Exposure to asbestos and asbestos-like minerals has been related to the development of severe lung diseases, including cancer and malignant mesothelioma (MM). A high incidence of non-occupational MM was observed in New Caledonia (France) in people living in proximity of serpentinite outcrops, containing chrysotile and fibrous antigorite. Antigorite is a magnesium silicate, which shares with chrysotile asbestos the chemical formula. To achieve information on antigorite toxicity, we investigated the physico-minero-chemical features relevant for toxicity and cellular effects elicited on murine macrophages (MH-S) and alveolar epithelial cells (A549) of three fibrous antigorites (f-Atg) collected in a Caledonian nickel lateritic ore and subjected to supergene alteration. Field Atg were milled to obtain samples suitable for toxicological studies with a similar particle size distribution. UICC chrysotile (Ctl) and a non-fibrous antigorite (nf-Atg) were used as reference minerals. A high variability in toxicity was observed depending on shape, chemical alteration, and surface reactivity. The antigorites shared with Ctl a similar surface area (16.3, 12.1, 20.3, 13.4, and 15.6 m2/g for f-Atg1, 2, 3, nf-Atg, and Ctl). f-Atg showed different level of pedogenetic weathering (Ni depletion f-Atg1 ≪ f-Atg2 and 3) and contained about 50% of elongated mineral particles, some of which exhibited high aspect ratios (AR > 10 µm, 20%, 26%, 31% for f-Atg1, 2, and 3, respectively). The minerals differed in bio-accessible iron at pH 4.5 (f-Atg1 ≪ f-Atg3, < f-Atg2, nf-Atg < Ctl), and surface reactivity (ROS release in solution, f-Atg1 ≪ f-Atg2, 3, nf-Atg, and Ctl). f-Atg2 and f-Atg3 induced oxidative stress and pro-inflammatory responses, while the less altered, poorly reactive sample (f-Atg1) induced negligible effects, as well nf-Atg. The slow dissolution kinetics observed in simulated body fluids may signal a high biopersistence. Overall, our work revealed a significative cellular toxicity of f-Atg that correlates with fibrous habit and surface reactivity.

New Caledonia is particularly affected by nickel open pit mining activities because of the presence of ultramafic soils rich in metals. The particles dispersed by atmospheric transport and soil erosion during the excavation of nickel end up by deposition or leaching in rivers where they may be bioaccumulated by organisms living downstream the mines. Despite alarming freshwater metals concentrations, no study investigated the level of their bioaccumulation in eels, and if high bioaccumulation levels occur, the potential consequences on their health. The aim of this study was to determine how eels Anguilla marmorata are impacted in situ by metals issued from mining activity by measuring: morphometric parameters; metal concentrations in tissues and organs and transcription levels of target genes encoding proteins involved in several metabolic key functions. Among organs, liver was found to be the most affected by mining with average nickel concentrations of 5.14 mg/kg versus 1.63 mg/kg for eels away from mines leading to dysregulation of numerous genes involved in oxidative stress, DNA repair, apoptosis, reproduction and both lipid and mitochondrial metabolisms. This study should allow us to define in an integrated way if metals released by mining activities influence metals bioaccumulation in eels and induce biological effects.

Herein we report an unprovoked shark attack on a scuba diver in New Caledonia. The species responsible for the attack was identified as a tiger shark (Galeocerdo cuvier), based on both the victim's testimony and forensic examination. The victim suffered significant loss of soft tissues from one thigh, which resulted in hemorrhagic shock. Even though the event occurred at a remote location, miles away from the nearest hospital, appropriate first aid, immediate deployment of an alert system, and prompt helicopter transfer by an emergency rescue team allowed the victim to be transferred to an intensive care unit in stable condition and to undergo surgery within 4 h of being bitten. Early coverage of exposed bone was performed, followed up with negative pressure dressing, antibiotic treatment, hyperbaric oxygen therapy, and a split skin graft. In spite of the massive muscular loss incurred, the victim was able to regain her ability to walk within 6 wk of the incident. Shark attacks on scuba divers are rare and seldom reported, especially in New Caledonia.

