RESUMO

Single tooth orange discoloration secondary to root canal calcification occurs as a consequence of dental trauma, orthodontic treatment, or for unknown rea- sons. A correct case history must be compiled and a CBCT study carried out in order to establish the diag- nosis and define the best treatment plan in each case. The aim of the present study was to offer a therapeutic protocol involving a clinical decision-making tree dia- gram based on the presence or absence of apical dis- ease and the degree of canal calcification. Dental bleaching and the use of ceramic veneers allow es- thetic restoration in such cases.

RESUMO

Background: Iron supplements prescribed to anemic children may results in teeth staining. Possible methods for preventing staining of primary teeth following exposure to iron supplements are need of an hour. Aim: This study was conducted with the aim to assess effect of enamel surface coating on staining capability of iron containing supplements in primary teeth. Settings and Design: This is an in vitro Experimental study. Methods: Hundred and forty-four primary incisors with intact crowns were sequentially numbered and randomly divided into four main groups of 36 samples receiving different enamel surface coating such as no surface coating, MI fluoride varnish™, GC G-coat Plus™ and GC-Equia®-forte-coat. Each group was subdivided into two groups, namely A (sound teeth) and B (artificially demineralized teeth). All the samples were immersed in 250 ml artificial saliva containing 10 ml of iron supplement to make the iron concentration 100 mg. The shade of the teeth was measured at baseline and at 90 days, using VITA Easy shade® V digital spectrophotometer. The data was statistically analyzed using the Kruskal-Wallis test and Mann-Whitney U test. Descriptive and inferential statistical analyses was made via SPSS 23. Results: Overall color change was found to be significantly less at the end of 90th day in sound samples when GC-Equia®-forte-coat was used as an enamel surface coating with pvalue of 0.017. In demineralized samples the overall color change was least at the end of 90th day when MI Varnish™ used as an enamel surface coating with P = 0.042. Conclusions: GC-Equia®-forte-coat, MI Varnish™ and GC-G-coat-plus™ can be used as a preventive or a precautionary measure to minimizes the staining of teeth. The amount of color change at the end of 90 days with GC-Equia®-forte-coat was least followed by MI varnish™, GC G coat Plus™ and control group.

RESUMO

OBJECTIVES: This in vitro study assessed the potential of tooth discoloration by aerosols generated from three heated tobacco products (HTPs) with different specifications: in-direct heating tobacco system platform 1.0a (IT1.0a), in-direct heating tobacco system platform 2.0a (IT2.0a), and direct heating tobacco system platform 3.0a (DT3.0a). In addition, three flavor types (regular, menthol, and berry menthol) were selected for each HTP to characterize the effect of flavor types on tooth discoloration. MATERIAL AND METHODS: Six bovine tooth samples were exposed directly to aerosols generated from one pack of each HTP: 350 puffs for IT1.0a, 325 puffs for IT2.0a, and 220 puffs for DT3.0a. Six bovine tooth samples were also exposed to air (350 puffs) and smoke generated from one pack of cigarettes (160 puffs) as negative and positive controls, respectively. The color of each tooth sample was measured before and after exposure. The overall color changes were assessed using overall color differences (ΔE) calculated according to the Commission International de I'Eclairage color system. A one-way analysis of variance followed by Tukey's post hoc test was used to compare ΔE among bovine tooth samples exposed to air, cigarette smoke, and aerosols generated from each HTP. RESULTS: ΔE values for tooth samples exposed to air and aerosols generated from the three HTPs (IT1.0a, IT2.0a, and DT3.0a) were significantly lower than ΔE value for tooth samples exposed to cigarette smoke. ΔE values obtained with DT3.0a were significantly higher than those obtained with air-exposed control samples. However, ΔE values obtained with IT1.0a and IT2.0a were not significantly different from that obtained with air-exposed control samples. No HTPs showed significant differences in ΔE values among the three flavor types. CONCLUSIONS: This study showed that HTP aerosols reduce tooth discoloration potential compared with cigarette smoke, regardless of flavor types, and the tooth discoloration potential of the product may depend on product specifications.

