RESUMO

El diagnóstico molecular, mediante la aplicación de técnicas molecu- lares, ha permitido estudiar microorganismos presentes en el inicio y progresión de la caries dental, enfermedad periodontal, y en los fracasos endodónticos. Las técnicas moleculares permiten la detección y cuan- tificación del material genético del ácido desoxirribonucleico (DNA), ácido ribonucleico (RNA) o proteínas, lo que posibilita el estudio del genoma completo o secuencias de DNA específicas. Estas técnicas surgen como una necesidad de detectar microorganismos de difícil o lento crecimiento en cultivos; la técnica más utilizada es la reacción en cadena de la polimerasa (PCR) que permite la amplificación de peque- ños segmentos de material genético al utilizar cebadores, por lo que es un método económico, preciso, sensible y rápido para la detección de microorganismos. El presente artículo de revisión bibliográfica servirá para informar sobre los avances de las técnicas moleculares utilizadas para el diagnóstico en la práctica odontológica (AU)


Molecular diagnosis, through the application of molecular techniques, has made it possible to study microorganisms present in the onset and progression of dental caries, periodontal disease, and endodontic failures. Molecular techniques allow the detection and quantification of the genetic material of deoxyribonucleic acid (DNA), ribonucleic acid (RNA) or proteins, allowing the study of the complete genome or specific DNA sequences, they arise as a need to detect difficult or slow growing microorganisms in cultures. The most widely used technique is the polymerase chain reaction (PCR) that allows the amplification of small segments of genetic material using primers, it is an economical, precise, sensitive and fast method for the detection of microorganisms. La microendodoncia involucra la visualización a través de un microscopio operatorio de todas las fases del tratamiento de conductos y procedimientos de cirugía apical y correctiva por parte del endodoncista. Existe sobrada evidencia acerca de las mejoras que puede aportar la magnificación al tratamiento; la literatura demuestra que la capacidad del operador mejora si su visión del campo gana claridad y precisión, ambos recursos pueden ser proporcionados por el microscopio operatorio, aunado a que posibilita diagnósticos más certeros junto con mejoras en el pronóstico, lo que permite evitar posibles complicaciones. La calidad de los tratamientos endodóncicos involucra infinidad de factores, cada uno relevante en sí mismo pero, en determinados casos, el microscopio puede significar la diferencia entre un tratamiento exitoso o un fracaso clínico. En la actualidad, se ha convertido en un tema de lo más relevante, por lo que el objetivo del presente trabajo es revisar la literatura con el fin de ayudar al entendimiento basado en evidencia científica de los criterios que determinan la relevancia del uso del microscopio en el ámbito endodóncico (AU)


Microendodontics involves the visualization through an operating microscope of all phases of root canal treatment and apical and corrective surgery procedures by the endodontist. There is plenty of evidence about the improvements that magnification can provide, the literature shows that the operator's ability improves if his vision of the field gains clarity and precision, both resources can be provided by the operating microscope, added to the fact that it enables more accurate diagnoses together with improvements in the prognosis allowing to avoid possible complications. The quality of endodontic treatments involves countless factors, each relevant in itself, but in certain cases the microscope can mean the difference between a successful treatment or a clinical failure. At present, it has become a very relevant topic, so the objective of this work is to review the literature in order to help understand the criteria that determine the relevance of the use of the microscope in the endodontic field based on scientific evidence (AU)