Coastal sediments downstream of ultramafic catchments can show Ni and Cr concentration well above sediment quality guidelines. Despite their potential ecological impact, the bioavailability of these trace metals in such sedimentary settings has been poorly investigated. In this study, we tried to fill this gap by performing kinetic EDTA-extractions across a shore-to-reef gradient in lagoon sediments downstream of an ultramafic catchment in New Caledonia and interpreting the results in regard of synchrotron-derived speciation. Measured bioavailability ranged from very low for Cr (below 1% of total Cr) to medium for Ni (below 5% of total Ni). Both trace metals showed a decreasing shore-to-reef bioavailability gradient reflecting the larger deposition of ultramafic sediments close to the shore. According to synchrotron-derived speciation data, the very low bioavailability of Cr is attributed to its major occurrence as Cr(III)-bearing Fe-(oxyhydr)oxides and phyllosilicates, with no evidence of Cr(VI). Considering the low occurrence of Fe-sulfides, the medium bioavailability of Ni is considered to arise mainly from the reductive dissolution of Ni-bearing Fe-(oxyhydr)oxides during early diagenesis. This reaction also explains the medium bioavailability of Fe (up to 15% of total Fe) and the positive correlation observed with Total Organic Carbon (TOC). In this regard, this latter parameter appears as a major driver of Ni and Fe bioavailability in coastal sediments downstream of ultramafic catchments. On the opposite, in the absence of Mn-oxides, TOC has no influence on Mn bioavailability (up to 30% of total Mn) that appears more likely driven by sediment sources. From an ecological point of view, considering the Australian and New-Zealand High Interim Sediment Quality Guidelines (ANZ-ISQG-H), Cr should not represent a significant risk towards benthic communities in coastal sediments downstream of ultramafic catchments. On the opposite, Ni, Fe and Mn might represent an ecological risk that should be further investigated in such sedimentary settings.

INTRODUCTION: Overseas France represents 18 % of French territory and is home to 4 % of its population for whom there is unequal treatment in the field of rare/complex cancer. AIM: To report our experience of intercontinental multidisciplinary videoconferencing between the French mainland and Pacific territories. METHODS: Every other friday, three centers located in Papeete, Nouméa and Paris-Villejuif connected between 6:30 AM and 8:00 AM GMT to discuss cases of rare/complex cancers. RESULTS: Between November 2019 and December 2020, 323 presentations implicating 233 patients involved sarcoma (n=93), digestive pathology (n=60), neuroendocrine tumors (n=35), urology (n=24), gynecology (n=24), neurology (n=16), thyroid pathology (n=14), dermatology (n=14), senology (n=11), hematology (n=11), ENT pathology (n=10), pathology thoracic (n=10) and pediatrics (n=1). Of the 233 patients, 134 (57.5 %) living in New Caledonia and 99 (42.5 %) in French Polynesia, 117 (50.5 %) had metastatic disease. 39 patients (16.7 %) were transferred to French mainland (EVASAN), for surgery (n=25), vectorized radiotherapy (n=7), biopsy (n=5), chemotherapy (n=1) or inclusion in a clinical trial (n=1). 195 patients (83.7 %) were treated at home, 15 (6.4 %) are still awaiting a decision and 4 (1.7 %) lost to follow-up. CONCLUSION: The use of videoconferencing to discuss rare/complex cancer cases was effective in guaranteeing French overseas population access to innovative therapies and clinical trials, limiting the need for intercontinental transfer to 16.7 %.

The hyperaccumulator Pycnandra acuminata is a New Caledonian rainforest tree known to have the highest concentration of nickel in any living organism, with 25 wt% nickel in its latex. All trees (with a diameter of >10 cm) and soil profiles in a 0.25-hectare permanent plot were sampled to assess the biogeochemical compartmentalisation of nickel in a dense stand of P. acuminata trees. Nickel stable isotope analysis permitted insights into the cycling of nickel in this ecosystem. The total tree biomass of the plot was calculated to be 281 tonnes ha-1 , which contained 0.44 kg of cobalt, 49.1 kg of manganese, 257 kg of nickel and 6.76 kg of zinc. Nickel stable isotope analysis identified the biotic origin of the nickel in the soil upper layers, with P. acuminata shoots enriched in lighter nickel isotopes. The δ60 Ni latex signature suggests that long-distance transport, radial xylem and phloem loading are at play in P. acuminata.

