BACKGROUND: As a facial feature, the auricle plays an important role in the integrity and aesthetics of the whole face. Auricular subunits are associated with patient satisfaction in auricular reconstruction, but there are few studies on auricular subunits. We want to evaluate the reproducibility of auricular subunits by measuring the coordinates of the marker points of auricular subunits, accordingly provide a reference for the improvement of auricular reconstruction and the aesthetics of auricular injection. METHODS: Mimics 19.0 was used to carry out three-dimensional (3D) reconstruction of the computed tomography (CT) scan data of patients' brains; measure the three-dimensional coordinates of the 13 auricular subunit markers, the morphological auricle length and width, and the physiological auricle length and width; and analyze the reproducibility as well as the differences between group. RESULTS: Reproducibility of auricle subunit markers: There are 1124 (58.82%) high reproducibility, 580 (30.35%) moderate reproducibility, and 207 (10.83%) low reproducibility. The superior tragus notch, tragus, and antitragus had the highest reproducibility. There was no significant difference between the groups in the marking points on the helix, and there were no statistically significant differences in the measurement values of the auricles on the two sides. The physiological ear length and width and the morphological ear length of males were larger than those of females. These showed significant differences between the age groups. CONCLUSION: Most auricular subunit markers have high reproducibility. The subunits with higher reproducibility are the structures that need to be optimized during auricle reconstruction surgery or auricle injection in the future. BACKGROUND: The superior auricular artery (SAA)-retroauricular flap is commonly used for the repair of defects of the superior auricle. There are few studies about the anatomy of the SAA. OBJECTIVE: This study mainly analyzed the anatomical pattern of SAA. MATERIALS AND METHODS: Computed tomography (CT) was performed on 26 cadaver heads infused with lead oxide. The anatomical pattern of the SAA was statistically analyzed by 3-dimensional CT images. RESULTS: The SAA was classified into 3 types according to whether it gave off the helix branch or the auricular dorsal branch. The SAA was located mainly in an area 2 cm above and below the horizontal line at the midpoint of the 2 base points (the otobasion superius and the apex of the external auditory canal). The origin of each branch of the SAA was mainly located in Areas 2, 3, and 4 within a circular area that had the otobasion superius as the center of the circle and a radius of 2 cm. CONCLUSION: In this study, the 3 anatomical types and anatomical patterns of the SAA were identified. These findings can provide a reference for the design of SAA-retroauricular flaps and for surgical planning.

Measurement of auricle parameters for planning and post-operative evaluation presents substantial challenges due to the complex 3D structure of the human auricle. Traditional measurement methods rely on manual techniques, resulting in limited precision. This study introduces a novel automated surface-based three-dimensional measurement method for quantifying human auricle parameters. The method was applied to virtual auricles reconstructed from Computed Tomography (CT) scans of a cadaver head and subsequent measurement of important clinically relevant aesthetical auricular parameters (length, width, protrusion, position, auriculocephalic angle, and inclination angle). Reference measurements were done manually (using a caliper and using a 3D landmarking method) and measurement precision was compared to the automated method. The CT scans were performed using both a contemporary high-end and a low-end CT scanner. Scans were conducted at a standard scanning dose, and at half the dose. The automatic method demonstrated significantly higher precision in measuring auricle parameters compared to manual methods. Compared to traditional manual measurements, precision improved for auricle length (9×), width (5×), protrusion (5×), Auriculocephalic Angle (5-54×) and posteroanterior position (23×). Concerning parameters without comparison with a manual method, the precision level of supero-inferior position was 0.489 mm; and the precisions of the inclination angle measurements were 1.365 mm and 0.237 mm for the two automated methods investigated. Improved precision of measuring auricle parameters was associated with using the high-end scanner. A higher dose was only associated with a higher precision for the left auricle length. The findings of this study emphasize the advantage of automated surface-based auricle measurements, showcasing improved precision compared to traditional methods. This novel algorithm has the potential to enhance auricle reconstruction and other applications in plastic surgery, offering a promising avenue for future research and clinical application.