Introducción: el empleo de software educativo es una alternativa motivadora en los diferentes escenarios docentes. Objetivo: mostrar la validación del software educativo sobre diagnóstico y tratamiento de enfermedades pulpares y periapicales para su generalización en estudiantes de Estomatología. Material y Métodos: se realizó un estudio de innovación tecnológica o producto terminado digital en la etapa comprendida desde marzo de 2018 a marzo de 2019. Se utilizó la herramienta Mediator v9.0. Se utilizaron métodos teóricos: histórico-lógico, analítico-sintético y el sistémico-estructural; y empíricos: la revisión documental y una encuesta a estudiantes y expertos en función de evaluadores externos del producto. Para evaluar la factibilidad del software se utilizó el método de Criterio de Experto y el Criterio de usuario. Las principales variables evaluadas fueron el diseño de las secciones del software, su funcionalidad, cientificidad del contenido, relación de las imágenes con el objetivo del software, así como su uso en la docencia. Resultados: el producto obtenido consta de un texto básico con hipervínculos para acceder a los demás componentes, así como galerías de imágenes y ejercicios de autoevaluación. Según Criterio de Experto la calidad técnica del producto y la calidad del contenido quedaron evaluadas en la categoría Muy Adecuado. Según Criterio de Usuario, la funcionalidad, la motivación individual y los aspectos de diseño fueron evaluados de Bien. Conclusiones: se desarrolló un software educativo que cuenta con recursos variados para el aprendizaje sobre el tema abordado(AU)


Introduction: The use of the educational software is a motivating alternative in different teaching scenarios. Objective: To validate the educational software related to the diagnosis and treatment of pulp and periapical diseases for its generalization in dental students. Material and Methods: A study linked with technological innovation or digital finished product was conducted during the period between March 2018 and March 2019. The tool used was Mediator v9.0. Theoretical methods such as historical-logical, analytical-synthetic, inductive-deductive, and systemic-structural were used; empirical methods, which included the documentary review and a survey, were applied to students and experts as external evaluators of the product. The methods of Expert Judgment and Customer Judgment were used to evaluate the feasibility of the software. The main variables evaluated were the design of software sections, its functionality, scientific content, relationship between the images and the objective of the software and its use in the teaching-learning process. Results: The product obtained is a basic text with hyperlinks to access the rest of the components, image galleries and self-evaluation exercises. According to Expert Judgment, the technical quality of the software and the quality of its content were evaluated as good. According to Customer Judgment, the functionality, self-motivation and design aspects were evaluated as optimal. Conclusions: An educational software with various resources for learning about the topic approached was developed(AU)

Las alteraciones pulpares y periapicales, son un conjunto de patologías que afectan a los tejidos pulpar y periapical del diente; constituyen una de las causas por las cuales el paciente acude a consulta. Objetivos: Determinar la frecuencia de enfermedades pulpares y periapicales, de acuerdo al sexo, así como el órgano dentario más afectado; en los pacientes atendidos en el Consultorio Odontológico del Hospital Básico de Catacocha, durante el periodo enero - diciembre 2013. Material y Métodos: Estudio documental, comparativo. La población estuvo constituida por los pacientes que acudieron al servicio de Especialidad de Endodoncia del Hospital. La muestra fue de 330 pacientes, que presentaron enfermedad pulpar, se consideró el primer y segundo diagnósticos. Se procedió a revisión de las historias clínicas del Hospital, partes diarios y sistema CARDEX del Ministerio de Salud Pública del Ecuador para corroborar los datos; previo el respectivo permiso de las autoridades del hospital. Se utilizó la prueba estadística chi cuadrado. Resultados: Se revisaron 330 historias clínicas. Los valores más altos, fueron: Pulpitis 40% (n=90) sexo femenino, pulpitis 40% (n=41) sexo masculino, como primer diagnóstico, y como segundo diagnóstico; Periodontitis apical crónica 34% (n=12) sexo femenino, periodontitis apical crónica 44% (n=7) sexo masculino. El órgano dentario más afectado resultó el primer molar inferior izquierdo 3.6, en el primer diagnóstico; y el diente No. 2.1 en el segundo diagnóstico. Conclusiones: Los tipos de diagnósticos no tienen diferencias significativas, entre varones y mujeres. (AU)