PURPOSE: The nematode Ascarophis distorta Fusco et Overstreet, 1978 (Cystidicolidae), originally described from the butterflyfish Chaetodon paucifasciatus Ahl (Perciformes, Chaetodontidae) in the Red Sea, was established based solely on the light microscopical (LM) examination of specimens. However, the present taxonomy of cystidicolid nematodes is mostly based on details of the cephalic structures properly visible only with the use of scanning electron microscopy (SEM). METHODS: Helminthological examinations of some marine fishes from coral reefs off New Caledonia, South Pacific, carried out in the years 2003-2007, revealed the presence of A. distorta in two Chaetodon spp. and thus enabled its redescription. The nematode specimens were studied with the use of both LM and SEM. RESULTS: The specimens of A. distorta were obtained from the stomach of the threadfin butterflyfish Chaetodon auriga Forsskål and the vagabond butterflyfish Chaetodon vagabundus Linnaeus. This is just the second record of this parasite since its description 43 years ago. SEM, used in this species for the first time, revealed some taxonomically important, previously unreported or insufficiently described morphological features, such as details in the cephalic structure, the shape of deirids, structure of the distal tip of the left spicule, or the exact number and distribution of male caudal papillae. The finding of A. distorta in C. auriga and C. vagabundus off New Caledonia represents new host and geographical records. The examination of 39 specimens of butterflyfishes from off New Caledonia, belonging to 14 Chaetodon spp. and 3 Heniochus spp., revealed that the nematode was only present in the 2 fish species listed above. CONCLUSIONS: This parasite seems to be host-specific to only certain species of butterflyfishes (Chaetodon spp.), and is probably widespread in the Indo-Pacific region, as well as its hosts.

Lichen biomonitoring and air mass trajectories were used to study the influence of mining activities in the atmospheric dispersion of metallic elements to assess the exposure of the population to dust emitted by mining activities. A map of forward trajectory densities from open mine surfaces throughout New Caledonia was computed and allowed to identify three preferred wind directions (trade wind, bent trade winds and oceanic winds) that could arise in mining particles dispersion all over New Caledonia. Areas where an air quality monitoring would be advisable to evaluate the exposure of the population to the Nickel dusts have been identified. Lichens collected around the industrial mining site KNS and in North Provence of New Caledonia were analysed for their Ni, Co, Cr, Zn and Ti contents. Backward trajectories were simulated from the lichen sampling point using FLEXTRA fed with ECMWF meteorological data, and densities of trajectories having overflown a mine were calculated. Ratio metal/Ti was then plotted as a function of air mass trajectory densities having overflown open pits. A positive correlation between trajectory densities and titanium-normalized metal in lichen for Ni, Co, Cr was highlighted, indicating that mining is a source of dispersion of these metals. For Zn, which is a tracer of fossil fuel or biomass (wood) combustion activity, no correlation was found. Graphical abstract.

BACKGROUND AND AIMS: Hybanthus austrocaledonicus (Violaceae) is a nickel (Ni) hyperaccumulator endemic to New Caledonia. One of the specimens stored at the local herbarium had a strip of bark with a remarkably green phloem tissue attached to the sheet containing over 4 wt% Ni. This study aimed to collect field samples from the original H. austrocaledonicus locality to confirm the nature of the green 'nickel-rich phloem' in this taxon and to systematically assess the occurrence of Ni hyperaccumulation in H. austrocaledonicus and Hybanthus caledonicus populations. METHODS: X-ray fluorescence spectroscopy scanning of all collections of the genus Hybanthus (236 specimens) was undertaken at the Herbarium of New Caledonia to reveal incidences of Ni accumulation in populations of H. austrocaledonicus and H. caledonicus. In parallel, micro-analytical investigations were performed via synchrotron X-ray fluorescence microscopy (XFM) and scanning electron microscopy with X-ray microanalysis (SEM-EDS). KEY RESULTS: The extensive scanning demonstrated that Ni hyperaccumulation is not a characteristic common to all populations in the endemic Hybanthus species. Synchrotron XFM revealed that Ni was exclusively concentrated in the epidermal cells of the leaf blade and petiole, conforming with the majority of (tropical) Ni hyperaccumulator plants studied to date. SEM-EDS of freeze-dried and frozen-hydrated samples revealed the presence of dense solid deposits in the phloem bundles that contained >8 wt% nickel. CONCLUSIONS: The occurrence of extremely Ni-rich green phloem tissues appears to be a characteristic feature of tropical Ni hyperaccumulator plants.