BACKGROUND: Ears are an important aesthetic feature that is vital to the overall attractiveness of the face. Although there have been many studies on the aesthetics of the auricle, there is currently a lack of consensus on the ideal proportion of auricle exposure for Asian women in frontal view. OBJECTIVES: This study aimed to investigate ideal proportion of auricle exposure in Asian women. METHODS: An observational study was carried out on the photographs of 84 women on the list of the 100 most beautiful faces in Asia (published by TCC Asia in 2020). The proportion of the distance between the outer canthus and the outermost point of auricle to the distance between the inner canthus and the outermost point of auricle was calculated as the auricle exposure proportion. Evaluators were asked to rank a set of photographs of the volunteer with varying auricle exposure proportions from most attractive to least attractive. RESULTS: Measurements of the photographs of the 84 women showed a mean ear exposure proportion of 0.600. With 487 questionnaire responses received, the proportion of auricle exposure that the evaluators considered most attractive was 0.600. People with aesthetic experience considered 0.625 the most attractive proportion, while the general group considered 0.600 the most attractive. CONCLUSIONS: The ideal proportion of the auricle exposure for Asian women is in the range of 0.60-0.625, which may help surgeons reconstruct aesthetically pleasing ears. SUMMARY: A detailed data of shape of auricular concha is currently not available. Therefore, this study constructed a framework to statistically analyze the shape of the auricular concha based on a database of 3D scanned images. The 3D models of the auricular concha from 308 study participants are converted into compatible NURBS surfaces generated through 795 data points to perform a requisite statistical analysis. Subsequently, the shape of the auricular concha of the participants is grouped into 29 clusters with a modified hierarchical clustering algorithm. Finally, a wear trial and simulation test are carried out to validate the wear comfort of a designed OTC hearing aids based on the average shape of each cluster. The experimental results show that the average shape obtained as per the cluster is capable of representing the common geometric properties of their corresponding members, and could thus be used as a reference in designing mass-customized OTC hearing aid. The method in this study is superior to conventional methods that rely on sparse results for shape analysis because it takes into account the intricate geometric shape of the auricular concha. The quantitative description of conchal morphometry will be beneficial for plastic surgeons, and for the ergonomic design of ear-related products.
RESUMEN: Actualmente no se dispone de datos específicos de la forma de la concha auricular. Por lo tanto, este estudio construyó un marco para analizar estadísticamente la forma de ella a partir de una base de datos de imágenes escaneadas en 3D. Los modelos 3D de la concha auricular de 308 participantes del estudio se convirtieron en superficies NURBS compatibles generadas a través de 795 puntos de datos para realizar un análisis estadístico requerido. Posteriormente, la forma de la concha auricular de los participantes se agrupó en 29 grupos con un algoritmo de agrupamiento jerárquico modificado. Finalmente, se llevó a cabo una prueba de uso y una prueba de simulación para validar la comodidad de uso de unos audífonos OTC diseñados en función de la forma promedio de cada grupo. Los resultados experimentales mostraron que la forma promedio obtenida de la concha auricular, según el grupo, es capaz de representar las propiedades geométricas comunes de sus miembros correspondientes y, por lo tanto, podría usarse como referencia en el diseño de audífonos OTC personalizados en masa. El método de este estudio es superior a los métodos convencionales que se basan en resultados escasos para el análisis de forma, porque tiene en consideración la intrincada forma geométrica de la concha auricular. La descripción cuantitativa de la morfometría de la concha auricular será beneficiosa para los cirujanos plásticos y para el diseño ergonómico de productos relacionados con el oído.

SUMMARY: Personal identification is mostly done by facial assessment. The auricle plays a key role in the evaluation and recognition of the craniofacial complex. The aim of this study was to provide normative database using the measurements of the auricle among young males and females by anthropometric methods. Anthropometric measurements were taken from 115 participants, (56 men and 59 women) between 18-30 years old. Participants werestudents at Yasar Dogu Faculty of Sport Sciences of Ondokuz Mayis University. Each participant signed an informed consent form and then participated to the study. We found that the mean length of the right and left auricles were 58.30-58.52 mm, the mean width of the right and left auricles were 30.04-29.73 mm, the mean length of right and left lobules were 16.05-16.45 mm, and the mean width of the right and left lobules were 17.61-18.76 mm in female participants. The mean length of right and left auricles were 62.33-62.49 mm, the mean width of the right and left auricles were 33.60-33.96 mm, the mean length of right and left lobules were 18.49-18.11 mm, and the mean width of the right and left lobules were 19.19-20.47 mm in male participants. The data obtained from the present study may be useful for further studies. Also, the results may be useful for reconstructive surgeons to analyze the size and shape of the auricle.
