DATA SOURCES: Electronic searches were conducted on databases (PubMed, EMBASE, and Google Scholar). In addition, websites of national organisations (US Food and Drug Administration, National Cancer Institute, Centres for Disease Control and Prevention, American Dental Association, Office of Disease Prevention and Health Promotion, National Institute on Drug Abuse, Agency for Healthcare Research and Quality) were also searched. STUDY SELECTION: To achieve the objectives of the study, systematic reviews, controlled clinical trials, and observational studies published between October 2021 and February 2022 were considered. DATA EXTRACTION AND SYNTHESIS: This narrative review included articles which investigated the role of Dentistry professionals and their impact on smoking cessation and the effects resulting from tobacco use on oral health. RESULTS: The review revealed that smokers have a significantly higher likelihood of developing oral cancer (95% CI: 3.19-6.77) compared to non-smokers. Passive smokers also have an increased risk (1.51 times) of developing oral cancer (95% CI: 1.20-1.91). Additionally, smokers have an 80% increased risk of periodontitis (RR = 1.82; 95% CI: 1.43-2.31), an 85% worsened periodontal condition (RR = 1.85; 95% CI: 1.5-2.2), and a 36.6% increase in caries prevalence (OR = 1.84; 95% CI: 1.64-2.07). Smoking is also associated with a higher potential for dental implant failure in a dose-dependent manner. Brief educational interventions by the dental team resulted in a smoking cessation rate of 74/1000 individuals versus 27/1000 individuals in the control group. When combined with pharmacological therapy, these interventions may lead to an additional 50 to 70% increase in long-term smoking abstinence. CONCLUSIONS: Smoking is strongly linked to an increased risk of oral cancer, dental caries, implant failure, and periodontal disease. Dental teams play a vital role in identifying and addressing oral pathologies related to smoking and providing necessary care for smoking cessation. Brief educational interventions, either alone or in combination with pharmacotherapy, offer valuable approaches for the dental team to support smoking cessation. However, establishing a comprehensive training and continuing education program is crucial to integrate dental professionals into a multidisciplinary smoking cessation program.

BACKGROUND: Smokeless tobacco (SLT) products of several different types are widely used in several South Asian countries including Pakistan. These products are consumed in different forms and with different names. The study aims to determine adverse effects of the SLT consumption on periodontal tissues. METHODS: This cross sectional study was conducted in Karachi, Pakistan. It recruited 377 users of (SLT) with 231 males (61.3%) and 146 females (38.7%) of age 15 to 45 years. After obtaining informed consent, quantitative data were collected via a questionnaire followed by intra oral clinical examination to determine presence of periodontal diseases using community periodontal index (CPI). To determine the association between periodontal diseases and smokeless tobacco consumption habits, Chi Square test was conducted. RESULTS: Gingival recession (Class II-IV) (65.8%) was the most prevalent periodontal disease among SLT users. CPI score was high (CPI score 3 and 4) in 31.3% participants, whereas tooth loss was found in 21%. Among types of SLT products, gutka (28.6%) and betel quid (23.3%) were most commonly used. Using SLT for five or more years was found to be associated with a high CPI score, gingival recession (Class II-IV), moderate to severe tooth mobility, and presence of tooth loss. CONCLUSIONS: The study found statistically significant association between duration in years of using SLT and periodontal disease including gingival recession, tooth mobility and tooth loss. However, no significant results were found between retention during single use and frequency of SLT use per day. However, the link of these factors with the periodontal disease cannot be eliminated.