New Caledonia is a French territory in the South Pacific Ocean. While COVID-19 is expanding over the world, the situation on our island seems controlled with a total of 18 documented cases. We report the measures implemented on our island that probably helped contain the epidemic spread.

BACKGROUND: GSTM1 and GSTT1 are involved in detoxification of xenobiotics, products of oxidative stress and in steroid hormones metabolism. We investigated whether GSTM1 and GSTT1 gene deletion was associated with DTC risk and explored interaction with non-genetic risk factors of DTC. METHODS: The study included 661 DTC cases and 736 controls from two case-control studies conducted in France and New Caledonia. Odds ratios (OR) and their confidence interval (CI) for DTC associated with GST genotypes, alcohol drinking, tobacco smoking, body mass index and hormonal factors were calculated using logistic regression models. RESULTS: Results are presented for Europeans and Melanesians combined, as no heterogeneity between groups was detected. We found that DTC risk increased with obesity and decrease with alcohol drinking. After stratification by gene deletion status, the OR for obesity was 5.75, (95%CI 2.25-14.7) among individuals with GSTT1 and GSTM1-deleted genotype, and 1.26, (95%CI 0.89-1.77) in carriers of both genes (p-interaction = 0.02). The OR for drinking ≥1 glass/week was 0.33 (95%CI 0.15-0.74) in GSTT1-null individuals while it was 1.01 (95%CI 0.67-1.52) in non-null carriers of the gene (p-interaction = 0.01). No interaction between GST genotypes and other non-genetic risk factors was detected. CONCLUSION: GSTM1 and GSTT1 genotypes may modulate the DTC risk associated with BMI and alcohol consumption.

Recent examinations of spirurid nematodes (Spirurida) from deep-sea or coral reef marine fishes off New Caledonia, collected in the years 2006-2009, revealed the presence of the following five species: Ichthyofilaroides novaecaledoniensis (Moravec et Justine, 2009) n. gen., n. comb. (transferred from Ichthyofilaria Yamaguti, 1935) (females) (Guyanemidae) from the deep-sea fish Hoplichthys citrinus (Hoplichthyidae, Scorpaeniformes), Philometra sp. (male fourth-stage larva and mature female) (Philometridae) from Epinephelus maculatus (Serranidae, Perciformes), Ascarophis (Dentiascarophis) adioryx Machida, 1981 (female) (Cystidicolidae) from Sargocentron spiniferum (Holocentridae, Beryciformes), Ascarophis (Ascarophis) nasonis Machida, 1981 (males and females) from Naso lituratus and N. unicornis (Acanthuridae, Perciformes), and Ascarophisnema tridentatum Moravec et Justine, 2010 (female) from Gymnocranius grandoculis (Lethrinidae, Perciformes). Two species, I. novaecaledoniensis and A. nasonis, are redescribed based on light microscopical (LM) and scanning electron microscopical (SEM) examinations, the latter used in these species for the first time. Morphological data on the specimen of A. tridentatum from the new host species are provided. Philometra sp. (from E. maculatus) most probably represents a new gonad-infecting species of this genus. The newly established genus Ichthyofilaroides n. gen. is characterized mainly by the presence of a small buccal capsule and by the number and distribution of cephalic papillae in the female; it is the sixth genus in the Guyanemidae.

Le genre nouvellement établi Ichthyofilaroides n. gen. se caractérise principalement par la présence d'une petite capsule buccale et par le nombre et la répartition des papilles céphaliques chez la femelle. C'est le sixième genre des Guyanemidae.