RESUMEN: La identificación personal se realiza principalmente mediante evaluación facial. El pabellón auricular tiene un papel clave en la evaluación y el reconocimiento del complejo craneofacial. El objetivo de este estudio fue proporcionar una base de datos normativa utilizando las medidas de la oreja entre hombres y mujeres jóvenes por métodos antropométricos. Se tomaron medidas antropométricas de 115 participantes, (56 hombres y 59 mujeres) entre 18 y 30 años. Los participantes eran estudiantse de la Facultad de Ciencias del Deporte Yasar Dogu de la Universidad Ondokuz Mayis. Cada participante firmó un formulario de consentimiento informado y luego participó en el estudio. Encontramos que en las mujeres la longitud media de las aurículas derecha e izquierda era de 58,30-58,52 mm, el ancho medio de las aurículas derecha y izquierdas era de 30,04 a 29,73 mm, la longitud media de los lóbulos derecho e izquierdo era de 16,05 a 16,45 mm y el ancho medio de los lóbulos derecho e izquierdo era de 17,61 a 18,76 mm. En los hombres, la longitud media de las aurículas derecha e izquierda era de 62,33-62,49 mm, el ancho medio de las aurículas derecha e izquierda era de 33,60-33,96 mm, la longitud media de los lóbulos derecho e izquierdo era de 18,49-18,11 mm, y el ancho medio de los lóbulos derecho e izquierdo era de 18,49-18,11 mm. Los datos obtenidos del presente estudio pueden ser útiles para estudios posteriores. Además, los resultados pueden ser útiles para que los cirujanos reconstructivos analicen el tamaño y la forma del pabellón auricular.

OBJECTIVES/HYPOTHESIS: To analyze the use of highly translatable three-dimensional (3D)-printed auricular scaffolds with and without novel cartilage tissue inserts in a rodent model. STUDY DESIGN: Preclinical rodent animal model. METHODS: This prospective study assessed a single-stage 3D-printed auricular bioscaffold with or without porcine cartilage tissue inserts in an athymic rodent model. Digital Imaging and Communications in Medicine computed tomography images of a human auricle were segmented to create an external anatomic envelope filled with orthogonally interconnected spherical pores. Scaffolds with and without tissue inset sites were 3D printed by laser sintering bioresorbable polycaprolactone, then implanted subcutaneously in five rats for each group. RESULTS: Ten athymic rats were studied to a goal of 24 weeks postoperatively. Precise anatomic similarity and scaffold integrity were maintained in both scaffold conditions throughout experimentation with grossly visible tissue ingrowth and angiogenesis upon explantation. Cartilage-seeded scaffolds had relatively lower rates of nonsurgical site complications compared to unseeded scaffolds with relatively increased surgical site ulceration, though neither met statistical significance. Histology revealed robust soft tissue infiltration and vascularization in both seeded and unseeded scaffolds, and demonstrated impressive maintenance of viable cartilage in cartilage-seeded scaffolds. Radiology confirmed soft tissue infiltration in all scaffolds, and biomechanical modeling suggested amelioration of stress in scaffolds implanted with cartilage. CONCLUSIONS: A hybrid approach incorporating cartilage insets into 3D-printed bioscaffolds suggests enhanced clinical and histological outcomes. These data demonstrate the potential to integrate point-of-care tissue engineering techniques into 3D printing to generate alternatives to current reconstructive surgery techniques and avoid the demands of traditional tissue engineering. OBJECTIVE: To explore the anthropometric changes of the auricle after auricular cartilage unfolding in moderate concha-type microtia patients, so as to provide the basis to help evaluate surgical timing and prognostic. METHODS: A total of 33 children with moderate concha-type microtia, who were treated with auricular cartilage unfolding between October 2016 and September 2018 and met the inclusive criteria, were included in the study. There were 24 boys and 9 girls with an average age of 1.4 years (range, 1-3 years). Sixteen cases were left ears and 17 cases were right ears. The follow-up time was 12-23 months (mean, 17.5 months). The affected auricular detailed structures were observed and quantitatively analyzed before operation and at immediate after operation. The width, length, and perimeter of auricle before operation and at immediate after operation and at last follow-up were noted with three dimensional-scanning technology. The normal auricle was noted as control. RESULTS: There were (7.5±1.0) and (11.3±0.8) structures of the affected auricle at pre- and post-operation, respectively, showing significant difference between pre- and post-operation ( t=23.279, P=0.000). The length, width, and perimeter of the affected auricle constantly increased after operation, and there were significant differences between pre-operation and immediately after operation and between immediately after operation and last follow-up ( P<0.05). The differences of length, width, and perimeter of the affected auricle between immediately after operation and last follow-up were (3.13±1.44), (2.44±0.92), and (8.50±3.76) mm, respectively. And the differences of length, width, and perimeter of the normal auricle between pre-operation and last follow-up were (3.16±1.54), (2.35±0.86), and (9.79±4.60) mm, respectively. There was no significant difference in the differences of length, width, and perimeter between the affected auricle and the normal auricle ( P>0.05). CONCLUSION: The auricular cartilage unfolding in treatment of the moderate concha-type microtia can receive more ear structures and increase auricle sizes, which make it possible for free composite tissue transplantation. In addition, the affected and the contralateral normal auricles have a very similar growth rate and it offers the theoretical foundation for the early treatment for moderate concha-type microtia.