For nearly a century, researchers have associated periodontal disease (PD) with risks of other adverse health outcomes such as cardiovascular disease, diabetes mellitus, and respiratory diseases, as well as adverse pregnancy outcomes. Those findings have led to the hypothesis that PD causes those adverse health outcomes either by increasing systemic inflammation or by the action of periodontopathic bacteria. However, experiments largely failed to support that hypothesis. Instead, the association is casual, not causal, and is due to shared underlying modifiable risk factors, including smoking, diet, obesity, low levels of physical activity, and low vitamin D status. Diabetes mellitus is also considered a risk factor for PD, whereas red and processed meat are the most important dietary risk factors for diabetes. Because PD generally develops before other adverse health outcomes, a diagnosis of PD can alert patients that they could reduce the risk of adverse health outcomes with lifestyle changes. In addition, type 2 diabetes mellitus can often be reversed rapidly by adopting an anti-inflammatory, nonhyperinsulinemic diet that emphasizes healthful, whole plant-based foods. This review describes the evidence that proinflammatory and prohyperinsulinemia diets and low vitamin D status are important risk factors for PD and other adverse health outcomes. We also make recommendations regarding dietary patterns, food groups, and serum 25-hydroxyvitamin D concentrations. Oral health professionals should routinely inform patients with PD that they could reduce their risk of severe PD as well as the risks of many other adverse health outcomes by making appropriate lifestyle changes.

BACKGROUND: smoking is considered the most modifiable risk factor for periodontal disease. OBJECTIVE: the aim of this narrative review is to emphasize the effect of smoking on periodontal and implant therapy. METHODS: The authors reviewed the literature reporting the clinical outcomes of smoking on periodontal surgical and nonsurgical treatment. The impact of smoking on implant therapy and sinus lifting procedures were also reviewed. RESULTS: Periodontal and implant therapy outcomes are adversely affected by smoking. Smokers respond less favorably to periodontal therapy and periodontal flap procedures as compared to nonsmokers. Clinical outcomes for smokers are 50-75% worse than for nonsmokers. Studies reveal that smokers experience a significantly lower reduction in pocket depth compared to nonsmokers as well as less bone growth after treating infra-bony defects with guided tissue regeneration. The relative risk of implant failure is significantly higher in patients who smoke 20 cigarettes or more per day compared to nonsmokers. Additionally, smoking has also been shown to increase postoperative wound dehiscence and infection rates following sinus floor elevation. Longitudinal studies on smoke cessation have shown a reduction in bone loss and probing depths for periodontitis patients after cessation compared to those who smoke. CONCLUSION: Smoking cessation can reduce probing depths and improve clinical attachment after nonsurgical periodontal therapy. There is insufficient evidence regarding the effect of smoking on peri-implantitis, as well as the loss of implants in the long-term.

Organ transplantation is an effective treatment for end-stage organ diseases. However, organ transplant recipients are susceptible to a wide variety of oral diseases, including gingival enlargement, periodontitis, oral mucosal diseases, oral malignant tumors, and dental caries. Oral microbiota may have played an important role in the organ transplant patients' increased susceptibility to oral diseases and is associated with adverse events after organ transplantation, which is gradually gaining more attention among scholars. We, herein, reviewed the common oral diseases, including periodontal tissue diseases, oral mucosal diseases, oral malignant tumors, and dental caries in organ transplantation patients. Furthermore, we discussed the characteristic changes in the oral microbiota of organ transplantation patients and the influencing factors of these changes. In-depth study of oral microbiota of organ transplant patients provides a reference for the prevention and treatment of relevant diseases after organ transplantation and serves an important role in oral and systemic health management of organ transplant patients.

Introducción: La enfermedad periodontal comprende un conjunto de trastornos asociados a la inflamación y a la pérdida de estructuras del soporte dental de origen infeccioso multifactorial. La investigación científica expone un abanico de cepas bacterianas probióticas en el tratamiento periodontal, con propiedades en la inhibición del crecimiento de periodontopatógenos, y mejoras significativas en los parámetros clínicos, cuando se combina con el tratamiento periodontal mecánico. Objetivo: Describir el efecto de la terapia combinada entre el tratamiento periodontal mecánico tradicional y un gel probiótico multicepa, aplicado a un caso de periodontitis. Presentación de caso: Paciente femenino de 53 años de edad, procedente de la comunidad de El Portachuelo, zona rural aledaña a la ciudad de Mérida. Presentó agrandamiento gingival, enrojecimiento y dolor espontáneo en la encía palatina, con concomitantes de sangrado y exudado purulento desde hace seis meses. La realización de un examen clínico periodontal, análisis radiográfico y análisis microbiológico permitieron diagnosticar periodontitis estadio IV grado B. Se aplicó tratamiento periodontal convencional no quirúrgico en combinación con cepas probióticas de Lactobacillus rhamnosus, L acidophilus y L. plantarum, en presentación de un gel adhesivo para la aplicación supra y subgingival y en suspensión vía oral para obtener un efecto sistémico durante 45 días. Los resultados obtenidos fueron favorables con mejoramiento de los parámetros clínicos como índice de sangrado periodontal y profundidad de los sacos periodontales. Conclusiones: En concordancia con diversos estudios, se ha demostrado que la terapia probiótica puede ser usada como tratamiento coadyuvante en casos de periodontitis. Se observa una franca y rápida mejoría al utilizar terapias multicepas(AU)