RESUMO

OBJECTIVES: The aim of this study was to compare the plaque-inhibiting effects of oil pulling therapy with sesame oil or coconut oil using 4-day plaque regrowth study model. METHODS: This clinical observer-masked, randomized, crossover designed study involved 24 participants. The participants received professional prophylaxis in the preparatory period and after that subjects started to use the allocated mouthrinse (coconut oil or sesame oil). On day 5, periodontal clinical parameters including plaque index (PI), gingival index (GI), stain index (SI) and bleeding on probing (BOP) were recorded. Subjects underwent a 14-day wash out period and then used the other mouthrinse for 4 days. RESULTS: Oil pulling therapy with coconut oil or sesame oil exhibited similar plaque regrowth inhibition (PI = 1.60 ± 0.28 and 1.49 ± 0.22, for oil pulling with coconut oil and sesame oil, respectively) and tooth staining (SI = 0.20 ± 0.11 and 0.21 ± 0.09, for oil pulling with coconut oil and sesame oil, respectively.) In addition, GI and BOP were similar in both groups (GI = 0.61 ± 0.19 and 0.69 ± 0.16; BOP = 0.09 ± 0.24 and 0.03 ± 0.03 for oil pulling with coconut oil and sesame oil, respectively). CONCLUSIONS: Oil pulling therapy with coconut or sesame oil showed similar results in terms of plaque regrowth inhibition and tooth staining. According to the present results, both coconut oil and sesame oil can be used for oil pulling therapy with the aim of plaque regrowth inhibition.

The discoloration of dental pieces can have an aesthetic and social impact that leads patients to seek an intervention to improve their smile. Superficial stains and enamel irregularities may be due to hypoplasia, molar hypomineralization, fluorosis, drug use, white spots caused by trauma or infection in the primary dentition, or post-orthodontic stains. The diagnosis of enamel defects is made through a visual examination by transillumination. Microabrasive techniques with different agents have been proposed to remove surface stains from the enamel, as well as the use of low-concentration whitening agents to balance the color of the teeth. If the stains are deep, a mega abrasion and subsequent anatomical restoration with composite resins are required. Technological advances in adhesive restorative materials make it possible to mimic natural teeth, allowing minimal destruction of tooth structure without compromising future restorative options. The objective of this work is to show the sequence of minimally invasive procedures to return the lost aesthetics in a patient who attends the Chair of Restorative Dentistry (AU)

RESUMO

ABSTRACT Objective: To verify, through clinical and radiographic evaluations, the in vivo response of the dentin-pulpal complex of human primary teeth after pulpotomy with MTA and Biodentine™ in a follow-up period of 3, 6, and 12 months. Material and Methods: Thirty teeth were divided into MTA pulpotomy (n = 15) and Biodentine™ pulpotomy (n = 15) from children between 5 and 9 years of age, a randomized clinical trial with simple random sampling. The materials were inserted into the cavity after opening and removing the coronary pulp tissue. The cavity base consisted of glass ionomer cement and light-cured composite resin restoration. Clinical and radiographic analyses were performed after 3, 6, and 12 months. Statistical analysis by Fisher's exact test for dichotomous data at a 5% significance level was utilized. Results: Both materials caused color change after 12 months. However, MTA showed a higher percentage than Biodentine™ (p<0.0001). Pain was detected only with Biodentine™ at six months and mobility at 12 months (p=0.0013). Radiographically, after 12 months, periapical lesions, interradicular lesions, and internal resorption were evidenced in 13% of the cases for Biodentine™-treated teeth (p<0.0013). MTA induced pulp calcification in 13% of cases, unlike Biodentine™ (p<0.0013). Conclusion: BiodentineTM and MTA are suitable for clinical use in pulpotomy treatment, yet both materials lead to tooth discoloration.