OBJECTIVE: Aesthetic reconstruction of the external ear is challenging due to the complex anatomical shape of the auricle. Recently, artificial scaffolds such as Medpor (Stryker, Kalamasoo, MI, USA) have become widely used in ear reconstruction. However, the Medpor scaffold is stiffer than the natural ear, which may lead to discomfort, and moreover has uniform design for every patient. In this study, we investigated whether three-dimensional (3D)-printed artificial polyurethane (PU) scaffolds are suitable for auricular reconstruction. METHODS: PU scaffolds were fabricated using 3D printing according to a design derived from a digital imaging and communications in medicine (DICOM) image of the human auricle. The microstructure of the scaffolds was observed using scanning electron microscopy, and the porosity was examined. Cell proliferation on the scaffolds was assessed in vitro using tonsil-derived mesenchymal stem cells to evaluate the biocompatibility of the scaffolds. The scaffolds were implanted in C57BL/6 mice, and histological analysis was performed. RESULTS: The structural study revealed that the 3D-printed porous PU scaffolds have rectangular microstructure with regular pitch and line, as well as high porosity (56.46% ± 10.22%) with a pore diameter of 200 µm. The mechanical properties of the 3D-printed PU scaffolds were similar to those of the human auricle cartilage. Cell proliferation on the PU scaffolds was greater than that on Medpor scaffolds. Histological evaluation demonstrated that the porous parts of the PU scaffolds became filled with collagen and vascular tissue. CONCLUSION: Elastic, porous PU scaffolds can be obtained using 3D printing, have biomechanical properties similar to those of the natural ear, and are suitable for use in auricular reconstruction. SUMMARY: A detailed data of concha is currently not available. Therefore, the present study aimed to determine twelve morphometric measurements of concha, to investigate its sexual dimorphism and bilateral asymmetry, and to establish basic shapes of concha for both sexes and sides. The study sample comprised of 310 young Chinese aged 18-28 years. 141 left and 141 right ear impressions for females, 169 left and 169 right ear impressions for males were collected and scanned. The 3D coordinates of seven landmarks on each auricular concha were obtained using 3D scanning technology and curvature theory. From the landmarks, twelve morphometric measurements of concha were calculated and analyzed. The conchal morphometry exist significantly sexual dimorphism in this study sample. On average, all measurements were larger in males than in females regardless of the sides. There was significantly bilateral asymmetry among left and right conchae in both sexes. Some measurements were larger in the right sides and some measurements were larger in the left sides, but the bilateral difference in both measurements found to be less than 1mm. Additionally, the basic shapes of concha for both sexes and sides were established on the basis of the mean 3D coordinates of each landmark and the mean value of each measurement. The anthropometric method of this study could overcome the difficulty in locating landmarks of auricle complex structures, and attain a higher level of accuracy in the procedure of measurement. The quantitative description of conchal morphometry will be beneficial for plastic surgeons, and for the ergonomic design of hearing aids.
RESUMEN: Actualmente no existen datos detallados de la concha auricular. Por lo tanto, el presente estudio tuvo como objetivo determinar doce medidas morfométricas de la concha auricular, investigar su dimorfismo sexual y asimetría bilateral, y establecer formas básicas de la concha para ambos sexos y lados. La muestra del estudio estaba compuesta por 310 jóvenes chinos de 18 a 28 años de edad. Se recolectaron y escanearon 141 impresiones de la oreja izquierda y 141 de la oreja derecha en mujeres; 169 impresiones de la oreja izquierda y 169 de la oreja derecha en hombres. Las coordenadas 3D de siete hitos en cada concha auricular se obtuvieron utilizando la tecnología de exploración 3D y la teoría de la curvatura. A partir de los hitos, se calcularon y analizaron doce medidas morfométricas de concha. La morfometría conchal indica la existencia significativa de dimorfismo sexual en esta muestra. En promedio, todas las mediciones fueron mayores en los hombres que en las mujeres, independientemente de los lados. Se observó una asimetría bilateral significativa entre las conchas izquierda y derecha en ambos sexos. Algunas medidas eran mayores en el lado derecho y otras medidas eran mayores en el lado izquierdo, pero la diferencia bilateral en ambas medidas fue menor a 1 mm. Además, las formas básicas de concha para ambos sexos y lados se establecieron sobre la base de las coordenadas 3D medias de cada punto de referencia y el valor medio de cada medición. El método antropométrico de este estudio podría superar la dificultad de localizar los hitos de las estructuras del complejo auricular y lograr un mayor nivel de precisión en el procedimiento de medición. La descripción cuantitativa de la morfometría conchal será util para los cirujanos plásticos y para el diseño ergonómico de audífonos.