Pulp and periapical alterations are a set of pathologies which affect the pulp and periapical tissues of the tooth. This is one of the causes why patients go to the dentist's office. Objectives: To determine the frequency of pulp and periapical diseases in relation with sex as well as the most affected dental organ in the patients of the Dentist's Office at Basic Hospital in Catacocha during the period JanuaryDecember 2013.Material and Methods: Comparative documentary study. The population was the patients who attended the Hospital Endodontics Specialty service. The sample was 330 patients, who presented the pulp disease. The first and second diagnoses were taken under consideration. In order to confirm the data collected, the clinical histories of the Hospital, the daily reports and the CARDEX system from the Ministry of Public Health of Ecuador, with the previous permission of the Hospital, were compared. The statistic test used was square chi. Results: 330 were taken under consideration. The highest values were: Pulpitis 40% (n=90) feminine sex, Pulpitis 40% (n=41) masculine sex, both at first and second diagnosis; chronic apical Periodontitis 34% (n=12) feminine sex, chronic apical periodontitis 44% (n=7) masculine sex. The most affected dental organ was the first lower left molar 3.6, in the first diagnosis; and tooth number 2.1 in the second diagnosis. Conclusions: The types of diagnosis do not present meaningful differences between men and women. (AU)

Lesões endo-perio são lesões inflamatórias que acometem, em diversos graus, tanto os tecidos periodontais como a polpa dental. O objetivo deste artigo é mostrar, através de uma revisão da literatura, a importância da classificação das lesões endo-perio, assim como o diagnóstico dessas lesões. A partir dos trabalhos revisados, conclui-se que as lesões endo-perio apresentam uma etiologia variada, sendo de fundamental importância o conhecimento do profissional quanto às causas e seu correto diagnóstico. Assim, para o sucesso e a resolução das lesões endo-perio, uma avaliação clínica e radiográfica eficaz determinará a correta classificação e, consequentemente, a melhor forma de tratamento.


Endo-periodontal lesions are inflammatory conditions that affect the periodontal tissues and the dental pulp. The aim of this article is to show the importance of the classification and the diagnosis of the endo-perio lesions. From the studies reviewed, it is concluded that endo-perio lesions present a varied etiology, being of fundamental importance the knowledge of the professional as to its causative factors and its correct diagnosis. Thus, for the success and resolution of endo-perio lesions an effective clinical and radiographic evaluation will determine the correct classification and consequently the best form of treatment.

Aim: To determine the prevalence of pulp and periradicular diseases in a Brazilian subpopulation, correlating the prevalence with sex, age and most affected teeth. Methods: Data collected from medical records of patients treated at the Clinic of Specialization in Endodontics of the Federal University of Pernambuco between 2003 and 2010. The following variables were recorded for each patient: sex, age, affected teeth and diagnosed endodontic disease. Using Pearson's chi-square and Fisher's exact tests, the collected data analysis was set at a 5% significance level. Results: From all the treated teeth, 57% were diagnosed with pulp diseases, with the symptomatic irreversible pulpitis being the most prevalent (46.3%), while among the diagnosed periradicular diseases (43%), chronic apical periodontitis was the most prevalent (81%). Pulp diseases were detected in men and women in an unequal mode (p=0.008). Subjects under 40 years old had higher prevalence of pulp disease (p=0.286), and patients over 50 years were most affected by periradicular diseases (p=0.439). Maxillary incisors and mandibular first molars were the most affected teeth by endodontic diseases. Conclusions: In the evaluated subpopulation, the endodontic diseases were more prevalente in the maxillary incisive, affected indiscriminately individuals of different age groups and chronic apical periodontitis was the most prevalent diagnosed disease (Au)