RESUMO

Aim: The objective of this study was to describe a case series concerning internal bleaching of anterior traumatized teeth that underwent regenerative endodontic procedures (REP). Methods: Seven non-vital maxillary anterior teeth discolored after regenerative endodontic procedures were included and divided into two groups according to the medication protocol used in the REP: (1) Triple antibiotic paste (TAP) group (n=4); (2) Calcium hydroxide and 2% chlorhexidine gel (HC+CHX) (n=3). The bleaching technique used was walking bleach, where sodium perborate associated with distilled water was used. Bleaching agent was replaced weekly until the darkened tooth was slightly lighter than the adjacent tooth. The color was recorded with the aid of a digital spectrophotometer in two moments (T1: prior the first session of bleaching, T2: fourteen days after the last session of bleaching). The change in color after the procedure (ΔE) was calculated and reported in a descriptive analysis. Results: The ΔE for all teeth showed color differences exceeding the perceptibility threshold (ΔE > 3.7). Both groups showed similar ΔE (TAP: 18.3 ± 11.5; HC+CHX: 14 ± 11.2) at the end of the treatment. The average number of sessions needed to achieve satisfactory results was 1.7 ± 0.6 for HC+CHX group and 2.3 ± 0.5 for TAP group. Conclusion: Internal bleaching with sodium perborate associated with distilled water is effective in treating discolored teeth after regenerative endodontic procedures

RESUMO

Abstract The aim of this in vitro study is to investigate the effects of acidic beverages on the discoloration of bulk-fill composites with different viscosities. 144 disc-shaped specimens (8mm diameter × 2mm thick) were prepared from four different composite resins including a nanohybrid (Filtek Ultimate), two packable bulk- fill (Filtek Bulk Fill, Tetric N Ceram Bulk Fill), and a flowable bulk-fill composite (Tetric N Flow Bulk Fill). The specimens of each group were divided into 3 subgroups and submerged in distilled water, orange juice and coke (n=12). Color difference (ΔE) were measured using CIELab color space and a compact spectrophotometer (VITA Easyshade) at baseline and after 30 days of immersion. Data were analyzed using two- way ANOVA and Tukeys HSD post-hoc tests (P2.7) was observed for all bulk-fill composites tested. It can be speculated that orange juice has a more negative effect on the color stability compared to coke. Additionally, flowable bulk-fill composite was more prone to discoloration than condensable bulk-fill composites.

BACKGROUND: Discoloration of teeth occurs for various reasons. Common ingredients like substances from tea and coffee, as well as antibiotics like tetracycline, or food dyes, can percolate into the teeth, and, as such, these stains may persist in the porous structure of the enamel. Smoking is also contributory to dental discoloration, with toxins of tobacco smoke accumulating in a similar way. With aging, teeth tend to be discolored with accumulation of various stains in addition to the enamel gradually eroding to expose the yellow dentin. HIGHLIGHT: This review focused on the effect of several natural ingredients with teeth-whitening properties and their daily clinical application. Metabolic dental bleaching mechanisms, as well as tooth discoloration and decay, were also reviewed. The current scientific literature (mostly from 2000 to 2020) was consolidated from manuscripts retrieved from Scopus, PubMed, ResearchGate, and Google Scholar. CONCLUSION: Natural teeth whitening effectively lightens the natural color of teeth without eroding dental surfaces. On the other hand, commercially available whiteners containing hydrogen peroxide and carbamide peroxide, in high concentrations, can lead to deproteinization and demineralization of teeth through oxidation processes. If used extensively, these compounds may cause a number of adverse effects. Alternative natural teeth-whiteners include ingredients like lemons, strawberries, oranges, papaya, and other fruits. Such natural ingredients offer a milder and safer way of whitening teeth than whiteners containing hydrogen peroxide or carbamide peroxide.

Discoloration remains an unfavorable complication of otherwise successful regenerative endodontic procedure of immature teeth with necrotic pulp. Objective: Compare the degree of discoloration of extracted human teeth after regenerative endodontic procedures, using MTA or Biodentine as a cervical barrier with a blood clot as a scaffold. In total 24 human upper anterior teeth were prepared and divided into two control groups (CG) and two experimental groups (EG). In the EG, fresh human blood was placed inside the root and a Biodentine (GE3) or MTA (GE4) barrier was made in. A sterile cotton swab saturated with sterile physiological solution was placed in the GCs and a Biodentine (GC1) or MTA (GC2) barrier was made in. The color was evaluated according to the CIE L* a* b* color space using standardized digital photographic images at two time points: day 0 (T0) and day 35 (T35). The description of the data included median (Md), first quartile (Q1), third quartile (Q3), means, and standard deviation (SD). Wilcoxon signed rank tests (RSW) and one-way ANOVA were used. p < 0.05 was considered significant. When ∆E was compared, significant differences were observed between GC1 and the rest of the materials (p < 0.05). ∆E was lower in CG1 (mean ± SD; 2.1 ± 1.6) than in the remaining groups, which did not present significant differences between them. There was no significant difference between tooth discolorations with materials in the presence of blood. However, in the absence of blood, Biodentine exhibited less tooth discoloration than MTA (AU)

