RESUMO

BACKGROUND: Sexual difficulties and vaginal pain are common following treatment for breast cancer. AIM: The goal of this study was to evaluate an online mindfulness-based group sex therapy vs an online supportive sex education group therapy to address these sexual difficulties. METHODS: Breast cancer survivors (n = 118) were randomized to 1 of the 2 arms; 116 provided informed consent and completed the time 1 assessment. Treatment included 8 weekly 2-hour online group sessions. Those randomized to the mindfulness group completed daily mindfulness exercises, and those in the comparison arm read and completed exercises pertaining to sex education. OUTCOMES: Assessments were repeated at posttreatment and 6 months after the completion of the group. RESULTS: There was a main effect of treatment on primary endpoints of sexual desire, sexual distress, and vaginal pain, with all outcomes showing significant improvements, with no differential impact by treatment arm. Secondary endpoints of interoceptive awareness, mindfulness, and rumination about sex also significantly improved with both treatments, with no group-by-time interaction. CONCLUSION: Both mindfulness-based sex therapy and supportive sex education delivered in group format online are effective for improving many facets of sexual function, vaginal pain, rumination, mindfulness, and interoceptive awareness in breast cancer survivors. STRENGTHS AND LIMITATIONS: We used a randomized methodology. Future studies should seek to diversify participants. CLINICAL IMPLICATIONS: These findings highlight the need to offer similar treatments to more breast cancer survivors immediately after and in the years following cancer treatment as a means of improving survivorship quality of life.

RESUMO

BACKGROUND: Unconsummated marriage (UCM) is the inability of the heterosexual married couple to have penovaginal sexual intercourse. AIM: The study sought to systematically review current evidence regarding the etiological factors and clinical management of UCM. METHODS: A comprehensive bibliographic search on the MEDLINE, Scopus, Web of Science, and Cochrane Library databases was performed in June 2023. Studies were selected if they described married couples who never had sexual intercourse in case report or case series evaluating the related causes and/or management and reporting data with qualitative, quantitative, or mixed methods. The review was reported according to PRISMA (Preferred Reporting Items for Systematic Review and Meta-analyses) statement and registered in PROSPERO with ID CRD42023433040. RESULTS: A total of 27 studies including 1638 males and 1587 females were selected. Eight (29.6%) articles were case reports involving a single couple and 19 (70.4%) studies were case series. Mean Murad score was 4.1 (range, 1-8) showing low-intermediate overall study quality. All articles had a level of evidence of 4. Most of studies were conducted in Egypt (n = 5 [18.4%]), Israel (n = 4 [14.9%]), and the United States (n = 4 [14.9%]). The mean age of males and females varied between 24.2 and 37.6 years and from 21 to 27.4 years, respectively. The reasons for the medical visit that led to the diagnosis of UCM were inability to consummate in 23 (85.2%) studies, inability to conceive in 1 (3.7%) article, and mixed in 3 (11.1%) articles. The mean duration of UCM varied from 7 days to 3.5 years. Eight studies involving both men and women showed that vaginismus (8.4%-81%) and erectile dysfunction (10.5%-61%) were the most common causes of UCM. Three articles reported that 16.6% to 26% of all UCM cases were due to both male and female factors. Sildenafil, tadalafil, intracavernosal injection, penile plication, female genital reconstructive surgery, vaginal dilators, lubricants, psychosexual therapy, and sex education were the various treatment modalities in 27 studies to achieve consummation rate of 66.6% to 100%. STRENGTHS & LIMITATIONS: A strength is that this is the first systematic review covering the entire spectrum of UCM. Limitations comprised the low quality of most of the included articles and the large percentage of UCM cases probably not published. CONCLUSION: Erectile dysfunction and vaginismus are the most reported causes of UCM; however, a strong psychological component certainly underlies a significant number of cases. A multidisciplinary approach based on strategic integration of sex education, medical therapy, psychosexual support, and surgical treatment would seem the most suitable option to manage couples with UCM.