Introduction: Periodontal disease comprises a set of disorders associated with inflammation and the loss of dental support structures of multifactorial infectious origin. Scientific research exposes a range of probiotic bacterial strains in periodontal treatment, with properties in inhibiting the growth of periodontal pathogens, and significant improvements in clinical parameters, when combined with mechanical periodontal treatment. Objective: Describe the effect of combined therapy between traditional mechanical periodontal treatment and a multi-strain probiotic gel, applied to a case of periodontitis. Case presentation: A 53-year-old female patient from the community El Portachuelo, a rural area next to the city of Mérida. She presented gingival enlargement, redness and spontaneous pain in the palatine gum, with concomitant bleeding and purulent exudate for six months. A clinical periodontal examination, radiographic analysis and microbiological analysis made it possible to diagnose stage IV grade B periodontitis. Conventional non-surgical periodontal treatment was applied in combination with probiotic strains of Lactobacillus rhamnosus, L. acidophilus and L. plantarum, in presentation of an adhesive gel for supra and subgingival application and in oral suspension to obtain a systemic effect for 45 days. The results obtained were favorable with improvement of clinical parameters such as periodontal bleeding index and depth of periodontal sacs. Conclusions: In accordance with several studies, it has been shown that probiotic therapy can be used as an adjuvant treatment in cases of periodontitis. A clear and rapid improvement is observed when using multi-strain therapies(AU)

OBJECTIVE: Many studies have been conducted to understand the association between e-cigarette use and different periodontal parameters, but the effect of conventional smoking in explaining this association remain to be elucidated. This study aimed to assess the association between e-cigarettes and self-reported periodontal disease, and whether smoking status explains this association. METHODS: This cross-sectional study was based on secondary data analysis of 8,129 participants of the National Health and Nutrition Survey (NHANES) 2015 -2018. E-cigarette use and smoking status were collected through questionnaires. Self-reported periodontal diseases and bone loss were derived from the 8-item CDC/AAP questionnaire. The associations between e-cigarette use and self-reported periodontal diseases were tested in logistic regression models adjusting for demographic factors, socioeconomic indicators, smoking status, diabetes and dental visits. Additionally, to test the effect of smoking on the relationship, three fully adjusted logistic regression models stratified by smoking status were constructed. RESULTS: E-cigarettes ever users and current users had higher odds of self-reported periodontal disease (OR = 1.43, 95% CI: 1.18, 1.73) and bone loss (OR = 1.80, 95% CI: 1.30, 2.49) respectively compared to non-users after adjusting for smoking and potential confounders. In the regression models stratified by smoking status, e-cigarette was only significantly associated with self-reported periodontal disease variables among current smokers, but not among previous or never smokers. CONCLUSION: E-cigarette use is associated with self-reported periodontal disease. However, smoking status appears to explain the relationship between e-cigarette use and periodontal disease. CLINICAL SIGNIFICANCE: This population-based cross-sectional study shows that e-cigarette use is associated with self-reported periodontal disease. Smoking status appears to explain the association. Dental professionals should consider the effect of conventional smoking and comprehend the risks of e-cigarette on oral health and its benefits when used as a smoking cessation aid.