Introduction: Tooth discoloration is an aesthetic problem commonly reported by patients as one of the main reasons to seek a dentist. Thus, this study aimed to evaluate the dentin discoloration caused by AH Plus, MTA Fillapex, Sealer Plus and Pulp Canal Sealer EWT endodontic sealers, associated or not with ultrasonic agitation during the obturation. Methods: Round cavities were prepared on the palatal faces of eighty bovine teeth slabs, preserving a 2 mm of dentin thickness. The slabs were randomly separated in groups by type of sealer employed and based on the use or not of ultrasonic agitation (n = 10). The color was measured by a spectrophotometer after sealer insertion into the cavities (T0), after 7 (T1) and 180 days (T2). Results: Clinically detectable discoloration (∆E > 3,7) was present in all groups at T1 and T2. Statistically significant differences were not found between T1 and T2 (P > .05). Conclusion: All endodontic sealers tested caused dentin discoloration after 7 days. The color-changing persisted thought the entire experimental period. Moreover, the ultrasonic agitation did not influence on the dentin discoloration caused by the endodontic sealers tested (AU).

RESUMO

ABSTRACT: Purpose: The purpose of this study was to evaluate the discoloration of a dual-cure bulk-fill (DCBF) and light-cure bulk-fill (LCBF) composites relative to conventional composites (CC) after water and coffee immersion. Methods: One- hundred disc-shaped specimens (8mm in diameter × 4mm thickness) were prepared using five commercially available composites (n=10); two LCBF (XB-X-trafil Voco, FB-FiltekTM Bulk Fill), one DCBF (FU-Fill-upTM Coltene), and two CC (CE-Clearfil Majesty ES2, EQ-Estelite ΣQuick). Initial and final color readouts were measured with a spectrophotometer (Easyshade,Vita) according to the CIELAB color system. Statistical analyses were performed using 2-way ANOVA with Bonferroni adjustment to evaluate ΔE, ΔL, Δa, and Δb parameters (pFB≥XB>EQ≥CE in water immersion groups and XB>FB>EQ≥FU≥CE in coffee immersion groups. Conclusion: Within the limitations of this study, DCBF composite is more susceptible to intrinsic discoloration compared to LCBF and CC. However, DCBF exhibited lower extrinsic discoloration than LCBF and comparable with CC after coffee immersion. Depending on their chemical and structural compositions, composites will exhibit color change, which compromises the esthetic performance of composite restorations.

ABSTRACT: Objective: The aim of this study was to evaluate and compare the effects of the superficial application of two different modeling resins on the surface microhardness and discoloration of composite resins. Material and Methods: The present study used two different composites and modeling resins. The composites were placed in plastic molds. Subsequently, the modeling resins were applied on the surface of the two composite groups. The microhardness and color pertaining to all the groups were evaluated. The current study used the One-Way ANOVA and Kruskal Wallis tests, in order to perform the statistical evaluation (p<0.05). Results: The present study compared the microhardness pertaining to the groups and the highest microhardness values were observed in the Estelite Asteria group (none), whereas the lowest values were observed in the GC Essentia group (Ultradent). Moreover, the current study evaluated the color stability and the greatest discoloration was observed in the control group of the GC Essentia group, whereas the least discoloration was observed in the Estelite Asteria group that included the samples prepared using Ultradent Wetting Resin. Conclusions: The results of the present study demonstrated that the use of modeling resins, which facilitate the placement of composite resins, reduced the microhardness and discoloration of composite resins. The aforementioned effect can be attributed to the variations in the structure of the filling. However, further studies are warranted to support and verify the results of the current study.

