RESUMO

Patients with thyroid disease must take long-term antithyroid drugs and go to the outpatient clinic for regular check-ups. This requires patients to have good compliance behaviors in order to better control their thyroid hormone levels. In order to improve patients' compliance behavior and seek effective family care interventions, this paper combines family care methods to evaluate the nursing effect of thyroid disease and combines investigation and analysis and experimental methods to verify the effectiveness of the method proposed in this paper. In addition, this paper compares family nursing methods with conventional methods under the guidance of family philosophy and objectively analyzes and evaluates the application value of accelerated rehabilitation surgery concepts and methods in thyroid surgery. Finally, this paper sets up a control group and a test group to carry out the nursing effect of home nursing in thyroid diseases. Through comparative experiments, it can be seen that the nursing intervention for thyroid diseases based on home nursing in this paper has a good nursing effect.

BACKGROUND: This study will assess the effect of high-quality nursing (HQN) on alleviating depression and anxiety (DA) in patients with thyroid cancer (TC) during perioperative period (PPP). METHODS: We will search the following electronic databases (MEDLINE, EMBASE, PsycINFO, Cochrane Library, CINAHL, Web of Science, CBM, WANGFANG, and CNKI) from inception to the present and other literature sources without language limitation. All potential randomized controlled trials reporting on effect of HQN on DA in patients with TC during PPP will be considered for inclusion. Two researchers will separately carry out study selection, data extraction, and study quality evaluation. Any different opinion will be solved by a third author through discussion. All statistical analysis will be performed by RevMan 5.3 software. RESULTS: We will appraise the effect of HQN on DA in patients with TC during PPP through assessing outcomes of depression, anxiety, pressure, quality of life, and adverse events. CONCLUSION: This study will provide evidence to determine whether HQN is effective or not on DA in patients with TC during PPP. OSF REGISTRATION:: osf.io/sb5r8.

BACKGROUND: Thyroidectomy has been considered an effective method to treat thyroid cancer. However, about 20% of patients have psychological distress before surgery. Psychological distress is considered common mental illnesses and it has been reported that the patients who suffer psychological distress have poor clinical outcomes than the patients without psychosocial disorder. Therefore, we design this randomized controlled study to explore the effect of psychological nursing intervention against quality of life and psychological distress of the patients with thyroid cancer. METHOD: The trial will be conducted from September 2020 to December 2020 at Wuhan Fourth Hospital on the basis of the International Council for Harmonisation's Good Clinical Practice Guidelines and the principles of the Helsinki Declaration. The study was authorized via the Research Ethics Committee of the Wuhan Fourth Hospital (Approval number: 20200721-046). This study is a single-center, randomized, 2-arm, evaluator-blinded clinical trial. In all, 90 patients with thyroid cancer undergoing thyroidectomy will be enrolled in this study. The inclusion criteria includes: patients aged between 20 and 60 years old; ASA I-II classification; normal platelet coagulation and count function. The exclusion criteria contains: people with the intellectual and cognitive impairment (behavioral-cognitive intervention); BMI above 35 kg/m; the history of renal and hepatic dysfunction; and patients refuse to participate in this study. Both the patients in psychological intervention group and control group should receive the routine care, while the psychological intervention group also needs to receive the additional proper psychological nursing interventions. The emotional disorders are detected with the Chinese version of Profile of Mood States-Brief. And the patients' life quality is evaluated with the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core Questionnaire (QLQ-C30, version 3.0). All the data are collated into Microsoft Excel 2010 and analyzed with SPSS 12.0 (IBM). RESULTS: It is assumed that psychological nursing intervention could alleviate the psychological distress of patients with thyroid cancer and improve their quality of life. CONCLUSION: This study can provide the reliable evidence regarding the influence of psychological nursing intervention against the life quality and psychological distress of the patients with thyroid cancer. The present research aimed to investigate the impact of a psychological nursing intervention on psychological distress and quality of life of thyroid cancer patients. A total of 286 participants were randomly divided into two groups: a control group (n = 143) and an intervention group (n = 143). In addition to routine nursing care, patients in the intervention group were given additional psychological nursing interventions. A reduced edition of the Profile of Mood States was used to evaluate mood disturbance, and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core Questionnaire (version 3.0) was applied to determine the quality of life of patients. The intervention group showed remarkably alleviated depression, anxiety, and mood disturbance when compared to the control group. In addition, global health status, functional status, and symptoms status were significantly improved in the intervention group. In conclusion, our findings suggest that a psychological nursing intervention could reduce psychological distress and enhance the life quality of thyroid cancer patients, which could have clinical value to the treatment of thyroid cancer.