Plaque control is one of the most recommended approaches in the prevention and therapy of caries and periodontal diseases. However, although most individuals in industrialized countries already perform daily oral hygiene, caries and periodontal diseases still are the most common diseases of mankind. This raises the question of whether plaque control is really a causative and effective approach to the prevention of these diseases. From an evolutionary, biological, and nutritional perspective, dental biofilms have to be considered a natural phenomenon, whereas several changes in human lifestyle factors during modern evolution are not "natural". These lifestyle factors include the modern "Western diet" (rich in sugar and saturated fats and low in micronutrients), smoking, sedentary behavior, and continuous stress. This review hypothesizes that not plaque itself but rather these modern, unnatural lifestyle factors are the real causes of the high prevalence of caries and periodontal diseases besides several other non-communicable diseases. Accordingly, applying evolutionary and lifestyle medicine in dentistry would offer a causative approach against oral and common diseases, which would not be possible with oral hygiene approaches used on their own.

BACKGROUND: The aim was to assess the association between levels of advanced glycation endproducts (AGEs) in the gingival crevicular fluid (GCF) and periodontal parameters among cigarette-smokers and waterpipe-users. METHODS: Self-reported cigarette-smokers; waterpipe-users and never-smokers were included. Demographic data was recorded using a questionnaire. Periodontal parameters (plaque index [PI], gingival index [GI], clinical attachment loss [AL], probing depth [PD], and marginal bone loss [MBL]) were assessed in all groups. The GCF samples were collected using standard techniques and assessed for AGEs levels using enzyme-linked immunosorbent assay. Sample-size estimation was done and group-comparisons were done. Correlation between levels of GCF AGEs levels and periodontal parameters was assessed using a logistic regression model. Level of significance was set at P < 0.01. RESULTS: Eighty-two individuals (28 cigarette-smokers, 28 waterpipe-users and 26 never-smokers) were included. There was no difference in mean ages of all patients. Cigarette-smokers had a smoking history of 5.1 ± 0.2 pack years and waterpipe-users were using waterpipe for 4.4 ± 0.6 years. There was no statistically significant difference in PI, GI, clinical AL, PD and MBL in all groups. Levels of AGEs were significantly higher among cigarette-smokers (P < 0.001) and waterpipe-users (P < 0.001) than never-smokers. There was no significant correlation between levels of GCF AGEs levels and periodontal parameters in all groups. CONCLUSION: Clinical periodontal status of individuals with a short history of cigarette-smoking and waterpipe-usage may appear similar to never-smokers. On a molecular level, cigarette-smoking and waterpipe-users express raised levels of AGEs than never-smokers that sirens about the ongoing yet latent periodontal inflammatory process.

The etiopathogenesis of dental implant failure is multifactorial and may include numerous local and systemic factors, however, studies including both local and systemic factors are still lacking. Therefore, the aim of this study was to evaluate whether periodontal disease, oral hygiene index, i.e. bleeding on probing (BOP), full mouth plaque index (FMPI), smoking, systemic diseases, as well as implant characteristics (length and diameter) affect failure of implant-prosthodontic therapy. Data on 670 patients were retrieved in whom 1260 dental implants had been placed and followed-up for at least five to ten years. Categorical data were analyzed by the χ2-test, whereas Mann-Whitney test was used for continuous variables (age, BOP and FMPI). The values of p<0.05 were considered significant. The effect of local and systemic factors on the success of implant-prosthodontic therapy was assessed by multiple logistic regression analysis. Forty-five (6.7%) patients had systemic diseases, of which diabetes mellitus was most common, followed by atherosclerosis, diabetes and atherosclerosis, diabetes mellitus type 1, lymphoma, and hepatitis C. One-third (33.4%) of the patients were smokers. Periodontal disease was present in 170 patients, while 500 patients were without periodontal disease. Nine implants were lost during the period of five years. There were no differences regarding the type of implant or type of connection to the prosthetic suprastructure. However, most of these patients had a periodontal disease. There were no significant differences in dental implant failure rates between smokers and non-smokers or between patients with and without systemic diseases. Furthermore, the results of this study showed that implant type (straight vs. tapered) and type of connection with prosthodontic appliance (cemented or screw retained) did not affect BOP and FMPI. In smokers, significant improvement of BOP and FMPI was noticed. Initially, smokers had a significantly worse BOP (0.0037) when compared to non-smokers; however, there were no differences regarding FMPI (p=0.4218) between the two groups. In patients with periodontal disease, improvement of BOP and FMPI was seen at 5-year follow-up and no significant differences were found when compared to patients without periodontal disease. There were no significant differences in BOP and FMPI between patients with and without diabetes at 5-year follow-up. Atherosclerosis had a significant negative effect on BOP, but not on FMPI at 5-year follow-up. It is concluded that periodontal disease had a significant impact on the implant-prosthodontic therapy.