The aim of this study is to evaluate the crown discoloration induced by bioceramic root canal filling materials (OrthoMTA and iRoot SP) compared to AH Plus. Material and Methods: Sixty intact mandibular single rooted premolars were sectioned 2 mm below the cemento-enamel junction, prepared, and randomly assigned into four groups according to the root filling materials: OrthoMTA, iRoot SP, AH Plus and unfilled. Results: Before placement of the materials in the pulp chamber and the coronal third of the root, the spectral reflectance lines of the crowns were recorded by a digital spectrophotometer at baseline, and after filling at 1 week and 1, 3 and 6 months and ∆Ε values were calculated. All materials used induced clinically perceptible crown discoloration (∆Ε>3.7) and no significant difference was detected between these materials (p>0.05). Regardless of the material, discoloration progressed significantly within the three months (p<0.05) however, at 6 months, the discoloration reduced for AH Plus and no further increase for bioceramic materials was detected. Conclusion: Bioceramic root filling materials tested induced clinically perceptible crown discoloration and their application in the esthetic zone should be performed with caution.

Tetracycline may cause tooth discoloration when used in young children during tooth development. Whether tigecycline, a tetracycline derivative, has either a similar adverse event or not remains unclear. We assessed the discoloration of the permanent teeth of patients <8 years old after tigecycline exposure. These patients were identified through a retrospective chart review in a Chinese children's hospital. Those who had at least one erupted permanent tooth after tigecycline exposure were interviewed, examined, and photographed by an experienced pediatric dentist and independently assessed by another senior dentist to detect tetracycline-like tooth discoloration. We identified 101 patients who were exposed to tigecycline, 12 of whom were included. The mean daily dose of tigecycline was 2.3 mg/kg of body weight (standard deviation, 0.6), and the median duration was 12.5 days (interquartile range [IQR], 8.0 to 19.3). The median age of exposure was 5.2 years (IQR, 4.5 to 7.4), and the median age of dental examination was 9.1 years (IQR, 9.0 to 10.3). Two patients (16.7%) developed yellow discoloration: a girl having yellow discoloration with white-to-yellow opacities in the upper lateral incisors and lower incisors and a boy with a suspicious buccal yellow discoloration and enamel dysplasia in the second molars. The incidence and extent of tigecycline-associated dental adverse events remain unclear due to the small sample size and inadequate follow-up period.

RESUMO

Abstract: Objective: This study investigated the effects of three types of mouthwash on the color stability of three resin-based restorative materials (nanohybrid resin composite, sonic-activated bulk-fill material, and compomer). Materials and Methods: To this end, 120 cylindrical disc specimens were prepared and polished, after which they were incubated in distilled water for 24 hours for post-polymerization. The baseline color values of each specimen were measured with a spectrophotometer, then the specimens were randomly divided into four groups. Following immersion in the mouthwashes for 24 hours, the specimens were rinsed with distilled water and dried before the color measurement was repeated. The values of color change (ΔE₀₀) were subsequently calculated. A two-way analysis of variance and Duncan's post hoc multiple comparison tests were conducted to determine statistically significant differences among the restorative materials. Results: All samples showed discoloration after immersion, albeit some instances of discoloration were imperceptible. In this respect, a statistically significant difference was found among the restorative materials and mouthwashes (p<0.05). Imperceptible ΔE₀₀ values were derived on the nanohybrid resin composite and compomer, but perceptible and acceptable ΔE₀₀ values were detected on the sonic-activated bulk-fill materials (p<0.05). The clinically unacceptable discoloration was observed on the sonic-activated bulk-fill composite immersed in Listerine®. Conclusions: Clinicians should make recommendations based on the potential staining capacity of mouthwashes prescribed for daily use in restorative materials. In this way, the need for renewal of restorations due to discoloration could be avoided.