La práctica endodóntica requiere el respaldo de imágenes de manera permanente, tanto para el diagnóstico como durante las distintas etapas del tratamiento. Desde siempre, estas imágenes han sido radiográficas. Con el advenimiento de las tomografías, se incluyó la visión de la tercera dimensión y se logró despejar interferencias gracias a la incorporación de cortes sectorizados. Sin embargo, estos estudios implican el uso de radiaciones ionizantes y de aparatología de alto costo. Con el tiempo, los estudios tomográficos han evolucionado; permiten obtener imágenes más fieles, isométricas, con mejor definición ymenos dosis de radiación para los pacientes. El presente trabajo tiene como objetivo brindar información clara a los colegas que deseen incorporar estas nuevas tecnologías en su práctica cotidiana. INTRODUCTION: This study aimed to analyze cases referred from a reference service in oral pathology that were initially misdiagnosed as periapical lesions of endodontic origin and to perform a review of the literature regarding lesions located in the apical area of teeth with a nonendodontic source. METHODS: A survey was made of clinical cases derived from the service of oral pathology from 2002 to 2012. The pertinent literature was also reviewed using ScienceDirect and PubMed databases. The lesions were grouped into benign lesions mimicking endodontic periapical lesions (BLMEPLs), malignant lesions mimicking endodontic periapical lesions (MLMEPLs), and Stafne bone cavities. The clinical presentations were divided into lesions with swelling without pain, lesions with swelling and pain, and lesions without swelling but presenting with pain. RESULTS: The results showed that 66% (37/56) of cases represented benign lesions, 29% (16/56) malignant lesions, and 5% (3/56) Stafne bone cavities. The most commonly reported BLMEPLs were ameloblastomas (21%) followed by nasopalatine duct cysts (13.5%). The most frequently cited MLMEPLs were metastatic injuries (31.5%) followed by carcinomas (25%). The main clinical presentation of BLMEPLs was pain, whereas that of MLMEPLs was swelling associated with pain; Stafne bone cavities displayed particular clinical findings. CONCLUSIONS: Clinical and radiologic aspects as well as the analysis of the patients' medical history, pulp vitality tests, and aspiration are essential tools for developing a correct diagnosis of periapical lesions of endodontic origin. However, if the instruments mentioned earlier indicate a lesion of nonendodontic origin, a biopsy and subsequent histopathological analysis are mandatory.

OBJECTIVE: The aim was to review the bactericidal efficacy of photodynamic therapy (PDT) against Enterococcus faecalis (E. faecalis) in infected root canals. METHODS: To address the focused question "Does PDT exhibit bactericidal effects against E. faecalis in infected root canals?" PubMed/Medline and Google-Scholar databases were searched from 1985 up to August 2013 using various combinations of the following key words: "antibacterial; "bactericidal; "endodontic; "root canal" and "photodynamic therapy". Original studies, experimental studies and articles published only in English language were included. Letters to the editor, historic reviews and unpublished data were excluded. The pattern of the present review was customized to primarily summarize the pertinent information. RESULTS: Seventeen studies (16 ex vivo and one in vivo) were included. In these studies, numbers of teeth used ranged between 30 and 220 teeth. In these studies, wavelengths of diode laser used, diameter of fiber and power output ranged between 625 and 805 nm, 200 µm and 0.4 cm, and 40 mW and 5 W respectively. Twelve studies reported PDT to be effective in eliminating E. faecalis from infected root canals. Four studies reported conventional irrigation and instrumentation to be more efficient in killing E. faecalis than PDT. One study reported PDT and conventional endodontic regimes to be equally effective in eliminating E. faecalis from infected root canals. In most studies, toluidine blue and/or methylene blue were used as photosensitizers. CONCLUSION: Efficacy of PDT in eliminating E. faecalis from infected root canals remains questionable.

Atypical odontalgia (AO) is a little known chronic pain condition. It usually presents as pain in a site where a tooth was endodontically treated or extracted, in the absence of clinical or radiographic evidence of tooth pathology. It is a rare clinical challenge for most clinicians, which leads to the patients being referred to several specialists and sometimes undergoing unnecessary surgical procedures. The pain mechanisms involved in AO are far from clear, and numerous potential mechanisms have been suggested. Currently, the most accredited hypothesis is that AO is a neuropathic pain condition caused by deafferentation. The differential diagnosis of AO remains difficult, because it shares symptoms with many others pathologies affecting this area. Patients have difficulties accepting the AO diagnosis and treatment. As a result, they frequently change physicians, and may potentially also receive several invasive treatments, usually resulting in an aggravation of the pain. Although some patients do get complete pain relief following treatment, for most patients the goal should be to achieve adequate pain management. Currently, most management is based on expert opinion and case reports. More research and high quality randomized controlled trials are needed in order to develop evidence-based treatments, currently based on expert opinion or carried over from other neuropathic pain conditions in the orofacial region.