Failure to attenuate inflammation coupled with consequent microbiota changes drives the development of bone-destructive periodontitis. Quercetin, a plant-derived polyphenolic flavonoid, has been linked with health benefits in both humans and animals. Using a systematic approach, we investigated the effect of orally delivered Quercetin on host inflammatory response, oral microbial composition and periodontal disease phenotype. In vivo, quercetin supplementation diminished gingival cytokine expression, inflammatory cell infiltrate and alveolar bone loss. Microbiome analyses revealed a healthier oral microbial composition in Quercetin-treated versus vehicle-treated group characterized by reduction in the number of pathogenic species including Enterococcus, Neisseria and Pseudomonas and increase in the number of non-pathogenic Streptococcus sp. and bacterial diversity. In vitro, Quercetin diminished inflammatory cytokine production through modulating NF-κB:A20 axis in human macrophages following challenge with oral bacteria and TLR agonists. Collectively, our findings reveal that Quercetin supplement instigates a balanced periodontal tissue homeostasis through limiting inflammation and fostering an oral cavity microenvironment conducive of symbiotic microbiota associated with health. This proof of concept study provides key evidence for translational studies to improve overall health.

During the COVID-19 pandemic, many manifestations of the disease appear in the oral, perioral and maxillofacial regions, either related to the virus itself or to the drugs used in the treatment. Aim: This study aims to identify the most common oral and perioral lesions in hospitalized patients with COVID-19 in the city of Mosul and their management; and also to evaluate the incidence and prevalence of these lesions. Material and Methods: Prospective study included 338 patients (138 females, 200 males) who had positive PCR results for SARS-CoV-2, with oral manifestations. All data were analyzed taking the means, frequencies, and percentage. Results: The most common lesions were colored lesions (31%) and stomatalgia (27%). The most common oral disorder and prior comorbidity combination was stomatalgia in patients with a history of hypertension. The most common associated systemic diseases were diabetes mellitus (19%) followed by hypertension (17%). Macular lesions had a higher response to drugs (40%) followed by stomatalgia (28%), in comparison with necrotic lesions (0%). Treatment of oral lesions included surgical interventions (26%) as well as conventional medical treatment (74 %). Conclusion: Oral and perioral disorders during COVID-19 are transitory and more evidence is warranted to efficiently address these comorbidities in the short term.


Durante la pandemia de COVID-19, muchas manifestaciones de la enfermedad aparecen en las regiones oral, perioral y maxilofacial, ya sea relacionadas con el propio virus o con los fármacos utilizados en el tratamiento. Objetivo: Este estudio tiene como objetivo identificar las lesiones orales y periorales más comunes en pacientes hospitalizados con COVID-19 en la ciudad de Mosul y su manejo; y también evaluar la incidencia y prevalencia de estas lesiones. Material y Métodos: Estudio prospectivo que incluyó a 338 pacientes (138 mujeres, 200 hombres) que tenían resultados positivos de PCR para SARS-CoV-2, con manifestaciones orales. Todos los datos se analizaron tomando las medias, frecuencias y porcentaje. Resultados: Las lesiones más frecuentes fueron las coloreadas (31%) y la estomatalgia (27%). La combinación de trastorno oral más común y comorbilidad previa fue la estomatalgia en pacientes con antecedentes de hipertensión. Las enfermedades sistémicas asociadas más comunes fueron la diabetes mellitus (19%) seguida de la hipertensión (17%). Las lesiones maculares tuvieron una mayor respuesta a los fármacos (40%) seguidas de la estomatalgia (28%), en comparación con las lesiones necróticas (0%). El tratamiento de las lesiones orales incluyó intervenciones quirúrgicas (26%) y tratamiento médico convencional (74%). Conclusión: Los trastornos bucales y periorales durante el COVID-19 son tran-sitorios; se necesita más evidencia para abordar de manera eficiente estas comorbilidades a corto plazo.