Despite research efforts, current knowledge of the etiology of thyroid carcinoma remains limited. To explore the potential role of diet-induced inflammation, we examined the association between differentiated thyroid cancer risk and the energy-adjusted Dietary Inflammatory Index (E-DII) in a population-based case-control study conducted in New Caledonia, a Pacific archipelago with one of the highest recorded thyroid cancer incidence rates in the world. The E-DII was computed from food frequency questionnaire information on usual dietary intake. Logistic regression analyses were performed on data from 324 histologically confirmed cases of papillary or follicular carcinoma, diagnosed from 1993 to 1999, and 402 controls. Positive associations between E-DII and thyroid cancer risk were observed (comparing extreme tertiles, odds ratio = 1.67, 95% confidence interval: 1.08, 2.58; P for trend = 0.002), with stronger associations found for larger carcinomas (P for trend = 0.0005). Stratified analyses showed an increased risk of thyroid cancer associated with the E-DII among Southern province residents (P for trend = 0.003), Melanesian women (P for trend = 0.02), obese participants (P for trend = 0.006), and ever-smokers (P for trend = 0.0005). Our results suggest that a proinflammatory diet-especially when concomitant with other inflammation-inducing conditions or habits (e.g., obesity, smoking)-is associated with increased risk of thyroid carcinoma.

BACKGROUND: The prevalence of overweight and obesity in children and adolescents has dramatically increased in the Pacific Island countries and territories over the last decade. Childhood overweight and obesity not only have short-term consequences but are also likely to lead to noncommunicable diseases in adulthood. A major factor contributing to the rising prevalence is an insufficient amount of daily moderate-to-vigorous physical activity (MVPA). In the Pacific region, less than 50% of children and adolescents meet the international recommendations of 11,000 steps and 60 min of MVPA per day. Although studies have shown the potential of digital technologies to change behaviors, none has been proposed to guide adolescents toward achieving these recommendations. OBJECTIVE: The aims of this study were (1) to investigate whether a technology-based educational program that combines education, objective measures of physical activity (PA), and self-assessment of goal achievement would be well received by Pacific adolescents and help change their PA behaviors toward the international PA recommendations and (2) to create more insightful data analysis methods to better understand PA behavior change. METHODS: A total of 24 adolescents, aged 12 to 14 years, participated in a 4-week program comprising 8 1-hour modules designed to develop health literacy and physical skills. This self-paced user-centered program was delivered via an app and provided health-related learning content as well as goal setting and self-assessment tasks. PA performed during the 4-week program was captured by an activity tracker to support learning and help the adolescents self-assess their achievements against personal goals. The data were analyzed using a consistency rate and daily behavior clustering to reveal any PA changes, particularly regarding adherence to international recommendations. RESULTS: The consistency rate of daily steps revealed that the adolescents reached 11,000 steps per day 48% (approximately 3.4 days per week) of the time in the first week of the program, and this peaked at 59% (approximately 4.1 days per week) toward the end of the program. PA data showed an overall increase during the program, particularly in the less active adolescents, who increased their daily steps by 15% and ultimately reached 11,000 steps more frequently. The consistency of daily behavior clustering showed a 27% increase in adherence to international recommendations in the least active adolescents. CONCLUSIONS: Technology-supported educational programs that include self-monitored PA via activity trackers can be successfully delivered to adolescents in schools in remote Pacific areas. New data mining techniques enable innovative analyses of PA engagement based on the international recommendations.

The present study focused on the characterization of 10 Curtobacterium citreum strains isolated from the rhizosphere of pioneer plants growing on ultramafic soils from New Caledonia. Taxonomic status was investigated using a polyphasic approach. Three strains (BE, BB, and AM) were selected in terms of multiple-metal resistance and plant-growth-promoting traits. They were tested on sorghum growing on ultramafic soil and compared with the reference strain C. citreum DSM20528T. To better understand the bacterial mechanisms involved, biosorption, bioaccumulation, and biofilm formation were investigated for the representative strain of the ultramafic cluster (strain BE) versus C. citreum DSM20528T. The polyphasic approach confirmed that all native isolates belong to the same cluster and are C. citreum. The inoculation of sorghum with strains BE and BB significantly reduced Ni content in shoots compared with inoculation with C. citreum DSM20528T and control values. This result was related to the higher Ni tolerance of the ultramafic strains compared with C. citreum DSM20528T. Ni biosorption and bioaccumulation showed that BE exhibited a lower Ni content, which is explained by the ability of this strain to produce exopolysaccharides involved in Ni chelation. We suggested that ultramafic C. citreum strains are more adapted to this substrate than is C. citreum DSM20528T, and their features allow them to enhance plant metal tolerance.