Varias son las enfermedades que llevan a los pacientes a acudir a la consulta de urgencias estomatológicas, la gran mayoría corresponde a afecciones pulpares debido a la sintomatología dolorosa que las caracteriza. El objetivo de este trabajo describir el comportamiento de las enfermedades pulpares en pacientes del área de salud de la clínica estomatológica 27 de Noviembre en el período comprendido entre los meses de octubre 2008 a diciembre 2009, se realizó un estudio analítico de corte transversal en el cual se incluyeron 173 pacientes adultos entre 19 y 59 años que acudieron al servicio con diagnóstico de cualquiera de las enfermedades pulpares agudas. Del total de casos diagnosticados el 81 para un 46,8 por ciento correspondió a la pulpitis aguda irreversible, el 34,7 por ciento a pulpitis transitoria y el 18,5 por ciento a hiperemia pulpar. El grupo dentario más afectado fue el de los molares y el tipo de lesión pulpar resultó independiente del grupo dentario al igual que resultó independiente del factor causal, entre los cuales aparece con un alto predominio la caries dental con un 65,9 por ciento; el dolor tanto en tipo como en intensidad dependen del tipo de lesión que tenga la pulpa según los resultados del estadígrafo X² de Pearson obtenidos en el estudio, donde el dolor espontáneo se presentó solamente para los pacientes con pulpitis aguda irreversible, en los que ningún caso se reportó el dolor con categoría leve(AU)


There is a range of diseases that make the patients go to the dental emergency service; most of them result from pulp disorders due to the painful symptoms that characterize them. The objective of this paper was to describe the behavior of pulp diseases in patients from the health area of 27 de Noviembre dental clinic in the period of October 2008 through December 2009. To this end, a cross-sectional analytical study was conducted in 173 adult patients aged 19 to 59 years and diagnosed as having any of the acute pulp diseases, Out of the total number of diagnosed cases, 81 (46.8 percent) had irreversible acute pulpitis, 34.7 percent suffered transient pulpitis and 18.5 percent presented pulp hyperemia. The most affected teeth were molars and the type of pulp lesion did not depend either on the dental group or on the causative factor, being the dental caries highly predominant, since it accounted for 65.9 percent. Pain, in terms of type and intensity, depends on the type of pulp lesion according to Pearson's statistic X² results achieved in the study. Spontaneous pain, which was not mild in any case, was just present in patients with irreversible acute pulpitis(AU)

El objetivo de este estudio fue comprender el efecto de la infección periapical asociada a un modelo de estrés sobre los parámetros de evaluación del sistema nervioso central. Para este experimento se seleccionaron 20 ratones machos de la especie Rattus Novergicus en dos grupos, respectivamente, de estrés y apertura (GEA) y apertura (GA). Los procedimientos de inducción de la enfermedad endodóntica se realizaron por medio de una apertura coronaria del 1º molar inferior derecho. Después de la institución del modelo de estrés fue realizada la evaluación motora y psicológica de los animales por medio de pruebas de comportamiento del tipo laberinto elevado en forma de cruz y campo abierto. En relación a la evaluación en el campo abierto no existieron diferencias estadísticas (p>0,05) entre los grupos. Para la evaluación del laberinto en cruz, fueron observadas dos variables. En relación al tiempo de permanencia en el brazo cerrado el GA demostró un mayor tiempo de permanencia comparado al GEA (p<0,05). En relación al brazo abierto, el GEA tuvo un tiempo mayor de permanencia en la región en comparación al GA, inclusive con diferentes estadísticas (p<0,05). Dentro de las limitaciones del estudio, se puede notar que el estrés asociado a la inflamación de los tejidos periapicales llevaron a la modificación del comportamiento del animal.


The aim of this study was to understand the effect of periapical infection associated with a model of stress on the parameters of the central nervous system evaluation. For this experiment we selected 20 male rats divided into two groups: stress and opening group (SOG) and opening group (OG). In the first group, rats were tested for induction of stress associated with endodontic disease through coronary opening of the first lower right molar, the second group, the same procedure was carried out but without the stress partner. After 30 days of the imposition of the stress model was evaluated for motor and mental development of animals through field testing of high-maze behavior in a cross and open. Regarding the open-field assessment there was no statistically significant differences (p> 0.05) for the evaluation of the maze, there are two variables. In terms of time spent in closed arm OG showed a longer hospital stay compared to SOG (p <0.05). Regarding the open design industry, the SOG had a longer hospital stay in the region against the OG, including statistically significant differences (p <0.05). It was seen that the stress associated with inflammation of periapical tissues led to the modification of animal behavior.