Despite growing knowledge of the adverse effects of cigarette smoking on general health, smoking is one of the most widely prevalent addictions around the world. Globally, about 1.1 billion smokers and over 8 million people die each year because of cigarette smoking. Smoking acts as a source for a variety of oral and systemic diseases. Various periodontal issues such as increased pocket depth, loss of alveolar bone, tooth mobility, oral lesions, ulcerations, halitosis, and stained teeth are more common among smokers. This systematic review was conducted according to the guidelines from PRISMA, and research articles were retrieved from the Web database sources on 31 May 2021. The quality of research articles was ensured by the type of evidence from combined schema incorporating as schema-13 evidence type description, Cochrane health promotion and public health field (CHPPHF), and the health gains notation framework-14 screening question for quality assessment of qualitative and quantitative studies. Smokers have been found to have bleeding on probing, periodontal pockets, and clinical attachment loss compared to nonsmokers. Oral and respiratory cancers are among the most lethal known diseases caused by cigarette smoking and other commonly occurring sequelae such as stained teeth, periodontal diseases, etc.

Periodontal disease is a chronic inflammatory, multifactorial condition, that, in the absence of an early and adequate treatment, may lead to a progressive damaging of the alveolar tissues that support the teeth (periodontal ligament, cement and alveolar bone) followed by teeth mobility and, subsequently, their loss. Periodontal disease is one of the most common inflammatory disease affecting adult individuals all over the world, being considered a real worldwide pandemic. This disease may influence the progression of certain systemic diseases: diabetes mellitus, cardiovascular diseases, ischemic cardiomyopathy, myocardial infarction, stroke, neurodegenerative diseases, chronic kidney diseases, cancer, etc. The association between smoking and periodontal disease was described in numerous clinical and epidemiological studies, suggesting that products derived from tobacco burning may change the clinical aspects and the disease progression. The present study analyzed microscopically and immunohistochemically 58 periodontal fragments, from 50 patients, chronic smokers, clinically diagnosed with severe periodontitis. There were highlighted major changes in the gingival epithelium (epithelium thickening, acanthosis, intraepithelial edema, infiltrates of neutrophils or lymphocytes, epithelial necrosis), in the periodontal conjunctive tissue (more or less intense inflammatory infiltrates, microhemorrhages, vascular congestion, intense immunohistochemical expression for some matrix metalloproteinases). The periodontal changes may be the expression of both toxic factors present in tobacco smoke and due to the changes caused by tobacco in the microbial flora of the oral cavity.

Los elementos utilizados en este procedimiento se encuentran en contacto directo con estructuras dentofaciales, causando diversos tipos de lesiones, principalmente bucales. Aunque existen cuidados clínicos durante procesos de intubación, hay poca información, particularmente de la zona norte del país donde se hayan evaluado las posibles asociaciones entre los motivos de consulta más frecuentes y las diversas características, tanto clínicas como no clínicas de pacientes que han sido intubados. Objetivo: Identificar las alteraciones bucodentales más frecuentes en pacientes intubados, así como explorar las posibles asociaciones de acuerdo con los motivos de intubación más frecuentes. Material y métodos: Se realizó un estudio observacional, transversal y comparativo en el cual se evaluaron alteraciones bucodentales y sistémicas de pacientes intubados. Los grupos de estudio se desarrollaron de acuerdo con el motivo de intubación y la determinación de las asociaciones fue con cada una de las alteraciones bucodentales y sistémicas. Resultados: El motivo de intubación más frecuente fue el evento cerebral vascular (EVC) y las alteraciones dentofaciales más prevalentes fueron caries, lengua saburral y cálculo dental, entre otras. Además, se encontraron diferencias significativas entre pacientes con EVC, mostrando una mayor frecuencia en tabaquismo, hipertensión arterial y diabetes mellitus, así como en la presencia de gingivitis y úlceras. Pacientes con traumatismo craneoencefálico (TCE) tuvieron mayor frecuencia en la presencia de periodontitis. Conclusión: El motivo de hospitalización y las condiciones sistémicas preexistentes pueden ser un factor de riesgo para desarrollar lesiones bucales específicas antes y durante el periodo de intubación (AU)