(AU)

The objective of this article is to map the academic production about interventions in the Brazilian school scene related to sexuality and gender issues. A Scoping Review was performed using search parameters in Scielo, Lilacs, Pepsic e Google Acadêmico databases. Based on this search, we found that the number of published research on the topic has increased from the year 2012; most interventions are targeted at students, especially those in the final years of elementary and high school; the most used methodological delineations are the qualitative ones; critical discourse grounds most of the publications found, followed by liberal and postmodern discourses. It is emphasized the need for investigations from the children's age group, targeting teachers and educators. In addition, studies in the areas of psychology and education should be strengthened to promote the guarantee of rights and quality of life for all people. (AU)

RESUMO

BACKGROUND: Closing the gap of unmet for family planning is crucial to eliminate new pediatric HIV infections likewise to improve maternal and child health among reproductive-age women living with HIV. However, studies conducted on unmet need for family planning among reproductive-age women living with HIV showed inconsistent and non-conclusive findings on the magnitude of the problem. Moreover, there was no meta-analysis conducted in this area. So this systematic review and meta-analysis were conducted to estimate the pooled prevalence unmet need for family planning among reproductive-age women living with HIV in Ethiopia. METHODS: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline was followed to review both published and unpublished studies in Ethiopia. All studies in PubMed, Cochrane Library, Hinari, Google Scholar, CINAHL, and Global Health databases were searched. Meta-analysis was performed using STATA 14 software. The heterogeneity and publication bias were assessed using the I2 statistics and Egger regression asymmetry test, respectively. Forest plots were used to present the pooled prevalence with a 95% confidence interval (CI). RESULTS: This review included 7 studies, and 3333 study participants. The pooled prevalence of unmet need for family planning among reproductive-age women living with HIV in Ethiopia was 25.13% (95%CI: 19.97, 30.29). The pooled prevalence of unmet need for spacing and limiting was 13.91% (95%CI: 10.11, 17.72) and 9.11% (95%CI: 6.43, 11.78), respectively. CONCLUSIONS: One-fourths of reproductive-age women living with HIV had an unmet need for family planning. A variety of programmatic investments are needed to achieve more meaningful progress toward the reduction of unmet need for family planning among reproductive-age women living with HIV.

This text emerges through the conceptual reflections of the Doctorate in Education at Universidad Pontificia Bolivariana, in Medellín, Colombia, and the Doctoral Internship at University of Murcia with the research group "Compartimos Educación" with the aim of characterizing the place of family schools in the promotion of affective-sexual education of boys, girls and adolescents at school. The used methodology is qualitative. As a result, the need to link actors that mainstream the work of the subjects, family and school in a common place, the family schools for the promotion of affective-sexual education to create actions of individual and social change is shown. The intrinsic need to generate discussion regarding gender for the deconstruction of sociocultural attributed practices, promote training spaces regarding sexuality and affectivity and encourage reflection as a pedagogical exercise is evident

RESUMO

Sexually transmitted infections (STIs) affect young people in a disproportionate way, with more than half of the infections occurring in 15- to 25-year-olds, although as an age group they constitute only 25% of the sexually active population. Pediatricians should be familiar with the social, behavioral, and biological factors that predispose adolescents to STIs. Preventive visits for teens and pre-teens should incorporate education and counseling about sexuality, safe sexual behavior, and STIs. Pediatricians should be able to identify, diagnose, and manage STIs presenting as genital "bumps" and genital "ulcers." Pediatricians should also offer human immunodeficiency virus testing and expedited partner treatment to all adolescents who are diagnosed as having an STI.

RESUMO

The Francophone Society of Bone Marrow Transplantation and Cellular Therapy (SFGM-TC) organises annual workshops in an attempt to harmonise clinical practices among different francophone transplantation centres. The SFGM-TC harmonisation workshops aim at establishing practical guidelines, on the one hand, from data from the literature and international recommendations and, on the other hand, by consensus in the absence of formally proven data. The sexual and emotional life of allogeneic hematopoietic stem cells transplanted (HSCT) patients is often very impacted and remains a subject relatively little addressed by patients and caregivers. This article is an update from a previous workshop and is accompanied by a patient booklet, which will be included in the post allograft follow-up workbook published by the SFGM-TC. The purpose of these two documents is to facilitate discussions between patients and caregivers on the subject and to present proposals for follow-up and tools to better manage the sexual and emotional life of allotransplanted patients.

RESUMO

Genital dissatisfaction in healthy young women is on the rise. Pubic hair removal reveals genital tissue previously hidden, while social trends promote the absence of any protruding genital tissue as the feminine ideal. Information with regard to the natural diversity of external genitalia anatomy, and lifestyle changes to improve physical symptoms related to labial tissue may suffice to reassure a distressed adolescent. Female genital cosmetic surgery is rarely supported, before attaining genital maturity. However, genital dissatisfaction may lead to significant embarrassment and anxiety, requiring formal psychological counselling. Sexual education needs to counterbalance perceived normal anatomy, portrayed in pornography or female genital cosmetic surgery advertisements.