BACKGROUND: Visible age-related signs indicate biological age, as individuals that appear old for their age are more likely to be at poor health, compared with people that appear their actual age. The aim of this study was to investigate whether alcohol and smoking are associated with four visible age-related signs (arcus corneae, xanthelasmata, earlobe crease and male pattern baldness). METHODS: We used information from 11 613 individuals in the Copenhagen City Heart Study (1976-2003). Alcohol intake, smoking habits and other lifestyle factors were assessed prospectively and visible age-related signs were inspected during subsequent examinations. RESULTS: The risk of developing arcus corneae, earlobe crease and xanthelasmata increased stepwise with increased smoking as measured by pack-years. For alcohol consumption, a high intake was associated with the risk of developing arcus corneae and earlobe crease, but not xanthelasmata. CONCLUSIONS: High alcohol consumption and smoking predict development of visible age-related signs. This is the first prospective study to show that heavy alcohol use and smoking are associated with generally looking older than one's actual age.

BACKGROUND: The incidence of neonatal ear deformities varies widely in the literature, and the incidence of those that correct spontaneously is largely unknown. To address these questions, the authors analyzed the auricles of 1500 newborns from the Pearl River Delta within the first 7 days of life and again at 30 days of age. METHODS: Using a combination of direct measurement and digital image measurement, the authors measured physiognomic ear length, physiognomic ear breadth, morphologic ear length, vertical distance between head and ear, and auriculocephalic angle of the newborns. The first measurement was within 1 week of birth, whereas the second measurement was performed for the same newborns 1 month after birth. RESULTS: Among 1500 newborns, 862 had various forms of auricular deformities. The incidence of neonatal auricular deformity in the Pearl River Delta area was 57.46 percent, and the self-healing rate was 31.55 percent by the 30-day follow-up. Seven factors were found to be statistically significant: mother's delivery situation, mother's hepatitis history, abnormal pregnancy, abnormal labor, cephalopelvic disproportion, premature rupture of membranes, and neonatal weight. Birth by cesarean delivery and mother with no abnormal pregnancy or no history of hepatitis were protective factors against the newborn's auricular deformity. CONCLUSIONS: Newborns have a high incidence of auricular deformities, and different types of auricular deformities have distinct incidences. Auricular deformities are caused by the interactions of a number of factors. There were large differences in the self-healing rates of different types of auricular deformities. BACKGROUND: Protruding ears represent the main abnormality of the external ear, which has required numerous anatomic and surgical studies. Most studies give attention to the absence of the antihelix as the anatomic defect responsible for the clinical deformity of the lateral aspect of the ear that leads to its anteversion. The reason for this study is the controversial origin of the fold of the antihelix within the auricle framework, a field of interest for aesthetic otoplasty. The current study examined the medial surface of the cartilaginous ear frame from cadaver specimens with right morphology to investigate the starting point of the fold of the antihelix. This allowed for verification of a natural plica at the anatomic base of this antihelical fold, which to date has not had its topography described morphologically. It is acknowledged that relevant literature makes no reference to this innominate natural plica at the origin of the antihelix, whose anatomic and surgical importance is related in this report. This study aimed to show that the existence of a natural plica at the base of the antihelix in ear framing represents a landmark between normal and protruding ear morphology. METHODS: For 8 years, 118 ears were carefully investigated within rigid ethical principles based on a thorough review of the pertinent literature. The study investigated 16 selected cadaver specimens and 102 protruding ears dissected by the senior author including 49 bilateral cases (26 males and 23 females) and 4 unilateral cases (2 males and 2 females). Bifacial anthropometric measurements by calipers were used for documentation. RESULTS: A natural plica at the base of the antihelix was found in all cadaver ears selected with right morphology, whereas it was totally absent in every surgically treated protruding ear irrespective of color, gender, age, or ethnic origin. Ambilateral measures of the antihelix