INTRODUCTION: Dental infections might predispose toward the onset of cardiovascular disease (CVD). To date, only a few studies, yielding inconclusive findings, have investigated the potential correlation between apical periodontitis (AP) and CVD. The aim of this study (as the first part of a prospective study) was to evaluate, in the absence of CV risk factors, whether subjects with AP were more exposed to the pathogenetic indices of an atherosclerotic lesion. METHODS: Forty men between the ages of 20 and 40 years who were free from periodontal disease, CVD, and traditional CV risk factors were enrolled in the study; 20 subjects had AP, and 20 acted as controls. All subjects underwent dental examination and complete cardiac assessment: physical examination, electrocardiogram, conventional and tissue Doppler echocardiography, and measurement of endothelial flow reserve (EFR). The following laboratory parameters were tested: interleukins -1, -2, and -6 (IL-1, IL-2, IL-6), tumor necrosis factor alpha, and asymmetrical dimethylarginine (ADMA). Data were analyzed by using the 2-tailed Student's t test, Pearson t test (or Spearman t test for nonparametric variables), and multivariate linear regression analysis. RESULTS: Echocardiography revealed no abnormalities in any of the subjects studied. ADMA levels were inversely correlated with EFR (P < .05) and directly correlated with IL-2 (P < .001). Patients with AP presented with significantly greater blood concentrations of IL-1 (P < .05), IL-2 (P < .01), IL-6 (P < .05), and ADMA (P < .05) and a significant reduction of EFR (P < .05). CONCLUSIONS: Increased ADMA levels and their relationship with poor EFR and increased IL-2 might suggest the existence of an early endothelial dysfunction in young adults with AP.

RESUMO

This article addresses technologic advances in endodontics pertaining to new and emerging technology. Cone-beam computed tomography and optical occurrence tomography are 2 new imaging technologies that can assist the practitioner in the diagnosis of pulpal disease. The self-adjusting file and the Apexum device can be used for instrumentation and bulk debridement of an apical lesion, respectively. Neodymium:yttrium-aluminum-garnet laser, erbium:chromium:yttrium-scandium-gallium-garnet laser, EndoActivator, EndoVac, and light-activated disinfection may assist the practitioner in cleaning the root canal system. Computed tomography-guided surgery shows promise in making endodontic surgery easier, as does mineral trioxide aggregate cement for regenerative endodontic procedures.

RESUMO

Se presenta un caso clínico de una lesión endoperiodontal tipo III (combinada o verdadera) en un paciente de sexo femenino de 41 años de edad sin antecedentes sistémicos. La paciente fue derivada del Curso de Especialización en Periodoncia de la Escuela de Graduados de la Universidad de Chile debido a una lesión periapical en la pieza 3.6. El pronóstico en este tipo de lesiones es dudoso, ya que es necesario que se efectúe el tratamiento endodóntico así como el periodontal, y el resultado recae más en el tratamiento periodontal. La pieza fue tratada endodónticamente dejando medicamento intraconducto a base de Clorhexidina al 2 por ciento en gel por 7 días. Una vez obturado el diente se citó a la paciente 3 meses después para un control radiográfico. Actualmente el diente está totalmente asintomático, sin movilidad y con señales de reparación.


We report a case of a type III periodontal-endodontic lesion (combined or true) in a 41-year-old female patient without systemic history. The patient was transferred from the Specialization Course in Periodontology at the Graduate School of the University of Chile due to a periapical lesion in the tooth 3.6. The prognosis for this type of lesion is uncertain, since it is necessary to perform endodontic and periodontal treatment, and the result depends more on the periodontal treatment. The piece was treated endodontically leaving intracanal medication based on 2 percent Chlorhexidine gel for 7 days. Once the tooth obturated, we gave the patient an appointment 3 months later for a control radiography. Currently, the tooth is completely asymptomatic, without mobility and with signs of repair.