RESUMO

BACKGROUND: Disparities in sexually transmitted infections (STI) are an urgent problem among Native American youth and young adults which are not fully explained by different sexual or related behaviors. These sexual health disparities are more likely attributed to social environments and structural determinants such as a shortage of sexual healthcare providers, lower socioeconomic status, and access barriers to STI screening and treatment, including geographic isolation and confidentiality concerns. Innovative, non-clinic based alternatives to promote STI screening and treatment are essential for alleviating these disparities. Self-care, or the care taken by individuals towards their own health and well-being may be such a strategy. This study will assess the efficacy of a self-care intervention, called Protecting Our Future Generation, for increasing uptake of STI screening and impacting sexual risk and protective behaviors among Native American youth and young adults living in a reservation-based community in the Southwestern United States. METHODS: The proposed study is a randomized controlled trial to test the efficacy of a self-care intervention compared to a control condition. Participants will be Native Americans ages 14-26 years old who have had vaginal or anal sex at least once in their lifetime. Participants will be randomized to the intervention which includes: 1) a sexual health self-assessment with embedded clinical prediction tool predicting STI positivity, and 2) personalized messaging with key steps to lower risk for STIs, or the control condition which includes: 1) a self-assessment about water, soda and sugar sweetened beverage consumption, and 2) personalized messaging to meet recommended daily intake. All participants will be offered a self-administered STI test. Participants will complete assessments at baseline, 3- and 6-months follow-up. The primary outcome measure is completion of STI screening. DISCUSSION: Protecting Our Future Generation is among the first self-care interventions uniquely focused on sexual health among a Native American population, who endure significant sexual health disparities and are under-represented in research. If efficacious, the intervention will be a model of sexual health self-care for Native American youth and young adults adaptable for use in healthcare and community-based settings. TRIAL REGISTRATION: Clinical Trials: http://clinicaltrials.gov; NCT03895320; Registered 03/28/2019.

Introduction: According to the National Unintentional Pregnancy Prevention Plan in Adolescence in Argentina, yearly 109 thousand adolescents and 3 thousand girls under 15 years of age give birth. Multiple factors are involved; however, the most common and important is the lack of integrated sexual education. Objectives: 1- To evaluate the knowledge on sex and reproductive health before and after a a workshop addressed to mothers and fathers of hospitalized children. 2- To detect if there was a difference between mothers and fathers regarding age. Material and methods: A prospective, longitudinal, analytical, descriptive, observational beforeand-after study was conducted. Mothers and fathers of hospitalized children who participated in the workshop for the first time were included. The participating mothers and fathers were categorized according to age into younger than 19 years and 11 months (Gr1= adolescents) and 20 years or older (Gr2= young/adults). The following variables were analyzed: age, sex, place of origin, whether or not the person received sex and reproductive health education, and the source of sex and reproductive health information. A selfadministered survey was used with 12 questions on contraceptive methods, sexually transmissible diseases (STDs), and sex and reproductive rights and data were analyzed before and after the intervention. Results: 116 parents who met the inclusion criteria and agreed to participate were surveyed. (Gr1 = 24 and Gr2 = 92) Median age: 22 años (15-47). Overall, 53% reported having received prior information. 30% of whom had received information at school. A previous lack of knowledge on the topic was observed before attending the workshop regardless of age. The intervention of the workshop significantly increased the knowledge in both groups. Conclusion: The educational strategy should be focused on participation and integration. The methodology of a workshop is the most adequate as it facilitates debate and acquisition of competencies and skills and additionally provides theoretical concepts (AU)

RESUMO

ABSTRACT Objective: To verify the association between sex (male and female) and sociodemographic, reproductive and sexual variables in teenagers and identify the highest rates of social and health issues among them. Method: This was a cross-sectional study conducted with 239 adolescents enrolled in a public school of Salvador, Bahia, Brazil, whose data were produced by applying a structured form processed in Stata. Results: The research indicated an association between females and higher education level (p = 0.02), living with both parents (p = 0.02) and a higher rate of mental, social and behavioral issues. Being a man was associated with sexual initiation (p = 0.00), which occurred before they turned 14 years old (p = 0.05). Conclusion: The study variables behave, depending on sex, with smaller or greater chances of experiencing harmful situations, this understanding being essential for subsidizing educational activities that promote the quality of life of teenagers.