eminence certify the study object in normal specimens as well as its lack in abnormal ones. CONCLUSION: Technical and topographic knowledge that a natural plica exists at the anatomic base of the antihelix is a valuable key point in recognizing the normal external ear. In addition, the making of a natural plica is the first and most effective factor in the reconstruction of the antihelical fold and its absolute absence results in the pathologic condition for protruding ears. IMPORTANCE: The prominent ear is a common external ear anomaly that is usually corrected through surgery. Electromechanical reshaping (EMR) may provide the means to reshape cartilage through the use of direct current (in milliamperes) applied percutaneously with needle electrodes and thus to reduce reliance on open surgery. OBJECTIVE: To determine the long-term outcomes (shape change, cell viability, and histology) of a more refined EMR voltage and time settings for reshaping rabbit auricle. DESIGN, SETTING, AND SUBJECTS: The intact ears of 14 New Zealand white rabbits were divided into 2 groups. Group 1 received 4 V for 5 minutes (5 ears), 5 V for 4 minutes (5 ears), or no voltage for 5 minutes (control; 4 ears). Group 2 received an adjusted treatment of 4 V for 4 minutes (7 ears) or 5 V for 3 minutes (7 ears). A custom mold with platinum electrodes was used to bend the pinna and to perform EMR. Pinnae were splinted for 6 months along the region of the bend. Rabbits were killed humanely and the ears were harvested the day after splint removal. Data were collected from March 14, 2013, to July 8, 2014, and analyzed from August 29, 2013, to March 1, 2015. MAIN OUTCOMES AND MEASURES: Bend angle and mechanical behavior via palpation were recorded through photography and videography. Tissue was sectioned for histologic examination and confocal microscopy to assess changes to microscopic structure and cell viability. RESULTS: Rabbits ranged in age from 6 to 8 months and weighed 3.8 to 4.0 g. The mean (SD) bend angles were 81° (45°) for the controls and, in the 5 EMR groups, 72° (29°) for 4 V for 4 minutes, 101° (19°) for 4 V for 5 minutes, 78° (18°) for 5 V for 3 minutes, and 126° (21°) for 5 V for 4 minutes. At 5 V, an increase in application time from 3 to 4 minutes provided significant shape change (78° [18°] and 126° [21°], respectively; P = .003). Pinnae stained with hematoxylin-eosin displayed localized areas of cell injury and fibrosis in and around electrode insertion sites. This circumferential zone of injury (range, 1.3-2.1 mm) corresponded to absence of red florescence on the cell viability assay. CONCLUSIONS AND RELEVANCE: In this in vivo study, EMR produces shape changes in the intact pinnae of rabbits. A short application of 4 V or 5 V can achieve adequate reshaping of the pinnae. Tissue injury around the electrodes is modest in spatial distribution. This study provides a more optimal set of EMR variables and a critical step toward evaluation of EMR in clinical trials. BACKGROUND: Earlobe crease (ELC) has been associated with cardiovascular diseases (CVD) or risk factors (CVRF) and could be a marker predisposing to CVD. However, most studies studied only a small number of CVRF and no complete assessment of the associations between ELC and CVRF has been performed in a single study. METHODS: Population-based study (n = 4635, 46.7% men) conducted between 2009 and 2012 in Lausanne, Switzerland. RESULTS: Eight hundred six participants (17.4%) had an ELC. Presence of ELC was associated with male gender and older age. After adjusting for age and gender (and medication whenever necessary), presence of ELC was significantly (p < 0.05) associated with higher levels of body mass index (BMI) [adjusted mean ± standard error: 27.0 ± 0.2 vs. 26.02 ± 0.07 kg/m(2)], triglycerides [1.40 ± 0.03 vs. 1.36 ± 0.01 mmol/L] and insulin [8.8 ± 0.2 vs. 8.3 ± 0.1 µIU/mL]; lower levels of HDL cholesterol [1.61 ± 0.02 vs. 1.64 ± 0.01 mmol/L]; higher frequency of abdominal obesity [odds ratio and (95% confidence interval) 1.20 (1.02; 1.42)]; hypertension [1.41 (1.18; 1.67)]; diabetes [1.43 (1.15; 1.79)]; high HOMA-IR [1.19 (1.00; 1.42)]; metabolic syndrome [1.28 (1.08; 1.51)] and history of CVD [1.55 (1.21; 1.98)]. No associations were found between ELC and estimated cardiovascular risk, inflammatory or liver markers. After further adjustment on BMI, only the associations between ELC and hypertension [1.30 (1.08; 1.56)] and history of CVD [1.47 (1.14; 1.89)] remained significant. For history of CVD, further adjustment on diabetes, hypertension, total cholesterol and smoking led to similar results [1.36 (1.05; 1.77)]. CONCLUSION: In this community-based sample ELC was significantly and independently associated with hypertension and history of CVD.