Bactrocera curvipennis (Froggatt) is a polyphagous pest fruit fly endemic to New Caledonia that is weakly attracted to the male lure cue-lure (CL). Effective male lures are important for the monitoring and management of numerous pest species of Dacinae fruit flies. However, if a species is weakly responsive to these lures its detection and control is difficult. Recently in Oceania and Asia, more attractive male lures (isoeugenol, methyl-isoeugenol, dihydroeugenol, and zingerone) were identified for several weakly CL- and methyl eugenol (ME)-responsive species. To determine if these lures may be more attractive to B. curvipennis, we field tested them in comparison to CL and ME in New Caledonia. Bactrocera curvipennis catch with isoeugenol-baited traps (mean 20.3 ± 3.0) was 15 times greater than with CL (1.3 ± 0.8) and catch with dihydroeugenol (5.7 ± 1.6) was four times greater than with CL. This is the first record of B. curvipennis responding to these lures. It was also the only species that responded to isoeugenol in this study. Bactrocera fulvifacies (Perkins) (Diptera: Tephritidae), a rarely encountered species 'nonresponsive' to male lures, was attracted to zingerone with its trap catch (2,574 flies) approaching that of Bactrocera tryoni (Froggatt) (Diptera: Tephritidae) at CL (2,724 flies). Another nonresponsive species, Dacus aneuvittatus (Drew) (Diptera: Tephritidae), was also trapped by zingerone-baited traps. This is the first record of these species responding to a male lure. The significantly greater response of B. curvipennis to isoeugenol would make it a considerably more effective attractant for use in surveillance and control programs.

Seasonal variations of CO2 and CH4 fluxes were investigated in a Rhizophora mangrove forest that develops under a semi-arid climate, in New Caledonia. Fluxes were measured using closed incubation chambers connected to a CRDS analyzer. They were performed during low tide at light, in the dark, and in the dark after having removed the top 1-2 mm of soil, which may contain biofilm. CO2 and CH4 fluxes ranged from 31.34 to 187.48 mmol m-2 day-1 and from 39.36 to 428.09 µmol m-2 day-1, respectively. Both CO2 and CH4 emissions showed a strong seasonal variability with higher fluxes measured during the warm season, due to an enhanced production of these two gases within the soil. Furthermore, CO2 fluxes were higher in the dark than at light, evidencing photosynthetic processes at the soil surface and thus the role of biofilm in the regulation of greenhouse gas emissions from mangrove soils. The mean δ13C-CO2 value of the CO2 fluxes measured was -19.76 ±â€¯1.19‰, which was depleted compared to the one emitted by root respiration (-22.32 ±â€¯1.06‰), leaf litter decomposition (-21.43 ±â€¯1.89‰) and organic matter degradation (-22.33 ±â€¯1.82‰). This result confirmed the use of the CO2 produced within the soil by the biofilm developing at its surface. After removing the top 1-2 mm of soil, both CO2 and CH4 fluxes increased. Enhancement of CH4 fluxes suggests that biofilm may act as a physical barrier to the transfer of GHG from the soil to the atmosphere. However, the δ13C-CO2 became more enriched, evidencing that the biofilm was not integrally removed, and that its partial removal resulted in physical disturbance that stimulated CO2 production. Therefore, this study provides useful information to understand the global implication of mangroves in climate change mitigation.

New Caledonia (NC) is a small French territory in the Pacific Ocean with a relatively young (32% under 20) and multiethnic population. It is divided into 3 districts: Loyalty Island, the North, and the South, each with specific population characteristics. The aim of this study was to describe childhood cancer age-standardized incidence rate (ASR) in NC, compare it with the estimated one in France , and to determine whether residence and ethnicity may have an influence on intrapopulation ASR. All incident cancer cases diagnosed between 1994 and 2012 in children (0 to 14 y) resident in NC for more than 6 months were included. With 162 registered cases, the ASR for all cancers combined was 142.2 (range, 104.9 to 193.3) cases per million children per year and was not different from what was estimated in France (156.6). However, incidence varies according to the place of residence, with a higher ASR in the Loyalty Island district for several types of neoplasms, but not according to the ethnicity. The small migratory flux between this district and the rest of the territory may have led to these results, but, because of the small number of cases, no conclusion can be drawn.