PURPOSE: To offer a better understanding of the experiences, preferences, and needs of patients with thyroid cancer. PARTICIPANTS & SETTING: 17 patients with thyroid cancer receiving treatment at a university-affiliated hospital in Montreal, Québec, Canada. METHODOLOGIC APPROACH: Interviews were conducted with patients, and descriptive phenomenology was used to explore patients' lived experience. FINDINGS: Coping with uncertainty was a major theme that emerged from interviews, with some of the main concerns being difficult treatment decisions, long surgery wait times, and fears about surgical complications, potential metastases, and death. Study participants reported that without a nurse and an interprofessional team, they would be lost in a system they believed minimized their illness and offered few resources to support them in a time of crisis. IMPLICATIONS FOR NURSING: Nurses must understand how the needs of individuals with thyroid cancer are often overlooked because of the good prognosis associated with the disease and should work to meet these information and support needs.

BACKGROUND: Medullary thyroid cancer (MTC) is a rare cancer that has historically been managed by endocrinologists. In 2011, the first of several multi- targeted tyrosine kinase inhibitors was approved as treatment for MTC. These drugs have changed the management of MTC to teams that include oncologists and oncology nurses. OBJECTIVES: This article illustrates MTC diagnostics, surveillance, management of adverse drug reactions, and disease progression through a case study. METHODS: An overview of MTC is offered, followed by an in-depth case study that examines MTC from the patient's perspective. FINDINGS: Oncology nurses can influence patient outcomes through the provision of patient education, support, and management of disease and treatment complications.

The study focused on the development of action strategies for the management ofnursing care in Radioiodine therapy from the perspective of clients with differentiated thyroidcancer. PURPOSE/OBJECTIVES: To describe the survivorship experience of young adult patients with thyroid cancer. RESEARCH APPROACH: A qualitative, descriptive study. SETTING: Four Canadian provinces, with most participants from Ontario. PARTICIPANTS: 12 young adult thyroid cancer survivors who participated in a larger study on follow-up care needs consisting of 55 young adult cancer survivors. METHODOLOGIC APPROACH: Telephone interviews were conducted with cancer survivors who were diagnosed from age 18-39 years and were 1-5 years post-treatment. FINDINGS: All 12 thyroid cancer survivors discussed the feeling that their cancer experiences often were downplayed because thyroid cancer is labeled as the "good" cancer. Many said that they were not considered real patients with cancer by healthcare providers and other patients with cancer, and they were unable or unwilling to access support programs or assistance from healthcare providers. CONCLUSIONS: Cancer can have an impact on a person's life regardless of the prognosis. Being diagnosed with thyroid cancer at a young age can pose additional challenges because of the lack of available support to address needs specific to young adults. INTERPRETATION: Healthcare providers must recognize the needs of thyroid cancer survivors and encourage them to access supportive services. KNOWLEDGE TRANSLATION: Patients with thyroid cancer believe that their needs often are overlooked because of high survival rates, and they have difficulty accessing support resources and finding help. Young adults with cancer often have unique support needs. Support needs may not be the same for all young adult patients with cancer, and those needs should be recognized and addressed.

O estudo visou subsidiar o planejamento da assistência de enfermagem a clientes com câncer de tireoide submetidos à radioiodoterapia. Objetivos: descrever as necessidades de ajuda expressas por clientes submetidos à radioiodoterapia e analisar os cuidados de enfermagem requeridos para atender às necessidades identificadas. Adotou-se como marco referencial a relação de ajuda na totalidade da prática de enfermagem. O método foi o estudo de caso representativo, longitudinal, descritivo, com 24 clientes em tratamento no Serviço de Medicina Nuclear do Instituto Nacional de Câncer – Rio de Janeiro, no período de julho a setembro de 2006. Os resultados revelaram que os clientes, no transcurso da radioiodoterapia, requerem da equipe de enfermagem cuidados relacionados às orientações para o autocuidado, apoio emocional e cuidados físicos nas contingências das necessidades referidas. O que indica a importância da presença da enfermeira junto aos clientes para planejar e implementar cuidados com foco em suas necessidades.


This study aims to inform planning of nursing care for clients with thyroid cancer undergoing radioiodine therapy by describing the needs for assistance as expressed by such clients and analyzing the nursing care required to meet the needs identified. The framework adopted was the ratio of assistance to total nursing care, and the method, the descriptive, longitudinal, representative case study of twenty-four clients undergoing treatment from July to September 2006 at the Nuclear Medicine Service of the Instituto Nacional de Câncer, Rio de Janeiro. The results revealed that, in the course of radioiodine therapy, the care clients require from the nursing team includes self-care guidance, emotional support and physical care in contingences raising these needs. This indicates the importance of the nurse’s being present with clients during the planning and implementation of care that focuses on their needs.