Introduction: Intubation is a procedure that allows ventilatory assistance when the airway and consciousness are compromised. The elements used in this procedure are in direct contact with dentofacial structures causing various types of injuries, mainly oral. Although there is clinical care during intubation processes, there is little information, particularly from the northern part of the country where the possible associations between the most frequent reasons for consultation and the various clinical and non-clinical characteristics of patients who have been intubated have been evaluated. Objective: The objectives of the present study were to identify the most frequent oral alterations in intubated patients, as well as to explore possible associations according to the most frequent reasons for intubation. Material and methods: An observational, cross-sectional and comparative study was carried out in which oral and systemic alterations of intubated patients were evaluated. The study groups were formed according to the reason for intubation and the association was determined with each of the oral and systemic disorders. Results: The most frequent reason for intubation was the vascular cerebral event (CVA) and the most prevalent dentofacial alterations were caries, coated tongue, and dental calculus, among others. In addition, significant differences were found between patients with CVA, showing a higher frequency in smoking, hypertension, and diabetes mellitus, as well as in the presence of gingivitis and ulcers. Patients with traumatic brain injury (TBI) had a higher frequency in the presence of periodontitis. Conclusion: The reason for hospitalization and pre-existing systemic conditions can be a risk factor for developing specific oral lesions before and during the intubation period (AU)

Introducción: La enfermedad periodontal es un padecimiento inflamatorio, infeccioso y multifactorial crónico, caracterizado por la inflamación de los tejidos blandos periodontales. En estadios avanzados (periodontitis), produce la destrucción progresiva de los tejidos duros periodontales, lo que conduce a la posterior pérdida de dientes, si esta no es tratada. Objetivo: Determinar la efectividad clínica y radiográfica de las estatinas en el tratamiento de la periodontitis. Métodos: Se realizó una búsqueda de la literatura hasta abril del 2019, en las bases de datos biomé dicas: PubMed, Embase, SciELO, Science Direct, Scopus, Sistema de información sobre literatura gris en Europa, Literatura Latinoamericana y del Caribe en Ciencias de la Salud, Google Académico y el Registro Central de Ensayos Clínicos Cochrane. Se definieron como criterios de selección de los estudios que fueran ensayos clínicos aleatorizados, con una antigüedad máxima de cinco años y que reportaran los efectos clínicos y radiográficos (profundidad al sondaje, nivel de inserción clínica, índice de placa, índice de sangrado, índice gingival, defecto intraóseo y profundidad del defecto) de las estatinas en el tratamiento de la periodontitis. Se analizó el riesgo de sesgo de los estudios por el Manual Cochrane de revisiones sistemáticas de intervenciones. Resultados: La estrategia de búsqueda arrojó 19 artículos, de los cuales el 100 por ciento reportó que había diferencia en la profundidad al sondaje, nivel de inserción clínica, índice de placa, índice de sangrado, índice gingival, defecto intraóseo y profundidad del defecto de las estatinas en el tratamiento de la periodontitis. Conclusiones: La literatura revisada sugiere que el uso de estatinas es efectivo, clínica y radiográficamente, en el tratamiento de la periodontitis(AU)


Introduction: Periodontal disease is a chronic multifactorial infectious inflammatory condition characterized by inflammation of soft periodontal tissue. In advanced stages (periodontitis) it causes progressive destruction of hard periodontal tissue, leading to eventual tooth loss if not treated. Objective: Determine the clinical and radiographic effectiveness of statins in the treatment of periodontitis. Methods: A search was carried out in the literature published until April 2019 in the biomedical databases PubMed, Embase, SciELO, Science Direct, Scopus, System for Information on Gray Literature in Europe, Latin American and Caribbean Health Sciences Literature, Google Scholar, and Cochrane Central Register of Clinical Trials. The following selection criteria were defined for the studies: randomized clinical trials published in the last five years and reporting on clinical and radiographic effects (probing depth, clinical insertion level, plaque index, bleeding index, gingival index, intraosseous defect and defect depth) of statins in the treatment of periodontitis. Bias risk analysis was based on the Cochrane manual of systematic reviews of interventions. Results: A total 19 papers were retrieved, of which 100 percent reported differences in the probing depth, clinical insertion level, plaque index, bleeding index, gingival index, intraosseous defect and defect depth of statins in the treatment of periodontitis. Conclusions: The literature review conducted suggests that the use of statins is clinically and radiographically effective in the treatment of periodontitis(AU)