OBJECTIVE: There are many scaffold materials of repairing nasal alar cartilage defects. Auricuiar cartilage was used extensively in terms of its abundant tissues, good elasticity, little donor-site malformation, good plasticity etc. The authors dissected auricular cartilage and nasal alar cartilage, measured cartilage's morphous data and found some similar territories with nasal alar cartilage in the structure of auricular cartilage. METHOD: An anatomical study was performed using 10 adult cadavers acquired through Plastic Surgery Hospital, Peking Union Medical College, Beijing, China. Seven male and three female cadav-ers were included in the study. Harvest 20 auricular cartilage specimens and 20 nasal alar cartilage specimens. Then, Computed Tomography Scan on the auricular cartilage and nasal alar cartilage were performed. The datas were imported into mimics and three-dimensional reconstructions of the auricular cartilage and nasal alar cartilage were carried on. RESULT: Parts of the auricular cartilage, such as conchal fossa, tragus, intertragic notch, and cymba of auricular concha, curs of helix and curs of helix, triangular fossa, are ana-tomically similar to nasal alar cartilage. CONCLUSION: This study reports the anatomy of auricular cartilage and nasal alar cartilage, found some territories in the auricular cartilage, such as conchal fossa, tragus, intertragic notch, and cymba of auricular concha, curs of helix and curs of helix, triangular fossa, are anatomically similar to nasal alar cartilage. This research provides the anatomical basis that auricular cartilage was used to repair the nasal cartilage defect.

The aims of this study were to determine various morphometric measurements of auricle, to investigate asymmetry and its relation with handedness in both sexes. Main morphometric measurements of both auricles and hand preferences were determined on 200 male and 200 female healthy university students. All measurements of the auricles were larger in males than the corresponding ones in females and all the differences except earlobe widths were statistically significant. All measurements of left and right auricles were statistically significantly different in both sexes except earlobe length in males, and distance from the highest point of the auricle to the bottom of the intertragic notch and earlobe length in females. Auricle length and distance from the highest point of the auricle to the bottom of the intertragic notch had higher values at the left side and all other measurements were larger at the right side. There was no significant difference between the measurements of the corresponding auricles of the righthanded and lefthanded subjects. All measurements were larger at the right side except auricle length and distance from the highest point of the auricle to the bottom of the intertragic notch in righthanded and lefthanded subjects and all differences except earlobe length were significantly different in righthanded subjects as in the total population but in lefthanded subjects the difference of the auricle width was not significant, either. There was an apparent asymmetry of right and left auricles of normal people in the current study which should be taken in to consideration in plastic surgeries of the external ear, designing the ear devices and forensic applications of earprints.
El objetivo de este estudio fue determinar diversas mediciones morfométricas de la oreja, para investigar la asimetría y su relación con el uso de las manos en ambos sexos. Se realizaron mediciones morfométricas de los pabellones auriculares y las manos de 400 estudiantes universitarios (200 hombres y 299 mujeres). Todas las medidas de las orejas fueron mayores en los hombres y todas las diferencias, excepto las correspondientes al ancho del lóbulo auricular, fueron estadísticamente significativas. Todas las mediciones de los pabellones auriculares izquierdos y derechos presentaron diferencias estadísticamente significativas en ambos sexos, excepto en el caso de la longitud del lóbulo auricular en los hombres, y la distancia desde el punto más alto de la oreja a la parte inferior de la incisura intertrágica en el lóbulo auricular de las mujeres. La longitud de la oreja y la distancia desde el punto más alto de la misma a la parte inferior de la escotadura intertrágica tenían valores más altos en el lado izquierdo y todas las demás mediciones fueron más elevadas en el lado derecho. No hubo diferencia significativa entre las mediciones de las orejas correspondientes a los sujetos diestros y zurdos. Todas las mediciones fueron superiores en el lado derecho, excepto la longitud del pabellón auricular y la distancia desde el punto más alto de la oreja a la parte inferior de la incisura intertrágica en sujetos diestros y sujetos zurdos, y todas las diferencias, excepto la longitud del lóbulo auricular, fueron significativamente diferentes en los sujetos diestros como en la población total, pero en sujetos zurdos la diferencia de la anchura de la oreja no fue significativa. Se encontró una aparente asimetría en las orejas derecha e izquierda de los sujetos del presente estudio que debe tenerse en consideración en las cirugías plásticas del oído externo, para el diseño de los dispositivos de audición y para su aplicación en estudios forenses de las huellas de la oreja.