El estudio visó subsidiar el planeamiento de la asistencia de enfermería a clientes con cáncer de tiroides sometidos a radioyodoterapia. Objetivos: describir las necesidades de ayuda expresas por clientes sometidos a la radioyodoterapia y analizar los cuidados de enfermería requeridos para atender a las necesidades identificadas. Se adoptó como marco referencial la relación de ayuda en la totalidad de la práctica de enfermería. El método fue el estudio de caso representativo, longitudinal, descriptivo, con 24 clientes en tratamiento en el Servicio de Medicina Nuclear del Instituto Nacional de Cáncer – Rio de Janeiro-Brasil, en el periodo de julio a septiembre.de 2006. Los resultados revelaron que los clientes, en el transcurso de la radioyodoterapia, requieren del equipo de enfermería cuidados relacionados a las orientaciones para el autocuidado, apoyo emocional y cuidados físicos en las contingencias de las necesidades referidas. Lo que indica la importancia de la presencia de la enfermera junto a los clientes para planear e implementar cuidados con foco en sus necesidades.

In nursing ethics the role of narratives and dialogue has become more prominent in recent years. The purpose of this article is to illuminate a relational-narrative approach to ethics in the context of palliative nursing. The case study presented concerns a difficult relationship between oncology nurses and a husband whose wife was hospitalized with cancer. The husband's narrative is an expression of depression, social isolation and the loss of hope. He found no meaning in the process of dying and death. The oncology nurses were not able to recognize his emotional and existential problems. A narrative perspective inspired by relational ethics indicates that participants may develop a relational narrative that seeks good for all involved in a situation. In palliative nursing this entails open communication about the fragility of life and approaching death. In relational narratives, answers to these ethical dilemmas are co-authored, contingent and contextual.

PURPOSE/OBJECTIVES: To enhance understanding of the experiences and educational needs of patients receiving iodine-131 (131I) therapy for the treatment of thyroid cancer. DESIGN: Qualitative design using interpretive description. SETTING: A 24-bed oncology/acute-care medical unit in a large tertiary hospital in western Canada. SAMPLE: 5 men and 22 women (18-80 years of age) who have received 131I therapy in the past two years. METHODS: Unstructured focus group and telephone interviews and field notes. Interviews were tape-recorded, transcribed verbatim, and subjected to thematic analysis. FINDINGS: Researchers elicited four major themes from the analysis: (a) recognizing the totality of the cancer experience, (b) being isolated, (c) recognizing the totality of the treatment experience, and (d) understanding barriers to treatment. CONCLUSIONS: Healthcare providers require a better understanding of thyroid disease and the treatment and side effects of 131I therapy. Furthermore, educational programs are required to adequately prepare nurses and patients for future care. IMPLICATIONS FOR NURSING PRACTICE: A need exists to improve the care and education provided to patients receiving 131I therapy. In particular, recognition of the totality of the cancer experience and the need for both staff and patient education were illustrated. If nurses are to provide comprehensive cancer care, both psychosocial and physical needs must be addressed and fulfilling these needs requires a collaborative approach among patients, nurses, and other healthcare professionals.

PURPOSE/OBJECTIVES: To describe and discuss the inherited syndrome multiple endocrine neoplasia (MEN) and one of its components medullary thyroid carcinoma. DATA SOURCES: Published books and articles; clinical experience. DATA SYNTHESIS: Three distinct types of MEN are known. Type 2a consists of medullary thyroid carcinoma, pheochromocytoma, and other conditions. The disorder is autosomal-dominant. Genetic counseling and screening and biochemical screening are possible. Early detection and treatment are key to successful control. CONCLUSIONS: Identification of kindreds affected by this inherited disorder can result in improved detection and early treatment. The merit and value of presumptive treatment remain unclear. IMPLICATIONS FOR NURSING PRACTISE: Nursing activities encompass prediagnosis through postoperative phases. Care involves all members of the family as well as psychological care.

RESUMO

Thyroidectomy procedures are the mainstay treatment for thyroid cancer and are safe and effective when they are performed by experienced surgeons. Preoperative evaluation of patients with suspected thyroid cancer consists of ultrasonography, radioisotope scanning, and computed tomography scans. Postoperative complications of thyroidectomy procedures include hemorrhage, edema of the glottis, muscle rigidity and spasm (ie, tetany), acute thyrotoxicosis, and damage to the laryngeal nerve. Most surgical patients are discharged home within three days after surgery, and many patients require lifelong thyroid hormone replacement to prevent hypothyroidism.