The ultramafic massifs of the New Caledonian archipelago contain about 10% of the world's nickel reserves, which also contain significant but lower amounts of cobalt, chromium, and manganese. Natural erosion of these massifs and mining activities may contribute to the exposure of local populations to these metals through contamination of air, food, and water resources. We conducted a biomonitoring survey to evaluate exposure to these four metals and its main determinants by constructing a stratified sample of 732 adults and children (>3 years old) from visitors to 22 health centers across the archipelago. Urine was collected and analyzed by inductively-coupled plasma mass spectrometry to determine metal concentrations. A face-to-face interview was conducted to document sociodemographic characteristics, lifestyle and dietary habits, and residence-mine distance. Environmental samples (soil, house dust, water, and foodstuffs) were collected from two areas (one with and one without mining activity) to delineate determinants of exposure in more detail. Nickel and chromium were metals with the highest concentrations found in urine, especially in children, at levels exceeding reference values derived from representative national surveys elsewhere throughout the world (for children: 4.7 µg/g creatinine for nickel and 0.50 µg/g creatinine for chromium): 13% of children exceeded the reference value for nickel and 90% for chromium. Large variations were observed by region, age, and sex. In this geological setting, urinary and environmental nickel concentrations appear to be driven mainly by soil content. This is the first archipelago-wide survey of metal exposure in New Caledonia. The potential health consequences of this chronic high exposure need to be assessed.

BACKGROUND: Human T-Lymphotropic Virus type 1 (HTLV-1) is endemic among people of Melanesian descent in Papua New Guinea, Solomon Islands and Vanuatu, and in Indigenous populations from Central Australia. Molecular studies revealed that these Australo-Melanesian strains constitute the highly divergent HTLV-1c subtype. New Caledonia is a French overseas territory located in the Southwest Pacific Ocean. HTLV-1 situation is poorly documented in New Caledonia and the molecular epidemiology of HTLV-1 infection remains unknown. OBJECTIVES: Studying 500 older adults Melanesian natives from New Caledonia, we aim to evaluate the HTLV-1 seroprevalence and to molecularly characterize HTLV-1 proviral strains. STUDY DESIGN: Plasma from 262 men and 238 females (age range: 60-96 years old, mean age: 70.5) were screened for anti-HTLV-1 antibodies by particle agglutination (PA) and indirect immunofluorescence assay (IFA). Serological confirmation was obtained using Western blot assay. DNAs were extracted from peripheral blood buffy coat of HTLV-1 seropositive individuals, and subjected to four series of PCR (LTR-gag; pro-pol; pol-env and tax-LTR). Primers were designed from highly common conserved regions of the major HTLV-1 subtypes to characterize the entire HTLV-1 proviral genome. RESULTS: Among 500 samples, 3 were PA and IFA positive. The overall seroprevalence was 0.6%. The DNA sample from 1 New Caledonian woman (NCP201) was found positive by PCR and the complete HTLV-1 proviral genome (9,033-bp) was obtained. The full-length HTLV-1 genomic sequence from a native woman from Vanuatu (EM5), obtained in the frame of our previous studies, was also characterized. Both sequences belonged to the HTLV-1c Australo-Melanesian subtype. The NCP201 strain exhibited 0.3% nucleotide divergence with the EM5 strain from Vanuatu. Furthermore, divergence reached 1.1% to 2.9% with the Solomon and Australian sequences respectively. Phylogenetic analyses on a 522-bp-long fragment of the gp21-env gene showed the existence of two major clades. The first is composed of strains from Papua New Guinea; the second includes strains from all neighboring archipelagos (Solomon, Vanuatu, New Caledonia), and Australia. Interestingly, this second clade itself is divided into two sub-clades: strains from Australia on one hand, and strains from Solomon Islands, Vanuatu and New Caledonia on the other hand. CONCLUSIONS: The HTLV-1 seroprevalence (0.6%) in the studied adult population from New Caledonia appears to be low. This seroprevalence is quite similar to the situation observed in Vanuatu and Solomon Islands. However it is very different to the one encountered in Central Australia. Taken together, these results demonstrated that Australo-Melanesia is endemic for HTLV-1 infection with a high diversity of HTLV-1c strains and a clear geographic clustering according to the island of origin of HTLV-1 infected persons.