AIM: Stiffness of the auricular cartilage is the main determining factor for the choice of operative technique of the prominent ear deformity. The aim of this study is to evaluate the stiffness of normal appearing ears objectively and quantitatively, compare the results with the operated prominent ear patients, and present prospective short-term dynamometric evaluation of the operated prominent ear patients. PATIENTS AND METHODS: A total of 190 volunteers without ear deformities were recruited and 9 age groups were formed: group (5-9), group (10-14), group (15-19), group (20-24), group (25-29), group (30-34), group (35-39), group (40-49), and group (50+). Total 28 ears (14 patients) with otoplasty were included in the study as group (operated 5-9) and group (operated 10-14). In addition, 3 patients with prominent ear deformity were prospectively followed for dynamometric changes that occur with otoplasty operation. The auriculocephalic angle (ACA) was measured once and auricle to scalp distance was measured at 4 different standardized levels. Ear stiffness was measured on each ear individually at 4 different points over the antihelix using digital computer-aided dynamometry. Each ear was compared in terms of ACA, distance, and dynamometric values. FINDINGS: Dynamometric values tend to increase with age, which increase and peak around 35 years of age and declines after 40 years of age. Measurements of the first 2 age groups were statistically different compared with the other groups. Postoperative dynamometric measurements (DNM) of group (operated 5-9) were similar with normative values of group (5-9) and postoperative satisfaction visual analogue scale (VAS) score was 92.8%. Postoperative DNM of group (operated 10-14) were higher compared with normative values of group (10-14) for each different measuring level and the postoperative satisfaction VAS score was 75.3. A total of 3 patients with prominent ears had lower dynamometric values preoperatively; these values approached closer to normative values of their age group postoperatively. CONCLUSIONS: Results show that auricular cartilage stiffens and malleability decreases with increased age. This stiffness peaks in the 35-39 age group and declines after 40 years of age. Dynamometric values increase, at all levels, suggesting increased cartilage stiffness is related to age. In the scope of these results, cartilage sparing techniques are more suitable for 5 to 14 years of age and cartilage-cutting techniques are more suitable for older patients.

Here we report a genome-wide association study for non-pathological pinna morphology in over 5,000 Latin Americans. We find genome-wide significant association at seven genomic regions affecting: lobe size and attachment, folding of antihelix, helix rolling, ear protrusion and antitragus size (linear regression P values 2 × 10(-8) to 3 × 10(-14)). Four traits are associated with a functional variant in the Ectodysplasin A receptor (EDAR) gene, a key regulator of embryonic skin appendage development. We confirm expression of Edar in the developing mouse ear and that Edar-deficient mice have an abnormally shaped pinna. Two traits are associated with SNPs in a region overlapping the T-Box Protein 15 (TBX15) gene, a major determinant of mouse skeletal development. Strongest association in this region is observed for SNP rs17023457 located in an evolutionarily conserved binding site for the transcription factor Cartilage paired-class homeoprotein 1 (CART1), and we confirm that rs17023457 alters in vitro binding of CART1.

BACKGROUND: The advent of three-dimensional stereo-photogrammetry in recent years has vastly helped the craniomaxillofacial field improve in terms of preoperative and intraoperative decision making. With regard to the auricle though, there is paucity of research as to the application of this promising technology. METHODS: A total of 20 normal adult ears were included in this study. Thirteen anthropometric measurements were taken, twice by two plastic surgeons using direct measurement (DM) and through images captured via 3dMD™. The purpose was to compare the reliability of measurements involving the two instruments. RESULTS: The overall mean absolute differences (MADs) of all ear anthropometries of DM and 3dMD™ were 0.52 mm (range: 0.28-0.72 mm) and 0.27 mm (range: 0.15-0.53 mm), respectively, and the grand mean relative error magnitudes (REMs) were 2.85% (range: 1.01-5.99%) and 1.57% (range: 0.48-3.62%), respectively, across observers. Thus, the precision of all ear anthropometries across observers was high in both methods, but the precision of 3dMD was better than DM irrespective of observers. In addition, the MADs were less than a millimeter across all measurements. CONCLUSION: The application of three-dimensional technology in microtia surgery for both template production and soft tissue analysis leads to improved planning and satisfactory results with fewer complications. We believe that with further refinement and enhancement, the use of this innovation will pave the way for prefabricated, individualized autologous or biocompatible alloplastic implantable frameworks based on an accurate mirror image of each patient's normal ear in unilateral cases and in bilateral cases, appropriately sized.