Periodontal disease (PD), as an age-related disease, prevalent in middle-aged and elderly population, is characterized as inflammatory periodontal tissue loss, including gingival inflammation and alveolar bone resorption. However, the definite mechanism of aging-related inflammation in PD pathology needs further investigation. Our study is aimed at exploring the effect of inflamm-aging-related cytokines of interleukin-17 (IL-17) and interferon-γ (IFN-γ) on osteoclastogenesis in vitro and periodontal destruction in vivo. For receptor activator of nuclear factor-κB ligand- (RANKL-) primed bone marrow macrophages (BMMs), IL-17 and IFN-γ enhanced osteoclastogenesis, with the expression of osteoclastogenic mRNA (TRAP, c-Fos, MMP-9, Ctsk, and NFATc1) and protein (c-Fos and MMP-9) upregulated. Ligament-induced rat models were established to investigate the role of IL-17 and IFN-γ on experimental periodontitis. Both IL-17 and IFN-γ could enhance the local inflammation in gingival tissues. Although there might be an antagonistic interaction between IL-17 and IFN-γ, IL-17 and IFN-γ could facilitate alveolar bone loss and osteoclast differentiation.

Porphyromonas gingivalis, a bacterium associated with periodontal disease, is a suspected cause of Alzheimer's disease. This bacterium is reliant on gingipain proteases, which cleave host proteins after arginine and lysine residues. To characterize gingipain susceptibility, we performed enrichment analyses of arginine and lysine proportion proteome-wide. Genes differentially expressed in brain samples with detected P. gingivalis reads were also examined. Genes from these analyses were tested for functional enrichment and specific neuroanatomical expression patterns. Proteins in the SRP-dependent cotranslational protein targeting to membrane pathway were enriched for these residues and previously associated with periodontal and Alzheimer's disease. These ribosomal genes are up-regulated in prefrontal cortex samples with detected P. gingivalis sequences. Other differentially expressed genes have been previously associated with dementia (ITM2B, MAPT, ZNF267, and DHX37). For an anatomical perspective, we characterized the expression of the P. gingivalis associated genes in the mouse and human brain. This analysis highlighted the hypothalamus, cholinergic neurons, and the basal forebrain. Our results suggest markers of neural P. gingivalis infection and link the cholinergic and gingipain hypotheses of Alzheimer's disease.

BACKGROUND: Combustible cigarettes have detrimental effects on periodontal disease. However, little evidence is available regarding new heated tobacco product (HTP) use and combined product use (both combustible cigarettes and HTPs). This study aimed to examine the association of combustible cigarettes, HTPs, and combined product use with periodontal disease simultaneously. MATERIALS AND METHODS: This cross-sectional study was conducted using data from the 2019 arm of the longitudinal Japan "Society and New Tobacco" Internet Survey. Combustible cigarette users, HTP users, combined product users, never-users, and former users' data were separately obtained. In the present study, the primary outcome was self-reported periodontal disease. We estimated adjusted prevalence ratios (PRs) and confidence intervals (CIs) using multivariable modified Poisson regression analysis after adjusting for 12 confounders. RESULTS: Of the 10,439 JASTIS respondents, the numbers of users of combustible cigarettes only, HTPs only, and both products were 1,304, 437, and 1,049, respectively. Compared with never-users, HTP use was significantly associated with the prevalence of self-reported periodontal diseases (PR 1.43, 95% CI 1.03-1.62). Moreover, former users, combustible cigarette users, and combined product users also showed significant associations (PR 1.56, 95% CI 1.35-1.80; PR 1.29, 95% CI 1.03-1.62; and PR 1.55, 95% CI 1.20-1.99, respectively). CONCLUSIONS: Users of HTPs, combustible cigarettes, and combined products as well as former users were all significantly associated with a higher prevalence of periodontal diseases compared to never-users.
