human skin and subsequent development of actinic keratoses (AKs) and skin cancers. It argues that field therapy rather than AK-selective therapy is the more logical approach to cancer prevention and hypothesizes that treatment early in the process of field cancerization, even prior to the appearance of AKs, may be more effective in preventing cancer as well as more beneficial for and better tolerated by at-risk individuals. Finally, the Perspective encourages use of rapidly advancing DNA analysis techniques to quantify mutational burden in sun-damaged skin and its reduction by various therapies.

RESUMO

Telemedicine is one of the most consequential technologies in modern healthcare. In certain situations, it allows for the delivery of care with high quality and minimal difficulty. This is particularly true in dermatology, in which many dermatological conditions can be treated remotely. The burden on dermatology patients has been greatly reduced for certain pathologies due to telemedicine. Health care providers also achieve improved job satisfaction following the convenience of meeting their patients. This paper details select dermatological conditions, and subsequently divides them into those treatable by telemedicine appointments, and those requiring face to face appointments.

RESUMO

As dermatologists, we are unique in our ability to diagnose and treat diseases that present in the skin. This includes special expertise in the core disciplines of medical dermatology, pediatric dermatology, dermatologic surgery, and dermatopathology. Just as individual dermatologists strive to remain on the forefront of advances in our field, dermatology residency programs have an obligation to ensure that residents demonstrate competence in all facets of our field, and in the United States the American Board of Dermatology is moving toward a modular examination format to help assure the public that their dermatologist is truly an expert in all aspects of diagnosis and treatment of skin disease. This contribution focuses on key advances in our field, how our societies help us to remain on the cutting edge of science, and how dermatology training has kept pace.

The management of lower extremity wounds is frequently performed by means of clinical examination, representing a challenge for the clinician due to the various conditions that can potentially enter differential diagnosis. Several diagnostic techniques are available in the dermatologist's arsenal as a support to diagnosis confirmation, including dermoscopy and ultrasonography. Recently, a novel ultrasonographic technique involving the use of ultra-high ultrasound frequencies has entered the scene, and appears a promising tool in the diagnostic workup of skin ulcerative lesions. The focus of this review is to discuss the potential role of ultra-high-frequency ultrasonography in the diagnostic workup of wounds in the light of the current applications of the technique.

Melanocortins are peptides that share a common central pharmacophor. Melanin pigmentation of interfollicular epidermis and hair via MC1R remains the key physiologic function of the naturally occurring melanocortin peptides in skin. Moreover, the melanocortins are crucially involved in the ultraviolet light-induced tanning response. Under pathophysiologic conditions, melanocortin peptides induce cutaneous hyperpigmentation, likewise via the MC1R axis, e.g. in patients with Addison's disease, ectopic precursor pro-opiomelanocortin (POMC) syndrome and in those with abnormally elevated melanocortin blood levels. Translational research on α­MSH (melanocyte-stimulating hormones) and their antagonists has further revealed a variety of other biological activities beyond pigmentation. They include cytoprotection, antioxidative effects, regulation of collagen metabolism and fibrosis, sebum production, and cutaneous wound healing. These findings have also promoted the development of novel therapies in clinical dermatology including the exploitation of afamelanotide. In 2015, this agent became the first in-class synthetic α­MSH analogue to be approved in dermatology for the treatment of erythropoetic protoporphyria. In addition to afamelanotide, setmelanotide has recently emerged as a highly selective MC4R agonist useful for the treatment of distinct forms of genetically determined obesity, e.g., POMC deficiency. Future perspectives in dermatology reside in treatment of other difficult-to-treat skin diseases with α­MSH analogues, either with topical or systemic formulations. Moreover, synthetic melanocortin peptide derivatives lacking the central pharmacophor but with maintained anti-inflammatory effects could become a promising strategy for the design of new therapies in dermatology.

BACKGROUND: Journal impact factor (JIF) is a bibliometric proxy of relative journal importance. Mean dermatology JIF has nearly doubled since 1997. The reasons behind the increase have not been previously explored. OBJECTIVE: To assess factors contributing to rising dermatology JIF. METHODS: This bibliometric study utilized publicly-available citation and JIF data from the Thomson-Reuters InCites Journal Citation Reports "Dermatology and Venereology" category, from 1997-2017. RESULTS: From 1997-2017, aggregate dermatology JIF increased by 70%, associated with a 64% increase in JIF numerator (total journal citations) and a 3% decrease in JIF denominator (total journal articles and reviews). In the four highest-JIF journals (JAAD, JAMA Dermatology/Archives of Dermatology, JID, and BJD), there was an increase in citations coming from non-dermatology specialty journals, including oncology, rheumatology, and multidisciplinary sciences. Journal impact factor was positively correlated with five JIF alternatives. Immediacy Index, a reflection of how fast dermatology journals are cited, increased four-fold (P<0.001). LIMITATIONS: Impact factor numerator/denominator data were not available before 1999. CONCLUSIONS: The nearly two-fold rise in dermatology JIF from 1997-2017 was associated with increased citations, an increasing proportion of which came from non-dermatology journals. This may reflect growing influence of dermatology research within both dermatology and other fields of medicine.

To better understand and interpret the trends in cutaneous research, we carried out a network analysis of all the titles of the submitted abstracts of the annual meetings of the European Society of Dermatological Research (ESDR), including the International Investigative Dermatology (IID) meetings between 2010 and 2019. Network analysis is a data science tool to process, analyze, and visualize big sets of data. As expected, psoriasis was the frontrunner in each of the annual meetings, followed by dermatitis and melanoma. Interestingly, alopecia, acne, squamous cell carcinoma, pruritus, basal cell carcinoma, and hidradenitis suppurativa were among the next most frequently named diseases and/or terms. We also looked at diversity to assess how broad the interest of the submitting community is and to identify whether "blockbusters" such as psoriasis and atopic dermatitis expand in expense of other interests. In contrast to our expectations, the diversity of submissions to the ESDR annual meetings remained high over the 10 years of our observation period. Interestingly, the diversity increased in the years of the IID, indicating an outreach to other research areas worldwide compared with the ESDR meetings. This is true for both 2013 in Edinburgh, UK, and 2018 in Orlando, USA. During these meetings, this rise in diversity was associated with a relative decrease of the three most often named diseases. Network analysis thus may be a useful tool for research societies like the ESDR to identify trends and allocate resources such as reviewers and sessions accordingly. In addition, it can serve as quality control monitoring whether the ESDR continues to offer a platform for all researchers in cutaneous biology or implements or focuses on emerging fields.