Background and objective: The aim of this study is to compare the bleaching effect of photon-induced photoacoustic streaming (PIPS) using carbamide peroxide or sodium perborate-20% hydrogen peroxide (H2O2) compared with walking-bleaching techniques using sodium perborate-20% H2O2 or carbamide peroxide without PIPS on artificially colored teeth. Materials and methods: Root canal treatments of 85 extracted human maxillary central incisors were completed and artificially stained using sheep's blood. Teeth were randomly grouped into five groups according to the bleaching techniques to be tested: Group 1: walking bleaching with carbamide peroxide; Group 2: walking bleaching with sodium perborate-20% H2O2; Group 3: PIPS+carbamide peroxide; Group 4: PIPS+sodium perborate-20% H2O2; and Group 5: control. Experimental specimens were bleached for 3 weeks. Buccal surfaces of the crowns were used for spectrophotometric measurements, limited with a 7-mm external diameter rounded strip, before bleaching and on days 7, 14, and 21. The total change in color (ΔE*) were analyzed with Welch's ANOVA at a significance level of p = 0.05 and Tamhane post-hoc test was used for multiple comparisons. Results: Starting from the second week, Group 4 showed significantly better bleaching effect compared with the other groups (p < 0.05). The other experimental groups also have bleaching effects, but there was no statistically significant difference between Group 1, Group 2, and Group 3 (p > 0.05). Conclusions: Sodium perborate-20% H2O2 activated with PIPS gave a bleaching effect superior to that of walking-bleaching techniques or PIPS using with carbamide peroxide.

Color is a visual effect of light rays reflecting and its concept is complex, for being a sensation that is perceived and for the electromagnetic characteristics. Teeth vary spatially because they are curved, have relatively small extensions, and are viewed against a non-uniform background variable as well as typically non-standardized illumination, which is why they differ in relation to their colorimetry. So the study of color is fundamental in dentistry. Currently, there are methods to evaluate color, from a simple visual check to instruments such as the colorimeter and spectrophotometers, which are devices used to measure the color of an object through its reflected wavelength. A dental pigmentation is produced by various factors, both intrinsic and extrinsic, these pigmentations are currently important factors in both aesthetics and physical appearance, so it is important to be able to evaluate the stability of natural teeth against different substances that could modify its natural color. In this study, we undertook the task of evaluating the change in the color of natural teeth when faced with different beverages. Three pigment and commonly used beverages were chosen: coffee, red wine and cranberry juice; 10 single-rooted teeth of the same color previously analyzed with the spectrophotometer were used. One tooth was the control sample and the remaining nine were immersed in separate bottles with 10 mL of the three chosen drinks. Carrying out the color evaluation at 15, 30 and 90 days with the help of the spectrophotometer, we could see that the tooth immersed in coffee did not change during the first 15 days and the most notable change in color was up to 90 days, unlike the samples immersed in wine and cranberry juice whose maximum color variation was presented in 15 days respectively (AU)

RESUMO

Abstract The endodontic revascularization may be an alternative treatment for necrotic immature teeth, however, several treatment steps may cause tooth discoloration. This study evaluated the use of three calcium silicate-based cements with different radiopacifying agents on the color alteration (∆E) of extracted premolars after simulation of revascularization. Forty single rooted extracted premolars were shaped with #1-6 gates Glidden drills, rinsed with sodium hypochlorite, and filled with fresh human blood. Three calcium silicate-based cements with different radiopacifying agents (bismuth oxide - CSBi, calcium tungstate - CSW, and zirconium oxide - CSZr) were applied over the blood clot (n=10). The control group received the application of a temporary zinc oxide-based cement (TFZn) (n=10). ∆E was measured with a spectrophotometer, using the L*a*b* color system of the International Commission on Illumination (CIELab), in different times: prior to the preparation of the access cavity (t0); right after treatment (t1); and after one (t2), two (t3), three (t4) and four (t5) months. The tooth site for color evaluation was standardized by silicon matrix, the color reading was performed 3 times per tooth, and the teeth were stored in 37º water between evaluations. ∆E, whiteness (WID index) and yellowness (b*) were evaluated. Data were subjected to one-way ANOVA and repeated measures ANOVA, followed by Tukey's post hoc test (α=0.05). All groups were similar in ∆E1 (t0-t1). The ∆E was the lowest and constant in the control group. In all evaluation times, CSBi presented the highest ∆E (p<0.01). CSW and CSZr were similar in all evaluated times and presented intermediate ∆E values. WID index from CSBi and CSW presented more distancing from 'white' reference. CSBi presented the greatest decrease in yellowness (b* value). The cement containing bismuth oxide presented the highest color alteration values. All tested calcium silicate-based cements presented clinically perceptible discoloration. Calcium tungstate and zirconium oxide may be used as alternative radiopacifiers to decrease tooth discoloration after endodontic tooth revascularization.