Abstract Sexuality is embedded in a social and cultural origin and its approach must go beyond biological aspects. Questionnaire on Sex Education in Schools (QUSES) assesses the knowledge, comfort, motivation and involvement of teachers in relation to sex education. This article aims to culturally adapt and verify the reliability of QUSES. The Experts Committee realized the Cultural adaptation and content validation. The Cronbach's alpha and its stratification were tested for reliability. It was applied to 100 teachers with an average of 44.45 (9.62) years of age. Words and phrases were modified, maintaining the equivalences of the original version. Question of knowledge was the most modified due to regulatory differences between countries. Reliability data was similar to the original instruments and were considered good. The lowest values were α = 0.741 for subscale of efficacy of results, α = 0.812 and α = 0.849 for subscale of involvement (cognitive). QUSES was culturally adapted for Brazilian elementary schools teachers presenting good reliability. It is relevant, because it evaluates sex education fully and may support in implementation and monitoring public policies related to health promotion in schools.

O objetivo deste artigo é adaptar culturalmente e verificar a confiabilidade do QUESME.

The aim of this study was to explore the relationship between different models of sexual morality education and the mental health level of female college students, to provide a scientific basis for college conduct an effective sexual morality education program that can improve the psychologic health level of female college students. The experimental and control groups were comprised of female college students who received different models of sexual morality education. Total and factor scores derived from the Symptom Checklist 90 (SCL90) were compared between these groups. The total mental health SCL90 score for female college students in the control group was 1.48 ±â€Š0.39, and the scores of 3 factors including SCL90-3 (interpersonal sensitivity, P < .01), SCL90-4 (depression, P < .05), and SCL90-5 (anxiety, P < .05) were all significantly higher than the national norm. The total mental health scores for female students in the purely theoretical experimental group and the integrated practical training experimental group were 1.40 ±â€Š0.42 and 1.33 ±â€Š0.39, respectively, both of which were significantly different from the control group (P < .05), while the difference between the 2 groups was near the threshold of significance (P = .052). There were significant differences between the integrated practical training experimental group and the control group for scores of 5 factors including SCL90-2 (obsessive-compulsive symptoms), SCL90-3 (interpersonal sensitivity), SCL90-4 (depression), SCL90-5 (anxiety), and SCL90-6 (hostility) (Ps < .01). There were also significant differences between the purely theoretical experimental group and the control group for scores of 3 factors including SCL90-2 (obsessive-compulsive symptoms), SCL90-3 (interpersonal sensitivity), and SCL90-6 (hostility) (Ps < .05). And, there were also significant differences between the 2 experimental groups for scores for 3 factors including SCL90-3 (interpersonal sensitivity), SCL90-4 (depression), and SCL90-5 (anxiety) (Ps < .05).Different models of sexual morality education have significantly different impacts on the mental health level of female college students. The integrated practical education model can significantly and effectively improve the mental health of female college students, and as such colleges and universities should adopt this integrated model to better improve these students' mental health.

RESUMO

BACKGROUND: Sexual and reproductive health (SRH) is an important disease-specific concern for adolescent and young adult (AYA) women that is not consistently addressed in cystic fibrosis (CF) centers. This study identifies educational needs and preferences of interprofessional CF providers regarding SRH in AYA women with CF. METHODS: Interprofessional CF providers participated in an anonymous survey regarding general and CF-specific SRH knowledge and skills, factors for optimizing SRH care, and preferred approaches for SRH training. We calculated descriptive statistics for all respondents and stratified by provider type. RESULTS: A total of 523 providers completed the survey (39% physicians/advanced practice providers, 19% nurses, 20% social workers, and 22% other disciplines). Half reported comfort responding to female AYA SRH concerns; however, only one-third were comfortable asking appropriate questions and confident taking a sexual history. Only 29% were comfortable with their current CF-specific SRH knowledge. Respondents' preferred SRH topics for further training included: pregnancy/parenthood planning, sexual functioning, urinary incontinence, intimate partner violence, and taking a sexual history. Nearly two-thirds felt having connection to women's health specialists familiar with CF would facilitate SRH care. Approximately one-third desired SRH educational materials for providers to view at point-of-care or through online case-based learning; <10% were interested in role playing SRH skills. CONCLUSION: Many interprofessional CF providers lack comfort and skills in addressing SRH with AYA women with CF. Provider training needs and approaches identified in this study can be used to develop tailored educational interventions to improve comprehensive CF care.
